  • Objective Health utility estimates for children and adolescents are critical for cost-utility analyses informing health technology assessment (HTA) authorities' decisions governing access to pediatric treatments. However, in a recent review, only 29% of published pediatric cost-utility models used a utility measure validated for children. We examined utility estimates used in pediatric HTAs.Methods A targeted review of pediatric HTAs was performed, focusing on agencies reporting utility estimate sources and methods.Results Searches identified 11 HTAs in pediatric indications and five in mixed populations with separate analyses for adults and children. Among 13 appraisals reporting methodological detail, five used pediatric utility estimates (based on the Health Utilities Index [HUI], n = 3; Atopic Dermatitis Quality of Life [ADQoL], n = 1; or mapping, n = 1). Issues were identified with mapping, use of adult data for some health states, and assumptions about ADQoL responses. In the remaining eight appraisals, adult utility estimates were applied. Caregiver utility was included in two of 16 appraisals.Conclusions Only 38% of pediatric HTAs reviewed used pediatric utility estimates, and HTA authorities raised concerns about these data in many cases; only 12% of HTAs included caregiver utility. Although several preference-based utility measures are available for pediatric populations, limited data and guidance on selection of measures are available. When estimating pediatric utility weights, alternative measures should be reviewed for suitability given the model population and health condition. Pediatric and adult utility estimates should be applied appropriately as patients age over time, and caregiver and/or family member utility should be included, where relevant. Gaps exist in utility measures for children aged less then 4 years and caregivers.Cancer patients learn about research studies outside of the clinical environment, including websites, print and online advertisements, and interpersonal interactions. When cancer patients share credible information about clinical trials, they also frequently help clarify misunderstandings that may exist in their social networks. The present study investigated how an interactive tailored information aid on clinical trial participation motivated patients' information sharing behaviors. In this study of 312 cancer patients and survivors, an interactive tailored information aid improved patients' likelihood of sharing online and offline information more than a non-interactive tool. Information sharing was directly predicted by cognitive absorption and perceived visual informativeness. In addition, perceived utility and ease of use indirectly impact information sharing positively through the antecedent factors of user engagement and design esthetics. Education level further moderated this effect; information sharing was higher among patients with more education. The latest increase in the fame of natural products and alternative medicine has revived interest in conventional remedies that have been consumed in the management of CVD. The cardio-protective properties of the various herbs are possibly due to their anti-oxidative, antihypercholesterolemic, anti-ischemic activities, and inhibition of platelet aggregation that reduce the risk of CVD. Ethno-pharmacological and biological properties of these plants are explored, based upon published scientific literature. Although a majority of medicinal plants having a biological mechanism that linked with CVD management, to date, published literature pertaining to their promising scientific properties are still poorly understood. Compared with synthetic medicines, alternative medicines do not need scientific studies before their formal approval from the government sector and due to this purpose; their safety, as well as efficacy, still remain elusive. Taken together, we addressed all accessible evidence on alternative medicines commonly consumed in CVD management. Our comprehensive analysis of the scientific literature indicated that many TCMs are available and valuable herbal medication would be the best alternative for the management of CVD as a complementary therapy. Furthermore, practitioners should always discuss possible benefits-risks of alternative medicines with patients so that they are aware of the consumption of alternative medications.AIMS In heart failure, oxygen uptake and cardiac output measurements at peak and during exercise are important in defining heart failure severity and prognosis. Several cardiopulmonary exercise test-derived parameters have been proposed to estimate stroke volume during exercise, including the oxygen pulse (oxygen uptake/heart rate). Data comparing measured stroke volume and the oxygen pulse or stroke volume estimates from the oxygen pulse at different stages of exercise in a sizeable population of healthy individuals and heart failure patients are lacking. METHODS We analysed 1007 subjects, including 500 healthy and 507 heart failure patients, who underwent cardiopulmonary exercise testing with stroke volume determination by the inert gas rebreathing technique. Stroke volume measurements were made at rest, submaximal (∼50% of exercise) and peak exercise. At each stage of exercise, stroke volume estimates were obtained considering measured haemoglobin at rest, predicted exercise-induced haemoconcentration and peripheral oxygen extraction according to heart failure severity. RESULTS A strong relationship between oxygen pulse and measured stroke volume was observed in healthy and heart failure subjects at submaximal (R2 = 0.6437 and R2 = 0.6723, respectively), and peak exercise (R2 = 0.6614 and R2 = 0.5662) but not at rest. In healthy and heart failure subjects, agreement between estimated and measured stroke volume was observed at submaximal (-3 ± 37 and -11 ± 72 ml, respectively) and peak exercise (1 ± 31 and 6 ± 29 ml, respectively) but not at rest. CONCLUSION In heart failure patients, stroke volume estimation and oxygen pulse during exercise represent stroke volume, albeit with a relevant individual data dispersion so that both can be used for population studies but cannot be reliably applied to a single subject. Accordingly, whenever needed stroke volume must be measured directly.

