Traditionally, local adaptation has been seen as the outcome of a long evolutionary history, particularly with regard to sexual lineages. By contrast, phenotypic plasticity has been thought to be most important during the initial stages of population establishment and in asexual species. We evaluated the roles of adaptive evolution and phenotypic plasticity in the invasive success of two closely related species of invasive monkeyflowers (Mimulus) in the UK that have contrasting reproductive strategies M. guttatus combines sexual (seeds) and asexual (clonal growth) reproduction while M. × robertsii is entirely asexual.
We compared the clonality (number of stolons), floral and vegetative phenotype, and phenotypic plasticity of native (M. guttatus) and invasive (M. guttatus and M. × robertsii) populations grown in controlled environment chambers under the environmental conditions at each latitudinal extreme of the UK. The goal was to discern the roles of temperature and photoperiod on the expression of phenocity. This indicates that even asexual taxa can rapidly (<200 years) adapt to novel environmental conditions in which alternative strategies may not ensure the persistence of populations.
This study provides experimental evidence of local adaptation in a strictly asexual invasive species with high clonality and phenotypic plasticity. Diagnostic radiology is an example of an activity that has undergone significant change due to such developments, which over the past 40 years have led to a huge increase in the volume of medical imaging data generated. However, these developments have by and large left the human elements of the radiological process (referrer, radiographer and radiologist) intact. Diagnostic radiology has now reached a stage whereby the volume of information generated cannot be fully utilised solely by employing human observers to form clinical opinions, a process that has not changed in over 100 years. In order to address this problem, the potential application of Artificial Intelligence (AI) in the form of Deep Learning (DL) techniques to diagnostic radiology indicates that the next technological development phase may already be underway. The paper outlines the historical development of AI techniques, including Machine Learning and DL Neural Networks and discusses how such developments may affect radiological practice over the coming decades. The ongoing growth in the world market for radiological services is potentially a significant driver for change. The application of AI and DL learning techniques will place quantification of diagnostic outcomes at the heart of performance evaluation and quality standards. The effect this might have on the optimisation process will be discussed and in particular the possible need for automation in order to meet more stringent and standardised performance requirements that might result from these developments. Changes in radiological practices would also impact upon patient protection including the associated scientific support requirements and these are discussed.Self-controlled study designs can be used to assess the association between exposures and acute outcomes while controlling for important confounders. Using routinely collected health data, a self-controlled case series design was used to investigate the association between opioid use and bone fractures in 2008-2017 among adults registered in the United Kingdom Clinical Practice Research Datalink. The relative incidence of fracture was estimated, comparing periods when these adults were exposed and unexposed to opioids, adjusted for time-varying confounders. Of 539,369 people prescribed opioids, 67,622 sustained fractures and were included in this study. The risk of fracture was significantly increased when the patient was exposed to opioids, with an adjusted incidence rate ratio of 3.93 (95% confidence interval (CI) 3.82, 4.04). Fracture risk was greatest in the first week of starting opioid use (adjusted incidence rate ratio 7.81, 95% CI 7.40, 8.25) and declined with increasing duration of use. Restarting opioid use after a gap in exposure significantly increased fracture risk (adjusted incidence rate ratio 5.05, 95% CI 4.83, 5.29) when compared with nonuse. These findings highlight the importance of raising awareness of fractures among patients at opioid initiation and demonstrate the utility of self-controlled methods for pharmacoepidemiologic research.Alteration of the hypothalamic-pituitary-adrenal (HPA) axis hormones has been associated with a range of chronic metabolic and cardiovascular health conditions. This study evaluated whether type of farming (organic versus conventional) or the number of self-reported days of spraying pesticides in the past 8 months was associated with diurnal cortisol levels. Salivary cortisol levels were measured four times a day (waking, 30 min after waking, 6 h after waking and bedtime) longitudinally, 8 months apart during three rounds of data collection. Pesticide using (conventional) and organic farmers were recruited to participate. Pesticide use in the previous 8 months was determined as the number of spray days for each type of pesticide used (herbicide, insecticide, fungicide) from self-reported questionnaires. Estimates of cortisol