Based on mental healthcare consumption, patients with non-affective psychosis were divided into two groups low (N = 579) and high (N = 488) intensive mental healthcare consumers. Three different techniques from the field of machine learning were applied on crosssectional data to identify risk factors logistic regression, classification tree and a random forest. Subsequently, the same techniques were applied longitudinally in order to predict future healthcare consumption. RESULTS Identified variables that affected healthcare consumption were the number of psychotic episodes, paid employment, engagement in social activities, previous healthcare consumption, and met needs. Analyses showed that the random forest method is best suited to model risk factors, and that these relations predict future healthcare consumption (AUC 0.71, PPV 0.65). CONCLUSIONS Machine learning techniques provide valuable information for identifying risk factors in psychosis. They may thus help clinicians optimize allocation of mental healthcare resources by predicting future healthcare consumption. MATERIAL AND METHODS A questionnaire including 16 single-choice questions about organ transplantation and donation was designed, and a survey was conducted using this questionnaire on 300 randomly selected respondents at Shanghai University of Traditional Chinese Medicine, Shanghai, China. RESULTS The ratio of male to female was 12.4. Of the participants, 98.1% had heard of organ transplantation, and 68.2% had received the information through network news. Only 10.6% expressed a positive attitude toward organ transplantation in China. Further, 39.8% of the participants were willing to donate their organs after death, while 51.9% of the participants hesitated because of perceived opacity in how donated organs were used. Even among those who did not share this concern, only 46.2% considered it. However, 80.7% were in favor of organ donation between relatives. Separately, 67.4% and 60.2% of the participants were interested in participating in activities and publicity related to organ donation, respectively. CONCLUSIONS A majority of the participants had an understanding of organ transplantation and had obtained this information through media. Most showed a positive attitude toward donation between relatives, but only some of them indicated willingness to donate their organs. Generally, acceptance and awareness of organ transplantation need to be improved. Media outreach and education might have a profound influence.   • The prevalence rates of poor self-rated health, disability, mental health disorders and musculoskeletal conditions were significantly higher among adults using food banks than the general population (p less then .05 for all outcomes). These associations remained after accounting for gender, age, marital status, employment and education. This comparative study highlights the many health disadvantages among adults using food bank compared to the general population. The higher risk of poor self-rated health, disability and mental health conditions was not fully explained by measures of low socioeconomic status. Possible explanations for these findings include first, that poor health may increase vulnerability to food insecurity, and, in turn, use of food banks; second, that experiences of food insecurity and food bank use may cause declines in health; and third, that food bank use and poor health may be joint outcomes arising from the last decade of austerity in the UK.Capsule endoscopy is ideally suited to artificial intelligence-based interpretation given its reliance on pattern recognition in still images. Time saving viewing modes and lesion detection features currently available rely on machine learning algorithms, a form of artificial intelligence. Current software necessitates close human supervision given poor sensitivity relative to an expert reader. However, with the advent of deep learning, artificial intelligence is becoming increasingly reliable and will be increasingly relied upon. We review the major advances in artificial intelligence for capsule endoscopy in recent publications and briefly review artificial intelligence development for historical understanding. Importantly, recent advancements in artificial intelligence have not yet been incorporated into practice and it is immature to judge the potential of this technology based on current platforms. Remaining regulatory and standardization hurdles are being overcome and artificial intelligence-based clinical applications are likely to proliferate rapidly.
    Opportunistic infections in chronic lymphocytic leukemia (CLL) have been described in clinical trials, single-center studies, and case reports. We performed a nationwide study to estimate the incidence and impact of inpatient opportunistic infections.

    The incidence rate (IR) and incidence rate ratio (IRR) for Swedish CLL patients diagnosed 1994-2013, and matched controls were calculated, as well as the case-fatality ratio (CFR).

