    The aim of this review was to characterise DCEs relating to cancer treatments that were conducted between 1990 and March 2020.

    Studies were included if they implemented a DCE and reported outcomes of interest (i.e. quantitative outputs on participants' preferences for cancer treatments), but were excluded if they were not focused on pharmacological, radioloHRQoL). Despite the emphasis of guidelines on the importance of using qualitative research to inform attribute selection and DCE designs, reporting on instrument development was mixed.

    No formal assessment of bias was conducted, with the scope of the paper instead providing a descriptive characterisation. The review only included DCEs relating to cancer treatments, and no insight is provided into other health technologies such as cancer screening. Only DCEs were included.

    Although there was variation in attribute importance between responder types, survival attributes were consistently ranked as important by both patients and clinicians. Observed challenges included the risk of attribute dominance for survival outcomes, limited sample sizes in some indications, and a lack of reporting about instrument development processes.

    This study aimed at assessing changes in condylar position (CP) in growing patients with unilateral posterior crossbite (UPC) undergoing rapid maxillary expansion (RME) followed by fixed orthodontic treatment (FOT) (experimental-group); and growing patients without posterior crossbite (PC) treated with FOT alone (control-group).

    Cone beam computed tomography (CBCT) images were obtained before treatment (T0), 6months after RME (T1) and after FOT (T2) for the experimental-group (n = 19); and at T0 and T2 for the control-group (n = 22). Condylar position-related measurements including the anterior joint space (AJS), superior joint space (SJS), posterior joint space (PJS), lateral position of condyle (LC) and condylar angle (CA) were measured. Non-parametric tests were used.

    On the crossbite side, significant increases were found in LC (P = 0.039) and CA (P = 0.007), and on the non-crossbite side significant increases were observed in SJS (P = 0.027) and LC (P = 0.001) between T0, T1 and T2 in patients with UPC. On the right and left sides in the control-group, significant increases were identified in LC (P < 0.001 and P = 0.012, respectively) between T0 and T2.

Ilex kudingcha (IK) C.J. Tseng is a nootropic botanical used in Asia for neuroprotection and improvement of cognition. This study establishes that a chemically characterized extract from IK (IKE) mitigates behavioral traits in the Drosophila melanogaster rugose mutant, whose traits resemble human ASD, and examines possible mechanisms. IKE treatment significantly ameliorated deficits in social interaction, short-term memory, and locomotor activity in Drosophila rugose, and significantly increased synaptic bouton number of size more than 2 μm2 in the neuromuscular junctions (NMJs) of Drosophila rugose. To clarify mechanism(s) of IKE action, methylphenidate (MPH), a dopamine transporter inhibitor, was included as a reference drug in the behavioral assays MPH significantly improved social interaction and short-term memory deficit in Drosophila rugose; administration of the dopamine D1 receptor antagonist SCH23390 and dopamine D2 receptor antagonist sulpiride reversed the ameliorative effects of both MPH and IKE on the social interaction deficits of Drosophila rugose. The study was designed to note the sequential changes along with the motor and memory dysfunction associated with biochemical and histo-pathological alterations following exposure and treatment for 2 weeks. The results so obtained showed decrease in the body weights, behavioral deficits with increased stress markers and also neuronal degeneration in histo-pathological examination after manganese intoxication in rats. To overcome the neurotoxic effects of manganese, N-acetylcysteine was used in the current study due to its pleiotropic potential in several pathological ailments.

