Disease burden prior to hematopoietic cell transplantation (HCT) is difficult to assess in myelodysplastic syndrome (MDS), particularly in patients without excess blasts. We assessed whether morphologic dysplasia at the time of transplant can be a metric of disease burden that is associated with post-transplant outcomes in MDS patients. We identified 84 MDS patients undergoing allogeneic HCT at our institution between 2010 and 2017 who received a bone marrow evaluation immediately prior to HCT. Dysplasia was independently determined by two hematopathologists blinded to existing pathology reports. Erythroid nuclear dysplasia, but not megakaryocytic or myeloid, was associated with post-HCT outcomes. Presence compared to absence of erythroid nuclear dysplasia was associated with lower 2-year progression-free survival (PFS; 34 % vs 62 %, p = 0.0495) and 2-year overall survival (OS; 34 % vs 62 %, p = 0.042). In a multivariate analysis including age, IPSS-R at the time of transplant, pre-HCT therapy, and donor type as covariates, erythroid nuclear dysplasia remained associated with lower PFS (HR 2.6, p = 0.036) and OS (HR 2.7, p = 0.028). Dysplasia assessment prior to transplant may serve as an estimate of disease burden in MDS and identify high-risk patients who merit additional therapies pre- or post-transplant.Viral infections in the central nervous system (CNS) can lead to severe disease manifestations often mediated by a combination of viral cytopathic effects and immunopathology. Recent data from genetic studies of patients with viral infections in the CNS as well as experimental cell and animal models have provided evidence of non-redundant roles for constitutive and latent immune mechanisms, which mediate a first line of antiviral control without significantly triggering inflammatory activities. Collectively, accumulating data suggest the existence of a layer of immune mechanisms in the CNS exerting immediate control of infection, hence buffering the need for activation of more potent immune reactions with inherent potential to induce immunopathology and disease.The relationship between externally measured and internal spine axial twist motion (rotation about a vertical axis) is not well understood. Ultrasound is a validated technique for measurement of vertebral axial twist motion and has the potential for measuring segmental vertebral axial twist in vivo. The objective of this study was to evaluate lumbar segmental axial twist in relation to external thoracopelvic twist from optical motion capture using an ultrasound imaging technique. Sixteen participants were tested in a custom-built axial twist jig, which isolated motion to the lumbar spine. Participants were moved from neutral to 75% of maximum axial twist range of motion in an upright kneeling posture. Thoracopelvic motion was recorded with a motion capture system and L1 to S1 vertebral axial twist was recorded using ultrasound. From motion capture, maximum thoracopelvic axial twist motion was 41.1 degrees. From ultrasound, the majority of axial twist motion occurred at the L2-L3 (46.8% of lumbar axial twist motion) and L5-S1 (33.5%) intervertebral joints. Linear regression linking axial twist at each vertebral level to thoracopelvic axial twist ranged from 0.43 to 0.79. These findings demonstrate a mathematical relationship between internal and external axial twist motion and the distribution of motion across the lumbar spine suggests that classic use of L4-L5 to represent lumbar spine motion may not be appropriate for axial twist modelling approaches.
Histiocytic sarcoma (HS) is a rare, aggressive malignant neoplasm of hematopoietic cell origin. Primary HS of the proximal humerus, without involvement of lymph nodes or bone marrow, or systemic features, is very rare.

We report a rare case of primary bony HS of the proximal humerus without bone marrow involvement in a healthy 33-year-old pregnant woman. She was successfully treated with surgical resection during pregnancy and radiotherapy post-delivery.

This is the first report of a patient with primary bony HS during pregnancy. This highlights the fact that although HS is a neoplasm of hemolymphoid cell lineage, it frequently arises in non-lymphoid organs.
