To evaluate the effect of adductor canal block (ACB) on short-term postoperative outcomes in patients who underwent medial open-wedge high tibial osteotomy (MOWHTO) compared to that of a placebo.

35 patients who underwent unilateral MOWHTO between 2017 and 2019 were prospectively reviewed and randomly divided into two groups 19 patients who received a single-shot ACB and 16 patients who received a saline injection (a placebo group). Primary outcomes were (1) pain measured using the visual analog scale and range of motion, (2) patient satisfaction, (3) postoperative need for additional opioids, (3) quadriceps strength (the time to straight leg raising [SLR]), (4) clinical outcomes, and (5) complications.

The pain score was lower in the ACB group than in the placebo group in the first 12h (p=0.04). ACB did not exhibit significantly less quadriceps strength weakness postoperatively. There was no statistical difference in the time to SLR (23.5±17.7h in ACB vs. 27.6±11.4 in placebo, p=0.520). The opioid consumption rate within postoperative 12h was significantly decreased after ACB (16.7% in ACB, 70% in placebo, p=0.017). The proportion of patients with more than 5 opioid injections within 72h postoperatively was lower in the ACB group (8.3% in ACB, 50% in placebo, p=0.043). Both groups did not show any localized and systemic complications.

ACB following MOWHTO exhibited better outcomes than a placebo with respect to opioid consumption with no changes in the quadriceps strength and complications.

There has not been an investigation to determine whether leg dominance affects the recovery of quadriceps and hamstring strength, muscle reaction time (acceleration time, AT), and postural stability after anterior cruciate ligament (ACL) reconstruction in recreational-level athletic patients.

A total of 100 patients with isolated ACL injuries (58 patients had dominant leg injuries; 42 patients had non-dominant leg injuries) participated. All patients received an anatomical single-bundle ACL reconstruction using an auto-hamstring tendon graft without preoperative rehabilitation. Leg dominance was defined as the kicking leg. The quadriceps and hamstring strength, AT, and postural stability (overall stability index (OSI)) of both legs were assessed at three different time points (preoperative, 6months, 12months), using an isokinetic dynamometer and postural stabilometry system.

All patients in both groups showed gradual improvement in quadriceps and hamstring muscle strength in the operated legs up to 1yeathe quadriceps and hamstring muscles was not different after 12 months between the operated dominant and non-dominant legs. Clinicians and physical therapists should consider these results during early rehabilitation and identify effective protocols to enhance quadriceps strength, especially in patients with non-dominant leg injuries.
Motion axial system may affect contact stress of hinge knee prosthesis. However, it is unclear which axial system provides the better biomechanical effect. Therefore, the aim of this study was to compare the contact stress and stress distribution on the tibial insert and the bushing of hinge knee prostheses with a biaxial (BA) system and a spherical center axial (SA) system during a gait cycle.

Three-dimensional finite-element (FE) models of the prostheses with different motion systems were included. The comparisons between experimental tests and FE analyses were performed to verify the models. Dynamic implicit FE analyses were performed to investigate the peak contact stresses and stress distributions on the tibial insert and the bushing.

The peak contact stresses on the tibial insert and the bushing of the BA prosthesis were higher than those of the SA prosthesis during most gait cycles. The contact time on the bushing is short in the SA prosthesis. The stress distributions on the superior surface of hould be able to achieve multiaxial motion and complete weight bearing by the tibial condylar to transmit the axial force better.A glassy carbon electrode modified with multi-walled carbon nanotubes (MWCNTs) decorated with gold nanoparticles has been investigated for the first time as an ultrasensitive electrochemical sensor for the determination of doxorubicin hydrochloride (DOX), an efficient antitumor agent. The developed nanocomposite has been characterized by scanning electron microscopy (SEM), besides cyclic and linear sweep voltammetry electrochemical techniques. An efficient catalytic activity for the reduction of DOX has been demonstrated, leading to a significant increase in peak current density and a remarkable decrease in reduction over-potential. Under the optimal condition, a wide linear DOX concentration range from 1×10-11 to 1×10-6 M with a very low detection limit of 6.5 pM was achieved with the modified electrode. Meanwhile, the functionalized MWCNTs/gold nanoparticles indicated an appropriate selectivity, reproducibility, and repeatability as well as long-term stability. The promising outcomes of this research approved the applicability of the developed nanocomposite sensor towards trace amounts of DOX in pharmaceutical and clinical preparations.