The primary data to determine when studying proteins are the per-atom NMR chemical shifts, which reflect the local environment of atoms and provide insights into amino acid residue dynamics and conformation. Within an amino acid residue, chemical shifts present multi-dimensional and complexly cross-correlated information, making them difficult to analyse. The ShiftCrypt method, based on neural network auto-encoder architecture, compresses the per-amino acid chemical shift information in a single, interpretable, amino acid-type independent value that reflects the biophysical state of a residue. We here present the ShiftCrypt web server, which makes the method readily available. The server accepts chemical shifts input files in the NMR Exchange Format (NEF) or NMR-STAR format, executes ShiftCrypt and visualises the results, which are also accessible via an API. It also enables the "biophysically-based" pairwise alignment of two proteins based on their ShiftCrypt values. Objective To explore the association between cannabis potency and substance use and mental health outcomes, accounting for preceding mental health and frequency of cannabis use. Design, setting, and participants This cohort study used data from the Avon Longitudinal Study of Parents and Children, a UK birth cohort of participants born between April 1, 1991, and December 31, 1992. Present data on outcomes and exposures were collected between June 2015 and October 2017 from 1087 participants at 24 years of age who reported recent cannabis use. Exposures Self-reported type of cannabis most commonly used in the past year, coded to a binary exposure of use of high-potency R, 1.42; 95% CI, 0.89-2.27), and other illicit drug use (AOR, 1.29; 95% CI, 0.77-2.17). There was no evidence of association between the use of high-potency cannabis and alcohol use disorder or depression. Conclusions and relevance To our knowledge, this study provides the first general population evidence suggesting that the use of high-potency cannabis is associated with mental health and addiction. Limiting the availability of high-potency cannabis may be associated with a reduction in the number of individuals who develop cannabis use disorders, the prevention of cannabis use from escalating to a regular behavior, and a reduction in the risk of mental health disorders.Heartland Virus is a tickborne phlebovirus first identified in Missouri in 2009; 11 human cases have been reported in the literature. Identifying and characterizing anti-CRISPR proteins opens an avenue to explore and control CRISPR-Cas machineries for the development of new CRISPR-Cas based biotechnological and therapeutic tools. Past studies have identified anti-CRISPRs in several model phage genomes, but a challenge exists to comprehensively screen for anti-CRISPRs accurately and efficiently from genome and metagenome sequence data. Here, we have developed an ensemble learning based predictor, PaCRISPR, to accurately identify anti-CRISPRs from protein datasets derived from genome and metagenome sequencing projects. PaCRISPR employs different types of feature recognition united within an ensemble framework. Extensive cross-validation and independent tests show that PaCRISPR achieves a significantly more accurate performance compared with homology-based baseline predictors and an existing toolkit. The performance of PaCRISPR was further validated in discovering anti-CRISPRs that were not part of the training for PaCRISPR, but which were recently demonstrated to function as anti-CRISPRs for phage infections. Previous kinetic, site-directed mutagenesis and crystallographic results are consistent with allosteric activation, but the existence of a G6P-allosteric site was questioned and competitive activation-in which G6P would bind to the active site eliciting the same positive homotropic effect as the substrate phosphoenolpyruvate (PEP)-was proposed. Here, we report the crystal structure of the PEPC-C4 isozyme from Zea mays with G6P well bound into the previously proposed allosteric site, unambiguously confirming its existence. To test its functionality, Asp239-which participates in a web of interactions of the protein with G6P-was changed to alanine. The D239A variant was not activated by G6P but, on the contrary, inhibited. Inhibition was also observed in the wild-type enzyme at concentrations of G6P higher than those producing activation, and probably arises from G6P binding to the active site in competition with PEP.

