    A total of 911/1249 (72.93%) eligible physicians completed the questionnaire. Regarding the most concerning topic of whether breast cancer patients could conceive, 65 (7.1%) physicians having low and 457 (50.2%) physicians having high propensity for recommending reproduction. For ductal carcinoma in situ (DCIS) after surgery and radiotherapy, 599 (65.8%) physicians did not agree with the recommendation to conceive. 231 (25.4%) highly agree with the recommendation ofion guidelines recommendation when to reproduce in different circumstances for breast cancer patients, physicians from China remained a relatively conservative attitude.     Cisplatin plus S-1 (CS) is the standard first-line chemotherapy for advanced gastric cancer (AGC) in Japan. A previous phase III trial showed that docetaxel plus S-1 (DS) was effective for AGC without measurable lesions, but no studies have compared these two regimens.

    Eligible patients had unresectable or recurrent HER2-negative AGC without measurable lesions. Patients were randomized to DS (docetaxel 40mg/m
    on day 1, S-1 80-120mg on days 1-14, every 3weeks) or CS (cisplatin 60mg/m
    on day 8, S-1 80-120mg on days 1-21, every 5weeks). The primary endpoint was overall survival (OS).

    All patients had unresectable primary disease. Sixty-one patients were randomly assigned to DS (n = 30) or CS (n = 31). One CS patient was ineligible due to HER2 positivity. The median number of cycles was 9.5 (range 2-49) with DS and 5.5 (range 1-10) with CS. There were no treatment-related deaths. The most common grade 3-4 non-hematological toxicity was fatigue (7% with DS, 13% with CS), followed by anorexia (3% with DS, 10% with CS) and diarrhea (3% with DS, 10% with CS). The 2-year OS rates were 43.3% with DS and 30.0% with CS (log-rank P = 0.113), with a hazard ratio of 0.617 (95% confidence interval 0.337-1.128), indicating non-inferiority of DS to CS with respect to OS (P < 0.001).

