    Although rectal neuroendocrine tumors (NETs) are considered to be rare low-grade malignancies when lymph node metastasis (LNM) is present, their degree of malignancy is comparable to that of colorectal cancer (CRC). However, it remains unclear as to which patients require radical lymph node dissection. The aim of this study was to elucidate the risk factors for LNM and develop a risk-scoring system for LNM to help determine appropriate therapeutic approaches.

    In this study, we examined 103 patients with rectal NETs who underwent local resection (n=55) or radical resection with LN dissection (n=48). We evaluated each pathological feature, including the depth of submucosal invasion (SM depth) and tumor budding grade.

    According to our univariate analyses and previous reports, the significant five risk factors for LNM were weighted with point values 2 points for tumor size≥15mm and muscularis invasion, and 1 point each for SM depth≥2000µm, positive lymphovascular invasion, budding grade 3, and vertical margin. The area under the receiver operating curve for the scoring system was 0.899 (95% CI 0.843-0.955). When a score of 2 was used as the cut-off value, the sensitivity and specificity for the prediction of LNM were 100% and 72.1%, respectively.

  • Medication prior authorizations (PA) required by insurance payers can be time-consuming to complete and may lead to delays in treatment for cancer patients. The primary objective of this study is to assess the impact of Medication Assistance Program (MAP) specialists embedded in adult hematology and oncology clinics on the PA and financial assistance process.

    This was a retrospective chart review study performed at a large academic medical center that examined medication referrals completed by MAP specialists in four hematology and oncology clinics. The primary outcome was the median PA turnaround time, defined as time from initial referral creation to final referral completion. Secondary outcomes assessed median turnaround time for financial assistance programs and total patient savings.

    A total of 176 prior authorization, 92 manufacturer patient assistance program (PAP), and 37 copay assistance referrals were completed. The median turnaround times were 24, 154, and 19 hours for PA, manufacturer PAP, and copay assistance program referrals, respectively. Total cost savings amounted to over $1.8 million for patients approved to receive medications through manufacturer PAPs.

    Adult glioblastoma patients receiving standard radiation therapy and concurrent temozolomide chemotherapy have a median survival of 14.6 months. Based on the pivotal trial data by Stupp etal., temozolomide doses were calculated based on body surface area. However, no details regarding the weight used to calculate body surface area was included in the study. As a result, temozolomide doses have been variable across the province.

    This retrospective chart review was conducted to determine the correlation between dose of first line temozolomide with overall survival. Patients between January 1st, 2009 and December 31st, 2014 who were newly diagnosed, pathology confirmed glioblastoma treated first line with temozolomide within Alberta Health Services were included in the study. Temozolomide doses above and below determined cut points were compared through the Kaplan-Meier method, then assessed using the log-rank test.

    A cut point of 97.8% of actual body weight calculated body surface area dosing was determined for concurrent phase temozolomide. At doses above this cut point, there was a statistically significant (p = 0.0158) increase of 0.3 years in median overall survival. As for toxicity concerns, there was a statistically significant increase in the proportion of temozolomide dose reductions due to toxicity in patients dosed above the cut point.

