Based on the findings, policy-makers are required to take the necessary steps to overcome pharmacist-related and policy-related barriers associated with the implementation of patient-oriented pharmacy services in Pakistan.Background Ethnic information regarding juvenile idiopathic arthritis (JIA) exists for various populations across the world but is fully lacking for Roma. We assessed the occurrence and clinical characteristics of JIA in Roma vs. non-Roma children. Methods We obtained data on all outpatients (n = 142) from a paediatric rheumatology centre (age 3 to 18 years) in the eastern part of Slovakia (Kosice region). We assessed patients' age, gender, disease type and related extra-articular conditions by ethnicity. We obtained population data from the 2011 census. Results The share of Roma children was higher in the clinical JIA sample than in the overall population (24.6%, n = 35, Roma in the sample vs. 10.8%, n = 142, Roma in the population, p less then 0.05). Moreover, Roma children had been diagnosed more frequently with extra-articular conditions but did not differ in other symptoms. Treatments also did not differ by ethnicity. Conclusion Roma children had been diagnosed more with JIA than their non-Roma peers. This calls for further research on the causes of this increased disease burden in Roma children.In the adult, many embryologic processes can be co-opted by during cancer progression. We use data from 774 WeChat users and apply the Structural Equation Modeling (SEM) approach. We also conduct multigroup analysis with demographic characteristics as moderating variables. The review aimed at providing guidance for using ultrasonic TWTP in polymers and a basic understanding of the welding mechanism, i.e., interdiffusion and molecular motion mechanisms between the phases.The effects of biochar on plant growth vary depending on the applied biochar type, study site environmental conditions, microbial species, and plant-microbial interactions. The objectives of the present study were therefore to assess 1) the response of growth parameters of lupin and root disease incidence to the application of three biochar types in a loamy sandy soil, and 2) the role of endophytic bacteria in biological control of root rot disease incidence in lupin after the amendment of soil with different biochar types.   • Objective This study aimed to evaluate variations in anterior condylar height (ACH) of the distal femur in varus knee osteoarthritis and evaluate the association between ACH and knee flexion. Design Computed tomography (CT) images of 171 knees (143 patients; age 73.7 ± 8.3 years; 132 females, 39 males) with symptomatic primary knee osteoarthritis and varus alignment undergoing primary total knee arthroplasty, unilateral knee arthroplasty, or high tibial osteotomy were evaluated. Several other anatomic parameters were measured on CT or radiography. The ACH and knee flexion correlation was analyzed, and factors contributing to knee flexion were determined using multivariable regression analysis. Results Medial ACH (mean, 8.1 mm; range, -2.8 to 19.9 mm) was smaller (P less then 0.001) but more variable (F = 1.8, P less then 0.001) than lateral ACH (mean, 10.7 mm; range, 3.6-18.3 mm). Medial ACH was moderately correlated with flexion (r = -0.44, 95% confidence interval [CI], -0.55 to -0.32), whereas lateral ACH was weakly correlated (r = -0.38; 95% CI, -0.50 to -0.25). On multivariable linear regression analysis of knee flexion, body mass index (B [partial regression coefficient] = -1.1), patellofemoral Kellgren-Lawrence grade (B = -4.3), medial ACH (B = -1.2), medial posterior condylar offset (B = 1.2), age (B = -0.4), and varus alignment (B = -0.6) remained significant independent variables (adjusted R2 = 0.35). Conclusions Wide variation and anteriorization of the anterior condyle of the distal femur was observed in advanced osteoarthritis, as an independent determinant of limited knee flexion.Presence of blood in colostrum may change the color of breast milk and it is known as "rusty pipe syndrome." The ideal therapeutic intervention for a colonic varix is still unclear. We report a 42 year-old man with 20 years of alcohol use who presented with hematochezia and abdominal distension. The patient was diagnosed with alcoholic liver cirrhosis. The colonoscopy revealed a dilated and tortuous varix in the transverse colon close to the hepatic flexure with oozing blood, a communicating branch and with "red sign", evidence of acute bleeding. Endoscopic band ligation (EBL), the most useful intervention for esophageal varices, was further successfully performed to arrest the bleeding colonic varices. One month after initial treatment, the colonic varices nearly vanished and were replaced by an ulcer. It is extremely rare for colonic varices to be treated with EBL. There is only one similar case in reported literature, but it seems to be safe and effective as an intervention for EBL for acute colonic variceal bleeding. Design This study was designed as a prospective single-arm, open label, multicenter study. Morphological magnetic resonance imaging (MRI) for MOCART assessment and T2 mapping was performed 1 week and 6, 12, 18, and 24 months after GelrinC implantation. Evaluation of T2 mapping was based on the assessment of global T2 indices (T2 of the repair tissue [RT] divided by T2 of healthy reference cartilage) and zonal variation. Results Fifty-six (20 female) patients were prospectively enrolled. The mean MOCART score significantly increased from baseline to the 24-month follow-up with 88.8 (95% CI, 85.8-91.9; P less then 0.001) for all lesions combined as well as 86.8 (95% CI, 83.0-90.6) for chondral lesions and 94.1 (95% CI, 68.55-100) for osteochondral lesions.

