We present a case of tabes dorsalis in a 56-year-old female with a history of extensive autoimmune disease who initially presented with neck pain and numbness of the right lower extremity. The unique nature of this case lies in the patient's clinical course, as her symptoms were initially attributed to her history of autoimmune disease. A reactive CSF-VDRL (cerebrospinal fluid Venereal Disease Research Laboratory) test and MRI findings led clinicians to suspect neurosyphilis and begin penicillin G. The patient began to show significant clinical improvement after penicillin G therapy was begun and was discharged to a rehabilitation facility to continue antibiotics and begin aggressive physical therapy.Inflammatory bowel disease (IBD) is a chronic condition of the bowel that can be further categorized into ulcerative colitis and Crohn's disease. Rarely, this condition can be associated with pericarditis, which can be an extraintestinal manifestation of the disease or drug-induced. The majority of studies suggest that pericarditis arises as a complication of drug therapy by 5-aminosalicylic acid derivatives such as sulfasalazine, mesalamine, and balsalazide, and it occurs due to IgE-mediated allergic reactions, direct cardiac toxicity, cell-mediated hypersensitivity reactions, and humoral antibody response to therapy. Drug cessation or the initiation of a corticosteroid regimen seems to be the most effective means of managing pericarditis in IBD due to drug therapy or an extraintestinal manifestation.Raynaud's phenomenon (RP) is a frequent syndrome and often indicative of connectivitis or hemopathy. The association with solid cancers is exceptional. We report the observation of a patient hospitalized for severe RP whose etiological assessment revealed the existence of colorectal cancer. Herein, we present the case of a 37-year-old female who presented with hemoptysis. A computed tomography scan of her thorax revealed a right-sided round lesion in the interlobar fissure with no additional findings. Surgical excision was performed, which demonstrated an uncommon and unique finding of a pulmonary hamartoma coexisting with a sclerosing pneumocytoma found on exploration. This highlights the importance of exploration during surgical procedures in order identify missed lesions on imaging.Bladder cancer is the seventh most common cancer in the world. Urothelial carcinoma is the most common type of bladder cancer. Other subtypes like squamous cell carcinoma and adenocarcinoma are less common. Urothelial carcinoma has a propensity for divergent differentiation. Sarcomatoid carcinoma is one of the variants of urothelial carcinoma. It is an aggressive tumor that presents at an advanced stage and has a poorer prognosis than conventional urothelial carcinoma. Therefore, identifying this variant histology is important clinically. The sarcomatoid component in sarcomatoid carcinoma can be spindle cell (not otherwise specified), myxoid, pseudoangiosarcomatous, and undifferentiated sarcoma like. Myxoid stroma in sarcomatoid urothelial carcinoma has been described but reported very rarely. We present a case of sarcomatoid urothelial carcinoma with myxoid stroma along with a review of the diagnostic approach to myxoid spindle cell lesions of the urinary bladder.
