  • In this study, a novel functional nanocomposite was synthesized, characterized and selectively used in pH-controlled separation, pre-concentration and speciation analysis of Cu(I) and Cu(II) from sample matrices where extraction is assisted, facilitated and greatly enhanced by ultrasound energy. The hydrophilic composite material functionalized with tris(2-hydroxymethyl)aminomethane (Tris) and Fe3O4 NPs was characterized in detail by ATR-FT-IR, 1H NMR, XRD, EDX peaks and SEM images. A good linearity was obtained in the range of 2-140 and 5-150 μg L-1 for Cu(II) and Cu(I) with r2 ≥ 0.993. The limits of detection (LODs) of 0.66 and 1.60 μg L-1 for each analyte, were obtained from a pre-concentration of 70-fold. After validation, the method was applied to speciation of Cu(I), Cu(II), and total Cu in the pre-treated and diluted beverage samples before and after pre-oxidation of Cu(I) to Cu(II) due to be more sensitive of extraction process to Cu(II) at pH 6.0. The biocompatibility and porosity of the cryogel increased the surface area available for AuNPs loading via amino groups and the population of anti-PSA, immobilized on the AuNPs via chemisorption, could be increased. The CS-GR-IL-Fc cry displayed excellent conductivity, enhancing electron transfer and amplifying the current signal. Differential pulse voltammetry was employed to determine PSA by measuring the reduction in the Fc oxidation peak current in response to the formation of PSA/anti-PSA immunocomplex. To achieve high sensitivity detection of them, the CaF and CaCl molecules were synthesized by combining calcium in calcite and F and Cl in sample. The temporal characteristics of CaF and CaCl molecular emissions were investigated. It shows that molecular emission of CaF and CaCl has a longer lifetime and high spectral intensity than that of their atomic emissions. Such unique feature of molecular emission inspired us to use it for high sensitivity analysis of Cl and F elements in water. The results show that these two elements can be sensitively and accurately detected using LIBS assisted with molecular emission. The limits of detections (LoDs) were 0.38 mg/L and 1.03 mg/L for F and Cl elements, respectively, and the limit of quantitation (LoQ) was 3.404 mg/L to 20.569 mg/L for fluorine elements and 9.986 mg/L to 39.757 mg/L for fluorine. These detection limits can meet the World Health Organization's detection requirements for F and Cl elements in water.

  • Apnoea of prematurity (AOP) is among the most common diagnoses in the neonatal intensive care unit. Caffeine treatment is a preferred treatment choice. However, neonatal caffeine therapy results in significant intersubject variability. This study aimed to determine the effects of plasma caffeine levels based on standard dose and genetic variability on clinical response to caffeine citrate in Chinese preterm infants.

    This single-center and retrospective study examined data from 112 preterm infants (< 35weeks gestational age) between July 2017 and July 2018. Subjects were divided into apnoea-free (n = 48) and apnoeic (n = 64) groups, and their clinical outcomes were summarized. Liquid chromatography-tandem mass spectrometry was used to measure levels of caffeine and its primary metabolites. Eighty-eight single-nucleotide polymorphisms were chosen for genotyping by a MassARRAY system.

    Preterm infants in the apnoea-free group were associated with a reduction in the incidence of bronchopulmonary dysplasiasure levels based on the standard dosing, were associated with variable responses to caffeine therapy in preterm neonates. Future studies are needed to uncover how these genetic variants affect responses to caffeine therapy in this patient population.
    While several sodium glucose co-transporter 2 (SGLT2) inhibitors are approved as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus (T2DM), there are no clinical trial data providing head-to-head comparisons of the efficacy and safety of these therapies. Real-world analyses can provide valuable evidence on the effectiveness of competing treatments. This study compared the real-world glycemic effectiveness of SGLT2 inhibitors in individuals with T2DM.

