Hepatic artery thrombosis (HAT), a serious complication after orthotopic liver transplantation, almost always leads to morbidity and mortality without urgent revascularization or retransplantation, especially if HAT occurs within a few days after transplantation.
Herein we describe a case report of an orthotopic liver transplantation patient surviving without hepatic artery flow due to HAT on postoperative day 1. Reanastomosis, thrombectomy, and intra-arterial thrombolysis were performed, but only retrograde arterial flow by Doppler ultrasound, not by angiography, could be demonstrated in the hepatic artery. This case report is in compliance with the Declaration of Helsinki and the Declaration of Istanbul.
Based on the evidence from this patient, we believe that patients with failed revascularization can experience a long-term survival with conservative treatment. Retransplantation should be evaluated based on laboratory findings because graft function in individual patients can recover.

The average time interval from KT to AML development is 5 years. We present the case of a 61-year-old man who was found to have peripheral blood blasts on a postoperative day 20 routine blood draw after an uneventful unrelated living donor kidney transplant. He subsequently had a bone marrow biopsy and next-generation sequencing (NGS)-based molecular testing, which demonstrated AML characterized by SMC1A and TET2 mutations. He received induction chemotherapy followed by hematopoietic cell transplantation (HCT) from the kidney donor, who happened to be matched at one haplotype. At 12 months after his HCT and 15 months after his KT, his AML remained in remission, normal renal function was preserved, no active graft-versus-host disease was present, and immunosuppression was tapering. With full donor-derived hematopoietic chimerism, we expect to be able to discontinue immunosuppression shortly, thereby achieving tolerance. The short time interval between KT and development of AML suggests the malignancy was likely present before KT. Modern NGS-based analysis offers a promising method of identifying transplant candidates with unexplained hematologic abnormalities on pre-KT testing who may benefit from formal hematologic evaluation.
The usage of extended-criteria donors (ECD) became a routinely accepted manner in the last decade. ECD is a potential risk factor for antibody-mediated rejection. Analysis of lymphocyte subsets might be a complementary diagnostic toolkit because there is limited knowledge about this term.
Between May 12, 2016, and September 4, 2019, a total of 130 patients who had undergone kidney transplant were investigated. Patients were divided in ECD and standard criteria donor (SCD) groups. Blood samples were collected before the operation, then in the first week and after 30, 60, 180, and 365 days. Besides routine laboratory tests, multicolor flow cytometry was performed for lymphocyte subsets.
ECD grafts were transplanted to older recipients. The number of CD4+ cells increased in the SCDs from the first week to until the end of first month, and then decreased. The number of CD4+ cells decreased from the beginning of the study until the end of first year to 66% of its original value in ECDs. At the first month, te poorer initial function, together with an aging immune system, explains the weaker immune response in ECD patients, which may be the cause of the decreased number of memory and regulatory T cells. Older patients with an ECD graft need a tailored, personalized, and less aggressive immunosuppressive treatment.
Coronavirus disease 2019 (COVID-19) has affected all facets of life and continues to cripple nations. COVID-19 has taken the lives of more than 2.1 million people worldwide, with a global mortality rate of 2.2%. Current COVID-19 treatment options include supportive respiratory care, parenteral corticosteroids, and remdesivir. Although COVID-19 is associated with increased risk of morbidity and mortality in patients with comorbidities, the vulnerability, clinical course, optimal management, and prognosis of COVID-19 infection in patients with organ transplants has not been well described in the literature. The treatment of COVID-19 differs based on the organ(s) transplanted. Preliminary data suggested that liver transplant patients with COVID-19 did not have higher mortality rates than untransplanted COVID-19 patients. Table 1 depicts a compiled list of current published data on COVID-19 liver transplant patients. Most of these studies included both recent and old liver transplant patients. No distinction wamanagement and recovery of severe COVID-19 pneumonia in post-liver transplant patients. Future studies and experiences from posttransplant patients are warranted to better delineate the clinical features and optimal management of COVID-19 infection in liver transplant recipients.