Abiotic stress response mechanisms are receiving considerable attention because global climate change is increasing their abiotic stress levels. However, our knowledge of how S. japonica broadly responds to stress is limited. In this study, we investigated the S. japonica responsive genes underlying acclimation to diverse stressors of acidification, high light, high temperature, hypersalinity, and hyposalinity and identified 408 core genes constantly and differentially expressed in response to all stressors. Our results confirm that stressors had strong effects on genes participating in photosynthesis, amino acid metabolism, carbohydrate metabolism, halogen metabolism, and reactive oxygen species defense. These findings will improve our understanding of brown algal response mechanisms linked to environmental stress and provide a list of candidate genes for improving algal stress tolerance in light of environmental stress in future studies.Discussion on the observed association between unique populations of circulating monocytes and severity of COVID-19.
    The propriety of cold forceps polypectomy (CFP) using jumbo biopsy forceps for diminutive polyps remains controversial. We conducted a prospective study to evaluate the complete CFP resection rate of 3-5-mm polyps using additional endoscopic mucosal resection (EMR) specimens following CFP.

    Patients with 3-5-mm protruded or flat elevated colorectal polyps diagnosed endoscopically as adenomas or serrated lesions were prospectively enrolled. CFP using jumbo biopsy forceps was used to remove the eligible polyps and repeated until the absence of residuals were confirmed via image-enhanced endoscopy or chromoendoscopy. After CFP, saline was injected at the defect, and the marginal specimen of the defect was resected using EMR to histologically evaluate the residue. The primary outcome was the complete CFP resection rate, which was defined as no residue at the EMR site. Other outcomes were the number of CFP bites and the complete resection rate by lesion size.

    Eighty patients with 120 polyps were enrolled. The mean polyp size was 4.1±0.7mm. The overall complete resection rate was 96.7% (95% confidence interval [CI], 91.7-98.7), and the rates for 3-, 4- and 5-mm polyps were 100% (95% CI, 86.7-100), 96.0% (95% CI, 86.5-98.9) and 95.5% (95% CI, 85.1-98.8), respectively. The one-bite CFP rates were 92%, 60% and 31% for the 3-, 4- and 5-mm polyps, respectively.

  • During the COVID-19 pandemic, remote consultations became a new norm for paediatric outpatient clinics. The objective of this survey was to find patients' perspective on telephone consultations. 200 patients, who had remote consultations since April 2020, were surveyed and their responses were analysed. Almost half (98/200) of the patients or their parents preferred remote consultations mixed with face-to-face consultations; only a fifth (40/200) preferred exclusively face-to-face consultations; and approximately a third (62/200) preferred exclusively remote consultations. In conclusion, remote consultations are becoming a popular choice for patients, although there are limitations, especially in the context of safeguarding.
    Excessive administration of antibiotics to preterm infants is associated with increased rates of complications. The purpose of the study was to evaluate the effect of an antimicrobial stewardship intervention on antibiotic use in extremely preterm infants.

    A before and after study of infants born at ≤28 weeks' gestational age was performed in the neonatal intensive care unit of Queen Silvia's Children's Hospital, Gothenburg, Sweden. Retrospective analysis of the baseline period (January-December 2014) guided the development of a limited antimicrobial stewardship intervention. The intervention consisted of updated local guidelines with a focus on shortened and standardised treatment duration plus increased access to infectious disease consultant advice.     Performance of everyday activities is often challenging for older adults. We evaluated a novel computer-based functional skills assessment and training (CFSAT) program, which includes simulations of everyday tasks (e.g., money and medication management).

    The sample included noncognitively impaired (NC) older adults (
    = 51) and cognitively impaired (CI) older adults (
    = 43), who ranged in age from 60 to 86 years (
    73.12;
    6.06), were primarily female (90%), and ethnically diverse (23% Hispanic, 51% African American). Participants (stratified by cognitive status) were randomized to 1 of the 2 conditions training alone (CFSAT) or CFSAT with computerized cognitive training and trained up to 24 training sessions. Task performance, using measures of completion time and efficiency (accuracy/completion time), was evaluated at baseline, the final training session, and immediately posttraining with an alternate form of the CFSAT assessment.

    Both NC and CI participants demonstrated significant performance improvements across all tasks following training (all
    s < .001). The CI participants demonstrated reduced training gains compared to the NC participants (all
    s < .001). Training gains did not vary as a function of training conditions.

    The findings suggest that CFSAT is an efficacious program for assessing and training everyday task performance. CFSAT can ultimately be used as an intervention strategy to enhance functional independence for aging adults with and without cognitive impairments.

