To systematically review randomized controlled trials assessing effects of sodium-glucose cotransporter 2 inhibitors (SGLT2is) on hospitalization for heart failure (HHF) and cardiac structure/function and explore randomized controlled trial (RCT)-derived evidence for SGLT2i efficacy mechanisms in heart failure (HF).
Systematic searches of Medline and Embase were performed. In seven trials [3730-17160 patients; low risk of bias (RoB)], SGLT2is significantly reduced the relative risk of HHF by 27-39% vs. placebo, including in two studies in patients with HF with reduced ejection fraction with or without type-2 diabetes mellitus (T2DM). Improvements in conventional cardiovascular risk factors, including glycaemic levels, cannot account for these effects. Five trials (56-105 patients; low RoB) assessed the effects of 6-12months of SGLT2i treatment on left ventricular structure/function; four reported significant improvements vs. placebo, and one did not. Five trials (low RoB) assessed SGLT2i treatment effectsdelling likely contributes to this efficacy. Hypothesis-driven mechanistic trials remain sparse, although numerous trials are planned or ongoing.
As first-degree relatives (FDRs) of HLA-B27-positive axial spondyloarthritis (axSpA) patients have an increased risk of developing axSpA, the objectives were 1) to evaluate presence of highly specific imaging features as well as clinical signs of SpA at baseline and after one year of follow-up, and 2) describe the evolution towards clinical disease within one year of follow-up in a cohort of seemingly healthy FDRs of HLA-B27 positive axSpA patients.
The Pre-SpA cohort is a 5-year prospective inception cohort of seemingly healthy FDRs of HLA-B27 positive axSpA patients. Clinical and imaging features were collected and recorded.
At baseline 19% of the FDRs reported inflammatory **** pain. Thirty-two percent reported current arthralgia, 3% arthritis (ever), 5% enthesitis (ever) and 1% dactylitis (ever). Three percent had an extra-articular manifestation. CRP was elevated in 16%, ESR in 7%. On MRI-SIJ, 10% had a SPARCC score ≥2, 4% ≥5, and 4% deep lesions. One percent fulfilled the mNY criteria for radiographic sacroiliitis. Clinical, MRI and acute phase findings were equally distributed between HLA-B27 positive and negative FDRs. After 1 year of follow-up, clinical parameters did not change on the group level but 6% of the FDRs were clinically diagnosed with axSpA, of whom 86% were HLA-B27 positive.

Cygnet-Cyrene blends could represent viable replacements for toxic polar aprotic solvents. The use of a 50 wt % Cygnet-Cyrene mixture makes a practical difference in the production of flat sheet membranes by nonsolvent-induced phase separation. New polymeric membranes from cellulose acetate, polysulfone, and polyimide are manufactured by using Cyrene, Cygnet 0.0, and their blend. The resultant membranes have different morphology when the solvent/mixture and temperature of the casting solution change. Moreover, Cyrene, Cygnet 0.0, and Cygnet-Cyrene are also explored for substituting diphenyl ether for the biocatalytic synthesis of polyesters. The results indicate that Cygnet 0.0 is a very promising candidate for the enzymatic synthesis of high molecular weight polyesters.
To assess the use, satisfaction, needs and preferences regarding physical therapy (PT) in patients with systemic sclerosis (SSc).
Four-hundred-and-five SSc patients, treated in the Leiden University Medical Center multidisciplinary care program and fulfilling ACR/EULAR-2013 SSc-criteria, received a questionnaire containing 37 questions on use and satisfaction regarding PT over a 2-year period, and their needs and preferences for future PT.
Two-hundred-and-four SSc patients (median age 63years, 81% female) completed the questionnaire. One-hundred-twenty-eight (63%) patients had used or were using PT in primary care setting. For 39% of patients not using PT, lack of referral or lack of knowledge was the reason for not using it. The most frequently reported active treatments were muscle-strengthening (n=92;72%), range of motion (n=77;60%) and aerobic exercises (n=72;56%). Specific SSc hand and mouth-opening exercises were reported by 20 (15%) and 7 (6%) patients, respectively. Manual treatment (massage or passive mobilization) was reported by 83 (65%) patients. Mean satisfaction score (range0-10) was 8.2 (SD1.6). Regarding patients' needs, 96 (47%) of the total group wanted to receive more information concerning PT and 128 (63%) to continue/(re)start PT in the near future, with 56/128 (44%) favoring individual treatment on a continuous basis.
