  • Previous surveys of adults with cancer have revealed increased levels of genetic knowledge, varying levels of worry, and high satisfaction with cancer genetic counseling. We sought to determine the impact of cancer genetic counseling on parental levels of genetic knowledge, worry about cancer, and satisfaction in the context of suspected cancer predisposition in a child. We hypothesized that parents would be satisfied with cancer genetic counseling and that cancer genetic counseling would improve baseline parental genetic knowledge and decrease levels of worry. Parental levels of genetics knowledge increased by an average of 1.9 points (p = .01), with 65.2% of parents demonstrating an increase in genetics knowledge score. Average worry levels did not change significantly (p = .37), with 52.2% of parents indicating decreased worry, and 34.8% indicating increased worry. Overall, 91.8% of parents reported high levels of satisfaction. Our results show that cancer genetic counseling in a pediatric cancer predisposition clinic improves parental levels of genetics knowledge. Satisfaction rates suggest that parents find this service beneficial. These results demonstrate the positive impacts of cancer genetic counseling on parents of children in which a hereditary cancer syndrome is known or suspected.Acceleration is an important consideration when imaging moving organs such as the heart. Parallel imaging was among the first of these innovations, using multiple receiver coils and mathematical algorithms for reconstruction; acceleration factors of 2 to 3 were readily achieved in clinical practice. However, in the context of imaging dynamic events, further decreases in scan time beyond those provided by parallel imaging were possible by exploiting temporal coherencies. This recognition ushered in the era of k-t accelerated MRI, which utilized predominantly statistical methods for image reconstruction from highly undersampled k-space. Despite the successes of k-t acceleration methods, however, the accuracy of reconstruction was not always guaranteed. To address this gap, MR physicists and mathematicians applied compressed sensing theory to ensure reconstruction accuracy. Reconstruction was, indeed, more robust, but it required optimizing regularization parameters and long reconstruction times. To solve the limitations of all previous methods, researchers have turned to artificial intelligence and deep neural networks for the better part of the past decade, with recent results showing rapid, robust reconstruction. This review provides a comprehensive overview of key developments in the history of CINE MRI acceleration, and offers a unique and intuitive explanation behind the techniques and underlying mathematics.Level of Evidence 5Technical Efficacy Stage 1.The present study investigated the distribution and environmental fate of Bisphenol A (BPA), the 4-hydroxyacetophenone (4-HAP) metabolite, and 5 other bisphenol congeners in 2 municipal wastewater treatment plants (WWTPs) and their receiving rivers in Romania. Accordingly, a new, highly sensitive and accurate solid-phase extraction-liquid chromatography-electrospray ionization-tandem mass spectrometry method was developed and validated. This technique generated low limit of quantitation values below 2.3 ng/L for surface water and less than 2.4 and 2.7 ng/L for WWTP effluent and influent water. The sum of detected analytes in wastewater was between 1337 and 16 118 ng/L for influent samples and between 15 and 96 ng/L for effluent samples. In surface water, the total of all compounds was somewhere between 34 and 240 ng/L. The highest concentration observed was for BPA in all 3 types of analyzed water (up to 9140 ng/L for influent, as high as 75 ng/L for effluent, and a maximum of 135 ng/L in surface waters). All analyzed samples were free of bisphenols B, C, and F. For all analytes detected in surface water, the concentration values were higher than those determined in the effluent samples, which may be caused by intrinsic contamination of the 2 rivers (Danube and Jiu Rivers). The aim of the study was to examine the diagnostic utility of SIAD in the diagnosis and monitoring of primary PNH disorders. In this retrospective study, we studied 26 patients who were admitted with a diagnosis of primary PNH to the department of Neurology from January 2013 to April 2019. Their clinical profile, immunological characteristics were extracted from the database and nerve conduction studies were relooked for the presence of SIAD. 76% of patients in the primary PNH cohort had SIAD with 90% of them being voltage-gated potassium channel complex antibody positive; predominantly against contactin-associated protein-2 antigen and rest being paraneoplastic. There was also resolution of SIAD following treatment indicating reversible hyperexcitability. SIAD is a sensitive marker for Primary PNH syndrome with monitoring and diagnostic implications.