  • This article investigates the consensus tracking problem of the heterogeneous multivehicle systems (MVSs) under a repeatable control environment. First, a unified iterative learning control (ILC) algorithm is presented for all autonomous vehicles, each of which is governed by both discrete- and continuous-time nonlinear dynamics. Then, several consensus criteria for MVSs with switching topology and external disturbances are established based on our proposed distributed ILC protocols. https://www.selleckchem.com/products/aminoguanidine-hydrochloride.html For discrete-time systems, all vehicles can perfectly track to the common reference trajectory over a specified finite time interval, and the corresponding digraphs may not have spanning trees. We relax this unpractical assumption and propose an extra distributed initial state learning protocol such that vehicles can take different initial states, leading to the fact that the finite time tracking is achieved ultimately regardless of the initial errors. Finally, a numerical example demonstrates the effectiveness of our theoretical results.Scene classification of high spatial resolution (HSR) images can provide data support for many practical applications, such as land planning and utilization, and it has been a crucial research topic in the remote sensing (RS) community. Recently, deep learning methods driven by massive data show the impressive ability of feature learning in the field of HSR scene classification, especially convolutional neural networks (CNNs). Although traditional CNNs achieve good classification results, it is difficult for them to effectively capture potential context relationships. The graphs have powerful capacity to represent the relevance of data, and graph-based deep learning methods can spontaneously learn intrinsic attributes contained in RS images. Inspired by the abovementioned facts, we develop a deep feature aggregation framework driven by graph convolutional network (DFAGCN) for the HSR scene classification. First, the off-the-shelf CNN pretrained on ImageNet is employed to obtain multilayer features. Second, a graph convolutional network-based model is introduced to effectively reveal patch-to-patch correlations of convolutional feature maps, and more refined features can be harvested. Finally, a weighted concatenation method is adopted to integrate multiple features (i.e., multilayer convolutional features and fully connected features) by introducing three weighting coefficients, and then a linear classifier is employed to predict semantic classes of query images. Experimental results performed on the UCM, AID, RSSCN7, and NWPU-RESISC45 data sets demonstrate that the proposed DFAGCN framework obtains more competitive performance than some state-of-the-art methods of scene classification in terms of OAs.The Gaussian-Bernoulli restricted Boltzmann machine (GB-RBM) is a useful generative model that captures meaningful features from the given n-dimensional continuous data. The difficulties associated with learning GB-RBM are reported extensively in earlier studies. They indicate that the training of the GB-RBM using the current standard algorithms, namely contrastive divergence (CD) and persistent contrastive divergence (PCD), needs a carefully chosen small learning rate to avoid divergence which, in turn, results in slow learning. In this work, we alleviate such difficulties by showing that the negative log-likelihood for a GB-RBM can be expressed as a difference of convex functions if we keep the variance of the conditional distribution of visible units (given hidden unit states) and the biases of the visible units, constant. Using this, we propose a stochastic difference of convex (DC) functions programming (S-DCP) algorithm for learning the GB-RBM. We present extensive empirical studies on several benchmark data sets to validate the performance of this S-DCP algorithm. It is seen that S-DCP is better than the CD and PCD algorithms in terms of speed of learning and the quality of the generative model learned.The linear discriminant analysis (LDA) method needs to be transformed into another form to acquire an approximate closed-form solution, which could lead to the error between the approximate solution and the true value. Furthermore, the sensitivity of dimensionality reduction (DR) methods to subspace dimensionality cannot be eliminated. In this article, a new formulation of trace ratio LDA (TRLDA) is proposed, which has an optimal solution of LDA. When solving the projection matrix, the TRLDA method given by us is transformed into a quadratic problem with regard to the Stiefel manifold. In addition, we propose a new trace difference problem named optimal dimensionality linear discriminant analysis (ODLDA) to determine the optimal subspace dimension. The nonmonotonicity of ODLDA guarantees the existence of optimal subspace dimensionality. Both the two approaches have achieved efficient DR on several data sets.The Sit-to-Stand (STS) test is used in clinical practice as an indicator of lower-limb functionality decline, especially for older adults. Due to its high variability, there is no standard approach for categorising the STS movement and recognising its motion pattern. This paper presents a comparative analysis between visual assessments and an automated-software for the categorisation of STS, relying on registrations from a force plate. 5 participants (30 ± 6 years) took part in 2 different sessions of visual inspections on 200 STS movements under self-paced and controlled speed conditions. Assessors were asked to identify three specific STS events from the Ground Reaction Force, simultaneously with the software analysis the start of the trunk movement (Initiation), the beginning of the stable upright stance (Standing) and the sitting movement (Sitting). The absolute agreement between the repeated raters' assessments as well as between the raters' and software's assessment in the first trial, were considered as indexes of human and software performance, respectively. No statistical differences between methods were found for the identification of the Initiation and the Sitting events at self-paced speed and for only the Sitting event at controlled speed. The estimated significant values of maximum discrepancy between visual and automated assessments were 0.200 [0.039; 0.361] s in unconstrained conditions and 0.340 [0.014; 0.666] s for standardised movements. The software assessments displayed an overall good agreement against visual evaluations of the Ground Reaction Force, relying, at the same time, on objective measures.

