PD-1/PD-L1 inhibitors have made unprecedented progress in the treatment of cancer.
A systemic search was conducted for randomized controlled trials that compared PD-1/PD-L1 inhibitor monotherapy or combination therapy with nonimmunotherapy. Totally, 13 studies with monotherapy and 5 with combination regimens were included, and the pooled HRs of OS were 0.74 (

< 0.001), respectively. EGFR wild-type patients could benefit from immunotherapy monotherapy (HR, 0.77;
< 0.001) while those of the mutant type had no survival benefit (HR, 1.11;
= 0.54), and the difference was statistically significant (interaction,
= 0.005). KRAS wild-type patients had no survival benefit from monotherapy (HR, 0.89;
= 0.49). For combination therapy, both male and female derived benefits but female had a significantly reduced risk of death (HR, 0.45;
< 0.001) compared with male (HR, 0.73;

= 0.004). Nonsmokers derived more survival benefits from combination therapy (HR, 0.29;
< 0.001) than smokers (HR, 0.63;

The accelerate loss of skeletal muscle mass, strength, and function, named sarcopenia, is a progressive and generalised skeletal muscle disorder, and it is always associated with increased outcomes including falls, frailty, and disability. Diabetes mellitus is associated with significant muscle and physical complications. We aimed at clarifying the changes and risk factors of skeletal muscle mass and strength in elderly with type 2 diabetes.
The study consisted of patients with type 2 diabetes (
= 120) and an older general population (
= 126). The skeletal muscle mass and muscle strength, as well as the serum levels of chronic inflammation, oxidative stress, homocysteine, and insulin-like factor-1 were assessed, and the correlation and regression analysis were conducted to evaluate outcomes.
T2DM patients exhibited lower muscle strength compared with the non-T2DM subjects (

To evaluate diabetic retinopathy (DR) screening via deep learning (DL) and trained human graders (HG) in a longitudinal cohort, as case spectrum shifts based on treatment referral and new-onset DR.
We randomly selected patients with diabetes screened twice, two years apart within a nationwide screening program. The reference standard was established via adjudication by retina specialists. Each patient's color fundus photographs were graded, and a patient was considered as having sight-threatening DR (STDR) if the worse eye had severe nonproliferative DR, proliferative DR, or diabetic macular edema. We compared DR screening via two modalities DL and HG. For each modality, we simulated treatment referral by excluding patients with detected STDR from the second screening using that modality.
There were 5,738 patients (12.3% STDR) in the first screening. DL and HG captured different numbers of STDR cases, and after simulated referral and excluding ungradable cases, 4,148 and 4,263 patients remained in the sg can be more difficult and induce lower sensitivity for both DL and HG, though the false negative rate was substantially lower for DL. Our data may be useful for health-economics analyses of longitudinal screening settings.
Previous studies of coronavirus disease 2019 (COVID-19) have focused on the general population. However, diabetes (DM) as one of the most common comorbidities is rarely studied in detail. This study is aimed at describing clinical characteristics and determining risk factors of ICU admission for COVID-19 patients with DM.
Data were extracted from 288 adult patients with laboratory-confirmed COVID-19 from Guangzhou Eighth People's Hospital. Demographic characteristics, laboratory results, radiographic findings, complications, and treatments were collected and compared between DM and non-DM groups. Binary logistic regression was used to identify the risk factors associated with ICU admission for COVID-19 patients with DM or non-DM.
