Based on first-principles calculations, we propose a novel two-dimensional (2D) germanium carbide, tetrahex-GeC2, and determine its electronic and optical properties. Each Ge atom binds to four C atoms, in contrast to the known 2D hexagonal germanium carbides. Monolayer tetrahex-GeC2 possesses a narrow direct band gap of 0.89 eV, which can be effectively tuned by applying strain and increasing the thickness. Its electron mobility is extraordinarily high (9.5 × 104 cm2/(V s)), about 80 times that of monolayer black phosphorus. The optical absorption coefficient is ∼106 cm-1 in a wide spectral range from near-infrared to near-ultraviolet, comparable to perovskite solar cell materials. We obtain high cohesive energy (5.50 eV/atom), excellent stability, and small electron/hole effective mass (0.19/0.10 m0). Tetrahex-GeC2 turns out to be a very promising semiconductor for nanoelectronic, optoelectronic, and photovoltaic applications.An efficient strategy that can guide the synthesis of materials with superior mechanical properties is important for advanced material/device design. Here, we report a feasible way to enhance hardness in transition-metal monocarbides (TMCs) by optimally filling the bonding orbitals of valence electrons. The top experimental polarization of 36.89 μC/cm2, optimal d33 value of 258 pC/N measured at room temperature, and ultrahigh d33 value of 303 pC/N measured at 370 °C were obtained at 0.20 PT, thereby further confirming the calculations. Furthermore, a high Curie point of 488 °C, as well as outstanding temperature stability ranging from room temperature to 430 °C of the 0.20 PT ceramic was observed. The availability and access to clinical genetic testing has improved dramatically such that genetic testing is considered an integral part of the clinical evaluation for patients with thoracic aortic disease. This review provides an update on our current understanding of the genetic basis of thoracic aortic disease, practical recommendations for genetic testing, and clinical implications.
In this video, we present our novel technique for myometrial defect closure following robot-assisted laparoscopic adenomyomectomy.
A narrated video demonstration of our technique. Our patient was a 47-year-old single woman with severe dysmenorrhea, who did not respond to medical therapy and wished to preserve her uterus. Surgery was performed after thorough counseling and obtaining informed consent from the patient (Institutional Review Board number KC17OESI0238; approval date March 19, 2018). After removal of the adenomyotic tissue during surgical intervention, the myometrial defect was closed in three steps. First, the defect between the anterior and posterior innermost myometrial layers was closed using a 2-0 Stratafix suture, CT-1 (circle taper) needle (Ethicon, Somerville, NJ, USA). Next, the two sides were approximated using a 2-0 PDS® (polydioxanone) Suture (Ethicon, Somerville, NJ, USA) and V-34 (TAPERCUT®) surgical needle (Ethicon, Somerville, NJ, USA). Finally, the serosa was sutured in a baseball fashion using a 2-0 PDS suture, slim half-circle [SH] needle (Ethicon, Somerville, NJ, USA).

