  • G-protein coupled receptors (GPCRs) are the largest family of membrane-spanning receptors in metazoans and mediate diverse biological processes such as chemotaxis, vision, and neurotransmission. Adhesion GPCRs represent an understudied class of GPCRs. Adhesion GPCRs (ADGRs) are activated by an intrinsic proteolytic mechanism executed by the G-protein autoproteolysis inducing domain that defines this class of GPCRs. It is hypothesized that agonist ligands modulate the proteolyzed receptor to regulate the activity of a tethered agonist peptide that is an intramolecular activator of ADGRs. The mechanism of activation of ADGRs in physiological settings is unclear and the toolbox for interrogating ADGR physiology in cellular models is limited. Therefore, we generated a novel enterokinase-activated tethered ligand system for ADGRG6(GPR126). Enterokinase addition to cells expressing a synthetic ADGRG6 protein induced potent and efficacious signal transduction through heterotrimeric G-protein coupled second messenger pathways including cyclic nucleotide production, intracellular calcium mobilization, and GPCR-pathway linked reporter gene induction. These studies support the hypothesis that ADGRG6(GPR126) is coupled to multiple heterotrimeric G-proteins including Gαs, Gαq, and Gα12. This novel assay method is robust, specific, and compatible with numerous cell pharmacology approaches. However, effective, safe treatments typically take over a decade to develop and there are still no clinically approved therapies to treat highly pathogenic coronaviruses. Repurposing of known drugs can speed up development and this strategy, along with the use of biologicals (notably monoclonal antibody therapy) and vaccine development programmes remain the principal routes to dealing with the immediate impact of COVID-19. Nevertheless, the development of broadly-effective highly potent antivirals should be a major longer term goal. Structural biology has been applied with enormous effect, with key proteins structurally characterised only weeks after the SARS-CoV-2 sequence was released. Open-access to advanced infrastructure for structural biology techniques at synchrotrons and high-end cryo-EM and NMR centres has brought these technologies centre-stage in drug discovery. We summarise the role of Diamond Light Source in responses to the pandemic and note the impact of the immediate release of results in fuelling an open-science approach to early-stage drug discovery.β-catenin is a multi-functional protein with a central role in regulating embryonic development and tissue homeostasis. The abnormal accumulation of β-catenin, due to disrupted β-catenin degradation or unregulated β-catenin synthesis, causes the development of cancer. A recent study showed that the overexpression of proto-oncogene serine/arginine-rich splicing factor 9 (SRSF9) promotes β-catenin accumulation via binding β-catenin mRNA and enhancing its translation in a manner that is dependent on the mechanistic target of rapamycin (mTOR). However, the regulation of the interaction between SRSF9 and mRNA of β-catenin remains unclear. Here, we show that AMP-activated protein kinase (AMPK) phosphorylates SRSF9 at the RNA-interacting SWQDLKD motif that plays a major role in determining substrate specificity. The phosphorylation by AMPK inhibits the binding of SRSF9 to β-catenin mRNA and suppresses β-catenin protein synthesis caused by SRSF9 overexpression without changing the β-catenin mRNA levels. Here, the presence of a new essential R94 residue, which is completely conserved among all identified Siglecs, was demonstrated. A mutation of R94 residue in Siglec-7 led to the disappearance of the Sia binding property, similar to a site 1 mutation (R124A). R94 is close to R67 in site 2, and site 2 mutations at either of them abolished the ligand-binding properties to both gangliosides and glycoproteins. These data suggest that, in addition to site 1, the conserved R residue among Siglecs in site 2 is another functional site.