Eight-week-old male C57BL/6 **** were intraperitoneally administered 30 mg/kg of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) once a day for 5 days, and orally given 100 mg/kg of KRG for 12 consecutive days. Alterations in the levels of occludin, zonula occludens-1 (ZO-1), tumor necrosis factor-alpha (TNF-α) and interleukin-1 beta (IL-1β) in the colon, and the expressions of aSyn and tyrosine hydroxylase (TH) in the colon and the SN were evaluated. Oral administration of KRG significantly prevents the MPTP-induced motor dysfunction, and suppresses the MPTP-induced disruption of occludin and ZO-1, and suppresses the increase in TNF-α and IL-1β in the colon of ****. To improve research-practice translations, investigations need to incorporate synthesised perspective and collective input from intervention stakeholders. By presenting this application example, we aim to underline the considerations and alleviate barriers to conducting **** needed implementation and sustainability studies in FMS interventions. The goal of this article was to develop a scale that measures this cultural value. METHOD A self-report scale measure of simpatía was developed and administered to a combined sample of Latinos (N = 296) drawn from 3 larger studies. The scale's factor structure was explored, and its internal consistency and validity were tested. RESULTS Exploratory factor analysis supported an 18-item scale and indicated 2 factors simpatía-related positivity/warmth and simpatía-related negativity/conflict avoidance. Cronbach's alphas for the overall scale and subscales showed internal consistency. Validity analyses revealed that across subscales, simpatía was positively associated with positive emotion expressivity and dispositional positive emotion. The simpatía-related positivity/warmth subscale was also positively associated with an orientation toward Latino culture. CONCLUSIONS The Simpatía Scale, which captures dual aspects of simpatía that emphasize the positive and avoid the negative, provides a new tool for advancing the study of Latino culture. METHOD Using postmortem data (N = 3,996, mean age = 16.01, 77% male) from the National Violent Death Reporting System, the authors investigated whether White, Black, Native American, Asian Pacific Islander American, and Latinx youths aged 10 to 18 varied in their rates of circumstances that either preceded or appear to have contributed to their suicides. Gender differences and gender by race/ethnicity moderation effects were examined in addition to racial/ethnic differences. These antecedents included, but were not limited to, mental health problems and treatment, history of suicide attempt and suicidal ideation, presence of a suicide note, interpersonal problems, substance abuse problems, school problems, and exposure to the death or suicide of a loved one. RESULTS Logistic regression analyses revealed racial/ethnic and gender differences in the prevalence of several antecedents of youth suicide. One race/ethnicity by gender moderation effect related to suicide notes was identified. CONCLUSIONS The findings suggest considerable gender and racial/ethnic heterogeneity in suicide risk factors and highlight the need for more research on suicidal behavior in minority youths, as well as culturally tailored prevention efforts. The Berkeley Expressivity Questionnaire (BEQ) was designed to assess positive and negative expressivity, as well as the intensity of the expressive behavior. Internal consistency was good, and test-retest reliability was excellent. As expected, emotion expression was larger in women. Convergent and divergent validity were confirmed. The goal of this article was to develop a scale that measures this cultural value. METHOD A self-report scale measure of simpatía was developed and administered to a combined sample of Latinos (N = 296) drawn from 3 larger studies. The scale's factor structure was explored, and its internal consistency and validity were tested. RESULTS Exploratory factor analysis supported an 18-item scale and indicated 2 factors simpatía-related positivity/warmth and simpatía-related negativity/conflict avoidance. Cronbach's alphas for the overall scale and subscales showed internal consistency. Validity analyses revealed that across subscales, simpatía was positively associated with positive emotion expressivity and dispositional positive emotion. The simpatía-related positivity/warmth subscale was also positively associated with an orientation toward Latino culture. METHOD Using postmortem data (N = 3,996, mean age = 16.01, 77% male) from the National Violent Death Reporting System, the authors investigated whether White, Black, Native American, Asian Pacific Islander American, and Latinx youths aged 10 to 18 varied in their rates of circumstances that either preceded or appear to have contributed to their suicides. Gender differences and gender by race/ethnicity moderation effects were examined in addition to racial/ethnic differences. These antecedents included, but were not limited to, mental health problems and treatment, history of suicide attempt and suicidal ideation, presence of a suicide note, interpersonal problems, substance abuse problems, school problems, and exposure to the death or suicide of a loved one. RESULTS Logistic regression analyses revealed racial/ethnic and gender differences in the prevalence of several antecedents of youth suicide. One race/ethnicity by gender moderation effect related to suicide notes was identified.     Here we present a case of amiodarone lung toxicity, with diagnosis suspected based on patient's history of chronic cardiac disease, amiodarone treatment, and imaging characteristics. Evaluation with dual energy CT demonstrated high iodine content in the mass-like consolidation further supporting the diagnosis. Biopsy of the mass-like consolidation confirmed amiodarone toxicity.

    We introduce a new sign on a Merchant view present in acute patellar tendon ruptures (APTR). We aim to determine the accuracy, sensitivity, and specificity of this new radiographic sign and measure the effect of a tutorial on these measures between trainees and non-trainees.

    Lateral and Merchant radiographs (22 images) of knees with four conditions (patellar instability, APTR, quadriceps tendon rupture, and controls) were randomly shown to 50 trainees and non-trainees who were asked to make a diagnosis based on radiographs. A brief tutorial was administered describing the "Empty Merchant Sign" and the same 22 images were randomly shown after the tutorial. Accuracy, sensitivity, specificity, and positive predictive value were calculated between the two image types and the effect of tutorial on these measures was assessed.