We observed a significant variation in the use and content of PT for SSc patients in primary care setting. Our results suggest potential under-use of PT care, in particular for hand and oral dysfunction, and underpin the need for initiatives to improve the quality and accessibility of PT care for SSc patients.

Hydro-acoustic actuation is elegantly utilized to remotely activate the piezoelectric effect of P(VDF-TrFE) scaffolds in a physiologically-safe manner for the generation of cell-relevant electric potentials. This mechano-electrical stimulation, which arose from the deflection of the scaffold and its consequent generation of electric charges on the scaffold surface under hydro-acoustic actuation, induces the multi-phenotypic differentiation of neural stem cells simultaneously toward neuronal, oligodendrocytic, and astrocytic phenotypes.

Moreover, the number of unique frames and the pattern update rate equally influence the discrimination thresholds of translational and circular GPs. These results show that form and motion integration is likely to be processed similarly for translational and circular GPs.
In 2017, the American College of Cardiology/ American Heart Association (ACC/AHA) guidelines redefined the threshold of high blood pressure (BP) for non-pregnant adults. Several studies aimed to determine whether lowering these thresholds should be considered for pregnancies to prevent poor neonatal outcomes, but the results were inconclusive.
We perform a systematic review and meta-analysis to evaluate the association between BP groups defined by the 2017 ACC/AHA guidelines and pregnancy outcomes. Relevant literature was searched in PubMed, Google Scholar, Embase, and Web of Science up to 18 May 2021.
Sixteen eligible studies from twelve articles with a total of 303,131 pregnancies were identified, encompassing 233,084, 20,859, 39,379 individuals with normal BP, elevated BP, and stage 1 hypertension, respectively. When compared with normal BP, the combined odds ratio (95% confidence interval) of elevated BP for adverse pregnancy outcomes was not significant; whereas, that of stage 1 hypertension were 1.25 (1.13-1.39), 1.16 (1.03-1.31), 1.50 (1.28-1.77) and 1.12 (1.00-1.25) for preterm delivery, small for gestational age, low birth weight, and early-term delivery, respectively.

Musculoskeletal (MSK) models based on literature data are meant to represent a generic anatomy and are a popular tool employed by biomechanists to estimate the internal loads occurring in the lower limb joints, such as joint reaction forces (JRFs). However, since these models are normally just linearly scaled to an individual's anthropometry, it is unclear how their estimations would be affected by the personalization of key features of the MSK anatomy, one of which is the femoral version angle.

We developed a bone-deformation tool in MATLAB (shared at https//simtk.org/projects/bone_deformity) and used it to create a set of seven OpenSim models spanning from 2Ëš femoral retroversion to 40Ëš anteversion. We used these models to simulate the gait of an elderly individual with an instrumented prosthesis implanted at their knee joint (5th Grand Challenge dataset) and quantified both the changK anatomical features might be necessary for accurate estimation of JRFs with these models.During the winemaking process, fining materials derived from milk and egg products are traditionally used to remove undesirable substances to reduce bitterness and astringency. The possible residues of allergens in treated wine may pose a potential risk for allergy patients. In this study, we developed a method for the simultaneous quantification of eight allergens (αS1-casein, αS2-casein, β-casein, κ-casein, β-lactoglobulin, lysozyme, ovalbumin and ovotransferrin) in red wine by ultrahigh-performance liquid chromatography coupled to tandem mass spectrometry (UPLC-MS/MS). The sample was extracted with polyvinylpolypyrrolidone (PVPP) solution, following trypsin digestion and peptide-level purification by solid-phase extraction (SPE). A strategy based on standard addition was used for the accurate quantification of the target allergens in wine products. The limits of detection (LODs) were shown to be 0.003-0.015 μg/mL for milk allergens and 0.1 μg/mL for egg allergens. This economical and reliable method would be appropriate for routine analysis and further allergen label management for red wine.Hepatolenticular degeneration (HLD) is an inherited disorder associated with human copper metabolism. Gandou decoction (GDD), a traditional Chinese medicinal formula, has been used as a therapeutic agent for the treatment of HLD in China for decades. Recent pharmacological evaluation in our laboratory has demonstrated that GDD exerts positive and beneficial effects on HLD model rats. However, its underlying therapeutic mechanisms are not yet well understood. To explore the potential therapeutic effects of GDD against HLD, liver and urine metabolomics approach combined with histopathological examination were performed to reveal the underlying mechanisms. Changes in metabolic profiles were estimated by ultra-performance liquid chromatography quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS) coupled with multivariate statistical analyses. Water pollution from heavy metals and their toxic oxo-anionic derivatives such as CrO42-, Cr2O72-, HAsO42-, and HAsO32- has become one of the most critical environmental issues. To address this, herein, we report a new hydrolytically stable luminescent Zn(ii) based cationic metal organic framework (MOF), iMOF-4C, which further successfully exhibited a rare dual "turn off/on" fluorescence response toward