In this paper, a new indicator, biological denitrification potential (BDP), was developed for evaluating the biological stability of digestate. Digestate samples collected along the digestion process from a mesophilic anaerobic batch digester fed with food waste were investigated under different solid retention time. The value of BDP based on nitrate removal ranged from 176.3 to 48.3 mg-N/g-VSdigestate, corresponding well to the digestion time, and strongly correlated with total organic carbon content. Evolution trends similar to respiration index (RI) and biochemical methane potential (BMP) can be also observed for BDP, indicating that values presented of these stability indices decreased with the degree of digestate stabilization. The mass balance of the BDP process indicated that nitrate was mainly converted into N2 gas with mineralizing organic carbon from digestate, implying that biostability evaluated by BDP depends on carbon source and denitrification activity in digestate. The denitrifying bacteria Thiopseudomonas and Pseudomonas accounted for the majority of microorganisms. These findings of this study concluded that BDP can be an efficient indicator to assess the bio-stability of digestate planned for agricultural or land use. Compared with the existing biostability index, BDP has the additional advantage of no exogenous inoculum addition, homogenous test condition and possibility of shortening incubation time.This study was performed to assess the Latvian population exposure to polybrominated diphenyl ethers (PBDEs), hexabromocyclododecanes (HBCDD), dechlorane-related compounds (DRCs), and emerging brominated flame retardants (EBFRs). Food items including fish, fish products, meat, dairy products, cereals and bread, eggs, vegetable oils, and sweets were analyzed for the content of these contaminants, followed by per capita intake calculations and risk assessment. The highest dietary exposure for general population was observed in the case of HBCDD, .reaching an estimated daily intake (EDI) value of 2.92 ng kg-1 b.w. (or 3.35 ng kg-1 b.w. if an outlying data point is included), followed by PBDEs with EDI of 1.24 ng kg-1 b.w., including ~25% contribution of PBDE-209 to the overall EDI from PBDEs. DRCs and EBFRs were secondary contributors to the total intake of selected flame retardants (FRs), with the observed EDIs of 0.46 and 0.47 ng kg-1 b.w, respectively. The obtained occurrence data and risk characterization according to the European Food Safety Authority (EFSA) approach showed the calculated margin of exposure (MOE) values higher than the critical values for PBDE-47, -99 and -153as well as for HBCDD, indicating that the estimated dietary exposures are unlikely to be of significant health concern for the Latvian population. At the same time, it should be pointed out that the risk assessment was performed only for five out of the twenty-five selected halogenated flame retardants (HFRs), while cumulative effects due to the potential presence of other HFRs and their biodegradation products were not considered.The huge amount of food waste (FW), containing high organic matter content and moisture, is difficult to be well treated. Supercritical water gasification (SCWG) can efficiently convert FW to H2-rich syngas. However, it requires high energy input due to the high temperature and high pressure. This study provided an innovative "two-steps heating process" for the SCWG of FW, which firstly utilized hydrothermal (HT) pretreatment to shorter time of SCWG. The effects of different HT temperature (200 °C, 250 °C, 300 °C, 30 min) to SCWG temperature (480 °C, 30 min) and the different residence time (20 min HT - 40 min SCWG, 30 min HT - 30 min SCWG, and 40 min HT - 20 min SCWG) on total syngas yield, carbon conversion efficiency (CE), cold gas efficiency (CGE), and hydrogen conversion efficiency (HE) were studied. Moreover, the energy input by means of electricity consumption in each experiment was measured to determine the energy saving rate. The optimal condition (200 °C, 20 min HT - 40 min SCWG), obtaining the gas yield (17.22 mol/kg), CE (20.10%), CGE (22.13%), and HE (41.54%), was higher than the gas yield (16.53 mol/kg), CE (19.98%), CGE (20%), and HE (38.08%) of directly SCWG (60 min, 0 °C-480 °C). Moreover, the TOC of derived liquid and the pyrolysis characteristics of solid residues were analyzed. Additionally, it was also observed the HT pretreatment helped to reduce the electricity consumption. The climate change conditions were simulated in a greenhouse, and the legacy effect was simulated by removing the greenhouse after twelve months. Simulated climate change significantly decreased the in situ CH4 emissions in the urea-treated soils while the legacy effect significantly decreased the in situ CH4 emissions in the control plots, but had very little effect in the urea-treated soils. This indicates that the impact of simulated climate change and its legacy on CH4 emissions was significantly modified by nitrogen fertilization. Methanotrophs were more sensitive than methanogens in response to simulated climate change and its legacy effect, especially in the urea treated soil. Significant negative correlations were observed between the abundances of ammonia oxidizers and methanotrophs. Additionally, results of partial least path modeling (PLS-PM) indicated that the interactions of methanogens and methanotrophs with ammonia oxidizing archaea (AOA) had significant positive relationships with in situ CH4 emissions under the simulated climate change condition. Our work highlights the important role of AOA for CH4 emissions under climate change conditions. Further research is needed to better understand this effect in other ecosystems.