Results showed that altering dimensions influenced mechanical properties (compressive force, tensile strength and resistance of removal thread), in vitro drug release and water uptake. The removal thread resistance was increasing with increasing height and width. Compression force was higher for the rings with smaller diameter. The total drug release was increasing with decreasing height and rising diameter, surface area and water uptake during dissolution test. The initial dissolution rate was slower for the rings with higher width. As the best candidate for use in model dog subjects, the ring with 30 mm diameter, 3 mm height and 7.5 mm width was found. Near-infrared imaging was used to determine the water content of rice, infrared was used to assess plant temperature, aote sensors are being applied to solve this problem.   • The process of nitrate dissimilation to ammonium (DNRA) is an important way for storing nitrogen in nature and DNRA is a key step in efficient recovery of nitrogen in wastewater. However, in view of the low conversion efficiency of DNRA, zero-valent iron (ZVI) was used to enhance the DNRA process of Desulfovibrio sp. CMX. ZVI can obviously promote the nitrate/nitrite reduction. The experiment indicated that 5 g/L 300 mesh ZVI could convert 5 mmol/L nitrate or nitrite to ammonium in 48 h or 36 h respectively, and the conversion ratio of NO2- to NH4+ could reach more than 90%. The ZVI provided a suitable growth environment for the Desulfovibrio sp. CMX through chemical reduction of nitrite, production of divalent iron (Fe2+), reduction of oxidation-reduction potential (ORP) and adjustment of pH, which strengthened the DNRA performance. As for AcoD system, dominant genera Methanosaeta, Methanoculleus, Methanobacterium, Fastidiosipila, Rikenellaceae RC9, Bifidobacterium and Xeromyces had close relationships with metabolism of cofactors and vitamins, energy metabolism, methane metabolism, carbohydrate metabolism and glycosaminoglycan degradation (P less then 0.05). In this review, different commonly used culture systems for microalgae are discussed; the current harvesting methods with different culture systems have also been evaluated. Also, the inherent characteristics of inefficiency in algae wastewater treatment are elaborated. Current literature collectively supports that a biofilm type device is a system designed for higher biomass productivity, and offers ease of harvesting, in small-scale algae cultivation. We employed a high-quality long-term mortality time series (1982-2017) to evaluate impacts of extreme winter weather in the Czech Republic. We aimed to clarify whether compound events of extreme weather cause larger impacts on mortality than do each type of extreme if evaluated individually. Using daily data from the E-OBS and ERA5 datasets, we analyzed 9 types of extreme events extreme wind gust, precipitation, snowfall, and sudden temperature and pressure changes. Relative mortality deviations from the adjusted baseline were used to estimate the immediate effect of the selected extreme events on excess mortality. The impact was adjusted for the effect of extreme cold. Extreme events associated with sudden rise of minimum temperature and pressure drops had generally significant impact on excess mortality (3.7% and 1.4% increase). The impacts were even more pronounced if these events occurred simultaneously or were compounded with other types of extremes, such as heavy precipitation, snowfall, maximum temperature rise, and their combinations (increase as great as 14.4%). Effects of some compound events were significant even for combinations of extremes having no significant impact on mortality when evaluated separately. On the other hand, a "protective" effect of pressure increases reduced the risk for its compound events. Meteorological patterns during extreme events linked to excess mortality indicate passage of a low-pressure system northerly from the study domain. We identified extreme winter weather events other than cold temperatures with significant impact on excess mortality. Our results suggest that occurrence of compound extreme events strengthen the impacts on mortality and therefore analysis of multiple meteorological parameters is a useful approach in defining adverse weather conditions.
    The process of nitrate dissimilation to ammonium (DNRA) is an important way for storing nitrogen in nature and DNRA is a key step in efficient recovery of nitrogen in wastewater. However, in view of the low conversion efficiency of DNRA, zero-valent iron (ZVI) was used to enhance the DNRA process of Desulfovibrio sp. CMX. ZVI can obviously promote the nitrate/nitrite reduction. The experiment indicated that 5 g/L 300 mesh ZVI could convert 5 mmol/L nitrate or nitrite to ammonium in 48 h or 36 h respectively, and the conversion ratio of NO2- to NH4+ could reach more than 90%. The ZVI provided a suitable growth environment for the Desulfovibrio sp. CMX through chemical reduction of nitrite, production of divalent iron (Fe2+), reduction of oxidation-reduction potential (ORP) and adjustment of pH, which strengthened the DNRA performance. This experiment is advantageous for increasing efficiency of DNRA and provides a new idea for efficient recovery of nitrogen resources.In order to evaluate microbial community structure dominated metabolic function profiles in large-scale food waste (FW) biotreatment systems, bacterial, archaeal and fungal community associated with metabolic function in high-temperature aerobic fermentation (AF) and anaerobic co-digestion (AcoD) processes were comprehensively investigated in this study. https://www.selleckchem.com/products/iwp-4.html The qPCR results showed the higher gene copies of bacteria and fungi in initial and AF-treated FW compared with AcoD-treated FW, as well as bacteria and archaea in AcoD-treated FW were highly abundant among detected samples. Abundant Aspergillus, Staphylococcus, Pelomonas, Corynebacterium, Faecalibacterium, Methanobacterium and Xeromyces in AF system were positively and significantly correlated with high metabolic activities of energy metabolism, carbohydrate metabolism, amino acid metabolism, fatty acid metabolism, glycosaminoglycan degradation, sulfur metabolism and nitrogen metabolism. As for AcoD system, dominant genera Methanosaeta, Methanoculleus, Methanobacterium, Fastidiosipila, Rikenellaceae RC9, Bifidobacterium and Xeromyces had close relationships with metabolism of cofactors and vitamins, energy metabolism, methane metabolism, carbohydrate metabolism and glycosaminoglycan degradation (P less then 0.05). Microalgae culture combined with wastewater treatment is a promising approach to improve the sustainability of the business model. However, algae culture and harvest account for the majority of the high costs, hindering the development of the microalgae-based wastewater utilization. Cost-effective culture systems and harvesting methods for enhancing biomass yield and reducing the cost of resource recovery have become extremely urgent and important. In this review, different commonly used culture systems for microalgae are discussed; the current harvesting methods with different culture systems have also been evaluated. Also, the inherent characteristics of inefficiency in algae wastewater treatment are elaborated. Current literature collectively supports that a biofilm type device is a system designed for higher biomass productivity, and offers ease of harvesting, in small-scale algae cultivation. Additionally, bio-flocculation, which uses one kind of flocculated microalgae to concentrate on another kind of non-flocculated microalgae is a low-cost and energy-saving alternative harvesting method. The primary outcome measure was shoulder pain and disability index (SPADI) at 26 weeks. An economic analysis was conducted. Results and Conclusion 136 patients were recruited, 68 randomised to each group. Recruitment was 68% of the target 200 participants. SPADI (from baseline to 26 weeks) demonstrated a difference (SE) in mean change between groups of -0.43 (5.7) (p-value = 0.050001), and the TOST (two-one-sided test for equivalence) 90% CI for this difference was (-10.0 to 9.14). This was borderline. In a secondary analysis using inputted data, patients without SPADI at week 26 were analysed by carrying forward scores at week 12 (mean difference (95% CI) = -0.14 (-7.5 to 7.3), p-value = 0.014). There is little difference in outcome at 26 weeks. Group physiotherapy was cheaper to deliver per patient (£252 versus £84). Here, we investigated the therapeutic effect of 4-PBA on hepcidin expression and HCV replication. We used HCV genotype 1b Huh 7.5-Con1 replicon cells and engraftment of NOD/SCID **** as in vitro and in vivo models to test the effect of 4-PBA. It was found that 4-PBA inhibited HCV replication in Huh7.5-Con1 replicon cells in a concentration- and time-dependent manner through the induction of hepcidin expression by epigenetic modification and subsequent upregulation of interferon-α signaling. HCV formed a membranous web composed of double-membrane vesicles and was utilized for RNA replication. Moreover, 4-PBA also disrupted the integrity of the membranous web and interfered with the molecular interactions critical for the assembly of the HCV replication complex. The time-domain performance of the filter antenna was investigated in comparison to that of the designed reference wideband antenna. By comparing both antennas' time-domain characteristics, it was seen that the switchable filter antenna had good time-domain resolution along with the frequency-domain operation. Additionally, the time-domain investigation revealed that the switchable filter wide-band antenna performed similarly to the reference wide band antenna. This antenna was also utilized for a tumor detection application, and it was seen that the switchable filter wide-band antenna could detect a miniaturized irregularly shaped tumor easily, which is quite promising. A review of the present stent monitoring technology as well as the practical treatment for addressing stent restenosis was conducted. The problems and challenges associated with current stent monitoring technology were illustrated, along with its typical applications. Brief suggestions were given and the progress of stent implants was discussed. It was revealed that prime requisites are needed to achieve good quality implanted stent devices in terms of their size, reliability, etc.