Centrins ubiquitously associate with BBs and function in BB assembly, maturation, and stability. Human POC5 (hPOC5) is a highly conserved centrin-binding protein that binds centrins through Sfi1p-like repeats and is required for building full-length, mature centrioles. Here, we use the BB-rich cytoskeleton of Tetrahymena thermophila to characterize Poc5 BB functions. Tetrahymena Poc5 (TtPoc5) uniquely incorporates into assembling BBs and is then removed from mature BBs prior to ciliogenesis. Complete genomic knockout of TtPOC5 leads to a significantly increased production of BBs yet a markedly reduced ciliary density, both of which are rescued by reintroduction of TtPoc5. A second Tetrahymena POC5-like gene, SFR1, is similarly implicated in modulating BB production. When TtPOC5 and SFR1 are co-deleted, cell viability is compromised, and levels of BB overproduction are exacerbated. Overproduced BBs display defective transition zone formation and a diminished capacity for ciliogenesis. This study uncovers a requirement for Poc5 in building mature BBs, providing a possible functional link between hPOC5 mutations and impaired cilia. © 2020. Using preclinical models, we show that RMC-4550, an allosteric inhibitor of SHP2, induces anti-tumor immunity with effects equivalent to or greater than those resulting from checkpoint blockade. In the tumor microenvironment, inhibition of SHP2 modulated T cell infiltrates similar to checkpoint blockade. Additionally, RMC-4550 drove direct, selective depletion of pro-tumorigenic M2 macrophages via attenuation of CSF-1 receptor signaling and increased M1 macrophages via a mechanism independent of CD8+T-cells or IFN-γ. These dramatic shifts in polarized macrophage populations in favor of anti-tumor immunity were not seen with checkpoint blockade. RESULTS Documented use of the Lifestart rose from 10% at the start of the project to 79% in the final month. Not all babies are eligible for DCC. Within this project, 40 (35%) of preterm infants were not eligible to receive DCC. Of those that were eligible, the rate of DCC increased from 17% in the first 3 months to 92% in the last 3 months of the project (p less then 0.0001). RESULTS Documented use of the Lifestart rose from 10% at the start of the project to 79% in the final month. Not all babies are eligible for DCC. Within this project, 40 (35%) of preterm infants were not eligible to receive DCC. Of those that were eligible, the rate of DCC increased from 17% in the first 3 months to 92% in the last 3 months of the project (p less then 0.0001).   • Radioactive Cs ions are extremely harmful to the human body, causing cancers and other diseases. Treatments were performed on radioactive Cs present in wastewater after use in industrial or medical fields. Prussian blue (PB) has been widely used for the removal of Cs ions from water but its colloidal structure hinders reuse, making it problematic for practical use. To solve this problem, we used a commercial macroporous polymer resin as a PB matrix. To provide an efficient anchor for PB, the surface of the polymer resin was decorated with sodium dodecylbenzenesulfonate to produce a negatively charged surface. The successful chemical binding between the polymer resin and PB prevented leakage of the latter during adsorption and crosslinked structure of the matrix provided regeneration of the adsorbent. The adsorbent maintained its removal efficiency after five repeats of the regeneration process. The