However, it is not clear whether RXRα has any role in intramuscular preadipocyte of goat. In the current investigation, we report that adenovirus overexpression of RXRα promotes lipid accumulation in intramuscular adipocyte and up-regulates the expression of positive indicator genes (PPARγ and GLUT4). Next, knockdown of RXRα does not affect adipogenesis of intramuscular adipocyte. This phenomenon may be caused by the compensatory increase of RXRγ. In addition, we found that KLF8 mediates the positive role of RXRα. Knockdown of KLF8 by siRNA attenuated the effect of RXRα on adipocyte differentiation. Importantly, the promoter activity of KLF8 is enhanced when overexpression of RXRα. Taken together, our data suggest that RXRα promotes the differentiation of intramuscular preadipocytes in goat through targeting KLF8, but it could be replaced by RXRγ. These results provide new insights into the quality improvement of goat meat.Globally, access to good quality abortion services and post-abortion care is a critical determinant for women's survival after unsafe abortion. Unsafe abortions account for high levels of maternal death in Kenya. We explored women's experiences and perceptions of their abortion and post-abortion care experiences in Kenya through person-centred care. This qualitative study included focus group discussions and in-depth interviews with women aged 18-35 who received safe abortion services at private clinics. Through thematic analyses of women's testimonies, we identified gaps in the abortion care and person-centred domains which seemed to be important throughout the abortion process. When women received clear communication and personalised comprehensive information on abortion and post-abortion care from their healthcare providers, they reported more positive experiences overall and higher reproductive autonomy. Communication and supportive care were particularly valued during the post-abortion period, as was social support more generally. Further research is needed to design, implement and test the feasibility and acceptability of person-centred abortion care interventions in community and clinical settings with the goal of improving women's abortion experiences and overall reproductive health outcomes.The effects of three side stream phosphorous recovery volume on the performance and microbial structure of biological phosphorous removal system were investigated. Results showed that the removal of COD and nitrogen had no significant impacts by side stream operation, but the removal of phosphorous were gradually enhanced with the increase of side stream volume (SSV). The secretion of extracellular polymeric substance (EPS) were variously promoted at stripping period. However, with the increase of SSV, the inhibition on EPS and phosphorus accumulating organisms (PAOs) phosphorous absorption were severer and the restoration were tougher. The high throughout 16S rRNA gene sequencing revealed the succession of microbial population were significant effected by side stream operation. The relative abundances of PAOs reduced to 0.17%, 0.09% and 0.07% with 30%, 60% and 90% side stream operations, respectively. At restoration period, the relative abundance could restore to 95.4%, 65% and 38% initial values, respectively. The relative abundances of glycogen accumulating organisms were variously enhanced under various SSV conditions. In conclusion, at SSV of 60% more abundant recovered phosphorous could obtained and had slighter and reversible effects on activated sludges. Since most people engage in different combinations of both behaviours every day, understanding the socio-demographic characteristics of adults with distinct PA and sitting time (ST) patterns is important to contribute to evidence-based planning of public health strategies. Data from a national survey on diet and activity behaviours (IAN-AF, 2015/16) including 1724 adults (50.5% women, 18-64 years) from a representative sample of Portuguese adults was used in this study. Participants were interviewed face-to-face, and the International Physical Activity Questionnaire (IPAQ) was used. Logistic regression examined the associations between socio-demographic factors each of the four-low/high PA-ST groups. PA low/high categories were defined as in IPAQ, while ST low/high categories were defined according to ST tertiles (≤180 min/day, ≥360 min/day). A 'higher risk' behaviour pattern (low PA/high ST) was present in 37.3% of the adults and was likely associated with a middle household income, and with having 12 or more years of education.

