Eighty-eight (7%) patients developed an S-SSI. Patients who developed S-SSI had higher final lactate, blood loss, transfusion requirements, and wound classification. Patients with S-SSI more frequently had mesenteric or large bowel injury than those without S-SSI. Superficial SSI was associated with increased complications and prolonged length of stay (LOS). We revisit Marr's levels-of-analysis framework, which has been remarkably productive and useful for cognitive psychological explanation. We discuss ways in which Marr's framework may be extended to other areas of psychology, such as social, developmental, and evolutionary psychology, bringing new benefits to these fields. We then show how theoretical analyses can endow a theory with minimal plausibility even before contact with empirical data We call this the theoretical cycle. Finally, we explain how our proposal may contribute to addressing critical issues in psychological science, including how to leverage effects to understand capacities better.
We recently compared clinical outcomes in secondary progressive MS (SPMS) clinical trials and found an association of timed 25 foot walk (T25FW) worsening events and baseline disability scores. It is unclear whether disability worsening in clinical trials is comparable to that seen in clinical practice.

The objective of this study is to compare disability worsening between the IMPACT and ASCEND data sets and data from the Calgary MS clinic and to characterize the association of baseline T25FW and expanded disability status scale (EDSS) scores with disability worsening.

We combined the three data sets and investigated the impact of baseline characteristics on disability worsening with a logistic regression model. We calculated T25FW, EDSS, and 'EDSS or T25FW' worsening events as a function of ascending cut-off baseline disability scores.

Data source was not associated with T25FW worsening at 12 months. There was a strong association of baseline T25FW and EDSS cut-off scores with T25FW worsening. No such association was present for the EDSS and 'EDSS or T25FW'.

Our results suggest that it is possible to 'enrich' a trial cohort for expected T25FW worsening events using specific baseline T25FW and EDSS cut-off scores. These analyses inform the selection of inclusion criteria for clinical trials in SPMS.

We studied the association between vitamin D and clinical outcomes in COVID-19 patients. This is a retrospective cohort study on COVID-19 patients with documented vitamin D levels within the last year. Vitamin D levels were grouped as ≥ 20 ng/mL or less then  20 ng/mL. Main outcomes were mortality, need for mechanical ventilation, new DVT or pulmonary embolism, and ICU admission. A total of 270 patients (mean ± SD) age, 63.81 (14.69) years); 117 (43.3%) males; 216 (80%) Blacks; 139 (51.5%) in 65 and older age group were included. Vitamin D levels were less than 20 ng/mL in 95 (35.2%) patients. During admission, 72 patients (26.7%) died, 59 (21.9%) needed mechanical ventilation, and 87 (32.2%) required ICU. Vitamin D levels showed no significant association with mortality (OR = 0.69; 95% CI, 0.39-1.24; P = 0.21), need for mechanical ventilation (OR = 1.23; 95% CI, 0.68-2.24; P = 0.49), new DVT or PE(OR= 0.92; 95% CI, 0.16-5.11; P = 1.00) or ICU admission (OR = 1.38; 95% CI, 0.81-2.34; P = 0.23). We did not find any significant association of vitamin D levels with mortality, the need for mechanical ventilation, ICU admission and the development of thromboembolism in COVID-19 patients.NEW & NOTEWORTHY Low vitamin D has been associated with increased frequency and severity of respiratory tract infections in the past. Current literature linking clinical outcomes in COVID-19 with low vitamin D is debatable. This study evaluated the role of vitamin D in severe disease outcomes among COVID-19 patients and found no association of vitamin D levels with mortality, the need for mechanical ventilation, ICU admission, and thromboembolism in COVID-19.Previous research on commercial sex has described fluidity between different forms of relationships, whereby commercial sexual relationships can be both long-term and viewed as intimate from the buyer's perspective. This article explores the construction of intimacy in long-term commercial relationships. More specifically, it examines the meaning of transactions in long-term paid sexual relationships in Sweden. Interviews were conducted with 23 Swedish men with experience purchasing sex as 'regulars'. An inductive thematic analysis was conducted. Findings show that the emotional experience is a key focus for these men when they purchase sex. The emotions involved are not delimited in time and space but are experienced both within and outside of the actual sexual encounter. Such emotions can be understood as the very precondition for the experiences of intimacy, while at the same time they create difficulties for the men who purchase sex. Experiences of intimacy are experienced in the ambiguity between unbounded and bounded authenticity and by not drawing a clear line between emotional subjectivity and consumer subjectivity.