It is possible that HMGB1 may be an important mediator of the "cytokine-storm syndrome." Notably, high plasma levels of HMGB1 have been reported in patients diagnosed with severe COVID-19, and there is a significant negative correlation between HMGB1 plasma levels and clinical outcomes. Nicotine can activate the cholinergic anti-inflammatory reflex, which attenuates the up-regulation and the excessive release of pro-inflammatory cytokines/chemokines. Therefore, we hypothesize that low molecular weight compounds that activate the cholinergic anti-inflammatory reflex, such as nicotine or GTS-21, may represent a potential therapeutic approach to attenuate the dysregulated inflammatory responses in patients with severe COVID-19.Topotecan is potent anti-cancer drug approved for various malignancies but hematopoietic toxicities undermine its wider application and use of its most effective dose. This study aims to improve these limitations through inhalation-delivery. The pharmacokinetics, efficacy, and toxicity of 2-5 times lower inhalation doses of topotecan dry-powder were compared with the standard intravenous (IV) delivery once/twice-a-week. Human-derived EGFR-mutant (H1975), KRAS-mutant (A549), and EGFR/KRAS wild-type (H358) orthotopic and distant lung tumors were evaluated in murine models. Inhalation of 1 mg/kg topotecan significantly improved the half-life and drug exposure (area under the curve, AUC) compared to 5 mg/kg via IV-delivery. AUCs (h*ng/mL) for inhaled/IV topotecan in plasma, lung, liver, and brain were, 831/888, 60,000/1080, 8380/4000, and 297/15, respectively; while the half-life was also greatly increased in these tissues. The average lung tumor burden of H358-derived tumors was reduced from 15.0 g to 8.4 g (44%) in rats treated once-a-week with 2 mg/kg IV and 1.8 g (88%) with 1 mg/kg inhaled topotecan, corroborating previous findings using A549- and H1975-derived orthotopic lung tumors. Importantly, inhaled topotecan showed superior efficacy in suppressing lung tumors at distant sites. The growth of H1975- and H358-derived subcutaneous xenografts were completely arrested and A549-derived tumors were significantly reduced in **** treated twice-a-week with 1 mg/kg inhaled topotecan compared to a minor (H1975 and H358) or no reduction (A549) with twice-a-week 5 mg/kg IV topotecan.Social attention is the ability to share with social partners the attentional focus on an object and to orient attentional resources after observing others' behaviors. Research institutes and pharmaceutical companies have been working on repurposing existing drugs as effective therapeutic options against TB. The identification of suitable drugs with multi-target affinity profiles is a widely accepted way to combat the development of resistance. Flavin-dependent thymidylate synthase (FDTS), known as ThyX, is in the class of methyltransferases and is a possible target in the discovery of novel anti-TB drugs. In this study, we aimed to repurpose existing drugs approved by Food and Drug Administration (FDA) that could be used in the treatment of TB. An integrated screening was performed based on computational procedures high-throughput molecular docking techniques, followed by molecular dynamics simulations of the target enzyme, ThyX. We performed molecular docking and molecular dynamic studies of DBL3x complexed with CSA and five analogues, where the sulfate group was replaced by phosphate, in order to evaluate if the better electrostatic interactions provided by phosphate groups could afford better binders capable of preventing the binding of CSA to DBL3x. Results suggest that all proposed compounds have high affinity towards DBL3x and could bind better to the DBL3x domain of PfEMP1 than CSA, qualifying as potential inhibitors of this protein and, therefore, new potential leads for the drug design against malaria.Communicated by Ramaswamy H.

Efficient removal of uranium (U) from aqueous solutions is crucial for ecological safety. Functionalized magnetic nanoparticles provide a promising strategy for radionuclide recovery and separation. However, designing and synthesizing magnetic adsorbents with high sorption capacity and selectivity, accompanied by excellent stability and reusability, remain a challenge. In this work, novel amidoxime-functionalized flower-like magnetic Fe3O4@TiO2 core-shell microspheres are designed and synthesized to efficiently remove U(VI) from aqueous solutions and actual seawater. The magnetic Fe3O4 core facilitates easy separation by an external magnetic field, and flower-like TiO2 nanosheets provide abundant specific surface areas and functionalization sites. The grafted amidoxime (AO) groups could function as a claw for catching uranium. The maximum adsorption capacity on U(VI) of the designed nanospheres reaches 313.6 mg·g-1 at pH 6.0, and the adsorption efficiency is maintained at 97% after 10 cycles. In addition, the excellent selectivity of the magnetic recyclable AO-functioning Fe3O4@TiO2 microspheres endows the potential of uranium extraction from seawater. A small percentage of data obtained from animal/2D culture models can be translated to humans. Therefore, there is a need to using native tumour microenvironment mimicking models to improve preclinical screening and reduce this attrition rate. For this purpose, currently, the utilization of organoids is expanding. Tumour organoids can recapitulate tumour microenvironment that is including cancer cells and non-neoplastic host components. Indeed, tumour organoids, both phenotypically and genetically, resemble the tumour tissue that originated from it. The unique properties of the tumour microenvironment can significantly affect drug response and cancer progression. In this review, we will discuss about various organoid culture strategies for modelling the tumour immune microenvironment, their applications and advantages in cancer research such as testing cancer immunotherapeutics, developing novel approaches for personalized medicine, testing drug toxicity, drug screening, study cancer initiation and progression, and we will also review the limitations of organoid culture systems.