  • Krüppel-like factor 5 (KLF5) is a member of the KLF family. Recent studies have suggested that KLF5 regulates the expression of a large number of new target genes and participates in diverse cellular functions, such as stemness, proliferation, apoptosis, autophagy, and migration. In response to multiple signaling pathways, various transcriptional modulation and posttranslational modifications affect the expression level and activity of KLF5. Several transgenic mouse models have revealed the physiological and pathological functions of KLF5 in different cancers. Studies of KLF5 will provide prognostic biomarkers, therapeutic targets, and potential drugs for cancers.Electromagnetic fields (EMFs) have emerged as a versatile means for osteoporosis treatment and prevention. However, its optimal application parameters are still elusive. Here, we optimized the frequency parameter first by cell culture screening and then by animal experiment validation. Osteoblasts isolated from newborn rats (ROBs) were exposed 90 min/day to 1.8 mT SEMFs at different frequencies (ranging from 10 to 100 Hz, interval of 10 Hz). SEMFs of 1.8 mT inhibited ROB proliferation at 30, 40, 50, 60 Hz, but increased proliferation at 10, 70, 80 Hz. SEMFs of 10, 50, and 70 Hz promoted ROB osteogenic differentiation and mineralization as shown by alkaline phosphatase (ALP) activity, calcium content, and osteogenesis-related molecule expression analyses, with 50 Hz showing greater effects than 10 and 70 Hz. Treatment of young rats with 1.8 mT SEMFs at 10, 50, or 100 Hz for 2 months significantly increased whole-body bone mineral density (BMD) and femur microarchitecture, with the 50 Hz group showing the greatest effect. Furthermore, 1.8 mT SEMFs extended primary cilia lengths of ROBs and increased protein kinase A (PKA) activation also in a frequency-dependent manner, again with 50 Hz SEMFs showing the greatest effect. Pretreatment of ROBs with the PKA inhibitor KT5720 abolished the effects of SEMFs to increase primary cilia length and promote osteogenic differentiation/mineralization. These results indicate that 1.8 mT SEMFs have a frequency window effect in promoting osteogenic differentiation/mineralization in ROBs and bone formation in growing rats, which involve osteoblast primary cilia length extension and PKA activation.The aim of this clinical report is to describe a complete digital workflow protocol for mandibular full-arch implant rehabilitation from guided surgery to definitive prosthesis in only 3 appointments. This expedited protocol allows for guided implant placement with a system of stackable surgical templates and ***/CAM prosthodontic rehabilitation using a digital workflow. At the first appointment, a guided implant placement protocol with the stackable template concept was done followed by immediate loading with a prefabricated interim prosthesis. At the same appointment, the Double Digital Scanning (DDS) technique was used for scanning, maxillomandibular relationship registration and virtual articulation. The anchor pins from guided surgery were used as fiducial markers for DDS data superimposition. At the second appointment, the prosthesis prototype was tried-in and adjustments were made. Minimal adjustments were needed, and patients and caregivers responded positively. Prostheses demonstrated good stability and retention at delivery. The use of digital technology seems to have several benefits as an alternative method for capturing impressions, especially in young children with cleft lip and palate deformity.
    This study aimed to retrospectively assess using computed tomography pulmonary angiography (CTPA) for predicting residual pulmonary hypertension (RPH) in patients with chronic thromboembolic pulmonary hypertension (CTEPH) after pulmonary endarterectomy (PEA).

    We retrospectively analyzed data of 131 patients with CTEPH who underwent PEA in our center (2008-2015). We measured several diameters of the pulmonary artery and thoracic aorta preoperatively. We evaluated the relationship between these measurements (and their indices) and signs of RPH represented by pulmonary artery systolic pressure (PASP) estimated by echocardiography.