Part 1 The water uptake behaviour of Aquasil-Ultra-Monophase (AqM) was studied gravimetrically in three media (DW, 1%NaOCl and Perform ID), at 23 °C and 37 °C, over a period of one and four-months (n = 5) to gain information on long-term immersion. Part 2 Five experimental materials were formulated Exp-I and II as hydrophobic and Exp-III-V as hydrophilic, containing an additional cross-linking agent (TFDMSOS) and Rhodasurf CET-2 surfactant. Their water uptake and desorption (both at 23 °C) properties (gravemetrically), solubility, pH and diffusion coefficient (DC) data were compared with three commercial, hydrophilic VPS impression materials, over seven days (n = 5). The results were analysed statistically.
Part 1 Significant differences in water absorbed by AqM were observed in the three media at 23 °C. Aq M had a significantly higher uptake in 2% Perform ID, than in DW and NaOCl.

Nonunion continues to be the most frequent and challenging complication to treat following fracture fixation. Herein, we carried out a bibliometric analysis aiming to identify the key researchers, centres and research trends developed during the past 30 years in this important clinical condition.
The Science Citation Index Expanded database and the Web of Science Core Collection were interrogated for manuscripts published between 1990 and 2019 in the topic domain, utilising title, abstract, author keywords and KeyWords Plus. Overall, such citation indicators were used as TC

to help analyse the identified manuscripts.
Over the prespecified period, there was a steady increase in the number of articles published in fracture nonunion. In total, 12 languages were the primary languages in the documents, with English being the most prevalent. The CPP sharply increased to reach a plateau in three full years and up to a peak in ten full years. A total of 8976 nonunion-related articles in Sciencemation on citation number, publication outputs, categories, journals, institutions, countries, research highlights and tendencies. The current research activity on fracture nonunion identified key opinion leaders and leading research institutions focusing on this important clinical condition. It is hoped that the informed included will aid to guide research work in the foreseeable future.
The ideal treatment method for periprosthetic fractures is controversial due to the risks of current methods. Single-cortex screw fixation in prosthesis may lead to implant failure. Therefore, we aimed to develop an implant that lowers the risk for complications. For this study, we designed and tested two new implant models. The first model was a plate with a combination of U nails and cerclage holes. The second model was a U nail plate with a screw, which combines a plate screw with U nail (staples). Our study aimed to compare the stability of two newly designed implants with classical treatment modalities. We used 27 (in 3 groups) artificial bone models and 9 different test models.
The ISO 7206-42010 (E) standards were used for 27 bones in nine groups tested under laboratory conditions. In our study, we examined nine different groups. In group 1, hipthe prosthesis was extracted, and a revision femoral stem was embedded. In group 2, periprosthetic fractures were repaired with a plate and cable. In group udy, so further studies, especially patient-specific studies, should be made to expand the findings of this study.
To demonstrate the magnetic resonance imaging (MRI) features of amputation neuromas in lower extremity amputees and investigate independent predictive MRI features for symptomatic neuromas.
This retrospective study included 45 amputation neuromas in 44 lower extremity amputees. Two radiologists assessed the imaging features, including shape, size, type (end-bulb or spindle), signal intensity (SI), heterogeneity, margins, enlarged fascicles, dark outer rim, tail sign, target sign, enhancement, perilesional fibrosis, and muscle denervation. The neuromas were categorized into symptomatic (n = 24) or asymptomatic (n = 21). Symptomatic neuromas were determined based on neuropathic pain characteristics, the presence of Tinel's sign or tenderness, and response to local anesthetic injection. Univariate and multivariate analyses were performed to identify independent predictive MRI features.
Of 45 neuromas, 80% (36/45) were end-bulb neuromas and 20% (9/45) were spindle-type neuromas. Eighty percent of the neuromb or spindle-type. They can show enlarged fascicles, dark outer rims, and enhancement. • Heterogeneity on T2-weighted images could be a predictive indicator for symptomatic neuromas. • Predicting the symptomatic neuroma on MRI would help in effective management of stump pain.

