  • There is no standard instrument for carrying out medical percussion even though the procedure has been in continuous use since 1761. This study developed one such instrument. It generates medical percussion sounds in a reproducible manner and accurately classifies them into one of three classes. Percussion signals were generated using a push-pull solenoid plessor applying mechanical impulses through a polyvinyl chloride plessimeter. Signals were acquired using a National Instruments USB 6251 data acquisition card at a rate of 8.192 kHz through an air-coupled omnidirectional electret microphone located 60 mm from the impact site. Signal acquisition, processing, and classification were controlled by an NVIDIA Jetson TX2 computational device. A complex Morlet wavelet was selected as the base wavelet for the wavelet decomposition using the maximum wavelet energy method. It was also used to generate a scalogram suitable for manual or automatic classification. Automatic classification was achieved using a MobileNetv2 convolutional neural network with 17 inverted residual layers on the basis of 224 × 224 x 1 images generated by downsampling each scalogram. Testing was carried out using five human subjects with impulses applied at three thoracic sites each to elicit dull, resonant, and tympanic signals respectively. Classifier training utilized the Adam algorithm with a learning rate of 0.001, and first and second moments of 0.9 and 0.999 respectively for 100 epochs, with early stopping. Mean subject-specific validation and test accuracies of 95.9±1.6% and 93.8±2.3% respectively were obtained, along with cross-subject validation and test accuracies of 94.9% and 94.0% respectively. These results compare very favorably with previously-reported systems for automatic generation and classification of percussion sounds.
    Despite a long history of ECG-based monitoring of acute ischemia quantified by several widely used clinical markers, the diagnostic performance of these metrics is not yet satisfactory, motivating a data-driven approach to leverage underutilized information in the electrograms. This study introduces a novel metric for acute ischemia, created using a machine learning technique known as Laplacian eigenmaps (LE), and compares the diagnostic and temporal performance of the LE metric against traditional metrics.

    The LE technique uses dimensionality reduction of simultaneously recorded time signals to map them into an abstract space in a manner that highlights the underlying signal behavior. To evaluate the performance of an electrogram-based LE metric compared to current standard approaches, we induced episodes of transient, acute ischemia in large animals and captured the electrocardiographic response using up to 600 electrodes within the intramural and epicardial domains.

    The LE metric generally detected ischemia earlier than all other approaches and with greater accuracy. Unlike other metrics derived from specific features of parts of the signals, the LE approach uses the entire signal and provides a data-driven strategy to identify features that reflect ischemia.

    The superior performance of the LE metric suggests there are underutilized features of electrograms that can be leveraged to detect the presence of acute myocardial ischemia earlier and more robustly than current methods.

    The earlier detection capabilities of the LE metric on the epicardial surface provide compelling motivation to apply the same approach to ECGs recorded from the body surface.

