  • This review addresses current views on prevention of infectious endocarditis (IE). History of establishing the concept of antibacterial prophylaxis (ABP), major approaches, and substantiation of changes in ABP in recent years are described. Recent international and national guidelines are highlighted, specifically, guidelines of the European Society of Cardiologists, American Heart Association/American College of Cardiology, and the Japanese Circulation Society. The review presents critical evaluation of previously approved international guidelines, including analysis of the effect of partial or complete ABP restriction on IE morbidity and incidence of complications. Special attention is paid to awareness of practitioners, particularly dentists, about ABP issues in their practice. Aspects of validity and key features of preventive approaches in implanting cardiac electronic devices and transcatheter aortic valve implantation are discussed.Echocardiography allows evaluating left ventricular (LV) myocardial contractility; however, the visual assessment of contractility is subjective and requires considerable experience. Modern technologies for assessment of LV myocardial contractility, such as tissue Doppler and speckle-tracking echocardiography, provide quantitative estimation of various parameters of myocardial strain, including the LV postsystolic shortening. Several studies have demonstrated the value of postsystolic shortening for evaluation of myocardial ischemia and "ischemic memory" in patients with cardiovascular diseases. This review analyzes experimental and clinical studies that addressed LV postsystolic shortening.Asymptomatic hyperuricemia (HU) is widespread in the population. Results of multiple studies have demonstrated independent associations between increased levels of uric acid and risk of arterial hypertension, cardiovascular diseases, and chronic kidney disease. Despite the extensive study of this issue, there is still no unified answer to questions regarding the necessity of urate-lowering therapy in asymptomatic HU, whereas results of studies on the effect of this therapy on outcomes of cardiovascular and kidney diseases are controversial.   • Newly introduced heat-not-burn or electronic cigarettes can cause lethal nicotine intoxication if ingested at higher doses. Although routine gastric lavage is not recommended, it should be considered if the amount of intoxicant is lethal. A 59-year-old man with a history of depression was brought to our emergency department after intentional ingestion of 8 heat-not-burn cigarettes, which were estimated to contain a total of 100 mg of nicotine. Abdominal computed tomography confirmed the gastric contents, detecting multiple stick-like and rod-shaped high-density structures. Gastric lavage was performed to minimize absorption of the potentially lethal nicotine dose. The patient exhibited only mild gastrointestinal symptoms. Emergency physicians should be aware of this novel heat-not-burn cigarette and its toxicity.
    Despite the growing investment in and adoption of artificial intelligence (AI) in medicine, the applications of AI in an emergency setting remain unclear. This scoping review seeks to identify available literature regarding the applications of AI in emergency medicine.

    The scoping review was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines for scoping reviews using Medline-OVID, EMBASE, CINAHL, and IEEE, with a double screening and extraction process. The search included articles published until February 28, 2020. Articles were excluded if they did not self-classify as studying an AI intervention, were not relevant to the emergency department (ED), or did not report outcomes or evaluation.

    Of the 1483 original database citations, 395 were eligible for full-text evaluation. Of these articles, a total of 150 were included in the scoping review. The majority of included studies were retrospective in nature (n = 124, 82.7%), with only 3 (2.0%) proographic imaging and prediction-based diagnoses. Higher quality evidence is needed to further assess both short- and long-term clinical outcomes.

    We conducted a retrospective cohort analysis using a 2016 nationwide emergency department (ED) sample. Patient factors considered for analysis were the following age, sex, acuity, chronic diseases, weekend visit, quarter of presentation, median household income quartile for patient's zip code, primary/secondary insurance, total charges for the visit, and urban/rural household. Hospital factors considered were urban/rural location, trauma center/teaching hospital, and annual ED volume. Multivariable logistic regression was used to find significant predictors and their interactions. A random forest algorithm was used to determine the order of importance of factors.

    A total of 32,680,232 hospital-based ED visits with 466,047 incidences of leaving without being seen were included. The cohort comprised 55.5% females, with a median (IQR) age of 3 and non-modifiable risk factors for leaving without being seen established previously while rejecting the importance of others.
    Accurate triage in the emergency department (ED) is critical for medical safety and operational efficiency. We aimed to predict the number of future required ED resources, as defined by the Emergency Severity Index (ESI) triage protocol, using natural language processing of nursing triage notes.