Cr(vi), As(v) and As(iii) based oxo-anions respectively in water medium. In addition, iMOF-4C was found to maintain its superior selectivity in the presence of other concurrent anions (e.g. SO42-, Cl-, Br-, ClO4-, NO3-, SCN- and CO32-). More importantly, iMOF-4C exhibited an excellent selective and sensitive luminescence "turn-off" response towards CrO42- and Cr2O72- anions in water medium with the quenching constant (Ksv) values as high as 1.31 × 105 M-1 (CrO42-) and 4.85 × 105 M-1 (Cr2O72-), which are found to be the highest among the values reported in the regime of MOFs. Water pollution from heavy metals and their toxic oxo-anionic derivatives such as CrO42-, Cr2O72-, HAsO42-, and HAsO32- has become one of the most critical environmental issues. To address this, herein, we report a new hydrolytically stable luminescent Zn(ii) based cationic metal organic framework (MOF), iMOF-4C, which further successfully exhibited a rare dual "turn off/on" fluorescence response toward Cr(vi), As(v) and As(iii) based oxo-anions respectively in water medium. In addition, iMOF-4C was found to maintain its superior selectivity in the presence of other concurrent anions (e.g. SO42-, Cl-, Br-, ClO4-, NO3-, SCN- and CO32-). More importantly, iMOF-4C exhibited an excellent selective and sensitive luminescence "turn-off" response towards CrO42- and Cr2O72- anions in water medium with the quenching constant (Ksv) values as high as 1.31 × 105 M-1 (CrO42-) and 4.85 × 105 M-1 (Cr2O72-), which are found to be the highest among the values reported in the regime of MOFs. To understand the acoustic response, samples are measured in an acoustic impedance tube and characterized with the Johnson-Champoux-Allard-Lafarge model for rigid-frame foams. Bayesian statistical inversion of the model parameters is performed to assess the applicability of commonly employed measurement and modeling methods for traditional foams to the additively manufactured, low porosity aluminum foams. This preliminary characterization provides insights into how emerging voxel-by-voxel additive manufacturing approaches could be used to fabricate acoustic metal foams and what could be learned about the microstructure using traditional measurement and analysis techniques.A high resolution direction-of-arrival (DOA) approach is presented based on deep neural networks (DNNs) for multiple speech sources localization using a small scale array. Then the DNN labels are designed to fit the Gaussian distribution, which is similar to the spatial spectrum of the multiple signal classification. Finally, DOAs are predicted by performing peak detection on the DNN outputs, where the maximum values correspond to speech signals of interest. The DNN-based DOA estimation method outperforms the existing high resolution beamforming techniques in numerical simulations. Furthermore, the tunable nature of the system allows it to switch the time-reversed pulse on and off by changing its geometrical configuration. A robustness analysis with respect to the perturbations in the sonic crystal configurations is also presented, showing that the time-reversed pulses with high temporal and spatial contrasts are preserved only for configurations that are close to the original one.When performing measurements with wall-installed microphone array, the turbulent boundary layer that develops over the measuring system can induce pressure fluctuations that are **** greater than those of acoustic sources. It then becomes necessary to process the data to extract each component of the measured field. For this purpose, it is proposed in this paper to decompose the measured spectral matrix into the sum of matrices associated with the acoustic and aerodynamic contributions. This decomposition exploits the statistical properties of each pressure field. On the one hand, assuming that the acoustic contribution is highly correlated over the sensors, the rank of the corresponding cross-spectral matrix is limited to a finite number. On the other hand, the correlation structure of the aerodynamic noise matrix is constrained to resemble a Corcos-like model, with physical parameters estimated within the separation procedure. This separation problem is solved by a Bayesian inference approach, which takes into account the uncertainties on each component of the model. This aesthetic labor is presented to women as freely chosen and empowering. Brazilian waxing is exemplary aesthetic labor directed at the self. Academic literature on aesthetic labor in general, and Brazilian waxing in particular, looks at white and middle-class women, as this category of women is considered the putative subject of postfeminism. Little attention is paid to racialized women from the global south who perform aesthetic labor on other women's bodies in the global north. In this paper, I draw on my ethnographic study of two beauty salons in London run by South Asian women to argue that these South Asian beauticians are postfeminist subjects as well. The aim of challenging the putative subject of postfeminism, using the example of Brazilian waxing, is not merely to include South Asian women in the discourse, but to advance a transnational theorization of postfeminism. Such theorization, I demonstrate, leads to a better understanding of how postfeminism is implicated in global structures of power as well as the affective qualities of postfeminism including intimacy and disgust.The aim of this article is to analyse, within the framework of discourse analysis, the social representations that circulated about the sex education in the parliamentary debate on the bill for the Voluntary Interruption of Pregnancy that took place in the Chamber