    Significant correlations were observed between the aortopulmonary index and prediction of any residual hypertension and moderate/severe hypertension 1 year after PEA, and any residual hypertension and severe hypertension 2 years after PEA. The aortopulmonary index was significantly related to a reduction in PASP 1 year after the operation. A lower aortopulmonary index (≤0.88 for the ascending aorta and ≤0.64 for the descending aorta) predicted lower RPH.

    The effect of high serum ferritin levels on long-term mortality in hemodialysis patients is unknown. The relationship between serum ferritin levels and 5-year all-cause mortality in hemodialysis patients was investigated in this study.

    A total of 173 prevalent hemodialysis patients were included in this study. The patients were followed for up to 5 years and divided into 3 groups according to time-averaged serum ferritin levels (group 1 serum ferritin <800 ng/mL, group 2 serum ferritin 800-1,500 ng/mL, and group 3 serum ferritin >1,500 ng/mL). Along with the serum ferritin levels, other clinical and laboratory variables that may affect mortality were also included in the Cox proportional-hazards regression analysis.

    Eighty-one (47%) patients died during the 5-year follow-up period. The median follow-up time was 38 (17.5-60) months.     The objective of this study was to evaluate the impact of radiological biomarkers suggestive of cerebral small vessel disease (SVD) on the evolution of cognitive performances after an ischemic stroke (IS).

    We studied patients with a supratentorial IS recruited consecutively to a prospective monocentric longitudinal study. A cognitive assessment was performed at baseline, 3 months, and 1 year and was based on a Montreal Cognitive Assessment, an Isaacs set test of verbal fluency (IST), and a Zazzo's cancellation task (ZCT) for the evaluation of attentional functions and processing speed. The following cerebral SVD biomarkers were detected on a 3-T brain MRI performed at baseline white matter hyperintensities (WMHs), deep and lobar microbleeds, enlarged perivascular spaces in basal ganglia and centrum semiovale, previous small deep infarcts, and cortical superficial siderosis (cSS). Generalized linear mixed models were used to evaluate the relationship between these biomarkers and changes in cognitive perfornt.

    This retrospective cohort study included 841 adult patients hospitalized with COVID-19 infection at the University of Chicago Medical Center between March 25 and June 22, 2020. Out of these 841, 453 patients had a personal history of hypertension. For the first part, we evaluated primary outcomes of in-hospital mortality and ICU admission in hospitalized COVID-19 patients based on their exposure to particular medications regardless of a personal history of hypertension and compared them with those who were not on these medications. The effect of high serum ferritin levels on long-term mortality in hemodialysis patients is unknown. The relationship between serum ferritin levels and 5-year all-cause mortality in hemodialysis patients was investigated in this study. A total of 173 prevalent hemodialysis patients were included in this study. The patients were followed for up to 5 years and divided into 3 groups according to time-averaged serum ferritin levels (group 1 serum ferritin <800 ng/mL, group 2 serum ferritin 800-1,500 ng/mL, and group 3 serum ferritin >1,500 ng/mL). Along with the serum ferritin levels, other clinical and laboratory variables that may affect mortality were also included in the Cox proportional-hazards regression analysis. Eighty-one (47%) patients died during the 5-year follow-up period. The median follow-up time was 38 (17.5-60) months. This retrospective cohort study included 841 adult patients hospitalized with COVID-19 infection at the University of Chicago Medical Center between March 25 and June 22, 2020. SeMet administration produced a significant increase of Se concentration in the liver (65%, p  less then  0.001), heart (40%, p  less then  0.01) and kidneys (45%, p  less then  0.05). In this study, we explore implementation facilitators, barriers, and strategies to overcome these barriers among 12 organizations that implemented the SHIELD intervention, an evidenced-based peer education intervention for people who use drugs. Guided by the Consolidated Framework for Implementation Research, we identified several facilitators and barriers at the outer, inner individuals, and intervention level of the implementation process. Future evidence-based public health programs should, in addition to addressing effectiveness, be relevant to the needs and lives of clients.