Multiple molecular diagnostic methods received FDA emergency use authorization (EUA) and were promptly validated for use nationwide. Several studies reported the analytical and/ or clinical evaluation of these molecular assays, however differences in the viral materials used for these evaluations complicated direct comparison of their analytical performance. In this study, we compared the analytical sensitivity (lower limit of detection, LOD) of seven commonly used qualitative SARS-CoV-2 molecular assays the Abbott Molecular RealTime SARS-CoV-2 assay, the NeuMoDx™ SARS-CoV-2 assay, the Roche Cobas®SARS-CoV-2 assay, the BD SARS-CoV-2 reagents for BD MAX™ system, the Hologic Aptima® SARS-CoV-2 assay, the Xpert Xpress SARS-CoV-2 test, and the GenMark ePlex SARS-CoV-2 test. The comparison was performed utilizing a single positive clinical specimen that was serially diluted in viral transport media and quantified by the EUA approved SARS-CoV-2 droplet digital PCR (ddPCR) assay. Replicate samples were prepared and evaluated for reproducibility across different molecular assays with multiple replicates per assay. Our data demonstrated that the seven assays could detect 100 % of replicates at a nucleocapsid gene concentration of (N1 = 1,267 and N2 = 1,392) copies/mL. At a one log less concentration, the Abbott, the Roche, and the Xpert Xpress assays detected 100 % of the tested replicates.
    Acute respiratory distress syndrome (ARDS) is a serious respiratory condition with high mortality and associated morbidity. The objective of this study is to develop and evaluate a novel application of gradient boosted tree models trained on patient health record data for the early prediction of ARDS.

