We conducted a hospital-based case-control study of patients admitted for COVID-19 with prolonged fever (fever >7 days) and saddleback fever (recurrence of fever, lasting <24 hours, after defervescence beyond day 7 of illness). Fever was defined as a temperature of ≥38.0°C. Cytokines were determined with multiplex microbead-based immunoassay for a subgroup of patients. Adverse outcomes were hypoxia, intensive care unit (ICU) admission, mechanical ventilation, and mortality.
A total of 142 patients were included in the study; 12.7% (18/142) of cases had prolonged fever, and 9.9% (14/142) had saddleback fever. Those with prolonged fever had a median duration of fever (interquartile range [IQR]) of 10 (9-11) days for prolonged fever cases, while fever recurred at a median (IQR) of 10 (8-12) days for those with saddleback fever. Both prolonged (27.8% vs 0.9%;
 < .01) and saddleback fever (14.3% vs 0.9%;
 = .03) were associated with hypoxia compared with controls. Cases with prolonged fever were also more likely to require ICU admission compared with controls (11.1% vs 0.9%;

The RCP2.6-SSP1 scenario produces the highest global GDP exposure for 2046-2065, with a 5.56-fold increase relative to the base period, of up to (2.29 ± 0.20) × 1015 purchasing power parity $-days. Socioeconomic effects are the primary contributor to the exposure changes at the global and continental scales. Population and GDP effects account for 64-77% and 78-91% of the total exposure change, respectively. To estimate spatiotemporal fluctuations in sea level due to water impoundment, we use a historical data set that includes 6,329 reservoirs completed between 1900 and 2011, as well as projections of 3,565 reservoirs that are expected to be completed by 2040. The GMSL change associated with the historical data (-0.2 mm yr-1 from 1900-2011) is consistent with previous studies, but the temporal and spatial resolution allows for local studies that were not previously possible, revealing that some locations experience a sea level rise of as **** as 40 mm over less than a decade. Future construction of reservoirs through ~2040 is projected to cause a GMSL fall whose rate is comparable to that of the last century (-0.3 mm yr-1) but with a geographic distribution that will be distinct from the last century, including a rise in sea level in more coastal areas. The analysis of expected construction shows that significant impoundment near coastal communities in the coming decades could enhance the flooding risk already heightened by global sea level rise.
A discoid meniscus is a morphological variant of normal knee joint meniscus shape and ultrastructure that can lead to traumatic tearing of this tissue and early joint osteoarthritis.
The purpose of this study was to determine the prevalence of discoid menisci in a large, ethnically diverse regional cohort and to evaluate possible risk factors. The hypothesis was that there would be no difference in the epidemiological distribution of discoid menisci based on ethnicity or sex.
Descriptive epidemiology study.
The study population was from a regional, integrated health care system cohort from Kaiser Permanente of Southern California that, as of 2016, included more than 4.5 million patients. Patient demographics included age, sex, and ethnicity within this cohort. Potential risk factors analyzed included age, sex, ethnicity, and body mass index (BMI). Unique characteristics of a discoid meniscus were analyzed, including a symptomatic versus asymptomatic meniscus, location of meniscal tear and type of meniscus, and frequency of meniscal surgical treatment.