  • With efforts to restore large mammal populations following extirpations, it is vital to quantify how they are impacted by human activities and gain insights into population dynamics in relation to conservation goals. Our objective was to characterize cause-specific mortality of black bears (Ursus americanus) throughout their range. We first quantified cause-specific mortality for 247 black bears in one harvested and two non-harvested populations. We then simulated a small recolonizing population with and without anthropogenic mortality. Lastly, we conducted a meta-analysis of all published black bear mortality studies throughout North America (31 studies of 2630 bears). We found anthropogenic mortality was greater than natural mortality, non-harvest anthropogenic mortality (e.g. poaching, defense of property, etc.) was greater in non-harvested populations, and harvesting was one of the major causes of mortality for bears throughout their range. Our simulation indicated that removing anthropogenic mortality increased population size by an average of 23% in 15 years. We demonstrated that bears are exposed to high levels of anthropogenic mortality, and the potential for human activities to slow population growth in expanding populations. Management and conservation of wide-ranging mammals will depend on holistic strategies that integrate ecological factors with socio-economic issues to achieve successful conservation and coexistence.The strong coupling between intense laser fields and valence electrons in molecules causes distortions of the potential energy hypersurfaces which determine the motion of the nuclei and influence possible reaction pathways. The coupling strength varies with the angle between the light electric field and valence orbital, and thereby adds another dimension to the effective molecular potential energy surface, leading to the emergence of light-induced conical intersections. Here, we demonstrate that multiphoton couplings can give rise to complex light-induced potential energy surfaces that govern molecular behavior. In the laser-induced dissociation of H2+, the simplest of molecules, we measure a strongly modulated angular distribution of protons which has escaped prior observation. Using two-color Floquet theory, we show that the modulations result from ultrafast dynamics on light-induced molecular potentials. These potentials are shaped by the amplitude, duration and phase of the dressing fields, allowing for manipulating the dissociation dynamics of small molecules.At present, it is not clear how memory B lymphocytes are maintained over time, and whether only as circulating cells or also residing in particular tissues. The HFNC therapy at a flow rate up to 50~60 L/min significantly decreased the apnea-hypopnea index from 52.0 events/h (IQR 29.9-61.9) to 26.5 events/h (IQR 3.3-34.6) and the total arousal index from 34.6 (IQR 18.6-42.3) to 15.0 (IQR 10.3-25.4). The oxygen desaturation index was also significantly decreased from 53.0 events/h (IQR 37.0-72.8) to 16.2 events/h (IQR 0.8-20.1), accompanied by a significant improvement in the minimum SpO2 level. Finally, only three participants tolerated flow rates of 50~60 L/minute in one-week treatment period. Conclusively, HFNC therapy at therapeutic flow rate is effective at reducing the OSA severity in post-acute ischemic stroke patients with nasogastric intubation. A total of 46 patients, aged 18 to 80 years, with unilateral inguinal hernia, one group that received general anesthesia and one that received sedation and QL block for TEP inguinal hernia repair. In the 46 patients the median pain score 6 hours after surgery was significantly lower (2 versus 4) among the QL block group than among the group receiving general anesthesia. Consequently, the former group showed a briefer median hospital stay (6 versus 24 hours, respectively). The anesthesia and hospital costs were also lower for the QL block group, with median reductions of 64.15% and 25%, respectively. QL block is a safe and effective option for patients undergoing TEP inguinal hernia repair, given the observed reduction in early postoperative pain, briefer hospital stay, and decreased anesthesia and hospital costs.
