  • Microplastics ingested by two bivalve species Perna viridis and Meretrix meretrix collected from three estuaries viz. Ariyankuppam, Panithittu, and Chunnambar in Pondicherry, India was analysed for the first time in this research. Nile Red dye was used for microplastic detection. A survey of 50 local families was conducted to determine the frequency and quantity in which they consume mussels/clams. On an average, the number of microplastics per gram of soft tissue (wet weight) is 0.18 ± 0.04, 1.84 ± 0.61, and 1.76 ± 0.48; and the number of microplastics per bivalve is 0.50 ± 0.11, 1.75 ± 0.35, and 4.80 ± 1.39 respectively for Ariyankuppam, Panithittu, and Chunnambar. 61.02% and 77.42% of the particles belonged to the size group of less then 100 μm in M. meretrix and P. viridis respectively. A moderate positive correlation of r (18) = 0.6985, p  less then  0.05 was calculated between bivalve weight and microplastic particles. An average person belonging to the local community is likely to ingest 3917.79 ± 144.71 microplastic particles per year through mussel consumption. Salt marshes in urban watersheds are prone to microplastics (MP) pollution due to their hydrological characteristics and exposure to urban runoff, but little is known about MP distributions in species from these habitats. In the current study, MP occurrence was determined in six benthic invertebrate species from salt marshes along the North Adriatic lagoons (Italy) and the Schelde estuary (Netherlands). The species represented different feeding modes and sediment localisation. 96% of the analysed specimens (330) did not contain any MP, which was consistent across different regions and sites. Suspension and facultative deposit-feeding bivalves exhibited a lower MP occurrence (0.5-3%) relative to omnivores (95%) but contained a **** more variable distribution of MP sizes, shapes and polymers. The study provides indications that MP physicochemical properties and species' ecological traits could all influence MP exposure, uptake and retention in benthic organisms inhabiting European salt marsh ecosystems. Rivers are the main sources of nutrients to coastal zones. Therefore, the effects of water quality degradation on octocoral assemblages and their relationship with microbiological and physical-chemical variables were determined in reefs close to river basins in the northwestern region of Cuba. The evaluations were carried out in 1 m2 frames at a depth of 10 m in 13 fore reefs. The highest concentrations of the microbiological variables and hydrochemicals and the lowest horizontal visibility in the water column were detected in the reefs near the river basins. The lowest richness, diversity and octocoral density were related to higher concentrations of fecal coliform, total coliform and fecal streptococcal bacteria, sediment accumulation on the bottom and lower visibility. The structure of the octocoral assemblages varied significantly between reefs due to fecal organic pollution. Water quality deterioration in the reefs near the river basins has negatively affected the octocoral assemblages. Within Southern California, east Pacific green sea turtles (Chelonia mydas) forage year-round, taking advantage of diverse food resources, including seagrass, marine algae, and invertebrates. Assessing persistent organic pollutants (POP) in green turtle aggregations in the Seal Beach National Wildlife Refuge (SBNWR, n = 17) and San Diego Bay (SDB, n = 25) can help quantify contamination risks for these populations. Blood plasma was analyzed for polychlorinated biphenyls (PCBs), organochlorinated pesticides (OCPs), and polybrominated diphenyl ethers (PBDEs). PCBs and body size explained **** of the separation of turtles by foraging aggregation in a principal component analysis. Turtles from SDB had significantly (p  less then  0.001) higher total PCBs than SBNWR turtles. Most PCBs detected in turtles were non-dioxin-like PCB congeners (153, 138, 99) that are associated with neurotoxicity. In the present study, the toxic effects of TPTCL on physiology and ultrastructure of cultivated sporophytes of Undaria pinnatifida were investigated under different TPTCL concentrations ranging from 0 to 100 μg L-1. Significant negative effects of increased TPTCL concentration were detected in the relative growth rates, survival percentages and chlorophyll a contents of young and adult sporophytes. Low TPTCL concentrations could significantly stimulate the activities of enzymes related to nitrogen metabolism. The chloroplast, mitochondria and nucleus inside cells were greatly damaged by TPTCL. Meanwhile, significant increases of electron dense deposits and physodes were found. Additionally, young sporophytes exhibited greater tolerance to TPTCL stress than adult sporophytes. This is the first study of heavy metals (As, Zn, Cd, Ni, Fe, Mn, Cu) contamination of microplastics on sandy beaches in Hong Kong. Three study sites are located in the eastern waters (Pak Lap Wan, Stanley Bay, Tung Lung Chau) and the other three in the western waters (Tai Pai Tsui, Ha Pak Nai, Shui Hau Wan). The three most abundant types of microplastics were polyethylene (42.2%), polypropylene (23.3%) and polystyrene (19.5%). The median concentration of Fe (302 mg kg-1) was the highest and followed by Zn (19.6 mg kg-1) and Mn (18.6 mg kg-1). Very low concentrations of Cu (0.89 mg kg-1), Ni (0.15 mg kg-1), As ( less then LOD) and Cd ( less then LOD) were measured. The western sites have significantly higher concentrations of Ni, Fe, Mn and Cu than the eastern sites, indicating that Pearl River was likely to be a major source of heavy metals on microplastics. In view of a continual increase in the abundance of microplastics in the marine environment and its potential impacts on marine organisms, immediate actions should be taken in establishing long term monitoring programs for heavy metals associated with microplastics.