    During the COVID-19 pandemic, remote consultations became a new norm for paediatric outpatient clinics. The objective of this survey was to find patients' perspective on telephone consultations. 200 patients, who had remote consultations since April 2020, were surveyed and their responses were analysed. Almost half (98/200) of the patients or their parents preferred remote consultations mixed with face-to-face consultations; only a fifth (40/200) preferred exclusively face-to-face consultations; and approximately a third (62/200) preferred exclusively remote consultations. In conclusion, remote consultations are becoming a popular choice for patients, although there are limitations, especially in the context of safeguarding. Excessive administration of antibiotics to preterm infants is associated with increased rates of complications. The purpose of the study was to evaluate the effect of an antimicrobial stewardship intervention on antibiotic use in extremely preterm infants. A before and after study of infants born at ≤28 weeks' gestational age was performed in the neonatal intensive care unit of Queen Silvia's Children's Hospital, Gothenburg, Sweden. Retrospective analysis of the baseline period (January-December 2014) guided the development of a limited antimicrobial stewardship intervention. The intervention consisted of updated local guidelines with a focus on shortened and standardised treatment duration plus increased access to infectious disease consultant advice.   • Piroplasmosis is a serious debilitating and sometimes fatal disease. Phylogenetic relationships within piroplasmida are complex and remain unclear. In the study, we assessed the relative resolution capabilities of the DNA sequences of the nuclear genes 40S ribosomal protein S5 (RPS5) and mitochondrial DNA Cytochrome c oxidase subunit III (cox3) gene in the phylogeny of Babesia and Theileria species isolates. We demonstrated that by using the cox3 gene can recover a better supported species tree for some Theileria species than when using the nuclear RPS5 gene alone, it tends to intra-specific diversity and considerable inter-specific difference. Additionally, the combined DNA sequences of the nuclear RPS5 and cox3 gene improved the inference of evolutionary relationships among Babesia and Theileria species. The mitochondrial cox3 gene outperforms nuclear RPS5 gene and yields better resolution on the intra-specific diversity of Babesia and Theileria species. However, the combined RPS5 nuclear DNA and cox3 DNA tree had more advantage in the phylogeny of Babesia and Theileria species than that of single gene alone. To elucidate the mechanisms underlying the transition from radial to vertical growth, the first step in melanoma invasion, we developed a zebrafish melanoma model in which constitutive activation of ribosomal protein S6 kinase A1 (Rps6ka1; RSK1) drives tumor invasion. Transcriptomic analysis of ribosomal protein S6 kinase A1-activated tumors identified metabolic changes, including up-regulation of genes associated with oxidative phosphorylation. Vertical growth phase human melanoma cells show higher oxygen consumption and preferential utilization of glutamine compared to radial growth phase melanoma cells. Peroxisome proliferator activated receptor gamma (PPARG) coactivator 1 alpha (PPARGC1A), also known as PPARG coactivator-1α (PGC1α), has been proposed as a master regulator of tumor oxidative phosphorylation. In human primary melanoma specimens, PGC1α protein expression was found to be positively associated with increased tumor thickness and expression of the proliferative marker Ki-67 and the reactive oxygen species scavenger scavenger receptor class A member 3 . PGC1α depletion modulated cellular processes associated with primary melanoma growth and invasion, including oxidative stress. These results support a role for PGC1α in mediating glutamine-driven oxidative phosphorylation to facilitate the invasive growth of primary melanoma. Repetitive transcranial magnetic stimulation (rTMS) is thought to modulate brain function through methods of electromagnetic induction. Over the last few decades, a large body of studies have investigated the clinical applications of rTMS in a variety of patient populations for a diverse range of symptoms from depressive symptomology to post-stroke motor functioning. There is still no clear consensus, however, on how rTMS influences cognitive functioning in the healthy brain. We conducted a quantitative meta-analysis in order to evaluate whether offline rTMS (the delivery of rTMS when not actively engaged in a cognitive task) influences cognition in healthy adults. More specifically, we examined studies that applied rTMS to the dorsal lateral prefrontal cortex (DLFPC) and that tracked cognitive outcomes both before and after a prescribed period of rTMS. Fifteen studies met our inclusion criteria. Cognitive performance was pooled and examined across studies for four cognitive domains (working memory, executive functioning, episodic memory, and visual perception) and under two types of stimulation conditions (excitatory and inhibitory rTMS). Whereas excitatory rTMS was associated with statistically reliable effects for improving executive functioning, inhibitory rTMS was associated with statistically reliable effects for improving episodic memory and visual perception. However, the magnitude of these effects was small and no other significant effects were observed. Though future studies are still needed, our findings suggest that offline forms of rTMS may have limited utility in affecting cognitive functioning when applied to the DLPFC in healthy adults, irrespective of cognitive domain or stimulation type.