Ultimately another compound free of aortic binding was developed, AZD7986.A detailed reaction mechanism of acetylene cyclotrimerization catalyzed by V(i PrNPMe2 )3 Fe-PMe3 (denote as CAT), a heterobimetallic complex featuring V-Fe triple bond, was computationally investigated using density functional theory. The calculated results show that the first acetylene firstly attaches to the V atom of CAT to get a four-membered ring structure through [2 + 2] cycloaddition reaction. For the second acetylene addition, there are two cyclotrimerization mechanisms, outer sphere mechanism and inner mechanism. The inner sphere reaction pathway is the main reaction pathway. By replacing the V with Nb and Ta, Fe with Ru and Os, a series of new catalysts are screened computationally. The calculated results show that, all of the nine heterobimetallic complexes show high activity at mild condition. The energy barrier of the rate determining step is related to the natural population analysis (NPA) charge of M' and the Wiberg bond index (WBI) of M-M' bond.     Retroperitoneal fibrosis (RPF) is a rare chronic fibro-inflammatory disorder that may be secondary to certain drugs, including β-blocking agents (BBAs). However, their causative role is unclear. We aimed to investigate this association.

    Disproportionality analysis was carried out on cases from 1985 to 4 October 2020 in VigiBase, the World Health Organization pharmacovigilance database. The Bayesian-based IC
    metric and reporting odds ratio were used in order to assess the adverse event signal. We also analysed all published case reports from the literature regarding BBA-associated RPF to assess the value of suggested supportive clinical evidence.

    In total, 1599 individual case safety reports of RPF were reported to VigiBase, of which 132 (32%) concerned 16 different single BBA. For 12 of these agents (75%), reporting of RPF was disproportionate, indicating a potential safety signal. Line listing analysis of individual case safety reports showed no consistent time interval from start of BBA to RPF diagnosis (range 0.7-264 mo). Dechallenge was negative or unknown in the majority of cases (74%). In 18 published cases from the literature, time from start of BBA to RPF diagnosis varied widely (range 3-156 mo). BBA were discontinued 6 months before (n = 1) or at the time of RPF diagnosis (n = 17). Most patients (84%) also received RPF specific treatment. Follow-up duration was short (median 5 mo [range 1-24 mo]) and in most cases (83%) relevant follow-up data were lacking.