F]fluoride) PET/CT can detect active arterial calcifications at the molecular stage. We investigated the relationship between arterial mineralization in the left common carotid artery (LCC) assessed by [
F]fluoride PET/CT and cardiovascular/thromboembolic risk.
In total, 128 subjects (mean age 48±14 years, 51% males) were included. [
F]fluoride uptake in the LCC was quantitatively assessed by measuring the blood-pool-corrected maximum standardized uptake value (SUVmax) on each axial slice. Average SUVmax (aSUVmax) was calculated over all slices and correlated with 10-year risk of cardiovascular events estimated by the Framingham model, CHA2DS2-VASc score, and level of physical activity (LPA).
The aSUVmax was significantly higher in patients with increased risk of cardiovascular (one-way ANOVA, p < 0.01) and thromboembolic (one-way ANOVA, p < 0.01) events, and it was significantly lower in patients with greater LPA (one-way ANOVA, p = 0.02). On mization at a molecular level could help guide clinical decisions in the context of cardiovascular risk assessment.

Early acute kidney injury (AKI) after burn contributes to disastrous prognoses for severely burned patients. Burn-induced renal oxidative stress and secondary proinflammatory mediator release contribute to early AKI development, and Toll-like receptor (TLR) 4 regulates inflammation. Heme oxygenase-1 (HO-1) is a stress-responsive enzyme that plays a vital role in protecting against ischemia-induced organ injury via its antioxidant properties and regulation of inflammation. We investigated the potential effect of HO-1 induction in preventing burn-induced early AKI and its related mechanism.

In epidemiological trials and in clinical practices, it is relevant to have affordable and reliable methods to measure the main lipid cardiovascular risk factors, and in particular low-density lipoprotein cholesterol (LDL-C) plasma level. In this context, we aimed to compare the reliability of the Friedewald's (LDL-Cf) and Sampson's (LDL-Cs) equations with the LDL-value dosed by a validated dosage method (LDL-Cd) in a large cohort of children.
We considered the lipid values of 145 infants, 278 preschoolers, 810 scholar children, and 1372 adolescents (Total N. 2605, 1291 males, 1314 females), with mean total cholesterol (TC)=169.8±39.7mg/dL, HDL-Cholesterol=50.8±12.7mg/dL, non HDL-Cholesterol=118.9±35.9mg/dL, Triglycerides (TG)=90.3±77.9mg/dL, LDL-Cd=106.2±29.9mg/dL, LDL-Cf=100.9±33.8mg/dL, and LDL-Cs=102.2±33.4mg/dL. Comparing the distance to the LDL-Cd, Friedewald's equation mildly but significantly underestimated in infants (3.4±5.3mg/dL), preschoolers (1.5±7.1mg/dL). Children (1.2±2.2mg/dL) and adolescents (1.1±5.9mg/dL) compared to Sampson's equation (all comparisons, p<0.001).
Our analysis, being carried out on a large population sample, shows that Sampson's equation is more reliable than Friedewald's one at each considered age class and even for extreme TG values.

Cholesteatoma is a benign tumoral lesion of squamous epithelial cells in middle ear that can exist as congenital or acquired forms.
A 35-year-old housewife presented to ENT clinic of a private hospital in Kabul, Afghanistan, with a complete facial nerve paralysis in the right side. In her antecedents, there is a tympanomastoidectomy due to chronic middle ear infection. First symptom was right side earache without any discharge. She started to notice a progressive nodule in the posterior-inferior side of her right ear. The patient was taken to the operating room. She underwent general anesthesia, an extensive cholesteatoma was removed, and a limited area of the fallopian canal in which facial nerve oedema or redness was evident. Post-operative House Brackmann grade was 1 on day 15 after the surgery.
Cholesteatoma is primarily managed surgically and currently there is no suitable medical substitute treatment strategy for cholesteatoma. Hearing improvement, making the ear dry and total omission of cholesteatoma are primary goals of surgical interventions in cholesteatoma management.