  • Since its emergence late in 2019, the COVID-19 pandemic continues to exude major public health and socio-economic burden globally. South Africa is currently the epicenter for the pandemic in Africa. This study is based on the use of a compartmental model to analyze the transmission dynamics of the disease in South Africa. A notable feature of the model is the incorporation of the role of environmental contamination by COVID-infected individuals. The model, which is fitted and parametrized using cumulative mortality data from South Africa, is used to assess the impact of various control and mitigation strategies. Rigorous analysis of the model reveals that its associated continuum of disease-free equilibria is globally-asymptotically stable whenever the control reproduction number is less than unity. The epidemiological implication of this result is that the disease will eventually die out, particularly if control measures are implemented early and for a sustainable period of time. For instance, numerical simulations suggest that if the lockdown measures in South Africa were implemented a week later than the 26 March, 2020 date it was implemented, this will result in the extension of the predicted peak time of the pandemic, and causing about 10% more cumulative deaths. In addition to illustrating the effectiveness of self-isolation in reducing the number of cases, our study emphasizes the importance of surveillance testing and contact tracing of the contacts and confirmed cases in curtailing the pandemic in South Africa.
    The objective of this study was to characterize the role of NOTCH signaling cascade activation in the angiogenic drive of infantile hemangioma (IH) tissue.

    Both IH tissue and normal skin were collected from 55 surgical patients. Of these, 14 were proliferating hemangiomas, 14 were stationary, 14 were involuted, and 13 had been treated with propranolol. Protein and RNA were extracted from all tissues and subjected to Western blotting and reverse transcription-polymerase chain reaction, respectively, for components of the NOTCH signaling pathway.

    Compared with normal skin from the same patients, proliferating IHs contained higher levels of messenger RNA and protein for the majority of NOTCH receptors and ligands as well as the downstream coactivator MAML1. Expression ofNOTCH receptor ligand messenger RNA and protein was significantly lower in involuting and propranolol-treated IHs.

The assay results confirmed that the functions of HaPrx4 involved DNA protection, hydrogen peroxide (H2O2) elimination, oxidoreductase activity, enhancing cell survival, and cell protection. In this study, we aimed to elucidate the timings of action of the swallowing organs in the oral and pharyngeal stages of swallowing by recording the barometric pressure (BP) and tongue muscle activities.

    Dry and water swallows were studied in 10 adults using electromyography and small barometers. BPs were recorded during swallowing in the oral cavity (BP-o) and pharynx (BP-p), associated with muscle activities of the anterior (TA) and posterior (TP) parts of the tongue and of the suprahyoid (SHy) muscle. To analyze the temporal pattern of each activity, times of onset, cessation, and peak were measured.

