Non-melanoma skin cancers are the most common malignancies globally. Although non-melanoma skin cancers exhibit low metastatic potential, they can be locally destructive, necessitating complex excisions and reconstructions. Mohs micrographic surgery is the gold-standard treatment for high-risk non-melanoma skin cancers in patients who are appropriate surgical candidates. Despite its efficacy, Mohs micrographic surgery is not readily available in most geographic regions, necessitating that plastic surgeons be well-versed in alternative treatment modalities for non-melanoma skin cancer. Herein, we will discuss the management of non-melanoma skin cancers in settings where Mohs micrographic surgery is not readily available.While the use of free ***** has become routine and is associated with a low complication rate, pedicled ***** remain a solid reconstructive option in various clinical situations. Pedicled ***** provide a reliable vascular supply and involve a simple surgical procedure. Although the procedure is advantageous from the standpoint of a low rate of flap ischemia, thrombosis, and total flap loss, these complications are still occasionally observed due to intraoperative pedicle injury, postoperative torsion, or compression. Here we report on a case of severe venous thrombosis in a pedicled latissimus dorsi (LD) flap used for breast reconstruction. The patient was a 52-year-old woman who underwent mastectomy and immediate breast reconstruction with a LD flap for left breast cancer. Postoperatively, the color of the skin paddle became dark blue over time. Emergent surgical exploration revealed kinking and narrowing of the thoracodorsal vessels and extensive venous thrombi. The kinked pedicles were repaired and selective thrombolytic therapy was performed. A thrombolytic agent was administered through the serratus anterior branch of the thoracodorsal artery in retrograde fashion while the thoracodorsal vessels were clamped just cephalad to the bifurcation. This allowed for draining of the thrombolytic agent and thrombi through the serratus anterior branch of the thoracodorsal vein without flowing into the systemic circulation. To the best of our knowledge, this is the first report of selective thrombolysis using a pedicle branch to treat venous thrombosis in a pedicled flap. If major vascular branches are available in a pedicled flap, selective thrombolytic therapy may be possible without disconnecting the pedicle, as in the present case.Head and neck surgery sometimes causes small defects, and salvage surgery after chemoradiotherapy poses some risk because of damage to the surgical site from the previous treatment. We have developed a novel thyroid gland flap for head and neck surgical reconstruction and here we describe elevating the flap, including arc rotation, size, and suture technique, and our outcomes to date.
Thyroid gland flap reconstruction was performed in 13 cases (11 patients) between July 2009 and May 2020. The clinical importance and adverse effects of the procedure were examined. Thyroid function and blood flow of the flap were assessed, and the status of the flap and irradiated recipient tissue was examined histopathologically.
Median age at surgery was 64.6 years (range 49-77 years). Two of the patients underwent reconstruction with a thyroid gland flap twice. There were 4 cases of primary head and neck cancer resection with neck dissection in which the flap was harvested from the thyroid gland as reinforcement. In 1 case, surgery was performed for cervical esophageal diverticulum. In all cases, the arc was limited to 6 cm and suturing was basic. There were no complications of the surgical procedure, and the postoperative course was uneventful. Contrast-enhanced computed tomography revealed adequate enhancement of the flap. Postoperative thyroid function was normal. The thyroid gland flap was firmly adapted and fused with the irradiated recipient tissue.

A total of 156 patients who underwent transaxillary breast augmentation between November 2019 and May 2020 were retrospectively reviewed. Submuscular pocket dissection using LigaSure was performed in 92 patients and a conventional technique using a monopolar electrocautery dissector was performed in the remaining 64 patients. A bloodless breast pocket was defined as a clear operating field with little or no blood staining at any stage of the procedure. All endoscopic procedures were recorded to determine whether bloodless pockets had been established. The amount of postoperative drainage at 1 day after surgery was also assessed to compare between the LigaSure and conventional groups.
Bloodless breast pockets were successfully established in 83 patients (90.2%) in the LigaSure group and in 38 patients (59.4%) in the conventional group (
< 0.001). Postoperative drainage amount at 1 day following surgery was significantly lower in the LigaSure group than in the conventional group (

Our findings indicate that the LigaSure system is a safe and effective alternative in breast augmentation requiring transaxillary submuscular dissection.

Current surgical techniques for trigger finger release fall short in that they are performed blindly with trauma to, or require incision of, the palmar fascia, which can be a source of significant and long-lasting morbidity. Retrograde endoscopic release of the A1 pulley was performed through a single incision at the proximal digital crease in cadaveric specimens. The fingers were then dissected to assess for completeness of release and inspected for injury to nearby structures. Complete release of the A1 pulley was noted in 16 of 16 fingers. No significant injuries to the A2 pulley and flexor tendon were found, and no injuries to the digital nerves or vasculature occurred. The described technique, as demonstrated in cadaveric specimens, is a feasible alternative approach in the treatment of trigger finger. The technique allows complete visualization of A1 pulley release through a single palmar fascia sparing incision.