    We constructed a retrospective cohort of all 265,572 consecutive ED encounters from 2015 to 2016 from 3 separate clinically heterogeneous academically affiliated EDs. We excluded encounters missing relevant information, leaving 226,317 encounters. We calculated the number of resources used by patients in the ED retrospectively and based outcome categories on criteria defined in the ESI algorithm 0 (30,604 encounters), 1 (49,315 encounters), and 2 or more (146,398 encounters). A neural network model was trained on a training subset to predict the number of resources using triage notes and clinical variables at triage. Model performance was evaluated using the test subset and was compared with human ratings.

    Overall model accuracy and macro F1 score for number of resources were 66.5% and 0.601, respectively. The model had similar macro F1 (0.589 vs 0.592) and overall accuracy (65.9% vs 69.0%) compared to human raters. Model predictions had slightly higher F1 scores and accuracy for 0 resources and were less accurate for 2 or more resources.

    Machine learning of nursing triage notes, combined with clinical data available at ED presentation, can be used to predict the number of required future ED resources. These findings suggest that machine learning may be a valuable adjunct tool in the initial triage of ED patients.

We sought to determine how often practicing emergency physicians in academic, community and freestanding emergency departments (EDs) perform 4 procedures central venous catheterization (CVC), tube thoracostomy, tracheal intubation, and lumbar puncture (LP).

    This was a retrospective study evaluating emergency physician procedural performance over a 12-month period. We collected data from the electronic records of 18 EDs in one healthcare system. The study EDs included higher and lower volume, academic, community and freestanding, and trauma and non-trauma centers. The main outcome measures were median number of procedures performed. We examined differences in procedural performance by physician years in practice, facility type, and trauma status.

  • The correlation between apical extrusion and surface tension was compared using Pearson's coefficient.

    The least amount of apically extruded debris was with EDTA-gel, peracetic acid and citric acid which were similar to each other. NaOCl had the highest surface tension whereas peracetic acid and EDTA-liquid had the lowest. There was no significant correlation exists between apical extrusion and surface tension.

    The permanent canine is the most widely used tooth for sex estimation because it is the tooth with the highest degree of sexual dimorphism; however, there are several factors that can limit the analysis (e.g., pathologies, postmortem loss). The aim of this work was to analyse the correlation between the dimensions of the canines and those of the premolars and molars, and evaluate the correspondence of real and predicted canine dimensions by applying the equations developed.

    The sample was composed of digital models of 80 adult individuals from Temuco, Chile. The buccolingual and mesiodistal diameters of the canine, premolar and molar dental crowns were measured and Pearson's linear regression analysis was performed in order to determine the correlation between the diameters. The equations obtained from the reference subsample were applied to a validation subsample to predict canine dimensions from the dimensions of the postcanine teeth.

    Four regression equations were obtained, all for prediction of the size of the lower canines, whose correlation coefficient ranged from 0.701 to 0.738. The regression equations developed with the reference sample were tested on the validation sample using a Student's t-test for paired samples and the intraclass correlation coefficient. The differences between actual dental size and that predicted by the equations were not significantly different, and concordance analysis showed a moderate degree (0.485-0.585).

    Previous studies have suggested that mouth breathing has harmful effects on atopic dermatitis (AD) and oral health in children, but the evidence has been insufficient. To investigate the association of mouth breathing with AD and oral health in Korean schoolchildren aged 8-11 years.

    Cross-sectional data were obtained from March to April 2016. A questionnaire was used to investigate children's mouth breathing habits and personal/family histories related to allergic disease. The aim of this meta-analysis was to evaluate long-term three dimensional skeletal effects on maxilla through face mask (FM) with or without rapid maxillary expansion (RME) in skeletal CIII growing patients.

Inclusion criteria were randomized controlled trials or cohort studies recruiting growing patients who received maxillary protraction and/or expansion and comparing the treatment groups with untreated controls. The follow-up periods were more than 3 years. Risk of bias was assessed using the Cochrane tools (RoB2.0 and ROBINS-I). GRADE was used to qualify the evidence.