To determine whether the portable Ceribell
electroencephalograph (EEG) (Mountain View, CA) used for suspected status epilepticus (SE) can reduce time to diagnosis and on-call workforce demands and whether it can be applied to patients in respiratory isolation.
A multidisciplinary team developed a protocol for the use of the Ceribell EEG. The staff deploying the device, the attending physician, and the interpreting neurologist completed evaluation tools for each patient. Data maintained for quality and resource planning of 18-channel electroencephalography ordered for suspected SE were used as controls. Times to diagnosis were compared by application of Welch-Satterthwaite tests and workforce call-in demands by Fisher's exact t test. We evaluated qualitative data related to the use of the EEG in COVID-19 isolation rooms and on its technical aspects and acceptance by staff members.
The Ceribell EEG reduced diagnosis time (P=.0000006) and on-call workforce demand (P=.02). The device can be used at any time of day in any hospital care area and has advantages in respiratory isolation rooms.
Compared with a standard 18-channel EEG, the Ceribell device allowed earlier diagnosis of SE and non-SE conditions and reduced workforce demands. Due to the ease of its use and its simple components, which can be readily disinfected, it is advantageous for COVID-19 patients in isolation.

To describe the development of the Pediatric Epilepsy Outcome-Informatics Project (PEOIP) at Alberta Children's Hospital (ACH), which was created to provide standardized, point-of-care data entry; near-time data analysis; and availability of outcome dashboards as a baseline on which to pursue quality improvement.
Stakeholders involved in the PEOIP met weekly to determine the most important outcomes for patients diagnosed with epilepsy, create a standardized electronic note with defined fields (patient demographics, seizure and syndrome type and frequency and specific outcomes- seizure type and frequency, adverse effects, emergency department visits, hospitalization, and care pathways for clinical decision support. These were embedded in the electronic health record from which the fields were extracted into a data display platform that provided patient- and population-level dashboards updated every 36hours. Provider satisfaction and family experience surveys were performed to assess the impact of the stand for children with epilepsy will be based.

Finally, biomaterials are used in microfabricated devices to replicate the physiological microenvironment in studies using so-called "organ-on-chip," "tissue-on-chip" or "disease-on-chip" models, which can reduce the use of animal models with their inherent high cost and ethical issues, and due to the possible use of human cells can increase the translation of research from lab to clinic. This review gives an overview of recent developments at the interface between microfabrication and biomaterials science, and indicates potential future directions that the field may take.