  • We found that survival outcomes did not change following the implementation of a "restrictive" transfusion policy. There was also a decrease in the extracorporeal blood flow rates with restrictive transfusion of 0.5 L/min. Nonsurvivors of venovenous extracorporeal membrane oxygenation had higher usage of RBC units following a change in transfusion practice. The restrictive strategy allowed a cost saving of £454 per patient. Conclusions These results suggest that the adoption of a more restrictive approach to RBC transfusion during venovenous extracorporeal membrane oxygenation is more cost-effective and associated with similar survival outcomes, than when compared with a more liberal approach. Data Sources Studies cited in 15 systematic reviews of various topical antibiotic and other infection prevention interventions among ICU patients. Study Selection Studies of topical antibiotics, stratified into concurrent control versus nonconcurrent control designs. Studies of nondecontamination-based infection prevention interventions provide additional points of reference. Studies with no infection prevention intervention provide the mortality benchmark. Data from additional studies and data reported as intention to treat were used within sensitivity tests. Data Extraction Mortality incidence proportion data, mortality census, study characteristics, group mean age, ICU type, and study publication year. Data Synthesis Two-hundred six studies were included. The summary effect milar to a literature-derived benchmark. The unexplained ventilator-associated pneumonia and mortality excess in the concurrent control groups implicates spillover effects within studies of topical antibiotics. Design Retrospective observational study from 2011-2018 with patients followed until hospital discharge. Setting Free-standing children's hospital with an annual ICU volume of more than 3,500 yearly admissions. Patients Pediatric patients, ages 0 to 18 years, admitted to an ICU and who received at least one dose of inhaled or endotracheally instilled tranexamic acid were included. Interventions Inhaled or endotracheally instilled tranexamic acid. Measurements and Main Results This study described the efficacy and adverse effects of patients who received inhaled or endotracheally instilled tranexamic acid. A total of 19 patients met inclusion criteria; median age was 72 months (11-187 mo), most patients were female (11, 58%), and almost half our patients (8, 42%) had congenital heart disease. Nine of 19 encounters (47%) had diffuse alveolar hemorrhage, four (21%) had pulmonary hemorrhage related to major aortopulmonary collateral arteries, two (11%) had mucosal airway bleeding, two (11%) were iatrogenic, one had a pulmonary embolism, and one patient did not have their etiology of pulmonary hemorrhage determined. Cessation of pulmonary hemorrhage was achieved in 18 of 19 patients (95%) with inhaled tranexamic acid with no major adverse events recorded. Conclusions and Relevance We demonstrate that inhaled tranexamic acid may be safely used to treat pulmonary hemorrhage from varied etiologies in critically ill pediatric patients. Prospective studies are required in this vulnerable population to determine optimal dosing and delivery strategies, as well as to define any differential effect according to etiology. Design Retrospective cohort study of the association between hyponatremia and mortality, stratified by outpatient diuretic use in three strata. Setting An inception cohort of 13,661 critically ill patients from a tertiary medical center. Measurements and Main Results Admission serum sodium concentrations, obtained within 12 hours of admission to the ICU, were the primary exposure. Hyponatremia was associated with 1.82 (95% CI, 1.56-2.11; p less then 0.001) higher odds of mortality, yet differed according to outpatient diuretic use (multiplicative interaction between thiazide and serum sodium less then 133 mEq/L; p = 0.002). Although hyponatremia was associated with a three-fold higher (odds ratio, 3.11; 95% CI, 2.32-4.17; p less then 0.001) odds of mortality among those prescribed loop diuretics, no increase of risk waon behalf of the Society of Critical Care Medicine.Objectives We observed that patients treated with continuous vecuronium or esmolol infusions showed elevated plasma sodium measurements when measured by the routine chemistry analyzer as part of the basic metabolic panel (Vitros 5600; Ortho Clinical Diagnostics, Raritan, NJ), but not by blood gas analyzers (RAPIDLab 1265; Siemens, Tarrytown, NY). Both instruments use direct ion-selective electrode technology, albeit with different sodium ionophores (basic metabolic panel methyl monensin, blood gas glass). We questioned if the basic metabolic panel hypernatremia represents artefactual pseudohypernatremia. Design We added vecuronium bromide or esmolol hydrochloric acid to pooled plasma samples and compared sodium values measured by both methodologies. We queried sodium results from the electronic medical records of patients admitted at Children's Hospital of Philadelphia from 2016 to 2018 and received vecuronium and/or esmolol infusion treatment during their admissions. Conclusions These results suggest that the adoption of a more restrictive approach to RBC transfusion during venovenous extracorporeal membrane oxygenation is more cost-effective and associated with similar survival outcomes, than when compared with a more liberal approach. Study Selection Studies of topical antibiotics, stratified into concurrent control versus nonconcurrent control designs. Studies of nondecontamination-based infection prevention interventions provide additional points of reference. Studies with no infection prevention intervention provide the mortality benchmark. Data from additional studies and data reported as intention to treat were used within sensitivity tests. Data Extraction Mortality incidence proportion data, mortality census, study characteristics, group mean age, ICU type, and study publication year. Design Retrospective observational study from 2011-2018 with patients followed until hospital discharge. Setting Free-standing children's hospital with an annual ICU volume of more than 3,500 yearly admissions. Patients Pediatric