levels at four time points, the cortisol awakening response (CAR), and the diurnal cortisol slope (DCS) were estimated with a longitudinal mixed model that accounted for the non-linearitye appears to be disrupting the HPA axis and depressing the normal diurnal cortisol rhythm among conventional Thai farmers.Breath-hold diving is a common practice as a part of military dive training. An association between prior lung injury and a propensity for lung barotrauma may have the potential to impact mission readiness for combat divers, Pararescue, Combat Controllers, Army Engineer divers, and various units in Naval Special Warfare and Special Operations. Barotrauma is a common complication of diving, typically occurring at depths greater than 30 m (98.4 ft). X-ray dosimeters are of significance for detecting the levels of ionizing radiation exposure in cells and phantoms; thus, they can further optimize X-ray radiotherapy in the clinic. In this paper, we designed a polyacrylamide-based nanogel sensor that is capable of measuring X-ray doses. The dosimeters were prepared by anchoring an X-ray-responsive probe (aminophenyl fluorescein, APF) to poly(acrylamide-co-N-(3-aminopropyl) methyl acrylamide) nanogels. The premise behind the dose measurement is the transition of APF to fluorescence in the presence of hydroxyl radicals that are caused by the radiolysis of water molecules under X-rays. Therefore, the dose of X-rays can be readily detected by measuring the fluorescence intensity of the resultant nanogel immediately after irradiation using fluorescence spectroscopy principles. Using an RS2000 X-ray biological irradiator, our dosimeters showed good linearity responsivity at X-ray doses ranging from 0 to 15 Gy, with a limit of detection (LOD) of 0.5 Gy. Additionally, the signals showed temperature stability (25-65 °C), durability (5 weeks), and dose-rate (1.177 and 6 Gy/min) and energy independence (160 kVp and 6 MV). As a proof-of-concept, we used our sensors to fluorescently detect X-ray doses in A549 tumor cells and 3D-printed eye phantoms. The results showed that our dosimeters were able to accurately predict doses similar to those used by treatment plan systems.Bisphenol A (BPA) is a well-recognized endocrine disruptor, and considering its adverse effects its use in infant bottles has been banned in many countries. Growing concern on the use of BPA has led to its replacement with its analogues in numerous applications. Present is the first report determining the occurrence of seven bisphenols (BPs BPA, BPAF, BPC, BPE, BPFL, BPS, and BPZ) in Indian infant formula. A reliable and efficient UPLC-MS/MS method for their simultaneous determination was developed and validated in powdered infant formula (n = 68). The limit of quantification of the method was 0.19 ng/g for BPA, BPAF, BPE, BPS and BPZ and 0.78 ng/g for ****and BPFL. The highest concentration was detected for BPA (mean = 5.46 ng/g) followed by BPZ and BPS. BPAF, BPFL, ****and BPE were detected in none of the samples. The estimated daily intake (EDI) of total BPs in infants (0-12 months old infants) was determined to be 54.33-213.36 ng/kg b.w./day. BPA mainly contributed to the total intake (EDI = 92.76 ng/kg b.w./day). The dietary exposure to total BPs evaluated in the present study was approximately 1 order of magnitude lower than the reference value of BPA set by EFSA (4 μg/kg b.w./day) and, thus, may not pose considerable risks to infants.Bacterial multidrug efflux pumps have come to prominence in human and veterinary pathogenesis because they help bacteria protect themselves against the antimicrobials used to overcome their infections. Here we undertake a broad survey of the proteins involved, allied to detailed examples of their evolution, energetics, structures, chemical recognition, and molecular mechanisms, together with the experimental strategies that enable rapid and economical progress in understanding their true physiological roles. Once these roles are established, the knowledge can be harnessed to design more effective drugs, improve existing microbial production of drugs for clinical practice and of feedstocks for commercial exploitation, and even develop more sustainable biological processes that avoid, for example, utilization of petroleum.Biodegradable mulch films have been developed as a suitable alternative to conventional nondegradable polyethylene films. The PBAT degradation rate differed with the soil type, with PBAT mineralization levels of 16, 9, 0.3, and 0.9% in LS, CS, BS, and RS, respectively, after 120 days. Metagenomic analysis showed that the microbial community in LS was more responsive to PBAT than the other three soils. The mechanochemical synthesis of valuable half-sandwich ruthenium complexes resulted in a fast reaction, good yield with simple workup, and the avoidance of harsh reaction conditions and organic solvents. The synthesized half-sandwich ruthenium complexes exhibited high catalytic activity for transfer hydrogenation of ketones using 2-propanol as the hydrogen source and solvent. Density functional theory was carried out to propose a mechanism for the transfer hydrogenation process. The modeling suggests the importance of the labile p-cymene ligand in modulating the reactivity of the catalyst.