  • The experimental results show that PLANet performs better than traditional and deep learning-based segmentation methods on geometrical and clinical metrics. Moreover, PLANet can complete the segmentation of heart structures in 2D echocardiography in real time, indicating a potential to assist cardiologists accurately and efficiently.Machine learning models for radiology benefit from large-scale data sets with high quality labels for abnormalities. We curated and analyzed a chest computed tomography (CT) data set of 36,316 volumes from 19,993 unique patients. This is the largest multiply-annotated volumetric medical imaging data set reported. To annotate this data set, we developed a rule-based method for automatically extracting abnormality labels from free-text radiology reports with an average F-score of 0.976 (min 0.941, max 1.0). We also developed a model for multi-organ, multi-disease classification of chest CT volumes that uses a deep convolutional neural network (CNN). This model reached a classification performance of AUROC >0.90 for 18 abnormalities, with an average AUROC of 0.773 for all 83 abnormalities, demonstrating the feasibility of learning from unfiltered whole volume CT data. We show that training on more labels improves performance significantly for a subset of 9 labels - nodule, opacity, atelectasis, pleural effusion, consolidation, mass, pericardial effusion, cardiomegaly, and pneumothorax - the model's average AUROC increased by 10% when the number of training labels was increased from 9 to all 83. All code for volume preprocessing, automated label extraction, and the volume abnormality prediction model is publicly available. The 36,316 CT volumes and labels will also be made publicly available pending institutional approval.The recent global outbreak and spread of coronavirus disease (COVID-19) makes it an imperative to develop accurate and efficient diagnostic tools for the disease as medical resources are getting increasingly constrained. Artificial intelligence (AI)-aided tools have exhibited desirable potential; for example, chest computed tomography (CT) has been demonstrated to play a major role in the diagnosis and evaluation of COVID-19. However, developing a CT-based AI diagnostic system for the disease detection has faced considerable challenges, which is mainly due to the lack of adequate manually-delineated samples for training, as well as the requirement of sufficient sensitivity to subtle lesions in the early infection stages. In this study, we developed a dual-branch combination network (DCN) for COVID-19 diagnosis that can simultaneously achieve individual-level classification and lesion segmentation. To focus the classification branch more intensively on the lesion areas, a novel lesion attention module was developed to integrate the intermediate segmentation results. Furthermore, to manage the potential influence of different imaging parameters from individual facilities, a slice probability mapping method was proposed to learn the transformation from slice-level to individual-level classification. We conducted experiments on a large dataset of 1202 subjects from ten institutes in China. The results demonstrated that 1) the proposed DCN attained a classification accuracy of 96.74% on the internal dataset and 92.87% on the external validation dataset, thereby outperforming other models; 2) DCN obtained comparable performance with fewer samples and exhibited higher sensitivity, especially in subtle lesion detection; and 3) DCN provided good interpretability on the loci of infection compared to other deep models due to its classification guided by high-level semantic information. We curated and analyzed a chest computed tomography (CT) data set of 36,316 volumes from 19,993 unique patients. This is the largest multiply-annotated volumetric medical imaging data set reported. To annotate this data set, we developed a rule-based method for automatically extracting abnormality labels from free-text radiology reports with an average F-score of 0.976 (min 0.941, max 1.0). We also developed a model for multi-organ, multi-disease classification of chest CT volumes that uses a deep convolutional neural network (CNN). This model reached a classification performance of AUROC >0.90 for 18 abnormalities, with an average AUROC of 0.773 for all 83 abnormalities, demonstrating the feasibility of learning from unfiltered whole volume CT data. Artificial intelligence (AI)-aided tools have exhibited desirable potential; for example, chest computed tomography (CT) has been demonstrated to play a major role in the diagnosis and evaluation of COVID-19. However, developing a CT-based AI diagnostic system for the disease detection has faced considerable challenges, which is mainly due to the lack of adequate manually-delineated samples for training, as well as the requirement of sufficient sensitivity to subtle lesions in the early infection stages. In this study, we developed a dual-branch combination network (DCN) for COVID-19 diagnosis that can simultaneously achieve individual-level classification and lesion segmentation. To focus the classification branch more intensively on the lesion areas, a novel lesion attention module was developed to integrate the intermediate segmentation results. Furthermore, to manage the potential influence of different imaging parameters from individual facilities, a slice probability mapping method was proposed to learn the transformation from slice-level to individual-level classification. We conducted experiments on a large dataset of 1202 subjects from ten institutes in China. The results demonstrated that 1) the proposed DCN attained a classification accuracy of 96.74% on the internal dataset and 92.87% on the external validation dataset, thereby outperforming other models; 2) DCN obtained comparable performance with fewer samples and exhibited higher sensitivity, especially in subtle lesion detection; and 3) DCN provided good interpretability on the loci of infection compared to other deep models due to its classification guided by high-level semantic information.     Herein, we describe the unique clinical course of a patient who was treated with the combined checkpoint inhibitors nivolumab and ipilimumab for Stage IV malignant melanoma, who subsequently developed TTP. Unlike many patients with TTP, this patient failed to develop a sustained response to therapeutic plasma exchange. Additional use of steroids, anti-CD20, and plasma cell-targeting therapy (bortezomib) also failed to substantially reverse thrombocytopenia in a sustainable fashion. During this time, her melanoma progressed, and she ultimately succumbed.

    The number of case reports of insulin autoimmune syndrome (IAS) induced by methimazole (MMI) is increasing. The purpose of this study is to explore the clinical characteristics and provide a scientific reference for clinical diagnosis, treatment and prevention.

    The literature on IAS cases and case series induced by MMI in Chinese and English was collected for retrospective analysis.