  • The potential association between receipt of direct-acting antiviral (DAA) therapy for chronic hepatitis C infection (HCV) infection and the development of de-novo or aggressive progression of hepatocellular carcinoma (HCC), surprisingly, continues to be debated, more than 3 years after first being raised in small single-center observational studies1,2,3 . After initial studies without a concurrent control group described both an increased incidence of de-novo HCC as well as more aggressive recurrence after curative anticancer therapy in patients with HCV who received DAAs compared to historic controls, several groups have explored this association in different populations across the world.2,3. All rights reserved.AIMS In univentricular hearts, selective lung vasodilators such as phosphodiesterase type 5 (PDE5) inhibitors would decrease pulmonary resistance and improve exercise tolerance. However, the level of evidence for the use of PDE5 inhibitors in patients with a single ventricle (SV) remains limited. We present the SV-INHIBITION study rationale, design, and methods. METHODS AND RESULTS The SV-INHIBITION trial is a nationwide multicentre, randomized, double blind, placebo-controlled, Phase III study, aiming to evaluate the efficacy of sildenafil on the ventilatory efficiency during exercise, in teenagers and adult patients (>15 years old) with an SV. Patients with a mean pulmonary arterial pressure >15 mmHg and a trans-pulmonary gradient >5 mmHg, measured by cardiac catheterization, will be eligible. The primary outcome is the variation of the VE/VCO2 slope, measured by a cardiopulmonary exercise test, between baseline and 6 months of treatment. A total of 50 patients are required to observe a decrease of 5 ± 5 points in the VE/VCO2 slope, with a power of 90% and an alpha risk of 5%. The secondary outcomes are clinical outcomes, oxygen saturation, 6 min walk test, SV function, NT-proBNP, peak VO2 , stroke volume, mean pulmonary arterial pressure, trans-pulmonary gradient, SF36 quality of life score, safety, and acceptability. CONCLUSIONS The SV-INHIBITION study aims to answer the question whether PDE5 inhibitors should be prescribed in patients with an SV. This trial has been built focusing on the three levels of research defined by the World Health Organization disability (exercise tolerance), deficit (SV function), and handicap (quality of life). © 2020 The Authors. The aim of this study was to evaluate the clinical significance of the radiologically defined PPFE in patients with IPF. METHODS Clinical data and chest CT images were retrospectively analysed in 445 patients with IPF (biopsy-proven cases, n = 165). The radiological criteria of PPFE were defined as follows (i) bilateral subpleural dense fibrosis with or without pleural thickening in the upper lobes, (ii) evidence of disease progression and (iii) no clinical evidence of identifiable aetiologies. RESULTS The median follow-up period was 43.0 months. The mean age of the patients was 66.4 years and 76.4% were male. PPFE was identified in 28 patients (6.3%). The PPFE group showed lower BMI and lung function (FVC and TLC) at baseline, more frequent pneumothorax and pneumomediastinum, higher decline rates in lung function and poorer prognosis during follow-up than the no-PPFE group. PPFE was an independent risk factor (HR = 2.953, 95% CI 1.350-6.460, P = 0.007) for pneumothorax or pneumomediastinum, but not for mortality in patients with IPF. CONCLUSION Among patients with IPF, the PPFE group, when compared to the no-PPFE group, showed lower BMI and lung function and showed more frequent complications and poorer survival during follow-up. © 2020 Asian Pacific Society of Respirology.OBJECTIVES Transcranial electrical stimulation (tES) is a promising tool for modulating neural activity, but tES has poor penetrability and spatiotemporal resolution compared to invasive techniques like deep brain stimulation (DBS). Interferential strategies for alternating-current stimulation (IF-tACS) and pulsed/intersectional strategies for transcranial direct-current stimulation (IS-tDCS) address some of the limitations of tES, but the comparative advantages and disadvantages of these new techniques is not well understood. This study's objective was to evaluate the suprathreshold and subthreshold membrane dynamics of neurons in response to IF-tACS and IS-tDCS. MATERIALS AND METHODS We analyzed the biophysics of IF-tACS and IS-tDCS using a bioelectric field model of tES. Neural responses were quantified for suprathreshold generation of action potentials in axons and for subthreshold modulation of membrane dynamics in spiking pyramidal neurons. RESULTS IF-tACS and IS-tDCS could not directly activate axons at or below 10 mA, but within this current range, these fields were able to modulate, albeit indirectly, spiking activity in the neuron model. IF-tACS facilitated phase synchronization similar to tACS, and IS-tDCS enhanced and suppressed spiking activity similar to tDCS; however, in either case, the modulatory effects of these fields were less potent than their standard counterparts at a matched field intensity. Moreover, neither IF-tACS nor IS-tDCS improved the spatial selectivity of their parent strategies. CONCLUSIONS Enhancing the spatiotemporal precision and penetrability of tES with interferential and intersectional strategies is possible at the human scale. However, IF-tACS or IS-tDCS will likely require spatial multiplexing with multiple simultaneous sources to counteract their reduced potency, compared to standard techniques, to maintain stimulation currents at tolerable levels. © 2020 International Neuromodulation Society.Internet gaming disorder (IGD) is a concerning issue that requires further research. We tested these hypotheses with a design with one between-subjects factor (gamers vs controls) and two within-subjects factors stimuli (gaming vs neutral; for all participants) and session (deprivation vs satiety; only for gamers).