PB-based, Cs ion-exchange resin showed excellent selectivity toward Cs ions and good reusability, maintaining its high adsorption capacity. As the most typical geological environment, limestone landforms are widespreading in the world and affect the waters that flow around them, which may also change the fate of organic contaminants in these waters. In this study, aquatic environment surrounding limestone was simulated with calcium carbonate, and the photolysis of tetracycline was evaluated under UV irradiation (30 μW/cm2). More tetracycline (up to 98%) was removed in 4 h in the presence of calcium carbonate while only 50% of tetracycline was eliminated in control experiment. The removal of tetracycline was greatly enhanced due to the major roles of alkaline pH and minor roles of Ca2+ and HCO3-/CO32-. In alkaline pH, tetracycline existed as TCs- with higher electronic density in the ring structures, which was more easily attacked by OH. Besides, it could also change the bond orbital energy to facilitate tetracycline absorbing more photon. Moreover, alkaline pH was beneficial to generate more OH and thus promote the indirect photolysis. In addition, alkaline pH also changed the degradation path of tetracycline and rapidly convert tetracycline to the byproducts with m/z 457 via hydroxylation and hydrogen abstraction. This work provides not only better understanding about the fate of tetracycline in aquatic environments but also new insights into the treatment of antibiotic-contaminated water. Although Solanum nigrum L. is a phytoremediator for different metals, its growth and physiology are still compromised by toxic levels of zinc (Zn). Thus, the development of eco-friendly strategies to enhance its tolerance, maintaining remediation potential is of special interest. This study evaluated the potential of 24-epibrassinolide (24-EBL) to boost S. nigrum defence against Zn towards a better growth rate and remediation potential. After 24 days of exposure, the results revealed that Zn-mediated inhibitory effects on biomass and biometry were efficiently mitigated upon application of 24-EBL, without affecting Zn accumulation. The evaluation of oxidative stress markers reported that Zn excess stimulated the accumulation of superoxide anion (O2.-), but reduced hydrogen peroxide (H2O2) levels, while not altering lipid peroxidation (LP). Chlorinated paraffins (CPs) are high production volume chemicals of which some show resistance to environmental degradation, long-rang transport, bioaccumulation and toxicity potential. Information regarding their presence in humans is limited, including their human bioaccumulation potential. The present study aimed to evaluate CP levels in human serum from Australia in order to better understand their exposure and current pollution status as well as trends associated with age and time between 2004 and 2015. For this, we selected a male sub-group of the Australian population under 60 years old (n = 16 pools, total 1600 serum samples). While long-chain CP (C18-20) and most short-chain CP (C10-13, SCCPs) levels were below method detection limits (MDL), medium-chain CPs (C14-17, MCCPs) were found in most serum samples (detection frequency 94%) as well as CPs with a carbon chain length of nine (detection frequency 76%). The levels of ΣSCCPs and ΣMCCPs ranged from less then MDL-140 and less then MDL-520 ng/g lipid weight (lw), respectively, with a median value of 97 ng/g lw for SCCPs and 190 ng/g lw for MCCPs.