patients, ages 0 to 18 years, admitted to an ICU and who received at least one dose of inhaled or endotracheally instilled tranexamic acid were included. Interventions Inhaled or endotracheally instilled tranexamic acid. Measurements and Main Results This study described the efficacy and adverse effects of patients who received inhaled or endotracheally instilled tranexamic acid. A total of 19 patients met inclusion criteria; median age was 72 months (11-187 mo), most patients were female (11, 58%), and almost half our patients (8, 42%) had congenital heart disease. Nine of 19 encounters (47%) had diffuse alveolar hemorrhage, four (21%) had pulmonary hemorrhage related to major aortopulmonary collateral arteries, two (11%) had mucosal airway bleeding, two (11%) were iatrogenic, one had a pulmonary embolism, and one patient did not have their etiology of pulmonary hemorrhage determined. Cessation of pulmonary hemorrhage was achieved in 18 of 19 patients (95%) with inhaled tranexamic acid with no major adverse events recorded. Conclusions and Relevance We demonstrate that inhaled tranexamic acid may be safely used to treat pulmonary hemorrhage from varied etiologies in critically ill pediatric patients. Prospective studies are required in this vulnerable population to determine optimal dosing and delivery strategies, as well as to define any differential effect according to etiology. Hyponatremia was associated with 1.82 (95% CI, 1.56-2.11; p less then 0.001) higher odds of mortality, yet differed according to outpatient diuretic use (multiplicative interaction between thiazide and serum sodium less then 133 mEq/L; p = 0.002). Because the construction of non-sustainable (engineered) dams is extremely costly, the only possible way to mitigate flood risk in these critical zones would be to couple flood attenuation with hydroelectric use, or through the implementation of an extensive reforestation program in the catchment with the purpose to increase evapotranspiration and reduce runoff. Harmful algal blooms (HABs) of Cochlodinium (aka Margalefidinium) polykrikoides cause huge economic and ecological damages and thus are considered environmental problems. Previous studies uncovered that the formation and collapse of phytoplankton blooms could be closely related to their associated microbes although their roles in C. polykrikoides bloom have not been elucidated yet. To explore the potential interactions between C. polykrikoides and other microbes (archaea, bacteria, and phytoplankton), we collected water samples in the free-living (FL) (0.22 to 3 μm), nanoparticle-associated (NP) (3 to 20 μm), and microparticle-associated (MP) (>20 μm) fractions when C. polykrikoides blooms occurred from July to August in 2016, 2017, and 2018 in the South Sea of Korea. The microbial composition of the C. polykrikoides-associated microbial cluster (Module I) significantly differed from those of other modules associated with Alexandrium, Chaetoceros or Chattonella. Data were collected from 624 households in the Dammam Metropolitan Area, Saudi Arabia using a structured questionnaire and analyzed using descriptive and inferential statistics. The results from logistic regression indicates that the likelihood of a household to accept reusing treated mixed wastewater is influenced by gender with odds ratio (OR) of 2.71-2.18, residential location (OR = 1.32-1.03), age (OR = 1.22-0.18) and educational level (OR = 1.33-0.98), with a tendency for more acceptance of treated grey wastewater than mixed wastewater. These findings showcase the difficulty that the country could face concerning the public acceptance of treated wastewater for non-domestic uses to augment current freshwater sources even among the educated class. This study is significant because sustainably meeting the country's rising water demands requires the stringent implementation of strategic wastewater reuse policy, including bold steps towards wastewater streams segregation, and intensive public awareness campaigns to change negative perceptions on treated sewage effluent. This study concludes that a substantial reduction in the country's reliance on costly desalinated water and fast depleting non-renewable groundwater requires complete reuse and recycling of treated wastewater for wider non-conventional purposes. Deep high-altitude mountain lakes can act as a natural laboratory, and have the potential to contribute ecological data for understanding the way natural climate and anthropogenic changes that can affect the ecosystems. We present a multi-proxy record from a sediment core from such a lake (Lake Lugu) in southwest China with emphasis on the changes in the testate amoebae community, along with sedimentological data (magnetic susceptibility, total organic carbon/TOC and total nitrogen/TN) over the last 2500 years. In total, 29 testate amoebae species belonging to eight genera (Arcella, Centropyxis, Cyphoderia, Difflugia, Netzelia, Phryganella, Pseudodifflugia and Zivkovicia) were identified. Three stages were clearly defined for the lake based on testate amoebae community. The first stage dated to about 500 BCE-800 CE, with the testate amoebae community dominated by Centropyxis and influenced by soil erosion. The second stage (about 800-1920 CE) was characterized by a gradual increase of TOC and TN and an abrupt. Transport and transformation of inorganic nutrients are influenced by abiotic-biotic interactions and determine downstream water quality. Macrophytes play an important role in these complex ecological interactions. The role of macrophytes was studied in three reaches of the groundwater-fed, oligotrophic River Fischa with different macrophyte coverage and biomass. This was done by measuring metabolism and calculating changes in nutrient loading and concentrations, which were determined via an upstream-downstream mass balance approach. Cytokine stability has also been addressed previously using plasma from normal individuals under different conditions of anticoagulant use, storage time and temperature of storage. No studies have as yet been undertaken to address cytokine stability in critically ill patients which may differ from normal, healthy individuals due to underlying cofounders such as inflammation.