  • The article An Integrated In Vitro-In Silico Approach for Silver Nanoparticle Dosimetry in Cell Cultures, written by Ahluwalia et al, was originally published electronically on the publisher's internet portal (currently SpringerLink) on 13 January 2020 without open access.This study aims to determine whether the use of remote monitoring (RM) in implantable cardiac devices decreases all-cause mortality and heart failure (HF)-related hospitalization. We sought to conduct a systematic review and a meta-analysis of published randomized controlled studies. The population is adult patients with a diagnosis of HF with implantable devices. The intervention is RM using implantable cardiac devices whether added or used alone compared to standard of care. The outcomes are HF-related hospitalization and all-cause mortality. Risk of bias was assessed using the criteria defined in the Revised Cochrane Collaboration's tool for assessment of risk of bias. Data were extracted and validity was assessed independently by two reviewers. Electronic databases EMBASE and MEDLINE (Ovid) were searched up to 14th of October 2019, supplemented by a second search in CENTRAL (Cochrane Central Register of Controlled Trials) and clinicaltrials.gov. Only randomized controlled studies published in peer-reviewed journals with full format text in English of adult HF patients with a minimum follow-up of 6 months reporting mortality and/or hospitalization. Observational studies and studies that did not meet inclusion criteria were excluded. Thirteen randomized controlled studies that enrolled a total of 7015 patients were identified, 7 of which reported on all-cause mortality only and included 4460 patients.     The article An Integrated In Vitro-In Silico Approach for Silver Nanoparticle Dosimetry in Cell Cultures, written by Ahluwalia et al, was originally published electronically on the publisher's internet portal (currently SpringerLink) on 13 January 2020 without open access.This study aims to determine whether the use of remote monitoring (RM) in implantable cardiac devices decreases all-cause mortality and heart failure (HF)-related hospitalization. We sought to conduct a systematic review and a meta-analysis of published randomized controlled studies. The population is adult patients with a diagnosis of HF with implantable devices. The intervention is RM using implantable cardiac devices whether added or used alone compared to standard of care. The outcomes are HF-related hospitalization and all-cause mortality. Risk of bias was assessed using the criteria defined in the Revised Cochrane Collaboration's tool for assessment of risk of bias. Data were extracted and validity was assessed independently by two reviewers. Electronic databases EMBASE and MEDLINE (Ovid) were searched up to 14th of October 2019, supplemented by a second search in CENTRAL (Cochrane Central Register of Controlled Trials) and clinicaltrials.gov. Only randomized controlled studies published in peer-reviewed journals with full format text in English of adult HF patients with a minimum follow-up of 6 months reporting mortality and/or hospitalization. Observational studies and studies that did not meet inclusion criteria were excluded. Thirteen randomized controlled studies that enrolled a total of 7015 patients were identified, 7 of which reported on all-cause mortality only and included 4460 patients. Presenting symptoms included lower extremity edema (n=66, 48.2%), superior vena cava syndrome (n=30, 21.9%), unilateral upper extremity swelling (n=20, 14.6%), hemodialysis fistula or catheter dysfunction (n=11, 8.0%), ascites (n=8, 5.8%), and both ascites and lower extremity edema (n=2, 1.5%). Most common etiologies of central venous occlusion were prior central venous access placement (n=58, 42.3%), extrinsic compression (n=29, 21.2%), and post-surgical anastomotic stenosis (n=27, 19.7%). Number of stents placed, stent implantation location, stent sizes, technical successes, adverse events, need for re-intervention, follow-up evaluation, stent patencies, and mortality were recorded. Tecne of which were directly attributable to Z-stent placement.

    We aimed to evaluate the safety and diagnostic accuracy of computed tomography (CT)-guided transthoracic biopsy of small lung nodules (≤20 mm) adjacent to the pericardium or great vessels.

    This retrospective study examined the safety and diagnostic accuracy of percutaneous CT-guided biopsy for small lung nodules (≤20 mm) located within 10 mm of the pericardium or great vessels. Technical aspects and factors influencing complications were assessed, and diagnostic accuracy was calculated.

    A total of 168 biopsies were performed in 168 patients. The complications were mainly pneumothorax (34.5%; 58 of 168 patients), chest tube insertion (5.3%; 9 of 168 patients), and pulmonary hemorrhage (61.3%; 103 of 168 procedures), with no patient mortality. One patient (0.6%) was admitted because of hemorrhage complications. Significant independent risk factors for pneumothorax were nodules resided in upper or middle lobes and lateral patient position, and for hemorrhage, longer distance from structures and longer needle trajectory through the lung parenchyma. Overall, the sensitivity, accuracy, and specificity were 91.0%, 92.2%, and 100%, respectively.

However, the functional relevance between MMP-14 and pathogenesis of genotype VII NDV remains to be investigated. In this study, expression, biofunction and regulation of MMP-14 induced by genotype VII NDV were analysed in chicken peripheral blood mononuclear cells (PBMCs). The results showed that JS5/05 significantly increased expression and membrane accumulation of MMP-14 in PBMCs, correlating to enhanced collagen degradation and cell migration. Specific MMP-14 inhibition significantly impaired collagen degradation and migration of JS5/05-infected cells, suggesting dependence of these features on MMP-14. In addition, MMP-14 upregulation correlated with activation of the extracellular signal-regulated kinase (ERK) pathway upon JS5/05 infection, and blockage of the ERK signalling significantly suppressed MMP-14-mediated collagen degradation and migration of JS5/05-infected cells. Using a panel of chimeric NDVs derived from gene exchange between genotype VII and IV NDV, the fusion and haemagglutinin-neuraminidase genes were identified as the major viral determinants for MMP-14 expression and activity. In conclusion, MMP-14 was defined as a critical regulator of collagen degradation and cell migration of chicken PBMCs infected with genotype VII NDV, which may contribute to pathology of the virus. Our findings add novel information to the body of knowledge regarding virus-host biology and NDV pathogenesis.

