Here, we report the findings on the regenerative properties of human PDLSCs with different donor ages and the underlying mechanisms. Human PDLSCs from 18 independent donors were divided into different age groups (≤ 20, 20-40, and > 40 years old). The proliferation of PDLSCs with donor age of ≤ 20 years old was significantly higher than that of the 20-40- and > 40-years-old groups, whereas the migration of PDLSCs with donor age of ≤ 20 and 20-40 years old was significantly higher than that of the > 40-years-old group. Moreover, the mesodermal lineage differentiation capabilities of PDLSCs were also higher in the donor age group of ≤ 20 years old than the donor age of > 40 years old. In addition, shorter telomere length and lower expression of SSEA4 were found in PDLSCs with donor age of > 40 years old, compared with those with donor age of ≤ 20-years-old group. Besides, PDLSCs with donor age of 20-40 and > 40 years old had higher IL6 and CXCL8 gene expressions. The purpose of this paper is to discuss the experimental and clinical evidence on the utilization of tissue engineering techniques in treating male infertility. Tissue engineering (TE) and regenerative medicine have developed new approaches to treat patients with reproductive disorders such as iatrogenic injuries, congenital abnormalities, and trauma. In some cases, including congenital defects and undescended testis or hypogonadism, the sperm samples are not retrieved. This makes TE a possible future strategy for restoration of male fertility. Here, we have summarized the recent advances in experimental and clinical application of cell-, tissue-, and organ-based regenerative medicine in male reproductive disorders.The RNASET2 ribonuclease, belonging to the highly conserved RH/T2/s RNase gene family, has been recently shown to modulate inflammatory processes in both vertebrates and invertebrates. Indeed, the RNASET2 protein acts as a chemoattractor for macrophages in both in vitro and in vivo experimental settings and its expression significantly increases following bacterial infections. Moreover, we recently observed that injection of human recombinant RNASET2 protein in the body wall of the medicinal leech (a consolidated invertebrate model for both immune response and tissue regeneration) not only induced immune cell recruitment but also apparently triggered massive connective tissue remodelling as well. Based on these data, we evaluate here a possible role of leech recombinant RNASET2 protein (rHvRNASET2) in connective tissue remodelling by characterizing the cell types involved in this process through histochemical, morphological and immunofluorescent assays. Moreover, a time-course expression analysis of newly synthesized pro-collagen1α1 (COL1α1) and basic FGF receptor (bFGFR, a known fibroblast marker) following rHvRNASET2 injection in the leech body wall further supported the occurrence of rHvRNASET2-mediated matrix remodelling.

Background Management of warfarin-associated major haemorrhage in prosthetic valve diseases is difficult as there is a fine line between haemorrhage and thrombosis. An individual's propensity towards thrombosis, such as pregnancy, makes this situation even more complicated. Cases like these are very rare in the literature. Case presentation A 26 weeks pregnant, gravida two, para one, 35-year-old patient with prosthetic aortic and mitral valves presented to an external emergency clinic with clouding of consciousness. Her international normalised ratio(INR) was 8.9 at presentation. Brain MRI revealed a left subdural haematoma with no significant mass effect. Warfarin treatment was discontinued. On the second day of follow-up, she was referred to our centre for further evaluation of her clinical deterioration. She was haemodynamically stable on admission to the intensive care unit and followed up with a stable condition until the fourth day when she developed right eye drop and subsequent loss of consciousness. Her haematoma was surgically evacuated, and her condition improved. Eventually, she and a healthy newborn were discharged. Conclusion Intracranial haemorrhage during pregnancy is a relatively rare complication that requires a multidisciplinary management plan. Although the thrombogenic risk is high, it is vital to complete a reversal of warfarin anticoagulation in pregnant women with major bleeding.Background Aortic stiffness is an independent predictor of cardiovascular (CV) events and mortality. However, no data exists for the prognosis of combined aortic stiffness and myocardial ischemia. Using cardiac magnetic resonance (CMR) imaging, we assessed the association of aortic stiffness by pulse wave velocity (PWV), myocardial ischemia, and CV events in patients with known or suspected coronary artery disease (***). Methods Velocity-encoded CMR was performed in 520 patients who had undergone adenosine stress CMR. The PWV was determined between the mid-ascending and mid-descending thoracic aorta. Patients were divided into 4 groups by PWV (higher or lower PWV) and myocardial ischemia (positive or negative ischemia). Combined CV events including mortality, acute coronary syndrome, heart failure, coronary revascularization, and stroke were analyzed among the 4 groups. https://www.selleckchem.com/products/shin1-rz-2994.html Results The median follow-up period was 46.5 months, and the median PWV was 10.54 m/sec. Myocardial ischemia was positive in 199 patients (38.3%). The group with a higher PWV and positive ischemia had the most CV events (hazard ratio 8.94, p less then 0.001). The group with a higher PWV and negative ischemia also was significantly associated with CV events (HR 2.19, p = 0.02). Groups with a lower PWV-positive ischemia and a higher PWV-negative ischemia showed no difference in terms of CV events (HR 0.60, p = 0.08). Patients with myocardial ischemia who had higher PWV demonstrated significantly higher event rates than those who had lower PWV (HR 2.41, p less then 0.001). Multivariate analysis demonstrated that myocardial ischemia and PWV were independent predictors for