Our aim was to analyse the feasibility of white blood cell and platelet counts along with their ratios as a prognostic factor in patients who underwent surgery for ovarian mass. We retrospectively studied the patients admitted in the Department of Gynaecology due to adnexal mass. The potential association of the neutrophil-to-lymphocyte-ratio (NLR), neutrophil-to-monocyte (NMR), platelet-to-lymphocyte (PLR), lymphocyte-to-monocyte (LMR), monocyte-to-platelet, and malignancy was evaluated. Patients with malignant tumours were found with significantly higher ratios of NLR ( < 0.001) and PLR ( < 0.001) and lower LMR ratio ( < 0.001) compared to those with benign tumours. Metal nanomaterials have been widely used to generate photoacoustic (PA) signals because of their high optical absorption characteristics. However, the PA conversion efficiency of metal nanomaterials is limited by the single-wavelength absorption at the resonant peak. To mitigate this issue, a three-layer ultrathin film containing a thin PDMS layer sandwiched between two ultrathin chromium films is proposed. This kind of film structure can attain high optical absorbance (>80%) through the visible light range (450-850 nm). The optical absorption characteristics can be easily modulated by varying the thickness of the PDMS layer. Under the same excitation condition, the PA signal generated by this film structure is twice that of an only Cr film and three times that of an only Au film. This film structure is easily fabricated and can operate with lasers having different central wavelengths or even white light sources, leading to its applications in many fields, including photoacoustic communications and audio transducers.We present a holmium-doped all-fiber master oscillator power amplifier (MOPA) system operating at 2108 nm targeting optical frequency conversion applications. The MOPA delivers pulses of 0.52 mJ energy at 10 kHz repetition rate after three amplification stages, with a close to square-shaped temporal profile of 50 ns duration, diffraction-limited beam quality and linear polarization. Challenges with achieving high gain and efficiency in the final amplification stage are discussed and attributed to quenching effects inferred from measurements of non-saturable absorption in the holmium fibers. Using this MOPA, we demonstrate a mid-IR conversion efficiency of 59% by direct pumping of a ZnGeP2 optical parametric oscillator.We investigate the topological skin effect in a ring resonator array which can be mapped into the square root of a Su-Schrieffer-Heeger (SSH) model with non-Hermitian asymmetric coupling. The asymmetric coupling is realized by integrating the same amount of gain and loss into the two half perimeters of linking rings that effectively couple two adjacent site rings. Such a square-root topological insulator inherits the properties from its parent Hamiltonian, which has the same phase transition points and exhibits non-Bloch features as well. We show the band closing points for open chain are different from that of periodic chain as a result of the skin effect. Moreover, the square-root insulator supports multiple topological edge modes as the number of band gaps is doubled compared to the original Hamiltonian. The full-wave simulations agree well with the theoretical analyses based on a tight-binding model. The study provides a promising approach to investigate the skin effect by utilizing ring resonators and may find potential applications in light trapping, lasers, and filters.We experimentally demonstrate an ultra-low-noise pre-amplification using a non-degenerate phase-sensitive amplifier (ND-PSA) with an optically dispersion-unmanaged link. Chromatic dispersion (CD) compensation is required for phase-sensitive amplification after fiber transmission. In the conventional transmitter configuration for ND-PSAs in which phase-conjugated light (idler light) is optically generated, it is necessary to optically compensate for the CD, for example, by using dispersion-compensating fibers. In this work, we propose an ND-PSA scheme using a digitally generated idler and CD pre-equalization by means of digital signal processing. We conduct an unrepeated transmission over a 200-km single-mode fiber with a 10-Gbaud 64QAM signal using the periodically poled LiNbO3-based PSA. The experimental results demonstrate that the proposed ND-PSA scheme provides a low-noise pre-amplification that outperforms the EDFA without optical CD compensation.A novel method to control the parameters of a chiral fiber grating structure is proposed. Mode couplings are controlled in real time during the twisting fabrication process. This chiral grating structure can satisfy the phase-matching condition for generating high-quality orbital angular momentum (OAM) beams, with an order mode of conversion efficiency over 99.9%. Both theoretical analysis and experimental results of this OAM mode conversion have been investigated, with good agreement. The results demonstrate a dual-OAM beam converter with a charge of ±1 for the right- and left-handed CLPGs, respectively. The high-quality OAM beam generated in this twisted single-mode fiber process may find excellent applications in optical communications.Flexible gradually tapered metal waveguides (GTMWs) are fabricated by an inner plating silver film in a polycarbonate (PC) capillary for the transmission and imaging at 0.3 THz. This approach, based on the SHWS image periodicity, is demonstrated using optometric lenses that translate lenslet images outside the projected lenslet boundaries.