of Deputies, on June 13, 2018, in Argentina. To do this, we review the functions attributed to sex education by both legislators who voted in favor of the bill and those who voted against. In turn, we analyze the topics that were related to this issue, as well as the social actors involved and those who were legitimized to address the problem. We aim to establish if the feminist discourse that was constituted and consolidated in Argentina from the Ni Una Menos march, in 2015, managed to impose its agenda on the political sphere. Specifically, we are interested in investigating whether the representations that were put into circulation about sex education were retaken in the parliamentary debate on the voluntary termination of pregnancy bill, a space in which sex education constituted a preponderant topic.Using a meta-synthesis approach, through the review of current literature, five published and peer reviewed qualitative research reports were studied. The intention was to identify interventions being used with West African children who have endured adverse childhood experiences. In sub-Saharan Africa, tick-borne pathogens identified include viruses, bacteria and protozoa, with Rickettsia being the most frequently reported. This study was conducted to screen and identify Rickettsia species in ticks (Family Ixodidae) infesting livestock in selected districts of southern Zambia. A total of 236 ticks from three different genera (Amblyomma, Hyalomma, and Rhipicephalus) were collected over 14 months (May 2018-July 2019) and were subsequently screened for the presence of Rickettsia pathogens based on PCR amplification targeting the outer membrane protein B (ompB). An overall Rickettsia prevalence of 18.6% (44/236) was recorded. Multi-locus sequencing and phylogenetic characterization based on the ompB, ompA, 16S rRNA and citrate synthase (gltA) genes revealed the presence of Rickettsia africae (R. africae), R. aeschlimannii-like species and unidentified Rickettsia species. While R. aeschlimannii-like species are being reported for the first time in Zambia, R. africae has been reported previously, with our results showing a wider distribution of the bacteria in the country. The aim of challenging the putative subject of postfeminism, using the example of Brazilian waxing, is not merely to include South Asian women in the discourse, but to advance a transnational theorization of postfeminism. In turn, we analyze the topics that were related to this issue, as well as the social actors involved and those who were legitimized to address the problem. We aim to establish if the feminist discourse that was constituted and consolidated in Argentina from the Ni Una Menos march, in 2015, managed to impose its agenda on the political sphere. Specifically, we are interested in investigating whether the representations that were put into circulation about sex education were retaken in the parliamentary debate on the voluntary termination of pregnancy bill, a space in which sex education constituted a preponderant topic.Using a meta-synthesis approach, through the review of current literature, five published and peer reviewed qualitative research reports were studied. The intention was to identify interventions being used with West African children who have endured adverse childhood experiences. In sub-Saharan Africa, tick-borne pathogens identified include viruses, bacteria and protozoa, with Rickettsia being the most frequently reported. This study was conducted to screen and identify Rickettsia species in ticks (Family Ixodidae) infesting livestock in selected districts of southern Zambia. A total of 236 ticks from three different genera (Amblyomma, Hyalomma, and Rhipicephalus) were collected over 14 months (May 2018-July 2019) and were subsequently screened for the presence of Rickettsia pathogens based on PCR amplification targeting the outer membrane protein B (ompB). An overall Rickettsia prevalence of 18.6% (44/236) was recorded. Multi-locus sequencing and phylogenetic characterization based on the ompB, ompA, 16S rRNA and citrate synthase (gltA) genes revealed the presence of Rickettsia africae (R. africae), R. aeschlimannii-like species and unidentified Rickettsia species. While R. aeschlimannii-like species are being reported for the first time in Zambia, R. africae has been reported previously, with our results showing a wider distribution of the bacteria in the country. Results Our fiber type-specific quantitative proteome analysis confirmed the distribution of fiber types in the soleus muscle, substantiates metabolic adaptions in oxidative and glycolytic fibers, and highlighted significant differences between the proteomes of type IIb fibers from different muscle groups, including a differential expression of desmin and actinin-3. A detailed map of the Lys-6 incorporation rates in muscle fibers showed an increased turnover of slow fibers compared to fast fibers. In addition, labeling of mitochondrial respiratory chain complexes revealed a broad range of Lys-6 incorporation rates, depending on the localization of the subunits within distinct complexes. Though banded gastric bypasses are effective in weight loss, band migration, whether it be slippage or erosion, occurs in up to 7% of banded bariatric procedures. Jejunal intussusception and internal herniation are both individually known complications of RYGB. It is, however, rare to have all three phenomenon occur in a single patient. Methods Interventional trials reporting changes in complex attention and other neurocognitive functions (Diagnostic and Statistical Manual of Mental Disorders-5) following administration of atomoxetine for at least 8 