    This study aims to explore the response trajectory of antipsychotics and compare the treatment responses of seven different antipsychotics over 6 weeks in patients with schizoprenia (trial registration Chinese Clinical Trials Registry Identifier ChiCTR-TRC-10000934).

    Data were collected from a multicentre, randomised open-label clinical trial.   • The oblique prepsoas retroperitoneal approach to the lumbar spine for interbody fusion or oblique lumbar interbody fusion (OLIF) provides safe access to nearly all lumbar levels. A wide interval between the psoas and aorta allows for a safe and straightforward left-sided oblique approach to the discs above L5. Inclusion of L5-S1 in this approach, however, requires modifications in the technique to navigate the complex and variable vascular anatomy distal to the bifurcation of the great vessels. While different oblique approaches to L5-S1 have been described in the literature, to our knowledge, no previous study has provided guidance for the choice of technique.

    Our objectives were to evaluate our early experience with the safety of including L5-S1 in OLIF using 3 different approach techniques, as well as to compare early complications between OLIF with and without L5-S1 inclusion.

This study pilots the use of an immersive virtual reality (VR) platform for defining objective observable behaviors as standards for evaluation of medical student recognition of impending respiratory failure.

    A cross-sectional observational study was conducted from July 2018 to December 2019, evaluating student performance during a VR scenario of an infant in impending respiratory failure using the OculusRift VR platform. Video recordings were rated by 2 pair of physician reviewers blinded to student identity. One pair provided a consensus global assessment of performance (not competent, borderline, or competent) while the other used a checklist of observable behaviors to rate performance. Binary discriminant analysis was used to identify the observable behaviors that predicted the global assessment rating.

    Twenty-six fourth year medical students participated. Student performance of 8 observable behaviors was found to be most predictive of a rating of competent, with a 91% probability. Correctly stating that the patient required an escalation of care had the largest contribution toward predicting a rating of competent, followed by commenting on the patient's increased heart rate, low oxygen saturation, increased respiratory rate, and stating that the patient was in respiratory distress.

    The oblique prepsoas retroperitoneal approach to the lumbar spine for interbody fusion or oblique lumbar interbody fusion (OLIF) provides safe access to nearly all lumbar levels. A wide interval between the psoas and aorta allows for a safe and straightforward left-sided oblique approach to the discs above L5. Inclusion of L5-S1 in this approach, however, requires modifications in the technique to navigate the complex and variable vascular anatomy distal to the bifurcation of the great vessels. While different oblique approaches to L5-S1 have been described in the literature, to our knowledge, no previous study has provided guidance for the choice of technique. Our objectives were to evaluate our early experience with the safety of including L5-S1 in OLIF using 3 different approach techniques, as well as to compare early complications between OLIF with and without L5-S1 inclusion. Retrospective cohort study. Of the 87 patients who underwent lumbar interbody fusion at 167 spinal levels via an OLIF apprter part of the series as the learning curve progressed.   • In this study, feedlot steers with clinical signs of BRD and rectal temperatures lower than traditional cutoffs displayed a positive response to antimicrobial therapy. A clear benefit of FTS was not observed in this study. Calves with a WS ≥ 2 were lighter at the time of first BRD treatment compared to calves with a WS = 1. However, standalone TIL therapy was the optimal BRD treatment modality across WS strata in this study.In October 2018, an outbreak of transmissible viral proventriculitis (TVP) occurred in 30-day-old commercial broiler chickens on a farm in Weifang, China. TVP, an infectious viral disease characterized by runting and stunting, is associated with many viruses, and has a significant economic impact on the global poultry industry. TVP is diagnosed according to clinical symptoms, gross and histological lesions, and negative PCR results for pathogenic bacteria, avian leukosis virus subgroup J, Marek's disease virus, reticuloendotheliosis virus, infectious bursa disease virus, avian reovirus, chicken anemia virus, infectious bronchitis virus, chicken proventricular necrosis virus, gyrovirus 3 and chicken circovirus. To further detect the possible causative pathogens of TVP, we used PacBio third-generation sequencing to examine proventricular samples. A dominant abundance of the novel cyclovirus (CyCV), chCyCV-SDAU-1, was identified in broilers with TVP. The complete chCyCV-SDAU-1 genome was verified via inverse PCR, was 1936 bp long, and consisted of Rep, Cp, and two intergenic regions. Phylogenetic tree analysis showed that chCyCV-SDAU-1 formed an independent branch with other cycloviruses. The homology of chCyCV-SDAU-1 with 20 others known cycloviruses was less then 40%. Retrospective investigation showed that the CyCV infection rate in the broilers with TVP was 80% (16/20), while no CyCV was found in healthy chickens. In conclusion, a novel CyCV was identified in chickens with TVP, though its role in this disease is unclear.Cardiopathy is a common, irreversible manifestation of the chronic phase of Chagas disease; however, there is controversy as to how the causes for progression from the acute to the chronic phase are defined. Cardiac tissue slides were stained with HE and Masson trichrome stain. A significant increase in parasitemia levels was observed after 15 days post-infection (dpi), with a maximum of 4.1 × 106 parasites on 33 dpi, ending on 43 dpi; amastigote nests were observed on 15-62 dpi. The imaging data were segmented with 3D Slicer software and refined with Autodesk Meshmixer. Images were visualized in 3D Slicer and surface models were converted to 3D PDFs to provide easier interactive access, and 3D prints were also generated for visualization purposes. Our second aim was to analyze how skull length and width relate to the surface areas of the prepiriform rhinencephalic, prefrontal, and non-prefrontal cerebral convexity areas of the endocasts. The rhinencephalic area ratio decreased with a larger skull index. The results showed that duGal-2 expression was higher in the gut, kidney, and liver tissue, and weakly expressed in the lung and brain, in healthy ducks; however, the expression level of duGal-2 was detected as being up-regulated after infection with APEC. In addition, knockdown or overexpression of duGal-2 in DEFs was achieved by small interference RNA (siRNA) transfection and plasmid transduction, respectively. The knockdown of duGal-2 led to a decrease in the expression of some inflammatory cytokines such as IL-1β, IL-6, and IL-8, while the expression levels of anti-inflammatory factor IL-10 were up-regulated. At the same time, the bacterial load of APEC was increased after knockdown of duGal-2 in vitro.