Frozen autograft recycling has been used for biological reconstruction of bone defects following tumor excision, more commonly in extremities. We report on the histological outcome of a pelvic recycled frozen autograft.
We investigated the pelvic frozen autograft removed in 2 years and 8 months after surgery because of soft tissue recurrence in pelvic floor. The autograft bone showed no evidence of revitalization and was non-viable with patchy inflammation, and no residual tumor. There was only fibrous union but the autograft bone remained mechanically stable.
We confirmed the clearance of tumor cells with the treatment with liquid nitrogen. The union at the host-graft junction might be affected by the previous radiotherapy, the presence of infection, the small contact area limited by the anatomy, and the inadequate compression across the osteotomy interface with the fixation.
Frozen autograft treated by liquid nitrogen can be used safely for biological reconstructions after pelvic tumor excision.

Having two or more sites of simultaneous ectopic thyroid tissue is a rare phenomenon. Thyroid ectopia should be considered in congenital hypothyroidism where no eutopic thyroid gland is found.
This case describes an incidental finding of dual ectopic thyroid tissue on computer tomography scan in an adult with known congenital hypothyroidism that was previously attributed to thyroid agenesis. The decision was made to proceed with a Sistrunk procedure to excise the ectopic submental thyroid as it became more noticeable after weight loss following bariatric surgery, and to monitor the remaining lingual thyroid with a combination of clinical symptomology, imaging and thyroid function studies given its challenging location.

Here, we deploy the SunTag method to visualize and quantify the timing, location, and kinetics of the translation of single mRNAs in living Drosophila embryos. By focusing on the translation of the conserved major epithelial-mesenchymal transition-inducing transcription factor Twist, we identify spatial heterogeneity in mRNA translation efficiency and reveal the existence of translation factories, where clustered mRNAs are cotranslated preferentially at basal perinuclear regions. If these actors were to proactively and deliberatively engage in transformative adaptation, they would need to address the material, relational and normative factors that hold current systems in place. Plutella xylostella (L.) is a serious worldwide pest that feeds on cruciferous plants and has evolved resistance to different classes of insecticides used for its control, including chlorantraniliprole. ATP-binding cassette (ABC) transporters, constituting the largest transport family in organisms, are involved in phase III of the detoxification process and may play important roles in insecticide resistance.
A total of 15 ABC transporter transcripts from subfamily G were identified in P.xylostella based on the latest DBM genome. Synergism studies showed that treatment with verapamil, a potent inhibitor of ABC transporters, significantly increased the toxicity of chlorantraniliprole against larvae of two chlorantraniliprole-resistant P.xylostella populations (NIL and BL). ABCG2, ABCG5, ABCG6, ABCG9, ABCG11, ABCG14 and ABCG15 were significantly overexpressed in NIL and BL compared with the susceptible population (SS), and ABCG1, ABCG6, ABCG8, ABCG9, ABCG14 and ABCG15 were significantly upregulated after tretransporters in chlorantraniliprole resistance in P. xylostella or other insect pests. © 2021 Society of Chemical Industry.

This study used pooled Texas Behavior Risk Factor Surveillance System datasets (2014 to 2018) for this assessment. The main outcome variable was categorized into pharmacy and non-pharmacy vaccination locations and analyzed using a logistic regression analysis. Further statistical analysis was done using a multinomial logistic regression after re-categorizing the outcome variable into pharmacy, doctor office, and other locations.