weeks in adults with idiopathic PD were included. Effect sizes (Cohen's d), the standardized mean difference in the scores of each cognitive domain, were compared using a random-effects model (MetaXL version 5.3). Results Three studies were included in the final analysis. For a change in complex attention in PD with mild cognitive impairment (MCI), the estimated effect size was small and nonsignificant (0.16 (95% CI -0.09, 0.42), n = 42). For changes in executive function, perceptual-motor function, language, social cognition, and learning and memory, the estimated effect sizes were small and medium, but nonsignificant. A deteriorative trend in executive function was observed after atomoxetine treatment in PD with MCI. For a change in global cognitive function in PD without MCI, the estimated effect size was large and significant. Conclusion In idiopathic PD with MCI, atomoxetine does not improve complex attention. Also, a deteriorative trend in the executive function was noted. Ecological and evolutionary studies often ignore the effects of micro-environment on plant population and community dynamics. Here, we explore the extent to which fine-grained variation in abiotic and biotic conditions contributes to within-population variation in trait expression and genetic diversity in natural plant populations. Furthermore, we consider whether benign microhabitats could buffer local populations of some plant species from abiotic stresses imposed by rapid anthropogenic climate change. If microrefugia sustain local populations and communities in the short term, other eco-evolutionary processes, such as gene flow and adaptation, could enhance population stability in the longer term. We caution, however, that local populations may still decline in size as they contract into rare microhabitats and microrefugia. We encourage future research that explicitly examines the role of the micro-environment in maintaining genetic variation within local populations, favouring the evolution of phenotypic plasticity at local scales and enhancing population persistence under global change. © The Author(s) 2020. The standard data related to chronic diseases have been collected from various worldwide resources. In healthcare data, special chronic diseases include ambiguous objects of the class. Therefore, the presence of ambiguous objects shows the availability of traits involving two or more classes, which reduces the accuracy of the machine learning algorithms.

Results Our fiber type-specific quantitative proteome analysis confirmed the distribution of fiber types in the soleus muscle, substantiates metabolic adaptions in oxidative and glycolytic fibers, and highlighted significant differences between the proteomes of type IIb fibers from different muscle groups, including a differential expression of desmin and actinin-3. A detailed map of the Lys-6 incorporation rates in muscle fibers showed an increased turnover of slow fibers compared to fast fibers. In addition, labeling of mitochondrial respiratory chain complexes revealed a broad range of Lys-6 incorporation rates, depending on the localization of the subunits within distinct complexes. Methods Interventional trials reporting changes in complex attention and other neurocognitive functions (Diagnostic and Statistical Manual of Mental Disorders-5) following administration of atomoxetine for at least 8 weeks in adults with idiopathic PD were included. Effect sizes (Cohen's d), the standardized mean difference in the scores of each cognitive domain, were compared using a random-effects model (MetaXL version 5.3). Results Three studies were included in the final analysis. For a change in complex attention in PD with mild cognitive impairment (MCI), the estimated effect size was small and nonsignificant (0.16 (95% CI -0.09, 0.42), n = 42). For changes in executive function, perceptual-motor function, language, social cognition, and learning and memory, the estimated effect sizes were small and medium, but nonsignificant. A deteriorative trend in executive function was observed after atomoxetine treatment in PD with MCI. For a change in global cognitive function in PD without MCI, the estimated effect size was large and significant. Conclusion In idiopathic PD with MCI, atomoxetine does not improve complex attention. Also, a deteriorative trend in the executive function was noted. Ecological and evolutionary studies often ignore the effects of micro-environment on plant population and community dynamics. Here, we explore the extent to which fine-grained variation in abiotic and biotic conditions contributes to within-population variation in trait expression and genetic diversity in natural plant populations. Furthermore, we consider whether benign microhabitats could buffer local populations of some plant species from abiotic stresses imposed by rapid anthropogenic climate change. If microrefugia sustain local populations and communities in the short term, other eco-evolutionary processes, such as gene flow and adaptation, could enhance population stability in the longer term. We caution, however, that local populations may still decline in size as they contract into rare microhabitats and microrefugia. We encourage future research that explicitly examines the role of the micro-environment in maintaining genetic variation within local populations, favouring the evolution of phenotypic plasticity at local scales and enhancing population persistence under global change. © The Author(s) 2020. To describe the first large population (n=297) with tuberous sclerosis complex (TSC) in China and to examine the relationship between variants (type and location) and epilepsy.