  • Toward that end, we review findings from the neuroscience of creativity, behavioral research on the impact of stress on creativity, and the systems-level view of the brain under stress to suggest ways in which creativity might be affected under extreme conditions. Although our focus is largely on acute stress, we also touch on the possible impact of chronic stress on creative cognition.Executive function is the center of cognitive function, emotional function, and social function, and plays an important role in children's cognitive development. Previous studies used music, sports, and other training methods to promote the development of children's executive function. but researchers are still exploring more comprehensive and effective training methods. Street-dance, as a comprehensive dance form integrating the characteristics of movement, music, rhythm, and so on, needs the coordination of individual sensory systems and a sense of musical rhythm and action. These are the same activity elements found in previous studies that can improve the individual executive function of children. In order to investigate the promoting effect of street-dance training on children's executive function, this study designed a street-dance training program integrating the characteristics of each component of executive function. Sixty preschool children around the age of four (M = 52.4, SD = 3.95) participated using the pretest-posttest experimental design. The dancing group conducted street-dance training 3 times a week, 40-50 min each time for a total of 24 times; the control group did not train. We discovered that 8 weeks of street-dance training can promote the development of executive function in preschool children, and we discussed about the potential mechanism of the street dance training effects and the implications of intervention programs.[This corrects the article DOI 10.3389/fpsyg.2020.01603.].This study examines whether character strengths predict resilience (operationalized as stable or higher mental health and subjective well-being despite an adverse event) over a period of approximately 1 month during the COVID-19 pandemic lockdown in Spain. Using a longitudinal design, participants (N = 348 adults) completed online measures of sociodemographic data, information regarding their situation in relation to the COVID-19, character strengths, general mental health, life satisfaction, positive affect and negative affect. All variables were measured at Time 1 and Time 2, except for sociodemographic and most COVID-related information (Time 1 only). Time 1 data collection was conducted between March 21, 2020 and April 2, 2020, i.e., approximately the second week of lockdown in Spain. Time 2 data collection was conducted between April 24, 2020 and May 18, 2020, after the Spanish government announced its intention to progressively release the lockdown. A principal component analysis of character strengths al strengths predicted an increase in life satisfaction. Finally, fortitude strengths, interpersonal strengths, and strengths of restraint, predicted a decrease in negative affect. None of the reversed analyses yielded significant effects. Limitations, implications, and possible character strengths-based interventions aimed at promoting mental health in the COVID-19 pandemic are discussed.Drawing on the job demands-resources (JD-R) model and event system theory (EST), this study constructed a moderated mediating model to investigate the direct effect of career calling on work fatigue, the mediating effect of role overload, and the moderating effect of COVID-19 event disruption in the above relationships. We administered an online questionnaire to 488 Chinese police officers who participated in frontline work to prevent and control the COVID-19 pandemic. The results showed a U-shaped curvilinear relationship of career calling with physical fatigue, mental fatigue, emotional fatigue, and role overload. Moreover, role overload partially mediated these curvilinear relationships. In addition, COVID-19 event disruption positively moderated the direct curvilinear effect of career calling on role overload, physical fatigue, and emotional fatigue, as well as the first stage of the mediating effect in the relationship between career calling and physical, mental, and emotional fatigue through role overload. Furthermore, the direct U-shaped curvilinear effects and the indirect effects were more significant when COVID-19 event disruption was high.This research aims to develop and validate a Spanish version of The Brief Scale of Fear of Loneliness (BSFL). Participants were 1385 youth and adults, 347 from a pilot sample and 1032 from the final version, whose ages were in the range of 18 to 40 years. Two instruments, the Rosenberg Self-Esteem Scale and the De Jong Gierveld Loneliness Scale, in their Peruvian versions, were used to support the relationship with other variables. Results show that the BSFL should be interpreted as a one-dimensional measure, the same ones that were examined at the exploratory level and verified at the confirmatory moment (RMSEA 0.88). In addition, the quality of the item content was reviewed by six expert judges for relevance and validity, with Aiken's V being greater than 0.70. It is concluded that the BSFL is a valid and precise short instrument that can be used in future research studies.Despite a growing body of research on the relationship between justice and perceptions of an authority's legitimacy, few studies have addressed the effects of changes in justice on perceived legitimacy. This work concerns the production and characterisation of an oxidative enzyme identified from the transcriptome of marine-derived fungus Stemphylium lucomagnoense. The laccase-encoding gene SlLac2 from S. lucomagnoense was cloned for heterologous expression in Aspergillus niger D15#26 for protein production in the extracellular medium of around 30 mg L-1. The extracellular recombinant enzyme SlLac2 was successfully produced and purified in three steps protocol ultrafiltration, anion-exchange chromatography, and size exclusion chromatography, with a final recovery yield of 24%. SlLac2 was characterised by physicochemical properties, kinetic parameters, and ability to oxidise diverse phenolic substrates. We also studied its activity in the presence and absence of sea salt.   • Microbial communities with the ability to convert long-chain fatty acids (LCFA) coupled to sulfate reduction can be important in the removal of these compounds from wastewater. In this work, an enrichment culture, able to oxidize the long-chain fatty acid palmitate (C160) coupled to sulfate reduction, was obtained from anaerobic granular sludge. Here, we used P. aeruginosa as a model microorganism to investigate the effect of traditional Chinese medicine Tanreqing (TRQ) formula on bacterial pathogenicity. Phenotypic analysis showed that TRQ treatment could completely inhibit the production of phenazine pyocyanin and moderately inhibit the production of virulence factors such as rhamnolipids, elastase, and alkaline protease. Further transcriptomic analyses revealed that TRQ treatment could significantly attenuate the expression of QS-regulated genes in P. aeruginosa and TRQ-treated P. aeruginosa regulon shared a large overlap with QS regulon. Component contribution to QS inhibition shed light on the indispensable role of all five components in TRQ formula. Further genetic analysis indicated that upstream regulators of QS systems, including two-component systems GacS/GacA and PprA/PprB, were both inhibited by TRQ treatment. Finally, our TRQ formula could efficiently protect Caenorhabditis elegans from killing by P. aeruginosa. Older age, male sex, active and severe disease correlate with asymmetric disease. However, the exact mechanisms causing asymmetry remain elusive. Herein, we review the literature on asymmetric GO and highlight its differences compared with bilateral GO.Thyroid-associated ophthalmopathy (TAO) has remained a vexing and poorly managed autoimmune component of Graves' disease where the tissues surrounding the eye and in the upper face become inflamed and undergo remodeling. This leads to substantial facial disfigurement while in its most severe forms, TAO can threaten eye sight. In this brief paper, I review some of the background investigation that has led to development of teprotumumab as the first and only US FDA approved medical therapy for TAO. This novel treatment was predicated on recognition that the insulin-like growth factor I receptor plays an important role in the pathogenesis of TAO. It is possible that a similar involvement of that receptor in other autoimmune disease may lead to additional indications for this and alternative insulin-like growth factor I receptor-inhibiting strategies.

