Endogenous antibodies (EA) include heterophile, human anti-animal, autoimmune and other nonspecific antibodies, and rheumatoid factors, that have structural similarities and can cross-react with the antibodies employed by the immunoassay, causing hyperprolactinemia misdiagnosis. The patient's job (butcher), led us to suspect the presence of anti-animal antibodies. Clinicians should also carefully investigate the use of supplements. Biotin can falsely increase hormone concentration in competitive assays. Many clinicians are still not informed about these pitfalls that are not mentioned in some recent reviews on PRL measurement.
Over the years, the silastic ring vertical gastroplasty (SRVG) has shown poor long-term outcomes with both weight regain and complications. Therefore, most bariatric surgeons have been presented with the need to perform a successful and safe conversion procedure. Yet the preferred and recommended conversion surgery regarding weight loss, comorbidity improvement, and postoperative complications remains under debate.

The aim of this study is to compare the outcomes of conversion from SRVG with either Roux-en-Y gastric bypass (RYGBP) or one anastomosis gastric bypass (OAGB).

A retrospective study was conducted from our bariatric surgery units' database. We reviewed the files of patients who underwent either a RYGBP or OAGB after a previous SRVG. Demographics, obesity-related comorbidities, BMI before and after the procedure, postoperative complications, and length of hospital stay were analyzed.

Between May 2008 and August 2018, fifty-four patients underwent conversion from a failed SRVG. Twenty-one patients underwent conversion to OAGB (39%), and thirty-three patients underwent conversion to RYGBP (61%). Major complications were reported in 9.5% of the OAGB group and 15.1% of the RYGBP group. At a mean follow-up of 28months, the OAGB group achieved a 78.5% excess BMI loss compared with 57.6% in the RYGBP group (p = 0.137). One patient (4.7%) of the OGBP group and 5 (15.1%) of the RYGBP group needed reoperations due to complications (p = 0.224).

The OGBP is gaining popularity and evidence as an effective and safe procedure. Here we show the successful utilization of the OGBP, when compared with RYGBP, as a revisional procedure after SRVG.

Since the advent of MRI neuroimaging, significant progress has been made in imaging of global white matter connectivity (connectomics), followed by promising new studies regarding imaging of grey matter laminar compartments. Despite progress in both fields, there still lacks mesoscale information regarding cortical laminar connectivity that could potentially bridge the gap between the current resolution of connectomics and the relatively higher resolution of cortical laminar imaging. Here, we systematically review a sample of prominent published articles regarding cortical laminar connectivity, in order to offer a simplified data-driven model that integrates white and grey matter MRI datasets into a novel way of exploring whole-brain tissue-level connectivity. Although it has been widely accepted that the cortex is exceptionally organized and interconnected, studies on the subject display a variety of approaches towards its structural building blocks. Our model addresses three principal cortical building blocks cortical layer definitions (laminar grouping), vertical connections (intraregional, within the cortical microcircuit and subcortex) and horizontal connections (interregional, including connections within and between the hemispheres). While cortical partitioning into layers is more widely accepted as common knowledge, certain aspects of others such as cortical columns or microcircuits are still being debated. This study offers a broad and simplified view of histological and microscopical knowledge in laminar research that is applicable to the limitations of MRI methodologies, primarily regarding specificity and resolution.Brain activation and connectivity analyses in task-based functional magnetic resonance imaging (fMRI) experiments with multiple subjects are currently at the forefront of data-driven neuroscience. In such experiments, interest often lies in understanding activation of brain voxels due to external stimuli and strong association or connectivity between the measurements on a set of pre-specified groups of brain voxels, also known as regions of interest (ROI). This article proposes a joint Bayesian additive mixed modeling framework that simultaneously assesses brain activation and connectivity patterns from multiple subjects. In particular, fMRI measurements from each individual obtained in the form of a multi-dimensional array/tensor at each time are regressed on functions of the stimuli. We impose a low-rank parallel factorization decomposition on the tensor regression coefficients corresponding to the stimuli to achieve parsimony. Multiway stick-breaking shrinkage priors are employed to infer activation patterns and associated uncertainties in each voxel.