combined CV events (HR 2.71, p less then 0.001 and HR 2.42, p less then 0.001, respectively). Conclusions Stress perfusion CMR provided prognostic utility in patients with known or suspected ***. This trial aims to evaluate the effects of two methods of surfactant administration, LISA or endotracheal surfactant administration followed by low peak pressure (LPPSA) ventilation, in preterm infants with respiratory distress syndrome (RDS). Methods The LISA Or Low Peak Pressure trial is to be conducted in 14 tertiary neonatal intensive care units in China. A total of 600 preterm infants born with gestational age between 250/7 and 316/7 weeks and with a primary diagnosis of RDS will be involved in the study. Infants will be randomized to the LISA or LPPSA group when surfactant therapy is indicated. Primary outcomes include mortality, severity oy BPD in preterm infants, remains unclear. The possible explanations are the active and uninterrupted delivery of continuous positive airway pressure during the LISA procedure and the avoidance of complications caused by intubation and relatively high pressure/volume ventilation following surfactant administration. We hypothesized that LISA's effectiveness lies mainly in avoiding relatively high-pressure positive ventilation immediately following surfactant administration. Thus, this multicenter randomized controlled trial will focus on issues of endotracheal intubation and the pressure/volume used during conventional surfactant administration. The effectiveness, safety and comorbidities of preterm infants following LISA or LPPSA will be evaluated. Trial registration Chinese Clinical Trial Registry ChiCTR1900020970. Skin wounds, including acute burns and chronic non-healing ulcers, are commonly observed in clinics. Healing of skin wounds is a complex process, consisting of infiltration of inflammatory cells, cellular proliferation, and tissue remodeling phases, which restore the integrity and functions of the skin. Epithelialization is involved in wound healing through re-establishing an intact keratinocyte layer. Epidermal stem cells are indispensable for epithelialization, and they are regulated by multiple proinflammatory cytokines or growth factors. In this review, we summarize recent advances in the effect of these cytokines on migration, proliferation, and differentiation processes of epidermal stem cells. We also introduce promising therapeutic strategies targeting epidermal stem cells or related proinflammatory cytokines for patients with skin wounds.

We report on occurrence and correlates of self-reported research misconduct (RM) by 100 Kenyan researchers who had received ethics approval for an HIV research in the 5 years preceding the survey. The survey used the Scientific Misconduct Questionnaire - Revised tool uploaded on a Research Electronic Data Capture (REDCAP) platform. The response rate was a low 17.3% (100 out of 577) with 53.9% reporting awareness of an incident of research misconduct in the preceding 5 years. Awareness was associated with being in academia, perception of vulnerability to being caught and the severity of possible punishment, if discovered. Two thirds (68.3%) reported ever-involvement in any misconduct. https://www.selleckchem.com/products/icrt14.html Self-report of involvement in misconduct was associated with knowledge of rules and procedures on RM and a disposition to support such rules and regulations. Nearly 36% reported ever-involvement in falsification, fabrication and or plagiarism (FFP). Measures of suicidal ideation, sociometric relational victimization, friendship stress, depressive symptoms, online-only friendship status and quality, and online-only friendship quality compared to in-person friendship quality, were obtained at Time 1 using sociometric procedures and self-report questionnaires. Self-report measures of suicidal ideation were collected at Time 2.Results Descriptive results suggested that online-only friendships are relatively common among youth (38.3%), particularly for those experiencing suicidal ideation (46.3%). Suicidal and non-suicidal adolescents reported comparable levels of intimate disclosure within their online-only friendships. Although adolescents without suicidal ideation endorsed more support from in-person friendships, suicidal adolescents endorsed similar levels of support from their online-only and in-person friendships. Moderation analyses indicated that the association between both relational victimization and friendship stress and prospective suicidal ideation was attenuated among youth who reported having one or more online-only friend.Conclusion Online-only friendships are common and may offer protective benefits for youth, particularly those experiencing suicidal ideation. Of those not prescribed insulin, 35/56 (61%) breached CGM cut-offs between 0000-0600hrs; 11/56 (20%) breached 6.00-00.00hrs CGM cut-offs for >10% of the time; and 21/45 (47%) with optimal CGM glucose levels during the daytime spent >10% time in hyperglycaemia between 00.00-0600 hrs. In contrast, SMBG measurements exceeded the clinical targets of less then 120mg/dL post-dinner in 5.4% and less then 100mg/dL fasting in 0% of the subjects. CONCLUSIONS CGM provides a more comprehensive assessment of nocturnal hyperglycemia than SMBG and could improve targeting of interventions in GDM. The four subscales were labelled Shared decision power, Adapted and individualized knowledge, Collaboration and Human approach. There were strong ceiling effects on all items. Analysis of convergent construct validity showed a moderate correlation (0.59) between the PPQ and another instrument measuring patient participation. Internal consistency for the total PPQ score was high 0.89. CONCLUSION In a mixed group of patients with cardiac disease, pulmonary disease and cancer, the PPQ showed promising psychometric properties in terms of factor structure, convergent construct validity and internal consistency. The PPQ may be used to shed light on the experience of patient participation and guide quality improvements.
