  • Type-2 inflammation is the most frequent endophenotype of asthma. Different biomarkers have been proposed to identify this inflammation because highly effective therapies have improved type-2 severe asthma control. We investigated the frequency of some biomarkers of type-2 inflammation (total IgE, sIgE, blood eosinophil, and FeNO) in the framework of severe asthma and assessed its ability to help us to choose the best biological therapy for each patient. A potential culprit agent of these vaccine-induced anaphylaxis events is polyethylene glycol, which has been reported as a cause of anaphylaxis. However, a cause-effect association has not been demonstrated, and the cases of anaphylaxis to mRNA COVID-19 vaccines should be further investigated. In this scenario, the recommendations are inaccurate and can lead to misinterpretation. At the moment, a more accurate recommendation would be the contraindication of mRNA COVID-19 vaccines in patients with immediate hypersensitivity reaction to polyethylene glycol or polysorbate. Patients with history of anaphylaxis to other or unknown causes should be referred to an allergist-immunologist for further orientation.Purpose The aim of this study was to determine the odds ratio of anaplastic large-cell lymphoma in late seroma formation. Methods In a PubMed search, 415 articles were found using the terms "breast implant AND seroma" (n = 232), "breast implant AND effusion" (n = 42), and "anaplastic large cell lymphoma AND breast (n = 141). Sixty-seven abstracts were read, and 27 full articles were reviewed. Results Three articles reported the incidence of late seroma in breast implants, with a total of 75 seromas out of 48,211 implants (0.16%). One article reported 48 cases of non-Hodgkin lymphoma from 43,537 implants (0.11%). Another article reported that 11 patients had anaplastic large-cell lymphoma among 389 primary lymphoma of the breast (2.83%). Two articles reported 143 seromas out of 236 anaplastic large-cell lymphomas (60.59%). The risk of anaplastic large-cell lymphoma was significantly higher in the patients having late seroma than those without seroma (odds ratio = 998.93; 95% confidence interval, 768.90-1297.78; P less then .001). The incidence of anaplastic large-cell lymphoma in seroma was calculated by dividing the number of anaplastic large cell lymphomas with seroma (n = 143) by total seroma (N = 11,843), which resulted in an incidence of 1.21%. The expected incidence of anaplastic large-cell lymphoma in seroma was 1.21%. Conclusion If late seroma develops after breast implant insertion, ultrasonography-guided aspiration should be performed, with enzyme-linked-immunosorbent serologic assay for CD30.Background We experienced a case of malunion of condylar fracture after miniplate fixation in a patient with a 40 pack-year smoking history who restarted smoking at 5 weeks postoperatively. He had a 40 pack-year smoking history. Radiology revealed a bilateral condylar fracture and a fracture of the parasymphysis. Intermaxillary fixation was done using the skeletal anchorage system on the first post-trauma day. On the third post-trauma day, vertical ramus osteotomy, miniplate fixation of the fractured condylar neck, and free grafting were performed. When the wire was changed to a rubber band at 5 weeks postoperatively, he started smoking (half-pack a day). At 7 weeks postoperatively, the skeletal anchorage system was removed and some absorption of the condylar head was observed. At 3 months postoperatively, his mouth opening was 24 mm and no malocclusion was present, although the condylar head was distorted and malunion was observed. At 4 months postoperatively, his mouth opening was 30 mm but he complained of pain on do so. Distortion of the condylar head was aggravated. At 5 months postoperatively, his pain continued but was endurable. He continued smoking (half-pack a day) since 5 weeks postoperatively. Thirty-nine titles were selected and after reviewing the abstracts, 10 articles fit the inclusion and exclusion criteria. A total of 220 free ***** were used to reconstruct defects. Age range was between 2 and 17 years. Complete flap failure rate was 4.5% (n = 10). The anterolateral thigh perforator flap was the commonest flap used (n = 59), and the latissimus dorsi flap was the commonest muscle flap used (n = 51).