In Senegal, moderncontraceptive utilization is low, with few studies focusing on its associated factors. This study examined modern contraceptive use and its associated factors among married women in Senegal.
Data from the 2017 Continuous Demographic and Health Survey (C-DHS) on 11,394 married women was analysed. We examined the associations between the demographic and socioeconomic characteristics of women and their partners and modern contraceptive use using multilevel logistic regression models. Adjusted odds ratios with 95% confidence intervals (CI) were estimated.
The utilization of modern contraceptives among married women was 26.3%. Individual level factors associated with modern contraceptive use were women's age (45-49 years-aOR = 0.44, 0.30-0.63), women's educational level (higher-aOR = 1.88, 1.28-2.76) husband's educational level (higher-aOR = 1.43, 1.ds family planning should be given priority, especially in rural areas and regions with low resources.

To date, limited data are available on metabolic syndrome prevalence among breast cancer survivors in Malaysia. Therefore, this study was conducted to determine the prevalence of metabolic syndrome and abnormal metabolic syndrome components among breast cancer survivors in East Coast of Peninsular Malaysia.
This cross-sectional study included 95 breast cancer survivors (age 53.7 ± 7.6 years) who have completed main cancer treatments for ≥6 months. Cancer survivors were recruited from two main government hospitals in Kelantan and Terengganu using a purposive sampling method.
According to the Harmonized criteria, the metabolic syndrome prevalence was 50.5%. Among those with metabolic syndrome, the most prevalent abnormal metabolic components were triglycerides (91.2%), fasting blood glucose (79.6%) and HDL-c level (78.4%). Except for total cholesterol and LDL-c, all other metabolic syndrome components were significantly different (p < 0.05) between those with and without metabolic syndrome. Significant differences between metabolic syndrome and non-metabolic syndrome groups were found for weight, BMI, waist circumference, body fat percentage and cancer stages (p < 0.05). However, no significant relationship was reported between sociodemographic, clinical parameters and metabolic syndrome among breast cancer survivors in this study.
Metabolic syndrome was highly prevalent among breast cancer survivors. It is recommended for health care professionals to closely monitor and improve the triglycerides, blood glucose and HDL-c level of the breast cancer survivors under their care to control the detrimental effect of metabolic syndrome.

Patients with mental illness report lower quality of life (QoL) compared to the general population. Prior research has found several differences in clinical features and experiences of male and female patients with schizophrenia. Given these differences, it is also important to explore if there are any gender differences in terms of their QoL. This study aimed to investigate differences in QoL between and within each gender among outpatients with schizophrenia in Singapore.
A total of 140 outpatients were recruited through convenience sampling at the Institute of Mental Health, Singapore. QoL was measured using the brief version of World Health Organization Quality of Life (WHOQOL-BREF) which consists of four domains physical health, psychological health, social relationships, and environment. QoL scores of males and females were compared using independent t-tests, and multiple linear regressions were used to examine sociodemographic correlates of QoL in the overall sample and within each gender.

Besides the clinical benefit of crizotinib in ALK-rearranged metastatic non-small cell lung cancer (NSCLC), concerns about its hepatotoxicity have arisen. It is not clear whether this is a drug class side effect or if the use of other selective ALKs inhibitors is safe after this serious adverse event. While evidence from clinical trials is scarce, reports of treatment after crizotinib-induces hepatitis may add to clinical decision.
Herein, we report a case of acute hepatitis induced by crizotinib in a 32-years-old female diagnosed with metastatic NSCLC, harboring the ALK-rearrangement. After 60days of crizotinib therapy, the patient presented with acute hepatitis, diagnosed after investigation of non-specific symptoms, such as nausea and fatigue. Serum aspartate aminotransferase and alanine aminotransferase levels had increased from baseline to 3010IU/L and 9145IU/L, respectively. Total bilirubin increased up to 7.91mg/dL, but she did not develop liver failure. After crizotinib discontinuation, a gradual hepatic function recovery occurred.