In view of the clinical features of rapidly progressing pneumonia despite standard treatment, history of haemoptysis, anaemia, renal impairment and CT scan findings suggestive of pulmonary haemorrhage, pulse intravenous Methylprednisolone was initiated. The patient showed marked clinical and radiological improvement. The vasculitic workup later revealed positive myeloperoxidase antibody and a definitive diagnosis of microscopic polyangiitis was made. She was discharged in a stable condition and advised to follow-up in rheumatology where she continues to follow-up 2 years after her initial diagnosis.This is the first case reported in literature, where treatment was initiated for this rare disease entity within a week of presentation based on highly suggestive clinical, laboratory and radiological evidence without waiting for a confirmatory diagnosis. Delay in this rapidly deteriorating patient could have been fatal and our prompt action was crucial in securing a favourable outcome.Forniceal ruptures after acute pyelonephritis are relatively common. Bilateral spontaneous subcapsular haematoma (SSCH) is an uncommon sequela to pyelonephritis. We report a case of SSCH where the underlying cause of haematoma was a vascular anomaly. A 38-year-old woman with diabetes presented with fever, flank pain and shock following cystoscopy and Double J stenting done elsewhere. CT abdomen showed left subcapsular renal haematoma, managed by pigtail insertion. Postoperative CT showed right renal subcapsular haematoma and left moderate renal subcapsular haematoma. Renal angiogram showed pseudo-aneurysm on the left middle polar artery for which selective embolisation was performed. Physicians and urologists who come across SSCH for which no definite aetiology could be identified should keep in mind the possibility of an underlying vascular anomaly that could precipitate such a condition. The purpose of this report is to highlight the importance of remembering vascular causes in such unexplained cases of SSCH.
We assessed the impact of key population variables (age, gender, income and education) on perceptions of governmental effectiveness in communicating about COVID-19, helping meet needs for food and shelter, providing physical and mental healthcare services, and allocating dedicated resources to vulnerable populations.

More than 60% of all respondents felt their government had communicated adequately during the pandemic. National variances ranged from 83.4% in China down to 37.2% in Brazil, but overall, males and those with a higher income were more likely to rate government communications highly. Almost half (48.8%) of the respondents felt their government had ensured adequate access to physical health services (ranging from 89.3% for Singapore to 27.2% for Poland), with higher ratings reported by younger and higher-income respondents. Ratings of mental health support were lower overall (32.9%, ranging from 74.8% in Cdents to rate their country's response to the pandemic more highly, suggests that government responses must be nuanced and pay greater attention to the needs of less-advantaged citizens as they continue to address this pandemic.

In this ongoing international prospective multicentre observational diagnostic accuracy study, we aim to recruit 350 neonates born <27 weeks PMA and/or birth weight <1000 g and perform echocardiograms in the third week of age and at 32 weeks PMA (early diagnostic assessments, EDA) in addition to the standard diagnostic assessment (SDA) for cPH at 36 weeks PMA. One major goal of the emergency department (ED) is to decide, whether patients need to be hospitalised or can be sent home safely. We aim at providing criteria for these decisions without knowing the SARS-CoV-2 test result in suspected cases.
Tertiary emergency medicine.
All patients were treated at the ED of the Charité during the pandemic peak and underwent SARS-CoV-2 testing. Patients with positive test results were characterised in detail and underwent a 14-day-follow-up.