We report on occurrence and correlates of self-reported research misconduct (RM) by 100 Kenyan researchers who had received ethics approval for an HIV research in the 5 years preceding the survey. The survey used the Scientific Misconduct Questionnaire - Revised tool uploaded on a Research Electronic Data Capture (REDCAP) platform. The response rate was a low 17.3% (100 out of 577) with 53.9% reporting awareness of an incident of research misconduct in the preceding 5 years. Awareness was associated with being in academia, perception of vulnerability to being caught and the severity of possible punishment, if discovered. Nearly 36% reported ever-involvement in falsification, fabrication and or plagiarism (FFP). CASE A 69-year-old woman presented with a brachial artery pseudoaneurysm causing median, ulnar, and radial nerve compressive neuropathies 8 weeks after a proximal humerus shaft fracture treated with intramedullary nailing. https://www.selleckchem.com/products/ana-12.html She underwent a brachial artery bypass with a reverse cephalic vein interposition graft and neurolysis after decompression of a large pseudoaneurysm. Postoperatively, the patient had a normal vascular examination with recovering neurological function. CONCLUSIONS A high index of suspicion for a brachial artery pseudoaneurysm should be maintained in patients with a displaced proximal humerus shaft fracture, particularly when an abnormal neurovascular examination is present.CASE A 78-year-old women presented with a closed left midshaft femur fracture after sustaining a fall from standing height and underwent proximal tibial traction pin placement. After subsequent intramedullary nailing of femur fracture, the patient returned 2 months later with a proximal tibia fracture through the unicortical defect left from the traction pin site and underwent successful intramedullary nailing of the tibia. CONCLUSION Errant anteriorly placed proximal tibial traction pins pass tangentially across the vertex of the tibia and create residual unicortical defects that may increase the risk for late iatrogenic pin site fractures.CASE We present the case of a 71-year-old man with right knee osteoarthritis (OA) and a varus deformity who developed a progressive common peroneal nerve palsy that resolved after total knee arthroplasty (TKA). After decades of knee pain, the patient gradually developed a foot drop as well as numbness and paresthesias over the foot dorsum during the course of 1 month. The patient underwent TKA and within 6 weeks postoperatively had complete resolution of the peroneal nerve symptoms. CONCLUSION A progressive common peroneal palsy in advanced varus knee OA may resolve after a properly aligned TKA without nerve decompression.CASE A 17-year-old boy had persistent knee pain 1 year after medial meniscal root repair augmented with bone marrow aspirate concentrate injection. Radiographs and magnetic resonance imaging (MRI) demonstrated an intrameniscal ossicle which was not present on MRI performed before 6 months. He underwent arthroscopic excision of the meniscal ossicle. At the 7-month follow-up, he had complete relief of his pain. CONCLUSIONS It is possible that the meniscal ossicle developed because of osteoinductive cells and cytokines from the injected bone marrow or the drill hole for root repair and should be considered as a possible complication of this procedure.CASE A 33-year-old man with recurrent intrathoracic scapular dislocation due to previous trauma-related chest wall resection successfully underwent the 2-stage induced membrane technique commonly known as the Masquelet technique; this procedure effectively created 2 new ribs that resolved his symptoms. CONCLUSIONS Techniques for chest wall reconstruction for bone loss are quite limited, and these often consist of filling defects with a layered patch; this often cannot withstand the cyclical respiratory motion. Use of the induced membrane technique appears to carry potential when used in the chest wall, and this report describes a technique by which this procedure can be reliably performed.CASE A 38-year-old woman presented with previously undiagnosed factor V Leiden (FVL), who suffered a complete superficial femoral arterial thrombosis after tourniquet use during the surgical repair of one of her bilateral tibial plafond fractures. CONCLUSION We recommend expanding hypercoagulable screening on patients with risk factors based on a detailed history and physical examination. At 1-year, the patient had no pain, no elbow instability, and full and symmetric elbow range of motion. CONCLUSION Excellent short-term pain and function outcomes can be observed in the pediatric patient after medial epicondyle fragment excision when there is concomitant avulsion of the ulnar collateral and flexor-pronator origins from the fracture fragment.CASE A 16-year-old male patient with severe kyphoscoliosis, paraplegia, and neurogenic bowel/bladder caused by a juvenile pilocytic astrocytoma was treated surgically using a hybrid fusion construct with polyethylene bands after neoplasm resection. Owing to the necessity of serial postoperative magnetic resonance imaging studies to evaluate the recurrence of pathology and known effect of metal artifact from spinal instrumentation, preservation of radiographic resolution was critical. CONCLUSION We describe the novel utility of polyethylene bands placed around the ribs as a safe and effective form of hybrid construct for reducing radiographic metal artifact in spinal deformity cases requiring serial imaging.CASE A 60-year-old man presented with left hip pain, and a radiograph showed reduced joint space. During the surgical procedure for a total hip replacement, a proximal femur mass was identified and biopsy was subsequently interpreted as grade 2 chondrosarcoma. A wide resection was needed, but he developed local recurrence after 2 years and was treated with an external hemipelvectomy. CONCLUSIONS Chondrosarcoma does not always present with a classical clinical picture or imaging, and it can be misdiagnosed. Practitioners should be highly suspicious of malignant disease as a cause for hip pain even if there is no direct indication of a neoplasm such as chondrosarcoma.CASE We report the case of an 82-year-old woman with diabetes, arteriosclerosis, chronic heart failure, and hypertension treated with an anatomical locking plate and multiple cables for a spiral-wedged periprosthetic fracture of the distal femur which was complicated by direct occlusion of the femoral artery and crush of the sciatic nerve, resulting in leg necrosis and, ultimately, through-knee disarticulation despite early recognition and arterial repair. CONCLUSIONS Neurovascular injury is a potential complication of cerclage cables placement around the femoral shaft during complex fracture fixation. Devastating complications cannot always be corrected despite early vascular intervention.

