  • OBJECTIVE The authors sought to perform a preliminary assessment of the safety and effectiveness of stereotactic laser interstitial thermal therapy (LITT) for patients with cerebral cavernous malformation (CCM)-related epilepsy. METHODS The authors retrospectively analyzed 6 patients with CCM-related epilepsy who underwent LITT. Pre-, intra-, and postoperative brain MRI studies were used to characterize preoperative CCM volume, ablation volume, and postablation hemosiderin volume. Clinical outcomes were assessed postoperatively during clinic follow-up visits or phone interviews. RESULTS LITT was performed in 7 CCMs in 6 patients. Two patients had familial CCM disease with multifocal lesions. Four treated CCMs were extratemporal, and 3 were in or near the visual pathways. The median follow-up was 25 (range 12-39) months. Five of 6 (83%) patients achieved seizure freedom (Engel I classification), of whom 4 (67%) were Engel IA and 1 was Engel IC after a single seizure on postoperative day 4. The remaining patient had rare seizures (Engel II). One patient had a nondisabling visual field deficit. There were no hemorrhagic complications. All patients were discharged within 24 hours postablation. MRI 3-11 months after ablation demonstrated expected focal necrosis and trace hemosiderin-related T2 hypointensity measuring 9%-44% (median 24%) of the original lesion volume, with significant (p = 0.04) volume reduction. CONCLUSIONS LITT is a minimally invasive option for treating CCM-related epilepsy with seizure outcomes comparable to those achieved with open lesionectomy. The precision of LITT allows for the obliteration of eloquent, deep, small, and multifocal lesions with low complication rates, minimal postoperative discomfort, and short hospital stays. METHODS This was a retrospective, single-institution, observational cohort study of pediatric patients who underwent evaluation and surgical treatment for medically refractory partial epilepsy between December 2003 and June 2016. During this interval, 108 children with medically refractory partial epilepsy underwent evaluation for localization and resective epilepsy surgery. Different noninvasive functional imaging modalities, including ictal SPECT, FDG-PET, and magnetoencephalography-magnetic source imaging, were utilized to augment a standardized paradigm (electroencephalography/semiology, MRI, and neuropsychology findings) for localization. Outcomes were evaluated at a minimum of 2 years (mean 7.5 years) utilizing area under the receiver operating characteristic curve analysis. Localizing modalities and other clinical covariates were examined in relation to long-term surgical outcomes. RESULTS There was variation in the contribution of each test, and no single presurgical workup modality could singularly and reliably predict a seizure-free outcome. However, concordance of presurgical modalities yielded a high predictive value. No difference in long-term outcomes between inconclusive (normal or diffusely abnormal) and abnormal focal MRI results were found. Long-term survival analyses revealed a statistically significant association between seizure freedom and patients with focal ictal EEG, early surgical intervention, and no history of generalized convulsions. The object of this study was to evaluate late postoperative imaging findings on the temporal lobe from a structural point of view. METHODS The authors conducted a retrospective evaluation of all patients with refractory MTLE who had undergone transsylvian selective amygdalohippocampectomy (SAH) in the period from 2002 to 2015. A surgical group was compared to a control group (i.e., adults with refractory MTLE with an indication for surgical treatment of epilepsy but who did not undergo the surgical procedure). The inferior frontooccipital fasciculus (IFOF), uncinate fasciculus (UF), and ORs were evaluated on diffusion tensor imaging analysis. The temporal pole neocortex was evaluated using T2 relaxometry. RESULTS For the IFOF and UF, there was a decrease in anisotropy, voxels, and fibers in the surgical group compared with those in the control group (p less then 0.001). An increase in relaxometry time in the surgical group compared to that in the control group (p less then 0.001) was documented, suggesting gliosis and neuronal loss in the temporal pole. CONCLUSIONS SAH techniques do not seem to totally preserve the temporal stem or even spare the neocortex of the temporal pole.   • The bioaccessibility of As in soil, rather than its total concentration, is closely related to its potential risk. In this study, the in situ formation of amorphous Fe oxides was applied to As-contaminated soil to induce As-Fe coprecipitates that can withstand the gastric digestion condition of human beings. To promote the formation of Fe oxides, 2% ferric nitrate (w/w) and 30% water (v/w) were introduced, and the pH was adjusted to ~7. The chemical extractability of As in soil was determined using the solubility/bioavailability research consortium method and five-step sequential extraction. In situ formation of Fe oxides resulted in a remarkable increase in the As associated with amorphous Fe oxides, decreasing most of the exchangeable As (i.e., the sum of SO42- and PO43- extractable As), and thereby reducing the bioaccessibility of As. The types of association between As and Fe oxides were investigated using X-ray absorption spectroscopy analysis. Linear combination fit (LCF) analysis demonstrated that As bound to amorphous Fe oxides could exist as coprecipitates with ferrihydrite and schwertmannite after stabilization. A commercially available multiplex canine cytokine panel was validated for the quantification of five pro- and anti-inflammatory cytokines in polar bear plasma tumor necrosis factor alpha (TNF-α), interleukin 6 (IL-6), IL-8, IL-10, and interferon gamma-induced protein 10 (IP-10). This panel was then used to measure cytokine concentrations in 49ential adverse consequences on a well-balanced immune system.Every year during the peak winter months (December-January), dense and prolonged fog envelops the Indo-Gangetic Basin (IGB) over the Indian sub-continent leading to economic loss. Many efforts are being made to understand its characteristics to improve forecasting skills. In the present work Indian National SATellite (INSAT-3D), a geostationary satellite, retrieved fog data available at every 30 min interval throughout the day and night is used to study its evolution, spatial and temporal variability for the winter months of December 2016 and January 2017 in conjunction with surface measurements. Fog data when compared with ground-based horizontal visibility over different stations in IGB showed good relationship, also depicted similar temporal variability in tune with the favorable meteorological parameters of temperature, relative humidity, and wind speed. It is observed that INSAT-3D captured fog very well for visibility ranging from 0 to 500 m representing very dense, dense, and moderate fog, while, the shallow fog with visibility >600 m is captured occasionally. From the maps of spatial and temporal variability of fog, the regions most affected are identified based on fog duration. The intense fog events covering the entire IGB for long duration are observed mostly in the midnight (0000-0400) and early morning (0400-0800) hours (local time) of December as compared to January. While in January, moderate intense fog spans the entire IGB during day time also. Due to increase in pollution levels, including biomass burning as inferred from MODIS fire counts, and stable atmosphere, high aerosol optical depths (AOD) are observed across IGB. Based on multi-year in situ water quality measurements and meteorological data during natural and artificial mixing periods, this study evaluates the effects of water-lifting aerators (WLAs) and climatic factors on convective mixing processes and their duration. WLA-supplied turbulent kinetic energy (TKE) and WLA-induced hypolimnion warming significantly decrease the water stability and extend the naturally complete mixing period for 2.6-fold. The results indicate that an optimal WLA implementation should immediately achieve complete mixing when surface mixing occurs in autumn. By evaluating the influence of WLAs and other factors on convective mixing, this study provides insights for successful destratification system operations (i.e., WLAs) to replenish oxygen concentrations across the water column and minimize operating costs by taking advantage of climatic conditions. Cells viability and apoptosis were investigated. In parallel, RNA was isolated, reverse transcribed and used to monitor the expression levels of TXR1, CYP 3A4 and CYP2C8 genes. RESULTS PTX-NPs were characterized by transmission electron microscopy as a nano size sphere-like shape. FTIR analysis revealed good coupling between PTX and PLGA. The encapsulation efficiency was 99% and the drug release demonstrated a progressive releasing phase followed by slower and sustained releasing phase. Although HepG2 cells demonstrated more resistance to PTX than MCF-7 cells, both cell types were more responsive to PTX-NPS compared to PTX. These modulatory mechanisms on resistance blood vessels exert an effect on the release of neuroactive substances from the autonomic or sensory fibers that modify blood pressure. Certainly, resistance blood vessels are innervated by perivascular (i) autonomic sympathetic fibers (producing vasoconstriction mainly by noradrenaline release); and (ii) peptidergic sensory fibers [producing vasodilatation mainly by calcitonin gene-related peptide (CGRP) release]. In the last years, by using pithed rats, several monoaminergic mechanisms for controlling both the sympathetic and sensory perivascular outflows have been elucidated. RESULTS We found that HOTAIR levels were inversely associated with sensitivity to trastuzumab in gastric cancer, and that overexpression of HOTAIR can promote the proliferation and invasion of gastric cancer cells. The sensitivity of cells overexpressing HOTAIR to two different types of human epidermal growth factor receptor 2 (HER2) inhibitors (trastuzumab and afatinib) showed that overexpression of HOTAIR is specific for trastuzumab resistance.