    It is recommended that cancer survivors incorporate physical activity into their daily lives after in-hospital rehabilitation. However, there is a lack of training programmes focusing on the specific needs of cancer survivors. TriaGO! - an 8-month intervention study of aerobic endurance training for cancer survivors - was therefore examined. The training programme aims to meet the participants' physical needs and provide socio-emotional support, in the form of an exercise programme that challenges participants to aim to compete in an Olympic- distance triathlon (1,000 m swimming, 45 km cycling, 10 km running) after 8 months' of training.

It has been observed that the recurrence in the treated patients is high and results in castration resistant prostate cancer for which treatment options are limited. This scenario has prompted us to consider immunotherapy with synthetic DNA vaccines, as this approach can generate antigen-specific tumor-killing immune cells. Given the multifocal and heterogeneous nature of prostate cancer, we hypothesized that synthetic DNA vaccines targeting different prostate specific antigens are likely to induce broader and improved immunity who are at high risk as well as advanced clinical stage of prostate cancer, compared to a single antigen approach. Secondary criteria, which include molecular examination, are helpful but not required for NIFTP diagnose. The aim of this study was to assess the molecular background of NIFTP and to answer the question if the aplication of revised criteria for NIFTP diagnosis is associated with the lack of oncogenic mutation. Repeat histopathological assessment of 1117 cases of papillary thyroid carcinoma (PTC) from 2000-2016 was conducted. Using initial (2016) and revised (2018) diagnostic criteria, NIFTP was diagnosed in 23 and 13 patients respectively. 50 tumor genes hotspots mutation analysis was conducted. BRAF V600E mutations were detected in patients who fulfilled only initial NIFTP criteria. Other high-risk mutations (TP53) were found in both groups of patients. Mice were then given 0 Gy or 4 Gy of whole-brain irradiation 2 days before tumor-cell injection or 5 days, 3 weeks, or 6 weeks after tumor-cell injection. Mice were sacrificed 4-weeks or 8-weeks after injection and brain tissues were examined for metastasis by fluorescent stereomicroscopy. In the unirradiated control group, brain metastases were present in 77% of mice at 4 weeks and in 90% of mice at 8 weeks; by comparison, rates for the group given PCI at 5 days after tumor-cell injection were 20% at 4 weeks (p=0.01) and 30% at 8 weeks (p=0.02). The PCI group also had fewer brain metastases per mouse at 4 weeks (p=0.03) and 8 weeks (p=0.006) versus the unirradiated control as well as a lower metastatic burden (p=0.01). Irradiation given either before tumor-cell injection or 3-6 weeks afterward had no significant effect on brain metastases compared to the unirradiated control. These results underscore the importance of timing for irradiating subclinical disease.   • The construction of a subsurface dam and/or physical cutoff barriers is one of the most known techniques used to prevent seawater intrusion during excessive exploitation of freshwater from a coastal aquifer. This method is widely used in many sites around the world (Japan, Brazil, India, Burkina Faso…). In this study, we present an innovative technique for constructing subsurface barriers based on geochemical reactions. A calcite cutoff wall is developed by mixing two aqueous solutions Na2CO3 and CaCl2 under pCO2 equal to 3.16·10-4 bar. The deposition of calcite in the mixing zone induces a high clogging, which greatly reduces the porosity and then the permeability of the aquifer into the injection zone. We use GEODENS code to study the effect of a developed geochemical cutoff wall on saltwater intrusion and to assess their protective effect on preventing seawater intrusion. The GEODENS code can solve these equations by a finite element procedure; it can handle density-dependent flow, transport, and geochemical reactions in porous media. The effect of depth and location of the geochemical cutoff wall is tested and results showed a significant reduction of seawater intrusion penetration length. According to the budget used in many barrier construction projects, we have shown that the developed geochemical cutoff wall presented in this work could produce a lower seawater intrusion penetration length than the traditionally used barriers at a very lower cost.Manganese activated carbon (Mn-AC) was successfully prepared by the incipient wetness method and characterized by SEM, XRD, and FTIR. This study chose N,N-dimethylformamide (DMF) as the target pollutant, and the removal rate of DMF and removal mechanism were systematically studied by dielectric barrier discharge (DBD) plasma combined with Mn-AC. This study indicated that DBD plasma