    9919 patient encounters were retrospectively analyzed from the Medical Information Mart for Intensive Care III (MIMIC-III) data base. XGBoost gradient boosted tree models for early ARDS prediction were created using routinely collected clinical variables and numerical representations of radiology reports as inputs. XGBoost models were iteratively trained and validated using 10-fold cross validation.

    On a hold-out test set, algorithm classifiers attained area under the receiver operating characteristic curve (AUROC) values of 0.905 when tested for the detection of ARDS at onset and 0.827, 0.810, and 0.790 for the prediction of ARDS at 12-, 24-, and 48-h windows prior to onset, respectively.

In a pandemic, RRT practice is likely to be determined by the number of patients, availability of machines, consumables and staff, clinical expertise, and acceptable alternatives. In this study, we compared the analytical sensitivity (lower limit of detection, LOD) of seven commonly used qualitative SARS-CoV-2 molecular assays the Abbott Molecular RealTime SARS-CoV-2 assay, the NeuMoDx™ SARS-CoV-2 assay, the Roche Cobas®SARS-CoV-2 assay, the BD SARS-CoV-2 reagents for BD MAX™ system, the Hologic Aptima® SARS-CoV-2 assay, the Xpert Xpress SARS-CoV-2 test, and the GenMark ePlex SARS-CoV-2 test. The comparison was performed utilizing a single positive clinical specimen that was serially diluted in viral transport media and quantified by the EUA approved SARS-CoV-2 droplet digital PCR (ddPCR) assay. Replicate samples were prepared and evaluated for reproducibility across different molecular assays with multiple replicates per assay. Our data demonstrated that the seven assays could detect 100 % of replicates at a nucleocapsid gene concentration of (N1 = 1,267 and N2 = 1,392) copies/mL. At a one log less concentration, the Abbott, the Roche, and the Xpert Xpress assays detected 100 % of the tested replicates. The whitefly Bemisia tabaci can establish mutualism with begomoviruses via their shared host plants. This mutualism is achieved by the interaction between virulence factors and their host proteins. While the virulence factor βC1 encoded by tomato yellow leaf curl China betasatellite (TYLCCNB), a subviral agent associated to the begomovirus tomato yellow leaf curl China virus (TYLCCNV), may interact with plant protein MYC2, thereby establishing the indirect mutualism between TYLCCNV and whitefly, whether other mechanisms are involved remains unknown. Here, we found the in vitro and in vivo interactions between βC1 and tobacco protein S-phase kinase associated protein 1 (NtSKP1). Silencing the expression of NtSKP1 enhanced the survival rate and fecundity of whiteflies on tobacco plants. NtSKP1 could activate the transcription of genes in jasmonic acid (JA) pathways by impairing the stabilization of JAZ1 protein. Moreover, βC1-NtSKP1 interaction could interfere JAZ1 degradation and attenuate the plant JA defense responses.     To determine the association between cataract surgery and age-related macular degeneration (AMD) in a representative US sample.

    Population-based, cross-sectional study.

    The US National Health and Nutrition Examination Survey 2005-2008.

    A total of 5401 participants aged ≥40 years had information in cataract surgery status and gradable retinal photographs for right eyes.

    Cataract surgery status was obtained from questionnaire. Non-mydriatic fundus photographs were collected and AMD status was assessed. The associations between AMD and cataract surgery were evaluated in right eyes using logistic regression models.

    Of 338 right eyes with any AMD, 107 right eyes (28.9%) had cataract surgery. After adjusting for multiple variables, there were significant associations between cataract surgery and any AMD (OR 1.36; 95% CI 1.03 to 1.81) or late AMD (OR 2.48; 95% CI 1.01 to 6.09). No significant association was found between cataract surgery and early AMD after adjusting for multiple covariates (OR 1.20; 95% CI 0.91 to 1.59).

    This study aimed to develop a detection scale for intimate partner violence (DS-IPV), excluding physical and sexual violence, and to validate the scale.