    Two characteristic waveforms were obtained. BP-o peaked immediately after onset and decreased to atmospheric pressure following a short plateau.     Since its emergence late in 2019, the COVID-19 pandemic continues to exude major public health and socio-economic burden globally. South Africa is currently the epicenter for the pandemic in Africa. This study is based on the use of a compartmental model to analyze the transmission dynamics of the disease in South Africa. A notable feature of the model is the incorporation of the role of environmental contamination by COVID-infected individuals. The model, which is fitted and parametrized using cumulative mortality data from South Africa, is used to assess the impact of various control and mitigation strategies. Rigorous analysis of the model reveals that its associated continuum of disease-free equilibria is globally-asymptotically stable whenever the control reproduction number is less than unity. The epidemiological implication of this result is that the disease will eventually die out, particularly if control measures are implemented early and for a sustainable period of time. For instance, numerical simulations suggest that if the lockdown measures in South Africa were implemented a week later than the 26 March, 2020 date it was implemented, this will result in the extension of the predicted peak time of the pandemic, and causing about 10% more cumulative deaths. In addition to illustrating the effectiveness of self-isolation in reducing the number of cases, our study emphasizes the importance of surveillance testing and contact tracing of the contacts and confirmed cases in curtailing the pandemic in South Africa.     To investigate the efficacy and safety of TAF therapy for preventing hepatitis B mother-to-child transmission.

    Mothers with chronic HBV infection, positive for hepatitis B e-antigen and with HBV DNA >200 000 IU/mL received TAF for preventing mother-to-child transmission were enrolled retrospectively from multiple centres with data collection on mother-infant dyads up to postpartum week 24-28. Primary measurements were the mother-to-child transmission rate and infants' malformation rate. Secondary assessments included maternal HBV DNA reduction at delivery, and maternal or infant adverse events during follow up.

    Among 71 mothers enrolled, the mean (±SD) age was 30.3 (±2.2) years. TAF was initiated during the second or third trimester and continued to delivery with a mean (±SD) duration of 12.8 (±4.0) weeks. At delivery, 85.9% (61/71) of the mothers achieved HBV DNA <200 000 IU/L. Seventy-three infants (two sets of twins) were born from mothers treated with TAF and none had congenital defects or malformations. All infants received HBV immunoglobulin and vaccine at birth with additional HBV vaccinations at one and six months. At age 24-28 weeks, all infants had negative hepatitis B surface antigen and undetectable levels of HBV DNA (<100 IU/mL). Body weight, height, and head circumferences were comparable to national standards for physical development. No severe adverse effects were reported in either mothers or infants.

The obtained IrOx /Ir interface showed an overpotential of 250 mV at 10 mA cm-2 in 0.1 m HClO4 and remained stable under electrochemical conditions. The IrOx that was mechanically separated from the surface of IrOx /Ir metal after operation showed a threefold increase in activity compared to the current benchmark IrO2 catalyst. Various characterization analyses were used to identify the structure and morphology of the catalyst, which suggested nanosized, porous, and amorphous IrOx on the surface of metallic Ir. This synthetic approach can inspire a variety of opportunities to design and synthesize efficient metal oxide-based electrocatalysts for sustainable energy conversion and utilization.

