Here, we describe the differential properties of type I and type III IFNs and present a model of IFN response during SARS-COV-2 infection, while highlighting the superior potential of type III IFNs in COVID-19.The study investigated the health profile of individuals exposed to particulate matter (Black) soot in Choba and Mgbuoba localities of Port Harcourt. The analysis was executed with the aid of 100 respondents (50 from Choba and 50 from Mgbuoba). A structured questionnaire with the outlined elements was utilized during data collection for the study Manner of living, Type of occupation, Environmental exposure, Known health issues, Fitness and Cleanliness and Key Informant Interviews (KII) were exploited to gain pertinent data from health professionals. A hand held Kestrel weather tracker was employed to ascertain the wind speed, temperature and relative humidity of the study areas while A met One Instrument, Inc. Aerosol Mass Monitor Model GT - 531 was utilized to estimate total suspended particulates. Biochemical parameters such as lipid profile, renal markers and liver markers activities were analyzed from blood samples obtained from the subjects using spectrophotometric methods. The air quality profile of Mgbe clearly shows that these air pollutants are higher in concentrations early in the morning and their concentrations decreased as the day went by. And so by implication, the residents in these areas may be at high risk of exposure to these air pollutants from 6am-8am. The present study has demonstrated that these air pollutants are more prominent very early in the mornings and variations in meteorological conditions at a particular time, and anthropogenic activities might have contributed to the elevations in the concentration of TSPM, PM10 and PM2.5 in Choba and Mgbuoba.Background Teledermatology may increase access to care but has not been widely implemented due, in part, to lack of insurance coverage and reimbursement. We assessed the impact of implementing a consultative store-and-forward teledermatology model on access to care, medical cost, and utilization. Materials and Methods Prospective implementation of teledermatology occurred at five University of Pennsylvania Health System primary care practices from June 27, 2016, to May 25, 2017. Primary outcomes included time to case completion, proportion of patients completing in-person dermatology visits, and total outpatient costs. Medical and pharmacy claims data were used for utilization and cost subanalysis. Results The study included 167 patients and 1,962 controls with a 6-month follow-up. Median time to definitive dermatologist response was 0.19 days (interquartile range [IQR] 0.03-2.92) for intervention and 83.60 days (IQR 19.74-159.73) for controls. In medical claims subanalysis, no significant differences in mean outpatient costs ($3,366 vs. $2,232, p = 0.1356) or total medical costs ($3,535 vs. $2,654, p = 0.2899) were detected. Conclusions Implementation of teledermatology improved access to care, and within this small sample, remained comparable in terms of cost and utilization. Thus, these data suggest teledermatology may improve access without increasing utilization or cost.
Hip fractures are an important cause of morbidity and mortality. Early surgery has been shown to reduce mortality rates and surgical complications. The American Society of Anesthesiologists (ASA) grade is a widely used tool to assess preoperative health of patients. This study aims to assess is whether delay in surgical time has a greater impact on the mortality rates for high risk patients.

Retrospective study using the National Hip Fracture Database (NHFD) of 4883 neck of femur fracture patients. Time of surgery, ASA grade, reason for delay and mortality at 120 days was analysed, using statistical analysis software.

There was a significant increase in mortality (
 < 0.001) with increasing ASA grade. Surgical delays of more than 36 hours increased mortality by 2.9%. The impact of delaying surgery became more pronounced as the ASA grade increased. ASA 3 and above had an optimum time to surgery of between 12 and 24 hours giving the statistically significant lowest mortality rate (
 = 0.004).

Surgical delay beyond the 36-hour target for surgery has a greater impact on mortality for patients with higher ASA grades. The effect is most profound in the high-risk ASA grade 5 patients with delayed patients showing a 37.5% increase in mortality in this group. This would imply that by prioritising this higher risk group and operating on it within a specific time frame there would be a subsequent fall in mortality associated with neck of femur fractures.

Therefore, we aimed to compare the feasibility and safety of DRA and CRA in a real life population. We conducted a prospective, observational multicentric trial, including all patients undergoing coronary procedures in September 2019. Patients with impalpable proximal or distal radial pulse were excluded. Thus, the choice of the approach is left to the operator discretion. The primary endpoints were cannulation failure and procedure failure. The secondary endpoints were time of puncture, local complications and radial occlusion assessed by Doppler performed one day after the procedure. We enrolled 177 patients divided into two groups CRA (n = 95) and DRA (n = 82). Percutaneous intervention was achieved in 37% in CRA group and 34% in DRA group (p = 0.7). Cannulation time was not significantly different between the two sets (p = 0.16).