    With efforts to restore large mammal populations following extirpations, it is vital to quantify how they are impacted by human activities and gain insights into population dynamics in relation to conservation goals. Our objective was to characterize cause-specific mortality of black bears (Ursus americanus) throughout their range. We first quantified cause-specific mortality for 247 black bears in one harvested and two non-harvested populations. We then simulated a small recolonizing population with and without anthropogenic mortality. Lastly, we conducted a meta-analysis of all published black bear mortality studies throughout North America (31 studies of 2630 bears). We found anthropogenic mortality was greater than natural mortality, non-harvest anthropogenic mortality (e.g. poaching, defense of property, etc.) was greater in non-harvested populations, and harvesting was one of the major causes of mortality for bears throughout their range. Our simulation indicated that removing anthropogenic mortality increased population size by an average of 23% in 15 years. We demonstrated that bears are exposed to high levels of anthropogenic mortality, and the potential for human activities to slow population growth in expanding populations. Management and conservation of wide-ranging mammals will depend on holistic strategies that integrate ecological factors with socio-economic issues to achieve successful conservation and coexistence.The strong coupling between intense laser fields and valence electrons in molecules causes distortions of the potential energy hypersurfaces which determine the motion of the nuclei and influence possible reaction pathways. The coupling strength varies with the angle between the light electric field and valence orbital, and thereby adds another dimension to the effective molecular potential energy surface, leading to the emergence of light-induced conical intersections. Here, we demonstrate that multiphoton couplings can give rise to complex light-induced potential energy surfaces that govern molecular behavior. In the laser-induced dissociation of H2+, the simplest of molecules, we measure a strongly modulated angular distribution of protons which has escaped prior observation. Using two-color Floquet theory, we show that the modulations result from ultrafast dynamics on light-induced molecular potentials. METHODS We performed a retrospective review of patients with unresectable locally advanced and metastatic CRC (mCRC) and known KRAS/NRAS/BRAF status, attended in the Medical Oncology Department of the Hospital General Universitario Gregorio Marañón (Madrid, Spain). The primary outcome was VTE defined as any venous thromboembolic event that occurred either 6 months before or at any time after the diagnosis of CRC. The biomarker status (KRAS, NRAS, and BRAF) and other predictors of thrombosis were collected. RESULTS One hundred and ninety-four patients were identified and included in the analysis. Forty-one patients (21.1%) experienced VTE. The incidence was 19.1% in RAS-mutated patients, 28.6% in BRAF-mutated patients and 21% in triple wild-type patients (p = NS). In multivariate analysis, ECOG ≥ 2 was the only independent predictor of VTE (OR 8.73; CI 95% 1.32-57.82; p = 0.025). CONCLUSIONS In our study, biomarkers have not been associated with an increased risk of VTE in CRC patients. METHODS Dense gate network (DG-Net) is proposed for biomedical image segmentation. In this model, the Gate Aggregate structure is utilized to reduce the semantic gaps between features in the encoder and the corresponding features in the decoder, and the gate unit is used to reduce the categorical ambiguity as well as to guide the low-level high-resolution features to recover semantic information. Through this method, the features could reach a similar semantic level before fusion, which is helpful for reducing semantic gaps, thereby producing accurate results. RESULTS Four medical semantic segmentation experiments, based on CT and microscopy images datasets, were performed to evaluate our model. In the cross-validation experiments, the proposed method achieves IOU scores of 97.953%, 89.569%, 81.870% and 76.486% on these four datasets. Compared with U-Net and MultiResUNet methods, DG-Net yields a higher average score on IOU and Acc. CONCLUSION The DG-Net is competitive with the baseline methods. The experiment results indicate that Gate Aggregate structure and gate unit could improve the performance of the network by aggregating features from different layers and reducing the semantic gaps of features in the encoder and the decoder.   • Purpose To determine whether femoral epicondylar width (FECW) obtained from either magnetic resonance imaging (MRI) or plain radiographs could be used to predict anterior cruciate ligament (ACL) length. A secondary purpose was to develop a formula to use maximum FECW on either MRI or plain radiographs to estimate ACL length preoperatively. Methods The MRIs and radiographs of 40 patients (mean age 41.0 years), with no apparent knee pathology, surgery, or trauma were included. The ACL length was measured on MRI followed by FECW on both MRI and radiograph of the same patient. This allowed the development of equations able to predict ACL length according to the FECW measured on either an MRI or radiograph. Results The mean ACL length was 40.6 ± 3.6 mm. FECW measured on both MRIs and radiographs was sufficient to predict ACL length. Pearson's correlations revealed a high positive relationship between ACL length and FECW on MRI (r = 0.89, P less then .0001) and ACL length and FECW on radiograph (r = 0.83, P less then .0001). The coefficient of determination (R2) was calculated to be MRI R2 = 0.78 and radiograph R2 = 0.68 and confirmed that FECW measured on both MRI and radiograph were sufficient to predict ACL length. Based on these models, ACL length can be predicted by FECW using the following formulas MRI ACL length = 0.47 (FECW) + 1.93 and radiograph ACL length = 0.31 (FECW) + 11.33. Methods The charts and imaging of 50 patients undergoing anterior cruciate ligament reconstruction were reviewed. A standardized method of intercondylar notch measurement on radiographs and MRI was used by 3 blinded reviewers. Arthroscopic measurements were made by the surgeon who was blinded to the imaging measurements. Interrater reliability was determined between reviewers and between imaging and arthroscopic measurements using interclass correlation coefficients (r). Results The average notch base width was 16.5 (± 2.7) mm on MRI, 19.0 (± 3.4) mm on radiographs, and 15.8 (± 3.0) mm on arthroscopic measurement. The radiographic notch base width measurements were on average 1.2 times greater than the arthroscopic measurements. There was no significant difference between males and females in notch base width (16.7 mm vs 15.3 mm, P = .19) or area (312.5 mm2 vs 284.3 mm2, P = .17). Interrater reliability was excellent between the reviewers for notch base width measurement on both MRI (r = 0.91) and radiographs (r = 0.95). Good-to-excellent interrater reliability between notch base width measurements on MRI and arthroscopy (r = 0.78, 0.73, 0.7) and fair-to-good interrater reliability between notch base width measurements on radiographs and arthroscopy were found (r = 0.61, 0.58, 0.55). Conclusions This study introduces a reliable method of using preoperative MRI to predict intercondylar notch width during arthroscopy. Methods Fifty-eight patients with chronic acromioclavicular separations underwent arthroscopic coracoclavicular ligament reconstructions using semitendinosus autografts. Constant and Simple Shoulder Test scores were determined before and 2 years after surgery as a part of standard clinical practice. General patient satisfaction with the outcome (poor, fair, or excellent) also was assessed. In addition, for purposes of routine clinical follow-up, the coracoclavicular distance was measured from the inferior cortex of the clavicle to the superior cortex of the coracoid using anteroposterior radiographs taken 2 years after surgery. The results were compared with postoperative radiographs and changes in the distance were recorded. Published by Elsevier Inc.Purpose To evaluate the clinical results following arthroscopic surgery in patients with anomaly of the anterior horn of the medial meniscus (AHMM) that was found unexpectedly during surgery and discuss whether resection is necessary in patients without anteromedial knee pain (AMKP). Methods Between May 2014 and April 2017, a total of 387 knee arthroscopies in 379 patients were performed. Among these, 11 knees in 11 patients showed an anomalous insertion of the AHMM (incidence, 2.8%), and all 11 patients were included in this study. For these 11 patients, medical records including preoperative diagnosis, arthroscopic findings, and pre- and postoperative clinical evaluations were analyzed. Results None of the patients complained of AMKP before arthroscopy. Two patients were diagnosed with lateral meniscus injury and the other 9 patients were diagnosed with medial meniscus injury. All anomalies of the AHMM were found incidentally during arthroscopic surgery. The anomaly formed a band-like structure arising from the anterior portion of the medial meniscus and was attached to the anterior aspect of the ACL and femoral intercondylar notch. All 11 patients underwent partial meniscectomy, but anomalies of the AHMM were not resected. One patient was excluded from clinical evaluation, as that patient required subsequent total knee arthroplasty due to osteoarthritis. For the other 10 patients, mean follow-up was 36.8 months (range, 26-61 months). Knee pain was relieved, and none developed postoperative AMKP. Mean Lysholm score improved significantly from 55.9 to 91.2 (P less then .001). Conclusions The incidence of the anomaly was 2.8% in our study. If the patient has no AMKP before arthroscopic surgery, anomaly of the AHMM is a silent lesion that does not warrant resection. Methods The MRIs and radiographs of 40 patients (mean age 41.0 years), with no apparent knee pathology, surgery, or trauma were included. The ACL length was measured on MRI followed by FECW on both MRI and radiograph of the same patient. This allowed the development of equations able to predict ACL length according to the FECW measured on either an MRI or radiograph. Results The mean ACL length was 40.6 ± 3.6 mm. FECW measured on both MRIs and radiographs was sufficient to predict ACL length. Pearson's correlations revealed a high positive relationship between ACL length and FECW on MRI (r = 0.89, P less then .0001) and ACL length