These pregnancies should be managed within a multidisciplinary team, comprising a transplant physician, an endocrinologist, a nephrologist, an obstetrician and a neonatologist. Last but not least, it is very important to continue collecting data on the pregnancies in pancreas-kidney transplantation with the aim to improve knowledge and to generate evidence-based guidelines for the care of women after pancreas-kidney transplants who are considering a pregnancy.Neurodegeneration disorders are complex and multifaceted, leading to many chronic diseases, like Parkinson's disease (PD). Electrolytes imbalance has a significant role in the pathophysiology of neurological disorders, which might serve as a bio-indicator of these problems. In the present study, the disturbances of the electrolytes, calcium (Ca), magnesium (Mg), sodium (Na), and potassium (K), concentrations, and their correlation with biochemical parameters of PD patients at three different age groups (15 to 75 years) and stages were studied. The all resulted data of PD patients was compared with non-diseased/referents (PD) of same age groups. The biological samples (scalp hair, serum and plasma) were collected from Parkinson's disease (PD) patients, diagnosed at three stages, early (15 to 30 years), mild (30 to 60 years) and severe (61 to 75 years). The electrolyte concentrations were measured after acid digested biological samples using flame atomic absorption spectrometry. The precision of the digestion method was verified by matrices matched certified reference materials. The resulted data indicated that the contents of Ca and Mg were found to be higher in scalp hair samples of PD patients at early to severe stages as compared with those results obtained from referents. The concentrations of Na and K were found to be lower for all PD patients groups as compared with referents; however, the difference was not significant (p less then 0.05). Electrolytes levels in blood serum and plasma were found to be lower in all patients than healthy groups (p less then 0.01). These results suggest a disturbance in the electrolytes homeostasis during the different clinical stages of PD, whereas the higher disturbance was observed in patients at severe stage (61 to 75 years).The purpose of this study was to evaluate the neuroprotective effect of local implantation of a controlled delivery system of chitosan hydrogel loaded with selenium nanoparticles in rats with spinal cord injury (SCI). For this purpose, 60 adult female rats were randomly divided into three equal groups. In all three groups, SCI was induced by aneurysm clamping at the level of thoracic vertebrae under inhaled anesthesia with isoflurane. In one group after spinal cord injury, chitosan hydrogels loaded with selenium nanoparticles (treatment group), and in the other group, only chitosan hydrogels (positive control group) were placed at the site of injury. In the last group (negative control), no material was placed in the injury site. Hematoxylin-eosin and glial fibrillary acidic protein (GFAP) staining evaluated histological changes at the site of injury on days 3, 7, 21, and 28 after surgery. Evaluations show that hemorrhage and inflammation also have a marked decrease in inflammatory cells at different times in the treatment group. This decrease was also seen in the chitosan group but was less severe than in the treatment group. The formation of nerve fibers was also observed in the treatment group over time of injury. Immunohistochemical studies of damaged tissue showed higher expression of GFAP protein in the astrocytes of the treatment group than in the other two groups and the chitosan group compared with the negative control group. A controlled drug delivery system containing selenium nanoparticles seems to play a role in the protection of nerve cells through its anti-inflammatory effect.Suboptimal selenium status may impair the antioxidant defense system in patients undergoing hemodialysis, compromising the selenoenzyme glutathione peroxidase activity. To evaluate the association between the duration of hemodialysis, nutritional selenium status, glutathione peroxidase activity (GPx), and thiobarbituric acid reactive substance (TBARS) levels in patients with chronic renal failure undergoing hemodialysis in a region of selenium-rich soils (Ceará, Northeast Brazil). The case-control study of 75 individuals aged 18 to 88 years was allocated between two groups hemodialysis (n = 41) and control (n = 34). Plasma and erythrocytes selenium levels were determined by inductively coupled plasma optical emission spectrometry. The GPx activity and TBARS levels were also evaluated. In addition, the hemodialysis group was stratified according to the duration of treatment (≤ 59 months and ≥ 60 months). The Mann-Whitney test, Student's t test, and Pearson's or Spearman's correlation were applied according to ative stress.Relationships of lead exposure with blood pressure and blood lead with inflammation have been previously established yet, but the conclusions are still controversial. The objective of our study was to investigate the role of systemic inflammation in the relationships between blood lead concentration and blood pressure. We quantified the levels of blood lead and white blood cell count in 505 lead-exposed workers with 842 observations. Associations between blood lead, white blood cell count, and blood pressure were evaluated by using linear mixed models. We further performed mediation analysis to investigate the role of white blood cell count in the associations between blood lead concentration and blood pressure. We observed that each 1% increase of blood lead levels was significantly positively associated with a 0.9%, 1.7%, and 1.1% increases in systolic blood pressure, white blood cell count, and blood platelet count, respectively. https://www.selleckchem.com/products/osmi-4.html Also, we found that the levels of white blood cell count were positively correlated with diastolic blood pressure and systolic blood pressure in a dose-response manner. Our sample included 554,122 individuals across 25 population-based surveys. Average height was 161.7 cm (95% CI 161.2-162.3), and the crude prevalence of diabetes was 7.5% (95% CI 6.9-8.2). Exclusive breastfeeding (EBF), that is, feeding only breastmilk without other food or fluids since birth and before discharge, and, early breastfeeding initiation, that is, during skin-to-skin contact (SSC) with the mother without separation.