    Post-baseline HbA1c was similar in the canagliflozin and empagliflozin cohorts (7.65% versus 7.57%), as was pe with T2DM.Eight new furan derivatives, irpexins A‒H (1‒8), two new polyketides, irpexins I and J (9 and 10), together with nine known compounds were isolated from the fermentation of Irpex lacteus. The structures and absolute configurations were elucidated on the basis of extensive spectroscopic methods and Mosher ester reaction. All compounds shows no cytotoxicity to human MCF-7 and Hela cancer cell lines at the concentration of 10 μM.Colonic interposition is rarely used as an oesophageal replacement after resection, as the preferred use of stomach involves less anastomoses and lower risks of major complications. The functional outcome from the colonic conduit is also unpredictable. This report documents the spectrum of experience of a high-volume oesophageal centre, highlighting indications, techniques and functional outcomes. A retrospective review was undertaken of a prospective database from 2012 to 2016. Four of 252 (1.5%) cases in this time period utilised colon interposition. Two cases were for gastric conduit necrosis following oesophageal cancer resections, one for caustic ingestion with both an oesophago-bronchial fistula and gastric injury, and one for a primary oesophageal malignancy in a patient whom previously had a total gastrectomy. All patients had either a retrosternal or posterior mediastinal isoperistaltic right colon conduit placed. Two of three cancer patients are alive and disease free at 3 and 5 years, respectively. Surviving patients are weight stable and tolerating a normal diet. Both report excellent quality of life using validated assessment tools. Colonic interposition is rarely required in modern oesophageal practice, but with this technique good long-term nutritional and functional outcomes can be obtained. It is required in the armamentarium of a specialist centre, and training given its rarity may require novel approaches such as simulation and cadaveric-based training.Multiple sclerosis (MS) is the most common inflammatory demyelinating disease in the central nervous system. It is one of the major causes of disability in young adults. Early diagnosis and treatment of this disease could decrease later disability and additional costs. In this cross-sectional analytical study, a total of 351 patients were selected from among the multiple sclerosis patients that went to MS clinic or neurologic clinic in 1990-2016. Data were collected and analysed by SPSS v16. This study was conducted on 82.6% females and 17.4% males. Family history of MS was positive in 12.8% of cases. Mean time of onset of symptoms till first medical visit was 3.25 months. Mean time from first medical visit to diagnosis was 14.98 months. Mean time from onset of symptoms till diagnosis was 18.01 months and the mean time from onset of symptoms till initiation of treatment was 18.73 months. Also, 29.3% of cases had delay in first medical visit and 42.2% of cases had delay in diagnosis of MS more than 6 weeks from first medical visit. Overall, delay in first medical visit and diagnosis of MS has decreased over the years. However, there is still delay in diagnosis of MS. Factors associated with delay are low education, male gender, living in rural areas, primary progressive MS, age at MS diagnosis and first clinical symptoms.1-Butanol (1-BD) is a promising fuel additive which can be biosynthesized via reversed β-oxidation pathway in bacteria. However, heterologous reversed β-oxidation pathway is a carbon chain prolongation process with several genes overexpressed in most of bacterial hosts, leading to low titer of 1-BD and high cost for production. Here we displayed a forward β-oxidation pathway for 1-BD production in a kitchen waste oil (KWO) degrading Pseudomonas aeruginosa PA-3, and we proved that aldehyde dehydrogenase (ALDH) is a checkpoint for diverting metabolic flux into 1-BD biosynthesis. https://www.selleckchem.com/products/odq.html With nitrogen source supplied, titer of 1-BD was increased accompanied with 12 ALDH coding genes transcriptionally promoted to different degrees. In this systematic review, we comprehensively analyse the content of 56 publications focused on the potentially deleterious effects of NPs on these non-target organisms. Current research on biosynthesised NPs, characterization, and impact on mosquito vectors and non-target larvivorous organisms is reviewed and critically discussed. Finally, we pinpoint some major challenges that merit future investigation. Plants (87.5%) were mainly used for synthesizing NPs in the studies. NPs were found to be spherical or mainly spherical in shape with a large distribution size. In most of the included studies, NPs showed interesting mosquitocidal activity (LC50 less then 50 ppm).