    A total of 106 patients (males 33, females 73) were described in the Chinese and English literature. The median age of patients with IAS induced by MMI was 37years (range 15-76) occurring during both regular and irregular MMI therapy or after resumption of medication. The onset of symptoms occurred at night or early morning, within days in some and up to 6months in others; the symptoms were neuropathic in 65.31% and related to the autonomic nervous system in 33.67%. Blood glucose concentration in samples presumably taken during the hypoglycaemic phase was 1.7mmol/L (median; range 0.03-4.7); insulin concentrations were elevated ≥100mU/L (ref range) and associated with low C-peptide levels (<10μg/L; ref range). Tests for IgG insulin autoantibodies (IAA) were positive in 104 patients (98.02%) and negative in two patients (1.98%). The 75-g oral glucose tolerance test (OGTT) showed impaired glucose tolerance and diabetic curves. Based upon meta-analyses of alcohol-related traits in African American and European Americans in the Collaborative Study on the Genetics of Alcoholism, we tested 296 single nucleotide polymorphisms (SNPs with meta-analysis p values≤0.001) that were located in 3'UTRs.

    We identified 60 SNPs that affected gene expression (false discovery rate [FDR]<0.05) in SH-SY5Y cells and 92 that affected expression in SK-N-BE(2) cells. Among these, 30 SNPs altered RNA levels in the same direction in both cell lines. Many of these SNPs reside in the binding sites of miRNAs and RNA-binding proteins and are expression quantitative trait loci of genes including KIF6,FRMD4A,CADM2,ADD2,PLK2, and GAS7.