    Single injection interscalene block (ISB) provides effective analgesia for shoulder surgery. However, the duration of these is limited. This review summarizes the effectiveness of three potential means of extending the duration of analgesia perineural infusion of local anesthetic agents, addition of adjuvant drugs to local anesthetics in single-injection nerve block, and utilization of liposomal bupivacaine in the surgical field or in the block itself.

    Perineural infusion of local anesthetics with interscalene catheters provides superior pain relief with reduction of postoperative opioids. Both dexmedetomidine and dexamethasone effectively extend the duration of single injection ISB. Liposomal bupivacaine holds promise when injected perineurally in ISB, either as a sole agent or when mixed with standard bupivacaine.

  • Sudden unexpected infant death (SUID), which includes the diagnosis of sudden infant death syndrome (SIDS), is a leading cause of infant mortality in the United States. Despite prevention efforts, many parents continue to create unsafe infant sleep environments and use potentially dangerous infant sleep and monitoring devices, ultimately leading to sleep-related infant deaths. Analyzing Facebook conversations regarding SIDS may offer a unique maternal perspective to guide future research and prevention efforts.

    This study aims to describe and analyze conversations among mothers engaged in discussions about SIDS on a Facebook mother's group. We were interested in understanding maternal knowledge of SIDS, identifying information sources for SIDS, describing actual infant sleep practices, exploring opinions regarding infant sleep products and monitoring devices, and discovering evidence of provider communication regarding SIDS.

All steps of literature search and selection, data charting, and synthesis followed the leading methodological guideline, the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) checklist. A total of 29 (17 peer reviewed) and 98 publications (32 peer reviewed) for sex dolls and sex robots, respectively, from 1993 to 2019 were included.

    According to the topics and methodologies, the sex doll and sex robot publications were divided into 5 and 6 groups, respectively. The majority of publications were theoretical papers. Thus far, no observational or experimental research exists that uses actual sex dolls or sex robots as stimulus material.

There has been significant work examining machine learning approaches to identify and predict intentional self-harm and suicide using existing data sets. With recent advances in computing, deep learning applications in health care are gaining momentum.

    This study aimed to leverage the information in clinical notes using deep neural networks (DNNs) to (1) improve the identification of patients treated for intentional self-harm and (2) predict future self-harm events.

    We extracted clinical text notes from electronic health records (EHRs) of 835 patients with International Classification of Diseases (ICD) codes for intentional self-harm and 1670 matched controls who never had any intentional self-harm ICD codes. The data were divided into training and holdout test sets. We tested a number of algorithms on clinical notes associated with the intentional self-harm codes using the training set, including several traditionaR. The modest performance on the predictive task notwithstanding, the results using DNN models on clinical text alone are competitive with other reports in the literature using risk factors from structured EHR data.