  • RESULTS Seizure frequency (median 2.51/month [0-6.67] versus 2.67/month [0-10.45]; P = .02) and seizure-day frequency were significantly (1.68/month [0-5.60] versus 1.99/month [0-7.42], P = .01) lower when dogs were fed MCT-DS in comparison with the control-DS. Two dogs were free of seizures, 3 had ≥50% and 12 had less then 50% reductions in seizure frequency, and 11 dogs showed no change or an increase in seizure frequency. RESULTS According to the study compliance categories, 45.6% of the participants reported 'insufficient compliance,' 24.8% 'weak compliance' and 29.6% 'sufficient compliance.' Nurses with more experience and previous education on ventilator-associated pneumonia reported higher compliance scores than their counterparts with less experience and no previous education. Nurses reported several barriers that limited their own compliance, such as lack of education, lack of policies and protocols, lack of resources and the shortage of staff. CONCLUSION The compliance of nursing staff with the prevention guidelines was poor. Applying tailored educational programs may help improve their level of compliance. The dependency on trafficking abilities is even higher for B cells, one of the arms of the adaptive immune system, considering that they must encounter specific antigens for their clonal receptor in the enormous tissue volume of the secondary lymphoid organs (spleen, lymph nodes, Peyer patches). The regulated interplay between cell motility and cell adhesion allows B cells to reach distinct lymphoid tissues and, within them, to explore the stromal cell networks where antigen might be exposed. In this meeting-invited review, I summarize the current knowledge on the molecular cues and mechanisms that shapes B cell dynamics at the initial phase of the humoral immune response, including homeostatic chemoattractants and innate/inflammatory stimuli. BACKGROUND Complete tooth loss is the leading cause of disability associated with oral conditions in 19 of 21 global regions, and it is among the most difficult treatment challenges in dentistry. METHODS Interviews with 18 participants were analysed inductively using interpretive description and qualitative synthesis to explain their beliefs and experiences relating to tooth loss. RESULTS Theoretical constructs from the five dominant theories constituting the metatheory explained the beliefs, concerns and experiences of the participants. For example, both before and after tooth loss they engaged in adaptive behaviours according to developmental regulation theory; implemented management strategies according to emotion theory, social cognitive theory, and resource theory; sought help from friends and dental professionals according to communication theory; and modified their physical and social activities according to social cognitive theory. CONCLUSION A metatheory synthesised from five dominant theories addressing communication, personal background, emotions, resources and social awareness, offers a comprehensive and plausible explanation of how people respond psychologically and socially to the loss of their teeth, and expands the scope of information needed to help manage their loss and subsequent treatment. © 2020 Gerodontology Association and John Wiley & Sons Ltd.The majority of acute myeloid leukemia (AML) patients will respond to standard chemotherapy, however, resistance is a prevalent problem contributing to incomplete responses, refractory disease, and ultimately patient death. Therefore, choosing more sensitive and effective chemotherapy regimens is of key clinical importance. In order to explore this issue, we investigated and optimized PharmaFlow, an automated flow cytometry method for evaluating the sensitivity of leukemia cells to multiple chemotherapeutic drugs ex vivo. We examined bone marrow samples from 38 Chinese AML patients and incubated them for 48 or 72 h with a panel of 7 single drugs and 6 combinations with cytarabine at different concentrations. Leukemic cell depletion was assessed by PharmaFlow and drug response parameter, called PharmaFlow score, was estimated using population pharmacodynamic models. We identified that most chemotherapeutic drugs and combinations could effectively eliminate pathological cells ex vivo. Estimated drug activities strongly correlated with the patients' duration to achieve clinical remission and PharmaFlow chemosensitivity measured ex vivo was highly predictive of the clinical outcome after chemotherapy.

  • Bone union rate was higher with bilateral than unilateral TAS fixation (P less then 0.05). There were no complications regarding screw malposition. Deep wound infection was observed in 1 case in the C-group. CONCLUSIONS O-arm use improved TAS fixation accuracy. Blood loss was equivalent between the groups. O-arm-assisted TAS fixation did not prolong operative time despite the time required for setting and scanning. The O-arm is safe and useful for TAS fixation in atlantoaxial instability. BACKGROUND Primary dedifferentiated liposarcoma of the orbit is a very rare condition. CASE DESCRIPTION A 41-year-old man presented with a history of right ophthalmalgia with eye movement and exophthalmos that had developed two months earlier. Neuroimages revealed a right orbital mass located above the superior rectus muscle. The tumor resection through the right frontal craniotomy was performed. Histopathological findings showed the dedifferentiated liposarcoma. CONCLUSIONS Primary orbital dedifferentiated liposarcomas remain challenging for their diagnosis and standard treatment. Clinicians should keep in mind the possibility of liposarcoma with orbital tumors. OBJECTIVE This study proposes to analyze the sellar floor morphology of patients with pituitary adenoma, and to find a simple yet reliable method to determine the location of bone window opening. METHOD Clinical information of 144 consecutively admitted patients was retrospectively analyzed. Enhanced MR image of the mid-sagittal plane was selected as the reference for classifying the sellar floor. Intra-operative tumor locating, extent of tumor resection, and follow-up results were analyzed for different types of sellar floor. For all patients, quick intra-operative locating of the sellar floor opening was achieved. 104 (72.2%) patients had total tumor resection, 28(19.40%) had subtotal tumor resection, and four (2.8%) had partial tumor resection. 20 (13.9%) patients experienced cerebrospinal fluid (CSF) leak, and there was no significant difference in CSF leak rate between groups. RESULTS We found consensus on 7 cases for posterior approach due to cervicothoracic deformity and of 15 patients within our sCD database that had cervicothoracic deformity, 13/15 had posterior approach. There was consensus on 2 cases for antero-posterior approach due to local kyphosis and of 25 patients that had local kyphosis, 18/25 had an anterior approach. In 4 cases there was consensus of UIV of C2 and of 35 cases that had posterior fusion >6 levels, 20/35 had a UIV of C2. In 3 cases there was consensus of a LIV below a previously fused spine. Of 36 patients that had a fusion of T6 or higher, 34 had a LIV below the previous UIV. In 6 cases there was consensus against an osteotomy due to cervical spine flexibility and 9/12 patients that had an osteotomy in our database had no flexibility on dynamic radiographs. CONCLUSION We outline here an algorithm for deciding approach, UIV, LIV and whether to do an osteotomy for patients with sCD based on consensus recommendations amongst spine surgeons. At our institution, every geographic region was represented in our most recent decade of experience. International visitor profiles appear to be associated with particular geographic patterns depending on their status as either a medical student or graduate.