All exons and intron-exon boundaries of TSC1/TSC2 were sequenced with next-generation sequencing, and the distribution of several variants and associations between variant types and epilepsy were investigated.
Epilepsy occurred in 83.5% (248/297) of the individuals. The TSC1/TSC2 gene variant detection rate was 89.6% (266/297). The rate of epilepsy was significantly higher in the TSC2 group than in the TSC1 (p=0.02) and no mutation identified (NMI) groups (p=0.0005). TSC2 individuals are more likely to have spasms than TSC1 individuals (p =0.03). The age at epilepsy onset of individuals in the TSC2 group was younger than that of individuals in the TSC1 group (p=0.008) and NMI group (p=0.01). The age at epilepsy onset with truncated variants in the TSC1 group was significantly younger than that of individuals with nontruncated varyounger age at epilepsy onset.

Patients with MCI-DLB had hypometabolism in the parieto-occipital cortex extending into temporal lobes, substantia nigra and thalamus. When compared to MCI-AD, medial temporal and posterior cingulate metabolism were preserved in patients with MCI-DLB, accompanied by greater hypometabolism in the substantia nigra in MCI-DLB compared to MCI-AD. In distinguishing MCI-DLB from MCI-AD at the maximum value of Youden's index, CIS ratio was highly specific (90%) but not sensitive (59%), but a higher medial temporal to substantia nigra ratio was both sensitive (94%) and specific (83%).

Environmental impact assessment (EIA) is a process that examines the potential impacts of development projects on society and the environment. The concept of EIA originated in 1969 by the US National Environmental Policy Act (NEPA), followed by other developed and developing countries all over the globe. This paper reviews the legal status of EIA and its implementation, effectiveness, and deficiencies in Pakistan. EIA received legal status in Pakistan when the Environmental Protection Ordinance (PEPO) was drafted in 1983. This ordinance was then converted into the Pakistan Environmental Protection Act (PEPA) in 1997. Currently, EIA is conducted for many development projects in Pakistan, including infrastructure and industry projects. Environmental protection acts containing sections related to EIA and Initial Environmental Examinations (IEE; a small preliminary study for an EIA project) have been established by all Pakistani provinces, which conduct its systematic facilitation in the country. The effectiveness of the EIA/IEE process in Pakistan can be enhanced by ensuring stakeholder participation, guaranteeing transparency, providing resources to environmental agencies, and post-monitoring of EIA/IEE processes. There is limited information about the incidence of metabolic acidosis (MA) after renal transplantation. This single centre prospective study aimed to delineate the incidence and risk factors of MA in the first 6months after renal transplantation (RTX).
Patients who underwent RTX between November 2018 and July 2020 were monitored with weekly measurement of serum bicarbonate level for 6months and those who were diagnosed with MA were evaluated further to characterize the type of MA.
One hundred and twenty-five patients were included in the study, 89 (71.2%) of whom developed MA. Seventy-two patients developed MA in the first month, 11 during the 2-3months and 6 between 4 and 6months after transplantation. Of the 89 patients, 55(61.8%) had type 1 renal tubular acidosis (T1RTA), 27 (30.3%) had type 2 RTA (T2RTA) and 7 (7.9%) type 4 RTA (T4RTA). Two patient who had T1RTA, subsequently developed high anion gap MA following severe graft rejection. On stepwise multivariate regression analysis, serum creatinine at time of diagnosis of MA [OR (95% CI) 12.02 (1.79 to 80.59), p=.01] and high tacrolimus C0 levels [OR (95% CI) 2.43 (1.0 to 5.90), p=.049], were independent risk factors for MA.