Fifty-nine per cent of newborns initiated breastfeeding early and 83.5% were EBF. Duration of SSC showed a strong dose-response relationship with early breastfeeding initiation. Here, using cutting-edge analyses of effective brain connectivity, we uncover the brain networks differentiating neutral and emotional body language. The data indicate greater activation of the right amygdala and midline cerebellar vermis to nonemotional as opposed to emotional body language. Most important, the effective connectivity between the amygdala and insula predicts people's ability to recognize the absence of emotion. These conclusions extend substantially current concepts of emotion perception by suggesting engagement of limbic effective connectivity in recognizing the lack of emotion in body language reading. Furthermore, the outcome may advance the understanding of overly emotional interpretation of social signals in depression or schizophrenia by providing the missing link between body language reading and limbic pathways. These conclusions extend substantially current concepts of emotion perception by suggesting engagement of limbic effective connectivity in recognizing the lack of emotion in body language reading. Thus, the notion of prolonged field care (PFC) may be a necessity on the future battlefield. As Special Operations Forces (SOF) continue to refine what PFC entails, it is imperative that an understanding of the incidence and type of diseases that require medical evacuation to higher levels of care be thoughtfully estimated. Armed with an understanding of the anticipated epidemiology, effective prioritization of training requirements and equipment acquisition is possible in a manner that is complementary to the overall success of the assigned mission. Furthermore, this prior planning mitigates risk, as the limitations of money and time impose significant opportunity costs in the short run should the disproportionate mix of disease states be pursued, which in turn, avoids jeopardizing Soldiers' lives over the long term.This is second of a two-part series on the history and effectiveness of mouthguards (MGs) for protection from orofacial injuries. MGs are hypothesized to reduce orofacial injuries by separating the upper and lower dentation, preventing tooth fractures, redistributing and absorbing the force of direct blows to the mouth, and separating teeth from soft tissue which helps prevent lacerations and bruises. The single study on MG use in military training found that when boil-and-bite MGs were required for four training activities, orofacial injury rates were reduced 56% compared with when MGs were required for just one training activity. A recent systematic review on the effectiveness of MGs for prevention of orofacial injuries included 23 studies involving MG users and nonusers and a wide variety of sports. For cohort studies that directly collected injury data, the risk of an orofacial injury was 2.33 times higher among MG nonusers (95% confidence interval, 1.59-3.44). More well-designed studies are needed on the effectiveness of MGs during military training. Despite some methodological limitations, the current data suggest that MGs can substantially reduce the risk of orofacial injuries in sport activities. The diminishing DO2 creates a state of systemic hypoxia, the severity of which will determine the magnitude of the shock (shock dose) and a growing deficit of oxygen, referred to as oxygen debt. Rapid accumulation of critical levels of oxygen debt results in coagulopathy and organ dysfunction and failure. Spontaneous respiration induced negative intrathoracic pressure (ITP) provides the pressure differential driving venous return. PPV subsequently increases ITP and thus right atrial pressure. The loss in pressure differential directly decreases CO and DO2 with a resultant increase in systemic hypoxia. If RSI and PPV are deemed necessary, prior or parallel resuscitation with blood products is required to mitigate post intervention reduction of DO2 and the potential for inducing cardiac arrest in the critically shocked patient.Early disclosure of possible concussive symptoms has the potential to improve concussion-related clinical outcomes. The presence of Merkel cell polyomavirus (MCPyV) was identified in Merkel cell carcinoma (MCC). However, there was sparse information on the link of other common nonmelanoma skin cancers - basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) - to MCPyV infection. The current study describes the phylogenetic information of MCPyV isolated from Iranian non-MCC (nonmelanoma skin cancers) focusing on tumorigenesis of mutations in large tumor (LT) antigen (LT-Ag) fragment.
Sixty patients with ****and 20 patients with SCC were included in this study (48 males and 32 females; average age 65 years). The MCPyV-DNA copy number in positive samples was measured by quantitative real-time PCR. Then, mutational analysis of the MCPyV LT gene was carried out by direct sequencing.
While MCPyV DNA was detected in 6 (10%) of 60 ****samples, no viral genome was found in SCCs. There was no distinct association of MCPyV positivity with gender, age, or type of tumor (****or SCC) (p value >0.05). Quantitative real-time PCR revealed that the median number of viral DNA copies per cell was 0.7 in 6 MCPyV-positive ****samples. Furthermore, full-length LT-Ag sequencing of positive samples indicated no stop codon or frameshift mutations compared to reference sequences.