The primary test population of suspected COVID-19 consisted of n=1255 cases, 45.2% were women (n=567). Of these, n=110 tested positive for SARS-CoV-2 (8.8%). In view of the clinical features of rapidly progressing pneumonia despite standard treatment, history of haemoptysis, anaemia, renal impairment and CT scan findings suggestive of pulmonary haemorrhage, pulse intravenous Methylprednisolone was initiated. The patient showed marked clinical and radiological improvement. The vasculitic workup later revealed positive myeloperoxidase antibody and a definitive diagnosis of microscopic polyangiitis was made. She was discharged in a stable condition and advised to follow-up in rheumatology where she continues to follow-up 2 years after her initial diagnosis.This is the first case reported in literature, where treatment was initiated for this rare disease entity within a week of presentation based on highly suggestive clinical, laboratory and radiological evidence without waiting for a confirmatory diagnosis. Delay in this rapidly deteriorating patient could have been fatal and our prompt action was crucial in securing a favourable outcome.Forniceal ruptures after acute pyelonephritis are relatively common. Bilateral spontaneous subcapsular haematoma (SSCH) is an uncommon sequela to pyelonephritis. We report a case of SSCH where the underlying cause of haematoma was a vascular anomaly. A 38-year-old woman with diabetes presented with fever, flank pain and shock following cystoscopy and Double J stenting done elsewhere. CT abdomen showed left subcapsular renal haematoma, managed by pigtail insertion. Postoperative CT showed right renal subcapsular haematoma and left moderate renal subcapsular haematoma. Renal angiogram showed pseudo-aneurysm on the left middle polar artery for which selective embolisation was performed. Physicians and urologists who come across SSCH for which no definite aetiology could be identified should keep in mind the possibility of an underlying vascular anomaly that could precipitate such a condition. The purpose of this report is to highlight the importance of remembering vascular causes in such unexplained cases of SSCH. We assessed the impact of key population variables (age, gender, income and education) on perceptions of governmental effectiveness in communicating about COVID-19, helping meet needs for food and shelter, providing physical and mental healthcare services, and allocating dedicated resources to vulnerable populations. Epidural analgesia is an efficient method of controlling pain and has a wide spectrum of therapeutic and diagnostic applications. Potential complications may occur in a delayed fashion, can remain undiagnosed, and can be a source of significant morbidity. We present a 37-year-old woman presented with severe spontaneous occipital headaches, diplopia, and dizziness that occurred spontaneously six weeks after giving birth. Her primary method of pain control during labor was epidural analgesia. Her neurologic exam revealed a cranial nerve six palsy with ptosis, and her brain MRI demonstrated a Chiari I malformation which had not been previously diagnosed. CT myelography of the lumbar spine revealed extradural contrast extravasation within the interspinous soft tissue at L1-L2, which was the site of her prior epidural procedure. She underwent epidural blood patch administration, and her cranial nerve palsy resolved along with all of her other symptoms. The development of concurrent Chiari I malformation and cranial nerve palsy after epidural anesthesia is an exceptionally rare occurrence. Neurologic complications after epidural anesthesia are likely under-reported, since patients are often lost to follow-up or have subtle neurologic signs which can easily be missed. This frequently delayed presentation emphasizes the importance of patient education and the necessity of a detailed neurological exam when symptoms occur.Purpose The purpose of this study is to compare the effect of pregabalin in reducing the neuropathic pain in postoperative patients who have undergone single-level microdiscectomy for prolapsed intervertebral lumbar disc. Methods A randomized control clinical trial was conducted from June 2018 to April 2020 in three campuses Dr. Ziauddin University Hospital, Karachi, by two spinal surgeons. This study included 84 patients who underwent either emergency or elective microdiscectomy surgery. Results The pain scores were significantly better in group-A compared to group-B with similar baseline variables. The mean VAS scores of pains in group-A on postoperative day seven on follow-up were compared to VAS pain scores in group-B showing better pain control. Conclusions The use of pregabalin in addition to the routine analgesia has better control of postoperative neuropathic pain in patients with single-level microdiscectomy compared to the patients who are receiving only routine analgesia. There is a very close clinical and radiological resemblance in the presentation of spinal metastasis of lung cancer and Potts's disease. It poses a diagnostic challenge to clinicians particularly in TB endemic areas to arrive at an accurate diagnosis, leading to disease progression and poor outcome. We