    To our knowledge, this is the first series on adult males with classic 21-OHD which concomitantly assesses clinical presentation, molecular biology, adrenal and testicular imaging studies, semen analysis and compliance to treatment. A high prevalence of adrenal masses and TARTs was observed, possibly associated with poor treatment compliance leading to elevated ACTH and increased proliferation. Our findings on TARTs agree with reports in international publications of CAH in males, with adrenal imaging being added in our group. Although we are aware that further studies with a larger sample size and more data are needed, we consider that our findings contribute to the clinical management of classical 21-OHD in the male population.
    Curcuma wenyujin is a multifunctional medicinal plant belonging to the ****** family (Zingiberaceae). It has been used to treat blood stasis, promote the flow of qi, dredge the meridians, and relieve pain for more than 1500 years. Its raw rhizomes, steamed rhizomes, and steamed roots constitute three herbal medicines currently listed in the Chinese Pharmacopoeia pian-jiang-huang (), wen-e-zhu () and wen-yu-jin (), respectively.

    The aim of this review was to comprehensively summarize the traditional use, phytochemistry, and pharmacology of C. wenyujin in order to provide theoretical support for its further investigation and utilization.

    Multiple databases (Scifinder, CNKI, Web of Science, PubMed, Google Scholar, and Baidu Scholar) were searched. Some information was also obtained from the literatures on traditional Chinese medicine.