    After the tutorial 1. the "Empty Merchant Sign" had a higher specificity (100%) and positive predictive value (99%) compared to the lateral radiograph (81% and 64% respectively, P<0.001), 2. There was significant improvement (from 56% to 95.3%; P<0.001) in the accuracy of the Merchant view, making it as accurate as the lateral view (95.3% vs. 90.7%, respectively; P=0.113). The results showed that knowledge scores were considerably greater in those who participated in related volunteer workshops than those who did not (P = 0.0001). The mean score of attitude toward HM was significantly higher in female subjects than male subjects (P = 0.028). Moreover, data indicated that attendance at the HCCII course and workshops was not associated with improvement in attitude. In adjusted logistic regression analysis, baseline neutrophil to lymphocyte ratio (NLR) (P=0.0035) and body mass index (P=0.03) were significant predictors of the risk for respiratory failure. Age (P=0.05) and non-use (vs. use) of psychotropic medications having serotonin 2A receptor antagonist properties (odds ratio 5.06; 95% confidence intervals 1.18-21.7; P= 0.029) was each a significant predictor of an increased risk of death. There was a significant interaction effect of age and non-use (vs.. use) of psychotropic serotonin 2A receptor antagonist medications on the odds ratio (OR) for death (P=0.011). At the prometaphase of mitosis, spindle assembly was asymmetric; one half of the spindle assembled before the other, with simultaneous chromosome alignment in the spindle mid-zone. At diakinesis in wheat and rye, microtubules formed a pro-spindle which was subsequently disassembled followed by a bipolar spindle assembly. In the first meiosis of hybrids 2R(2D)xR, a bipolar spindle was not found and the kinetochore microtubules distributed the chromosomes. Univalent chromosomes are characterised by a monopolar orientation and maintenance of sister chromatid and centromere cohesion. In adjusted logistic regression analysis, baseline neutrophil to lymphocyte ratio (NLR) (P=0.0035) and body mass index (P=0.03) were significant predictors of the risk for respiratory failure. Age (P=0.05) and non-use (vs. use) of psychotropic medications having serotonin 2A receptor antagonist properties (odds ratio 5.06; 95% confidence intervals 1.18-21.7; P= 0.029) was each a significant predictor of an increased risk of death. There was a significant interaction effect of age and non-use (vs.. use) of psychotropic serotonin 2A receptor antagonist medications on the odds ratio (OR) for death (P=0.011). In selected, ventilator-dependent COVID-19 pneumonia patients treated with psychotropic serotonin 2A receptor antagonist medications to control agitation and ICU delirium, there was an apparent positive association between medication use and significant rise in the absolute lymphocyte count and decrease in the neutrophil lymphocyte ratio. Taken together, these data are the first to suggest that certain psychotropic medications used in the treatment of chronic psychiatric illness and/or for acute delirium are inversely associated with mortality in severe COVID-19 infection by unknown mechanism which may involve (in part) immunomodulatory effects.The assembly of the microtubule-based spindle structure in plant meiosis remains poorly understood compared with our knowledge of mitotic spindle formation. One of the approaches in our understanding of microtubule dynamics is to study spindle assembly in meiosis of amphyhaploids. Using immunostaining with phH3Ser10, CENH3 and α-tubulin-specific antibodies, we studied the chromosome distribution and spindle organisation in meiosis of F1 2R(2D)xR wheat-rye hybrids (genome structure ABDR, 4× = 28), as well as in wheat and rye mitosis and meiosis. At the prometaphase of mitosis, spindle assembly was asymmetric; one half of the spindle assembled before the other, with simultaneous chromosome alignment in the spindle mid-zone. At diakinesis in wheat and rye, microtubules formed a pro-spindle which was subsequently disassembled followed by a bipolar spindle assembly. In the first meiosis of hybrids 2R(2D)xR, a bipolar spindle was not found and the kinetochore microtubules distributed the chromosomes. Univalent chromosomes are characterised by a monopolar orientation and maintenance of sister chromatid and centromere cohesion. Presence of bivalents did not affect the formation of a bipolar spindle. Since the central spindle was absent, phragmoplast originates from "interpolar" microtubules generated by kinetochores. Cell plate development occurred with a delay. However, meiocytes in meiosis II contained apparently normal bipolar spindles. Thus, we can conclude that (1) cohesion maintenance in centromeres and between arms of sister chromatids may negatively affect bipolar spindle formation in the first meiosis; (2) 2R/2D rye/wheat chromosome substitution affects the regulation of the random chromosome distribution in the absence of a bipolar spindle.