Background There is limited literature on patterns of everolimus use and subsequent hospitalizations and emergency room (ER) visits in real-world clinical practice. In this study, we describe patterns of everolimus use and hospitalizations and ER visits in a large cohort of patients with breast cancer (**). Materials and methods Patients with ** treated with everolimus were identified in the MarketScan database from 2009 to 2016. The pattern of everolimus use and frequency of associated ER visits and hospitalizations during treatment (between the first claim and 30 days after the last claim for everolimus) were identified. Descriptive statistics and regression models were used. Results A total of 3,556 everolimus users were identified (median age of 60 years; median days of use, 112). The initial prescribed dose was 10 mg in 74.8% of the patients. Compared with the initial dose, 23.5% of patients had a dose change. Forty-six percent of patients were hospitalized or had an ER visit during the treatment with evolled clinical trial setting. By controlling the interfacial energy and growth kinetics, a series of Au-on-AuNR hybrid structures are successfully obtained, with the newly grown Au domains being sphere, polyhedron, and branched wire (nanocoral). Background There is limited literature on patterns of everolimus use and subsequent hospitalizations and emergency room (ER) visits in real-world clinical practice. In this study, we describe patterns of everolimus use and hospitalizations and ER visits in a large cohort of patients with breast cancer (BC). Materials and methods Patients with BC treated with everolimus were identified in the MarketScan database from 2009 to 2016. The pattern of everolimus use and frequency of associated ER visits and hospitalizations during treatment (between the first claim and 30 days after the last claim for everolimus) were identified. Descriptive statistics and regression models were used. Results A total of 3,556 everolimus users were identified (median age of 60 years; median days of use, 112). The initial prescribed dose was 10 mg in 74.8% of the patients. Compared with the initial dose, 23.5% of patients had a dose change. Forty-six percent of patients were hospitalized or had an ER visit during the treatment with evolled clinical trial setting. By controlling the interfacial energy and growth kinetics, a series of Au-on-AuNR hybrid structures are successfully obtained, with the newly grown Au domains being sphere, polyhedron, and branched wire (nanocoral). Therefore, we developed an integrated multidisciplinary rehabilitation programme consisting of inpatient neurorehabilitation treatment using goal attainment scaling, home based self-management training, and group based stroke education for patients and informal caregivers. We performed a process evaluation to assess to what extent this programme was performed according to protocol. Furthermore, we assessed the participation of the patients in the programme, and the opinion of patients, informal caregivers and care professionals on the programme. Methods In this multimethod study, process data were collected by means oeasibility of the programme needs further attention. Because of persisting cognitive deficits and specific care needs in our frail and multimorbid target population, some widely used methods such as goal attainment scaling, and self-management training seemed not feasible in their current form. To optimize feasibility of the programme, it is recommended to tailor these elements more optimally to the population of frail older patients.Background A particular ability of the Staphylococcus aureus clonal complex 398 (CC398) to cause bone and joint infections (BJI) remains questionable, since some studies have described high prevalence of MSSA CC398 in prosthetic joint infection (PJI) and diabetic foot ostemolyelitis (DFO). Here, we described the long-term epidemiology of CC398 among S. aureus isolated from BJI and identified risk factors associated with CC398. Methods We included all bone and joint samples with S. aureus-positive culture in our university hospital between January 2010 and December 2017. Logistic regression was used for univariate and multivariate analysis. Results We identified 124 CC398 isolates among the 958 BJI-associated S. aureus. The proportion of CC398 among S. aureus increased steadily from 4% in 2010 to 26% in 2017. Only 4 isolates of CC398 were resistant to methicillin. The distribution of BJI types due to CC398 and non CC398 isolates was similar. In multivariate analysis, age (p = 0.034, OR = 3.9), McCabe score (p = 0.005, OR = 5) and inoculation mechanism (p = 0.020, OR = 3.7) were associated with PJI-related CC398. The year of infection (p less then 0.001, OR = 1.6), Charlson's score (p = 0.001, OR = 1.5) and grade 4 (severe) of the International Working Group of the Diabetic Foot classification (p less then 0.001, OR = 8.5) were associated with DFO-related CC398. Conclusion We highlighted here