Children and youth with cancer may find it challenging to integrate illness into their pre-existing identity-a phenomenon known as illness identity. In this critical narrative review, we explored illness identity among children and youth with cancer.
Three academic databases were searched. Twenty-two articles were included in this review and each underwent thematic analysis.
Cancer has both positive and negative influences on the identities of children and youth. Illness identity is expressed creatively through various communicative outlets. Further, external processes such as social support may influence cancer identity. A few studies cited cancer as a hindrance to adult identity development.
Cancer has a profound impact on identity formation among children and youth. Many normative assumptions about time, identity, and child and youth development underlie the existing literature. Future researchers may adopt a critical lens to be inclusive of diverse identity experiences among children and youth with cancer.

Using computer simulations can streamline the process by tuning different geometrical and structural features, emphasizing the osseointegration of the implant design a priori, leading to the optimal designs in preparation for in-vivo trails. This research aims to elucidate the interrelationship between 12 geometrical variables that holistically define the shape of the implant. The approach to achieve optimality hinged on coupling the finite element analysis results with the fractional factorial design method. The latter was used to determine the most influential variables during the screening process, followed by the parameter optimization process using the response surface method, regarding four different objectives, namely bone-implant contact area, volume of trabecular bone dead cells, volume of cortical bone dead cells, and axial displacement. This resulted in reducing the number of virtual experiments and substantially decreasing the computational cost without compromising the accuracy of the solution. It was found that the optimized values improved the performance significantly. The validity of all models was verified by comparing optimized responses with simulation results. Digitaria insularis is a weed species that has gained considerable importance in Brazil's soybean production areas that rely on glyphosate-resistant cultivars. Herbicide-resistant weed populations of this species have been reported in many regions in Brazil, first in the south, followed by later reports in the north. We hypothesized that the spread of herbicide-resistant D. insularis is facilitated by movement of agricultural machinery from the southern regions of Brazil.
Population genomics revealed a weak or no genetic structure (F
=[0; 0.16]), moderate expected heterozygosity (H
=0.15; 0.44) and low inbreeding (F
=[-0.1; 0.1]) in D. insularis populations. Our data supported the hypothesis that herbicide resistance gene flow predominantly occurred in a south-to-north direction based on a migration analysis. We also found evidence of local adaptation of resistant populations in the northern soybean-growing regions of Brazil.
Evidence in our work suggests that gene flow of glyphosate-resistant D. insularis is associated with movement of agricultural machinery, although local selection pressure seems to play an important role in the evolution of herbicide resistance throughout the country.

Using computer simulations can streamline the process by tuning different geometrical and structural features, emphasizing the osseointegration of the implant design a priori, leading to the optimal designs in preparation for in-vivo trails. This research aims to elucidate the interrelationship between 12 geometrical variables that holistically define the shape of the implant. The approach to achieve optimality hinged on coupling the finite element analysis results with the fractional factorial design method. The latter was used to determine the most influential variables during the screening process, followed by the parameter optimization process using the response surface method, regarding four different objectives, namely bone-implant contact area, volume of trabecular bone dead cells, volume of cortical bone dead cells, and axial displacement. This resulted in reducing the number of virtual experiments and substantially decreasing the computational cost without compromising the accuracy of the solution. It was found that the optimized values improved the performance significantly. The validity of all models was verified by comparing optimized responses with simulation results. Digitaria insularis is a weed species that has gained considerable importance in Brazil's soybean production areas that rely on glyphosate-resistant cultivars. Herbicide-resistant weed populations of this species have been reported in many regions in Brazil, first in the south, followed by later reports in the north. We hypothesized that the spread of herbicide-resistant D. insularis is facilitated by movement of agricultural machinery from the southern regions of Brazil. Population genomics revealed a weak or no genetic structure (F =[0; 0.16]), moderate expected heterozygosity (H =0.15; 0.44) and low inbreeding (F =[-0.1; 0.1]) in D. insularis populations. Our data supported the hypothesis that herbicide resistance gene flow predominantly occurred in a south-to-north direction based on a migration analysis. We also found evidence of local adaptation of resistant populations in the northern soybean-growing regions of Brazil. There has been a pronounced increase in online shopping since the start of the COVID-19 pandemic. We study the effect of the pandemic on demand for online grocery shopping specifically, using municipality-level data from a Dutch online supermarket. We find that an additional hospital admission increased app traffic by 7.3 percent and sales per order by 0.31 percent. Local hospital admissions do not correlate with the variety of groceries ordered, but online search behavior does, suggesting that hoarding behavior is driven by the general perception and impact of the virus rather than local conditions. Local COVID-19 conditions also have different effects in urban versus non-urban municipalities, with local hospital admissions increasing app traffic in urban areas but lowering sales per order as compared to non-urban areas. It remains to be seen whether the demand for online grocery shopping will permanently increase as a result of the COVID-19 pandemic.