  • To assess adherence to COVID-19 preventive measures and its associated factors among pregnant women in Ghana.

    This was a cross-sectional study conducted in the Nabdam district, Ghana. Data were collected from 527 pregnant women randomly selected from antenatal care clinics from 16 healthcare facilities. Descriptive statistics were used to assess the prevalence of adherence to COVID-19 preventive measures. Multivariable logistic regression was used to estimate the factors associated with COVID-19 preventive measures, whilst adjusting for potential confounders.

    The prevalence of wearing a face mask 18.0% (95% CI 14.73%, 21.32%); of handwashing/hand sanitising 31.7% (95% CI 27.70%, 35.67%), and of social distancing, 22.0% (95% CI 18.46%, 25.56%). Multivariable logistic regression analysis revealed that knowledge of COVID-19 symptoms [Adjusted odds ratios (aOR) 2.86, 95% CI 1.03, 7.89] and knowledge of COVID-19 transmission via contaminated surfaces/objects (aOR 4.60, 95% CI 1.23, 17.18) were associated with wearing a face mask. Pregnant women who knew that avoiding the touching of eyes, nose and mouth can prevent COVID-19 (aOR 2.71, 95% CI 1.01, 7.28), and knowledge of the virus being transmitted via contaminated objects/surfaces (aOR 4.08, 95% CI 1.42, 11.76), were associated with handwashing/hand sanitising. Knowledge of COVID-19 transmission via contaminated surfaces/objects (aOR 15.27, 95% CI 1.87, 124.43) was also associated with social distancing.

    The findings of our study suggest that knowledge of COVID-19 symptoms, transmission and preventive measures may play an important role in the practice of preventive measures against COVID-19 among pregnant women.

Older dentulous patients in good health have become increasingly common. The development of a post-traumatic malocclusion is a common sequela resulting from mandibular condyle fracture, as in the case reported in this paper. The decision-making process led the authors to rule out conservative treatment options and to perform orthognathic surgery on an 81-year-old patient, an unprecedented report in the literature. At one-year follow-up, prophylactic therapy, a specific surgical technique, and osteotomy fixation have restored the occlusion to the pre-traumatic condition.
    Basal cell carcinoma (BCC) is the most common cutaneous malignancy. Occasionally, it may have an appearance similar to that of some benign pigmented skin lesions. Therefore, additional information is needed to differentiate these lesions.

    A diagnostic accuracy study was performed from February 2018 to April 2019. All lesions underwent ultrasound examination with 50 and 20MHz probes. The high-frequency ultrasound (HFUS) images were evaluated independently by 2 experienced doctors for the presence of predefined features, including the depth, shape, margin, anechoic area, hyperechoic spots, epidermal interrupted echo, mushroom sign, flat-bottom sign, and superficial hyperechoic focus (SHEF).

    A total of 54 ****, 51 melanocytic nevi and 55 seborrheic keratoses (SK), were included. **** often involved the subcutaneous tissue (11/54, 20.4%; P<.001) and had an irregular shape (26/54, 48.1%; P<.001) and ill-defined borders (26/54, 48.1%; P<.001), while most benign pigmented lesions had a regular shape (101/106, 95.3%; P<.001) and well-defined borders (95/106, 89.6%; P<.001). **** occasionally showed anechoic areas (10/54, 18.5%; P<.001) and epidermal interrupted echo (18/54, 33.3%; P<.001).     Larvae of many hoverfly species prey upon aphids, whereas the adults, by relying on nectar and pollen, contribute to the pollination of many plant species. Despite their great potential for pest control and pollination, important gaps still exist regarding the efficacy of hoverflies in regulating infestations of major aphid pests in augmentative biological control programs. Here, we tested the potential of the commercially available hoverflies Eupeodes corollae and Sphaerophoria rueppellii to regulate populations of the foxglove aphid Aulacorthum solani in sweet pepper.