combined with Mn-AC could effectively remove DMF. With the addition of Mn-AC, the removal rate and mineralization rate of DMF within 40 min increased from 51.5% and 36.0% to 82.2% and 58.2%, respectively. The discharge power, initial concentration of DMF, initial pH of the solution, and dosage of Mn-AC affect the removal of DMF. The optimal discharge power is 16.19 W, and energy efficiency is 20.79 mg·kJ-1; low concentration DMF could be removed more effectively. Neutral and alkaline conditions showed better removal effect of DMF than acid conditions; Mn-AC optimal dosage is 1.0 g L-1. The concentration variations of O3, H2O2, and ·OH manifested that Mn-AC could effectively convert O3 and H2O2 to ·OH, thereby increasing the DMF removal rate. Quenching experiments showed that ·OH is the main active species in the reaction. Based on reaction products of DMF such as N-methylformamide, methanol, formaldehyde, and formic acid, possible degradation pathways were proposed. Generalized linear models and restricted cubic spline plots curve were undertaken to elucidate the relationship. Stratified subgroup analysis was tested to exclude interaction between different variates and cobalt. Our results showed that the adjusted odds ratios (ORs) with 95% confidence intervals (CIs) for CVD prevalence across the quartiles of cobalt were 0.94 (0.67, 1.30), 1.55 (1.15, 2.10), and 1.74 (1.28, 2.35) compared with lowest quartile. The restricted cubic spline curve also suggested nonlinear and positive association between cobalt and CVD (P for nonlinearity = 0.007). In summary, our cross-sectional results verify that higher cobalt levels are associated with a higher prevalence of cardiovascular disease.The layout of urban spatial functions may play an important role in sustainable regional development. The results indicate that high market activity is conducive to the formation of functional specialization synergy in space, and the level of social productivity directly affects the functional specialization and the level of synergy within a region. With the continuous strengthening of the implementation of local government policies, the functional specialization and synergy in metropolitan areas have gradually strengthened. However, the promotion effects and the expression of functional specialization, the coordinated development of government actions, and spatial functions differ across metropolitan areas in China.Denitrification and NH3 volatilization are the main removal processes of nitrogen in coastal saline soils.   • Parahydrogen-induced polarization (PHIP) is a source of nuclear spin hyperpolarization, and this technique allows for the preparation of biomolecules for in vivo metabolic imaging. PHIP delivers hyperpolarization in the form of proton singlet order to a molecule, but most applications require that a heteronuclear (e.g. 13C or 15N) spin in the molecule is hyperpolarized. Here we present high field pulse methods to manipulate proton singlet order in the [1-13C]fumarate, and in particular to transfer the proton singlet order into 13C magnetization. We exploit adiabatic pulses, i.e., pulses with slowly ramped amplitude, and use constant-adiabaticity variants the spin Hamiltonian is varied in such a way that the generalized adiabaticity parameter is time-independent. This allows for faster polarization transfer, and we achieve 96.2% transfer efficiency in thermal equilibrium experiments. We demonstrate this in experiments using hyperpolarization, and obtain 6.8% 13C polarization. This work paves the way for efficient hyperpolarization of nuclear spins in a variety of biomolecules, since the high-field pulse sequences allow individual spins to be addressed.The default mode network (DMN) overlaps with regions showing early Alzheimer's Disease (AD) pathology. Age, sex, and apolipoprotein E ɛ4 are the predominant risk factors for developing AD. How these risk factors interact to influence DMN connectivity and connectivity-cognition relationships before the onset of impairment remains unknown. Here, we examined these issues in 475 cognitively normal adults, targeting total DMN connectivity, its anticorrelated network (acDMN), and the DMN-hippocampal component. There were four main findings. First, in the ɛ3 homozygous group, lower DMN and acDMN connectivity was observed with age. Second, sex and ɛ4 modified the relationship between age and connectivity for the DMN and hippocampus with ɛ4 vs. ɛ3 males showing sustained or higher connectivity with age. Third, in the ɛ3 group, age and sex modified connectivity-cognition relationships with the oldest participants having the most differential patterns due to sex. Fourth, ɛ4 carriers with lower connectivity had poorer cognitive performance. Taken together, our results show the three predominant risk factors for AD interact to influence brain function and function-cognition relationships.