The reinforcement effect of silica on non-polar polymer is restricted due to their poor compatibility. In this work, amphipathic lignin modified by quaternization and alkylation was used as a modifier for silica to prepare hydrophobic lignin/SiO2 nanoparticles by in-situ one-pot co-precipitation method. In alkaline solution, hydrophobic lignin and SiO2 (from Na2SiO3) were self-assembled to form nanospheres through electrostatic and hydrophobic interactions. The results showed that the lignin/SiO2 nanoparticles were highly hydrophobic nanospheres with macropores in the surface. When the lignin/SiO2 nanoparticles (10 wt%) were added to reinforce high-density polyethylene (HDPE), the mechanical properties of HDPE were improved with the strength of 24.5 MPa and the elongation of 1096%, which were increased by 10.4% and 14.3% compared with the control HDPE, because of the good compatibility and large bonding area. This work puts forward a new solution for the application of lignin in reinforcement of non-polar polymers.In this work, 2,2,6,6-tetramethylpiperidine-1-oxyl-oxidized cellulose nanocrystals (TOCNs) were loaded into sodium alginate/chitosan multilayer film as nanofillers to investigate the modulation of the surface charge density of TOCNs on the film properties. First, the surface charge density of TOCNs was controlled by adjusting the carboxyl content and morphological size by varying the oxidant dosage. After oxidation, TOCN with higher surface charge density was observed to display a higher crystallinity, a more open internal structure, a better dispersibility and a slightly weaker thermal stability. In addition, a 15-layer film composed of sodium alginate and chitosan, called (SA/CH)15, was constructed by layer-by-layer assembly. Both in situ deposition monitoring and free-standing multilayer film formation indicated that TOCNs relied on strong electrostatic interactions and hydrogen bonding to achieve a compact and uniform interlayer and a thinner thickness of (SA/CH)15, which was more evident at a high surface charge density. The addition of TOCNs also enhanced the mechanical properties, thermal stability, hydrophobicity, and barrier properties of (SA/CH)15. In this work, amphipathic lignin modified by quaternization and alkylation was used as a modifier for silica to prepare hydrophobic lignin/SiO2 nanoparticles by in-situ one-pot co-precipitation method. In alkaline solution, hydrophobic lignin and SiO2 (from Na2SiO3) were self-assembled to form nanospheres through electrostatic and hydrophobic interactions. The results showed that the lignin/SiO2 nanoparticles were highly hydrophobic nanospheres with macropores in the surface. When the lignin/SiO2 nanoparticles (10 wt%) were added to reinforce high-density polyethylene (HDPE), the mechanical properties of HDPE were improved with the strength of 24.5 MPa and the elongation of 1096%, which were increased by 10.4% and 14.3% compared with the control HDPE, because of the good compatibility and large bonding area. In addition, a 15-layer film composed of sodium alginate and chitosan, called (SA/CH)15, was constructed by layer-by-layer assembly. Single-molecule magnets exhibit magnetic bistabililties at the molecular level, making them promising for molecule-based spintronics due to high magnetic densities. The incorporation of SMM behavior and electrical conductivity in one compound is rare because these two physical properties often do not operate in the same temperature range, which further hinders their use in practical applications. Here we present an organic-inorganic molecular hybrid, β″-(BEDO-TTF)3[Co(pdms)2]·(****)(H2O)2 (BO3) (BEDO-TTF = bis(ethylenedioxy)tetrathiafulvalene and H2pdms = 1,2-bis(methanesulfonamido)benzene), which manifests both metallic conduction (electrical conductivity up to 1000 S cm-1 at 12 K under 2.0 gigapascal pressure) and SMM behavior in the temperature range 12-26 K for the first time.Multifunctional composites that couple high-capacity adsorbents with catalytic nanoparticles (NPs) offer a promising route toward the degradation of organophosphorus pollutants or chemical warfare agents (CWAs). We couple mesoporous TiO2 aerogels with plasmonic Cu nanoparticles (Cu/TiO2) and characterize the degradation of the organophosphorus CWA sarin under both dark and illuminated conditions. Cu/TiO2 aerogels combine high dark degradation rates, which are facilitated by hydrolytically active sites at the Cu||TiO2 interface, with photoenhanced degradation courtesy of semiconducting TiO2 and the surface plasmon resonance (SPR) of the Cu nanoparticles. The TiO2 aerogel provides a high surface area for sarin binding (155 m2 