  • The occurrence of various micropollutants such as pharmaceuticals personal care products, endocrine disrupting chemicals (PPCPs/EDCs) and metals in municipal wastewater, and their poor removal efficiencies can lead to toxicity impact on humans, and freshwater and terrestrial ecosystems. Life cycle assessment is an efficient and effective tool to evaluate the environmental impact of wastewater treatment plants, but guidelines for toxicity assessment are lacking due to the complexity. This study aims to evaluate both life cycle inventory by including metals and PEC, and life cycle toxicity assessment (LCIA) methods namely CML-IA, Recipe, USEtox, EDIP 2003 and IMPACT 2002+ in midpoint category with a large centralised wastewater treatment plant in Malaysia as a case study. The removal efficiencies of metals and PPCPs/EDCs in the wastewater ranged from 9% to 99% and no clear patterns were found about occurrence and removal efficiencies of metals and PPCPs/EDCs in developing and developed countries. Here, we studied freshwater plankton communities in ten tropical reservoirs to record the impact of algal blooms, comprising different phytoplankton taxa, on water quality, plankton biodiversity, and ecosystem functioning. We compared water quality parameters (water transparency, mixing depth, pH, electrical conductivity, dissolved inorganic nitrogen, total dissolved phosphorus, total phosphorus, chlorophyll-a, and trophic state), plankton structure (composition and biomass), biodiversity (species richness, diversity, and evenness), and ecosystem functioning (phytoplanktonphosphorus and zooplanktonphytoplankton ratios as a metric of resource use efficiency) through univariate and multivariate analysis of variance, and generalized additive mixed models in five different bloom categories. Most of the bloom events were composed of Cyanobacteria, followed by Dinophyta and Chlorophytacteria and Chlorophyta blooms negatively impacted zooplankton species richness, while Dinophyta blooms decreased phytoplankton richness. In general, Cyanobacteria blooms presented low water quality and major threats to plankton biodiversity, and ecosystem functioning. Therefore, in this paper, we examine the impact of different routes of the China-Africa relationship on the environment. Using Fully Modified Ordinary Least Square (FMOLS) model on data from 50 African countries, we find that different Chinese activities affect the environment differently. We find a positive relationship between construction revenue and carbon emission, suggesting that China's construction activities negatively affect the environment. Similarly, export increases carbon emission and harms the environment. However, we find a negative relationship between importation from China and carbon emissions, implying a positive environmental footprint by China in Africa. In the case of foreign direct, the results show that foreign direct investment improves the environment, and the relationship is stronger in non-resource countries. Given that most exports from Africa are natural resources, our results imply that African non-resources-rich countries are likely to benefit from China's large investment in cleaner energy in the long-run, especially after the construction of the infrastructures. This study aims to evaluate both life cycle inventory by including metals and PEC, and life cycle toxicity assessment (LCIA) methods namely CML-IA, Recipe, USEtox, EDIP 2003 and IMPACT 2002+ in midpoint category with a large centralised wastewater treatment plant in Malaysia as a case study. We compared water quality parameters (water transparency, mixing depth, pH, electrical conductivity, dissolved inorganic nitrogen, total dissolved phosphorus, total phosphorus, chlorophyll-a, and trophic state), plankton structure (composition and biomass), biodiversity (species richness, diversity, and evenness), and ecosystem functioning (phytoplanktonphosphorus and zooplanktonphytoplankton ratios as a metric of resource use efficiency) through univariate and multivariate analysis of variance, and generalized additive mixed models in five different bloom categories. Most of the bloom events were composed of Cyanobacteria, followed by Dinophyta and Chlorophytacteria and Chlorophyta blooms negatively impacted zooplankton species richness, while Dinophyta blooms decreased phytoplankton richness. In general, Cyanobacteria blooms presented low water quality and major threats to plankton biodiversity, and ecosystem functioning. Moreover, we demonstrated that biodiversity losses decrease ecosystem functioning, with cascading effects on plankton dynamics.China has strategically engaged with African countries through different routes. However, the growing presence of China in Africa has attracted a lot of praise and criticism. As a leader in smart technology, China may fill the technological gaps in Africa, which improve the environment. Conversely, China may be exploiting natural resources and rapidly deteriorating the environment. Therefore, in this paper, we examine the impact of different routes of the China-Africa relationship on the environment. Using Fully Modified Ordinary Least Square (FMOLS) model on data from 50 African countries, we find that different Chinese activities affect the environment differently. We find a positive relationship between construction revenue and carbon emission, suggesting that China's construction activities negatively affect the environment. Similarly, export increases carbon emission and harms the environment. However, we find a negative relationship between importation from China and carbon emissions, implying a positive environmental footprint by China in Africa. In the case of foreign direct, the results show that foreign direct investment improves the environment, and the relationship is stronger in non-resource countries. Given that most exports from Africa are natural resources, our results imply that African non-resources-rich countries are likely to benefit from China's large investment in cleaner energy in the long-run, especially after the construction of the infrastructures. Here, the McGill DRAW (Data Rescue Archives and Weather) project shows how climatologists, geographers, archivists, data scientists, and coders together built a citizen-science-based transcription platform to transform the McGill Observatory paper records into a traceable and sustainable database.Alan Turing and Bletchley Park are rightly recognized for their work on breaking the Enigma code. However, this was built on a foundation of work during the 1930s by the Polish cryptographer, Marian Rejewski. Often working alone, and with limited resources, he found ways to break early Enigma code. Repositories can play an important role in this process by augmenting metadata through time with persistent identifiers and connections they facilitate. Data providers need to work with repositories to encourage metadata evolution as new capabilities and connections emerge.Traditionally, networks have been studied in an independent fashion. With the emergence of novel smart city technologies, coupling among networks has been strengthened. To capture the ever-increasing coupling, we explain the notion of interdependent networks, i.e., multi-layered networks with shared decision-making entities, and shared sensing infrastructures with interdisciplinary applications. The main challenge is how to develop data analytics solutions that are capable of enabling interdependent decision making. One of the emerging solutions is agent-based distributed decision making among heterogeneous agents and entities when their decisions are affected by multiple networks. We first provide a big picture of real-world interdependent networks in the context of smart city infrastructures. We then provide an outline of potential challenges and solutions from a data science perspective. We discuss potential hindrances to ensure reliable communication among intelligent agents from different networks. We explore future research directions at the intersection of network science and data science.

