OBJECTIVE. Yellow fever is a hemorrhagic disease caused by an arbovirus endemic in South America; outbreaks have occurred in recent years. The purpose of this study was to describe abdominal ultrasound findings in patients with severe yellow fever and correlate them with clinical and laboratory data. MATERIALS AND METHODS. A retrospective cohort study was performed between January and April 2018. The subjects were patients admitted to an ICU with polymerase chain reaction-confirmed yellow fever. Bedside sonography was performed within 48 hours of admission. Images were independently analyzed by two board-certified radiologists. Laboratory test samples were collected within 12 hours of image acquisition. Multivariable logistic regression analysis was performed to identify 30-day mortality predictors; p less then .05 was considered statistically significant. Laboratory tests showed high serum levels of aspartate aminotransferase (5319 U/L), total bilirubin (6.2 mg/dL), and creati-nine (4.3 mg/dL). Twenty-six (56.5%) patients died within 30 days of admission (median time to death, 5 days [interquartile range, 2-9 days]). The most frequent ultrasound findings were gallbladder wall thickening (80.4%), increased renal cortex echogenicity (71.7%), increased liver parenchyma echogenicity (65.2%), perirenal fluid (52.2%), and ascites (30.4%). Increased renal echogenicity was associated with 30-day mortality (84.6% versus 55.0%; p = .046) and was an independent predictor of this outcome after multivariate analysis (odds ratio, 10.89; p = .048). CONCLUSION. Reproducible abdominal ultrasound findings in patients with severe yellow fever may be associated with severity of disease and prognosis among patients treated in the ICU.OBJECTIVE. In this institutional review board-approved, HIPAA-compliant study, all TIPS reductions performed at two institutions from January 1, 2008 to January 31, 2016, were retrospectively identified. Patients were divided into two groups according to pre-TIPS symptoms volume overload due to ascites or hydrothorax (VO; n = 14) or variceal bleeding (VB; n = 12). Patient demographics, pre-TIPS model for end-stage liver disease score, pre- and post-TIPS portosystemic gradients, and clinical parameters were recorded. The primary endpoint was change in symptoms of hepatic encephalopathy (HE; West Haven criteria), right heart failure, or liver dysfunction. Secondary endpoints included paracentesis rate for the VO group and rebleeding for the VB group. RESULTS. The degree of HE increased in 24 of 26 patients (92%) after TIPS placement and decreased in 24 of 26 patients (92%) after TIPS reductn patients with VB, TIPS reduction with variceal embolization results in a low risk of short-term recurrent VB. However, in patients with VO, ascites may return or worsen after TIPS reduction despite improvement in HE.OBJECTIVE. This retrospective study included 182 patients (136 men and 46 women; median age, 63 years [interquartile range, 57-70 years]) who underwent 338 consecutive doxorubicin drug-eluting bead (DEB) chemoembolization procedures between 2011 and 2014. Outcomes were assessed until November 2019. In 97% of procedures, two or fewer segments were targeted. The Barcelona Clinic Liver Cancer (BCLC) stage was 0 or A for 77 procedures (22.8%), B for 75 (22.2%), C for 122 (36.1%), and D for 64 (18.9%). Hepatotoxicity was defined as worsened ascites or encephalopathy or as grade 3 or 4 elevations in liver function test results, creatinine levels, or the international normalized ratio within 30 days. Risk factors were assessed by univariate and multivariable generalized estimating equations. Transplant-free survival was assessed using Cox prop it may hasten death among patients who are ineligible for liver transplant.OBJECTIVE. MATERIALS AND METHODS. Thirty-eight patients with implanted hardware after spinal instrumentation (24 patients with short- or long-term instrumentation-related complications [i.e., adjacent segment disease, screw loosening or implant failure, or postoperative hematoma or seroma] and 14 control subjects with no complications) underwent ****. Scans were simulated as if they were performed with 50% (P50), 25% (P25), 10% (P10), and 5% (P5) of the projections of the original acquisition using an in-house-developed SIR algorithm for advanced image reconstructions. Two readers performed qualitative image evaluations of overall image quality and artifacts, image contrast, inspection of the spinal canal, and diagnostic confidence (1 = high, 2 = medium, and 3 = low confidence). RESULTS. Although overall image quality decreased and artifacts increased with reductions in the number of projections, all complications were detected by both readers when 100% of the projections of the original acquisition (P100), P50, and P25 imaging data were used. For P25 data, diagnostic confidence was still high (mean score ± SD reader 1, 1.2 ± 0.4; reader 2, 1.3 ± 0.5), and interreader agreement was substantial to almost perfect (weighted Cohen κ = 0.787-0.855). The mean volumetric CT dose index was 3.2 mGy for P25 data in comparison with 12.6 mGy for the original acquisition (P100 data). CONCLUSION. The use of sparse sampling and SIR for low-dose **** in patients with spinal instrumentation facilitated considerable reductions in radiation exposure. The use of P25 data with SIR resulted in no missed complications related to spinal instrumentation and allowed high diagnostic confidence, so using only 25% of the projections is probably enough for accurate and confident diagnostic detection of major instrumentation-related complications.