    During the outbreak of Coronavirus disease of 2019 (COVID-19), the preparedness of emergency departments (EDs) for triaging of the patients and safety of staff is of utmost importance. The aim of our study was to develop and implement COVID-19 ED triage and protected intubation protocols for COVID-19 patients with in-situ simulation (ISS) training. The latent safety threats (LST) detection also served as a platform to test new system amendments and refine the protocols and workflowswith infection control issues. We also explored the effectiveness of this approach based on Kirkpatrick's model of evaluating training outcomes.

    The protocols and simulation scenarios were developed and validated. A total of 22 triage and 13 intubation simulation sessions were conducted in the ED with multidisciplinary staff (physicians=18, nurses=20) during a period of four months. Each simulation was followed by a debriefing session to discuss the team performance. Pre- and post-simulation performances were compared. LSTs werely. We recommend such an approach to train the multidisciplinary staff and continue to improve ourselves through ISS addressing the changing nature of the pandemic.

Repeated evaluations including chest X-ray, ECG, and focused assessment with sonography for trauma did not reveal any abnormal findings. While waiting for discharge, she started having mild chest discomfort. Unexpectedly, the third echocardiogram showed mild pericardial effusion, and a CT with contrast showed aortic dissection in the ascending aorta and infra-left subclavian aortic dissection. She was immediately transferred by a helicopter to a tertiary trauma center for emergent repair surgery. Considering the site of dissections, progression of existing intrinsic intimal tear triggered by energy impact was suspected. However, traumatic causes could not be excluded.   • Mechanobiology at the cellular level is concerned with what phenotypes that cells exhibit to maintain homeostasis in their normal physiological mechanical environment, as well as what phenotypical changes that cells have to make when their environment is altered. Mechanobiology at the molecular level aims to understand the molecular underpinning of how cells sense, respond to, and adapt to mechanical cues in their environment. In this Perspective, we use our work inspired by and in collaboration with Professor Shu Chien as an example with which we connect the mechanobiology between the cellular and molecular levels. We discuss how physical forces acting on intracellular proteins may impact protein-protein interaction, change protein conformation, crosstalk with biochemical signaling molecules, induce mechanotransduction, and alter the cell structure and function. © Author(s).Background Anxiety disorders are the most common neuropathologies worldwide, but the precise neuronal mechanisms that underlie these disorders remain unknown. The hippocampus plays a role in mediating anxiety-related responses, which can be modeled in rodents using behavioral assays, such as the elevated plus maze. Yet, the molecular markers that underlie affect-related behavior on the elevated plus maze are not well understood. Methods We used herpes simplex virus vector delivery to overexpress extracellular signal-regulated kinase-2, a signaling molecule known to be involved in depression and anxiety, within the dorsal hippocampus of adult Sprague-Dawley male rats. Three days post virus delivery, we assessed anxiety-like responses on the elevated plus maze or general locomotor activity on the open field test. Results When compared to controls, rats overexpressing extracellular signal-regulated kinase-2 in the dorsal hippocampus displayed an anxiolytic-like phenotype, per increases in time spent in the open arms, and less time in the closed arms, of the elevated plus maze. Furthermore, no changes in locomotor activity as a function of virus infusion were observed on the open field test between the experimental groups. Conclusion This investigation demonstrates that virus-mediated increases of extracellular signal-regulated kinase-2 signaling, within the hippocampus, plays a critical role in decreasing anxiogenic responses on the rat elevated plus maze. Here, we report mechanically adaptive, physiologically responsive polymers that can be photopolymerized and thus patterned via soft lithography and photolithography. The adaptive polymer networks produced exhibit, in optimized compositions, a ca. 500-fold decrease of their storage modulus when exposed to simulated physiological conditions, for example, from 2.5 GPa to 5 MPa. This effect is caused by modest swelling (30% w/w), which in turn leads to plasticization so that the polymer network's glass transition temperature is reduced from 145 to 25 °C. The polymer networks can further be rendered pH-responsive by the incorporation of methacrylic acid. The dual stimuli-responsive materials thus made show promise as coatings or substrates for drug delivery devices. The crystal phase of PBABI copolyesters tended to prefer thermodynamics in the presence of a small amount of EDTA. The isothermal crystallization analysis revealed that the PBABI with EDTA exhibited a higher crystallization rate and a shorter half-time of crystallization than neat PBABI copolyesters. Dependence was observed between the breakdown potentials and the inclusion fractions in the ELI stent (inclu.% = 0.2-0.8%), in which the breakdown potentials were found to be inversely proportional to inclusion fractions and densities (E b dropped from ∼800 to ∼400 mV). No breakdown occurred on the samples using high-purity NiTi materials (inclu.% less then 0.1%). The roles of inclusions in pitting mechanisms were investigated using scanning electron microscopy (SEM) characterizations.