  • This paper presents a comprehensive kinetic study of the catalytic pyrolysis of high-density polyethylene (HDPE) utilizing thermogravimetric analysis (TGA) data. Nine runs with different catalyst (HZSM-5) to polymer mass ratios (0.5, 0.77, and 1.0) were performed at different heating rates (5, 10, and 15 K/min) under nitrogen over the temperature range 303-973 K. Thermograms showed clearly that there was only one main reaction region for the catalytic cracking of HDPE. In addition, while thermogravimetric analysis (TGA) data were shifted towards higher temperatures as the heating rate increased, they were shifted towards lower temperatures and polymer started to degrade at lower temperatures when the catalyst was used. Furthermore, the activation energy of the catalytic pyrolysis of HDPE was obtained using three isoconversional (model-free) models and two non-isoconversional (model-fitting) models. Moreover, a set of 900 input-output experimental TGA data has been predicted by a highly efficient developed artificial neural network (ANN) model. Results showed a very good agreement between the ANN-predicted and experimental values (R2 > 0.999). Besides, A highly-efficient performance of the developed model has been reported for new input data as well.We aimed to investigate the management of urgent dental care, the perception of risk and workplace preparedness among dental staff in Norway during the COVID-19 pandemic. An electronic questionnaire regarding the strictest confinement period in Norway (13 March-17 April 2020) was distributed to dental staff. Among the 1237 respondents, 727 (59%) treated patients, of whom 170 (14%) worked in clinics designated to treat patients suspected or confirmed to have COVID-19. Out of them 88% (143) received training and 64% (103) simulation in additional infection prevention procedures, while 27 (24%) respondents reported deviation. In total, 1051 (85%) respondents perceived that dental staff had a high risk of being infected, 1039 (84%) that their workplace handled the current situation well, 767 (62%) that their workplace had adequate infection control equipment and 507 (41%) agreed that their workplace is well equipped to handle an escalation. Before an appointment, 1182 (96%) respondents always/often inquired per phone information if a patient experienced symptoms of COVID-19, and 1104 (89%) asked about a history of travel to affected areas. Twice as many patients on average per week were treated by phone than in a clinic. A lower proportion of dental staff in high incidence counties applied additional infection prevention measures compared to low and medium incidence counties. To conclude, urgent dental health care was managed relatively well in Norway. Additional training of the dental staff in adequate infection prevention and step-by-step procedures may be needed.     This paper presents a comprehensive kinetic study of the catalytic pyrolysis of high-density polyethylene (HDPE) utilizing thermogravimetric analysis (TGA) data. Nine runs with different catalyst (HZSM-5) to polymer mass ratios (0.5, 0.77, and 1.0) were performed at different heating rates (5, 10, and 15 K/min) under nitrogen over the temperature range 303-973 K. Thermograms showed clearly that there was only one main reaction region for the catalytic cracking of HDPE. In addition, while thermogravimetric analysis (TGA) data were shifted towards higher temperatures as the heating rate increased, they were shifted towards lower temperatures and polymer started to degrade at lower temperatures when the catalyst was used. Furthermore, the activation energy of the catalytic pyrolysis of HDPE was obtained using three isoconversional (model-free) models and two non-isoconversional (model-fitting) models. Moreover, a set of 900 input-output experimental TGA data has been predicted by a highly efficient developed artificial neural network (ANN) model. Results showed a very good agreement between the ANN-predicted and experimental values (R2 > 0.999). An electronic questionnaire regarding the strictest confinement period in Norway (13 March-17 April 2020) was distributed to dental staff. Among the 1237 respondents, 727 (59%) treated patients, of whom 170 (14%) worked in clinics designated to treat patients suspected or confirmed to have COVID-19. Out of them 88% (143) received training and 64% (103) simulation in additional infection prevention procedures, while 27 (24%) respondents reported deviation. In total, 1051 (85%) respondents perceived that dental staff had a high risk of being infected, 1039 (84%) that their workplace handled the current situation well, 767 (62%) that their workplace had adequate infection control equipment and 507 (41%) agreed that their workplace is well equipped to handle an escalation. Before an appointment, 1182 (96%) respondents always/often inquired per phone information if a patient experienced symptoms of COVID-19, and 1104 (89%) asked about a history of travel to affected areas. Twice as many patients on average per week were treated by phone than in a clinic. A lower proportion of dental staff in high incidence counties applied additional infection prevention measures compared to low and medium incidence counties. Knocking it out or suppressing it by siRNA leads to the upregulation of inflammation-associated genes on lymphatic endothelial cells and a more pro-inflammatory phenotype of interacting dendritic cells in vitro and in vivo. In striking contrast, anti-CD73 antibodies had only negligible effects on the gene expression of lymphatic- and blood-endothelial cells. Our data thus reveal new functions of lymphatic CD73 and indicate a low likelihood of endothelial cell-related adverse effects by CD73 targeting therapeutic antibodies.

    To identify whether sporadic anovulation is an important determinant of cumulative pregnancy rates and time to pregnancy among fertile women with regular menstrual cycles.