  • Heterotrophic Gamma-proteobacterium Shewanella algae **** 12715, associated with an intertidal red algae Hypnea valentiae, presented broad-spectra of antibacterial activities against pathogenic bacteria bringing about nosocomial infection. Bioassay-guided fractionation of the bacterial crude extract resulted in two undescribed macrocyclic polyketide analogs, with anti-infective activities against methicillin-resistant Staphylococcus aureus and vancomycin-resistant Enterococcus faecalis (****3.1-5.0 µg/mL). In order to identify the polyketide biosynthetic machinery termed type-I polyketide synthase (pks-I) encoding biologically active secondary metabolites in this strain, the ketosynthase-coding regions of DNA with ≈700 bp size, were amplified, and the partial sequence was submitted in the GenBank (accession number MH157093). The titled compounds were classified under macrocyclic polyketides bearing dodecahydropyrano-trioxacyclooctadecine-dione and trioxo-octadecahydro-1H-benzo[o]tetraoxacyclopentacosine-carboxylate functionalities. Structure-activity correlation analysis displayed that hydrophobic descriptor of the studied compounds could play a prominent role in its anti-infective property against the opportunistic pathogens. The titled compounds were classified under macrocyclic polyketides bearing dodecahydropyrano-trioxacyclooctadecine-dione and trioxo-octadecahydro-1H-benzo[o]tetraoxacyclopentacosine-carboxylate functionalities. Structure-activity correlation analysis displayed that hydrophobic descriptor of the studied compounds could play a prominent role in its anti-infective property against the opportunistic pathogens. Based on factorial design, F6 had the best formula with desirability value of 0.54, but the design recommended that formula with the composition bases of poloxamer 407 and HPMC at the ratio of 8.16 % w/v and 0.77 % w/v, respectively, was the optimum formula with a desirability value of 0.69.