Furthermore, different locomotion patterns can also affect joint specific work contributions. While mean behavior of joint work is important to understanding gait, changes in joint kinetic modulation, or the regulation/control of stride-to-stride joint work variability is necessary to elucidate locomotor system function. Suboptimal modulation exhibited as a stochastic time-series (large fluctuation followed by an opposite smaller fluctuation) could potentially affect locomotion efficiency and portend injury risk. Some have suggested that addiction treatment should make use of these insights, tailoring therapy according to people's degree of guilt-proneness versus shame-proneness. In this paper, we challenge this dominant view, reviewing empirical findings from others as well as our own to question (1) whether shame and guilt can be so easily disentangled in the experience of people with addiction, and (2) whether shame and guilt have the opposing action tendencies standardly attributed to them. We recommend a shift in theoretical perspective that explains our main finding that both emotions can be either destructive or constructive for recovery, depending on how these emotions are managed.

Histology may lead to more detailed information on the individual's age and life story (osteobiography).A dominant view of guilt and shame is that they have opposing action tendencies guilt- prone people are more likely to avoid or overcome dysfunctional patterns of behaviour, making amends for past misdoings, whereas shame-prone people are more likely to persist in dysfunctional patterns of behaviour, avoiding responsibility for past misdoings and/or lashing out in defensive aggression. Some have suggested that addiction treatment should make use of these insights, tailoring therapy according to people's degree of guilt-proneness versus shame-proneness. In this paper, we challenge this dominant view, reviewing empirical findings from others as well as our own to question (1) whether shame and guilt can be so easily disentangled in the experience of people with addiction, and (2) whether shame and guilt have the opposing action tendencies standardly attributed to them. We recommend a shift in theoretical perspective that explains our main finding that both emotions can be either destructive or constructive for recovery, depending on how these emotions are managed.

The Bernaerts scale was more sensitive than the Barath scale, and several relationships between the radiological and clinical data were found. The increased perilymphatic enhancement of the cochlea and an extra low-grade vestibular hydrops distinguished in the Bernaerts scale may increase MD diagnosis sensitivity. Magnetic resonance findings in MD support the clinical diagnosis and may help to understand MD pathophysiology better. This review provides an overview of recent research advances on algae as source of proteins, including production strategies from relevant protein-producing species. Particular emphasis will be given to algae protein current applications and forthcoming challenges of their use. In this framework, the present article will propose a theoretical analytical framework for study of PA in chronic illness. It will be built around two key concepts, namely the "standards of life" drawn from the notion of health in the philosophy of Georges Canguilhem and the "illness trajectory" drawn from the interactionist sociology of Anselm Strauss.     Obesity is a risk factor for complications after total joint arthroplasty (TJA). This study analyzed the impact of individual surgeon demographics, financial concerns, and other factors in determining patient candidacy for TJA based on body mass index (BMI).

    A 21-question survey was approved by the American Association of Hip and Knee Surgeons Research Committee for distribution to its membership. Objective questions asked about surgeon or hospital BMI thresholds for offering TJA. Subjective questions asked about physician comfort discussing topics including obesity, bariatric surgery, and weight loss before TJA, as well as insurance and age considerations.

    For TJA procedures, 49.9% of surgeons had a BMI cutoff at 40, 24.5% at 45, and 8.3% at 50. At a BMI cutoff of 40, 23.8% of surgeons felt their patient volume would be adversely affected, whereas at a BMI cutoff of 35, 50% of surgeons felt their patient volume would be adversely affected. Surgeons were more likely to not perform total hip arthroplasty on patients with morbid obesity than total knee arthroplasty (P= .037). Significantly more academic surgeons did not have cutoffs for total hip arthroplasty (P= .003) or total knee arthroplasty (P < .001) compared with all other practice settings.

    There are myriad factors that affect surgeon BMI thresholds for offering elective TJA including poor outcomes, hospital thresholds, financial considerations, and the well being of the patient.   • To investigate the knowledge, perceptions, and attitude of young doctors and nurses at Kind Abdulaziz University Hospital (KAUH) Jeddah, KSA, regarding the effectiveness of telemedicine in dealing with the patients for either follow-up or continuous monitoring of chronic diseases.