Testicular torsion (TT) is a serious surgical emergency. Prompt diagnosis and treatment of TT are essential to improve the incidence of salvaged testes. The aim of this study was to evaluate the historical features, physical examination findings, laboratory tests, and ultrasound examinations in children with TT, as well as to identify the predictors of testicular salvage in children.
We retrospectively reviewed the records of 136 males who presented with TT to our institution. Clinical findings, physical examinations, laboratory data, color Doppler ultrasound findings, operating results, and the results of follow-up were collected and analyzed. Patients with neonatal torsion, negative scrotal exploration, or testicular appendix torsion were excluded. The study objectives were to assess the control of PTH, Ca, and Phos over time in patients receiving cinacalcet or etelcalcetide as well as dosing and time to discontinuation for etelcalcetide.
This was a retrospective cohort study using electronic medical records from small and independent dialysis centers. Adults ≥18 years of age were identified as cinacalcet or etelcalcetide users based on the first calcimimetic received in 2018 (index date). Patients were followed from the index date until parathyroidectomy, kidney transplant, death, or end of data (December 31, 2018). Analyses of mean PTH, Ca, and Phos, as well as target achievement of PTH, Ca, and Phos were conducted over a 9-month period. Discontinuation with etelcalcetide was measured with the Kaplan-Meier estimator.

Opioid overdose prevention education and naloxone distribution (OEND) programs include information on general risk factors, overdose recognition, and naloxone utilization. This study evaluated a personally-tailored OEND (PTOEND) intervention designed to promote harm reduction and treatment readiness for illicit opioid users by also including education about personal overdose-risk factors and medication for opioid use disorder (MOUD).
A secondary analysis of a randomized controlled trial testing a Peer recovery support service (PRSS) intervention, relative to Control, in adult illicit opioid users reporting treatment for an overdose in the prior 6 months. PTOEND, a 30-minute computer-guided intervention, was administered by a research assistant at the randomization visit to all participants (N = 80). Participants completed a telephone visit 3 weeks post-randomization (n = 74) to assess changes in opioid overdose/MOUD knowledge and treatment readiness. Participants completed in-person visits at 3 (n = 66), 6 (n = 58), and 12 (n = 44) months post-randomization to assess illicit opioid use and naloxone utilization (all time points) and overdose-risk behaviors (12 months). We conducted pre-post analyses of the impact of PTOEND controlling for the PRSS effect.

This study was a randomized, single-blind, placebo-controlled trial in which 34 women were randomly assigned to either 10 g leucine (LEU) or placebo daily, while following a calorie-restricted diet A dual energy x-ray absorptiometry (DXA) analysis, metabolic rate assessment via a BodyGem® and anthropometrics were performed at baseline and after the 12-week study to determine changes in fat mass, lean mass and RMR. At 3 and 12 months after discharge, we assessed global cognition, executive function, and activities of daily living. We analyzed associations between markers and outcomes using multivariable regression, adjusting for age, sex, education, comorbidities, baseline cognition, doses of sedatives and opioids, stroke risk (in cognitive models), and baseline disability scores (in disability models).Measurements and Main Results We included 548 participants who were a median (interquartile range) of 62 (53-72) years old, 88% of whom were mechanically ventilated, and who had an enrollment Sequential Organ Failure Assessment score of 9 (7-11). After adjusting for covariates, no markers were associated with long-term cognitive function. Two markers, CRP and MMP-9, were associated with greater disability in basic and instrumental activities of daily living at 3 and 12 months. No other markers were consistently associated with disability outcomes.Conclusions Markers of systemic inflammation and coagulation measured early during critical illness are not associated with long-term cognitive outcomes and demonstrate inconsistent associations with disability outcomes. In the absence of mucosal or structural disease, the aim of investigating the oesophagus is to provide clinically relevant measurements of function that can explain the cause of symptoms, identify pathology and guide effective management. One of the most notable recent advances in the field of oesophageal function has been high-resolution manometry (HRM). This review explores how innovation in HRM has progressed and has far from reached a plateau.
HRM technology, methodology and utility continue to evolve; simple additions to the swallow protocol (e.g. eating and drinking), shifting position, targeting symptoms and adding impedance sensors to the HRM catheter have led to improved diagnoses, therapeutic decision-making and outcomes. Progress in HRM persists and shows little sign of abating. The next iteration of the Chicago Classification of motor disorders will highlight these advances and will also identify opportunities for further research and innovation.