Oxygen vacancy-rich CeO2/BiOBr was prepared via solvothermal method combined with rGO to design a Z-scheme heterojunction, which was used for photocatalytic oxidation of gaseous elemental mercury. The Z-scheme heterojunction constructed by interface engineering significantly promotes charge carriers transfer at the interface. Moreover, the surface oxygen vacancies and Ce3+/Ce4+ redox centers tend to capture electrons to accelerate the Z-scheme path of charge transfer to maintain efficient redox performance and facilitate molecular oxygen activation to boost photocatalytic removal of Hg0. The collaboration of oxygen vacancies, Ce3+/Ce4+ and heterojunction enhances the photocatalytic oxidation activity, which achieves a removal efficiency of 76.53%, which is 1.29 times that of BiOBr and 1.91 times that of CeO2. The effect of actual flue gas components (SO2, NO and HCl) on the performance of photocatalytic Hg0 removal was further investigated. Combined with DFT theoretical calculations, the photocatalytic reaction mechanism of Z-scheme heterojunction with oxygen vacancies-rich was proposed. It provides a feasible strategy for the development of high-efficiency Z-scheme heterojunction photocatalytic system for environmental purification.In preparing polymer capsules by vesicle templated emulsion polymerization, the initial size and morphology of the biomimetic vesicle template dictate the final size and morphology of the capsules. The presence of salts (NaCl, NaBr and LiCl) influences the size, dispersity (PDI) and morphology of dimethyldioctadecylammonium bromide or chloride (DODAX, X = Br- or Cl-) vesicles, prepared via membrane extrusion. DODAX vesicles in pure water exhibit broad size distributions with PDI of 0.5 and 0.3 for DODAB and DODAC, respectively. Addition of salts in water before (pre-addition) and after (post-addition) extrusion reduces the size and PDI of the vesicles significantly and results in various morphology investigated with cryo-TEM. It is observed that at low salt concentration (≤0.5 mM) in pre-addition, DODAX exists as a nice quasi spherical unilamellar vesicle, suitable for vesicle templated polymerization whereas in post-addition of salt at any concentration, the morphology is dominated by structures not suitable for templating application. The information obtained here is crucial for vesicle templated emulsion polymerization and it will be shown that there is a relationship between vesicle template morphology and final polymer capsule morphology.Nanometric ions, such as polyoxometalates (POMs) or ionic boron clusters, with low charge density have previously shown a strong propensity to bind to macrocycles and to adsorb to neutral surfaces micellar, surfactant covered water-air and polymer surfaces. Duodenal gastrointestinal stromal tumors (GISTs) are uncommon, making up only 3-5% of all GISTs. [1,2] Historically, the treatment of choice for duodenal GIST tumors was pancreaticoduodenectomy. [3]Currently, newer surgical intervention methods including local resection via laparotomy, endoscopic resection, and robotic resection are feasible. When doing a local resection, the defect can be closed either primarily or via a Roux-en-Y duodenojejunostomy. [3] Case presentation Our patient is a 64-year- old female who presented initially with shortness of breath and was found to have a pulmonary embolism. She then developed upper GI bleeding from anticoagulation and was found to have an ulcerated GIST tumor in the anti-mesenteric border of the third portion of the duodenum (D3). Initial surgery was postponed due to high pulmonary artery pressure from the pulmonary embolism. The patient underwent argon beam coagulation of the bleeding mass to control the bleeding, followed by localized radiotherapy plus Gleevec. Unfortunately, the tumor grew in size during follow-up. The patient was then taken to the OR for a robot-assisted partial duodenal resection (D3) with Roux-en-Y duodenojejunostomy to reconstruct the large defect. She did well post operatively and her final pathology showed a GIST tumor, c-kit and DOG1 positive, 3.5 cm in size, with negative margins.