The standardized screening protocol assessed parental mental health symptoms including depression, anxiety and trauma. Screening began at 14 days post NICU admission and was implemented as part of routine medical care for all caregivers with infants admitted to the NICU at two weeks of age. Screenings were facilitated by pediatric social workers and psychology postdoctoral fellows and included review of critical self-harm items. A total of 158 parents ages 18-42 years (mean = 31.04) were eligible for screening, with 150 completed screenings. Positive screens on any of the three measures resulted in a mental health referral. Approximately 27% of parents had a positive screen that resulted in a mental health referral. The standardized screening protocol was found to be feasible, widely accepted, and effective in establishing referrals for in house mental health services. This model can be used as an example to help other NICUs implement their own universal screening protocols.
    Hypoxemia is the most common barrier to lungs being transplanted from eligible organ donors who are brain dead (BD). Atelectasis is the principal reversible contributing factor to hypoxemia after brain death. We evaluated prospectively whether ventilation in the prone position in donors who are BD would reverse atelectasis, improve oxygenation, and result in more lungs being transplanted.

    Organ donors managed at the recovery center of 1 organ procurement organization over a 2-year period who exhibited hypoxemia (partial pressure of arterial oxygen [PaO

    A search of the literature on MBI in mild cognitive impairment (MCI), cognitively normal (CN), and subjective cognitive impairment (SCI) and CN but at risk (CN-AR) subjects published between 1 January 2003 and 28 September 2020 was conducted. Meta-analysis using a random effects model was performed to determine the pooled estimate of the prevalence of MBI. Meta-regression was performed to identify factors contributing to the variance of prevalence rate. A systematic review was also performed to study the impact of MBI in cognitive outcomes and its correlation to the pathology and genetics of Alzheimer's disease.

    To develop a logistic regression model, artificial neural network (ANN) model and decision tree (DT) model for the progression of mild cognitive impairment (MCI) to Alzheimer's disease (AD) to compare the performance of the three models.

    A total of 425 patients with MCI were screened from the original cohort. The actual follow up included 361 patients, with AD as the outcome variable. Three kinds of prediction models were developed a logistic regression model, ANN model and DT model. The performance of all three models was measured with accuracy, sensitivity, positive predictive value and area under the receiver operating characteristic curve.

    A total of 121 patients with MCI developed AD, and the average conversion rate was 9.49% per year. The ANN model had higher accuracy (89.52 ± 0.36%), area under the receiver operating characteristic curve (92.08 ± 0.12), sensitivity (82.11 ± 0.42%) and positive predictive value (75.26 ± 0.86%) than the other two models. The first five important predictors of the ANN model were, in order, ADL score, age, urine AD-associated neuronal thread protein, alcohol consumption and smoking.

Notwithstanding, several countries continue to depend on unclaimed bodies as the primary source for cadavers, regardless of the cause of death. Besides, body donation is also usually reported to be strained during disease outbreaks thereby putting countries that depend solely on it in a dilemma.     Abdominal paracentesis is performed as a diagnostic test in children with ascites. Serum albumin to ascitic fluid albumin gradient (SAAG) is frequently used in adults to distinguish types of portal hypertension. We aim to investigate the utilization of SAAG and other biomarkers in determining the etiology of significant ascites in children.

    In this retrospective study, children aged 0-21 years with significant ascites were identified using International Classification of Diseases, Ninth Revision (ICD-9) codes and medical records during the period 1983-2010. Medical records of children who had abdominal paracentesis were examined in detail.

