  • As COVID-19 spread all over the world, most of the countries adopted some kind of restrictions to avoid the collapse of health systems. In Brazil, São Paulo and Rio the Janeiro, the two most populated cities in the country, were the first to determine social distancing. In this study, the impact of the social distancing measures on the concentrations of the three main primary air pollutants (PM10, NO2 and CO) was analyzed. CO levels showed the most significant reductions (up to 100%) since it is related to light-duty vehicular emissions. NO2 also showed reductions (9.1%-41.8%) while PM10 levels were only reduced in the 1st lockdown week. The decrease of pollutants was not directly proportional to the vehicular flux reduction, because it depends on other factors such as the transport of air masses from industrial and rural areas. The differences observed can be explained considering the fleet characteristics in the two cities and the response of the population to the social distancing recommendations.The objective of this study was to determine whether (a) cancer-related coping profiles change across time; (b) coping profile transition types predict changes in depressive and physical symptoms. Latent transition analysis was conducted with repeated measures of seven cancer-related coping processes from 460 women recently diagnosed with breast cancer. In multilevel models, coping profile transition groups were entered as predictors of symptoms across 12 months. Three coping profiles emerged at study entry, with two profiles at later assessments. Forty-eight percent of women maintained high-moderate approach-oriented coping over time. Specific factors (e.g., age, acceptance of emotions) differentiated the transition groups. Women who increased and then maintained high-moderate approach-oriented coping had relatively high initial depressive symptoms that declined steeply. When cancer-related acceptance predominated, women experienced increasing physical symptoms. Distinct cancer-related coping patterns are related to the level of and change in depressive and physical symptoms longitudinally. Early intervention to increase approach-oriented coping strategies could yield favorable outcomes.Purpose This study aims to evaluate the clinical features of adenoma of the retinal pigment epithelium (RPE) and eye-sparing treatment for it. The tumors were endoresected and studied histopathologically. Results Of the 16 Asian patients, 15 had RPE adenoma and 1 had RPE adenocarcinoma. Visual acuity decreased in three cases, mainly due to the macular detachment. All tumors were solitary and unilateral and measured from 1.7 × 3.2 × 2.4 to 9.3 × 8.0 × 6.6 mm3. The tumor was located in the macular area in 2 patients, in the juxtapapillary area in 1 patient, and in the peripheral fundus in 13 patients. The tumors were yellow-pink in 3 patients and brown in 13 patients. The tumors showed hypofluorescence in early stage and mottled hyperfluorescence with prominent leakage in late stage on fluorescein angiography. CDI demonstrated arterial blood signals within tumor, and MRI demonstrated hyperintensity and hypointensity in the T1- and T2-weighted images, respectively. The tumors were positive for S-100, neuron-specific enolase (NSE), CK, epithelial membrane antigen (EMA), and vimentin; occasionally positive for melanin-A; and usually negative for melanoma-specific antigen HMB45 and synaptophysin (Syn). The endoresection surgery was performed in all 16 patients by the microinvasive vitrectomy for the excision of intraocular tumors and reconstruction of the eyeball. The follow-up time was 1.5-13 years (mean, 5 years). No tumor recurrence occurred, and the retina remained attached in 16 eyes. Conclusions RPE-derived adenomas are rare and difficult to diagnose clinically. Local resection by the mircoinvasive vitrectomy is a feasible alternative treatment for RPE adenoma.Purpose To investigate the optical coherence tomography (OCT) en face reconstruction of the choroid in different phenotypes of non-neovascular age-related macular degeneration (AMD), to identify the relative distribution of the vascular patterns of the Haller's layer in each AMD category. Qualitative analysis of the en face images provided by CIRRUS HD-OCT 5000 (Carl Zeiss Meditech, Inc., Dublin, USA) was performed, identifying five arrangements of Haller's vessels temporal herringbone, branched from below, laterally diagonal, double arcuate, and reticular. Choroidal thickness (CT) was measured from structural OCT. Healthy age-matched subjects were included as a control group. Results Fifty-eight eyes of 58 patients (20 eyes with RPD; 22 eyes with drusen; 16 eyes with GA) and 18 control eyes were enrolled. The laterally diagonal configuration was the most prevalent (40.0%) in the RPD group; the reticular pattern was the most frequent in the drusen group (50.0%); the double arcuate (62.5%) was the most recurrent pattern in patients with GA. In the control group, the temporal herringbone (38.9%) arrangement was the most represented. The CT associated with the temporal herringbone and reticular arrangement was significantly higher compared to the branched from below (p less then 0.001), the laterally diagonal (p = 0.014), and the double arcuate pattern (p = 0.009). Conclusion Different phenotypes of non-neovascular AMD present a specific distribution of vascular arrangement on en face OCT. The temporal herringbone and the reticular pattern (the ones more associated in a physiological setting) disclosed a thicker choroid compared to the arrangements more represented in non-neovascular AMD-correlated phenotypes.