    In a semi-field experiment, aphid numbers were 93.2% and 78.4% lower in the E. corollae and S. rueppellii treatments, respectively, compared to the control. Fruit yield was increased by 390% and 361% and seed set by 395% and 399% for E. corollae and S. rueppellii. In a separate laboratory trial, we found that under conditions of limited prey, hoverfly larvae did not complete development, but that larvae of S. rueppellii survived significantly longer than larvae of E. corollae.

    We have shown for the first time that E. corollae and S. rueppellii can reduce infestations of foxglove aphid in sweet pepper. The limited amount of prey, related to the small size of the A. solani colonies, means that hoverfly larvae were often not able to complete development.     A total of 246 of 565 participants who were followed for > 6 months from a MM cohort at our institution were eligible for the retrospective study. A generalized additive model and smooth curve fitting were performed to investigate the cross-sectional relationship between the serum calcium level and RI at baseline. Together, this suggests that cell-free Hb is central in the pathophysiology of the injury to the immature brain following GM-IVH.   • Collective decision making is important for maximizing total benefits while preserving equality among individuals in the competitive multi-armed bandit (CMAB) problem, wherein multiple players try to gain higher rewards from multiple slot machines. The CMAB problem represents an essential aspect of applications such as resource management in social infrastructure. In a previous study, we theoretically and experimentally demonstrated that entangled photons can physically resolve the difficulty of the CMAB problem. This decision-making strategy completely avoids decision conflicts while ensuring equality. However, decision conflicts can sometimes be beneficial if they yield greater rewards than non-conflicting decisions, indicating that greedy actions may provide positive effects depending on the given environment. In this study, we demonstrate a mixed strategy of entangled- and correlated-photon-based decision-making so that total rewards can be enhanced when compared to the entangled-photon-only decision strategy. We show that an optimal mixture of entangled- and correlated-photon-based strategies exists depending on the dynamics of the reward environment as well as the difficulty of the given problem. In contrast, sRAGE levels did not differ due to the causing pathogen. Patients with atypical pathogen pneumonia related ARDS showed significantly lower Ang-2 levels compared to patients with viral and indirect ARDS. Patients with viral and atypical pneumonia related ARDS possessed significantly lower serum IL-6 levels compared to bacterial pneumonia related ARDS and IL-6 levels in atypical pneumonia related ARDS were significantly lower than in indirect ARDS. Current findings report a potential difference in ARDS biomarkers due to the underlying disease and pathogen.

The capability of the essential oil (EO) from inflorescences as antimicrobial agent has been reported. However, literature data are still lacking about the hemp EO antiprotozoal efficacy in vivo. The present study aims to unravel this concern through the evaluation of the efficacy of hemp EOs (2.5 mL/kg, intraperitoneally) of three different cultivars, namely Futura 75, Carmagnola selezionata and Eletta campana, in mice intraperitoneally infected with Leishmania tropica. A detailed description of EO composition and targets-components analysis is reported. Myrcene, α-pinene and E-caryophyllene were the main components of the EOs, as indicated by the gas-chromatographic analysis. Data regarding sociodemographic information, physical distancing profile, alcohol and cigarette usages, Alcohol Use Disorders Identification Test (AUDIT), Cigarette Dependence Scale (CDS), Symptom Checklist-90, and Pittsburg Sleep Quality Index (PSQI) were collected. Results Thirty-eight studies measured gray matter and 18 studies measured white matter in 2,473 high risk subjects and 990 healthy volunteers. GHR, CHR, and PE subjects show an accelerated decline in gray matter primarily in temporal, and also frontal, cingulate and parietal cortex. In those who remain symptomatic or transition to psychosis, gray matter loss is more pronounced in these brain regions. White matter volume and fractional anisotropy, which typically increase until early adulthood, did not change or reduced in high risk subjects in the cingulum, thalamic radiation, cerebellum, retrolenticular part of internal capsule, and hippocampal-thalamic tracts. In those who transitioned, white matter volume and fractional anisotropy reduced over time in the inferior and superior fronto-occipital fasciculus, corpus callosum, anterior limb of the internal capsule, superior corona radiate, and calcarine cortex. To understand Parkinson's disease patients' unmet needs and optimize communication between doctors and patients, social media listening was performed to investigate concerns in Chinese patients. A comprehensive search of publicly available social media platforms with Chinese-language content posted between January 2005 and April 2019 in mainland China was performed using defined Parkinson's disease-related terms. After multiple steps of machine screening were performed, a series of posts were derived. The content was summarized and classified manually to analyze and map psychological insights, and descriptive statistics were applied to aggregate findings. A total of 101,899 patient-related posts formed the basis of this study. The topics mainly focused on motor symptoms (n = 54,983), choice of pharmaceutical drugs (n = 45,203) and non-motor symptoms (n = 44,855). The most common symptoms mentioned were tremor (54.5%), pain (22.9%), and rigidity (22.1%). Psychological burden (51%) and work/social burden (48%) were the most concerning burdens for patients and their families. The compound levodopa (43%) and dopamine agonists (23%) were the most common options for the patients, while concerns about new-generation anti-Parkinson's disease medication increased. The portraits of patients suggested varying characteristics across different periods and advocate for personalized service from doctors. In the management of patients, it is imperative to plan individualized therapy and education strategies as well as strategies for social support.