The purpose of this review is to describe barriers and suggest strategies to encourage medical students to pursue urology.
Barriers to interest in urology include educational factors, such as timing of exposure to urology in medical school, USMLE scores, research experience, and deciding in time for an early match, as well as socioeconomic barriers, such as cost, being underrepresented in medicine, and gender. Steps the urological community can take include increasing involvement in medical school curricula, increasing faculty mentor availability, and broadening students' range of urological experiences. Strategies to encourage interest in urology fall into three categories creating interest, supporting interest, and removing barriers for students considering urology. Traditional antimicrobial stewardship principles have been evaluated to assess their impact on the gut microbiota diversity and the consequent repercussions, taking into consideration antibiotic pharmacokinetic and pharmacodynamic properties. Novel strategies of selective digestive decontamination and fecal microbiota transplantation to regulate the gut microbiota have also been tested in different conditions with variable results. This review seeks to provide an overview of the available literature on the modulation of the gut microbiota and its implications for infection control and antimicrobial stewardship. With increased understanding, gut microbiota profiling through metataxonomic analysis may provide further insight into modulating microbial communities in the context of infection prevention and control.
The safety and tolerability of nintedanib in patients with idiopathic pulmonary fibrosis (IPF) have been characterized using data from clinical trials.
We further characterized the safety and tolerability of nintedanib in patients with IPF in clinical practice using the global pharmacovigilance database. The database included spontaneously reported adverse events and data collected via solicited reporting in patients treated with nintedanib from 15 October 2014 to 15 October 2018. Adverse events were coded using the Medical Dictionary for Regulatory Activities. Cumulative exposure to nintedanib was estimated on the basis of sales data.
Cumulative exposure to nintedanib was estimated as 60,107 patient-years. Diarrhea was the most frequent event (301.6 events per 1000 patient-years). Most (97.0%) diarrhea events were non-serious. The median (25th, 75th percentile) time to onset of the first diarrhea event was 60 (11, 182) days. Elevated liver enzyme or bilirubin levels were reported at a rate of 31.5 events per 1000 patient-years.

In response to the reduction of reactive oxygen species, a decrease in superoxide dismutase and glutathione peroxidase activities was observed; these activities were elevated by cholesterol. Moreover, all the flavanone metabolites improved mitochondrial function and insulin secretion, and reduced apoptosis. Flavanone metabolites were found uptake by β-cells, and therefore could be responsible for the observed protective effects. These results demonstrated that circulating phase-II hesperetin and naringenin metabolites, and also phenolics derived from gut microbiota, protect pancreatic-β cells against oxidative stress, leading to an improvement in β-cell function and could be the bioactive molecules derived from the citrus consumption.In active matter systems, self-propelled particles can self-organize to undergo collective motion, leading to persistent dynamical behavior out of equilibrium. In cells, cytoskeletal filaments and motor proteins form complex structures important for cell mechanics, motility, and division. Collective dynamics of cytoskeletal systems can be reconstituted using filament gliding experiments, in which cytoskeletal filaments are propelled by surface-bound motor proteins. These experiments have observed diverse dynamical states, including flocks, polar streams, swirling vortices, and single-filament spirals. Recent experiments with microtubules and kinesin motor proteins found that the collective behavior of gliding filaments can be tuned by altering the concentration of the crowding macromolecule methylcellulose in solution. Increasing the methylcellulose concentration reduced filament crossing, promoted alignment, and led to a transition from active, isotropically oriented filaments to locally aligned polar streamnts, locally aligned flocks, and polar streams.A novel geometry of sharp-edged electrodes for planar micro-electrochemical capacitors is utilized for an enhanced performance compared to the conventionally used interdigitated electrodes. The sharp-edged electrode geometry achieves a 68% enhancement in the electric field at the sharp-edge of the electrodes as compared to interdigitated electrodes. Moreover, carbon foam with high specific surface area loaded with iron oxide nanoparticles allows a large mass loading for the pseudocapacitance in addition to electric double layer capacitance (EDLC). Thus, an enhancement of 235% was obtained in both the areal specific capacitance and energy density when the performance was compared with the interdigitated electrode based supercapacitors. Moreover, an excellent cycling stability (∼99.5%) over 10 000 charge-discharge cycles was also achieved. The high-performance architecture of sharp-edged electrodes paves a way for smart electrochemical capacitors using an efficient planar structure in combination with high-loading materials for large pseudocapacitance as well as EDLC.Chronic hyperglycaemia and inflammation are present in diabetes and both processes have been related to the pathogenesis of diabetic kidney disease. In vitro cellular accumulation of 64Cu was measured in Hank's Balanced Salt Solution in the presence of 100 μM l-histidine, l-methionine, l-cysteine and l-threonine. l-Cysteine markedly increased 64Cu accumulation and retention in DU145, PC3 and SK-OV-3 cells, while some other cell lines did not show an effect. This effect was not due to 64Cu delivery in the form of a 64Cu-cysteine complex, nor to reduction of 64Cu(ii) to 64Cu(i) by l-cysteine. Pre-incubation of cells with l-cysteine increased 64Cu accumulation, even if l-cysteine was removed from HBSS before 64Cu was added. The effect of l-cysteine on 64Cu accumulation was not mediated by increased glutathione synthesis.