Oxygen vacancy-rich CeO2/BiOBr was prepared via solvothermal method combined with rGO to design a Z-scheme heterojunction, which was used for photocatalytic oxidation of gaseous elemental mercury. The Z-scheme heterojunction constructed by interface engineering significantly promotes charge carriers transfer at the interface. Moreover, the surface oxygen vacancies and Ce3+/Ce4+ redox centers tend to capture electrons to accelerate the Z-scheme path of charge transfer to maintain efficient redox performance and facilitate molecular oxygen activation to boost photocatalytic removal of Hg0. The collaboration of oxygen vacancies, Ce3+/Ce4+ and heterojunction enhances the photocatalytic oxidation activity, which achieves a removal efficiency of 76.53%, which is 1.29 times that of BiOBr and 1.91 times that of CeO2. The effect of actual flue gas components (SO2, NO and HCl) on the performance of photocatalytic Hg0 removal was further investigated. Combined with DFT theoretical calculations, the photocatalytic reaction mechanism of Z-scheme heterojunction with oxygen vacancies-rich was proposed. It provides a feasible strategy for the development of high-efficiency Z-scheme heterojunction photocatalytic system for environmental purification.In preparing polymer capsules by vesicle templated emulsion polymerization, the initial size and morphology of the biomimetic vesicle template dictate the final size and morphology of the capsules. The presence of salts (NaCl, NaBr and LiCl) influences the size, dispersity (PDI) and morphology of dimethyldioctadecylammonium bromide or chloride (DODAX, X = Br- or Cl-) vesicles, prepared via membrane extrusion. DODAX vesicles in pure water exhibit broad size distributions with PDI of 0.5 and 0.3 for DODAB and DODAC, respectively. Addition of salts in water before (pre-addition) and after (post-addition) extrusion reduces the size and PDI of the vesicles significantly and results in various morphology investigated with cryo-TEM. It is observed that at low salt concentration (≤0.5 mM) in pre-addition, DODAX exists as a nice quasi spherical unilamellar vesicle, suitable for vesicle templated polymerization whereas in post-addition of salt at any concentration, the morphology is dominated by structures not suitable for templating application. Intraoperative ultrasonography is the golden standard method for evaluation of liver tumors during hepatectomy. However, in laparoscopic surgery, accurate assessment of tumors may be difficult, particularly if the lesion is located nearby the liver surface because of the challenges in handling the intraoperative ultrasound and the lack of tactile sensation. In this study, we demonstrate the preliminary results of examining the microvascular architecture of subcapsular hepatic tumors using laparoscopic narrow-band imaging (NBI) to distinguish between malignant and benign tumors.
Thirty-five lesions were examined by NBI during laparoscopic hepatectomy for the presence of abnormal microvasculature on the liver surface in relation to hepatic tumors from January 2016 to August 2018. The microvascular findings were correlated with tumor localization and pathological diagnosis.
The 35 examined nodules included 11 hepatocellular carcinoma, 18 colorectal liver metastasis, and six benign nodules. The mean tumor diameter was 26.3 (3-70) mm, and the distance from the liver surface to the tumor was 0.5 (0-5) mm. Microvessels on the liver surface were clearly visualized by NBI, consistent with malignant tumor localization. The tumors were thoroughly examined for the presence of three pathological microvascular features (enlargement, tortuosity, and heterogeneity). Abnormal microvascular patterns were found in 90.9% of hepatocellular carcinoma and 77.8% of colorectal liver metastasis, whereas neither normal sites nor benign lesions displayed microvascular abnormality.
This study suggests that observing the microvessel image on the liver surface by NBI could be useful in tumor localization and differentiation between benign and malignant lesions.

Monitoring of decrease in fibrinogen levels with surgical blood loss is crucial for timely transfusion of fresh frozen plasma (FFP) to avoid coagulopathic bleeding. Here, we validated a simulation model to predict hemorrhagic reductions in fibrinogen levels during major noncardiac surgery.
We retrospectively performed exponential regression analysis of intraoperative blood loss and fibrinogen levels to develop a simulation model in the initial 50 patients and applied the model to another 59 patients to compare the measured and simulated fibrinogen levels. We examined the relationship between FFP transfusion and the measured fibrinogen levels or blood loss. The fibrinogen trigger level of FFP transfusion was below 130mg/dL, although the decision of a perioperative blood transfusion was at the discretion of the anesthesiologists and surgeons.
Application of the simulation model based on the initial 50 patients to another 59 patients showed no difference between the measured and estimated fibrinogen levels (189±61 versus 186±62, P=0.60, mean difference -2.28, limits of agreement -69.42 to 64.84). The estimated fibrinogen level (mg/dL)=preoperative fibrinogen×exp (-1.90×[blood loss/estimated circulation volume]), in which the estimated circulation volume = (70 [mL/kg]×body weight [kg]). FFP transfusion was significantly related to the measured fibrinogen level (cutoff 145; 95% confidence intervals 124-168; P=0.0003) but not blood loss (P=0.12).

Although there is evidence that self-inflicted abdominal stab wounds are less severe than those from assault, it is unclear if this is true in other anatomic regions. This study compares severity and injury pattern between self-inflicted stab wounds (SISWs) and wounds from assault (ASW).
Stab wounds from our level I trauma registry from 2013 to 2018 were reviewed. Data included age, gender, self-inflicted versus assault, psychiatric or substance use history, anatomic location, operative intervention, injury severity, length of stay, and outcomes.
Over the study period, 1390 patients were identified. History of psychiatric diagnoses or previous suicide attempts was more frequent in SISWs (47% versus 6.5%, P<0.01; 35% versus 0.4%, P<0.01). SISWs had a higher incidence of wounds to the neck and abdomen (44% versus 11%, P<0.01; and 34% versus 26%, P=0.02). Overall, injuries from ASW had a higher injury severity score, but more procedures were performed on SISWs (46% versus 34%, P<0.01). SISWs to the neck were more likely to undergo procedures (26% versus 15%, P=0.04). Median hospital charges were higher in patients with SISWs ($58.6K versus $39.4K, P<0.01).