  • In this work, a Monte Carlo ray-tracing model for the simulation and optimization of a fiber Luminescent Solar Concentrator (LSC) based on stacked layers of fiber arrays is developed and validated. The fiber LSC efficiency improvements are compared against a conventional planar LSC. We developed a new model to analyze the performance of different configurations of bulk-doped fibers and fibers constituted by a doped coating and a passive core. These configurations are analyzed also varying fiber packing geometry diameters, and length. Due to the exceptionally low absorption coefficient of the silica fibers (αwg ≈ 10-4 cm-1), concentration factors of up to 1.9 are predicted when dimensions are scaled over 1 m2, which improve more than twice the maximum concentration factor ever reported. These results serve as a preliminary theoretical study for the future development of a new LSC design based on flexible silica micro-fibers coated with Si-QDs doped poly(lauryl methacrylate) (PLMA) layers.A highly productive ablation process of 100 nm thick platinum films with a processed area rate of up to 378 cm2/min is presented using radially and azimuthally polarized laser beams. This was achieved by developing a laser amplifier generating 757 fs long laser pulses at a maximum average power of 390 W and a repetition rate of 10.6 MHz with adjustable polarization states, i.e., linear, radial, and azimuthal polarization on the work piece. The pulse train emitted from the laser was synchronized to a custom-designed polygon scanner and directed into an application machine.In this paper, we propose and design a multi-orbital-angular-momentum multi-ring air-core fiber, which has seven high-index rings with each ring supporting 62 radially fundamental OAM modes across C and L bands (from 1530 nm to 1625 nm), i.e. 434 OAM modes in total. The designed fiber features >4×10-4 intra-ring modal indices difference for OAM modes with the same topological charge l in a ring across the C and L bands. Moreover, it can keep less then -52 dB crosstalk between the OAM modes in the adjacent rings at 1550 nm, and less then -24 dB crosstalk across C and L bands after 100-km fiber propagation. The tapping ratio of the fabricated coupler was measured to be - 32 dB. The fundamental mode coupled in a core of the lower side-polished fiber (SPF) was transferred to the upper SPF without clad-mode coupling. Two SPFs were directly faced to increase an optical handling power up to 740 W. The tapping ratio of the coupler was constantly maintained at the applied laser output. The beam quality of the laser including the fabricated coupler was maintained to be 1.22, without mode distortion by the coupler.We propose and experimentally demonstrate a microwave frequency measurement system based on the photonic technique. An amplitude comparison function is constructed to perform frequency-to-power mapping based on a non-sliced broadband optical source. The results are fed into a machine learning module which can be utilized to minimize the differential mode noise of the system caused by the polarization fluctuation. The system is reconfigurable with adjustable measurement bandwidth by adjusting the dispersion group delay of the signals at orthogonal polarizations by a polarization division multiplexed emulator (PDME). In addition, the mapping relationship is reconstructed by stacking method. The results are fed into four machine learning models support vector regressor (SVR), KNeighbors regressor (KNN), polynomial regressor (PR) and random forest regressor (RFR). The output of the four models then combined by adding them together using linear regression method. By fitting the relationship between frequency and microwave power ratio with machine learning method, the accuracy of microwave frequency measurement system is further improved. The results show that for a measurement system with a bandwidth of 2 GHz and 4 GHz, the maximum error and the average measurement errors are all reduced. The pulse train emitted from the laser was synchronized to a custom-designed polygon scanner and directed into an application machine.In this paper, we propose and design a multi-orbital-angular-momentum multi-ring air-core fiber, which has seven high-index rings with each ring supporting 62 radially fundamental OAM modes across C and L bands (from 1530 nm to 1625 nm), i.e. 434 OAM modes in total. The designed fiber features >4×10-4 intra-ring modal indices difference for OAM modes with the same topological charge l in a ring across the C and L bands. Moreover, it can keep less then -52 dB crosstalk between the OAM modes in the adjacent rings at 1550 nm, and less then -24 dB crosstalk across C and L bands after 100-km fiber propagation. Functional annotation revealed that the identified enzymes were mainly involved in translation, transcription, ribosomal structure and biological processes, showing that lysine acetylation can regulate various aspects of primary carbon and nitrogen metabolism and secondary metabolism.     Opioid-related overdose deaths are the top accidental cause of death in the United States, and development of regional strategies to address this epidemic should begin with a better understanding of where and when overdoses are occurring.