  • This good clinical practice paper provides an overview of the current evidence around second stage care, highlighting the challenges and the importance of maintaining high-quality, safe, and respectful care in all settings. It includes a series of recommendations based on best available evidence regarding length of second stage, judicious use of episiotomy, and the importance of competent attendants and adequate resource to facilitate all aspects of second stage management, from physiological birth to assisted vaginal delivery and cesarean at full dilatation. The second stage of labor is potentially the most dangerous time for the baby and can have significant consequences for the mother, including death or severe perineal trauma or fistula, especially where there are failures to recognize and repair. This paper sets out principles of care, including the vital role of skilled birth attendants and birth companions, and the importance of obstetricians and midwives working together effectively and speaking with one voice, whether to women or to policy makers. The optimization of high-quality, safe, and personalized care in the second stage of labor for all women globally can only be achieved by appropriate attention to the training of birth attendants, midwives, and obstetricians. FIGO is committed to this aim alongside the WHO, ICM, and all FIGO's 132 member societies.The inability to thoroughly examine diatom type material existing only in resin-mounted permanent slides is a common frustration for diatomologists. Here, we present an efficient, inexpensive, and straightforward technique to extract siliceous exoskeletons of diatoms from permanent slides prepared with commonly used mounting media. Van Heurck's slide constituting an isotype duplicate of Proschkinia bulnheimii var. belgica, an entity with an uncertain taxonomic status, was deconstructed to allow thorough examination using scanning electron microscopy. Numerous specimens of the taxon, including intact frustules and specimens with complete areola occlusions, were recovered and documented. The extracted diatom material was sufficient to prepare two scanning electron microscopy specimens and reconstruct the permanent slide. Based on ARF cases reported in VigiBase, and despite the caveats inherent to COVID-19 circumstances, we detected a statistically significant pharmacovigilance signal of nephrotoxicity associated with remdesivir, deserving a thorough qualitative assessment of all available data. Meanwhile, as recommended in its Summary of Product Characteristics, assessment of patients with COVID-19 renal function should prevail before and during treatment with remdesivir in COVID-19.
    The purpose of this study was to evaluate the effects of trigger point (TrP) dry needling alone or as an adjunct to other interventions on pain intensity and related disability in nontraumatic shoulder pain.

    Ten databases were searched from inception to January 2020 for randomized clinical trials in which at least 1 group received TrP dry needling for shoulder pain of musculoskeletal origin with outcomes collected on pain intensity and related disability. Data extraction including participant and therapist details, interventions, blinding strategy, blinding assessment outcomes, and results were extracted by 2 reviewers. The risk of bias (Cochrane Risk of Bias, Cochrane Guidelines), methodological quality (Physiotherapy Evidence Database score), and evidence level (Grading of Recommendations Assessment, Development, and Evaluation approach) were assessed. The search identified 551 publications with 6 trials eligible for inclusion.

These consequences could erode public trust in AI, which could in turn undermine trust in our healthcare institutions. This article makes 2 contributions. First, it describes the major conceptual, technical, and humanistic challenges in medical AI. Second, it proposes a solution that hinges on the education and accreditation of new expert groups who specialize in the development, verification, and operation of medical AI technologies. These groups will be required to maintain trust in our healthcare institutions.Multicellular organisms have evolved sophisticated mechanisms to recover and maintain original tissue functions following injury. Injury responses require a robust transcriptomic response associated with cellular reprogramming involving complex gene expression programs critical for effective tissue repair following injury. Ten databases were searched from inception to January 2020 for randomized clinical trials in which at least 1 group received TrP dry needling for shoulder pain of musculoskeletal origin with outcomes collected on pain intensity and related disability. Data extraction including participant and therapist details, interventions, blinding strategy, blinding assessment outcomes, and results were extracted by 2 reviewers. The risk of bias (Cochrane Risk of Bias, Cochrane Guidelines), methodological quality (Physiotherapy Evidence Database score), and evidence level (Grading of Recommendations Assessment, Development, and Evaluation approach) were assessed. The search identified 551 publications with 6 trials eligible for inclusion. There was moderate-quality evidence that TrP dry needling reduces shoulderic shoulder pain. Dry needling is commonly used for the management of musculoskeletal pain. This is the first meta-analysis to examine the effects of dry needling on nontraumatic shoulder pain.Artificial intelligence (AI) is increasingly of tremendous interest in the medical field. How-ever, failures of medical AI could have serious consequences for both clinical outcomes and the patient experience. These consequences could erode public trust in AI, which could in turn undermine trust in our healthcare institutions. This article makes 2 contributions. All cases were performed open reduction and internal fixation using 1 or 2 titanium plates with both the postoperative immediate CT (T1) and the over 6 months' follow-up CT (T2). Mandibles and implants were segmented and reconstructed respectively using SIMPANT 14.04 software. For 1 miniplate group, the sagittal crossing angle (<1) between miniplate (P1) and posterior border of ipsilateral ramus was measured. For 2 miniplates group, the sagittal crossing angle (<2) between 2 miniplates (P2a, P2b) was measured. Both anteroposterior (∠AP) and mediolateral angle change (∠ML)calization showed potential for the stability evaluation of internal fixation of condylar fracture.     It is challenging to repair postoperative defect caused by skin tumor resection on the maxillofacial, which not only affects appearance but also impairs functions. To better repair skin defect on the maxillofacial, the application value of V-Y vascular myocutaneous flap was introduced in our study.