    A cross-sectional survey was undertaken using an anonymous 26-item self-administered questionnaire with volunteer women aged 18-59 residing in one prefecture in Japan. A total of 1,937 responses were obtained, and 1,915 were valid. The responses of the 1,277 women who had a partner at the time were included in the analysis. Statistical analysis examined exploratory/confirmatory factor analysis, internal consistency, and construct validity.

    A four-factor scale with 22 items was extracted for the DS-IPV. In the confirmatory factor analysis, the indices of fitness supported these results. Prior to the factor analysis, the Kiser-Mayer-Olkin measurement (.936) and Bartlett's sphericity test (p < .001) showed that all samples met the statistical criteria. Cronbach's alpha coefficient was .92 for total score and varied between .70 and .90 for each of the four factors, and internal consistency was confirmed. The following four factors emerged from an oblique factor analysis, with a cumulative variance of 58.1% "arousing anxiety"; "controlling behavior"; "coercion and threat"; and "daily emotions". These four factors had a moderate correlation (.23-.48, p < .01) with each other, which indicated construct validity. Confirmatory factor analysis showed the indices of fitness supported these results.

This research was conducted to explore the immunomodulatory influence and underlying mechanisms of GC/Alg DN hydrogel on DCs in vitro and in vivo. Stimulation of DCs with DN hydrogel obviously induced the maturation of DCs in vitro. In vivo, DN hydrogel did not have obvious influence on the maturation of splenic DCs on postimplantation days 3, 10, and 30. Mechanistically, we found that DN hydrogel induced the maturation of DCs via phosphorylation of phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin in vitro.     The clock-drawing test (CDT) is a widely used screening tool for detecting cognitive decline. However, normative data for Chinese individuals are scarce. Our study aimed to provide standardized values for the three quantitative CDT scoring methods that were tailored for Chinese-speaking adults in Shijiazhuang City and explore the discriminant validity of the CDT scores in patients with acute ischemic stroke.

    We conducted the CDT among 418 healthy individuals aged between 35 and 84years. The CDT was administered and scored by five raters using the method derived from the Montreal Cognitive Assessment (****), Rouleau's, and Babins' scoring systems. The influence of age, education, and sex on the performance in the CDT was analyzed. Furthermore, 336 patients with acute ischemic stroke were enrolled to explore the discriminant validity of CDT scores.

Our study aimed to provide standardized values for the three quantitative CDT scoring methods that were tailored for Chinese-speaking adults in Shijiazhuang City and explore the discriminant validity of the CDT scores in patients with acute ischemic stroke. We conducted the CDT among 418 healthy individuals aged between 35 and 84years. The CDT was administered and scored by five raters using the method derived from the Montreal Cognitive Assessment (MoCA), Rouleau's, and Babins' scoring systems. The influence of age, education, and sex on the performance in the CDT was analyzed. Furthermore, 336 patients with acute ischemic stroke were enrolled to explore the discriminant validity of CDT scores. In all three scoring systems, CDT scores were significantly correlated with age and years of education but not with sex. Normative data stratified for age and years of education were established.