    We simulated cumulative pregnancy rates and time to pregnancy for 12 consecutive menstrual cycles among 100000 women based on data-driven probabilities of implantation, fertilisation, ovulation, and intercourse occurring in the fertile window. We assumed anovulation probabilities of 1%, 8%, or 14.5% and intercourse averaging once per week, every other day, or daily. The model incorporated reductions in implantation and fertilisation rates for successive cycles of non-pregnancy.

    After 12 cycles, a reduction in the per cycle incidence of anovulad fertile women with regular menstrual cycles, routine investigation of anovulation may not be an informative outcome in studies of fecundability, and routine testing to ensure ovulation and treatment of anovulation are unlikely to be medically necessary. While biomarkers or cervical fluid may help time intercourse to the fertile window, time to pregnancy can also be improved through increasing the frequency of unscheduled intercourse. The aim of this study was to determine whether a queuing theory model that involves lipopolysaccharide-mediated macrophage activation in tandem with changes in intracellular Cd and zinc (Zn) content or a lack thereof, would be useful to predict their impact on immune activation. We then validate our model with biologic cytokine output from human macrophages relative to the timing of innate immune activation. We believe that our results further prove the validity of the queuing theory approach to model intracellular molecular signaling and postulate that it can be useful to predict additional cell signaling pathways and the corresponding biological outcomes.
    The aim of this study was to estimate the incidence of and risk factors for Pneumocystis pneumonia (PCP) infection in diffuse large B-cell lymphoma (DLBCL) patients treated with rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP).

    The medical records of 739 DLBCL patients who received R-CHOP between May 2004 and January 2019 were retrospectively evaluated. Patients were divided into two groups those who received primary PCP prophylaxis (prophylaxis group) and those who did not (control group). The incidence rate of PCP in each group was calculated, and risk factors for PCP were evaluated in the control group.

    Baseline characteristics were significantly different between the two groups. Reader reproducibility was determined using the κ coefficient. The main outcome was the accuracy of MP dichotomy in predicting whether patients had tumor responsiveness or not. Results Among 278 cases, MTMP and STMP accounted for 49.6 and 50.4% of MPs, respectively. A total of 72 patients received neoadjuvant chemoradiotherapy and 88 received chemotherapy. The tumor responsiveness rate in the chemoradiotherapy group was higher than that in the chemotherapy group (58.3 vs. 21.6%, P less then 0.001). In the chemotherapy group, the tumor responsiveness rate in patients with MTMPs was lower than that in patients with STMPs (4.9 vs. 25.5%, P = 0.002). The baseline MRI-defined MTMP was associated with lower responsiveness rates after NAT in the chemotherapy group (odds ratio, 11.050, with 95% CI, 2.368-51.571, P = 0.002). Methods We retrospectively analyzed the records of 122 adult T-ALL patients diagnosed and treated at our center between January 2014 and June 2019. All the patients enrolled were categorized into ETP and non-ETP ALL by immunophenotype, and further statistical analyses about clinical data and prognostic factors were performed. Results Among the 122 cases, the male-to-female ratio was 2.81, and the median age is 29 (range, 16-82) years. Except for 10 patients with insufficient immunophenotyping results, 47.3% (53/112) are ETP and 52.7% (59/112) are non-ETP. Compared with non-ETP patients, ETP-ALL patients had lower white blood cell counts and lactate dehydrogenase levels, while they were older and had higher platelet counts and fibrinogen levels (all p 0.05). In the landmark analysis of CR1 patients who had a survival of more than 6 months, the allo-SCT group had significantly better survival outcomes than the chemotherapy group, and the 2-year OS rates and RFS rates were 80.1 ± 7.3 vs. 28.4 ± 8.4% and 68.9 ± 8.8 vs. 12.8 ± 7.2%, respectively (both p less then 0.0001). Methods We retrospectively analyzed the records of 122 adult T-ALL patients diagnosed and treated at our center between January 2014 and June 2019. All the patients enrolled were categorized into ETP and non-ETP ALL by immunophenotype, and further statistical analyses about clinical data and prognostic factors were performed. Results Among the 122 cases, the male-to-female ratio was 2.81, and the median age is 29 (range, 16-82) years. Except for 10 patients with insufficient immunophenotyping results, 47.3% (53/112) are ETP and 52.7% (59/112) are non-ETP. Compared with non-ETP patients, ETP-ALL patients had lower white blood cell counts and lactate dehydrogenase levels, while they were older and had higher platelet counts and fibrinogen levels (all p 0.05). In the landmark analysis of CR1 patients who had a survival of more than 6 months, the allo-SCT group had significantly better survival outcomes than the chemotherapy group, and the 2-year OS rates and RFS rates were 80.1 ± 7.3 vs. 28.4 ± 8.4% and 68.9 ± 8.8 vs. 12.8 ± 7.2%, respectively (both p less then 0.0001). A multivariate analysis suggests that allo-SCT acts as an independent prognostic factor for both OS and RFS. Conclusions Our results revealed that ETP accounted for a high proportion of T-ALL in Chinese. There are no CR rates and prognosis differences between ETP and non-ETP.     There is little published data regarding bivalirudin anticoagulation for surgical neonates on extracorporeal membrane oxygenation (ECMO). This study described our perioperative anticoagulation protocol and evaluated the relationship of bivalirudin dose to activated partial thrombin time (aPTT) and thromboelastography reaction time (TEG-R) monitoring assays.