In vivo adsorption of physiological proteins onto carbon nanotubes, through noncovalent interactions, forms a protein corona or bio corona, able to influence biological properties and biocompatibility of CNT. This study aimed to explore the composition of protein corona formed onto PEGylated Multi-Walled Carbon Nanotubes (MWCNT-PEG5k), after their incubation in human plasma. Plasma proteins were sequentially eluted in different conditions by using both native and denaturant buffers, useful to characterize soft and hard corona. Proteomic methods and mass spectrometry analysis have identified proteins in soft corona, involved in the regulation of immune response and in the CNT transport, and biomolecules in hard corona with a role in the maintenance of host homeostasis. These promising results have demonstrated the potential of PEGylated Multi-Walled Carbon Nanotubes as future candidates for drug delivery.

The purpose of this study was to review literature concerning voice disorders in school-aged children.
Integrative review.
A database search was conducted using PubMed, Web of Science, Academic Search Complete, CINAHL Complete, and Medline. All records included in this review were peer-reviewed journal articles that discussed voice disorders in children, conducted in the United States or Canada, written in English, and published between 2009 and 2019.
Database searching identified 1,771 records and 551 duplicates were removed. https://www.selleckchem.com/products/jsh-150.html A total of 1,220 records were screened and 949 records were excluded. Two hundred and seventy-one full-text records were screened and 12 records met inclusion criteria. Vocal fold nodules were the most commonly reported vocal fold pathology. The Consensus Auditory Perceptual Evaluation of Voice and endoscopy were the most commonly reported assessments. However, variations in practice patterns and access to voice services may exist.
The findings highlight that school-aged children may face barriers in accessing voice services. As a result, continued analyses of the potential barriers that hinder identification and treatment of voice disorders in this population appear warranted.

To investigate the extent to which vocal load is associated with previous diagnosis of a vocal pathology among four major genres of singers (primarily classical, primarily musical theatre (MT), classical and MT combined, and contemporary commercial music only).
Cross sectional survey.
An anonymous online survey was sent out to about 1000 professional singers through convenience sampling to touring companies, opera companies, MT companies, agents, directors and musical directors. Social media and email were used to solicit participation in the study. We utilized means and standard deviations for continuous characteristics and frequencies and percentages for categorical characteristics and calculated P values to assess whether differences were statistically significant.
A total of 396 professional singers completed the survey, yielding a 40% response rate. Nonprofessional singers, incomplete surveys, and respondents <18 years old were excluded, resulting in a total of 238 responses. Among the 238 parrds to water intake. Participants with acid reflux were three times more likely to have ever reported vocal pathology.
Vocal load was not significantly associated with vocal pathology across singing genres; however other nonsinging factors such as allergy, reflux and water intake were significantly associated with vocal pathology.

Point-of-care (POC) C-reactive protein (CRP) testing is employed in European primary care settings to differentiate viral from bacterial respiratory tract infections (RTIs) the latter, requiring referral for antibiotics. This service has yet to be trialled in Australian community pharmacy to support over-the-counter RTI management.
To evaluate the feasibility, based on clinical and operational outcomes, of POC CRP testing to support Western Australian community pharmacists' management of RTIs.
Patients with RTI signs and symptoms were recruited from June-August 2019at 5 community pharmacies. Trained pharmacists made recommendations based on participants' POC CRP levels and routine clinical assessment. Participants completed questionnaires and telephone follow-ups on Days 3 (by pharmacists) and 5 (by researchers) post-testing. Service provision and uptake were assessed in 3 separate weekly tally sheets.
Clinical outcomes CRP levels among the 131 participants recruited were < 5mg/L (bacterial infectice, supported by CRP testing, potentially reduced unnecessary GP visits and antibiotic prescribing.

Reopening schools during the COVID-19 pandemic is a challenging policy issue that requires mitigation strategies, such as mask-wearing and social distancing. Whether youth will conform to these strategies is unclear.
We leverage data on 1,152 youth's mask-wearing and social-distancing behaviors from observations of five in-person live-streamed high school graduations from one U.S. public school district in July 2020. Multiple researchers took ethnographic field notes and systematically recorded public health behaviors for each graduation. We also use data from the local public health department, school district, newspapers, community observations, and the National Center for Education Statistics. We then conducted a descriptive quantitative analysis of mask-wearing status by gender, ethnicity, and school, along with a qualitative thematic analysis.
Nearly 70% of students wore their masks properly while receiving their diploma, although 9.6% wore no mask and 18.7% struggled with mask fit. Almost all students removed masks for graduation photos after adults suggested they should, although 80% of them were wearing their mask properly before the photo. We found significant school variation, but no gender or ethnic variation in student mask-wearing. School variation aligned with political themes of student speeches and in adult commitment to safety measures. Student speakers at all schools mentioned altruistic concern about COVID-19. All schools struggled with social distancing throughout the ceremony, except when students were seated in socially distanced chairs.