    This prospective cross-sectional study was carried out on the young doctors and nursing staff at KAUH. An online questionnaire addressing all the required fields, i.e., background essential demography, perceptions, acceptance, and knowledge regarding telemedicine's utility, was sent to the targeted population of health care workers. Completed responses were analyzed statistically as per study objectives.

    The response rate in our study was 85%. A total of 335 participants responded to our questionnaire. One hundred seventy-one (51.1%) were doctors, whereas one hundred sixty-four (48.9%) were the nursing staff. Among doctors, 50 (29.4%) were recent graduates, whereas, in the nursing side, 77 (46.7%) were the senior nursing studee promotion of telemedicine as an established way of managing patients with special requirements in an effective way. The ongoing COVID-19 pandemic has highlighted the importance of telemedicine in emergencies to protect both patients and health care workers by reducing chronic patients' avoidable hospital visits.The use of dental stem cells (DSCs) has emerged as a promising new approach for therapeutic purposes to treat dental and non-dental diseases. Thus, the aim of this systematic review was to compile all current information on the role and clinical applications of DSCs in modern regenerative medical therapy. PubMed and Google Scholar electronic databases were used to search the literature for relevant studies after applying specific inclusion and exclusion criteria. The search included articles that were published from 2009 to 2019. Several keywords were combined for the search (1) "Clinical applications", (2) "Dental Stem Cell", and (3) "Medicine". Only the 17 studies that fulfilled both the inclusion and exclusion criteria were included in this systematic review. These studies investigated different aspects of DSCs, including cell types, clinical applications, and updates of their use in regenerative medicine. All 17 studies favored the use of different DSCS in regenerative medicine to treat diseases, such as bone defects, neural and skin injuries, Parkinson's disease, ischemia, and others. None of the studies were conducted on humans. This systematic review demonstrated the growing body of evidence supporting the role of DSCs in the field of modern generative medicine. The noninvasive methods of isolating these cells compared to those for isolating non-DSCs make them promising potential sources for the treatment of chronic and devastating diseases. However, more studies are needed to develop the proper guidelines for cases in which DSCs could be considered an accurate and reliable tool for modern regenerative medicine in clinical trials.We present a case of a 48-year-old man's unexpected death affected by a relapsed clivalchordoma. After partial excision surgery of the neoplasm, he manifested 5 days later, in conditions of well-being, a sudden lethal extracranial hemorrhage from nose and mouth. The lesion showed features of GCMN along with large nests of a tumor composed of round cells with a vesicular nucleus, prominent nucleolus, plentiful mitoses, and areas of necrosis. Immunostaining for desmin, LCA, CD 20, CD 34, CD 99, BCL-2, and FLI1 was negative. Tumor cells showed immunopositivity for S-100 and HMB-45 confirming the diagnosis of melanoma. Immunostaining for BRAF V600E was negative; however, NRAS mutation was detected on next-generation sequencing. Unlike adult melanomas BRAF mutations are rare but NRAS mutations have been reported in pediatric melanomas. Adjunctive molecular testing will be important to understand the genetic basis of this disease and future targeted therapy.Malignant eccrine spiradenoma is an extremely rare neoplasm of adnexal origin. It almost always originates from a preexisting long standing eccrine spiradenoma. We present a case of malignant eccrine spiradenoma arising from benign counterpart and having both carcinomatous and sarcomatous differentiation. Here we present a case of a 46 years old lady who presented with a long standing small nodule on her left leg of 7 years' duration with suddenly increase in size. Grossly the mass was partly solid and partly cystic measuring 11.5 cm in maximum dimension with cystic area forming the deeper plane. On microscopy, the superficial dermis showed well demarcated lobules of benign eccrine spiradenoma. Deeper dermis showed tumor cells with features of malignant transformation having both carcinomatous and sarcomatous component. After wide local excision patient is now doing well. The diagnosis of malignant eccrine spiradenoma requires a thorough histopathological examination of the lesion and requires finding a focus of benign spiradenoma within or adjacent to malignant tumour.