    In this study, we relied on emergency medical services data to investigate the geographical and temporal patterns in opioid-suspected overdose incidents in one of the largest and most ethnically diverse metropolitan areas (Houston Texas). Using a cross sectional design and Bayesian spatiotemporal models, we identified zip code areas with excessive opioid-suspected incidents, and assessed how the incidence risks were associated with zip code level socioeconomic characteristics. Our analysis suggested that opioid-suspected overdose incidents were particularly high in multiple zip codes, primarily south and central within the city. Zip codes with high percentage of renters had higher overdose relative risk (RR = 1.03; 95% CI [1.01, 1.04]), while crowded housing and larger proportion of white citizens had lower relative risks (RR = 0.9; 95% CI [0.84, 0.96], RR = 0.97, 95% CI [0.95, 0.99], respectively).

    We searched Pubmed, Embase and the Cochrane Library systematically for RCTs, observational studies and case studies on the effect of VNS treatment for tinnitus on October 29, 2019. Studies including adult patients with subjective tinnitus, comparing transcutaneous or implantable VNS to placebo or no treatment or before and after application of VNS treatment on tinnitus distress and tinnitus symptom severity measured with a validated questionnaire were eligible. The risk of bias was assessed with the appropriate tool for each type of study.

    Our search identified 9 primary studies of which 2 RCTs, 5 cohort studies and 2 case series or reports. 5 studies used transcutaneous VNS treatment and 4 used implanted VNS treatment. 6 studies combined VNS treatment with sound therapy. There was a serious risk of bias in all studies, especially on confounding. Most studies reported a small decrease in tinnitus distress or tinnitus symptom severity.

Yet, access to relevant and timely disease information that could strengthen their adaptive capacity remain challenging and poorly characterised in the empirical literature. This paper addresses this gap by exploring the role of disease information in shaping the adaptive capacity of smallholder farmer and tribal groups to Kyasanur Forest Disease (KFD), a tick-borne viral haemorrhagic fever. We carried out household surveys (n = 229) and in-depth interviews (n = 25) in two affected districts-Shimoga and Wayanad-in the Western Ghats region. Our findings suggest that, despite the generally limited awareness about KFD, access to disease information improved households' propensity to implement adaptation strategies relative to households that had no access to it.