So, this work aimed to compare the chemical composition and physical chemistry of powdered and jambu leaf mass. Acmella oleracea plants were selected, sanitized and their leaves were crushed (without adding water). The mass was dried in an oven with forced air circulation under different conditions of temperature (60, 70 and 80 °C) and relative humidity (13.09, 8.14 and 5.45%, respectively), in two layer thicknesses (0.5 and 1.0 cm). The physical, chemical, physicochemical parameters moisture content, water activity, ash, protein, lipids, pH, total titratable acidity and color were analyzed in powdered and fresh leaves. Contrast test was used to evaluate the interaction between the layer thicknesses at 5% level of significance. Considering the contrasts, the parameters total soluble solids, ash, protein and lipids do not differ from those in the fresh sample, demonstrating that these parameters are not modified or degraded with the drying conditions. The pH and color parameters differed from those of the fresh material, due to degradation, which can be linked to the rise in temperature sensitivity. The moisture content and water activity also differed and were reduced, contributing to conservation of the material. Contrast analysis made it possible to conclude that the physicochemical composition did not undergo modifications through the use of drying. Two-hundred and eight questionnaires were used to gather quantitative data on socio-demographics and knowledge levels while 18 focus group discussions were used to collect qualitative data on perceptions and social constructs. The Fisher's exact and Chi-square tests were used to determine the differences in community knowledge levels based on localities, gender, religion and age.

    We used a United Kingdom primary healthcare database, the Clinical Practice Research Datalink (CPRD), linked with secondary healthcare and mortality data, to derive an open 2005-2013 cohort of adults (≥18 years of age) with reduced kidney function (≥2 measures of estimated glomerular filtration rate [eGFR] <90 mL/min/1.73 m2 ≥90 days apart). Data on individuals' sociodemographic and clinical characteristics at entry and outcomes (first occurrences of stroke, myocardial infarction (MI), and hospitalisation for heart failure; annual kidney disease stages; and cardiovascular and nonvascular deaths) during follow-up were extracted. The cohort was used to estimate risk equations for outcomes and develop a chronic kidney disease-cardiovascular disease (CKD-CVD)tments is optimised.
    The CKD-CVD policy model is a novel resource for projecting long-term health outcomes and assessing treatment strategies in people with reduced kidney function. The answer to this question may have implication for understanding where morality comes from and how people make moral judgments. This study aimed to compared the neural mechanisms underlying two kinds of moral violation by using an affective priming task to test the effect of sentences depicting moral violation behaviors with and without physical impurity on subsequent detection of disgusted faces in a visual search task. After reading each sentence, participants completed the face search task. Behavioral and electrophysiological (event-related potential, or ERP) indices of affective priming (P2, N400, LPP) and attention allocation (N2pc) were analyzed. Results of behavioral data and ERP data showed that moral violations both with and without impurity promoted the detection of disgusted faces (RT, N2pc); moral violations without impurity impeded the detection of neutral faces (N400). No priming effect was found on P2 and LPP. The results suggest both types of moral violation influenced the processing of disgusted faces and neutral faces, but the neural activity with temporal characteristics was different.

    A cohort study was designed to evaluate whether a therapeutic algorithm using steroids with or without interleukin-1 antagonist (anakinra) could prevent death/invasive ventilation. Patients with a ≥5-day evolution since symptoms onset, with hyper-inflammation (CRP≥50mg/L), requiring 3-5 L/min oxygen, received methylprednisolone alone. Patients needing ≥6 L/min received methylprednisolone + subcutaneous anakinra daily either frontline or in case clinical deterioration upon corticosteroids alone. Death rate and death or intensive care unit (ICU) invasive ventilation rate at Day 15, with Odds Ratio (OR) and 95% CIs, were determined according to logistic regression and propensity scores. A Bayesian analysis estimated the treatment effects.