  • The method proposed in this work showed good performances, including wide linearity (14.6-250 ng/g) with high correlation coefficients (R ≥ 0.9991), low limits of detection (LODs, 0.7-6.5 ng/g for all SAs), satisfactory precision (Intra-day RSDs ≤ 3.4%, inter-day RSDs ≤ 4.7%), and high accuracy (recovery 83.4-103.8%). Furthermore, benefiting from the mesoporous structure and the Zr cluster center as well as the stability, UiO-67 shows great potential to be an excellent adsorbent of SPE for extraction of SAs from other complex matrices. Two thermo-responsive molecularly imprinted polymers (MHNTs@MIP and MCNTs@MIP) for the selective extraction of sterigmatocystin have been prepared on the surface of the magnetic halloysite nanotubes (MHNTs) and magnetic carbon nanotubes (MCNTs), respectively. 1, 8-dihydroxyanthraquinone, n-isopropyl acrylamide, methacrylic acid, ethylene dimethacrylate and dimethyl sulfoxide were used as the dummy template, thermo-sensitive functional monomer, co-monomer, cross-linker and porogen, respectively. Separation was carried out on a silica gel 60F-254 plate eluted with the (W/O) microemulsion in about 30 min development time. The images of TLC plates were processed using 4 different programs, by comparing their results it was found that "integrated density" measured by Fiji software was the most accurate response that could measure the concentration of lamotrigine in spiked plasma in the range of (20-200) ng/spot. This method was applied also for determination of lamotrigine in lamictal® tablet dosage form using the same mobile phase. The precision of the method was satisfactory; the maximum value of relative standard deviations did not exceed 1.5%. V.Lateral wedge insoles (LWI) have been proposed to reduce the knee adduction moment (KAM) during walking; a biomechanical modification notably sought in case of medial knee osteoarthritis. However, the inter-individual inconsistency in KAM changes with LWI limits their therapeutic use. Although the foot progression angle (FPA) has been frequently discussed in KAM modifications literature, there is a lack of data regarding a possible relationship between this gait measure and changes in KAM with LWI. This study aimed to test if KAM changes with LWI differ with respect to the natural FPA and to compare KAM-related variables between individuals walking with smaller and larger natural FPA. Twenty-two healthy participants (14 males, 24 ± 3 years, 22.7 ± 2.7 kg/m2) underwent gait analysis with and without LWI. They were divided into two groups based on their natural FPA, and changes in KAM 1st peak, KAM impulse, and KAM-related variables were compared between groups. KAM 1st peak and impulse decreased with LWI in the smaller natural FPA group (p ≤ 0.006), while only KAM impulse decreased in the larger natural FPA group (p  less then  0.001). The difference in KAM 1st peak changes was explained by a less reduced lever arm in participants walking with larger natural FPA. In conclusion, this study brought new insight into the variability in KAM response to LWI. If the findings are confirmed in patients with medial knee osteoarthritis, the FPA could become a simple measure to help identify the patients more likely to reduce their KAM with LWI. The tibiofemoral compressive forces experienced during functional activities are believed to be important for maintaining tibiofemoral stability. Previous studies have shown that both knee-spanning and non-knee-spanning muscles contribute to tibiofemoral joint compressive forces during walking. However, healthy individuals typically engage in more vigorous activities (e.g. jumping and cutting) that provide greater challenges to tibiofemoral stability. Despite this, no previous studies have investigated how both knee-spanning and non-knee-spanning muscles contribute to tibiofemoral compressive loading during such tasks. The present study investigated how muscles contributed to the medial and lateral compartment tibiofemoral compressive forces during sidestep cutting. Three-dimensional marker trajectories, ground reaction forces and muscle electromyographic signals were collected from eight healthy males whilst they completed unanticipated sidestep cutting. OpenSim was used to perform musculoskeletal simulations to compute the contribution of each lower-limb muscle to compressive loading of each compartment of the knee.
    The method proposed in this work showed good performances, including wide linearity (14.6-250 ng/g) with high correlation coefficients (R ≥ 0.9991), low limits of detection (LODs, 0.7-6.5 ng/g for all SAs), satisfactory precision (Intra-day RSDs ≤ 3.4%, inter-day RSDs ≤ 4.7%), and high accuracy (recovery 83.4-103.8%). However, pouchitis may develop in up to 70% of patients after this procedure and significantly impair quality of life, more so if the inflammation becomes a chronic condition. About 10% of patients with IBD who develop pouchitis require pouch excision, and several risk factors of the failure have been reported. A phenotype that has features similar to Crohn's disease may develop in a subset of ulcerative colitis patients following proctocolectomy with IPAA and is the most frequent reason for pouch failure. In this review, we discuss the diagnosis and prognosis of pouchitis, risk factors for pouchitis development, and treatment options for pouchitis, including the newer biological agents.Focused ultrasound (FUS) has been utilized for the treatment of localized prostate cancer. Initially, FUS was performed as a whole-gland treatment comparable to radical prostatectomy or radiation therapy. However, after overall downward stage migration due to health screening programs involving prostate-specific antigen testing, as well as advances in conservative or observative strategies such as active surveillance, FUS has evolved from a whole-gland treatment to a focal treatment. This new treatment technique aims to ablate tumors while preserving the normal prostate tissue, thereby ensuring better preservation of urinary and erectile function. In this article, we review the mechanism and clinical outcomes of the FUS procedure.
    Isoniazid (INH) is metabolized by polymorphic N-acetyltransferase 2 (NAT2) enzyme, which noticeably alters INH plasma concentration. We aimed to determine the distribution of NAT2 genotype in Thai tuberculosis (TB) patients and correlate their genotype with plasma INH concentrations.