    207 children had significant ascites and of those children, 20 (9.6%) had abdominal paracentesis. Our data showed that high albumin gradient (SAAG ≥ 1.1 g/dL) differentiates causes of ascites secondary to portal hypertension (cirrhosis, hepatic vein outflow obstruction, or congestive hepatopathy) from other causes. In addition, ascitic fluid total protein (AFTP) may help in differential diagnosis of ascites.   • Interactions between structured optical fields (SOFs) and meta-atoms have been intensively studied, and stimulated by recent advancements on the generation of SOFs and on the synthesis of exotic meta-atoms. Multipole expansion is an efficient and accurate theoretical framework for studying such problems. In this work, explicit expressions of SOFs and their beam-shape coefficients are provided, and their properties are also briefly discussed; the considered SOFs include Laguerre-Gaussian (LG) beams, tightly-focused LG beams, Bessel beams, and cylindrical vector beams. Using the multipole expansion, selective excitations of multipolar resonances of a sphere is discussed. In addition, angular momentum dichroisms of a chiral sphere and an anisotropically chiral meta-atom are calculated to demonstrate selective excitation of multipoles with the desired order, parity, and orientation using engineered SOFs with angular momentum.We propose an interactive optical 3D-touch user interface (UI) by using a holographic light-field (LF) 3D display and a color detection system of the scattered light from the touched 3D image. In the proposed system, color information embedded in the LF is used to realize the 3D position identification and movement detection of the interaction point in 3D space only with a single RGB camera. We demonstrate the real-time interactive implementation of the interface working at 12 frames per second, which verifies the feasibility of our proposed concept.An adaptive optics optical coherence tomography (AO-OCT) system is used to assess sixty healthy eyes of thirty subjects (age 22 to 75) to evaluate how the outer retinal layers, foveal eccentricity and age effect the mean cone density. The cone mosaics of different retinal planes (the cone inner segment outer segment junction (IS/OS), the cone outer segment combined with the IS/OS (ISOS+), the cone outer segment tips (COST), and the full en-face plane (FEF)) at four main meridians (superior, nasal, inferior, temporal) and para- and perifoveal eccentricities (ecc 2.5° and 6.5°) were analyzed quantitatively. The mean overall cone density was 19,892/mm2 at ecc 2.5° and 13,323/mm2 at ecc 6.5°. A significant impact on cone density was found for eccentricity (up to 6,700/mm2 between ecc 2.5° and 6.5°), meridian (up to 3,700/mm2 between nasal and superior meridian) and layer (up to 1,400/mm2 between FEF and IS/OS). These factors as well as inter-individual variability have to be taken into account when comparing cone density measurements between healthy and pathologically changed eyes, as their combined effect on density can easily exceed several thousand cones per mm2 even in parafoveal regions.A new alternative to calculate the null-screen for highly freeform or complex surfaces for any desired pattern to be observed on the detector is presented. To validate the proposed method, we used the Zernike polynomials to design complex surfaces with sagittas greater or equal to 40 mm, and peak to valley greater or equal to 30 mm, between the used surface and the best fit sphere. The freeform surfaces were fabricated using a 3D printer and a five-axis CNC machine. With the proposed method we can calculate the image that will be observed over the detector for any null-screen and any freeform surface that want to be analyzed. Our all-optical scalar gradiometer performs a differential measurement between two regions of a single vapor cell on a 4 cm baseline.     Interactions between structured optical fields (SOFs) and meta-atoms have been intensively studied, and stimulated by recent advancements on the generation of SOFs and on the synthesis of exotic meta-atoms. Multipole expansion is an efficient and accurate theoretical framework for studying such problems. In this work, explicit expressions of SOFs and their beam-shape coefficients are provided, and their properties are also briefly discussed; the considered SOFs include Laguerre-Gaussian (LG) beams, tightly-focused LG beams, Bessel beams, and cylindrical vector beams. Using the multipole expansion, selective excitations of multipolar resonances of a sphere is discussed. In addition, angular momentum dichroisms of a chiral sphere and an anisotropically chiral meta-atom are calculated to demonstrate selective excitation of multipoles with the desired order, parity, and orientation using engineered SOFs with angular momentum.We propose an interactive optical 3D-touch user interface (UI) by using a holographic light-field (LF) 3D display and a color detection system of the scattered light from the touched 3D image. In the proposed system, color information embedded in the LF is used to realize the 3D position identification and movement detection of the interaction point in 3D space only with a single RGB camera. We demonstrate the real-time interactive implementation of the interface working at 12 frames per second, which verifies the feasibility of our proposed concept.An adaptive optics optical coherence tomography (AO-OCT) system is used to assess sixty healthy eyes of thirty subjects (age 22 to 75) to evaluate how