report a 54-year-old female patient presented with constitutional symptoms of on and off fever and **** pain. Her chest X-ray revealed miliary shadows, and acid-fast bacilli (AFB) sputum smear and TB polymerase chain reaction (PCR) test came negative; radiological diagnosis of tuberculous spondylitis was done on computerized tomography (CT) chest and magnetic resonance imaging (MRI) spine. Subsequent bronchoscopy and bronchoalveolar lavage (BAL) cytology showed malignant cells and CT-guided lung biopsy confirmed lung adenocarcinoma with spinal and brain metastasis. Despite being started on chemo-immunotherapy and radiotherapy her outcome was poor due to advanced metastatic disease. This case highlights the significance of considering metastatic adenocarcinoma of the lung a rare but ominous possibility in the differential diagnosis of miliary shadows on chest imaging. Early bronchoscopy and biopsy must be considered in all patients presenting with miliary pulmonary lesions and spinal lesions to make a correct diagnosis, preventing an unnecessary delay in starting proper treatment and poor outcome. We report a case of perforation of the MD in a 60-year-old man following a blunt abdominal trauma due to a fall from a bike, which was diagnosed promptly and managed successfully by timely operative intervention.Football sign is a rare radiographic finding on abdominal x-ray that suggests massive pneumoperitoneum. Free air outlines the abdominal cavity and falciform ligament which creates the radiolucent oval contour of a football. Football sign is hardly reported in older children or adults. We present the first clear image of football sign caused by gastric perforation in an adult patient. A 57-year-old male with a history of hepatocellular carcinoma was diagnosed with an undrainable liver abscess and partial gastric outlet obstruction. He developed acute onset of severe abdominal pain afterward and abdominal plain film showed a large oval radiolucency over the central part of the abdomen without interruption by intestine, a classical finding of pneumoperitoneum also known as a "football sign". Emergent laparotomy revealed a 0.5 cm perforation hole at the anterior surface of the gastric antrum. Despite timely intervention, the patient died from progression of multiorgan failure. This case describes an alarming radiographic finding that rarely occurs in the adult population.

At the next appointment, inferior alveolar nerve block was done with lidocaine 2% on the right side of the mandible (epinephrine 1/80 000). Notably, for all patients in the second group, the first injection was performed on the right second primary molar with articaine 4% (epinephrine 1/100 000), and inferior alveolar nerve block was also done with lidocaine 2% on the left side of the mandible (epinephrine 1/80 000). MethodsThis study was conducted on 94 children undergoing lumbar puncture. All data were extracted from medical records. Administration of primary treatments and initial diagnosis including febrile convulsion, meningitis, and encephalitis in these patients were based on a physician's opinion. ResultsThe majority of subjects were diagnosed with febrile convulsion. Antibiotic treatment before lumbar puncture was taken by 58 children. After lumbar puncture, 35 children discontinued antibiotic therapy, two patients were switched to another antibiotic treatment and 21 subjects continued antibiotic medication. In addition, 36 children did not take antibiotics. Positive PCR was found in four cases from the encephalitis group. Conclusion After lumbar puncture, antibiotic treatment was continued in 23 cases, whereas administration of antibiotics could be justified only in four cases based on positive PCR. Given that antibiotic treatment of our subjects was initiated prior to lumbar puncture but it was changed in two cases and continued in 21 cases after the medical procedure, conducting lumbar puncture seemed to be not very useful in these patients, assuming that laboratory signs or symptoms could justify an antibiotic treatment.Objectives This study aimed to assess satisfaction and report on perceptions of General Practice (GP) residents during residency in Greece, through an online questionnaire at a national level. The training program was reported as insufficient to prepare residents for their future work as primary health care physicians, with a median rating of 4.09 (95% CI 3.78-4.41). The overall educational gain was regarded as poor compared to residents' expectations, with a median rating of 4.71 (95% CI 4.38-5.07). From free text responses of 83 participants, an organized educational program based on logbook skill acquirement with interaction between coordinator and resident emerged as a priority. Conclusion Understanding general practitioners' perceptions, satisfaction level and expectations may help to design reform initiatives and cover their educational needs during residency in a holistic manner. Experience from local settings may be useful to offer more comprehensive messages. In a rapidly changing health environment, quickly collecting and analyzing emerging data appears to be a practical way for correcting decisions and avoiding previous errors.