    This meta-analysis included 6 studies comprising 327 participants in total. No statistically significant changes were observed on the degree of Sella-Nasion-A point (SNA) in the treated groups when compared with the untreated controls. However, significant increase on maxillary rotation degree (mean difference 8.20, 95% CI = 6.87-9.53,
     < 0.001) and maxillary base width (mean difference 2.27, 95% CI = 1.39-3.15,

    The correlation between apical extrusion and surface tension was compared using Pearson's coefficient. The least amount of apically extruded debris was with EDTA-gel, peracetic acid and citric acid which were similar to each other. Finally, it calls the true inversions by a trained support vector machine (SVM) classifier. CONCLUSIONS Our results on real sequence data from the 1000 Genomes Project show that by combining feature mining and a machine learning model, InvBFM outperforms existing tools. RESULTS This retrospective study comprised a total of 62 paraffinized cervical biopsies, which were distributed in five groups low-grade squamous intraepithelial lesions (LSIL), high-grade squamous intraepithelial lesions (HSIL), squamous cervical carcinoma (SCC), uterine cervical adenocarcinoma (UCAC), and a group of cervicitis (as a control for non-abnormal/non-transformed cells). Cervical sections were stained by immunohistochemistry to explore the expression of B7-H6, which was reported according to the immunoreactive score (IRS) system. We observed a complete lack of B7-H6 in LSIL abnormal epithelial cells. Interestingly, B7-H6 began to be seen in HSIL abnormal epithelial cells; more than half of this group had B7-H6 positive cells, with staining characterized by a cytoplasmic and membranous pattern. B7-H6 in the SCC group was also seen in the majority of the sections, showing the same cytoplasmic and membranous pattern. Strong evidence of B7-H6 was notably found in UCAC tumor columnar cells (in 100% of the specimens, also with cytoplasmic and membranous pattern). Moreover, consistent B7-H6 staining was observed in infiltrating plasma cells in all groups. Therefore, this study aimed to characterize DGAT family genes in buffalo and to find candidate markers or DGAT genes influencing lactation performance. RESULTS We performed a genome-wide study and identified eight DGAT genes in buffalo. All the DGAT genes classified into two distinct clades (DGAT1 and DGAT2 subfamily) based on their phylogenetic relationships and structural features. Chromosome localization displayed eight buffalo DGAT genes distributed on five chromosomes. Collinearity analysis revealed that the DGAT family genes were extensive homologous between buffalo and cattle. Afterward, we discovered genetic variants loci within the genomic regions that DGAT genes located in buffalo. Seven haplotype blocks were constructed and were associated with buffalo milk production traits. Single marker association analyses revealed four most significant single nucleotide polymorphisms (SNPs) mainly affecting milk protein percentage or milk fat yield in buffalo. Genes functional analysis indicated that these DGAT family genes could influence lactation performance in the mammal through regulating lipid metabolism. CONCLUSION In the present study, we performed a comprehensive analysis for the DGAT family genes in buffalo, which including identification, structural characterization, phylogenetic classification, chromosomal distribution, collinearity analysis, association analysis, and functional analysis. We estimated that, to prevent local outbreaks to escalate to major widespread epidemics, stringent NPIs need to remain in place at least one year after the start of vaccination. Should NPIs be capable to keep the reproduction number (Rt) around 1.3, vaccination could reduce up to 99% of COVID-19 burden and bring Rt below the epidemic threshold in 9 months. Even optimistic estimates suggest that most countries will likely take 6 to 24 months to vaccinate their citizens. These time estimates and the emergence of new viral strains urge us to find quick and effective ways to allocate the vaccines and contain the pandemic. We found robust antibody durability out to six months, as well as significant positive associations with the magnitude of the neutralizing antibody response and male sex. We also show that SARS-CoV-2 convalescent neutralizing antibodies are higher in individuals with cardio-metabolic comorbidities. In this study we found that neutralizing andependently and significantly associated with male sex compared to female sex. We also show for the first time, that higher convalescent antibody titers in male donors are associated with increased age and symptom grade.     Previous studies have related vitamin D supplementation to a lower risk of acute respiratory tract infection. Emerging evidence suggests that vitamin D insufficiency is related to a higher risk of coronavirus disease 2019 (COVID-19) infection.

    We aimed to investigate the prospective association between habitual use of vitamin D supplements and risk of COVID-19 infection, and assess whether such an association differed according to the different levels of circulating and genetically predicted vitamin D.

    This study included 8297 adults who have records of COVID-19 test results from UK Biobank (from 16 March 2020 to 29 June 2020). The use of vitamin D supplements, circulating vitamin D levels, and main covariates were measured at baseline (2006-2010). Genetically predicted vitamin D levels were evaluated by genetic risk score.