We have investigated whether β-hydroxybutyrate (βOHB), the main ketone body (KB) produced in ketogenic diet (KD), is neuroprotective in aralar-knock-out (KO) neurons and ****. We report that βOHB efficiently recovers aralar-KO neurons from deficits in basal-stimulated and glutamate-stimulated respiration, effects requiring βOHB entry into the neuron, and protects from glutamate excitotoxicity. Aralar-deficient **** were fed a KD to investigate its therapeutic potential early in development, but this approach was unfeasible. Therefore, aralar-KO pups were treated without distinction of gender with daily intraperitoneal injections of βOHB during 5 d. This treatment resulted in a recovery of striatal markers of the dopaminergic sysurons, highly expressing aralar, its deficiency causes a metabolic blockade hampering mitochondrial energetics and respiration. Here, we find that βOHB, the main metabolic product in KD, recovers defective mitochondrial respiration bypassing the metabolic failure in aralar-deficient neurons. βOHB oxidation in mitochondria boosts the synthesis of cytosolic aspartate (Asp) and NAA, which is impeded by aralar deficiency, presumably through citrate-malate shuttle. We find that attention spreads from a spatially cued location to the underlying object, and enhances all spatial locations that comprise the object. Importantly, this spreading was also evident when the object was not task relevant. Here, we demonstrate that the base excision repair protein DNA polymerase β (Polβ) is involved in hippocampal pyramidal neuron differentiation via a TET-mediated active DNA demethylation during early postnatal stages using Nex-Cre/Polβ fl/fl mice of either sex, in which forebrain postmitotic excitatory neurons lack Polβ expression. Polβ deficiency induced extensive DNA double-strand breaks (DSBs) in hippocampal pyramidal neurons, but not dentate gyrus granule cells, and to a lesser extent in neocortical neurons, during a period in which decreased levels of 5-methylcytosine and 5-hydroxymethylcytosine were observed in genomic DNA. Inhibition of the hydroxylation of 5-methylcytosine by expression of microRNAs miR-29a/b-1 diminished DSB formation. Conversely, its induction by TET1 catrons. This aberrant double-strand break formation was attributed to active DNA demethylation as an epigenetic regulation. We have investigated whether β-hydroxybutyrate (βOHB), the main ketone body (KB) produced in ketogenic diet (KD), is neuroprotective in aralar-knock-out (KO) neurons and mice. We report that βOHB efficiently recovers aralar-KO neurons from deficits in basal-stimulated and glutamate-stimulated respiration, effects requiring βOHB entry into the neuron, and protects from glutamate excitotoxicity. Aralar-deficient mice were fed a KD to investigate its therapeutic potential early in development, but this approach was unfeasible. Therefore, aralar-KO pups were treated without distinction of gender with daily intraperitoneal injections of βOHB during 5 d. This treatment resulted in a recovery of striatal markers of the dopaminergic sysurons, highly expressing aralar, its deficiency causes a metabolic blockade hampering mitochondrial energetics and respiration. Here, we find that βOHB, the main metabolic product in KD, recovers defective mitochondrial respiration bypassing the metabolic failure in aralar-deficient neurons. βOHB oxidation in mitochondria boosts the synthesis of cytosolic aspartate (Asp) and NAA, which is impeded by aralar deficiency, presumably through citrate-malate shuttle. In aralar-knock-out (KO) mice, βOHB recovers from the drastic drop in specific dopaminergic and myelin markers. The βOHB-induced myelin synthesis occurring together with the marked increment in neuronal NAA synthesis supports the role of NAA as a lipid precursor during postnatal myelination.Oscillatory α-band activity is commonly associated with spatial attention and multisensory prioritization. It has also been suggested to reflect the automatic transformation of tactile stimuli from a skin-based, somatotopic reference frame into an external one. Previous research has not convincingly separated these two possible roles of α-band activity. A tactile cue to uncrossed or crossed hamation to a location in external space has been proposed to rely on the modulation of oscillatory brain activity in the α-band range, across the multiple cortical areas that are involved in tactile, multisensory, and spatial processing. We report two findings that are inconsistent with this view. First, α-band activity reflected the remapped stimulus location only when touch was task relevant. Second, α-band modulation occurred too late to account for spatially directed behavioral responses and, thus, only after remapping must have taken place. Theories of object-based attention have suggested that when spatial attention is directed to part of an object, attention does not simply enhance the attended location but automatically spreads to enhance all locations that comprise the object. Here, we tested this hypothesis by reconstructing