Incidence of prostate cancer (PC) is increasing, but androgen deprivation therapy (ADT) and other therapies are substantially improving survival. In this context, careful consideration of skeletal health is required to reduce the risk of treatment-related fragility fractures and their associated morbidity and mortality. Although the number of true SBO cases requiring emergency attention is few, distinguishing SBO from the more common less urgent pediatric abdominal conditions is vital for the most effective patient care. Therefore, immediate and accurate identification of this condition is essential in order to avoid delays in diagnosis that may increase morbidity and mortality. We report a case involving a 7-year-old girl who initially presented with abdominal pain and vomiting. A bedside ultrasonography performed by the pediatric emergency physician allowed for the initiation of appropriate, rapid decision making including extensive examination and management. Although bedside ultrasonography has been used in the past to diagnose SBO in adult patients, this is the first report on diagnosing a pediatric patient with SBO utilizing bedside ultrasonography considering criteria that are different from those for children in the past case series. Here, we also review SBO and its diagnostic modalities and the role of the pediatric emergency physician in performing ultrasonography.Rivastigmine is a non-competitive reversible inhibitor of acetylcholinesterase which is approved as one of the fi rst-line treatment options for Alzheimer's disease. We present the case of a 33-year-old woman with acute cholinergic syndrome secondary to deliberate rivastigmine poisoning. The patient presented at the emergency department (ED) with drowsy consciousness, dizziness, vomiting, diarrhea, sweating, and hypertension (171/103 mmHg). At the scene, an empty bottle of Rivast 120 mL/Bot, containing rivastigmine 2 mg/mL, was found beside the patient. Two hours later, we noted bronchorrhea and persistent salivation along with drowsiness, agitation, fatigue, incontinence, and limbs paralysis. A notably low serum cholinesterase level (651 U/l) was identified. Acute cholinergic syndrome secondary to rivastigmine intoxication was diagnosed. Endotracheal intubation with ventilator support was required due to respiratory failure. Atropine (0.5 mg intravenous injection) was administered. She was subsequently admitted to the intensive care unit for further care. Extubation was performed on the third day. The patient insisted on being discharged on the second day after extubation, and after administration of a total of 11 mg of atropine, no signs of either intermediate syndrome or delayed polyneuropathywere noted. rivastigmine, an acetylcholinesterase inhibitor, can precipitate an acute cholinergic crisis in cases of intoxication. Typical clinical features of cholinergic excess include increased secretions in the airway and oral cavity, miosis, diarrhea, anxiety, twitching, bronchoconstriction, convulsions, confusion, and gastrointestinal and muscular cramps. The treatment for acute cholinergic crisis is administration of atropine alone or in combination with an antidote to the cholinesterase inhibitor (such as pralidoxime). Patients often recover well with atropine supplements and optimal supportive care.

We conducted a study using 2002-2012 data from the nationwide health insurance database of Taiwan. Sepsis hospitalizations were identified by Angus algorithm to include all cases with ICD-9-CM codes for specific sepsis diagnosis and both an infectious process and a diagnosis of acute organ dysfunction. The primary outcome was the trend of incidence and in-hospital mortality of specific type of organ dysfunction in sepsis patients.
We identified 1,259,578 adult patients with sepsis. Acute respiratory dysfunction, cardiovascular dysfunction/shock, and renal system dysfunction were the leading three types of acute organ dysfunction, accounting for 65.6, 30.5, and 18.3% of all sepsis patients, respectively. All types of organ dysfunction increased over time, except for hepatic and metabolic systems. Renal system (annual increase 13.5%) and cardiovascular system dysfunction (annual increase 4.3%) had the fastest increase. Mortality from all sources of infection has decreased significantly in the study period (trend
< 0.001).

Seizures are one of the most common complications of stroke. We aimed to establish the incidence and clinical profile of post-stroke early seizure (ES) in patients with intracerebral hemorrhage (ICH).
Patients with ICH within 10 days of onset who were admitted to Landseed International Hospital were recruited consecutively between January 1, 2006, and December 31, 2009. The National Institutes of Health Stroke Scale (NIHSS) and the modified Rankin Scale were used to access patients initial stroke severity and functional outcome at discharge, respectively. The occurrence of epileptic seizures within 30 days after onset of the index ICH was recorded. Early post-ICH seizure was defined by the occurrence of clinically identifid seizure episodes or non-epileptic seizure within 7 days after the stroke onset.