    After adjustment for covariates, the habitual use of vitamin D supplements was significantly associated with a 34% lower risk of COVID-19 infection (OR, 0.66; 95% CI, 0.45-0.97; P=0.034). Circulating vitamin D levels at baseline or genetically predicted vitamin D levels were not associated with the risk of COVID-19 infection. The association between the use of vitamin D supplements and the risk of COVID-19 infection did not vary according to the different levels of circulating or genetically predicted vitamin D (P-interactions=0.75 and 0.74, respectively).

Recent technological developments have advanced our understanding of nuclear organization and gene expression control in trypanosomes, opening novel research avenues. This review is focused on Trypanosoma brucei nuclear organization and how it impacts gene expression, with an emphasis on antigen expression. It highlights several dedicated sub-nuclear bodies that compartmentalize specific functions, whilst outlining similarities and differences to more complex eukaryotes. Patients were excluded if they had a background of otological problems including previous acute otitis media, chronic otitis media, otological surgery, and trauma or radiotherapy to the head and neck.

    Otitis media was found in eight patients with coronavirus disease 2019 and no background of otological problems. Six patients had middle-ear effusion, three had typical signs of acute otitis media, and one had a tympanic membrane perforation. Most patients had hearing loss; conductive hearing loss and mild sensorineural hearing loss at high frequencies were the underlying mechanisms.

We calculated the infection growth factor, doubling times of infection and death cases, reproductive number and infection and death cases in relation to the blood type distribution. The growth factor of infection and death cases significantly and positively correlated with the proportion of the population with blood type A and negatively correlated with the proportion of the population with blood type B.

    Acute lymphoblastic leukaemia with mixed lineage leukaemia gene rearrangement (MLL-ALL) frequently affects infants and is associated with a poor prognosis. Primary refractory and relapsed disease due to resistance to glucocorticoids (GCs) remains a substantial hurdle to improving clinical outcomes. In this study, we aimed to overcome GC resistance of MLL-ALL.

    Using leukaemia patient specimens, we performed bioinformatic analyses to identify target genes/pathways. To test inhibition of target pathways in vivo, we created pre-clinical therapeutic mouse patient-derived xenograft (PDX)-models by transplanting human MLL-ALL leukaemia initiating cells (LIC) into immune-deficient NSG ****. Finally, we conducted B-cell lymphoma-2 (BCL-2) homology domain 3 (BH3) profiling to identify BH3 peptides responsible for treatment resistance in MLL-leukaemia.

The objective of present study was to understand the metal dissolution from PVs under four standard waste characterization regulatory tests of U.S., Germany, and Japan and their representativeness with actual landfill leachate. Modules were exposed to real municipal solid waste (MSW) landfill leachate for extended extraction duration, agitation and diluted leachate to investigate the effect of various parameters on metal dissolution. The results indicated that extractions using landfill leachates resulted in lower metal release than standard methods. The leached metal concentrations were found to be within the threshold limits except for cadmium, copper, lead and selenium, with maximum lead release from amorphous-PV of 8.68 mg/L and 6.91 mg/L with respect to TCLP and WET tests, respectively. Arsenic showed negligible release with maximum concentration of 0.046 mg/L from copper indium gallium de-selenide(CIGS) PV. Regardless of small size (1-2 cm pieces) and agitation, Germany and Japan's standard tests resulted in minimal release except of copper from copper indium gallium de-selenide PV. Leaching without agitation, showed negligible release from all photovoltaics whereas when agitation is applied to diluted leachate, significant release was observed with aluminum and copper leached up to 145.32 mg/L (multi-crystalline silicon) and 139.01 mg/L (amorphous-PV), respectively. CIGS was found to be most hazardous with a Metal Hazard Score (calculated on the basis of magnitude of leached metals with respect to their threshold limit and subsequent health effects) of 23.19, when exposed to standard tests. For all scenarios, increased metal release was observed with decrease in sample size and increase in leachate dilution and thus, leaching in highly acidic conditions are by no means representative for modules dumping in realistic conditions.Biogas production is a relatively novel and developing branch of the renewable fuel sector, which allows agricultural waste, and more, to be used as a feedstock. New technologies have been integrated into the process to improve its efficiency. In this study, a pump-mixed anaerobic digestion concept is considered for both experimental and modeling approaches. The experiment included a total of nine configurations with the same geometry (140 dm3 of total reactor volume) but different hydraulic retention times and mixing intervals. The measurements were used to create and optimize a mathematical model. The complete-stirring assumption, which underlies most anaerobic digestion (AD) simulations, is no longer valid in this case. Thus, the novel concept is developed by assuming that the liquid phase is split into three separate sections, which approximates the concentration gradient in a real reactor. In this study, quantitative structure-activity relationship (QSAR) models for the prediction of toxicity of binary mixtures towards bioluminescent bacteria Vibrio fischeri were developed at three toxicity levels (EC10, EC30 and EC50). For model development, experimentally determined toxicity values of 14 pollutants (pharmaceuticals and pesticides) were correlated with their structural features, applying multiple linear regression together with genetic algorithm. Statistical analysis, internal validation and external validation of the models were carried out. The toxicity is accurately predicted by all three models.