the distribution of attention from primary visual cortex (V1) population neuronal activity patterns in 24 human adults (17 female) using functional magnetic resonance imaging (fMRI) and population-based receptive field (prf) mapping. We find that attention spreads from a spatially cued location to the underlying object, and enhances all spatial locations that comprise the object. Importantly, this spreading was also evident when the object was not task relevant. SEARCH STRATEGY We conducted a search of UK health and social care regulators' websites to establish a list of regulated professional titles, obtain numbers of registrants and identify documents detailing CPD policy. We searched Applied Social Sciences Index and Abstracs (ASSIA), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Medline, EMCare and Scopus Life Sciences, Health Sciences, Physical Sciences and Social Sciences & Humanities databases to identify a list of common features used to describe CPD systems internationally and these were used to organise the review of CPD requirements for each profession. RESULTS CPD is now mandatory for the approximately 1.5 million individuals registered to work under 32 regulated titles in the UK. Eight of the nine regulators do not mandate modes of CPD and there is little requirement to conduct interprofessional CPD. Overall 81% of those registered are required to engage in some form of reflection on their learning but only 35% are required to use a personal development plan while 26% have no requirement to engage in peer-to-peer learning. CONCLUSIONS Our review highlights the wide variation in the required characteristics of CPD being undertaken by UK health professionals and raises the possibility that CPD schemes are not fully incorporating the best practice. © Author(s) (or their employer(s)) 2020. This study aimed to prospectively examine the relationship between resting heart rate (RHR) and all-cause mortality in Chinese patients with hypertension. DESIGN An observational, prospective and population-based cohort study. SETTING The Kailuan cohort study was conducted in Tangshan City in northern China. PARTICIPANTS We enrolled 46 561 patients who did not receive beta-blocker treatment and were diagnosed with hypertension for the first time during an employee health examination in Kailuan Group in 2006 and 2008. OUTCOME The primary outcome of this study was all-cause mortality. METHODS The patients in this study were followed for 9.25±1.63 years. All patients were followed up face to face every 2 years. According to the distribution of RHR in the study population, RHR was categorised into five groups on the basis of quintiles Q1 RHR ≤68 beats per minute (bpm); Q2 RHR >68 and ≤72 bpm; Q3 RHR >72 and ≤76 bpm; Q4 RHR >76 and ≤82 bpm; Q5 RHR >82 bpm. Cox proportional hazards model, which was adjusted for traditional risk factors, was used. RESULTS During follow-up, 4751 deaths occurred. After adjustment for potential confounders, restricted cubic spline regression showed that the risk of all-cause mortality increased with heart rate. In multivariate Cox regression analyses adjusted for age, sex and major covariates, the HR for all-cause mortality was 1.31 (95% CI 1.27 to 1.33) in the highest quintile group (Q5) compared with the lowest quintile group (Q1). CONCLUSION An increase in RHR is a long-term risk factor of all-cause mortality in Chinese patients with hypertension. The group concept mapping (GCM) approach was applied to identify, organise and prioritise values and preferences in the example of health professionals' choice of analgesia for patients with acute trauma pain. SETTING Prehospital and emergency care centres in the Nordic countries of Denmark, Norway, Sweden, Finland and Iceland. PARTICIPANTS Acute care health professionals with qualifications to administer analgesic agents to patients in emergency and prehospital settings, including advanced ambulance assistants, rescue officers, paramedics, emergency physicians and emergency nurses, participated in an online survey in which statements were generated (n=40) and structured (n=11) and finally analysed and interpreted in a validation meeting (n=4).   • Infant massage is a traditional practice for newborns in some parts of the world; its beneficial effects in reduction of jaundice in the neonates are a matter of investigation. This study aims to find out the effects of massage therapy in term neonates with neonatal jaundice receiving phototherapy compared with a control group. This randomized controlled trial (RCT) includes total 100 term newborns evenly divided into the massage group and control group after obtaining informed consent and was conducted in the Neonatal ward of the Sher-e-Bangla Medical College, Barisal, Bangladesh from 1st January 2018 to 30th June 2018. The massage group received both massage therapy and phototherapy for neonatal jaundice whereas the control group received only phototherapy. Data were collected and analyzed and results were prepared by student's 't' test for continuous variables and chi-square