Thus, probing the kinetics and dynamics of protein turnover lends insight into how cells regulate essential processes such as growth, differentiation, and stress response. Here, we outline historical and current approaches to measuring the kinetics of protein turnover on a proteome-wide scale in both steady-state and dynamic systems, with an emphasis on metabolic tracing using stable isotope-labeled amino acids.     A number of studies have suggested a pathophysiological link between allergic diseases and epilepsy. Understanding the association between allergic diseases and epilepsy can help establish healthcare policies, implement prevention strategies, and provide a new direction for treatment.     PubMed, EMBASE, and Web of Science were searched for relevant primary articles. Two individuals independently conducted abstract screening, full-text review, data extraction, and quality assessment. Random-effects models were used to pool the risk estimates.

    From the 3124 citations identified, 32 were reviewed in full text. Finally, 11 studies with a total of 3,312,033 subjects were eligible for the analyses. Few studies reported the type of epilepsy, and there were inconsistent attempts to control for confounding. The pooled result showed that there was an 81% increase in the prevalence of epilepsy among individuals with asthma compared with those without asthma (odds ratio 1.81, 95% confidence interval [CI]1.47-2.21). The incidence of epilepsy in patients with eczema was 2.57 (95%CI 1.54-4.27). Sensitivity analyses confirmed that no single study qualitatively influenced the pooled OR. All funnel plots were asymmetric upon visual inspection, suggesting publication bias.

    Our findings suggest that patients with allergic diseases might have a high risk of epilepsy. Additional high-quality primary studies are required to confirm the association, obtain information regarding the mechanism of association, and determine prevention opportunities.

    Bilateral tonic-clonic seizures are encountered in both focal and generalized epilepsies. We reviewed the literature regarding the presence of focal signs in generalized-onset tonic-clonic seizures (GOTCS) and the utility of semiology in differentiating those from focal to bilateral tonic-clonic seizures (FBTCS).

    We conducted a comprehensive literature search using four electronic databases (Medline, Embase, Web of Science, and Psychinfo) and constructed a systematic review in keeping with the Preferred Items for Systematic Reviews and Meta-analyses guidelines.

    We included 13 studies on focal semiological features of GOTCS. These studies included a total of 952 participants. The key focal signs described in GOTCS included early head version, figure of four sign, asymmetric seizure termination, and a multitude of auras as well as automatisms. Additionally, we reviewed five studies that investigated the use of semiology to differentiate GOTCS from FBTCS; these studies had a total of 289 participants. Asymmetry in clonic phase, side-to-side axial movements, asymmetrical seizure termination, figure of four sign, index finger pointing, and fanning posture of the hand were found to be significantly more frequent in FBTCS compared with GOTCS. Furthermore, combinations of focal semiological features occurring in a single seizure were found to be suggestive of FBTCS rather than GOTCS.

The key focal signs described in GOTCS included early head version, figure of four sign, asymmetric seizure termination, and a multitude of auras as well as automatisms. Additionally, we reviewed five studies that investigated the use of semiology to differentiate GOTCS from FBTCS; these studies had a total of 289 participants. Asymmetry in clonic phase, side-to-side axial movements, asymmetrical seizure termination, figure of four sign, index finger pointing, and fanning posture of the hand were found to be significantly more frequent in FBTCS compared with GOTCS. Furthermore, combinations of focal semiological features occurring in a single seizure were found to be suggestive of FBTCS rather than GOTCS. Focal signs are often evident in GOTCS. Though the observation of multiple focal signs within a given seizure may be in favor of an FBTCS, our findings caution against differentiating between the two seizure types based on semiology alone due to considerable overlap in focal features.