the outer retinal layers, foveal eccentricity and age effect the mean cone density. The cone mosaics of different retinal planes (the cone inner segment outer segment junction (IS/OS), the cone outer segment combined with the IS/OS (ISOS+), the cone outer segment tips (COST), and the full en-face plane (FEF)) at four main meridians (superior, nasal, inferior, temporal) and para- and perifoveal eccentricities (ecc 2.5° and 6.5°) were analyzed quantitatively. The mean overall cone density was 19,892/mm2 at ecc 2.5° and 13,323/mm2 at ecc 6.5°. A significant impact on cone density was found for eccentricity (up to 6,700/mm2 between ecc 2.5° and 6.5°), meridian (up to 3,700/mm2 between nasal and superior meridian) and layer (up to 1,400/mm2 between FEF and IS/OS). Age showed only a weak negative effect. These factors as well as inter-individual variability have to be taken into account when comparing cone density measurements between healthy and pathologically changed eyes, as their combined effect on density can easily exceed several thousand cones per mm2 even in parafoveal regions.A new alternative to calculate the null-screen for highly freeform or complex surfaces for any desired pattern to be observed on the detector is presented. To validate the proposed method, we used the Zernike polynomials to design complex surfaces with sagittas greater or equal to 40 mm, and peak to valley greater or equal to 30 mm, between the used surface and the best fit sphere. The freeform surfaces were fabricated using a 3D printer and a five-axis CNC machine. With the proposed method we can calculate the image that will be observed over the detector for any null-screen and any freeform surface that want to be analyzed. The results showed that the proposal works very well for extremely fast complex freeform surfaces (with slopes ≤ 80°), obtaining an error smaller than 0.66% in PV and 0.36% in rms in sagitta differences.In this work we demonstrate a high sensitivity atomic gradiometer capable of operation in earth-field level environments. The disruption associated with these technologies will most acutely be experienced by the human combatant at the tactical level, with increasing cognitive demands associated with the employment and use of new capabilities. New research priorities may include augmented performance of humans-machine teams, enhanced cognitive and immunological resilience based manoeuvre to prioritise research activities today in line with future needs and requirements.
    This paper outlines key recommendations to assist military human performance researchers to adapt their practice in order to match the increasing pace of military modernisation. By anticipating technological change and forecasting possible emerging technologies the military human performance research community can manoeuvre to prioritise research activities today in line with future needs and requirements.It has been previously shown that magnetite (Fe3O4) nanoparticles stimulate the anaerobic digestion process in several anaerobic reactors. Here we evaluate the effect of magnetite nanoparticles on the efficiency of anaerobic digestion of distillers grains with solubles and sugar beet pulp in mesophilic batch experiments. The addition of magnetite nanopowder had a positive effect on the anaerobic digestion process. CH4 was produced faster in the presence of 50 mg of Fe3O4 per 1 g of added total solids than from treatments without addition of Fe3O4. These results demonstrate that the addition of magnetite enhances the methanogenic decomposition of organic acids. Microbial community structure and dynamics were investigated based on bacterial and archaeal 16S rRNA genes, as well as mcrA genes encoding the methyl-CoM reductase. Depending on the reactor, Bacteroides, midas_1138, Petrimonas, unclassified Rikenellaceae (class Bacteroidia), Ruminiclostridium, Proteiniclasticum, Herbinix, and Intestinibacter (class Clostridia) were the main representatives of the bacterial communities. The archaeal communities in well-performed anaerobic reactors were mainly represented by representatives of the genera Methanosarcina and Methanobacterium. In addition to the detection or verification of deficits, neuropsychology reveals the patient's cognitive strengths and, thus, information about the patient reserve capacities for functional restitution and compensation. Neuropsychology is an integral part of diagnostic evaluations mainly in the context of epilepsy surgery to avoid new or additional damage to preexisting neurocognitive impairments. In addition and increasingly, neuropsychology is being used as a tool for monitoring of the disease and its underlying pathologies, and it is suited for the quality and outcome control of pharmacological or other non-invasive medical intervention. This narrative review summarizes the present state of neuropsychological assessments in epilepsy, reveals diagnostic gaps, and shows the great need for education, homogenization, translation and standardization of instruments.Axillary lymph node metastasis (ALNM) of well- or poorly-differentiated thyroid cancer is rare. Hence, the subclavian region would probably need to be dissected besides axillary dissection. All 21 cases underwent surgical treatment for ALNM. Metastatic lymph nodes in the venous angle, subclavian or axilla, occasionally invade or adhere to the vessels. Hence, a surgical strategy is required in those cases rather than molecular targeted therapy or radioactive iodine irradiation.