    Bone union rate was higher with bilateral than unilateral TAS fixation (P less then 0.05). There were no complications regarding screw malposition. Deep wound infection was observed in 1 case in the C-group. CONCLUSIONS O-arm use improved TAS fixation accuracy. Blood loss was equivalent between the groups. O-arm-assisted TAS fixation did not prolong operative time despite the time required for setting and scanning. The O-arm is safe and useful for TAS fixation in atlantoaxial instability. BACKGROUND Primary dedifferentiated liposarcoma of the orbit is a very rare condition. CASE DESCRIPTION A 41-year-old man presented with a history of right ophthalmalgia with eye movement and exophthalmos that had developed two months earlier. Neuroimages revealed a right orbital mass located above the superior rectus muscle. The tumor resection through the right frontal craniotomy was performed. Histopathological findings showed the dedifferentiated liposarcoma. CONCLUSIONS Primary orbital dedifferentiated liposarcomas remain challenging for their diagnosis and standard treatment. Clinicians should keep in mind the possibility of liposarcoma with orbital tumors. OBJECTIVE This study proposes to analyze the sellar floor morphology of patients with pituitary adenoma, and to find a simple yet reliable method to determine the location of bone window opening. METHOD Clinical information of 144 consecutively admitted patients was retrospectively analyzed. Enhanced MR image of the mid-sagittal plane was selected as the reference for classifying the sellar floor. Intra-operative tumor locating, extent of tumor resection, and follow-up results were analyzed for different types of sellar floor.   • Bone morphogenetic proteins (BMPs), have been shown to enhance the osteogenic differentiation of mesenchymal cells (MCs) and to promote bone formation. BMP6 is known to play an important role in the process of MCs towards osteogenic differentiation by virtue of their osteoinductive and cell type specific proliferative activity. However, the molecular mechanism relate to BMP6 osteoinductive activity is still unclear and continues to warrant further investigation. Msx2 is a member of the homeobox gene family of transcription factors and promotes calcification. Hence, we wondered if it might also play a role in BMP6-induced osteogenesis. In this study, two mouse mesenchymal cell lines were treated with BMP6, adenovirus-Msx2 (Ad-Msx2) or adenovirus-siMsx2 (Ad-siMsx2). Based on the results of mRNA and protein expression, it was indicated that BMP6 could enhance the expression of Msx2 and activate the phosphorylation of Smad 1/5/8, p38 and ERK1/2. Being transfected by Ad-Msx2, the BMP6-induced activation of phosphorylation was significantly promoted. On the contrary, two cell lines transfected by Ad-siMsx2 presented an inhibited expression of three phosphorylated proteins even after being induced by BMP6. The evaluation of ALP, OPN, OC and calcium deposits revealed the osteogenic results those were corresponding to the results of mRNA and protein. To better understand the pathomechanism of GDD we have generated via Crispr/CAS9 gene editing a mouse model harboring the murine equivalent (Ano5 p.T491F) of a GDD-causing ANO5 mutation identified in a previously reported patient. Skeletal phenotyping by contact radiography, μCT and undecalcified histomorphometry was performed in male ****, heterozygous and homozygous for the mutation, at the ages of 12 and 24 weeks. These **** did not display alterations of skeletal microarchitecture or mandible morphology. The results were confirmed in female **** and animals derived from a second, independent clone. Finally, no skeletal phenotype was observed in **** lacking ~40% of their Ano5 gene due to a frameshift mutation. BMD was assessed at the lumbar spine, total hip, femoral neck, and distal and ultradistal radius via dual energy x-ray absorptiometry (DXA). Circulating sclerostin was assessed via commercial ELISAs. Univariate and multivariate linear regression modeling tested associations between sclerostin and BMD after adjusting for a variety of BMD-modifying variables. Results HIV-seropositive women had significantly reduced BMD at all skeletal sites compared to HIV-seronegative women. There was no difference in sclerostin levels according to HIV-serostatus (0.25 vs 0.27 ng/mL in HIV-seronegative and HIV-seropositive, respectively, p = 0.71). Circulating sclerostin was positively associated with BMD at all sites in both univariate and multivariate models adjusting for HIV status, age, BMI, and race, although the coefficients of association were attenuated in HIV-seropositive women. The positive association between sclerostin and BMD among seropositive women remained statistically significant after adjusting for ART or tenofovir disoproxil fumarate (TDF) use. Conclusions The current study suggests that circulating sclerostin is a biomarker for bone mass for both HIV seronegative and seropositive women using and not using ART. We describe the case of a female who presented with acute encephalopathy, hypercalcemia, and new-onset seizure. An extensive hypercalcemia workup and ruling out of other causes led to the diagnosis of MAS from excessive intake of calcium carbonate. Brain magnetic resonance imaging revealed signal abnormalities in the occipital and posterior parietal lobes that were indicative of posterior reversible encephalopathy syndrome. The patient's encephalopathy resolved after treatment of her hypercalcemia with fluid resuscitation and cessation of her calcium supplements. We present our case to highlight this unusual presentation of MAS, challenges in diagnosis, and briefly discuss the pathophysiology underlying hypercalcemia-induced encephalopathy.Tumour-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by a fibroblast growth-factor-23 (FGF-23)-secreting phosphaturic mesenchymal tumour (PMT) and is characterised by hypophosphataemic osteomalacia.   • Embedded and mobile smart devices face problems related to limited computing power and excessive power consumption. To address these problems, we propose Mixed YOLOv3-LITE, a lightweight real-time object detection network that can be used with non-graphics processing unit (GPU) and mobile devices. Based on YOLO-LITE as the backbone network, Mixed YOLOv3-LITE supplements residual block (ResBlocks) and parallel high-to-low resolution subnetworks, fully utilizes shallow network characteristics while increasing network depth, and uses a "shallow and narrow" convolution layer to build a detector, thereby achieving an optimal balance between detection precision and speed when used with non-GPU based computers and portable terminal devices. The experimental results obtained in this study reveal that the size of the proposed Mixed YOLOv3-LITE network model is 20.5 MB, which is 91.70%, 38.07%, and 74.25% smaller than YOLOv3, tiny-YOLOv3, and SlimYOLOv3-spp3-50, respectively. The mean average precision (mAP) achieved using the PASCAL VOC 2007 dataset is 48.25%, which is 14.48% higher than that of YOLO-LITE. When the VisDrone 2018-Det dataset is used, the mAP achieved with the Mixed YOLOv3-LITE network model is 28.50%, which is 18.50% and 2.70% higher than tiny-YOLOv3 and SlimYOLOv3-spp3-50, respectively. The results prove that Mixed YOLOv3-LITE can achieve higher efficiency and better performance on mobile terminals and other devices.The control of the main environmental factors that influence the quality of perishable products is one of the main challenges of the food industry. Temperature is the main factor affecting quality, but other factors like relative humidity and gas concentrations (mainly C2H4, O2 and CO2) also play an important role