    Radioactive Cs ions are extremely harmful to the human body, causing cancers and other diseases. Treatments were performed on radioactive Cs present in wastewater after use in industrial or medical fields. Prussian blue (PB) has been widely used for the removal of Cs ions from water but its colloidal structure hinders reuse, making it problematic for practical use. To solve this problem, we used a commercial macroporous polymer resin as a PB matrix. To provide an efficient anchor for PB, the surface of the polymer resin was decorated with sodium dodecylbenzenesulfonate to produce a negatively charged surface. The successful chemical binding between the polymer resin and PB prevented leakage of the latter during adsorption and crosslinked structure of the matrix provided regeneration of the adsorbent. The adsorbent maintained its removal efficiency after five repeats of the regeneration process. The PB-based, Cs ion-exchange resin showed excellent selectivity toward Cs ions and good reusability, maintaining its high adsorption capacity. As the most typical geological environment, limestone landforms are widespreading in the world and affect the waters that flow around them, which may also change the fate of organic contaminants in these waters. In this study, aquatic environment surrounding limestone was simulated with calcium carbonate, and the photolysis of tetracycline was evaluated under UV irradiation (30 μW/cm2). More tetracycline (up to 98%) was removed in 4 h in the presence of calcium carbonate while only 50% of tetracycline was eliminated in control experiment. The removal of tetracycline was greatly enhanced due to the major roles of alkaline pH and minor roles of Ca2+ and HCO3-/CO32-. In alkaline pH, tetracycline existed as TCs- with higher electronic density in the ring structures, which was more easily attacked by OH. Besides, it could also change the bond orbital energy to facilitate tetracycline absorbing more photon. Moreover, alkaline pH was beneficial to generate more OH and thus promote the indirect photolysis. In addition, alkaline pH also changed the degradation path of tetracycline and rapidly convert tetracycline to the byproducts with m/z 457 via hydroxylation and hydrogen abstraction. This work provides not only better understanding about the fate of tetracycline in aquatic environments but also new insights into the treatment of antibiotic-contaminated water. Although Solanum nigrum L. is a phytoremediator for different metals, its growth and physiology are still compromised by toxic levels of zinc (Zn). Thus, the development of eco-friendly strategies to enhance its tolerance, maintaining remediation potential is of special interest. This study evaluated the potential of 24-epibrassinolide (24-EBL) to boost S. nigrum defence against Zn towards a better growth rate and remediation potential. After 24 days of exposure, the results revealed that Zn-mediated inhibitory effects on biomass and biometry were efficiently mitigated upon application of 24-EBL, without affecting Zn accumulation.   • Respiratory syncytial virus (RSV) is the most common pathogen associated with acute lower respiratory tract infections in young children. RSV is also a major viral pathogen causing severe lung disease in the adult population, particularly among the elderly. We conducted a review of adult RSV studies published from January 1970 to February 2017 to determine the burden of disease among adults worldwide. There were no restrictions on health care setting or definition of RSV infection. A total of 1530 published studies were identified, 95 of which were included in this review. The incidence rates of hospitalised RSV acute respiratory tract infection (ARI) in adults >65 years old ranged from 7.3 to 13.0/105 population in Africa and Asia and from 190 to 254/105 population in the USA. Higher incidence rates (195-1790/105 population) were observed in adults ≥50 years old for outpatient or emergency visits in the USA. Of all ARI patients, RSV accounted for 1-10% in adults and 2-14% in patients with chronic diseases or transplantation. Given the limitations in the existing data, significant efforts should be made to generate evidence on the burden of RSV infections in adults and to estimate the potential impact of future preventive interventions.In June 2017, an outbreak of Salmonella Kottbus infection was suspected in Germany. We investigated the outbreak with whole-genome sequencing (WGS) and a case-control study. Forty-six isolates from 69 cases were subtyped. Three WGS clusters were identified cluster 1 (n = 36), cluster 2 (n = 5) and cluster 3 (n = 3). Compared to controls, cluster 1 cases more frequently consumed raw smoked ham (odds ratio (OR) 10, 95% confidence interval (CI) 1.2-88) bought at supermarket chain X (OR 36, 95% CI 4-356; 9/10 consumed ham Y). All four cluster 2 cases interviewed had consumed quail eggs. Timely WGS was invaluable in distinguishing concurrent