The frequency of penicillin allergy was 4.4% (95% CrI, 0%-23.0%; I2 = 75%) for the 8 studies that exclusively assessed surgical patients allergic to cefazolin.

These findings suggest that most patients with a penicillin allergy history may safely receive cefazolin. The exception is patients with confirmed penicillin allergy in whom additional care is warranted.

After weaning from cardiopulmonary bypass, protamine administration as well as standard substitution of plasmatic and cellular coagulation adjusted to thrombelastometry, excessive oozing persisted leading to a complicated postoperative course in all 3 patients.
Is there a relationship between maternal occupational exposure to endocrine-disrupting chemicals (EDCs) during pregnancy and the semen quality of their sons?

Our results suggest an association between maternal occupational exposure to potential EDCs, especially to pesticides, phthalates and heavy metals, and a decrease in several semen parameters.

Sexual differentiation, development and proper functioning of the reproductive system are largely dependent on steroid hormones. Although there is some animal evidence, studies on maternal exposure to EDCs during pregnancy and its effect on the semen quality of sons are scarce and none have focused on maternal occupational exposure.

A cross-sectional study aiming to evaluate semen quality was carried out among Swiss conscripts aged 18 to 22 years between 2005 and 2017.

Conscript and parent questionnaires were completed prior to the collection of a semen sample. Semen parameters were categorised according to the guidelines of the World Health Organization (onal exposure to heavy metals was additionally associated with a low sperm concentration (OR 1.89, 95% CI 1.06-3.37).

Several limitations should be noted, such as the indirect method for maternal occupational exposure assessment during the pregnancy (JEM) and the cross-sectional design of the study.

Heart failure with preserved ejection fraction (HFpEF) is a joint metabolic and cardiovascular disorder with significant noncardiac contributions.

To define and quantify the metabolic cost of initiating exercise in individuals with and without HFpEF and its functional consequences.

This prospective cohort study included individuals with hemodynamically confirmed HFpEF from the Massachusetts General Hospital Exercise Study (MGH-ExS) and community-dwelling participants from the Framingham Heart Study (FHS). Analysis began April 2016 and ended November 2020.

Internal work (IW), a measure of work equivalents required to initiate movement.

Using breath-by-breath oxygen uptake (V̇o2) measurements and V̇o2-work rate associations, cost of initiating exercise (IW) in patients with HFpEF (MGH-ExS) and in community-dwelling individuals (FHS) was quantified. Linear regression was used to estimate associations between IW and clinical/hemodynamic measures.

Of 3231 patients, 184 (5.7%) had HFpEF and were from MG evaluating functional intolerance in individuals at risk for or with HFpEF.When storing multiple objects in visual working memory, observers sometimes misattribute perceived features to incorrect locations or objects. These misattributions are called binding errors (or swaps) and have been previously demonstrated mostly in simple objects whose features are easy to encode independently and arbitrarily chosen, like colors and orientations. Here, we tested whether similar swaps can occur with real-world objects, where the connection between features is meaningful rather than arbitrary. In Experiments 1 and 2, observers were simultaneously shown four items from two object categories. Within a category, the two exemplars could be presented in either the same or different states (e.g., open/closed; full/empty). After a delay, both exemplars from one of the categories were probed, and participants had to recognize which exemplar went with which state. We found good memory for state information and exemplar information on their own, but a significant memory decrement for exemplar-state combinations, suggesting that binding was difficult for observers and swap errors occurred even for meaningful real-world objects.