We develop a working definition and provide data on the demographic and labor market composition of these workers. We first apply the official industry guidelines issued by the Department of Homeland Security (DHS) in March 2020 to microdata from the 2018 and 2019 American Community Survey to identify essential workers regardless of actual operation status of their industry. We then use the feasibility of work from home in the worker's occupation group (Dingel and Neiman 2020) to identify those most likely to be frontline workers who worked in-person early in the COVID-19 crisis in March/April 2020. In a third step, we exclude industries that were shut down or running under limited demand at that time (Vavra 2020). We find that the broader group of essential workers comprises a large share of the labor force and tends to mirror its demographic and labor market characteristics. In contrast, the narrower category of frontline workers is, on average, less educated, has lower wages, and has a higher representation of men, disadvantaged minorities, especially Hispanics, and immigrants. These results hold even when excluding industries that were shut down or operating at a limited level. Results for essential and frontline workers are similar when accounting for changes in the federal guidelines over time by using the December 2020 guidelines which include a few additional groups of workers, including the education sector.Locoregional delivery of chimeric antigen receptor (CAR) T cells has resulted in objective responses in adults with glioblastoma, but the feasibility and tolerability of this approach is yet to be evaluated for pediatric central nervous system (CNS) tumors. Here we show that engineering of a medium-length CAR spacer enhances the therapeutic efficacy of human erb-b2 receptor tyrosine kinase 2 (HER2)-specific CAR T cells in an orthotopic xenograft medulloblastoma model. We translated these findings into BrainChild-01 ( NCT03500991 ), an ongoing phase 1 clinical trial at Seattle Children's evaluating repetitive locoregional dosing of these HER2-specific CAR T cells to children and young adults with recurrent/refractory CNS tumors, including diffuse midline glioma. Primary objectives are assessing feasibility, safety and tolerability; secondary objectives include assessing CAR T cell distribution and disease response. In the outpatient setting, patients receive infusions via CNS catheter into either the tumor cavity or the ventricular system. The initial three patients experienced no dose-limiting toxicity and exhibited clinical, as well as correlative laboratory, evidence of local CNS immune activation, including high concentrations of CXCL10 and CCL2 in the cerebrospinal fluid. This interim report supports the feasibility of generating HER2-specific CAR T cells for repeated dosing regimens and suggests that their repeated intra-CNS delivery might be well tolerated and activate a localized immune response in pediatric and young adult patients.Cellular identity in complex multicellular organisms is determined in part by the physical organization of cells. However, large-scale investigation of the cellular interactome remains technically challenging. Here we develop cell interaction by multiplet sequencing (CIM-seq), an unsupervised and high-throughput method to analyze direct physical cell-cell interactions between cell types present in a tissue. CIM-seq is based on RNA sequencing of incompletely dissociated cells, followed by computational deconvolution into constituent cell types. CIM-seq estimates parameters such as number of cells and cell types in each multiplet directly from sequencing data, making it compatible with high-throughput droplet-based methods. When applied to gut epithelium or whole dissociated lung and spleen, CIM-seq correctly identifies known interactions, including those between different cell lineages and immune cells. In the colon, CIM-seq identifies a previously unrecognized goblet cell subtype expressing the wound-healing marker Plet1, which is directly adjacent to colonic stem cells. Our results demonstrate that CIM-seq is broadly applicable to unsupervised profiling of cell-type interactions in different tissue types.The CNS is ensheathed by the meninges and cerebrospinal fluid, and recent findings suggest that these CNS-associated border tissues have complex immunological functions. Unlike myeloid lineage cells, lymphocytes in border compartments have yet to be thoroughly characterized. Based on single-cell transcriptomics, we here identified a highly location-specific composition and expression profile of tissue-resident leukocytes in CNS parenchyma, pia-enriched subdural meninges, dura mater, choroid plexus and cerebrospinal fluid.