test for categorical variables and statistical significance was found if p value 0.05). The defecation frequency was significantly more in massage group than control group and serum bilirubin levels were significantly lower in massage group than control group on day 3 onward (p less then 0.05). Total 166 Escherichia coli, Klebsiella pneumoniae and Klebsiella oxytoca were isolated from urine, wound swab, pus, sputum and blood samples of patients of Dhaka Medical College Hospital. Antibiotic susceptibility test was performed by disk-diffusion technique. AmpC beta-lactamase producers were detected phenotypically by Modified three-dimensional test (MTDT). AmpC beta-lactamase genes (DHA, ACC, EBC, CIT, MOX, FOX) among the cefoxitin resistant Escherichia coli and Klebsiella species were detected by polymerase chain reaction (PCR). Sixty seven cefoxitin resistant Escherichia coli, Klebsiella pneumoniae and Klebsiella oxytoca were isolated during disk-diffusion technique. Among the 67 cefoxitin resistant strains, 30(44.78%) AmpC beta-lactamase producers were detected by MTDT and 59(88.06%) were detected by PCR. The dominant genotype found was CIT (62.69%) followed by DHA (53.73%). The results of this study showed high proportion of AmpC beta- lactamase enzyme producing Escherichia coli and Klebsiella species in Bangladesh. Regular surveillance of antibiotic resistance should be done in every tertiary care hospital to prevent spread of these strains.The aim of this prospective study was to evaluate the role and application of laparoscopy in the management of acute appendicitis and its complications. The study was conducted between January 2011 to February 2019 in Bangabandhu Sheikh Mujib Medical University (BSMMU) and some private hospitals of Dhaka, Bangladesh. A total of 641 patients who presented with acute appendicitis and its complications were treated by laparoscopic method. Around 70% (449) patients had classical presentation, 20% (128) were atypical and 10% (64) were recurrent or chronic presentation. Maximum (527, 82%) came with acute appendicitis. Eighty eight (13.72%) patients had burst (perforated) appendicitis, 12(1.87%) patients had appendicular abscess, 11(1.71%) showed early appendicular lump, 2(0.4%) had stump appendicitis and 1(0.2%) had previous laparoscopic clip control of stump. Energy source was unipolar diathermy. Intracorporeal knotting, endoloop and haemoclip were used for haemostosis and ligation of appendicular stump. Mean duration of surgery was 20minutes and mean hospital stay was 36 hours. Histopathological patterns were acute appendicitis (554, 86.42%), gangrenous appendicitis (58, 9.04%), catarrhal (13, 2.02%), chronic appendicitis (10, 1.56%) lymphoid hyperplasia (6, 0.93%) and carcinoid tumor (1, 0.15%), among the complications (25) were single or multiple port infections, port TB, paralytic ileus and diarrhea. Conversion was 1(0.15%) for controlling haemorrhage of burst appendicitis. The rest of the patients recovered quickly and uneventfully. In conclusion, it can be stated that laparoscopic appendicectomy is safe, convenient and achievable and can become the gold standard of management of acute appendicitis and its complications.Visceral Leishmaniasis is the most severe form of leishmaniasis and can be fatal in the absence of treatment. Mortality is very high in untreated cases 90%. Clinical case series document a 5% case fatality rate for kala-azar patients in the hospital as complication of the disease. Nepal, India, Bangladesh, Brazil and Sudan constitute five countries of the world where more than 90% of visceral leishmaniasis occurs. Mymensingh accounted for more than 50% of total kala-azar case reported in Bangladesh. This observational study was done at SKKRC under Mymensingh Medical College Hospital, Mymensingh, Bangladesh from 1st august 2013 to 28th February 2014, to find out various clinical features and hematological picture in visceral leishmaniasis. Total 100 diagnosed case of visceral leishmaniasis was taken. Among total patient male and female were nearly equally affected by kala-azar. All age group were affected by kala-azar from below 2 years to above 50 years. Poor people in the society were affected more by kala-azar. Mymensingh was the highest incidence of kala-azar among different district in Bangladesh. Pyrexia, pallor, weight loss were the main symptoms of visceral leishmaniasis. Abdominal distension, jaundice, vomiting, cough, bleeding manifestation were less frequent symptoms of kala-azar. Hepatosplenomegaly were the frequent sign of kala-azar. Majority of patient had anemia, leucopenia and thrombocytopenia. ESR was frequently raised in kala-azar patient among those patient many had high ESR more than 100 mm in 1st hour. In conclusion prolong fever, progressive pallor, anorexia, weight loss, hepatosplenomegaly in endemic area were the main clinical features of visceral leishmaniasis. Anemia, leucopenia, thrombocytopenia, bi-cytopenia, pancytopenia and high ESR were frequently found hematological pictures in visceral leishmaniasis.