in maintaining the postharvest quality of horticultural products. For this reason, monitoring such environmental factors is a key procedure to assure quality throughout shelf life and evaluate losses. Therefore, in order to estimate the quality losses that a perishable product can suffer during storage and transportation, a real-time monitoring system has been developed. This system can be used in all post-harvest steps thanks to its Wi-Fi wireless communication architecture. Several laboratory trials were conducted, using lettuce as a model, to determine quality-rating scales during shelf life under different storage temperature conditions. As a result, a multiple non-linear regression (MNLR) model is proposed relating the temperature and the maximum shelf life. This proposed model would allow to predict the days the commodities will reduce their theoretical shelf-life when an improper temperature during storage or in-transit occurs. The system, developed as a sensor-based tool, has been tested during several land transportation trips around Europe.We compared nutrient compositions of honey bee (Apis mellifera) drones of different developmental stages from two different populations-the Italian honey bee reared in Korea and Buckfast bees from Denmark. Analyses included amino acid, fatty acid, and mineral content as well as evaluations of antioxidant properties and haemolysis activities. The compositions of total amino acids, and thus protein content of the insects, increased with development. A similar trend was observed for minerals presumably due to the consumption of food in the adult stage. In contrast, total fatty acid amounts decreased with development. Altogether, seventeen amino acids, including all the essential ones, except tryptophan, were determined. Saturated fatty acids dominated over monounsaturated fatty acids in the pupae, but the reverse held true for the adults. Drones were found to be rich in minerals and the particularly high iron as well as K/Na ratio was indicative of the nutritional value of these insects. Among the three developmental stages, adult Buckfast drones exhibited the highest antioxidant activity. Bearing in mind the overall high nutritional value, i.e., high amino acids, minerals and less fatty acids, late pupae and adult drones can be useful for human consumption while the larvae or early pupal stage can be recommended as feed.     Embedded and mobile smart devices face problems related to limited computing power and excessive power consumption. To address these problems, we propose Mixed YOLOv3-LITE, a lightweight real-time object detection network that can be used with non-graphics processing unit (GPU) and mobile devices. Based on YOLO-LITE as the backbone network, Mixed YOLOv3-LITE supplements residual block (ResBlocks) and parallel high-to-low resolution subnetworks, fully utilizes shallow network characteristics while increasing network depth, and uses a "shallow and narrow" convolution layer to build a detector, thereby achieving an optimal balance between detection precision and speed when used with non-GPU based computers and portable terminal devices. The experimental results obtained in this study reveal that the size of the proposed Mixed YOLOv3-LITE network model is 20.5 MB, which is 91.70%, 38.07%, and 74.25% smaller than YOLOv3, tiny-YOLOv3, and SlimYOLOv3-spp3-50, respectively. The mean average precision (mAP) achieved using the PASCAL VOC 2007 dataset is 48.25%, which is 14.48% higher than that of YOLO-LITE. When the VisDrone 2018-Det dataset is used, the mAP achieved with the Mixed YOLOv3-LITE network model is 28.50%, which is 18.50% and 2.70% higher than tiny-YOLOv3 and SlimYOLOv3-spp3-50, respectively. The results prove that Mixed YOLOv3-LITE can achieve higher efficiency and better performance on mobile terminals and other devices.The control of the main environmental factors that influence the quality of perishable products is one of the main challenges of the food industry. Temperature is the main factor affecting quality, but other factors like relative humidity and gas concentrations (mainly C2H4, O2 and CO2) also play an important role in maintaining the postharvest quality of horticultural products. For this reason, monitoring such environmental factors is a key procedure to assure quality throughout shelf life and evaluate losses. Therefore, in order to estimate the quality losses that a perishable product can suffer during storage and transportation, a real-time monitoring system has been developed. This system can be used in all post-harvest steps thanks to its Wi-Fi wireless communication architecture. Several laboratory trials were conducted, using lettuce as a model, to determine quality-rating scales during shelf life under different storage temperature conditions. As a result, a multiple non-linear regression (MNLR) model is proposed relating the temperature and the maximum shelf life. This proposed model would allow to predict the days the commodities will reduce their theoretical shelf-life when an improper temperature during storage or in-transit occurs. The compositions of total amino acids, and thus protein content of the insects, increased with development. A similar trend was observed for minerals presumably due to the consumption of food in the adult stage. In contrast, total fatty acid amounts decreased with development. Altogether, seventeen amino acids, including all the essential ones, except tryptophan, were determined. Saturated fatty acids dominated over monounsaturated fatty acids in the pupae, but the reverse held true for the adults. Drones were found to be rich in minerals and the particularly high iron as well as K/Na ratio was indicative of the nutritional value of these insects. In morphology, plants from Miyako-jima Island bore a close resemblance to the other plants of S. formosanum, but differed in being perennial, polycarpic, and having lateral axillary branches. Molecular analyses based on ITS of nrDNA and six regions of cpDNA sequencing indicated that the Miyako-jima plants formed a distinct subclade. This subclade was part of a polytomy with three other subclades comprising nine taxa endemic to Taiwan and S. formosanum from other areas, including the type locality. Therefore, we propose and describe the Miyako-jima plants as a new subspecies, Sedum formosanum subsp. miyakojimense.Based on morphological study and corroborated by unpublished molecular phylogenetic analyses, five grass species of high-mountain grasslands in Mexico, Central and South America, Agrostis bacillata, A. exserta, A. liebmannii, A. rosei, and A. trichodes, are transferred to Podagrostis and bring the number of species of this genus recognized in the New World to ten. The name Apera liebmannii is lectotypified and epitypified. We provide an updated genus description for Podagrostis, and updated species descriptions, images, and notes on the new combinations. Objective To assess the use of Xpert for accurate diagnosis, timely initiation, and rational use of anti-TB treatment among childhood TB. Methods In 2016, the hospital facilitated the installation of the Xpert machine. We reviewed data trends over four consecutive years; two years before the arrival of the machine and two years following the implementation of Xpert. Data were extracted retrospectively from electronically stored databases and medical records and entered to SPSS 21 for analysis. Results In the pre-intervention period (2014-2015), 404 cases of children presenting with symptoms or signs suggestive of TB ("presumptive TB") were evaluated using AFB microscopy. A total of 254 (62.8%) TB diagfirmed TB cases, reduced unnecessary anti-TB prescription, and shortened the