    Neonates with congenital diaphragmatic hernia (CDH) on ECMO and single-agent bivalirudin anticoagulation at our institution from 2016 to 2018 were included. Bivalirudin infusion rates, laboratory results, transfusions, and clinical events during the initial (cannulation to repair) and postoperative (up to 60h post-repair) periods were recorded.

    Forty-two neonates met inclusion criteria. Bivalirudin was started at 0.16mg/kg/h and titrated in 10-20% increments to target aPTT of 70-85s and TEG-R of 9-12min. All patients achieved target anticoagulation levels within the first 12h on doses ranging from 0.12-0.36mg/kg/h. Analyses of Escherichia coli (EC) and spores of Clostridium perfringens (CpSP) in raw and treated effluents as well as pathogen / indicator ratios from the literature were used to estimate the concentrations of Escherichia coli O157H7 (EC O157H7) and Cryptospodirium spp. (Crypto) in the air, and the results were applied to an atmospheric microbiological dispersion model. From the concentrations of pathogens in the air, infectious risks for downwind receptors were calculated. The risks of infection by EC O157H7 to workers at 10 m and 50 m away from the emission source ranged between 3.81 × 10 1 and 2.68 × 10 3 pppy (per person per year), whereas to residents at 100 m and 500 m ranged from 4.59 × 10 1 to 1.51 × 10 4 pppy. Peak values (95th percentile) of occupational and public risks associated with the exposure to Crypto were 3.41 × 10 3 and 6.84 × 10 4 pppy at 10 m and 50 m from the source, respectively, and were lower than 1.48 × 10 6 pppy regarding exposures to CpSP. Anaerobic digestion reduced risks by approximately one order of magnitude. The distance from the source was inversely proportional to the risk of exposure. It is recommended that wastewater is treated prior to its reuse and the adoption of application methods with low aerosolization potential. In addition, the need for workers to use personal protective equipment (PPE) is highlighted.
    Psychological morbidity in both patients and family members related to the intensive care unit (ICU) experience is an often overlooked, and potentially persistent, healthcare problem recognized by the Society of Critical Care Medicine as Post-intensive Care Syndrome (PICS). ICU diaries are an intervention increasingly under study with potential to mitigate ICU-related psychological morbidity, including ICU-related post-traumatic stress disorder (PTSD), depression and anxiety. As we encounter a growing number of ICU survivors, in particular in the wake of the coronavirus pandemic, clinicians must be equipped to understand the severity and prevalence of significant psychiatric complications of critical illness.