    A total of 169 compounds have been isolated from C. wenyujin so far. Sesquiterpenoids are the major constituents and are crucial chemotaxonomic markers. Modern pharmacological studies and clinical trials have demonstrated that the extracts or active compounds from C. wenyujin have anti-inflammatory, antitumor, antioxidant, antibacterial, antiviral, and hepatoprotective properties.

    Until now, significant progress has been witnessed in phytochemistry and pharmacology of C. wenyujin. Some traditional uses of C. wenyujin have been supported by modern pharmacological studies. However, the establishment of quality control standards and additional clinical studies are warranted.

(A. fragrans) is used to treat infection of the lower genital tract in gynecology, such as cervicitis and pelvic inflammatory disease. This study analyzed the therapeutic efficiency of A. fragrans on cervicitis and the inhibition mechanism of AF-p2 in MALP-2-stimulated RAW264.7 cells. Materials and methods The anti- Ureaplasma urealyticum (Uu) activity of A. fragrans and AF-p2 were determined by antimicrobial susceptibility testing. The activity of A. fragrans extracts (AFext) was evaluated in female BALB/c **** with cervicitis induced by Uu. Furthermore, the therapeutic mechanism of AFext and AF-p2 on myeloid differentiation factor 88 (MyD88) pathway were studied in macrophage activating lipopeptide-2 (MALP-2) irritated RAW264.7 cells. Results AFext could suppress the proliferation of Uu in vitro, including the azithromycin resistant strains. Meanwhile, AFext prevented cervicitis caused by Uu infection in BALB/c ****. Moreover, both AFext and AF-p2 could significantly suppress the nitric oxide (NO) production as well as other proinflammatory cytokines (IL-1β,IL-6,TNF-α) in MALP-2 stimulated RAW264.7 cells. Moreover, AF-p2 also down-regulated iNOS, p65, Iκ-Bα, MyD88 and cyclooxygenase-2 (COX-2) levels in RAW264.7 cells. Conclusion This study indicated that AFext had a therapeutic effect in cervicitis induced by Uu infection.   • Findings from the present study highlight the importance of investigating individual variability and its contributing factors in future studies on neural responses of mindfulness interventions, as well as the distinct responses of the left and right amygdala.We have previously shown that activity and connectivity within and between the action observation and mentalizing brain systems reflect the degree of positive dimensions expressed by social interactions such as cooperativity and affectivity, respectively. Here we aim to extend this evidence by investigating the neural bases of processing negative dimensions of observed interactions, such as competition and affective conflict, possibly representing a benchmark for different pathological conditions. In this fMRI study 34 healthy participants were shown pictures depicting interactions characterized by two crossed dimensions, i.e. positively- vs. negatively- connotated social intentions mainly expressed in terms of motor acts vs. mental states, i.e. cooperative, competitive, affective and conflicting interactions. We confirmed the involvement of the action observation and mentalizing networks in processing intentions mainly expressed through motor acts (cooperative/competitive) vs. mental states (affective/conflicting), respectively. Results highlighted the selective role of the left pSTS/TPJ in decoding social interactions, even when compared with parallel actions by non-interacting individuals. Its right-hemispheric homologue displayed stronger responses to negative than positive social intentions, regardless of their motor/mental status, and decreased connectivity with the medial prefrontal cortex (mPFC) when processing negative interactions. The resulting mPFC downregulation by negative social scenes might reflect an adaptive response to socio-affective threats, via decreased mentalizing when facing negative social stimuli. This evidence on the brain mechanisms underlying the decoding of real complex interactions represents a baseline for assessing both the neural correlates of impaired social cognition, and the effects of rehabilitative treatments, in neuro-psychiatric diseases or borderline conditions such as loneliness.

In this review, we focus on how infectious diseases of wild and managed bees can be used as a OneHealth model system, informing fundamental questions on ecological immunology and disease transmission, while addressing how this knowledge can be used to tackle the issues facing pollinator health.Fungal keratitis (FK) accounts for approximately half of the microbial keratitis encountered in low middle income countries (LMICs) and predominantly affect the working rural-poor. FK causes significant morbidity with the majority of patients left with moderate or worse visual impairment and approximately 25% requiring expensive and often unsuccessful surgical interventions. The severity of FK and the resultant corneal damage or resolution can be attributed to i) the virulence and bioburden of the fungal pathogen, ii) the host defense mechanism and immune response and iii) sub-optimal diagnostics and anti-fungal treatment strategies. This review provides a comprehensive overview of the multifaceted components that drive FK progression and resolution, highlighting where knowledge gaps exist and areas that warrant further research.