g-1), while the addition of Cu NPs increases the abundance of hydrolytically active OH sites. Degradation is accelerated on TiO2 and Cu/TiO2 aerogels with O2. Under broadband illumination, which excites the TiO2 bandgap and the Cu SPR, sarin degradation accelerates, and the products are more fully mineralized compared to those of the dark reaction. With O2 and broadband illumination, oxidation products are observed on the Cu/TiO2 aerogels as the hydrolysis products subsequently oxidize. In contrast, the photodegradation of sarin on TiO2 is limited by its slow initial hydrolysis, which limits the subsequent photooxidation. Accelerated hydrolysis occurs on Cu/TiO2 aerogels under visible illumination (>480 nm) that excites the Cu SPR but not the TiO2 bandgap, confirming that the Cu SPR excitation contributes to the broadband-driven activity. The high hydrolytic activity of the Cu/TiO2 aerogels combined with the photoactivity upon TiO2 bandgap excitation and Cu SPR excitation is a potent combination of hydrolysis and oxidation that enables the substantial chemical degradation of organophorphorus compounds.Investigating catalytic reaction mechanisms could help guide the design of catalysts. The NH3-SCR performance of CeSi2 was promoted by an enhanced Eley-Rideal (E-R) mechanism in which more active acid sites were preserved under the reaction conditions and gaseous NO could directly react with adsorbed NH3. This mechanism-enhanced process was even further promoted on sulfated CeSi2. More controlled experiments avoiding high temperatures and exposing the ribbons only to low pressures of pure oxygen show that also under these more gentle conditions the ribbons are oxidized. From these results, we obtain additional insights into the preferential reaction sites and the nature of the main defects that are caused by oxygen. Mounting evidence has suggested that long noncoding RNAs (lncRNAs) are involved in stress responses to internal and environmental stimuli. However, the underlying molecular interactions remain to be elucidated. In this study, we performed integrative analyses of lncRNA-mRNA coexpression in the transformed human bronchial epithelial BEAS-2B cell line exposed to DMSe-derived SOA. We identified a total of 971 differentially expressed lncRNAs in BEAS-2B cells exposed to SOA derived from O3 and OH oxidation of DMSe. Gene ontology (GO) network analysis of cis-targeted genes showed significant enrichment of DNA damage, apoptosis, and p53-mediated stress response pathways.

A 57-year-old woman presented with a 3-month history of an enlarging thyroid mass causing breathlessness and dysphagia. Cross-sectional imaging showed a thyroid tumour infiltrating the trachea and abutting the oesophagus. She underwent panendoscopy, total thyroidectomy and planned tracheal resection, but due to intraoperative findings, a staged procedure was planned instead. Histological analysis revealed a poorly differentiated squamous cell carcinoma (SCC) which was positive on paired box gene 8 (PAX8) immunostaining, suggesting a diagnosis of primary thyroid SCC. She subsequently underwent total laryngectomy and bilateral neck dissections followed by radiotherapy with concurrent cisplatin chemotherapy. The patient remains disease-free 22 months after treatment. Median overall survival is 10 months for macroscopically completely resected tumours. He had a background of alcohol excess and smoking. The patient was tachycardic and febrile with an elevated white blood cell count and C reactive protein. CT demonstrated extensive upper abdominal free fluid, without free air, with a large cystic lesion arising from the greater curvature of the stomach, and a second smaller cystic lesion arising from the posterior aspect of the gastric fundus. The patient was managed with nasogastric drainage, parenteral nutrition, intravenous antibiotics and proton pump inhibitors, and CT-guided abdominal drainage, with resolution of sepsis, and further outpatient care was transferred to our unit. Follow-up endoscopy demonstrated a diverticulum arising from the posterior aspect of the gastric fundus, with normal mucosa throughout the remaining stomach, while CT showed an additional cystic lesion arising from the greater curvature, with thickening of the adjacent gastric wall consistent with a gastric duplication cyst (GDC). Laparoscopy confirmed a small diverticulum at the fundus, and a large GDC anteriorly with associated omental adhesions consistent with prior perforation-two wedge resections were performed. Histology demonstrated no evidence of malignancy or ectopic mucosa. The patient recovered uneventfully and remained free from recurrent