time taken to start TB treatment.Small-cell carcinoma of the uterine cervix is a rare and aggressive tumor, and the prognosis is poor compared with those of squamous cell carcinoma and adenocarcinoma of the uterine cervix, even when discovered at an earlier stage. We treated a patient with progressive small-cell carcinoma of the uterine cervix that metastasized to the cervical spine. The patient, a 73-year-old woman, presented with the symptom of numbness in her limbs. As she had difficulty moving her limbs (ie, quadriplegia), she was carried to an emergency room. A metastatic cervical spine tumor from the uterine cervical cancer was revealed by a computed tomography scan, and the patient was then transferred to our hospital's neurosurgery department for treatment. We performed a resection of the cervical spine tumor and fixation of the spinal bone. Because the patient's performance status was 4 and she remained bedridden 24 h/day, we could not perform systemic chemotherapy. We thus provided palliative care, including palliative radiotherapy, pain control, and rehabilitation to improve her limbs' functioning. The patient died of the uterine cancer within approx. 6 months after the initiation of treatment. There is no established treatment for small-cell carcinoma as a gynecological lesion. For patients with progressive uterine cancer, the optimal treatments, including palliative care, must be determined.The increasing indications for allogeneic stem-cell transplant in patients with hematologic malignancies and non-malignant diseases combined with improved clinical outcomes have contributed to increase the number of long-term survivors. However, survivors are at increased risk of developing a unique set of complications and late effects, besides graft-versus-host disease and disease relapse. In this setting, the management capacity of a single health-care provider can easily be overwhelmed. Thus, to provide appropriate survivorship care, a multidisciplinary approach for the long-term follow-up is essential. This review aims at summarizing the most relevant information that a health-care provider should know to establish a follow-up care plan, in the light of individual exposures and risk factors, that includes all organ systems and considers the psychological burden of these patients.

  • OBJECTIVES To determine the frequency of advance directives or directives disclosed by healthcare agents and their influence on decisions to withdraw/withhold life-sustaining care in neurocritically ill adults. DATA SOURCES PubMed, Embase, and Cochrane databases. STUDY SELECTION Screening was performed using predefined search terms to identify studies describing directives of neurocritically ill patients from 2000 to 2019. The review was registered prior to the screening process (International Prospective Register of Systematic Reviews [PROSPERO]-Identification number 149185). DATA EXTRACTION Data were collected using standardized forms. Primary outcomes were the frequency of directives and associated withholding/withdrawal of life-sustaining care. DATA SYNTHESIS Out of 721 articles, 25 studies were included representing 35,717 patients. The number of studies and cohort sizes increased over time. A median of 39% (interquartile range, 14-72%) of patients had directives and/or healthcare agents. The presence ofted number of patients having directives is worrisome and studies aiming to increase the use and translation of directives are scarce. Efforts need to be made to increase the perception, use, and translation of directives of the neurocritically ill.OBJECTIVES Lateral displacement and impaired cerebral autoregulation are associated with worse outcomes following acute brain injury, but their effect on long-term clinical outcomes remains unclear. This review evaluates surgical practices and equipment choices during endoscopic hysterectomy, highlighting opportunities for the gynecologic surgeon to reduce costs and maximize surgical efficiency. RECENT FINDINGS There are opportunities to economize at every step of the endoscopic hysterectomy. When surgeons are provided education about instrumentation costs, the cost of hysterectomy has been shown to decrease. Colpotomy has been found to be the rate-limiting step in laparoscopic hysterectomy; use of a uterine manipulator likely saves time and money. When evaluating the economic impact of route of surgery, the cost differential between laparoscopic and robotic-assisted hysterectomy has decreased. Robotic-assisted hysterectomy may be more cost-effective in some cases, such as for larger uteri. From a systems-level perspective, dedicating a specific operating room team to the gynecology service can decrease operative time. SUMMARY The gynecologic surgeon is best equipped to control surgery-related costs by making choices that improve surgical efficiency and decrease operating room time. If a costlier piece of equipment leads to a more efficient case, the choice may be more cost-effective. There are multiple systems-level changes that can be implemented to decrease surgery-related costs.PURPOSE Transfusion is associated with increased perioperative morbidity and mortality in patients undergoing total knee arthroplasty (TKA). Patient blood management (PBM) is an evidence-based approach to maintain blood mass via haemoglobin maintenance, haemostasis optimisation, and blood loss minimisation. The aim of the present study was to assess the effectiveness of a multimodal PBM approach in our centre. METHODS This was a single-centre retrospective study of patients who underwent primary TKA in Queen Mary Hospital in Hong Kong in 2013 or 2018, using data from the Clinical Data Analysis and Reporting System and a local joint registry database. Patient demographics, preoperative haemoglobin, length of stay, readmission, mean units of transfusion, postoperative prosthetic joint infection, and mortality data were compared between groups. RESULTS In total, 262 and 215 patients underwent primary TKA in 2013 and 2018, respectively. The mean transfusion rate significantly decreased after PBM implementation (2013 31.3%; 2018 1.9%, P less then 0.001); length of stay after TKA also significantly decreased (2013 14.49±8.10 days; 2018 8.77±10.14 days, P less then 0.001). However, there were no statistically significant differences in readmission, early prosthetic joint infection, or 90-day mortality rates between the two groups. CONCLUSION Our PBM programme effectively reduced the allogeneic blood transfusion rate in patients undergoing TKA in our institution. Thus, PBM should be considered in current TKA protocols to reduce rates of transfusions and related complications.Peripheral sources of individual variation in taste intensity perception have been well-described. Here we used functional magnetic resonance imaging in healthy human participants (20 women, 8 men) to evaluate the hypothesis that the amygdala exerts an inhibitory influence that affects the "gain" of the gustatory system during tasting. Consistent with the existence of a central gain mechanism (CGM), we found that central amygdala response was correlated with mean intensity ratings across multiple tastants. In addition, psychophysiological and dynamic causal modeling analyses revealed that the connection strength between inhibitory outputs from amygdala to medial dorsal and ventral posterior medial thalamus predicted individual differences in responsiveness to taste stimulation. These results imply that inhibitory inputs from the amygdala to the thalamus act as a CGM that influences taste intensity perception.Significance StatementWhether central circuits contribute to individual variation in taste intensity perception is unknown.

