Every third 55-year-old will develop AF. Despite improvements of disease management, a significant risk for cardiovascular events remains. The current AF guidelines of the European Society of Cardiology focus on an integrative therapy approach. The new algorithm "CC to ABC" comprises recommendations for diagnosis ("confirm" and "characterise") and treatment ("avoid stroke", "better symptom control", "comorbidities") of AF. Direct oral anticoagulants administered according to the CHA2DS2-VASc score remain the corner stones of stroke prevention. Besides the concept of heart rate control, rhythm control therapy like antiarrhythmic drugs or catheter ablation is recommended to relieve symptoms and in certain patient groups for the improvement of prognosis. Therapy of comorbidities and reduction of risk factors like hypertension, diabetes mellitus, obesity and obstructive sleep apnoea should be part of any comprehensive treatment approach. The results of the randomized, prospective EAST-AFNET 4 trial were published in August 2020. The trial shows that an early rhythm control therapy can lead to a reduction of cardiovascular mortality and incidence of stroke additionally to guideline-based AF management. Given the safety profile and potential positive effects of antiarrhythmic drugs and catheter ablation, early initiation of rhythm control therapy should be considered in every patient during the first months after diagnosis of AF.Dysphagia and symptoms of depression, anxiety and stress are common in persons with multiple sclerosis (MS). We posited a relationship between dysphagia and increased frequency of psychological symptoms. Therefore, the aim of the present study was to examine associations between symptoms of psychological difficulties, use of emotional suppression and cognitive reappraisal strategies, and dysphagia status in persons with MS. One hundred persons with MS were prospectively assessed in multiple domains of functioning cognitive, psychological, and dysphagia-related. Participants underwent cognitive screening with the Mini Mental State Examination and completed two psychological inventories The depression, anxiety and stress scale (DASS)-21 and the Emotion Regulation Questionnaire. Further, they completed the dysphagia in Multiple Sclerosis questionnaire. A speech-language pathologist evaluated persons suspected of having dysphagia with the Mann Assessment of Swallowing Ability. Dysphagia was present in 29 persons with MS, and the sample was split accordingly. The two groups differed at baseline with respect to Expanded Disability Status Scale scores. There were significant between-group differences for mental health symptoms and for use of emotional regulation strategies. Accordingly, multivariate logistic regressions showed that increased symptoms of psychological stress, decreased use of cognitive reappraisal strategies, and increased indicators of emotional suppression independently predicted the presence of dysphagia. There was a clear pattern towards poorer psychological well-being in persons with dysphagia compared to those without. Psychological difficulties may contribute to the manifestation or worsening of dysphagia and should be addressed in treatment planning and future investigations. Therapeutic interventions that promote improvement in mental state alongside swallowing function may be highly beneficial.The successful outcome of treatment for infant and childhood cataract depends on many factors. It is crucial that the treatment falls into a phase in which neither the eye nor the visual pathway and visual cortex are fully developed. This review summarizes the current state of knowledge and provides an overview of the epidemiology, causes and clinical forms, early detection and, above all, treatment options. Special attention is paid to time-critical stages of development, according to which the therapeutic concepts are based. Complications, such as amblyopia and glaucoma are discussed in detail. In addition to surgical aspects, **** emphasis is placed on orthoptic-refractive aftercare, the quality and execution of which is the essential predictor of a good functional outcome.
Despite current progress in research of congenital diaphragmatic hernia, its management remains challenging, requiring an interdisciplinary team for optimal treatment.
Aim of the present study was to evaluate potential risk factors for mortality of infants with congenital diaphragmatic hernia.
Asingle-center chart review of all patients treated with congenital diaphragmatic hernia over aperiod of 16years, at the Medical University of Vienna, was performed. Acomparison of medical parameters between survivors and non-survivors, as well as to published literature was conducted.
During the observational period 66patients were diagnosed with congenital diaphragmatic hernia. Overall survival was 84.6%. Left-sided hernia occurred in 51patients (78.5%) with amortality of 7.8%. In comparison, right-sided hernia occurred less frequently (n = 12) but showed a higher mortality (33.3%, p = 0.000). Critically instable patients were provided with venoarterial extracorporeal membrane oxygenation (ECMO, 32.3%, n = 21).