    We conducted semistructured interviews in Australian general practice settings with purposively sampled general practitioners and patients. The audio-recorded interviews were transcribed, coded, and thematically analyzed by two researchers.

This review focuses on the continuous and distinct perturbations arising in innate immune cells during cirrhosis development including their impact on disease progression as well as potential future therapeutic targets. Activation of liver macrophages (Kupffer cells) and resident mastocytes generate proinflammatory and vaso-permeating mediators that induce accumulation ofg the TYRO3/AXL/MERTK (TAM) receptors AXL or MERTK during disease progression especially during acute decompensation and ACLF stages. Impaired antimicrobial responses can be reversed with potent toll-like receptor (TLR)3/7/8 agonists, or inhibitors of glutamine synthase and TAM receptors. TLR7/8 activation boosts bacterial elimination and increases survival in a rat model of cirrhosis. This raises perspectives for their use in immunocompromised patients and supports the notion that innate immune dysfunction promotes disease progression. BACKGROUND & AIMS Accurate hepatitis C virus (HCV) incidence estimates are critical for monitoring progress towards HCV elimination goals, including an 80% reduction in HCV incidence by 2030. Moreover, incidence estimates can help guide prevention and treatment programming, particularly in the context of the US opioid epidemic. METHODS An inexpensive, Genedia-based HCV IgG antibody avidity assay was evaluated as a platform to estimate cross-sectional, population-level primary HCV incidence using 1840 HCV antibody and RNA positive samples from 875 individuals enrolled in 5 cohort studies in the US and India. Using samples collected less then 2 years following HCV seroconversion, the mean duration of recent infection (MDRI) was calculated by fitting a maximum likelihood binomial regression model to the probability of appearing recent. Among samples collected ≥2 years post-HCV seroconversion, a subject-level false recent ratio (FRR) was calculated by estimating the probability of appearing recent using an exact binomial test. Factors associated with falsely appearing recent among samples collected ≥2 years post seroconversion were determined by Poisson regression with generalized estimating equations and robust variance estimators. RESULTS An avidity index cutoff of less then 40% resulted in an MDRI of 113 days (95%CI84-146), and an FRR of 0.4% (95%CI0.0-1.2), 4.6% (95%CI2.2-8.3), and 9.5% (95%CI3.6-19.6) among persons who were HIV-uninfected, HIV-infected, and HIV-infected with a CD4 count less then 200, respectively. No variation was seen between HCV genotypes 1 and 3. In hypothetical scenarios of high-risk settings, a sample size of less then 1000 individuals could reliably estimate primary HCV incidence. CONCLUSIONS This cross-sectional approach can estimate primary HCV incidence for the most common genotypes. Nevertheless, it is the predominant pediatric liver cancer, with limited therapeutic options for patients with aggressive tumors. Here we sought to increase knowledge of HB pathobiology in an effort to move towards precision medicine. To this end, we used high-throughput technologies to identify new biomarkers and therapeutic targets for HB patients with poor prognosis METHODS We performed a comprehensive genomic, transcriptomic and epigenomic characterization of 159 clinically-annotated samples from 113 HB patients RESULTS We discovered an unprecedented widespread epigenetic footprint of HB that includes hyperediting of the tumor suppressor BLCAP concomitant with a genome-wide dysregulation of RNA editing and the overexpression of mainly non-coding genes of the oncogenic 14q32 DLK1-DIO3 locus. By unsupervised analysis, we identified two epigenomic clusters (Epi-CA, Epi-CB) with distinct degree of DNA hypomethylation and CpG island hypermethylation that are associated with the C1/C2/C2B transcriptomic subtypes. Based on these findings, we defined the first Molecular Risk Stratification of HB (MRS-HB), which encompasses three main prognostic categories and improves the current clinical risk stratification approach. The MRS-3 category (28%), defined by strong 14q32 locus expression and Epi-CB methylation features, was characterized by CTNNB1 and NFE2L2 mutations, a progenitor-like phenotype and clinical aggressiveness. Finally, we identified choline kinase alpha as a promising therapeutic target for intermediate and high-risk HBs since its inhibition in HB cell lines and patient-derived xenografts strongly abrogated tumor growth CONCLUSIONS Our findings expand our knowledge about the molecular features of HB and may contribute to improve current clinical stratification approaches and treatments to increase the survival of patients with HB.