We assessed the relationship between lateral displacement, disturbances to cerebral autoregulation, and clinical outcomes in acutely comatose patients. This review evaluates surgical practices and equipment choices during endoscopic hysterectomy, highlighting opportunities for the gynecologic surgeon to reduce costs and maximize surgical efficiency. RECENT FINDINGS There are opportunities to economize at every step of the endoscopic hysterectomy. When surgeons are provided education about instrumentation costs, the cost of hysterectomy has been shown to decrease. Colpotomy has been found to be the rate-limiting step in laparoscopic hysterectomy; use of a uterine manipulator likely saves time and money. When evaluating the economic impact of route of surgery, the cost differential between laparoscopic and robotic-assisted hysterectomy has decreased. Robotic-assisted hysterectomy may be more cost-effective in some cases, such as for larger uteri. From a systems-level perspective, dedicating a specific operating room team to the gynecology service can decrease operative time. SUMMARY The gynecologic surgeon is best equipped to control surgery-related costs by making choices that improve surgical efficiency and decrease operating room time. If a costlier piece of equipment leads to a more efficient case, the choice may be more cost-effective. There are multiple systems-level changes that can be implemented to decrease surgery-related costs.PURPOSE Transfusion is associated with increased perioperative morbidity and mortality in patients undergoing total knee arthroplasty (TKA). Patient blood management (PBM) is an evidence-based approach to maintain blood mass via haemoglobin maintenance, haemostasis optimisation, and blood loss minimisation. The aim of the present study was to assess the effectiveness of a multimodal PBM approach in our centre. METHODS This was a single-centre retrospective study of patients who underwent primary TKA in Queen Mary Hospital in Hong Kong in 2013 or 2018, using data from the Clinical Data Analysis and Reporting System and a local joint registry database. Patient demographics, preoperative haemoglobin, length of stay, readmission, mean units of transfusion, postoperative prosthetic joint infection, and mortality data were compared between groups. RESULTS In total, 262 and 215 patients underwent primary TKA in 2013 and 2018, respectively. The mean transfusion rate significantly decreased after PBM implementation (2013 31.3%; 2018 1.9%, P less then 0.001); length of stay after TKA also significantly decreased (2013 14.49±8.10 days; 2018 8.77±10.14 days, P less then 0.001). However, there were no statistically significant differences in readmission, early prosthetic joint infection, or 90-day mortality rates between the two groups. CONCLUSION Our PBM programme effectively reduced the allogeneic blood transfusion rate in patients undergoing TKA in our institution. Thus, PBM should be considered in current TKA protocols to reduce rates of transfusions and related complications.Peripheral sources of individual variation in taste intensity perception have been well-described. Here we used functional magnetic resonance imaging in healthy human participants (20 women, 8 men) to evaluate the hypothesis that the amygdala exerts an inhibitory influence that affects the "gain" of the gustatory system during tasting. Consistent with the existence of a central gain mechanism (CGM), we found that central amygdala response was correlated with mean intensity ratings across multiple tastants. In addition, psychophysiological and dynamic causal modeling analyses revealed that the connection strength between inhibitory outputs from amygdala to medial dorsal and ventral posterior medial thalamus predicted individual differences in responsiveness to taste stimulation. These results imply that inhibitory inputs from the amygdala to the thalamus act as a CGM that influences taste intensity perception.Significance StatementWhether central circuits contribute to individual variation in taste intensity perception is unknown.