the emergence and spread of CC398-MSSA in BJI. Patients with comorbidities are at high risk of CC398 MSSA PJI and DFO. It is responsible for the presented consensus statement, which as a final document was reached after review of the pertinent literature and again revised after the recent publication of the STAR trial and related commentaries. Results In contrast to the PGDIA-PS, we recommend restrictions to the increasing, and by IVF centers now often even mandated, utilization of PGT-A in IVF cycles. We reinforce the fact that the dog is susceptible to natural R. rickettsii infection. Although this is a rare finding, preventive measures should be taken against BSF in the studied areas. Finally, R. parkeri infection is possibly being demonstrated in A. sculptum for the first time.This study investigated the seropositivity for five different tick-borne agents, namely Anaplasma marginale, Babesia bovis, Babesia bigemina, Coxiella burnetii, Anaplasma phagocytophilum, and Trypanosoma vivax in beef cattle in the Brazilian Pantanal. The serum samples collected from animals (200 cows; 200 calves) were used in indirect enzyme-linked immunosorbent assays (iELISA) to detect IgG antibodies against A. marginale, B. bovis, B. bigemina, and T. vivax, and Indirect Fluorescent Antibody Test (IFAT) for detecting IgG antibodies against C. burnetii and A. phagocytophilum. No correlation was observed between seropositivity for C. burnetii and A. phagocytophilum with other agents whereas moderate correlation was observed for A. marginalexB. bigemina x B. bovis. Cows were more seropositive for T. vivax whereas calves were more seropositive for B. bovis and B. bigemina. The highest number of seropositive animals by a single agent was observed for T. vivax (15.2%). Co-seropositivity for T. vivax + A. marginale was higher in cows (25.5%) and for T. vivax + B. bovis + B. bigemina + A. marginale was higher in calves (57.5%). The high seropositivity correlation for A. marginale x B. bovis x B. bigemina is probably due to the presence of the tick biological vector, Rhipicephalus microplus, in the studied farms. Common transmission pathways, mediated by hematophagous dipterans and fomites, may explain the high co-seropositivity of cows for A. marginale and T. vivax. Seropositivity for A. phagocytophilum in only one animal suggests the occurrence of a cross-serological reaction with another agent of the genus Anaplasma.in English, Portuguese, O objetivo foi avaliar os fatores sociodemográficos e clínicos relacionados à falta de assistência hospitalar em óbitos por doença cerebrovascular (DCV) e DCV hemorrágica, no Estado de São Paulo, Brasil, nos triênios 1996-1998 e 2013-2015. Foram utilizados dados dos óbitos provenientes do Sistema de Informações sobre Mortalidade. Para analisar a associação entre a falta de atendimento hospitalar e as variáveis consideradas no estudo utilizou-se a regressão de Poisson. Dos 127.319 indivíduos que morreram por DCV nos dois triênios, 19.362 (15,2%) não tiveram assistência hospitalar. A falta de atendimento hospitalar em óbitos por DCV manteve-se praticamente inalterada para as características sociodemográficas e clínicas, exceto a distribuição por sexo. No período mais recente, identificou-se maior risco de óbito por DCV sem assistência hospitalar entre indivíduos de cor da pele amarela (RR = 1,48), já em pessoas de cor preta (RR = 0,85), parda (RR = 0,86), nos casados (RR = 0,70), naqueles que residiam no Município de São Paulo (RR = 0,92), nos que tiveram assistência médica (RR = 0,17) e naqueles acometidos pela DCV hemorrágica (RR = 0,47) o risco de óbito sem assistência hospitalar foi menor. Além disso, a falta de atendimento hospitalar em óbitos por DCV hemorrágica foi menor entre os casados (RR = 0,67), naqueles que residiam no Município de São Paulo (RR = 0,74) e nos que tiveram assistência médica (RR = 0,08). Foi realizado um estudo transversal utilizando-se os dados das AIH não identificadas dos recém-nascidos com até 1 dia de vida, saídos entre janeiro de 2013 e dezembro de 2015, e do Cadastro Nacional de Estabelecimentos de Saúde. O método TwoStep Cluster foi usado para classificar os estabelecimentos com características similares. Foram observadas diferenças regionais na frequência de uso das unidades de terapia intensiva neonatal e nos procedimentos especiais registrados nas bases de dados. O método TwoStep Cluster identificou três agrupamentos de prestadores o 1º com 1.151 estabelecimentos e com internações de baixa complexidade; o 2º com 84 estabelecimentos e internações de perfil cirúrgico; e o 3º com 393 estabelecimentos e internações de maior complexidade do cuidado neonatal, incluindo cirurgias. The objectives of this study were to evaluate changes in mean birth weight of term newborns and to identify factors associated with them. All cohorts are population-based studies in which random samples of births (Ribeirão Preto, São Paulo State in 1978/1979, 1994 and 2010; Pelotas, Rio Grande do Sul State