Symptomatic extrinsic compression of the external iliac artery (EIA) is a rare complication of surgical intervention. Previous cases of EIA compression have presented in the postoperative period after orthopedic surgery or vascular stenting. We report a case of right EIA compression postvenous stenting causing acute limb ischemia.Testicular torsion is a urologic emergency that requires surgical intervention. Its diagnosis is rarely made in elderly men especially the subset of men on urethral catheter. As a result, delayed diagnosis and surgical exploration occur leading to testicular infarction with necrosis, abscess formation and ultimately orchidectomy. We report a 73-year-old urologic patient referred with a 2-month history of transurethral catheterization to relieve retention of urine with subsequent scrotal pains and fever. Physical examination showed left hemi-scrotal swelling and normal right hemi-scrotal findings. A Doppler scan done showed an intratesticular fluid collection with no blood flow in the left testes. This case illustrates the need to include testicular torsion when diagnosing geriatric men with transurethral catheter presenting with any acute scrotal pains. We, therefore, recommend a detailed history and physical examination in addition to a colour Doppler ultrasound scan in making a diagnosis.We describe a case of lung lobectomy and resection of the rib neck and head in a lung cancer patient with an invasion of the chest wall. The tumor was located in the upper lobe, adjacent to the neck of the third rib. We performed a right upper lobectomy and en bloc resection of the third rib, including the rib neck and head, by video-assisted thoracoscopic surgery with an additional 6 cm posterior incision along the right paravertebral line. The costovertebral joint incision procedure is a useful technique to ensure tumor-free margins in cases where the tumor is located close to the rib's neck and head.Acute pelvic pain is a common complaint in reproductive age women and has a large differential diagnosis. Decision for conservative vs. surgical management is often dependent on clinical, biochemical and imaging findings. Isolated tubal torsion is a rare cause of pelvic pain that requires prompt diagnosis and surgical management to avoid morbidity. Here, we report non-pregnant women of reproductive age presenting with acute lower abdominal pain. Raised inflammatory markers and ultrasound findings prompted management for Pelvic Inflammatory Disease. Despite some improvement with antibiotics, the patient had ongoing symptoms. At surgery, bilateral para-tubal cysts and a left sided hydrosalpinx were found, along with an isolated left tubal torsion. Isolated tubal torsion most commonly occurs in reproductive aged women, and risk factors include intrinsic tubal pathology and extrinsic lesions. Clinically, biochemically and radiographically, it is often indistinguishable from other pelvic pathology, potentially leading to diagnostic delay, and necrosis of the tube.We experienced the case of successful conversion to distal gastrectomy for gastric cancer with direct invasion to the pancreatic head after the pre-operative chemotherapy. Esophagogastroduodenoscopy in a 66-year-old man revealed a tumor at the gastric antrum. We present an exceedingly unique case of a patient surviving multiple gunshot wounds with two bullet fragments lodged in the interventricular septum. A 25-year-old male sustained four gunshot wound injuries to the upper body. Two cardiac interventricular septal bullet fragments were identified during his recovery. Management included serial echocardiographic surveillance and a two-month regimen of empiric colchicine for prophylaxis against post-traumatic pericarditis. Pursuing non-operative management especially in asymptomatic or stable patients should be evaluated against surgical extraction and possible sequelae of complications. The consideration of scheduled colchicine for pericarditis prophylaxis is warranted as well as interval echocardiogram. Retained myocardial bullets are exceedingly rare clinical events with scant literature available to guide clinical decisions. Management requires intricate decision-making and close consideration of risk benefit analysis weighing surgical extraction against non-operative management.Pulmonary valve stenosis and regurgitation can occur in the distant stages after intracardiac repair of tetralogy of Fallot (TOF). Aortic regurgitation (AR) can also occur, although it is rare in postoperative patients. However, there are few reports of simultaneous replacement of the pulmonary and aortic valves in young patients after intracardiac repair of TOF, and there are no clear guidelines for selecting a valve prosthesis in such patients. We report a case of severe pulmonary valve stenosis and regurgitation with severe AR 38 years after the TOF operation, in which urgent double valve replacement and right ventricular outflow tract patching were performed with a mechanical valve in the aortic valve position and a bioprosthetic valve in the pulmonary valve position, with a successful outcome.Negative pressure wound therapy (NPWT) with instillation therapy (V.A.C. Vera-Flow™) and dwell time (NPWTi-d) is an innovative method for complex wound healing. NPWTi-d combines vacuum-aided drainage of wounds with the precise distribution of topical cleansing solution over the wounds. Furthermore, super oxidized solutions have illustrated their ability to potentiate wound healing and decrease bacterial contamination. Furthermore, aided with this method, infected wounds can be disinfected. If surgical debridement or removal of the infected site is not possible or desired. Therefore, in the case of a 66-year-old patient with diabetic foot syndrome (DFS) with severe infection, our approach was to couple NPWTi-d with instillation and dwelling of super oxidized solution to bolster benefits. This is the first case report using NPWTi-d with instillation of super oxidized solution in DFS in Switzerland. This case indicates that this approach is beneficial in the treatment of complex and critically infected wounds in DFS.