Activation of liver macrophages (Kupffer cells) and resident mastocytes generate proinflammatory and vaso-permeating mediators that induce accumulation ofg the TYRO3/AXL/MERTK (TAM) receptors AXL or MERTK during disease progression especially during acute decompensation and ACLF stages. Impaired antimicrobial responses can be reversed with potent toll-like receptor (TLR)3/7/8 agonists, or inhibitors of glutamine synthase and TAM receptors. TLR7/8 activation boosts bacterial elimination and increases survival in a rat model of cirrhosis. This raises perspectives for their use in immunocompromised patients and supports the notion that innate immune dysfunction promotes disease progression. BACKGROUND & AIMS Accurate hepatitis C virus (HCV) incidence estimates are critical for monitoring progress towards HCV elimination goals, including an 80% reduction in HCV incidence by 2030. Moreover, incidence estimates can help guide prevention and treatment programming, particularly in the context of the US opioid epidemic. METHODS An inexpensive, Genedia-based HCV IgG antibody avidity assay was evaluated as a platform to estimate cross-sectional, population-level primary HCV incidence using 1840 HCV antibody and RNA positive samples from 875 individuals enrolled in 5 cohort studies in the US and India. Using samples collected less then 2 years following HCV seroconversion, the mean duration of recent infection (MDRI) was calculated by fitting a maximum likelihood binomial regression model to the probability of appearing recent. Among samples collected ≥2 years post-HCV seroconversion, a subject-level false recent ratio (FRR) was calculated by estimating the probability of appearing recent using an exact binomial test. Factors associated with falsely appearing recent among samples collected ≥2 years post seroconversion were determined by Poisson regression with generalized estimating equations and robust variance estimators. RESULTS An avidity index cutoff of less then 40% resulted in an MDRI of 113 days (95%CI84-146), and an FRR of 0.4% (95%CI0.0-1.2), 4.6% (95%CI2.2-8.3), and 9.5% (95%CI3.6-19.6) among persons who were HIV-uninfected, HIV-infected, and HIV-infected with a CD4 count less then 200, respectively. No variation was seen between HCV genotypes 1 and 3. In hypothetical scenarios of high-risk settings, a sample size of less then 1000 individuals could reliably estimate primary HCV incidence. CONCLUSIONS This cross-sectional approach can estimate primary HCV incidence for the most common genotypes. This tool can serve as a valuable resource for program and policy planners seeking to monitor and reduce HCV burden. V.Hepatoblastoma (HB) is a rare disease. Nevertheless, it is the predominant pediatric liver cancer, with limited therapeutic options for patients with aggressive tumors. Here we sought to increase knowledge of HB pathobiology in an effort to move towards precision medicine. To this end, we used high-throughput technologies to identify new biomarkers and therapeutic targets for HB patients with poor prognosis METHODS We performed a comprehensive genomic, transcriptomic and epigenomic characterization of 159 clinically-annotated samples from 113 HB patients RESULTS We discovered an unprecedented widespread epigenetic footprint of HB that includes hyperediting of the tumor suppressor BLCAP concomitant with a genome-wide dysregulation of RNA editing and the overexpression of mainly non-coding genes of the oncogenic 14q32 DLK1-DIO3 locus. By unsupervised analysis, we identified two epigenomic clusters (Epi-CA, Epi-CB) with distinct degree of DNA hypomethylation and CpG island hypermethylation that are associated with the C1/C2/C2B transcriptomic subtypes. Based on these findings, we defined the first Molecular Risk Stratification of HB (MRS-HB), which encompasses three main prognostic categories and improves the current clinical risk stratification approach. The MRS-3 category (28%), defined by strong 14q32 locus expression and Epi-CB methylation features, was characterized by CTNNB1 and NFE2L2 mutations, a progenitor-like phenotype and clinical aggressiveness. Finally, we identified choline kinase alpha as a promising therapeutic target for intermediate and high-risk HBs since its inhibition in HB cell lines and patient-derived xenografts strongly abrogated tumor growth CONCLUSIONS Our findings expand our knowledge about the molecular features of HB and may contribute to improve current clinical stratification approaches and treatments to increase the survival of patients with HB.