    We compared the efficacy of the ICU diary, written by family and healthcare workers during the patient's intensive care course, versus education alone in reducing acute PTSD symptoms after discharge.   • Background Hydatid cyst is a zoonotic infection caused by Echinococcosis granulosus. Primary single-intramuscular hydatid disease is rare, even in endemic regions of the world. Here we report the case of exceptional thigh mass due to a hydatid cyst in an Iranian man. Case presentation An 86-year-old man, initially presented to Velayat teaching hospital surgery clinic in May 2017 with a single right-thigh mass, but physical examinations of other organs were unremarkable. Based on sonographic findings, the differential diagnosis was hydatid cyst. He underwent surgical resection of the cyst. Histopathological results confirmed the diagnosis. There was no evidence of recurrence of the lesion during the 23-month follow-up. Conclusion Increase thigh mass due to a hydatid cyst is a rare event. In endemic regions with the presence of hydatid cysts, especially physicians of surgical clinics have to consider differential diagnosis of hydatid cysts in unusual locations in case of such a lesion.A 67-year-old man was referred for iris color change. Peau d'orange appearance was seen as alternating dark and bright patches on color fundus photography with corresponding hyporeflective and hyperreflective patches on infrared reflectance. Comet lesions showed increased signal on infrared reflectance and hyperautofluorescence. Multicolor imaging is a non-invasive imaging modality which helps in clearly delineating these lesions.A healthy 6-year-old boy presented with acute bilateral vision loss, multiple serous retinal detachments between the vascular arcades and a thickened choroid. Spontaneous resolution occurred over several weeks. We hypothesize that the clinical constellation in our patient is suggestive of acute exudative polymorphous vitelliform maculopathy (AEPVM) or might be an atypical presentation of Vogt-Koyanagi-Harada (VKH) disease. The central corneal thickness of the graft measured 584 µm (OD) and 544 µm (OS), whilst the peripheral host thickness (8 mm zone), however, was 1233 µm (OD, cranial) and 1131 µm (OS, nasal). The original graft diameter measured 6 mm in both eyes and the recipient cornea was cloudy and gray. The endothelial cell count was measured centrally (OD 1162 c/mm2, OS 1320 c/mm2). The visual acuity was 20/100 (OD) and 20/40 (OS). After excimerlaser-assisted repeated PKP (8.0/8.1 mm, OD), the histological analysis of the former graft revealed deposits of acid mucopolysaccharides (AMP) subepithelially, within the interface, in the donor stroma, and in the endothelium, which proved the peripheral recurrence of a macular corneal stromal dystrophy on the graft. This involves targeting the ventricular septum through resection or ablation, combined at times with mitral valve replacement or plication of the valve leaflet to relieve LVOT obstruction. In patients with symptoms refractory to medical management who are at prohibitive surgical risk, additional options for less-invasive approaches for the management of HCM are needed. We describe here the successful non-surgical catheter-based management of a 72-year-old woman at high surgical risk, debilitating symptoms refractory to maximal medical management, and severe, diffuse-type HCM. Edge-to-edge repair with MitraClip (Abbott Vascular) was used to target SAM causing dynamic LVOT obstruction, with resulting significant reduction in LVOT gradient and dramatic clinical improvement. Her postprocedure outcomes to 2 years are reported herein. Additionally, we review the current management strategies for HCM management, and include a discussion of minimally invasive options.

Developing unobtrusive remote monitoring systems to detect early warning signs of impending symptomatic relapses would allow clinicians to intervene before the patient's condition worsens.

    In this study, we aim to create the first models, exclusively using passive sensing data from a smartphone, to predict behavioral anomalies that could indicate early warning signs of a psychotic relapse.

    Data used to train and test the models were collected during the CrossCheck study. Hourly features derived from smartphone passive sensing data were extracted from 60 patients with SSDs (42 nonrelapse and 18 relapse >1 time throughout the study) and used to train models and test performance. We trained 2 types of encoder-decoder neural network models and a clustering-based local outlier factor model to predict behavioral anomalies that occurred within the 30-day period before a participant's date of relapse (the near relapse period).