and FECW on radiograph (r = 0.83, P less then .0001). The coefficient of determination (R2) was calculated to be MRI R2 = 0.78 and radiograph R2 = 0.68 and confirmed that FECW measured on both MRI and radiograph were sufficient to predict ACL length. Based on these models, ACL length can be predicted by FECW using the following formulas MRI ACL length = 0.47 (FECW) + 1.93 and radiograph ACL length = 0.31 (FECW) + 11.33. Methods The charts and imaging of 50 patients undergoing anterior cruciate ligament reconstruction were reviewed. A standardized method of intercondylar notch measurement on radiographs and MRI was used by 3 blinded reviewers. Arthroscopic measurements were made by the surgeon who was blinded to the imaging measurements. Interrater reliability was determined between reviewers and between imaging and arthroscopic measurements using interclass correlation coefficients (r). Results The average notch base width was 16.5 (± 2.7) mm on MRI, 19.0 (± 3.4) mm on radiographs, and 15.8 (± 3.0) mm on arthroscopic measurement. The radiographic notch base width measurements were on average 1.2 times greater than the arthroscopic measurements. There was no significant difference between males and females in notch base width (16.7 mm vs 15.3 mm, P = .19) or area (312.5 mm2 vs 284.3 mm2, P = .17). Interrater reliability was excellent between the reviewers for notch base width measurement on both MRI (r = 0.91) and radiographs (r = 0.95). Good-to-excellent interrater reliability between notch base width measurements on MRI and arthroscopy (r = 0.78, 0.73, 0.7) and fair-to-good interrater reliability between notch base width measurements on radiographs and arthroscopy were found (r = 0.61, 0.58, 0.55). Conclusions This study introduces a reliable method of using preoperative MRI to predict intercondylar notch width during arthroscopy. Methods Fifty-eight patients with chronic acromioclavicular separations underwent arthroscopic coracoclavicular ligament reconstructions using semitendinosus autografts. Constant and Simple Shoulder Test scores were determined before and 2 years after surgery as a part of standard clinical practice. General patient satisfaction with the outcome (poor, fair, or excellent) also was assessed. In addition, for purposes of routine clinical follow-up, the coracoclavicular distance was measured from the inferior cortex of the clavicle to the superior cortex of the coracoid using anteroposterior radiographs taken 2 years after surgery. Published by Elsevier Inc.Purpose To evaluate the clinical results following arthroscopic surgery in patients with anomaly of the anterior horn of the medial meniscus (AHMM) that was found unexpectedly during surgery and discuss whether resection is necessary in patients without anteromedial knee pain (AMKP). Methods Between May 2014 and April 2017, a total of 387 knee arthroscopies in 379 patients were performed. Among these, 11 knees in 11 patients showed an anomalous insertion of the AHMM (incidence, 2.8%), and all 11 patients were included in this study. For these 11 patients, medical records including preoperative diagnosis, arthroscopic findings, and pre- and postoperative clinical evaluations were analyzed. Results None of the patients complained of AMKP before arthroscopy. Two patients were diagnosed with lateral meniscus injury and the other 9 patients were diagnosed with medial meniscus injury. All anomalies of the AHMM were found incidentally during arthroscopic surgery. The anomaly formed a band-like structure arising from the anterior portion of the medial meniscus and was attached to the anterior aspect of the ACL and femoral intercondylar notch. All 11 patients underwent partial meniscectomy, but anomalies of the AHMM were not resected. One patient was excluded from clinical evaluation, as that patient required subsequent total knee arthroplasty due to osteoarthritis. For the other 10 patients, mean follow-up was 36.8 months (range, 26-61 months). Knee pain was relieved, and none developed postoperative AMKP. Mean Lysholm score improved significantly from 55.9 to 91.2 (P less then .001). Conclusions The incidence of the anomaly was 2.8% in our study. If the patient has no AMKP before arthroscopic surgery, anomaly of the AHMM is a silent lesion that does not warrant resection. Under the relaxation condition, the sound energy decay on the room boundary, generated by finite initial excitation, can be expanded as a summation of decay components, which are composed of real and/or complex conjugate decay modes. Each decay mode is a decaying and oscillating L-eigenfunction corresponding to an L-eigenvalue. The real part of the L-eigenvalue is the exponential decay rate, and the image part is the angular frequency of the oscillation. The reverberant sound field inside the room space has a similar decay structure to the boundary. As an example, the decay structure in a sphere is analyzed. The relaxation of the sound field is explained by the geometrical significance of the sound field decay.Listeners are able to classify talkers by regional dialect of their native language when provided with even short speech samples. In both experiments, listeners in the unmodified and monotonized conditions made more accurate groupings, reflecting their reliance on segmental properties for