symptoms at 6 weeks postoperatively. GDC is a rare entity, which may be associated with ectopic mucosa, malignant transformation and upper gastrointestinal perforation. No previous report describes the coexistence of a GDC and gastric diverticulum. A 57-year-old woman presented with a 3-month history of an enlarging thyroid mass causing breathlessness and dysphagia. Cross-sectional imaging showed a thyroid tumour infiltrating the trachea and abutting the oesophagus. She underwent panendoscopy, total thyroidectomy and planned tracheal resection, but due to intraoperative findings, a staged procedure was planned instead. Histological analysis revealed a poorly differentiated squamous cell carcinoma (SCC) which was positive on paired box gene 8 (PAX8) immunostaining, suggesting a diagnosis of primary thyroid SCC. She subsequently underwent total laryngectomy and bilateral neck dissections followed by radiotherapy with concurrent cisplatin chemotherapy. The patient remains disease-free 22 months after treatment. Median overall survival is 10 months for macroscopically completely resected tumours. CT demonstrated extensive upper abdominal free fluid, without free air, with a large cystic lesion arising from the greater curvature of the stomach, and a second smaller cystic lesion arising from the posterior aspect of the gastric fundus. The patient was managed with nasogastric drainage, parenteral nutrition, intravenous antibiotics and proton pump inhibitors, and CT-guided abdominal drainage, with resolution of sepsis, and further outpatient care was transferred to our unit. Follow-up endoscopy demonstrated a diverticulum arising from the posterior aspect of the gastric fundus, with normal mucosa throughout the remaining stomach, while CT showed an additional cystic lesion arising from the greater curvature, with thickening of the adjacent gastric wall consistent with a gastric duplication cyst (GDC). Laparoscopy confirmed a small diverticulum at the fundus, and a large GDC anteriorly with associated omental adhesions consistent with prior perforation-two wedge resections were performed. Histology demonstrated no evidence of malignancy or ectopic mucosa. The patient recovered uneventfully and remained free from recurrent symptoms at 6 weeks postoperatively. GDC is a rare entity, which may be associated with ectopic mucosa, malignant transformation and upper gastrointestinal perforation. No previous report describes the coexistence of a GDC and gastric diverticulum. Aspirin use for cardiovascular indications is widespread despite evidence not supporting use in patients without cardiovascular disease (CVD). This study characterises aspirin prescribing among people aged ≥50 years in Ireland for primary and secondary prevention, and factors associated with prescription. We identified participants reporting use of prescribed aspirin, other antiplatelets/anticoagulants, and doctor-diagnosed CVD (MI, angina, stroke, TIA) and other cardiovascular conditions. We examined factors associated with aspirin use for primary and secondary prevention in multivariate regression. For a subset, we also examined 10-year cardiovascular risk (using the Framingham general risk score) as a predictor of aspirin use. Among 6618 participants, the mean age was 66.9 years (SD 9.4) and 55.6% (3679) were female. Prescribed aspirin was reported by 1432 participants (21.6%), and 77.6% of aspirin users had no previous CVD. Adolescent (12-19 years) smoking trajectories were also identified using GBTM. Associations between maternal smoking and offspring smoking trajectories were estimated using multinomial logistic regressions. We used propensity score inverse probability weighting (IPW) to account for the differential distribution of maternal and familial characteristics across exposure groups. We identified four distinct groups for maternal smoking no (66.1%), decreasing (5.6%), increasing (9.5%) and persistent (18.8%) smoking, and three adolescent smoking trajectories abstinent, early-onset (before age 15) and late-onset (after age 15). In IPW-adjusted models, youth with mothers with decreasing, increasing and persistent smoking had higher risk of being early-onset smokers compared with youth with mothers in the non-smoking group. We also found that only youth whose mothers were persistent smokers had an increased risk of late-onset smoking. Regardless of timing, offspring exposure to maternal smoking is associated with increased risk of smoking during adolescence. Participants were categorised as being in a 'less sedentary' or 'sedentary' job, based on occupational activity patterns. Odds of excess weight (BMI ≥ 25) at baseline and of being