Objetivo La insuficiencia cardiaca (IC) es un síndrome clínico caracterizado por disminución en la capacidad funcional. Los programas de rehabilitación cardiaca y prevención secundaria (PRHCyPS) han mostrado mejorar la calidad de vida y tolerancia al esfuerzo en este grupo de pacientes, pero sus efectos son dependientes del volumen. Nuestro objetivo es evaluar el grado de correlación del volumen de entrenamiento medido en equivalentes metabólicos (MET)-min/semana con el porcentaje de ganancia de consumo pico de oxígeno (VO2p) (MET-carga) posterior a un PRHCyPS en pacientes con IC. Método Estudio cuasiexperimental que evaluó la ganancia de VO2p (MET-carga) en 31 pacientes posterior a un PRHCyPS, antes y después de una prueba de ejercicio convencional, que consistió en 30 min de entrenamiento diná**** al 70% frecuencia cardíaca de reserva (FCR) durante seis semanas, así como entrenamiento de kinesioterapia e intervención interdisciplinaria. Se calculó el volumen de entrenamiento de cada paciente en MET-min/semana (método de Kaminsky). Se midió el índice de correlación con Rho de Spearman y se consideró significancia estocástica con valor de p less then 0.05. Resultados El 70.6% fueron de sexo masculino, promedio de edad 61.5 ± 8.9 años, con fracción de expulsión del ventrículo izquierdo promedio de 38 ± 4.6%; el 96.8% de la IC fue de origen isqué****; un 55.9, un 29.4 y un 5.9% en clase funcional según la New York Heart Association I, II y III, respectivamente. Sin embargo, se ha producido recientemente una gran revolución, especialmente tras la llegada de las prótesis aórticas sin suturas que, unido a un nuevo impulso por la cirugía cardiaca hacia un rumbo menos invasivo, ha provocado que el reemplazo de dicha válvula se lleve a cabo cada vez más frecuentemente por dichas prótesis y por incisiones de mínimo acceso. Por ello, realizamos una revisión de los casos intervenidos en nuestro servicio desde el inicio del programa de cirugía de mínimo acceso comparándolos con los resultados de los casos intervenidos en la misma época mediante cirugía convencional.
Cardiovascular diseases (CVDs) are the main cause of morbidity and mortality in the adult population worldwide. Most studies on risk factors have focused on children and adults of mature age. Quite the opposite, there are few which specifically analyze young individuals.
The objectives of the study were to determine cardiovascular risk factors in young Mexican adults.
A cross-sectional, descriptive, and observational study was carried out on a healthy sample of 198 young Mexican university-level adults, aged from 18 to 25 years who, after completing a questionnaire, were evaluated on anthropometric and blood pressure parameters. A comparative analysis of the results was made according to sex and with findings from other studies.
In 46% of the subjects, an atherogenic diet was identified (predominant among males). About 59.1% of the individuals were classified as sedentary (mostly women). About 91.4% of the sample had two or more associated antecedents of CVD (and other associated conditions) in their family background, the most frequent being diabetes mellitus (71.2%), systemic hypertension (64.6%), and overweight or obesity (56.6%). In 25.8% of the individuals, overweight was observed (more frequent among women). In males, a higher proportion of alterations in blood pressure levels was described.