classifying regional variation. The maximum yield of anthocyanins was achieved by freezing the samples for 5.43 min in liquid nitrogen at the liquid-solid ratio of 24.071 mL/g, followed by ultrasonic thawing at 41.64 °C for 23.56 min. The yield and antioxidant effects of ABVS extracted using FUTE, ultrasound-assisted extraction (UAE) and freeze-thawing extraction (FTE) were compared in order to determine the overall efficacy of FUTE. In addition to the higher content, FUTE extracted ABVS showed greater ability to scavenge DPPH·, ABTS+ and superoxide anions, and inhibit lipid peroxidation compared to the ABVS extracted by UAE or FTE. The reducing power of the FUTE-derived ABVS was intermediate between that of the UAE and FTE samples. Materials and methods Inspired from web analytics, SCALPEL3 relies on distributed computing, data denormalization and columnar storage. It was compared to the existing SAS-Oracle SNDS infrastructure by performing several queries on a dataset containing a three years-long history of healthcare claims of 13.7 million patients. Results and discussion SCALPEL3 horizontal scalability allows handling large tasks quicker than the existing infrastructure while it has comparable performance when using only a few executors. SCALPEL3 provides a sharp interactive control of data processing through legible code, which helps to build studies with full reproducibility, leading to improved maintainability and audit of studies performed on LODs. The aim of this study is to compare the AD clinical outcomes between patients aged up to 80 years and those ≥80 years. Secondary outcomes included major complications, in-hospital length of stay (LOS), and need for surgical procedures. Results Patients ≥80 years with AD had different clinical presentation compared to younger patients they had less fever (21.4% vs. 35.2%; P less then 0.001) and abdominal pain (47.8% vs. 65.6%; P less then 0.001) rates, but a higher digestive tract bleeding (31.5% vs. 12.3%; p less then 0.001), and fatigue (12.7% vs. 7.1%; p = 0.004) rates. We focused on the role of thyroid tumor cell-fibroblast interaction and EVs as mediators of tumor-stroma interplay, in the promotion of thyroid tumor aggressiveness. Thyroid tumor (TPC-1, 8505c) or non-tumor thyroid cells (NThyOri) were co-cultured with human fibroblasts (Fb). Thyroid cell migration was investigated by the wound-healing assay and actin-network staining. Cell-CD147 expression was characterized by flow cytometry. EVs, obtained by ultracentrifugation of conditioned media (CMs), were characterized by transmission electron-microscopy and CD81 and CD147 expression. Metalloproteinases (MMPs) were evaluated by zymography in CMs. A migratory phenotype was triggered in thyroid tumor cells treated with CMs from Fb or from Fb-thyroid tumor cell co-cultures.

To simulate diaphyseal bone loading in individuals with lower limb amputations, virtual joints were defined during post-processing at 25, 50, and 75 % of residual limb length of both the femur and the tibia, representing six clinically relevanion of percutaneous OI patients.Objective This article introduces SCALPEL3 (Scalable Pipeline for Health Data), a scalable open-source framework for studies involving Large Observational Databases (LODs). It focuses on scalable medical concept extraction, easy interactive analysis, and helpers for data flow analysis to accelerate studies performed on LODs. Materials and methods Inspired from web analytics, SCALPEL3 relies on distributed computing, data denormalization and columnar storage. It was compared to the existing SAS-Oracle SNDS infrastructure by performing several queries on a dataset containing a three years-long history of healthcare claims of 13.7 million patients. Results and discussion SCALPEL3 horizontal scalability allows handling large tasks quicker than the existing infrastructure while it has comparable performance when using only a few executors. SCALPEL3 provides a sharp interactive control of data processing through legible code, which helps to build studies with full reproducibility, leading to improved maintainability and audit of studies performed on LODs. Conclusion SCALPEL3 makes studies based on SNDS much easier and more scalable than the existing framework [1]. It is now used at the agency collecting SNDS data, at the French Ministry of Health and soon at the National Health Data Hub in France [2].Objective Contrasting results are reported on the clinical course of acute diverticulitis (AD) in the geriatric population. The aim of this study is to compare the AD clinical outcomes between patients aged up to 80 years and those ≥80 years. The present study describes a protocol for a systematic review of the available evidence on the level of health literacy specific to breast and cervical cancer among Black African women globally. We will perform a systematic review of the available quantitative and qualitative studies. The search will include studies that describe the level of health literacy specific to breast and cervical cancer among Black African women. We will conduct a preliminary search on Google scholar to build the concepts for search terms, and a full search strategy using the identified concepts and keywords across four databases namely PubMed, SCOPUS, CINAHL and Web of Sciences. We will use Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) to schematically present the search strategy. We will use the standardized Joanna Briggs Institute quality appraisal and selection tool to recruit studies, and the data extraction tool to synthesise the information extracted from the recruited studies. We will be guided by socioecological theory and Indigenous epistemology to synthesise the non-quantifiable information thematically, and pool the quantitative information using meta-analysis, based on the availability of information.Intensive training and exhausting exercise can disrupt innate and acquired immunity. The flavanone hesperidin has shown immunomodulatory properties in physiological and some pathological conditions, and positive effects on exercise-induced oxidative stress. Nevertheless, it remains uncertain whether it also prevents exhausting exercise-induced immune alterations. The aim of this study was to establish the effect of oral hesperidin supplementation on the systemic immune system in rats following an intensive training and exhausting exercise. For this purpose, female Wistar rats were randomized into an intensive training group or a sedentary group. Intensive training was induced by running in a treadmill 5 days per week (including two exhausting tests) for five weeks. Throughout the training period, 200 mg/kg of hesperidin or vehicle was administered by oral gavage three times per week. At the end, blood, thymus, spleen and macrophages were collected before, immediately after and 24 h after an additional final exhaustion test. Hesperidin supplementation enhanced natural killer cell cytotoxicity and the proportion of phagocytic monocytes, attenuated the secretion of cytokines by stimulated macrophages, prevented the leukocytosis induced by exhaustion and increased the proportion of T helper cells in the thymus, blood and spleen. These putative monohydrate intermediates and their regioisomers were synthesized to identify the reaction products of assays of heterologously expressed enzymes. An enzyme involved in the synthesis of a phenylpropanoid volatile component was identified in perilla. Expression of this enzyme in Saccharomyces cerevisiae showed that it is a member of the cytochrome P450 family and catalyzes the introduction of a hydroxy group onto myristicin to form an intermediate of dillapiole. The enzyme had high sequence similarity to a CYP71D family enzyme, high regiospecificity, and low substrate specificity. This study may aid the elucidation of generally unexploited biosynthetic pathways of phenylpropanoid volatile components.

В результате комплексного клинико-инструментального и лабораторного обследования у пациентки, помимо синдрома гипервязкости крови, выявлены очаги ишемического поражения головного мозга (по данным магнитно-резонансной томографии), гипоперфузия глаза с выраженным дефицитом ретинального и хориоидального кровотока (по результатам допплеровских исследований), что может свидетельствовать о наличии глазного ишемического синдрома. Для оценки контрастной чувствительности применяли прибор Optec 6500. Степень выраженности световых феноменов и показатели качества зрения оценивали при помощи анкеты. Результаты. Некорригированная ОЗ (НКОЗ) вдаль, вблизи и на промежуточном расстоянии была выше в группе TFN в сравнении с аналогичным показателем в группе TRI по результатам измерения с использованием как децимальной шкалы, так и ИКП и системы LogMAR. В фотопических и мезопических условиях с засветом и без него на всех пространственных частотах функция контрастной чувствительности (ФКЧ) была выше после имплантации TFN в сравнении с аналогичным показателем в группе TRI. После имплантации TFN средний показатель VF-14 составил 97,2, а после имплантации TRI - 96,7. Процент очковой коррекции после операции был на 5,5% меньше в группе TFN в сравнении с данными группы TRI. Заключение. После имплантации обеих трифокальных ИОЛ отмечались высокие функциональные результаты ОЗ у пациентов на разных расстояниях с преимуществом таковых в группе TFN. Оценка значений ОЗ с использованием различных систем показала преимущество ИКП. После имплантации обеих ИОЛ ФКЧ имела типичный вид с максимальным показателем в области средних частот и снижением такового в области высоких частот с наилучшими результатами после имплантации TFN. Основной способ лечения макулярного разрыва - проведение витрэктомии с удалением внутренней пограничной мембраны (ВПМ). Однако в последнее время доказано негативное влияние пилинга на нейрофизиологические процессы в макулярной зоне. Цель исследования - предложить новую технику и оценить динамику анатомических и функциональных параметров после операции по поводу макулярного разрыва с сохранением ВПМ. Материал и методы. Изучены результаты лечения 52 пациентов (52 глаза) со сквозными макулярными разрывами 2-4-й стадии по классификации Гасса. Пациенты 1-й группы оперированы по стандартному методу после витрэктомии проводили круговой макулорексис, пациенты 2-й группы - по оригинальной методике с частичным сохранением ВПМ в фовеолярной зоне. Всем пациентам до и после операции выполняли стандартное офтальмологическое обследование. Оценку морфометрических данных осуществляли на основании ОКТ-исследования. Статистическую обработку результатов проводили при помощи программы IBM SPSS Statistics 23.