    Of 108 consecutive patients, 70 patients received glucocorticoids alone. The control group comprised 63 patients receiving standard of care. In the corticosteroid±stanakinra group (n = 108), death rate was 20.4%, versus 30.2% in the controls, indicating a 30% relative decrease in death risk and a number of 10 patients to treat to avoid a death (p = 0.15). Data on individuals' sociodemographic and clinical characteristics at entry and outcomes (first occurrences of stroke, myocardial infarction (MI), and hospitalisation for heart failure; annual kidney disease stages; and cardiovascular and nonvascular deaths) during follow-up were extracted. This study aimed to compared the neural mechanisms underlying two kinds of moral violation by using an affective priming task to test the effect of sentences depicting moral violation behaviors with and without physical impurity on subsequent detection of disgusted faces in a visual search task. After reading each sentence, participants completed the face search task. Behavioral and electrophysiological (event-related potential, or ERP) indices of affective priming (P2, N400, LPP) and attention allocation (N2pc) were analyzed. Results of behavioral data and ERP data showed that moral violations both with and without impurity promoted the detection of disgusted faces (RT, N2pc); moral violations without impurity impeded the detection of neutral faces (N400). No priming effect was found on P2 and LPP. The results suggest both types of moral violation influenced the processing of disgusted faces and neutral faces, but the neural activity with temporal characteristics was different. A cohort study was designed to evaluate whether a therapeutic algorithm using steroids with or without interleukin-1 antagonist (anakinra) could prevent death/invasive ventilation. Patients with a ≥5-day evolution since symptoms onset, with hyper-inflammation (CRP≥50mg/L), requiring 3-5 L/min oxygen, received methylprednisolone alone.     We reviewed the oncologic and surgical outcomes of endoscopic treatments for low grade upper tract urothelial carcinoma, and assessed the prognostic significance of tumor size, location and multifocality.

    We retrospectively reviewed all patients who underwent endoscopic treatment for low grade upper tract urothelial carcinoma at our institution between 2014 and 2019. Tumors were treated with a dual laser generator, which alternately produces holmium and neodymium lasers. A stringent ureteroscopic followup protocol was conducted. We looked for an association between outcome and tumor size, location or multifocality, and for predictive factors for time to local recurrence and progression.

    The cohort included 59 patients (62 renal units), 27% of tumors were multifocal and 40% were >2 cm. The median followup time was 22 months (IQR 11-41), and the median number of ureteroscopies was 5.5 (4-9). From the lens of this "new normal" state, we have an opportunity to rapidly implement, test, and iterate models of diabetes care to achieve the quadruple aim of improving medical outcomes, patient experience, provider satisfaction, and reducing costs. In this perspective, we discuss several innovative diabetes models of care which promote collaborative care models and improve access to high-quality specialty diabetes care. We discuss ongoing threats to diabetes care innovation, and offer practical solutions to foster evolution and sustain current strides made during the pandemic.
    The significance of lymphadenectomy is yet to be fully examined in segmentectomy. We compared the oncological outcomes of mediastinal lymph node dissection (LND) and hilar LND for lung cancer treated with segmentectomy via a multicenter database using propensity score-matched analysis.

    We reviewed 357 clinical stage IA radiologically solid-dominant lung cancer patients who underwent segmentectomy with lymphadenectomy. The extent of LND was classified into systematic/lobe-specific mediastinal LND and hilar LND only groups. Postoperative results after segmentectomy with mediastinal LND (n=179) and hilar LND (n=178) were analyzed for all patients and their propensity score-matched pairs.

    Cancer-specific survival (CSS) and recurrence-free interval (RFI) rates for the mediastinal LND group were determined to be not significantly different compared with the hilar LND group in all non-adjusted cohorts. In the propensity score-matched cohort (129 pairs), mediastinal LND harvested more lymph nodes compared with hilar LND, and both groups had significantly different pathological stages (P=0.015). Adjuvant chemotherapy was performed in 10 (7.8%) patients in the mediastinal LND group and 4 (3.1%) in the hilar LND group. The mediastinal LND group tended to have better prognosis than the hilar LND group (5-year CSS rates, 97.4% vs 93.2%; 5-year RFI rates, 93.5% vs 88.5%).