obese (BMI ≥ 30) at follow-up, by indicators of energy intake and expenditure, were compared in the two occupational groups. In multivariate analyses, high non-work sitting time and saturated fat intake were associated with increased odds of obesity at 3-year follow-up in both occupational groups. In the sedentary job group, high physical activity (in leisure and transport) was associated with a 51% reduction in odds of obesity (OR 0.49, 95%CI 0.25-0.97). In the less-sedentary job group, energy intake and high soft drink consumption were associated with markedly increased odds of obesity (OR 1.67 95%CI 1.07-2.61; OR 2.08 95%CI1.42-3.05, respectively). In this cohort of young Australian women, sedentariness at work did not markedly affect the prevalence of excess weight or obesity. Therefore, it is of great significance to study the transient thermal effect of metal materials and the mechanism of material removal to improve the processing quality. In order to explore the heat conduction model of WJGL processing metal materials, this paper is based on the "element birth and death" technique in the finite element method, and the three-dimensional transient temperature field of four typical metal materials (titanium alloy, stainless steel, aluminum alloy, copper) and material removal model are established. Under this model, the removal mechanism of different metal materials and the influence of different process parameters on the temperature field distribution of the material are studied, and the influence of fixed-position drilling and helix drilling on the microporous morphology is compared. The results show that copper and aluminum alloys can obtain a larger depth-to-diameter ratio and a smaller hole taper. Titanium alloy and stainless steel have better hole roundness, lower hole edge temperature, and smaller thermal deformation. Hole roundness error and hole taper decrease with the increase of laser power. The roundness error of each material is reduced to within 10 μm when the laser power is 10 W, and the average hole taper is 8.73°.Night work has been highlighted by the International Agency for Research on Cancer (IARC) as a likely carcinogenic factor for humans, associated with breast cancer and professions that require continuity of work. For this, a cross-sectional study through an online questionnaire on breast cancer risk variables and working life was designed. The study was conducted in Spain and the sample consisted of 966 nurses, of whom 502 were healthy participants and 56 were breast cancer patients. These two groups were compared in the analyses. In this study, we aimed to investigate the lipid-lowering effects of tanshinone IIA on the regulation of lipogenesis, lipid accumulation, and the underlying mechanisms in hepatic cells. We demonstrated that tanshinone IIA can significantly inhibit the gene expression involved in de novo lipogenesis including FASN, ACC1, and SCD1, in HepG2 and Huh 7 cells. Tanshinone IIA could increase phosphorylation of ACC1 protein in HepG2 cells. We further demonstrated that tanshinone IIA also could suppress the fatty-acid-induced lipogenesis and TG accumulation in HepG2 cells. Furthermore, tanshinone IIA markedly downregulated the mRNA and protein expression of SREBP1, an essential transcription factor regulating lipogenesis in hepatic cells. Moreover, we found that tanshinone IIA attenuated liver X receptor α (LXRα)-mediated lipogenic gene expression and lipid droplet accumulation, but did not change the levels of LXRα mRNA or protein in HepG2 cells. The molecular docking data predicted tanshinone IIA binding to the ligand-binding domain of LXRα, which may result in the attenuation of LXRα-induced transcriptional activation. Our findings support the supposition that tanshinone IIA possesses a lipid-modulating effect that suppresses lipogenesis and attenuates lipid accumulation by modulating the LXRα/SREBP1 pathway in hepatic cells. Total phenolic content (TPC), total flavonoid content (TFC), antioxidant activity (ABTS) and reduction capacity (FRAP) were determined spectrophotometrically, and the phenolic compounds were determined using high-performance liquid chromatography (HPLC). The buckwheat honeys showed the strongest antioxidant activity, most likely because they had the highest concentrations of total phenols, total flavonoids, p-hydroxybenzoic acid, caffeic acid, p-coumaric acid, vanillic acid and chrysin. The principal component analysis (PCA) of the data showed significant relationships between the botanic origin of the honey, the total content of phenolic compounds and flavonoids and the antioxidant activity of the six Polish varietal honeys.