We designed and implemented a cross-sectional, population-based online survey to evaluate the willingness, hesitancy, and coverage of the COVID-19 vaccine among the Chinese population. 8742 valid samples were recruited and classified as the vaccine-priority group (n=3902; 44.6%) and the non-priority group (n=4840; 55.4%).
The proportion of people's trust in the vaccine, delivery system, and government were 69.0%, 78.0% and 81.3%, respectively. 67.1% of the participants were reportedly willing to accept the COVID-19 vaccination, while 9.0% refused it. 834 (35.5%) reported vaccine hesitancy, including acceptors with doubts (48.8%), refusers (39.4%), and delayers (11.8%). The current coverage was 34.4%, far from reaching the requirements of herd immunity. The predicted rate of COVID-19 vaccination was 64.9%, 68.9% and 81.1% based on the rates of vaccine hesitancy, willingness, and refusal, respectively.

We find California Paid Family Leave reduced late vaccinations by up to 5 percentage points or approximately 10% for children born to parents in California after the policy was implemented. Further, the policy had a stronger impact on families below the poverty line. Thus access to paid family leave can improve on-time immunization of infants.

To investigate and report the instruments used to assess function or functionality in adults after a burn injury; to identify the characteristics and evidence on their measurement properties; and to evaluate their clinical utility.
We systematically searched the literature via six electronic databases and via screening reference lists of relevant studies. The review was registered in PROSPERO (CRD42016048065) and reported according to the PRISMA statement. Studies in which function or functionality of upper and/or lower limbs of adults after a burn injury was assessed were included. Exclusion criteria comprised studies in pediatric populations and conference abstracts.

Then gene set enrichment anicantly correlated with MAPK14, which also contributed to the activation of oxidated phosphorylation, interferon-alpha (IFNα) response and PI3K-Akt-mTOR signaling, etc. Moreover, MAPK14-high group have more adverse clinical outcomes (splenectomy, thrombosis, disease aggressiveness) and inferior survival than MAPK14-low group.

Malignant pleural mesothelioma (MPM) is a rare and aggressive tumour. For patients with inoperable disease, few treatment options are available after first line chemotherapy. The combination of ipilimumab and nivolumab has recently shown increased survival compared to standard chemotherapy, but most patients do not respond and improvements are called for. Telomerase is expressed in mesothelioma cells, but only sparsely in normal tissues and is therefore an attractive target for therapeutic vaccination. Vaccination against telomerase is tolerable and has shown to induce immune responses associated with increased survival in other cancer types. There is a well-founded scientific rationale for the combination of a telomerase vaccine and checkpoint inhibition to improve treatment response in MPM patients.

Erectile dysfunction (ED) is a condition that is very common all over the world, concerns about men and causes very important social problems. In our study, we handled the data of 101 participants in total. The average age of the participants was 56.36. We divided the participants after color Doppler US (CDUS) into three groups normal group (n=30), arterial insufficiency (n=51), and venous insufficiency (n=20). SWE measurements were made in both groups in the CC flaccid (f) and rigid (r) positions, and the results were noted. International Index of Erectile Function (IIEF-5) questionnaire and Erection score (ES) were evaluated.
The mean CC f SWE and r SWE measurement values were 20.2 ± 0.8 and 17.15± 0.54kPa, 3.84 ± 0.13 and 2.78 ± 0.11m/s in the arterial insufficiency group; 15.72 ± 0.58 and 12.52 ± 0.33kPa, 2.88 ± 0.06 and 2.09 ± 0.11m/s in the venous insufficiency group; 14.75 ± 0.51 and 13.41 ± 0.36kPa, 2.63 ± 0.1 and 2.34 ± 0.11m/s in the control group, respectively. In boys, testosterone might be responsible for about 65% of the increase in haemoglobin mass during puberty. The aim of this study is to perform quantitative penile corpus cavernosum (CC) stiffness measurements by using SWE for patients with both types of vascular ED. In our study, we handled the data of 101 participants in total. The average age of the participants was 56.36. We divided the participants after color Doppler US (CDUS) into three groups normal group (n=30), arterial insufficiency (n=51), and venous insufficiency (n=20). SWE measurements were made in both groups in the CC flaccid (f) and rigid (r) positions, and the results were noted. International Index of Erectile Function (IIEF-5) questionnaire and Erection score (ES) were evaluated.