    Blood samples from 55 newly diagnosed pulmonary tuberculosis participants from three hospitals were collected to classify the subject by NAT2 genotype performed by the Multiplex haplotype-specific polymerase chain reaction method. Patients were grouped into three acetylators (fast, intermediate, and slow). On day 14 of tuberculosis treatment, the second blood sample was taken to estimate the peak plasma concentration at 2 hours after oral administration. INH plasma concentration was analyzed by liquid chromatography‒tandem mass spectrometry/mass spectrometry method.

In this work, utilizing the Mimaki UV inks jetting system with a variable layer thickness, a new framework to print a photogrammetry-based oil painting 3D model has been proposed with the tunable coloring layer sequence to improve the jagged challenge between adjacent layers. Based on contour tracking, a height-rendering image of the oil painting model is generated, which is further segmented and pasted to the corresponding slicing layers to control the overall printing sequence of coloring layers and white layers. The final results show that photogrammetric models of oil paintings can be printed vividly by UV-curable color polymers, and that the proposed reverse-sequence printing method can significantly improve the staircase effect based on visual assessment and color difference. Finally, the case of polymer-based oil painting 3D printing provides new insights for optimizing color 3D printing processes based on other substrates and print accuracy to improve the corresponding staircase effect.Vietnam is experiencing a diet and nutrition transition. We empirically disentangle these relationships for urban Vietnamese households using novel intra-household data covering 4047 adults and 737 adolescents from 1590 households in Hanoi and Ho Chi Minh City. The data are from comprehensive household surveys and 24-hour food diaries. Simultaneous equation regression models are estimated using three-stage least squares (3SLS) to account for possible endogeneity. Nutrition label use is significantly associated with a lower share of calories from foods and beverages with added sugars.