AmpC beta-lactamase genes (DHA, ACC, EBC, CIT, MOX, FOX) among the cefoxitin resistant Escherichia coli and Klebsiella species were detected by polymerase chain reaction (PCR). Sixty seven cefoxitin resistant Escherichia coli, Klebsiella pneumoniae and Klebsiella oxytoca were isolated during disk-diffusion technique. Among the 67 cefoxitin resistant strains, 30(44.78%) AmpC beta-lactamase producers were detected by MTDT and 59(88.06%) were detected by PCR. The dominant genotype found was CIT (62.69%) followed by DHA (53.73%). The results of this study showed high proportion of AmpC beta- lactamase enzyme producing Escherichia coli and Klebsiella species in Bangladesh. Regular surveillance of antibiotic resistance should be done in every tertiary care hospital to prevent spread of these strains.The aim of this prospective study was to evaluate the role and application of laparoscopy in the management of acute appendicitis and its complications. The study was conducted between January 2011 to February 2019 in Bangabandhu Sheikh Mujib Medical University (BSMMU) and some private hospitals of Dhaka, Bangladesh. A total of 641 patients who presented with acute appendicitis and its complications were treated by laparoscopic method. Around 70% (449) patients had classical presentation, 20% (128) were atypical and 10% (64) were recurrent or chronic presentation. Maximum (527, 82%) came with acute appendicitis. Eighty eight (13.72%) patients had burst (perforated) appendicitis, 12(1.87%) patients had appendicular abscess, 11(1.71%) showed early appendicular lump, 2(0.4%) had stump appendicitis and 1(0.2%) had previous laparoscopic clip control of stump. Energy source was unipolar diathermy. Intracorporeal knotting, endoloop and haemoclip were used for haemostosis and ligation of appendicular stump. Mean duration of surgery was 20minutes and mean hospital stay was 36 hours. Histopathological patterns were acute appendicitis (554, 86.42%), gangrenous appendicitis (58, 9.04%), catarrhal (13, 2.02%), chronic appendicitis (10, 1.56%) lymphoid hyperplasia (6, 0.93%) and carcinoid tumor (1, 0.15%), among the complications (25) were single or multiple port infections, port TB, paralytic ileus and diarrhea. Conversion was 1(0.15%) for controlling haemorrhage of burst appendicitis. The rest of the patients recovered quickly and uneventfully. In conclusion, it can be stated that laparoscopic appendicectomy is safe, convenient and achievable and can become the gold standard of management of acute appendicitis and its complications.Visceral Leishmaniasis is the most severe form of leishmaniasis and can be fatal in the absence of treatment. Mortality is very high in untreated cases 90%. Clinical case series document a 5% case fatality rate for kala-azar patients in the hospital as complication of the disease. Nepal, India, Bangladesh, Brazil and Sudan constitute five countries of the world where more than 90% of visceral leishmaniasis occurs. Mymensingh accounted for more than 50% of total kala-azar case reported in Bangladesh. This observational study was done at SKKRC under Mymensingh Medical College Hospital, Mymensingh, Bangladesh from 1st august 2013 to 28th February 2014, to find out various clinical features and hematological picture in visceral leishmaniasis. Total 100 diagnosed case of visceral leishmaniasis was taken. Among total patient male and female were nearly equally affected by kala-azar. All age group were affected by kala-azar from below 2 years to above 50 years. Poor people in the society were affected more by kala-azar. Mymensingh was the highest incidence of kala-azar among different district in Bangladesh. Pyrexia, pallor, weight loss were the main symptoms of visceral leishmaniasis. Abdominal distension, jaundice, vomiting, cough, bleeding manifestation were less frequent symptoms of kala-azar. Hepatosplenomegaly were the frequent sign of kala-azar. Majority of patient had anemia, leucopenia and thrombocytopenia. ESR was frequently raised in kala-azar patient among those patient many had high ESR more than 100 mm in 1st hour. In conclusion prolong fever, progressive pallor, anorexia, weight loss, hepatosplenomegaly in endemic area were the main clinical features of visceral leishmaniasis. Anemia, leucopenia, thrombocytopenia, bi-cytopenia, pancytopenia and high ESR were frequently found hematological pictures in visceral leishmaniasis.