outbreaks of a rare Salmonella serotype.Hepatitis B vaccination is recommended for men who have sex with men (MSM) in many countries, but information on vaccine coverage is scarce. We studied hepatitis B vaccination programmes and coverage among MSM in Europe to guide prevention. From a large (N = 174 209) pan-European MSM survey (EMIS-2010), we used data on self-reported hepatitis B vaccination, age, education, settlement size and disclosure of the same-sex sexual orientation ('outness'). We excluded participants with a history of hepatitis B. In multilevel (participants, countries) logistic regression models, we calculated adjusted odds ratios (aOR) with 95% confidence intervals (95% CI). We analysed data of 163 987 MSM in 38 European countries 38.3% were 'out' to all or almost all, 56.4% reported vaccination against hepatitis B and 65.5% lived in countries with free recommended hepatitis B vaccination for MSM. In the final model the odds for being vaccinated increased with outness ('out to all or almost all' aOR 1.76, 95% CI 1.70-1.83 vs. https://www.selleckchem.com/products/2-hydroxybenzylamine.html 'out to no one') and with living in countries, where hepatitis B vaccination was recommended and free-of-charge for MSM (aOR 2.21, 95% CI 1.47-3.32 vs. 'no or unclear recommendation'). To increase hepatitis B vaccination coverage among MSM, implementation of MSM-specific recommendations and improvement of the societal climate for MSM is needed.Thermodynamic modeling (in the software suite Selector) of the processes controlling the surface and groundwater chemistry in the catchment area of the Malaya Belaya River as part of the water-rock system made it possible to identify the migration forms and ratios of vital, carcinogenic, and toxic elements. It was found that the surface and groundwater chemistry is controlled by the interactions of surface, ground, and fissure water with the nepheline syenites and chibinites of the Khibiny massif, which is supported by the data on the concentrations of heavy isotopes of hydrogen and oxygen (δ18O and δ2H). The natural water chemistry and the element ratios may be causes of some of the diseases common among the area's residents, since these diseases are due to the excessively high or low or out-of-balance concentrations of many biologically important components and ratios thereof.Patterns and correlates of substance use among urban African American young women (ages 18-19, n = 459) were examined. Four patterns were identified no/infrequent alcohol and marijuana use (64.9%); recent alcohol only use (18.2%); recent marijuana only use (7.9%); and recent alcohol and marijuana use (9.0%). Having a recent male sexual partner and a history of sexual coercion were associated with increased odds of marijuana-only and dual use. Greater family support and childhood sexual abuse were associated with increased odds of alcohol-only use. Results suggest that sexual relationships and history of abuse/coercion are important factors in young African American women's substance use.Training with low carbohydrate availability enhances endurance training adaptations but training volume may be compromised. We explored whole-body metabolism and performance with delayed carbohydrate feeding during exercise undertaken following acute sleep-low training. We hypothesised this strategy would not suppress fat oxidation and would maintain exercise performance. The study involved three experimental trials and included 9 men and 1 woman (â©’O2peak = 58.8 ± 5.5 mL kg-1 min-1). Each trial started in the afternoon with an exhaustive cycling protocol. The following morning 1-h of steady-state cycling (SS) was followed by a time trial (TT). Carbohydrates (CHO) were not ingested in recovery from exhaustive exercise or during next day exercise in the Placebo trial (PLA); CHO were not ingested during recovery but were fed (15 g every ∼15-min) from 30-min into SS and continued during the TT in the delayed feeding trial (DELAY); CHO were provided during recovery (1.2 g/kg/h for 7 h) and next day exercise (as in DELAY) in a third condition (CHO). Exercise metabolism was assessed using indirect calorimetry and blood sampling. Fat oxidation rates during SS were similar in PLA (0.83 ± 0.17 g/min) and DELAY (0.78 ± 0.14 g/min) (p > 0.05) and higher than CHO (0.57 ± 0.27 g/min) (p  0.05). Delayed carbohydrate feeding could be a strategy to maintain high-fat oxidation rates typically associated with exercise undertaken after the sleep-low approach to training but the acute performance effects remain inconclusive.

Fat oxidation rates during SS were similar in PLA (0.83 ± 0.17 g/min) and DELAY (0.78 ± 0.14 g/min) (p > 0.05) and higher than CHO (0.57 ± 0.27 g/min) (p  0.05). Delayed carbohydrate feeding could be a strategy to maintain high-fat oxidation rates typically associated with exercise undertaken after the sleep-low approach to training but the acute performance effects remain inconclusive.