  • Objective To understand the clinical characteristics, prognoses, and prognostic factors of vegetative state (VS) and minimal consciousness state (MCS) patients hospitalized for comprehensive rehabilitation. Materials and methods The data of VS and MCS patients admitted in the past six years were retrospectively analyzed. Age of onset, etiology, course of disease, complications, and retention of tracheal intubation, gastric tube, and urethral catheter were recorded. All patients were followed for one year after onset, and their Glasgow Outcome Scale (GOS) scores were assessed. Long-term prognostic factors were analyzed using logistic regression. Results The proportion of patients with traumatic brain injuries in the MCS group was higher than in the VS group (p0.05). Conclusions Infection, spasticity, and epilepsy are common complications in both VS and MCS patients. Over a 12-month period, 174 patients (70 sepsis patients, 69 sepsis with AKI and 35 controls) were enrolled. Blood and urinary specimens were collected at admission as soon as possible (within 24 hours) and KIM-1 and NGAL levels were tested. Results Levels of uKIM-1, uNGAL, sNGAL were significantly higher in the sepsis patients who developed AKI compared to those sepsis with no-AKI (0.88 ng/ml (0.37, 2.14) vs. 1.21 ng/ml (0.67, 3.26) p=0.003, 63.54 ng/ml (21.66, 125.45) vs. 249.85 ng/ml (86.60, 585.97) p less then 0.001, and 108.08 ng/ml (67.74, 212.22) vs. 200.01 ng/ml (102.76, 300.77) p=0.001, respectively). sKIM-1 also had significant differences between the two groups (83.98 pg/ml (54.00,147.08) vs. 193.41 pg/ml (106.90, 430.60) p less then 0.001). The four biomarkers (uKIM-1, sKIM-1, uNGAL, sNGAL) all could be predictive for AKI, and the areas under the receiver operating characteristic curves (AUROC) were 0.607, 0.754, 0.768, 0.658, respectively. The uNGAL was an independent risk factor for septic AKI, and the AUROC was 0.768 (95% CI 0.689 to 0.835). The uNGAL and sNGAL were related to the prognosis of sepsis. Diarrhea was evaluated by the frequency and consistency of stools (Bristol Stool form scale). (3) Results After 8 weeks of OEN protocol, the nutritional status improved in 48.3% of patients, with an increased proportion of patients at risk of malnourishment (+27.3%) at the expense of a decrease of moderately (-19.9%) and severely (-7.3%) malnourished patients (p less then 0.001). Serum albumin and BMI significantly increased after 8 weeks of OEN treatment (p less then 0.005). OEN showed a 71.1% effectiveness in the improvement of stool consistency. The mean number of stools per day significantly decreased from baseline (4.17 stools/day) to week 8 (1.42 stools/day; p = 0.0041). The nutritional status significantly improved even in those patients with persistent diarrhea. (4) Conclusion The proposed OEN protocol seemed to be effective in improving the nutritional status, frequency and consistency of stools in patients with oncology treatment-related diarrhea even in persistent cases.Preserving skeletal muscle mass and functional capacity is essential for healthy ageing. Mechanisms underlying disuse and/or inactivity-related muscle deterioration in the older adults, whilst multifaceted, ultimately manifest in an imbalance between rates of muscle protein synthesis and breakdown, resulting in net muscle loss. To date, the most potent intervention to mitigate disuse-induced muscle deterioration is mechanical loading in the form of resistance exercise. However, the feasibility of older individuals performing resistance exercise during disuse and inactivity has been questioned, particularly as illness and injury may affect adherence and safety, as well as accessibility to appropriate equipment and physical therapists. Our objective was to determine whether supplementing individual HMOs to a weanling diet alters growth and gut health in rats. Healthy three-week-old Sprague Dawley rat pups were randomized to control, 2'-O-fucosyllactose (2'FL)- and 3'sialyllactose (3'SL)-fortified diets alone or in combination at physiological doses for eight weeks. Body composition, intestinal permeability, serum cytokines, fecal microbiota composition, and messenger RNA (mRNA) expression in the gastrointestinal tract were assessed. Males fed a control diet were 10% heavier and displayed elevated interleukin (IL-18) (p = 0.01) in serum compared to all HMO-fortified groups at week 11. No differences in body composition were detected between groups. In females, HMOs did not affect body weight but 2'FL + 3'SL significantly increased cecum weight. All female HMO-fortified groups displayed significant reductions in intestinal permeability compared to controls (p = 0.02). All HMO-fortified diets altered gut microbiota composition and mRNA expression in the gastrointestinal tract, albeit differently according to sex. Supplementation with a fraction of the HMOs found in breast milk has a complex sex-dependent risk/benefit profile. Further long-term investigation of gut microbial profiles and supplementation with other HMOs during early development is warranted.The purpose of this 3-year prospective cohort study was to explore the relationship between an increase in dental caries and oral microbiome among Japanese university students. We analyzed 487 students who volunteered to receive oral examinations and answer baseline (2013) and follow-up (2016) questionnaires. Of these students, salivary samples were randomly collected from 55 students at follow-up and analyzed using next-generation sequencing. Students were divided into two groups increased group (Δdecayed, missing, and filled teeth (ΔDMFT) score increased during the 3-year period) and non-increased group (ΔDMFT did not increase). Thirteen phyla, 21 classes, 32 orders, 48 families, 72 genera, and 156 species were identified. Microbial diversity in the increased group (n = 14) was similar to that in the non-increased group (n = 41). Relative abundances of the family Prevotellaceae (p = 0.007) and genera Alloprevotella (p = 0.007) and Dialister (p = 0.039) were enriched in the increased group compared with the non-increased group. Some bacterial taxonomic clades were differentially present between the two groups. These results may contribute to the development of new dental caries prevention strategies, including the development of detection kits and enlightenment activities for these bacteria.

Transient periods of disuse and/or inactivity in combination with sub-optimal dietary intake have been shown to accelerate the age-related loss of muscle mass and strength, predisposing to disability and metabolic disease. Mechanisms underlying disuse and/or inactivity-related muscle deterioration in the older adults, whilst multifaceted, ultimately manifest in an imbalance between rates of muscle protein synthesis and breakdown, resulting in net muscle loss. To date, the most potent intervention to mitigate disuse-induced muscle deterioration is mechanical loading in the form of resistance exercise. However, the feasibility of older individuals performing resistance exercise during disuse and inactivity has been questioned, particularly as illness and injury may affect adherence and safety, as well as accessibility to appropriate equipment and physical therapists. Therefore, optimising nutritional intake during disuse events, through the introduction of protein-rich whole-foods, isolated proteins and nutrient compounds with purported pro-anabolic and anti-catabolic properties could offset impairments in muscle protein turnover and, ultimately, the degree of muscle atrophy and recovery upon re-ambulation. Our objective was to determine whether supplementing individual HMOs to a weanling diet alters growth and gut health in rats. Healthy three-week-old Sprague Dawley rat pups were randomized to control, 2'-O-fucosyllactose (2'FL)- and 3'sialyllactose (3'SL)-fortified diets alone or in combination at physiological doses for eight weeks. Body composition, intestinal permeability, serum cytokines, fecal microbiota composition, and messenger RNA (mRNA) expression in the gastrointestinal tract were assessed. Males fed a control diet were 10% heavier and displayed elevated interleukin (IL-18) (p = 0.01) in serum compared to all HMO-fortified groups at week 11. No differences in body composition were detected between groups. In females, HMOs did not affect body weight but 2'FL + 3'SL significantly increased cecum weight. All female HMO-fortified groups displayed significant reductions in intestinal permeability compared to controls (p = 0.02). All HMO-fortified diets altered gut microbiota composition and mRNA expression in the gastrointestinal tract, albeit differently according to sex. Supplementation with a fraction of the HMOs found in breast milk has a complex sex-dependent risk/benefit profile. Further long-term investigation of gut microbial profiles and supplementation with other HMOs during early development is warranted.The purpose of this 3-year prospective cohort study was to explore the relationship between an increase in dental caries and oral microbiome among Japanese university students. We analyzed 487 students who volunteered to receive oral examinations and answer baseline (2013) and follow-up (2016) questionnaires. Of these students, salivary samples were randomly collected from 55 students at follow-up and analyzed using next-generation sequencing. Students were divided into two groups increased group (Δdecayed, missing, and filled teeth (ΔDMFT) score increased during the 3-year period) and non-increased group (ΔDMFT did not increase). Thirteen phyla, 21 classes, 32 orders, 48 families, 72 genera, and 156 species were identified. Microbial diversity in the increased group (n = 14) was similar to that in the non-increased group (n = 41). Relative abundances of the family Prevotellaceae (p = 0.007) and genera Alloprevotella (p = 0.007) and Dialister (p = 0.039) were enriched in the increased group compared with the non-increased group. Some bacterial taxonomic clades were differentially present between the two groups. These results may contribute to the development of new dental caries prevention strategies, including the development of detection kits and enlightenment activities for these bacteria.