The most frequently identified findings correspond to the group of modifiable minor risk factors, which could allow the development of preventive measures, inasmuch as these are subjects in which the modification of harmful behaviors and habits is achievable and timely.

Objetivo La insuficiencia cardiaca (IC) es un síndrome clínico caracterizado por disminución en la capacidad funcional. Los programas de rehabilitación cardiaca y prevención secundaria (PRHCyPS) han mostrado mejorar la calidad de vida y tolerancia al esfuerzo en este grupo de pacientes, pero sus efectos son dependientes del volumen. Nuestro objetivo es evaluar el grado de correlación del volumen de entrenamiento medido en equivalentes metabólicos (MET)-min/semana con el porcentaje de ganancia de consumo pico de oxígeno (VO2p) (MET-carga) posterior a un PRHCyPS en pacientes con IC. Método Estudio cuasiexperimental que evaluó la ganancia de VO2p (MET-carga) en 31 pacientes posterior a un PRHCyPS, antes y después de una prueba de ejercicio convencional, que consistió en 30 min de entrenamiento dinámico al 70% frecuencia cardíaca de reserva (FCR) durante seis semanas, así como entrenamiento de kinesioterapia e intervención interdisciplinaria. Se calculó el volumen de entrenamiento de cada paciente en MET-min/semana (método de Kaminsky). The 2019 coronavirus disease (COVID-19) pandemic, with its associated restrictions on daily life, is like a perfect storm for poor mental health and wellbeing. The purpose of this study was therefore to examine the impacts of COVID-19 on mental health and wellbeing during the ongoing pandemic in Sweden.
Standardized measures of depression, anxiety, and insomnia as well as measures of risk and vulnerability factors known to be associated with poor mental health outcomes were administered through a national, online, cross-sectional survey (n=1,212; mean age 36.1years; 73% women).
Our findings show significant levels of depression, anxiety, and insomnia in Sweden, at rates of 30%, 24.2%, and 38%, respectively. The strongest predictors of these outcomes included poor self-rated overall health and a history of mental health problems. The presence of COVID-19 symptoms and specific health and financial worries related to the pandemic also appeared important.
The impacts of COVID-19 on mental health in Sweden are comparable to impacts shown in previous studies in Italy and China. Importantly, the pandemic seems to impose most on the mental health of those already burdened with the impacts of mental health problems. These results provide a basis for providing more support for vulnerable groups, and for developing psychological interventions suited to the ongoing pandemic and for similar events in the future.

To explore the epidemiology and outcomes of takotsubo cardiomyopathy in children.
A retrospective analysis of the Healthcare Cost and Utilization 2012 and 2016 Kids' Inpatient Database was performed. Patients admitted with the diagnosis of takotsubo cardiomyopathy in the age group of 1 month-20 years were identified using International Classification of Diseases (ICD)-9 code 429.83 and ICD-10 code I51.81.
Among a total of 4,860,859 discharges, there were 153 with the diagnosis of takotsubo cardiomyopathy (3.1 per 100,000 discharges). Among patients with takotsubo cardiomyopathy, 55.0% were male, 62.4% were white, and 16.7% were black. Eighty-nine percent of patients were between 12 and 20 years. Psychiatric diagnosis was documented in 46% and substance use disorder in 36.2%. Sepsis was documented in 22.8% of patients. The median length of stay was 5 days (interquartile range 2.7-15), and median total charges were $75,080 (interquartile range 32,176-198,336). The overall mortality for takotsubo cardiomyopathy was 7%. On multivariable regression analysis, mortality was higher in the presence of anoxic injury (odds ratio = 34.42, 95% confidence interval 4.85-320.11, p = 0.00).

Seroprevalence rates were 56.3% among cattle farmers, 88.9% among veterinarians and 12.7% among blood donors. This suggests that a specific risk for acquiring C. burnetii infection from cattle in endemically infected areas exists, mainly for occupational risk groups, but also for the general population. In the present research paper, we present a brief study aiming to predict the probability of reaching a new record number of COVID-19 cases in Lebanon, based on the record theory, giving more insights about the rate of its quick spread in Lebanon. The main advantage of the records theory resides in avoiding several statistical constraints concerning the choice of the underlying distribution and the quality of the residuals. In addition, this theory could be used, in cases where the number of available observations is somehow small. Moreover, this theory offers an alternative solution in case where machine learning techniques and long-term memory models are inapplicable because they need a considerable amount of data to be performant.