    Between June 2011 and December 2018, 16 patients with maxillofacial skin tumors who received extensive resection were enrolled in our study. The defect on the maxillofacial was repaired by V-Y vascular myocutaneous flap. The follow-up period lasted for 12 to 24 months.

    All 16 cases of myocutaneous ***** survived with 1 case of partial venous congestion and 1 case of partial distal necrosis. No recurrence occurred during follow-up. The color and texture of myocutaneous ***** like those of the surrounding skin.

    Sinonasal inverted papilloma (IP) is a benign but locally aggressive tumor for which an endoscopic or external surgical approach is the treatment of choice. Complete resection of IP involving the frontal sinus/recess forms one of the most challenging procedures in the field of sinonasal surgery. This study aims to present our experience in the management of extensive frontal sinus IP based on the attachment sites of the tumor.

    Thirteen patients with IP involving the frontal sinus/recess between 2010 and 2018 were presented. The data collected include demographic data, tumor attachment sites, tumor extension, tumor staging according to Meng's staging system, surgical approach, recurrence, and follow-up.

    The patients were successfully treated by endoscopic surgery without any additional external approaches. The attachment sites of the IP were multifocal in some patients. No recurrence was identified after an average follow-up period of 52.88 months. No major intra- or postoperative complications were observed.

  • The coronavirus disease 2019 (COVID-19) appears to be the largest pandemic of our times. The aim was to recognize the risk factors for nonpsychotic postpartum mood and anxiety disorders (NPMADs) in women during the pandemic and state of emergency police lockdown in Serbia.

    We assessed 108 postpartum women who completed the Edinburgh Postnatal Depression Scale (EPDS) and an additional survey constructed for this study. We also used the additional, previously mentioned survey, in 67 healthy age-matched women with children who were ≥2 years of age. The additional survey allowed us to gain insight into the impact of the pandemic as well as postpartum period on the risk of NPMADs.

    In 16 (14.8%) subjects we found a score ≥10 on EPDS. Higher rates on the EPDS were noticed in elderly, single, and unemployed, women who lost their jobs due to the pandemic, or women who were dissatisfied with their household income (p < 0.05). The risk of NPMADs was linked significantly to quarantine, and social isolation, the absence of social support, as well as having emotional problems. Postpartum women, compared to non-postpartum women, were more anxious and had feelings of helplessness during social isolation.

    Understanding the factors that increase the risk of NPMADs during the pandemic could help prevent mental disorders during a possible future pandemic.

The mean prevalence of injuries among the included studies was 35.3%, with an incidence rate varying between 0.2 and 18.9 per 1000 hours of training. The most injured areas were shoulder (26%), spine (24%), and knee (18%). Among the studies that reported the injuries requiring surgery, the mean percentage was 8.7%. Regarding the risk factors associated with injuries, older age, male sex, a greater body mass index, the existence of previous injuries, the lack of coach supervision, the experience on CrossFit® and the participation in competitions were reported by the studies.Conclusions CrossFit® training has an injury incidence rate similar to weightlifting and powerlifting. Findings from the studies suggest that the most affected areas are shoulder, spine, and knee. A 33-year-old male presented with a progressively increasing swelling in the temporal part of the right upper eyelid of 5-month duration. The patient was misdiagnosed as chalazion and underwent incision and curettage locally. Subsequently, the patient presented with recurrence of the swelling and was treated with excision of the lesion. Histopathology showed the presence of spindle- to stellate-shaped cells in a myxoid matrix. Alcian blue stain was positive for mucin. The systemic evaluation did not show any components of Carney's complex. It is important to be aware of this entity of cutaneous eyelid myxoma as a rare cause of eyelid swelling and its propensity to recur rapidly after incomplete excision.