Knowing the impact that short and long-term night work has on the nurses' collective seems a priority, therefore, this study aims to analyse the relationship between night work and the development of breast cancer risk factors in nurses. For this, a cross-sectional study through an online questionnaire on breast cancer risk variables and working life was designed. The study was conducted in Spain and the sample consisted of 966 nurses, of whom 502 were healthy participants and 56 were breast cancer patients. These two groups were compared in the analyses. A descriptive analysis was performed, and the relationship was tested using χ2 independence test and OR calculation. The phylogenomic tree built on 553 core genes from genomes of 20 species in the genus Pelistega and other adjacent genera further confirmed that strains NLN63T and NLN82 form a distinct subline and exhibit specific phylogenetic affinity with P. europaea CCUG 39967T. In digital DNA-DNA hybridization analyses, strain NLN63T showed low estimated DNA reassociation values (21.4-22.6 %) with the type strains of the species in the genus Pelistega. Chronic ectopic pregnancy is a variant of ectopic pregnancy presenting as chronic lower abdominal pain, menstrual irregularity and pelvic mass. Often, chronic ectopic may pose diagnostic conundrum due to unusual presentations. We are presenting an unusual case of chronic ectopic with negative urine pregnancy test, who presented with pain in right hypochondrium. The patient had bilateral adnexal mass with omental deposit on imaging masquerading adnexal malignancy, leading to decision for surgical management. Intraoperatively blood clots were present in supramesocolic compartment along with bilateral adnexal masses. Total abdominal hysterectomy and bilateral salpingo-oophorectomy with clots evacuation was done and postoperatively, patient had complete relief of her symptoms. Final histopathology came out as ectopic gestation. Culminating point is to keep ectopic pregnancy as differential in all reproductive age group women presenting with pain in abdomen regardless of other symptom particularly with pelvic mass.Kimura's disease (KD) is a rare, chronic inflammatory disorder of unknown aetiology, which commonly affects men of the Asian race. Here, we present a case capsule of a 39- year-old man with KD of the left cheek, managed initially by surgery alone. He developed local recurrence after 6 months and was treated with steroids and isotretinoin. Eventually, steroids were discontinued due to toxicity and the lesion progressively increased in size. The patient was successfully treated using intensity-modulated radiotherapy with simultaneous integrated boost as a primary modality with minimal adverse effects. We present an account of the Nigerian COVID-19 response based on co-production of evidence between political decision-makers, health policymakers and academics from Nigerian and foreign institutions, an approach that allowed a multidisciplinary group to collaborate on issues arising in real time. Key aspects of the process were the central role of policymakers in determining priority areas and the coordination of multiple, sometime conflicting inputs from stakeholders to write briefing papers and inform effective national decision making. However, the co-production approach met with some challenges, including limited transparency, bureaucratic obstacles and an overly epidemiological focus on numbers of cases and deaths, arguably to the detriment of addressing social and economic effects of response measures. Larger systemic obstacles included a complex multitiered health system, fragmented decision-making structures and limited funding for implementation. A retrospective cohort study using IQVIA Medical Research Data looking at people newly diagnosed with type 2 diabetes and pre-diabetes through primary care registers in the UK between 1 January 2009 and 31 December 2018.
A cohort of 426 717 people were clinically diagnosed with type 2 diabetes and 418 656 people met the criteria for a diagnosis of pre-diabetes in that time period. The incidence of clinically diagnosed type 2 diabetes per 1000 person years at risk (PYAR) in men decreased from a peak of 5.06 per 1000 PYAR (95% CI 4.97 to 5.15) in 2013 to 3.56 per 1000 PYAR (95% CI 3.46 to 3.66) by 2018. For women, the incidence of clinically diagnosed type 2 diabetes per 1000 PYAR decreased from 4.45 (95% CI 4.37 to 4.54) in 2013 to 2.85 (2.76 to 2.93) in 2018. The incidence rate of pre-diabetes tripled by the end of the same study period in men and women.

A retrospective cohort study using IQVIA Medical Research Data looking at people newly diagnosed with type 2 diabetes and pre-diabetes through primary care registers in the UK between 1 January 2009 and 31 December 2018. A cohort of 426 717 people were clinically diagnosed with type 2 diabetes and 418 656 people met the criteria for a diagnosis of pre-diabetes in that time period. The incidence of clinically diagnosed type 2 diabetes per 1000 person years at risk (PYAR) in men decreased from a peak of 5.06 per 1000 PYAR (95% CI 4.97 to 5.15) in 2013 to 3.56 per 1000 PYAR (95% CI 3.46 to 3.66) by 2018. For women, the incidence of clinically diagnosed type 2 diabetes per 1000 PYAR decreased from 4.45 (95% CI 4.37 to 4.54) in 2013 to 2.85 (2.76 to 2.93) in 2018. The incidence rate of pre-diabetes tripled by the end of the same study period in men and women.