  • BACKGROUND We evaluated the association of antiretroviral therapy (ART), CD4+ count and HIV plasma viral load (PVL) on high-grade cervical intraepithelial neoplasia (CIN2+) detection at follow-up after CIN management among women living with HIV (WLHIV). METHODS Medline, Embase, Global Health and PubMed were searched from January 1, 1996 to January 15, 2020. Eligible studies investigated the association of ART, CD4+ count or HIV PVL on histology-confirmed CIN2+ detection at follow-up. Summary estimates were obtained using random-effects meta-analyses; heterogeneity was examined using I2 statistic. PROSPERO registrationCRD42018115631. RESULTS Eight studies representing 9 populations were identified, including 1,452 WLHIV followed between 6 to 33 months post-CIN management. METHODS After propensity score matching, we studied 340 KTRs from the French Données Informatisées et Validées en Transplantation cohort and 605 non-transplant patients with CKD (CKDps) from the French Chronic Kidney Disease-Renal Epidemiology and Information Network cohort. The mean ± standard deviation eGFR was 42 ± 13 and 41 ± 12 mL/min/  1.73 m2, respectively (P = 0.649). In multivariable analysis, KTRs were found to have a 2.7-fold greater risk of mortality [hazard ratio 2.7 (95% confidence interval 1.6-4.7); P = 0.005]. There was no between-group difference concerning the risk of CV events (P = 0.448). CV death rates in KTRs (29.0%) approximated those of CKDps (22.5%), whereas death rates due to infections were higher in KTRs (19.4% versus 10.0%). CONCLUSION Beyond 1 year after transplantation, KTRs, who possibly had a longer CKD history, had a significantly greater mortality risk than eGFR-matched CKDps. In this study, we analyzed complications following abdominoplasty in post-bariatric patients and evaluated potential risk factors associated with these complications. OBJECTIVES The authors sought to determine the complications and risk factors following abdominoplasty in post-bariatric patients. METHODS A retrospective study of patients undergoing abdominoplasty was performed from January 2009 to December 2018 at our institution. Variables analyzed were sex, age, body mass index (BMI), smoking, surgical technique, operative time, resection weight, drain output, and complications. RESULTS 406 patients were included in this study (320 female and 86 male) with a mean age of 44.4 years and a BMI of 30.6 kg/m2. Abdominoplasty techniques consisted of traditional (64.3%), fleur-de-lis technique (27.3%), and panniculectomy without umbilical displacement (8.4%). Overall complications recorded were 41.9%, the majority of these were wound healing problems (32%). Minor and major complications were found in 29.1% and 12.8% of patients, respectively. BMI ≥ 30 kg/m2 was associated with an increased risk for wound healing problems (p=0.001). The frequency of total complications was significantly related to age (p=0.007), BMI (p=0.004) and resection weight (p=0.001). Abdominoplasty technique tended to influence total complications. CONCLUSIONS This study demonstrates in a fairly large sample of post-bariatric patients (N=406), that abdominoplasty alone can be performed safely, with an acceptable complication rate. Age, BMI, and resection weight are shown to be significant risk factors for total complications. The role of surgical technique needs to be evaluated further. © 2020 The Aesthetic Society. Since the immunosuppressant cyclosporine-A produces pEMT in TECs and inhibits the peptidyl-prolyl isomerase (PPIase) activity of cyclophilin (Cyp) proteins, we hypothesized that cyclophilins could regulate TEC phenotype. Here we demonstrate that in cultured TECs, CypA silencing triggers loss of epithelial features and enhances TGFβ-induced EMT in association with up-regulation of epithelial repressors Slug and Snail. This pro-epithelial action of CypA relies on its PPIase activity. By contrast, CypB emerges as an epithelial repressor, since CypB silencing promotes epithelial differentiation, prevents TGFβ-induced EMT, and induces tubular structures in 3D cultures. In addition, in the kidneys of CypB knockout **** subjected to unilateral ureteral obstruction (UUO), inflammatory and pro-fibrotic events were attenuated.