As a model, we investigated marine molluscs (Mytilus galloprovincialis) grown in the Adriatic Sea. We characterized histological sections from two organs (gonads and digestive glands) of molluscs collected during several surveys at different growth time and distance from gas extraction platforms. We evaluated the effects of nano-pollutants mostly on the local tissue structure by combining AFM microscopy with scanning electron microscopy (SEM). Furthermore, the AFM images allowed evidencing the presence of nano- or micro-sized structures that exhibit different nanomechanical properties compared to the rest of the tissue. The main objective of this study was to explore the molecular mechanism of metformin in starvation-induced autophagy in peripheral blood mononuclear cells (PBMCs) of type 2 diabetic patients. PBMCs were isolated from 10 diabetic patients and 7 non-diabetic healthy volunteers. The autophagic puncta and markers were measured with the help of monodansylcadaverine staining and western blot. Additionally, transmission electron microscopy was also performed. No significant changes were observed in the initial autophagy marker protein levels in PBMCs of T2D after metformin treatment though diabetic PBMCs showed a high level of phospho-mammalian target of rapamycin, p62 and reduced expression of phospho-AMP-activated protein kinase and lysosomal membrane-associated protein 2, indicating a defect in autophagy. Also, induction of autophagy by tunicamycin resulted in apoptosis in diabetic PBMCs as observed by caspase-3 cleavage and reduced expression of Bcl2.

  • This study aimed to assess the effects of cotinine on age-induced memory and learning impairment and related downstream pathways in ****. Thirty aged (18-month old) and 10 young **** (8-week old) were randomly divided into 4 groups (n = 10 each) and subjected to cotinine at 5 mg/kg dose and/or methyllycaconitine (MLA) at 1 mg/kg, i.p. dose (α7 nAChRs antagonist) for 4 weeks. This systematic review analysed 74 studies on olfactory function in FTD, depression, schizophrenia and bipolar disorder to identify differences in olfactory dysfunction profiles, focusing on the most common smell measures odour identification and discrimination. Results revealed that FTD patients were severely impaired in odour identification but not discrimination; in contrast, patients diagnosed with schizophrenia showed impairments in both measures, while those diagnosed with depression showed no olfactory impairments. Findings in bipolar disorder were mixed. Therefore, testing odour identification and discrimination differentiates FTD from depression and schizophrenia, but not from bipolar disorder.     Central-line associated bloodstream infection (CLABSI) is associated with increased mortality, morbidity, and cost in hospitalized children. An evidence-based bundle of care can decrease CLABSI, but bundle compliance is imperfect. We explored factors impacting bundle performance in the pediatric intensive care unit (PICU) by bedside nurses.

    Single-center cross-sectional electronic survey of PICU bedside nurses in an academic tertiary care center; using the COM-B (capability, opportunity, motivation) and TDF (theoretical domains framework) behavioral models to explore CLABSI bundle performance and identify barriers to compliance.

    We analyzed 160 completed surveys from 226 nurses (71% response rate). CLABSI knowledge was strong (capability). However, challenges related to opportunity were identified 71% reported that patient care requirements impact bundle completion; 32% described the bundle as stressful; and CLABSI was viewed as the most difficult of all bundles. Seventy-five percent reported being highly impacted by physician attitude toward the CLABSI bundle (motivation).

    PICU nurses are knowledgeable and motivated to prevent CLABSI, but face challenges from competing clinical tasks, limited resources, and complex family interactions. Physician engagement was specifically noted to impact nurse motivation to complete the bundle. Interventions that address these challenges may improve bundle performance and prevent CLABSI in critically ill children.

We explored factors impacting bundle performance in the pediatric intensive care unit (PICU) by bedside nurses. Single-center cross-sectional electronic survey of PICU bedside nurses in an academic tertiary care center; using the COM-B (capability, opportunity, motivation) and TDF (theoretical domains framework) behavioral models to explore CLABSI bundle performance and identify barriers to compliance. We analyzed 160 completed surveys from 226 nurses (71% response rate). CLABSI knowledge was strong (capability). However, challenges related to opportunity were identified 71% reported that patient care requirements impact bundle completion; 32% described the bundle as stressful; and CLABSI was viewed as the most difficult of all bundles. Seventy-five percent reported being highly impacted by physician attitude toward the CLABSI bundle (motivation). PICU nurses are knowledgeable and motivated to prevent CLABSI, but face challenges from competing clinical tasks, limited resources, and complex family interactions. Physician engagement was specifically noted to impact nurse motivation to complete the bundle. Interventions that address these challenges may improve bundle performance and prevent CLABSI in critically ill children.