  • SARS-CoV-2 infections are rapidly spreading around the globe. The rapid development of therapies is of major importance. However, our lack of understanding of the molecular processes and host cell signaling events underlying SARS-CoV-2 infection hinders therapy development. We use a SARS-CoV-2 infection system in permissible human cells to study signaling changes by phosphoproteomics. We identify viral protein phosphorylation and define phosphorylation-driven host cell signaling changes upon infection. Growth factor receptor (GFR) signaling and downstream pathways are activated. Drug-protein network analyses revealed GFR signaling as key pathways targetable by approved drugs. The inhibition of GFR downstream signaling by five compounds prevents SARS-CoV-2 replication in cells, assessed by cytopathic effect, viral dsRNA production, and viral RNA release into the supernatant. This study describes host cell signaling events upon SARS-CoV-2 infection and reveals GFR signaling as a central pathway essential for SARS-CoV-2 replication. It provides novel strategies for COVID-19 treatment.Neural stem cells (NSCs) in the dentate gyrus (DG) reside in a specialized local niche that supports their neurogenic proliferation to produce adult-born neurons throughout life. Specifically, stimulating CCK release supports neurogenic proliferation of NSCs through a dominant astrocyte-mediated glutamatergic signaling cascade. In contrast, reducing dentate CCK induces reactive astrocytes, which correlates with decreased neurogenic proliferation of NSCs and upregulation of genes involved in immune processes. Previously, an in vitro bioguided study showed the important ovicidal and larvicidal effects of an organic fraction (EtOAc-F) from a hydroalcoholic extract of A. farnesiana pods against Haemonchus contortus. The present study aimed to assess the in vivo anthelmintic effect of EtOAc-F from A. farnesiana pods on the H. contortus faecal egg elimination in female lambs and on the infective larvae (L3) population reduction in coprocultures. The EtOAc-F was obtained from a hydroalcoholic extract from A. farnesiana pods through chromatographic procedures; additionally, some secondary compounds were identified using high-performance liquid chromatography (HPLC). Twenty-one 'Katahdin' crossbred female lambs ranging from three to four months of age, with body weights 21.9 ± 0.39 kg were used. Animals were orally infected with H. contortus (L3) by a single dose of 350 L3/kg BW. Three experimental groups (n = 7) were assigned as follows 1) Control (untreated), 2) Albendazole, as a positive control (at 7.5 mg/kg BW, unique dose) and 3) EtOAc-F (at 100 mg/kg BW, once every third day, with three applications in total). Individual faecal samples were collected once a week for 5 weeks (at days 38, 45, 52, 59 and 66) post-treatment, to measure the faecal egg counts (FEC) and to obtain the H. contortus (L3) population from faecal cultures. The highest FEC reduction caused by EtOAc-F was 67.7%; meanwhile, albendazole showed a total FEC reduction after the second week post-treatment (day 45). On the other hand, the fraction caused an important reduction in the larval population in coprocultures (54.3-68.5%). The phytochemical analysis revealed the presence of galloyl derivatives and flavonoids as major compounds. However, the therapeutic effects of PP9 on CRC and the underlying molecular mechanism remain undefined. Here, we demonstrated that treatment with PP9 time- and dose-dependently inhibited HT-29 and HCT116 cells without significantly inhibiting normal NCM460 cells. Furthermore, our results indicated that PP9 effectively induced G2/M phase arrest by upregulating p21 and suppressing cdc25C, Cyclin B1 and cdc2. Meanwhile, PP9 upregulated cleaved Caspase 3, cleaved Caspase 9 and cleaved PARP and Bax, while downregulating Bcl-2 to stimulate cell apoptosis. Mechanistically, PP9-suppressed PI3K/Akt/GSK3β signaling, while the PI3K inhibitor LY294002 augmented PP9-mediated apoptosis, G2/M arrest and effects on PI3K/Akt/GSK3β related proteins. Finally, we showed that PP9 (10 mg/kg) significantly reduced tumor growth in nude mouse CRC xenografts, more potently than 5-Fu (20 mg/kg). Jointly, these data firstly demonstrated that PP9 promotes G2/M arrest and apoptotic death in CRC cells through PI3K/Akt/GSK3β signaling suppression, suggesting that PP9 could be considered a new and promising candidate for CRC therapy.
    This study aimed at assessing current efficacy and safety of endoscopic combined intrarenal surgery (ECIRS) for the treatment of large and/or complex urolithiasis and identifying relevant tips and tricks able to improve its outcomes, mainly deriving from the adjunct of retrograde flexible ureteroscopy to the traditional antegrade approach of percutaneous nephrolithotomy (PNL).

    A systematic review was conducted using relevant databases (Ovid Medline, PubMed, Scopus, and Web of Sciences), employing "ECIRS" as the search term in all cases, and then adding "endoscopic combined intrarenal surgery" and "flexible ureteroscopy AND percutaneous nephrolithotomy" as search terms for PubMed and Scopus. Original articles and systematic reviews were selected according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Additionally, the reference lists of the selected publications were checked manually.

    A total of 14 studies were selected for analysis two systematic reviews, one randomized controlled trial, five nonrandomized comparative studies, three prospective case series, and three retrospective case series.