in 1982, 1993 and 2004; and São Luís, Maranhão State in 1997/1998 and 2010, Brazil). A total of 32,147 full-term, singleton live births were included. Seropositivity for A. phagocytophilum in only one animal suggests the occurrence of a cross-serological reaction with another agent of the genus Anaplasma.in English, Portuguese, O objetivo foi avaliar os fatores sociodemográficos e clínicos relacionados à falta de assistência hospitalar em óbitos por doença cerebrovascular (DCV) e DCV hemorrágica, no Estado de São Paulo, Brasil, nos triênios 1996-1998 e 2013-2015. Foram utilizados dados dos óbitos provenientes do Sistema de Informações sobre Mortalidade. Para analisar a associação entre a falta de atendimento hospitalar e as variáveis consideradas no estudo utilizou-se a regressão de Poisson. Dos 127.319 indivíduos que morreram por DCV nos dois triênios, 19.362 (15,2%) não tiveram assistência hospitalar. A falta de atendimento hospitalar em óbitos por DCV manteve-se praticamente inalterada para as características sociodemográficas e clínicas, exceto a distribuição por sexo. No período mais recente, identificou-se maior risco de óbito por DCV sem assistência hospitalar entre indivíduos de cor da pele amarela (RR = 1,48), já em pessoas de cor preta (RR = 0,85), parda (RR = 0,86), nos casados (RR = 0,70), naqueles que residiam no Município de São Paulo (RR = 0,92), nos que tiveram assistência médica (RR = 0,17) e naqueles acometidos pela DCV hemorrágica (RR = 0,47) o risco de óbito sem assistência hospitalar foi menor. Além disso, a falta de atendimento hospitalar em óbitos por DCV hemorrágica foi menor entre os casados (RR = 0,67), naqueles que residiam no Município de São Paulo (RR = 0,74) e nos que tiveram assistência médica (RR = 0,08). Foi realizado um estudo transversal utilizando-se os dados das AIH não identificadas dos recém-nascidos com até 1 dia de vida, saídos entre janeiro de 2013 e dezembro de 2015, e do Cadastro Nacional de Estabelecimentos de Saúde. O método TwoStep Cluster foi usado para classificar os estabelecimentos com características similares. Foram observadas diferenças regionais na frequência de uso das unidades de terapia intensiva neonatal e nos procedimentos especiais registrados nas bases de dados. O método TwoStep Cluster identificou três agrupamentos de prestadores o 1º com 1.151 estabelecimentos e com internações de baixa complexidade; o 2º com 84 estabelecimentos e internações de perfil cirúrgico; e o 3º com 393 estabelecimentos e internações de maior complexidade do cuidado neonatal, incluindo cirurgias. The objectives of this study were to evaluate changes in mean birth weight of term newborns and to identify factors associated with them. All cohorts are population-based studies in which random samples of births (Ribeirão Preto, São Paulo State in 1978/1979, 1994 and 2010; Pelotas, Rio Grande do Sul State in 1982, 1993 and 2004; and São Luís, Maranhão State in 1997/1998 and 2010, Brazil). A total of 32,147 full-term, singleton live births were included. Our purpose was to determine the effect of chemoradiotherapy (CRT) on patient-reported quality of life (QOL) for patients with intact pancreas cancer.
We reviewed a prospective QOL registry for patients with intact, clinically localized pancreatic ductal adenocarcinoma treated with CRT between June 2015 and November 2018. QOL was assessed pre-CRT (immediately before CRT, after neoadjuvant chemotherapy) and at the completion of CRT with the Functional Assessment of Cancer Therapy-Hepatobiliary (FACT-Hep) and its component parts FACT-General (FACT-G) and hepatobiliary cancer subscore (HCS). A minimally important difference from pre-CRT was defined as ≥ 6, 5, and 8 points for FACT-G, HCS, and FACT-Hep, respectively.
Of 157 patients who underwent CRT, 100 completed both pre- and post-CRT surveys and were included in the primary analysis. Median age at diagnosis was 65 years (range, 23-90). National Comprehensive Cancer Network resectability status was resectable (3%), borderline resectable (40%), or locallyeatic adenocarcinoma, modern CRT is well tolerated with minimal decline in QOL during treatment.

Malnutrition is one of the leading causes of death among children younger than five years. In this study, we aimed to formulate a ready-to-use supplementary food (RUSF), based on local food products, and investigate its efficacy on growth indicators in children with mild to moderate malnutrition.
This is a randomized controlled clinical trial.
This study was performed in six health centers in Shahr-e-Rey, Tehran, Iran, between April and October 2017. Children in the RUSF group had more improvement in WHZ (1.18±0.41 vs 0.41±0.31, P<0.001) after supplementation. Besides, 92% of the RUSF and 12% of the control group reached to WHZ>-1 at the end of the study (P<0.001). There was lower prevalence of diarrhea (12% vs 28.6%, respectively, P=0.01) and marginally lower fever (16% vs 36.7%, respectively, P=0.05) in the intervention than in the control group.