The purpose of this study was to review literature concerning voice disorders in school-aged children. Integrative review. A database search was conducted using PubMed, Web of Science, Academic Search Complete, CINAHL Complete, and Medline. All records included in this review were peer-reviewed journal articles that discussed voice disorders in children, conducted in the United States or Canada, written in English, and published between 2009 and 2019. Two hundred and seventy-one full-text records were screened and 12 records met inclusion criteria. Vocal fold nodules were the most commonly reported vocal fold pathology. To investigate the extent to which vocal load is associated with previous diagnosis of a vocal pathology among four major genres of singers (primarily classical, primarily musical theatre (MT), classical and MT combined, and contemporary commercial music only). An anonymous online survey was sent out to about 1000 professional singers through convenience sampling to touring companies, opera companies, MT companies, agents, directors and musical directors. Social media and email were used to solicit participation in the study. We utilized means and standard deviations for continuous characteristics and frequencies and percentages for categorical characteristics and calculated P values to assess whether differences were statistically significant. A total of 396 professional singers completed the survey, yielding a 40% response rate. Non-covalent binding complexes of the resulting MBP-ScFv fusion protein with diverse maltosylated therapeutic cargos (a near-infrared dye, a maltosylated supramolecular β-cyclodextrin container for doxorubicin, and non-viral polyplex gene vector) were easily prepared and characterized. In vitro and in vivo assays using cell lines that express or not the HER2 epitope, and **** xenografts of HER2 expressing cells demonstrated the capability and versatility of MBP-ScFv for diagnosis, imaging, and drug and plasmid active targeted tumor delivery. Remarkably, the modularity of the MBP-ScFv platform allows the flexible interchange of both the cargos and the coding sequence for the ScFv, allowing ad hoc solutions in targeting delivery without any further optimization since the MBP acts as a pivotal element.Microfluidization (50-150 MPa) and thermal treatment (45-85 °C) were applied to modulate the physicochemical stability, molecular interaction and microstructure of zein-proplyene glycol alginate (PGA)-tea saponin (TS) complex nanoparticles for delivery of curcumin. The size of these complex nanoparticles was decreased from 583.1 to 267.4 nm as the microfluidization pressure was increased from 0 to 100 MPa. In the combined treatment of microfluidization and heating, 100 MPa and 75 °C were the optimum parameters to prepare zein-PGA-TS complex nanoparticles for a better protection of curcumin against various environmental stresses. SEM revealed a synergistic effect of microfluidization and heating on the fabrication of complex nanoparticles with a more uniform size and spherical shape. During in vitro gastrointestinal digestion, the complex nanoparticles showed an excellent gastric stability and a sustained release of curcumin in the small intestinal phase. These findings interpreted the effects of microfluidization and thermal treatment on the functional properties of protein-polysaccharide-surfactant complex nanoparticles that can be utilized to develop food grade nanoparticles with enhanced stability and controllable digestion behaviour.Shan Zha has garnered increasing attention in the field of functional foods and medicines due to its widely reported healing effects. However, the potential mechanisms of Shan Zha for human health benefits have not been fully interpreted. Therefore, in the current study, a systems-based method that integrates ADME evaluation, target fishing, gene ontology enrichment analysis, network pharmacology, and pathway analysis is proposed to clarify the underlying pharmacological mechanisms of Shan Zha. As a result, 45 active components of Shan Zha that interacted with 161 protein targets were screened and identified. Moreover, gene ontology enrichment, network and pathway analysis indicated that Shan Zha is beneficial for the treatment of cardiovascular system diseases, digestive system diseases, immune system diseases, inflammatory diseases, cancer, and other diseases through multiple mechanisms. Our study not only proposed an integrated method to comprehensively elucidate the complicated mechanisms of Shan Zha for the treatment of various disorders at the system level, but also provided a reference approach for the mechanistic research of other functional foods.Drinking water consumption results in exposure to complex mixtures of organic chemicals, including natural and anthropogenic chemicals and compounds formed during drinking water treatment such as disinfection by-products. Existing approaches focus primarily on individual chemicals and/or the evaluation of mixtures, without providing information about the chemicals causing the toxic effect. Thus, there is a need for the development of novel strategies to evaluate chemical mixtures and provide insights into the species responsible for the observed toxic effects. This critical review introduces the application of a novel approach called Reactivity-Directed Analysis (RDA) to assess and identify organic electrophiles, the largest group of known environmental toxicants. In contrast to existing in vivo and in vitro approaches, RDA utilizes in chemico methodologies that investigate the reaction of organic electrophiles with nucleophilic biomolecules, including proteins and DNA. The molecular interactions between FNs and Fe(ii) ions, and the digestion and absorption of the FN-Fe(ii) complex through the gastrointestinal system were investigated. The thermodynamic analysis revealed that the FNs spontaneously interacted with Fe(ii) having negative Gibbs free energy change. The data showed that approximately one FN can bind with four Fe(ii) (n = 4.23) through hydroxyl and amino groups. Flow cytometric analysis indicated that the FN-Fe(ii) had no effect on the cell viability at concentration less then 1 mg mL-1. The FN-Fe(ii) complex was stable in the digestive tract with a retention rate of 95.18% ± 3.11% after gastrointestinal digestion. Moreover, 1 μg mm-2 FN-Fe(ii) complex could cross the intestinal wall for Fe(ii) delivery. This research revealed that FNs produced from roasted sturgeon have the potential as biocompatible, efficient and stable nanocarriers for Fe(ii) nutritional delivery.