    Respiratory syncytial virus (RSV) is the most common pathogen associated with acute lower respiratory tract infections in young children. RSV is also a major viral pathogen causing severe lung disease in the adult population, particularly among the elderly. We conducted a review of adult RSV studies published from January 1970 to February 2017 to determine the burden of disease among adults worldwide. There were no restrictions on health care setting or definition of RSV infection. A total of 1530 published studies were identified, 95 of which were included in this review. The incidence rates of hospitalised RSV acute respiratory tract infection (ARI) in adults >65 years old ranged from 7.3 to 13.0/105 population in Africa and Asia and from 190 to 254/105 population in the USA. Higher incidence rates (195-1790/105 population) were observed in adults ≥50 years old for outpatient or emergency visits in the USA. Of all ARI patients, RSV accounted for 1-10% in adults and 2-14% in patients with chronic diseases or transplantation. Given the limitations in the existing data, significant efforts should be made to generate evidence on the burden of RSV infections in adults and to estimate the potential impact of future preventive interventions.In June 2017, an outbreak of Salmonella Kottbus infection was suspected in Germany. We investigated the outbreak with whole-genome sequencing (WGS) and a case-control study. Forty-six isolates from 69 cases were subtyped. We excluded participants with a history of hepatitis B. In multilevel (participants, countries) logistic regression models, we calculated adjusted odds ratios (aOR) with 95% confidence intervals (95% CI). We analysed data of 163 987 MSM in 38 European countries 38.3% were 'out' to all or almost all, 56.4% reported vaccination against hepatitis B and 65.5% lived in countries with free recommended hepatitis B vaccination for MSM. P. gingivalis invasion in OKF6/Tert cells was assessed before and after exposure to commensal species according to CFU counts. Viability assays show that only S. gordonii and S. intermedius display low toxicity toward OKF6/Tert cells. Both commensals adhere to OKF6/Tert cells at an average ratio of 1 CFU to 10 cells. P. gingivalis invasion into host cells is significantly reduced by 25% or 60% after exposure to S. gordonii or S. intermedius, respectively. The results suggest that these commensal species bind to host cells and diminish P. gingivalis invasion. The instrument provides manifold possibilities such as fitting different parameters for the considered signal and testing the efficiency under different percentages of missing data. The reconstruction accuracy is measured by the mean square error (MSE) between original and reconstructed signal. Computational time is important from the aspect of power requirements, thus enabling the selection of a suitable algorithm. Results The study yielded four discriminant proteins that include three angiogenesis proteins (fibroblast growth factor 2 (FGF2), vascular endothelial growth factor A (VEGFA), vascular endothelial growth factor C (VEGFC)) and one cytokine interleukin 7 (IL-7). A clear elevation of FGF2 and IL-7 concentrations was observed in FD compared to other LD samples. No correlation was observed between these proteins and globotriaosylsphingosine (LysoGb3). A significant correlation exists between IL-7 and residual enzyme activity in a non-classical phenotype. This highlights the orthogonal biological information yielded by these proteins that might help in stratifying Fabry patients. Conclusion This work highlights the potential of using proteomics approaches in exploring FD and enhancing FD diagnosis and therapeutic monitoring performances.