    Patients with metastatic disease to the cervical spine have historically had poor outcomes, with an average survival of 15 months. Every effort should be made to avoid complications of surgical intervention for stabilization and decompression.

    We identified patients who had undergone anterior cervical corpectomy and fusion (ACCF) or posterior cervical laminectomy and fusion (PCLF) for metastatic disease of the cervical spine using the American College of Surgeons National Surgical Quality Improvement Program database from 2006 to 2016. Patients meeting the inclusion criteria were subsequently propensity matched 11. We compared the overall complications, intensive care unit level complications, mortality, and return to the operating room between the 2 groups.

    After identifying the patients who met the inclusion criteria and propensity matching, a cohort of 240 patients was included, with 120 (50%) in the ACCF group and 120 (50%) in the PCLF group. The patients in the ACCF group were more likely to have et not apply to patients with metastatic tumors.
    The modified iliac screw fixation technique was proposed to be an interesting alternative option for spinopelvic fixation. In this study, we describe a new minimally invasive technique for percutaneous placement of modified iliac screws.

    A 64-year-old man with ankylosing spondylitis suffered from a 3-column fracture at the L5 vertebra without any neurologic deficit. We performed percutaneous pedicle screw fixation from L2 to S1 with additional modified iliac screws augmentation using an intraoperative navigation. We inserted both S1 pedicle screw and modified iliac screw within a stab incision on each side of the patient's **** without using rod connector.

    The patient's severe low **** pain subsided on the day following the operation. There was no clinical low **** pain at 2-week follow-up. The 1-year follow-up x-ray showed that the fracture was healed successfully without signs of screw loosening or breakage.

    Gamma knife radiosurgery (GKRS) is often performed to treat brain metastases (BrMs). Widely referenced guidelines have suggested post-treatment imaging studies at 3-month intervals. However, clinicians frequently obtain magnetic resonance imaging (MRI) studies at <3 months after GKRS.

    We performed a retrospective medical record review study to assess the utility of early (<3 months) MRI after GKRS in patients with BrMs.

    A total of 415 GKRS procedures were performed. For 325 patients, early MRI studies were obtained. A total of 31 patients had new or worsened neurological symptoms. The early MRI studies showed adverse findings in 25 patients (78%), which in 23 (72%) had resulted in a change in treatment. For 294 patients, no new or worsened neurological symptoms were found on early MRI studies. Of these 294 patients, 86 (29%) had ≥1 adverse finding on MRI, and 60 (20%) had a change in management as a result. However, no rapidly growing tumors or other emergent adverse findings were seen.

    Balloon-assisted coiling (BAC) is an endovascular treatment that may be associated with increased complications and thromboembolic events compared with other coiling techniques. We compared clinical outcomes of endovascular treatment (simple coiling, stent-assisted coiling, and BAC) in patients with unruptured intracranial aneurysms at the internal carotid artery and assessed risk factors that could cause cerebral infarction in patients who underwent BAC.

    We retrospectively reviewed the records of 528 patients with 544 aneurysms who underwent endovascular treatment for unruptured intracranial aneurysms between January 2013 and November 2019. Demographic features, clinical information, balloon inflation time, fetal posterior cerebral artery, anterior cerebral artery (ACA) variants, and angiographic results were analyzed to determine risk factors for cerebral infarction.