After lumbar puncture, 35 children discontinued antibiotic therapy, two patients were switched to another antibiotic treatment and 21 subjects continued antibiotic medication. In addition, 36 children did not take antibiotics. Positive PCR was found in four cases from the encephalitis group. Conclusion After lumbar puncture, antibiotic treatment was continued in 23 cases, whereas administration of antibiotics could be justified only in four cases based on positive PCR. From free text responses of 83 participants, an organized educational program based on logbook skill acquirement with interaction between coordinator and resident emerged as a priority. In this article, we describe the adaptation of attachment-based family therapy (ABFT) in the context of telehealth and COVID-19. ABFT is an empirically supported treatment model designed for adolescents and young adults struggling with depression, anxiety, trauma, and suicide. ABFT is a semi-structured, process-oriented, and trauma-informed family therapy model which presents its own unique challenges and benefits in telehealth environments. We present our adaptations based on years of telehealth clinical experience and address how this model supports the impact of COVID-19 on families.Pluripotency maintenance and lineage differentiation are two major characteristics of human embryonic and induced pluripotent stem cells. Chia seed oil alleviated histopathological alteration in DOX-treated rats. It also significantly inhibited DOX-induced GSH depletion and elevation of MDA. The present study revealed that chia seed oil exerts cardioprotection against doxorubicin-induced cardiotoxicity in female Wistar rats. Our study opens the perspective to clinical studies to precisely consider chia seed oil as a potential chemoprotectant nutraceutical in the combination chemotherapy with doxorubicin to limit its cardiotoxicity.Epilepsy is a common neurological condition in children. We interviewed HPV vaccine eligible adolescent and young adult (AYA) cancer survivors (18-26 years) and caregivers of HPV vaccine eligible childhood cancer survivors approaching the AYA age range (9-17 years) about their HPV vaccine experiences and preferences as informed by the HPV Vaccination Roundtable Best Practices. Interviews (N = 20) were recorded, transcribed, and analyzed using interpretive description. Of AYA survivors (n = 10), 50% had received at least one HPV vaccine dose. Of caregivers (n = 10), 30% reported their child had received at least one HPV vaccine dose. Three distinct categories emerged including (1) HPV vaccine knowledge, (2) HPV vaccination recommendation preferences, and (3) HPV vaccination reminder preferences. The first two categories were oncology focused. Participants lacked knowledge regarding HPV vaccine safety and applicability due to their complex cancer history. Most participants strongly preferred that their oncologist provide an HPV vaccine recommendation. The third category was mixed, with some participants preferring oncology-based HPV vaccine reminders while others preferred primary care-based reminders. Almost all participants preferred digital reminders (e.g., text messages). Our results suggest that oncologists play an essential role in recommending the HPV vaccine and providing information to assist survivors and caregivers with vaccine decision-making. Additionally, HPV vaccination uptake among pediatric and childhood and AYA survivors could be improved through the use of tailored, electronically delivered vaccine reminders.

ALIF is commonly advocated to improve lordosis and fusion in adult deformity surgery. Improved fractional curve correction may help level the pelvis and minimize proximal malalignment.
Patients undergoing thoracolumbar fusion to the pelvis with S2AI screws for deformity were identified and stratified into patients who had an ALIF as part of their deformity correction procedure (ALIF + PSF), and those who had a posterior approach alone. The posterior approach (PSF) includes patients who had a posterolateral fusion with or without a transforaminal lumbar interbody fusion (TLIF). Radiographic parameters measured included pre-op and post-op fractional coronal curve Cobb angle, lumbar lordosis, pelvic tilt, pelvic incidence and sacral slope, major Cobb angle, coronal and sagittal SVA.

In this paper, both sexes of a new species of the genus are described, Liosphex amazonensis Bulbol & Somavilla sp. n. Three other species, Liosphex achuar Lohrmann, 2010, Liosphex atratus Lohrmann, 2010, and Liosphex bribri Lohrmann, 2010, are recorded for the first time in Brazil.
