Background Data regarding the successful ablation site of idiopathic outflow tract (OT) ventricular arrhythmias (VAs) in the modern era of mapping and ablation are limited. Methods and results Over a 4-year period, a total of 309 patients underwent detailed activation mapping of OT VAs including the right ventricular outflow tract (RVOT), the left ventricular outflow tract (LVOT) and the aortic cusps (AC), and the coronary venous system. 244 cases were successfully ablated at the index procedure (78.9%). The successful ablation site was more frequently located at the LVOT/ACs (51.6%) followed by RVOT (36.2%). In particular, the ACs was the predominant successful ablation site of idiopathic OT VAs (46.7%). An epicardial site of origin was predictor of ablation failure (p less then 0.05). Conclusions The ACs is the predominant successful ablation site of idiopathic OT VAs. The traditional method of primary mouse cardiomyocyte isolation was used as the control group to assess the extraction efficiency of cardiomyocytes by the two-step method, and the purity and viability of cardiomyocytes were evaluated by immunofluorescence staining and autonomous beating analysis, respectively. Results Compared with the control method, the two-step method enabled acquisition of more cells from mouse hearts (1.28 ± 0.11 × 106vs 0.59 ± 0.15 × 106 cells/heart), and the resulting cells exhibited higher adherence rates and cell purity (93.25 ± 1.69% vs 73.62 ± 9.76%) after 48 h of culture. Moreover, the viability of cardiomyocytes was also evidently higher in the two-step group than in the control group (124.67 ± 10.50 vs 88.50 ± 6.61 beats/min). Discussion Compared with the traditional method, the two-step method exhibited significantly better efficiency in extraction of primary cardiomyocytes and yielded cells with greater purity and viability. The two-step method will likely become a standard method for studies based on primary mouse cardiomyocytes in the future.Objective Evaluate dynamic balance and postural stability in patients with adult spinal deformity (ASD) compared with published age-matched normative data. Methods Eleven patients with ASD were prospectively enrolled. Postural stability was tested using static and dynamic posturography; patients stood on a movable platform with integrated force plate and performed standardized sensory organization testing (SOT), evaluating the influence of sensory processing on postural stability under 6 conditions, and motor control testing, assessing reflexive postural reactions to an external perturbation. Patient performance was compared with that of published age-matched controls. Quality of life metrics included scores on the SRS-22 questionnaire, SF-36, and Morse Fall Scale. Correlations between postural stability and radiographic measurements were performed. Results ASD patients demonstrated significantly lower SOT scores (P≤0.03) in 5 of 6 conditions tested, and greater latency of limb movement during backward translation (P=0.04) compared with controls. Lower SOT scores were associated with a history of falls. ASD patients who self-reported falling in the previous 6 months, when compared to nonfallers, demonstrated significantly lower SOT scores (P=0.04) and significantly lower SRS-22 self-image subscores (P=0.003). Thoracic kyphosis and mediolateral sway (predictor of falls) were positively correlated in the eyes open and eyes closed conditions (P≤0.04). Conclusions ASD patients demonstrated impaired postural stability, diminished sensory integration, and delayed response to external perturbations compared with normal control data. Postural stability and quality of life metrics correlated with self-reported falls. Patterns of epidural gas were classified as air pseudocyst, air cyst, air-contained disc herniation, and honeycomb-like air cyst. These gas patterns were compared, and possible correlations between these gas patterns and radiculopathy were evaluated. Results Overall agreement between CT and MRI findings for evaluation of all lesions and for differentiation of epidural gases was good (kappa [κ] = 0.775). Air pseudocysts demonstrated a moderate correlation (κ=0.496) and air cysts showed a good correlation (κ=0.661) with radiculopathy on MRI, whereas air-contained disc herniation and honeycomb-like cysts demonstrated a strong correlation (κ=0.810 and 0.927, respectively) with radiculopathy on MRI. Methods All consecutive patients who underwent open cerebrovascular surgery and intraoperative 2D-IOA with 3D-RA in a hybrid operating room at 2 academic centers between August 2018 to December 2019 were identified from a prospectively maintained institutional database. Medical charts and operative videos including intraoperative angiography were reviewed and clinical and angiographic outcomes assessed. Results A total of 40 cerebrovascular surgeries in 39 patients (mean age 53 ± 13 years; 51% female) were carried out with the addition of 3D-RA to 2D-IOA in the hybrid operating room. Following 3D-RA in addition to 2D-IOA, 1 (2.5%) surgical alteration occurred in an aneurysm clipping.