    There were no significant differences among the 3 groups in terms of general characteristics. In the ****group, 14 of 39 patients showed a significantly higher incidence of cerebral infarction on diffusion magnetic resonance imaging compared with the stent-assisted coiling (37/238) and simple coiling (21/267) groups (P<0.001). There was no significant difference between the ACA variants (normal vs. hypoplasia or aplasia) and cerebral infarction in the simple coiling and stent-assisted coiling groups, but the proportion of aplasia or hypoplasia in the ****group was significantly higher (P=0.001).

We compared the overall complications, intensive care unit level complications, mortality, and return to the operating room between the 2 groups. After identifying the patients who met the inclusion criteria and propensity matching, a cohort of 240 patients was included, with 120 (50%) in the ACCF group and 120 (50%) in the PCLF group. In this study, we describe a new minimally invasive technique for percutaneous placement of modified iliac screws. A 64-year-old man with ankylosing spondylitis suffered from a 3-column fracture at the L5 vertebra without any neurologic deficit. We performed percutaneous pedicle screw fixation from L2 to S1 with additional modified iliac screws augmentation using an intraoperative navigation. We inserted both S1 pedicle screw and modified iliac screw within a stab incision on each side of the patient's back without using rod connector. The patient's severe low back pain subsided on the day following the operation. There was no clinical low back pain at 2-week follow-up. The 1-year follow-up x-ray showed that the fracture was healed successfully without signs of screw loosening or breakage. The modified iliac screws can be placed safely via percutaneous approach with an intraoperative image guidance. By using this new technique, surgeons can easily adjust the screw head to align with S1 pedicle screw via the same small incision. The rod connectors are not necessary for this technique. This method also prevents the acute angle that can develop between the screw head and shaft of the screw, which is typically found in S2 alar-iliac screw technique. Gamma knife radiosurgery (GKRS) is often performed to treat brain metastases (BrMs). Widely referenced guidelines have suggested post-treatment imaging studies at 3-month intervals. However, clinicians frequently obtain magnetic resonance imaging (MRI) studies at <3 months after GKRS. We performed a retrospective medical record review study to assess the utility of early (<3 months) MRI after GKRS in patients with BrMs. A total of 415 GKRS procedures were performed. For 325 patients, early MRI studies were obtained. A total of 31 patients had new or worsened neurological symptoms. The early MRI studies showed adverse findings in 25 patients (78%), which in 23 (72%) had resulted in a change in treatment. For 294 patients, no new or worsened neurological symptoms were found on early MRI studies. Of these 294 patients, 86 (29%) had ≥1 adverse finding on MRI, and 60 (20%) had a change in management as a result. However, no rapidly growing tumors or other emergent adverse findings were seen. We compared clinical outcomes of endovascular treatment (simple coiling, stent-assisted coiling, and BAC) in patients with unruptured intracranial aneurysms at the internal carotid artery and assessed risk factors that could cause cerebral infarction in patients who underwent BAC. We retrospectively reviewed the records of 528 patients with 544 aneurysms who underwent endovascular treatment for unruptured intracranial aneurysms between January 2013 and November 2019. Demographic features, clinical information, balloon inflation time, fetal posterior cerebral artery, anterior cerebral artery (ACA) variants, and angiographic results were analyzed to determine risk factors for cerebral infarction. There were no significant differences among the 3 groups in terms of general characteristics. In the BAC group, 14 of 39 patients showed a significantly higher incidence of cerebral infarction on diffusion magnetic resonance imaging compared with the stent-assisted coiling (37/238) and simple coiling (21/267) groups (P<0.001). There was no significant difference between the ACA variants (normal vs. hypoplasia or aplasia) and cerebral infarction in the simple coiling and stent-assisted coiling groups, but the proportion of aplasia or hypoplasia in the BAC group was significantly higher (P=0.001). There is a significant association between anatomic ACA variants and cerebral infarction occurrence after BAC. Identifying the variant of the anatomic ACA using digital subtraction angiography would help to predict cerebral infarction after BAC.