We recommend reassessment by the IUCN of each species, particularly P. taeniata and P. poncetii, which both show evidence of decline.Temperature-dependent sex determination (TSD) can be observed in multiple reptile and fish species. Here, by manipulating temperature during development, we confirm that cold temperature (16°C) increases the proportion of fish that develop as females in European sea bass (Dicentrarchus labrax) until 56 days posthatching, but show that it has an opposite effect at later stages, with the proportion of males reaching ~90% after 230 days at 16°C. This is the first observation of opposite effects of temperature at different time periods on the sex ratio of a vertebrate. Our results highlight the potential complexity of environmental effects on sex determination.Wetland environment and habitat loss increase the rate of biodiversity decline and affect our ecosystems. Yancheng National Nature Reserve (YNNR) is a protected area dedicated to endangered migratory bird species to overwinter. However, it currently has a record low influx of migrating birds and might therefore be losing its founding purpose. We used remote sensing technology to assess and quantify the impacts and effects of invasive halophytes Spartina alterniflora in the habitat loss and shrinkage of endangered bird wintering habitat from 2003 to 2018. We also attempted to ascertain the causes and triggers of avian population decline and its relationship with habitat loss, as these phenomena threaten and endanger species both locally and globally. Our study shows how YNNR has lost about 80% of migratory bird habitat to invasive S. alterniflora and Phragmites australis, a native halophyte plant in the reserve. Furthermore, shoreline erosion triggered the retraction of S. alterniflora and its backward growth toward Suaeda Salsa, the preferred foraging habitat for migratory birds in the zone, which is a possible cause of their decline.Urban areas are often considered to be a hostile environment for wildlife as they are highly fragmented and frequently disturbed. However, these same habitats can contain abundant resources, while lacking many common competitors and predators. The urban environment can have a direct impact on the species living there but can also have indirect effects on their parasites and pathogens. To date, relatively few studies have measured how fine-scale spatial heterogeneity within urban landscapes can affect parasite transmission and persistence.Here, we surveyed 237 greenspaces across the urban environment of Edinburgh (UK) to investigate how fine-scale variation in socio-economic and ecological variables can affect red fox (Vulpes vulpes) marking behavior, gastrointestinal (GI) parasite prevalence, and parasite community diversity.We found that the presence and abundance of red fox fecal markings were nonuniformly distributed across greenspaces and instead were dependent on the ecological characteristics of a site. Specifically, common foraging areas were left largely unmarked, which indicates that suitable resting and denning sites may be limiting factor in urban environments. In addition, the amount of greenspace around each site was positively correlated with overall GI parasite prevalence, species richness, and diversity, highlighting the importance of greenspace (a commonly used measure of landscape connectivity) in determining the composition of the parasite community in urban areas.Our results suggest that fine-scale variation within urban environments can be important for understanding the ecology of infectious diseases in urban wildlife and could have wider implication for the management of urban carnivores.In the genus Aphaenogaster, workers use tools to transport liquid food to the colony. Here, we investigated the debris dropping and retrieving behavior of the ant Aphaenogaster subterranea to establish which of the two hypotheses is more probable by conducting manipulative experiments. We tested the responses of eight colonies (a) to liquid food (honey-water) and nonfood liquids (water) in different distances from the nest and (b) to nonthreatening liquids previously covered or presented as small droplets. We also tested whether the nutritional condition of colonies (i.e., starved or satiated) would affect the intensity and rate of debris dropping. Our results were consistent with the tool-using behavior hypothesis. Firstly, ants clearly differentiated between honey-water and water, and they directed more of their foraging effort toward liquids farther from the nest. Secondly, ants performed object dropping even into liquids that did not pose the danger of drowning or becoming entangled. Lastly, the nutritional condition of colonies had a significant effect on the intensity and rate of object dropping, but in the opposite direction