  • Tinnitus is the perception of a self-generated sound and an individual's psychological reaction to it. This article discusses one element of the reaction depression. Epidemiologic studies have noted high comorbidity of tinnitus and depression. Findings from recent brain imaging studies have noted shared neural networks in depression and severe tinnitus. As further evidence of this overlaps, antidepressants, counseling, and psychology-based approaches have been used to treat tinnitus. Multifaceted treatment strategies, using both sound-based therapies (not discussed in this paper) and psychology-based approaches, are a necessary part of the treatment options, with the aim of enhancing self-efficacy in patients with tinnitus. Tinnitus distress results from a weave of physical and psychological processes. Reducing the power of the psychological processes will therefore reduce the degree of suffering. The main psychological therapy in this context is cognitive behavioral therapy (CBT). This seeks to understand and change the influence of thinking processes, including information processing biases, and the behaviors that these motivate, on the experience of tinnitus. The results of systematic reviews and meta-analyses indicate that CBT is the tinnitus management approach for which there is the most robust evidence. In spite of this, it remains difficult to access for people with tinnitus. Despite recent, major steps forward in our understanding of tinnitus pathophysiology and improved research methodology, tinnitus remains a clear unmet clinical need. Here, the authors identify current active clinical and preclinical development programs of tinnitus drug candidates using publications, databases, and company communications. The current drug development programs hold promise for new therapeutic options for tinnitus patients, but further fundamental research is needed to validate additional targets for treating tinnitus. Weight loss is an essential part of treatment for patients with OSA and overweight or obesity. Lifestyle interventions are cornerstones of weight management. However, most patients have difficulty achieving and maintaining clinically significant weight loss with lifestyle interventions alone. Health care providers who treat patients with OSA should be familiar with advanced treatment options for overweight and obesity including antiobesity medications, bariatric surgery, and devices. The future of weight management is a customized, multidisciplinary approach for each patient. Successful palatopharyngoplasty is critical for successful sleep apnea surgery.   • BACKGROUND In recent years, UK health policy makers have responded to a GP shortage by introducing measures to support increased healthcare delivery by practitioners from a wider range of backgrounds. AIM To ascertain the composition of the primary care workforce in England at a time when policy changes affecting deployment of different practitioner types are being introduced. DESIGN AND SETTING This study was a comparative analysis of workforce data reported to NHS Digital by GP practices in England. METHOD Statistics are reported using practice-level data from the NHS Digital June 2019 data extract. Because of the role played by Health Education England (HEE) in training and increasing the skills of a healthcare workforce that meets the needs of each region, the analysis compares average workforce composition across the 13 HEE regions in England RESULTS The workforce participation in terms of full-time equivalent of each staff group across HEE regions demonstrates regional variation. Differences persist when expressed as mean full-time equivalent per thousand patients. Despite policy changes, most workers are employed in long-established primary care roles, with only a small proportion of newer types of practitioner, such as pharmacists, paramedics, physiotherapists, and physician associates. CONCLUSION This study provides analysis of a more detailed and complete primary care workforce dataset than has previously been available in England. This raises problems when these children develop minor illness or require management of other chronic diseases. AIM To investigate the association between GP attendance patterns and hospital urgent and emergency care use. DESIGN AND SETTING Retrospective cohort study using a primary care data source (Clinical Practice Research Datalink) in England. The cohort numbered 19 888. METHOD CYP aged 0-25 years with an LLC were identified using Read codes (primary care) or International Classification of Diseases 10 th Revision (ICD-10) codes (secondary care). Emergency inpatient admissions and accident and emergency (A&E) attendances were separately analysed using multivariable, two-level random intercept negative binomial models with key variables of consistency and regularity of GP attendances. RESULTS Face-to-face GP surgery consultations reduced, from a mean of 7.12 per person year in 2000 to 4.43 in 2015. Those consulting the GP less regularly had 15% (95% confidence interval [CI] = 10% to 20%) more emergency admissions and 5% more A&E visits (95% CI = 1% to 10%) than those with more regular consultations. CYP who had greater consistency of GP seen had 10% (95% CI = 6% to 14%) fewer A&E attendances but no significant difference in emergency inpatient admissions than those with lower consistency. CONCLUSION There is an association between GP attendance patterns and use of urgent secondary care for CYP with LLCs, with less regular GP attendance associated with higher urgent secondary healthcare use. METHOD Adults with first-recorded breathlessness between 1997 and 2010 and no prior diagnostic or prescription record for IHD or a respiratory disease ('exposed' cohort) were matched to individuals with no record of breathlessness ('unexposed' cohort). Analyses were adjusted for sociodemographic and comorbidity characteristics. RESULTS In total, 75 698 patients (the exposed cohort) were followed for a median of 6.1 years, and more than one-third subsequently received a diagnosis of COPD, asthma, or IHD. METHOD Thirteen GPs and 10 pharmacists were sampled from practices enrolled in the 3D trial (a complex intervention for people with multimorbidity). Participants' views on collaborative working were explored with interviews that were audiorecorded, transcribed, and analysed thematically. Saturation of data was achieved with no new insights arising from later interviews. RESULTS GPs from surgeries that employed a pharmacist tended to value their expertise more than GPs who had not worked with one.