The corresponding activities were demonstrated for the first time in purified mitochondria of chickpicular, stress-induced expression of the CaNDB2 gene showed a striking positive correlation with that of CaAOX1 across all genotypes and time points. High precision chamfered billets were obtained with roundness errors of 1.1-2.8%, bending deflections of 0.5-1.5mm, and angles of inclination of 0.7°-3.4°. Additionally, the notch effectively reduced the maximum impact force by 21.6-23.9%, splitting displacement by 7.6-18.6%, and impact energy by 27.8-39.1%. The crack initiation zone displayed quasi-parabolic shallow dimples due to shear stress, and the pinning effect was larger in AISI 52100 and 1045 steel; the final rupture zone was characterized by less elongated and quasi-equiaxial deeper dimples due to the combination of shear and normal stress.

In China, a considerable number of convalescent SARS patients with steroid-associated ONFH had undergone conservative treatment by Traditional Chinese Medicine (TCM), and this study aims to evaluate the long-term results of a spleen-invigorating Huo-** Formula (HGF) therapy in these patients. Participants and methods A total of 33 convalescent SARS patients (nine males and 24 females) with bilateral steroid-associated ONFH (66 hips) were enrolled in this study. All patients received oral HGF therapy for six months when they were confirmed the diagnosis of steroid-associated ONFH. They had been regularly followed up at an interval of one year. I describe how I built a research lab centered around Cyclic Adsorption/Reaction Processes focusing in adsorption technologies to help solving environmental problems. I stress the basic concepts of adsorption processes and the need to use proper diffusion models for intraparticle mass transfer instead of pseudo first order or second order kinetic models. I also consider that adsorbent metrics should be linked to the process where the material is used materials and processes together! In the last section I review some challenging areas where adsorption technologies are useful. Carbon Capture and Utilization involving Pressure Swing Adsorption to capture CO2 from flue gas in a pilot plant, 3D printed composite monoliths for Electric Swing Adsorption, and Utilization of CO2 to be transformed in methanol or Synthetic Natural Gas (SNG) (Power-to-Gas concept). I also address the general topic "Processing of diluted aqueous solutions" with special attention for the development of Simulated Moving Bed coupled with Expanded Bed Adsorption. In order to treat patients safely while protecting medical teams, the entire health care system must optimize the way it approaches prevention and treatment at a time when social distancing is key to stemming this pandemic. All indications and treatment modalities must be re-discussed. This is particularly the case for radiotherapy of bone metastases for which it is possible to reduce the number of sessions, the frequency of transport and the complexity of treatments. These changes will have to be discussed according to the organization of each radiotherapy department and the health situation, while medical teams must remain vigilant about the risks of complications of bone metastases, particularly spinal metastases. In this short piece, the members of the GEMO (the European Study Group of Bone Metastases) offer a number of recommendations to achieve the above objectives, both in general and in relation to five of the most common situations on radiation therapy for bone metastases.

In this short piece, the members of the GEMO (the European Study Group of Bone Metastases) offer a number of recommendations to achieve the above objectives, both in general and in relation to five of the most common situations on radiation therapy for bone metastases.