    Idiopathic Pulmonary Fibrosis (IPF) is a chronic degenerative disease with a median survival of 2-5 years after diagnosis. Therefore, IPF patient identification represents an important and challenging clinical issue. Current research is still searching for novel reliable non-invasive biomarkers. Therefore, we explored the potential use of long non-coding RNAs (lncRNAs) and mRNAs as biomarkers for IPF.

    We first performed a whole transcriptome analysis using microarray (
    = 14 7 Control, 7 IPF), followed by the validation of selected transcripts through qPCRs in an independent cohort of 95 subjects (
    = 95 45 Control, 50 IPF). Diagnostic performance and transcript correlation with functional/clinical data were also analyzed.

    1059 differentially expressed transcripts were identified. We confirmed the downregulation of FOXF1 adjacent non-coding developmental regulatory RNA (FENDRR) lncRNA, hsa_circ_0001924 circularRNA, utrophin (UTRN) and Y-box binding protein 3 (YBX3) mRNAs. The two analyzed non-coding RNAs correlated with Forced Vital Capacity (FVC)% and Diffusing Capacity of the Lung for carbon monoxide (DLCO)% functional data, while coding RNAs correlated with smock exposure. All analyzed transcripts showed excellent performance in IPF identification with Area Under the Curve values above 0.87; the most outstanding one was YBX3 AUROC 0.944, CI 95% = 0.895-0.992, sensitivity = 90%, specificity = 88.9%,

A narrative review was conducted with the aim of guiding clinicians on the management of pregnant women with respect to congenital, intrapartum, and postnatal maternal-fetal-neonatal SARS-CoV-2 infections and breastfeeding during the COVID-19 pandemic.

    Searches were conducted in Web of Science, PubMed, Scopus, Dialnet, CUIDEN, Scielo, and Virtual Health Library to identify observational, case series, case reports, and randomized controlled trial studies assessing the transmission of SARS-CoV-2 from mother to baby and/or through breastfeeding during the COVID-19 pandemic.

    A total of 49 studies was included in this review, comprising 329 pregnant women and 331 neonates (two pregnant women delivered twins). The studies were performed in China (

    = 7), Italy (
    = 3), Iran (
    = 2), Switzerland (
    = 1), Spain (
    = 1), Turkey (
    = 1), Ademic. Accumulating evidence has shown that the skin barrier function is regulated via competition between the aryl hydrocarbon receptor (AHR) axis (up-regulation of barrier) and the IL-13/IL-4‒JAK‒STAT6/STAT3 axis (down-regulation of barrier). This latter axis also induces oxidative stress, which exacerbates inflammation. Conventional and recently developed agents for treating AD such as steroid, calcineurin inhibitors, cyclosporine, dupilumab, and JAK inhibitors inhibit the IL-13/IL-4‒JAK‒STAT6/STAT3 axis, while older remedies such as coal tar and glyteer are antioxidative AHR agonists. In this article, I summarize the pathogenic and therapeutic implications of the IL-13/IL-4‒JAK‒STAT6/STAT3 axis and the AHR axis in AD.Schizophrenia is a mental disorder that affects approximately 1-2% of the population and develops in early adulthood. The disease is characterized by positive, negative, and cognitive symptoms. A large percentage of patients with schizophrenia have a treatment-resistant disease, and the risk of developing adverse effects is high. Many researchers have attempted to introduce new antipsychotic drugs to the clinic, but most of these treatments failed, and the diversity of schizophrenic symptoms is one of the causes of disappointing results. The present review summarizes the results of our latest papers, showing that the simultaneous activation of two receptors with sub-effective doses of their ligands induces similar effects as the highest dose of each compound alone. The treatments were focused on inhibiting the increased glutamate release responsible for schizophrenia arousal, without interacting with dopamine (D2) receptors. Ligands activating metabotropic receptors for glutamate, GABAB or muscarinic receptors were used, and the compounds were administered in several different combinations.