UV-vis absorption, CL spectrum, ESR, and radical scavenger experiments were conducted for clarifying the catalytic mechanism of Co MOFs. All results revealed that Co MOFs can accelerate decomposition of H2O2 and production of OH•, O2•-as well as 1O2 radicals. The rapid reaction between these reactive oxygen species and ABEI resulted in the generation of ABEI-ox∗. The excited-state oxidation product emitted a very intensive CL signal with a maximal emission wavelength of 430 nm as it returned to the ground state. To explore their application potential in CL assay, Co MOFs were used as powerful CL reaction catalyst for establishing a very sensitive method for immunoassay of aflatoxin B1. The detection range was 0.05-60 ng mL-1, and the limit of detection was 4.3 pg mL-1. The result for detecting herbal medicine samples demonstrates the acceptable reliability of the Co MOFs-based CL immunoassay. The nanoprobe was rationally designed with a double-flapped dumbbell conformation, where its 5' flap was produced with DNA-AgNCs, and the 3' flap was elongated by a guanine-rich enhancer sequence (GRS). Rigidified by the DNA scaffold, DNA-AgNCs and the GRS are in close proximity, resulting in high fluorescence because of the GRS-induced activation of DNA-AgNCs. Upon the addition of FEN1, the 5' flap of the nanoprobe is cleaved due to the structure-specific endonuclease activity of FEN1. This cleavage released the DNA-AgNCs from the nanoprobe, broke the proximity between DNA-AgNCs and the GRS, and caused decreased fluorescence. This nanoprobe can be applied in the sensitive detection of FEN1 with a detection limit of 40 fM, and it showed high specificity for the monitoring of FEN1 in clinical samples. Compared with other amplification strategies, RCA has many obvious advantages. Firstly, because of the strict complementarity required in ligation of a padlock probe, it endows the RCA reaction with high specificity and can even be utilized to distinguish single base mismatches. Secondly, through the introduction of multiple primers, exponential amplification can be achieved easily and leads to a good sensitivity. Thirdly, RCA products can be customized by manipulating circular templates to generate functional nucleic acids such as aptamer, DNAzymes and restriction enzyme sites. Moreover, the RCA has good biocompatibility and is especially suitable for in situ detection. Therefore, RCA has attracted considerable attention as an efficient and potential tool for highly sensitive detection of biomarkers. Herein, we comprehensively introduce the fundamental principles of RCA technology, summarize it from three aspects including initiation mode, amplification mode and signal output mode, and discuss the recent application of RCA-based biosensor in this review.As an important protease, trypsin (TRY) has been identified as a key indicator of various diseases. A simple and sensitive strategy for TRY detection by using an environment-friendly biosafe probe is significant. Herein, we introduced negatively charged fluorescent polydopamine nanoparticles (PDNPs) with 4.8 nm diameter obtained through a controllable method as an effective probe for TRY. PDNPs exhibited excellent fluorescence property but integrated with protamine (Pro) to form an aggregation-caused quenching system via a static quenching mechanism. The quenching mechanism of Pro to PDNPs revealed the significant effect of the surface charge, functional groups, and appropriate size of PDNPs on quenching process. Given the specific hydrolysis of Pro by TRY, PDNPs were released from the quenching integration of PDNPs and Pro (PDNPs-Pro) and recovered their fluorescence. Thus, a fluorescence sensor for TRY with a linear range of 0.01 and 0.1 μg/mL and a detection limit of 6.7 ng/mL was developed without the disturbing from other proteases.