than we expected. To date, relatively few studies have measured how fine-scale spatial heterogeneity within urban landscapes can affect parasite transmission and persistence.Here, we surveyed 237 greenspaces across the urban environment of Edinburgh (UK) to investigate how fine-scale variation in socio-economic and ecological variables can affect red fox (Vulpes vulpes) marking behavior, gastrointestinal (GI) parasite prevalence, and parasite community diversity.We found that the presence and abundance of red fox fecal markings were nonuniformly distributed across greenspaces and instead were dependent on the ecological characteristics of a site. Specifically, common foraging areas were left largely unmarked, which indicates that suitable resting and denning sites may be limiting factor in urban environments. In addition, the amount of greenspace around each site was positively correlated with overall GI parasite prevalence, species richness, and diversity, highlighting the importance of greenspace (a commonly used measure of landscape connectivity) in determining the composition of the parasite community in urban areas.Our results suggest that fine-scale variation within urban environments can be important for understanding the ecology of infectious diseases in urban wildlife and could have wider implication for the management of urban carnivores.In the genus Aphaenogaster, workers use tools to transport liquid food to the colony. We tested the responses of eight colonies (a) to liquid food (honey-water) and nonfood liquids (water) in different distances from the nest and (b) to nonthreatening liquids previously covered or presented as small droplets. We also tested whether the nutritional condition of colonies (i.e., starved or satiated) would affect the intensity and rate of debris dropping. Our results were consistent with the tool-using behavior hypothesis. Firstly, ants clearly differentiated between honey-water and water, and they directed more of their foraging effort toward liquids farther from the nest. Secondly, ants performed object dropping even into liquids that did not pose the danger of drowning or becoming entangled. Atherosclerotic renal artery stenosis is a risk factor for cardiovascular death. Observational studies support the benefit of renal revascularization on outcomes in selected patients with high-risk clinical manifestations. In this context, we evaluated the factors associated with long-term mortality after renal artery stenting in patients with severe renal artery stenosis, impaired kidney function and/or uncontrolled hypertension.
The medical records of patients undergoing renal artery stenting between 2004 and 2014 were extracted. Blood pressure and creatinine were recorded at baseline, 24 hours post-stenting and in the one month to one year interval that followed revascularization. Long-term follow-up was performed in March 2020.
The cohort consisted of 65 patients. Median follow-up was 120 months. In the first year after stenting, less patients had chronic kidney disease (CKD) class 3b - 5 as compared to baseline (35.3 vs. 56.9%, p=0.01). Thirty-two of these studies reported AUCs for subjective appetite (hunger, fullness, satiety, desire to eat, or prospective consumption) and/or energy intake and were included in the meta-analysis. Pooled estimates from meta-analyses are expressed as standardized mean differences (SMDs). Compared with RG foods, intake of WG foods resulted in significant differences in AUCs for subjective hunger (SMD -0.34; 95% CI -0.46, -0.22; P less then 0.001), fullness (SMD 0.49; 95% CI 0.31, 0.66; P less then 0.001), satiety (SMD 0.33; 95% CI 0.18, 0.47; P less then 0.001), and desire to eat (SMD -0.33; 95% CI -0.46, -0.20; P less then 0.001). There were small, nonsignificant reductions in prospective consumption ratings (P = 0.08) and energy intake (P = 0.07) with WG intake compared with RG. The coronavirus disease 2019 (COVID-19) pandemic raises the concern that other non-COVID conditions will be affected by a decline in care. Therefore, we aimed to investigate the decline in ambulatory presentations for vascular events (stroke, transient ischemic attack [TIA], and myocardial infarction [MI]) during the COVID-19 pandemic.
Patients with a diagnosis of ischemic stroke, TIA, or MI documented anonymously in 1,262 general practices in Germany were included. We studied the differences between 2019 and 2020 (between April and June) in terms of rates and baseline characteristics by comparing monthly absolute frequencies.
A total of 3,496 patients with stroke (mean age 72.2 years), 1,608 patients with TIA (mean age 71.5 years), and 2,385 patients with MI (mean age 66.8 years) were identified between April and June 2020, indicating a decrease of 10% (stroke), 16% (TIA), and 9% (MI) compared to 2019.