Chest radiography is considered gold standard for definition of pneumonia, however no previous study assessed the relationship between immune response and radiographic-confirmed-pneumonia. We assessed association between cytokines/chemokines levels and radiographic abnormalities in children with CAP. Children less then 5-years-old hospitalized with CAP were investigated in a prospective study at the Federal University of Bahia Hospital, Brazil. On admission, clinical data and biological samples were collected to investigate 20 aetiological agents and determine serum cytokines/chemokines levels; chest radiographs were performed. Among 158 patients, radiographic diagnosis of pneumonia was confirmed in 126(79.7%) and 17(10.8%) had pleural effusion. Viral, bacterial and pneumococcal infection were detected in 80(50.6%), 78(49.4%) and 37(23.4%) cases. The surrounding tumor microenvironment (TME) is important in tumor progression, as cytokines and soluble mediators including tumor necrosis factor (TNF-α) or lipocalin-2 (LCN2) can influence tumor growth and formation of metastasis. The exact mechanisms on how these pleiotropic factors affect PCa are still unknown. In this study, we showed for the first time that LCN2 mRNA and protein expression are strongly inducible by TNF-α in the highly metastatic human PCa cell line PC-3. In addition, we observed higher levels of secreted LCN2 in cell culture medium of TNF-α-treated PC-3 cells. We found that different signaling pathways such as p38, NF-κB or JNK were activated shortly after TNF-α treatment. Moreover, the mRNA levels of IL-1β and IL-8 were also significantly increased after 24 h stimulation. Mechanistically, the NF-κB pathway and the JNK signaling axis are directly responsible for LCN2 upregulation. This was shown by the fact that pretreatment with the JNK inhibitors SP600125 or JNK-IN-8 strongly downregulated phosphorylation of c-Jun protein and markedly reduced TNF-α-mediated LCN2 upregulation in PC-3 cells. Likewise, the NF-κB inhibitor QNZ was able to repress TNF-α-induced LCN2 expression in PC-3 cells. Taking into consideration that LCN2 has been described as a tumor promoting factor in PCa, our results indicate that JNK regulates LCN2 expression and unmasks the JNK signaling axis as a possible therapeutic target for patients with PCa.
Cytotoxic chemotherapy remains the standard of care first-line treatment for advanced and metastatic soft-tissue sarcomas (STSs). Certain patients may not be chemotherapy candidates based upon age or co-morbidities, leaving limited treatment options. Pazopanib is a multi-targeted tyrosine kinase inhibitor that is FDA-approved for metastatic STS after the first line. We proposed a phase II study evaluating pazopanib as a first-line agent in patients with advanced disease who are deemed not to be candidates for chemotherapy.
Eligible patients were at least 18 years old, not candidates for chemotherapyand were treatment naive. Pazopanib was titrated from 200mg twice daily to a goal of 800mg daily. The primary end point was the clinical benefit rate (CBR) (CBR = completed response+partial response +stable disease per Response Evaluation Criteria in Solid Tumours [RECIST 1.1]) at 16 weeks. The sample size of 56 evaluable patients was calculated to provide 80% power to test a hypothesised CBR of ≥35% against an unfavourable CBR of ≤20%.

Direct catheterization of the translocated deep draining vein was useful to bypass the dangerous access to the fistula and may be a feasible alternative strategy for treating selected AVFs.Background A novel viral strain known as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has created a worldwide pandemic known as Coronavirus 2019 (COVID-19). Early reports from China highlighted the risks associated with performing endoscopic endonasal skull base surgery in patients with SARS-CoV-2. We present a rare complication of nasoseptal flap (NSF) necrosis associated with COVID-19, further emphasizing the challenges of performing these procedures in this era. Case description A 78-year-old male underwent an extended endoscopic endonasal transplanum resection of a pituitary macroadenoma for decompression of the optic chiasm. https://www.selleckchem.com/products/frax486.html The resulting skull base defect was repaired using a pedicled nasoseptal flap (NSF). The patient developed meningitis and a cerebrospinal fluid (CSF) leak on post-operative day thirteen requiring revision repair of the defect. Twelve days later, he developed persistent fevers and rhinorrhea. The patient was re-explored endoscopically, and the NSF was noted to be necrotic and devitalized with evident CSF leakage. At that time, the patient tested positive for Sars-CoV-2. Post-operatively, he developed acute respiratory distress syndrome complicated by hypoxic respiratory failure and death. Conclusion To our knowledge, this is the first reported case of NSF necrosis in a patient with COVID-19. We postulate that the thrombotic complications of COVID-19 may have contributed to vascular pedicle thrombosis and NSF necrosis. Although the pathophysiology of SARS-CoV-2 and its effect on the nasal tissues is still being elucidated, this case highlights some challenges of performing endoscopic skull base surgery in the era of COVID-19.Ependymomas are the most common adult intramedullary spinal tumors.1 Although uncommon in the brainstem, ependymomas make up a large proportion of tumors of this location.2-8 We present an operative video case report of an intrinsic ependymoma at the cervicomedullary junction. The purpose of this report is to present the clinical picture, operative setup, and surgical technique involved in resection of an intramedullary tumor of this region. For best outcome for intramedullary ependymomas, the goal should be gross total resection.1,9,10 These tumors have a relatively distinct plane between tumor and normal parenchyma, making a gross total resection more probable than cases of infiltrative intramedullary astrocytomas.11 Despite this, significant morbidity can be associated with treatment.1 Proper microsurgical technique with use of operative adjuncts can maximize resection while minimizing neurologic injury to optimize outcomes in patients., We present the case of a 42-year-old gentleman presenting with neck and shoulder pain, upper extremity paresthesias, and gait instability. MRI of the neuroaxis revealed a heterogeneously enhancing expansile lesion in the lower medulla and multiple lesions in the thecal sac, representing drop metastases. Due to symptoms and mass effect from the lesion, the patient underwent a suboccipital craniotomy for tumor resection. We highlight operative techniques in our case, including use of neurophysiologic monitoring, intraoperative ultrasound, ultrasonic aspirator, and dissection with micro-cottonoid pledgets and bimanual technique. Pathology revealed a WHO grade II ependymoma. Post-operative MRI revealed a small amount of residual. He underwent full craniospinal proton beam therapy with boost to the residual and metastases with good outcome. In this review, we discuss the importance of RON in TNBC tumorigenesis and the development of anti-RON antibody-drug conjugates (ADCs) for clinical application. PDI is based on the combination of three non-toxic elements a photosensitizer (PS), light and molecular oxygen, which lead to the formation of reactive oxygen species (ROS) that cause lethal oxidative damage into the target pathogenic bacteria. For that, clinical approved tetrapyrrolic macrocycles, with particular emphasis on photoactive porphyrin (Por) dyes, which have been used as PS in PDI for different biomedical applications. Two novel unsymmetrical free-base thiopyridyl Pors conjugated with α- or γ-CD units (Pors 2 and 3) were prepared and the corresponding cationic ones (Pors 2a and 3a) were assessed as water-soluble photosensitizer (PS) agents by photophysical, photochemical and E. coli photobiological studies. The presence of the CD unit and the positive charges on the Por periphery (2a and 3a) enhance their solubility in aqueous media. The photoactivity of the two cationic Pors 2a and 3a ensures their potential as PDI drugs against Gram-negative bacteria model, a bioluminescent E. coli, which the best PDI efficiency was determined for Por 3a that achieved the highest bacterial reduction of 4.0 log10 (ANOVA, p less then 0.0001), reaching the detection limit of the method after 15 min.

For best outcome for intramedullary ependymomas, the goal should be gross total resection.1,9,10 These tumors have a relatively distinct plane between tumor and normal parenchyma, making a gross total resection more probable than cases of infiltrative intramedullary astrocytomas.11 Despite this, significant morbidity can be associated with treatment.1 Proper microsurgical technique with use of operative adjuncts can maximize resection while minimizing neurologic injury to optimize outcomes in patients., We present the case of a 42-year-old gentleman presenting with neck and shoulder pain, upper extremity paresthesias, and gait instability. MRI of the neuroaxis revealed a heterogeneously enhancing expansile lesion in the lower medulla and multiple lesions in the thecal sac, representing drop metastases. Due to symptoms and mass effect from the lesion, the patient underwent a suboccipital craniotomy for tumor resection. We highlight operative techniques in our case, including use of neurophysiologic monitoring, intraoperative ultrasound, ultrasonic aspirator, and dissection with micro-cottonoid pledgets and bimanual technique. Pathology revealed a WHO grade II ependymoma. Post-operative MRI revealed a small amount of residual. He underwent full craniospinal proton beam therapy with boost to the residual and metastases with good outcome. Clinical studies have revealed that aberrant RON expression is crucial in regulating TNBC malignant phenotypes. Increased RON expression also has prognostic value for breast cancer progress. These features provide the rationale to target RON for TNBC treatment. In this review, we discuss the importance of RON in TNBC tumorigenesis and the development of anti-RON antibody-drug conjugates (ADCs) for clinical application. The findings from preclinical studies lay the foundation for clinical trials of this novel biotherapeutic for TNBC therapy.Photodynamic inactivation (PDI) of microorganisms have been used for the treatment of bacterial infections. PDI is based on the combination of three non-toxic elements a photosensitizer (PS), light and molecular oxygen, which lead to the formation of reactive oxygen species (ROS) that cause lethal oxidative damage into the target pathogenic bacteria. For that, clinical approved tetrapyrrolic macrocycles, with particular emphasis on photoactive porphyrin (Por) dyes, which have been used as PS in PDI for different biomedical applications. Two novel unsymmetrical free-base thiopyridyl Pors conjugated with α- or γ-CD units (Pors 2 and 3) were prepared and the corresponding cationic ones (Pors 2a and 3a) were assessed as water-soluble photosensitizer (PS) agents by photophysical, photochemical and E. coli photobiological studies. The presence of the CD unit and the positive charges on the Por periphery (2a and 3a) enhance their solubility in aqueous media. The photoactivity of the two cationic Pors 2a and 3a ensures their potential as PDI drugs against Gram-negative bacteria model, a bioluminescent E. coli, which the best PDI efficiency was determined for Por 3a that achieved the highest bacterial reduction of 4.0 log10 (ANOVA, p less then 0.0001), reaching the detection limit of the method after 15 min.0 Comments 0 Shares 16 Views 0 Reviews