  • OBJECTIVE  The aim of this study is to assess the impact of specialized pediatric palliative care (PPC) on neonates with life-limiting conditions compared to standard care. STUDY DESIGN  MEDLINE, PsycINFO, Cochrane Central Register of Controlled Trials, Web of Science, CINAHL, Scopus, and Embase databases were searched from January 2000 to September 2018. Randomized clinical trials, experimental or observational studies, and secondary administrative database analyses published in English, Spanish, French, and German were included. Two independent reviewers extracted data, and used the Newcastle-Ottawa Scale and the Cochrane Risk of Bias Tool for quality analysis. Discrepancies were resolved as a team. RESULTS  From the 37,788 records obtained, only eight articles met the inclusion criteria. A meta-analysis was not possible due to the heterogeneity in how the outcomes were defined; however, a qualitative synthesis of the results was possible; organizing outcomes into eight different categories psychological, social and spiritual support; communication; location of care; symptom management; bereavement care; predicted versus actual neonatal outcomes; and parental coping, stress, and satisfaction. https://www.selleckchem.com/products/epz005687.html CONCLUSION  Specialized versus may have an impact on neonates with life-limiting conditions and their families. STUDY DESIGN  This is a retrospective cohort study of triplet pregnancies from 2007 to 2014. Maternal data were manually collected and assessed until 6 weeks postpartum, and neonatal data were assessed until hospital discharge or death. RESULTS  Eighty-two triplet pregnancies were identified with 246 babies born. Mean gestational age at delivery was 32.3 (standard deviation [SD] ± 3.6) weeks and average birth weight was 1,726 g (SD ± 500). There were 12 perinatal deaths and 25 (10.2%) infants diagnosed with a congenital anomaly. Prior preterm birth and nulliparity had a negative impact on gestational age at delivery (p = 0.016) as compared with prior full-term births. Pregnancy complications (preeclampsia or hypertensive disorders [46.3%], gestational diabetes [28%], postpartum hemorrhage [9.8%], and blood transfusions [8.5%]) also impacted gestational age at delivery (33.0 vs. 34.0 weeks, p = 0.031). Spontaneous conception, chorionicity, and maternal medical problems did not have an impact on median gestational age. CONCLUSION  Nulliparity and presence of pregnancy complications are pertinent to triplet outcomes. This study provides valuable information for anticipatory guidance and preconception counseling to patients considering artificial reproductive technology. Esteroides fluoretados (dexametasona e betametasona) e terbulina foram utilizados para aumentar a frequência cardíaca fetal, mas o bloqueio cardíaco fetal não foi reversível, e a administração dos medicamentos foi interrompida devido a efeitos colaterais maternos. Ecocardiogramas fetais de acompanhamento foram realizados, e o feto evoluiu com derrame pericárdico, presença de fibroelastose no ventrículo direito, e disfunção ventricular. A interrupção da gravidez por cesariana foi indicada com 34 semanas, e um marca-passo cardíaco foi implantado no recém-nascido do sexo masculino imediatamente após o nascimento. Os termos thromboembolism (Mesh) AND pregnancy (Mesh) OR postpartum (Mesh) foram utilizados como descritores, incluindo ensaios clínicos randomizados, metanálises, revisões sistemáticas e diretrizes publicadas entre 2009 a 20019, apresentando estratégias de prevenção de tromboembolismo venoso durante a gravidez e o pós-parto. RESULTADOS  Oito artigos abordando estratégias de tromboprofilaxia primária e secundária durante a gestação, parto e puerpério foram selecionados para a presente revisão sistemática. Muitos estudos avaliados foram excluídos por não abordarem estratégias de prevenção. Foram compiladas as recomendações das seguintes sociedades American Society of Hematologists, American College of Obstetricians and Gynecologists, Royal College of Obstetricians and Gynecologists, Society of Obstetricians and Gynaecologists of Canada, American College of Chest Physicians e Royal Australian and New Zealand College of Obstetricians and Gynaecologists.

  • The findings of the present study call for further research using other sensitive biomarkers of effect and different exposure scenarios. In this study, we have investigated biotransformation and oxidative stress responses in relation to tissue contaminant burden in the African sharptooth catfish (Clarias gariepinus) exposed to simulated leachate from a solid waste dumpsite in Calabar, Nigeria. Fish were exposed to simulated leachate, diluted to 00 (negative control), 110, 150, 1100 and phenanthrene (a PAH 50 μg/L used as a positive control) for 3, 7 and 14 days. Hepatic transcripts for cat, sod1, gpx1, gr, gst, cyp1a, cyp2d3, and cyp27 were analyzed by real-time PCR, while enzymatic assays for ethoxyresorufin O-deethylase (EROD), buthoxyresorufin O-deethylase (BROD), methoxyresorufin O-deethylase (MROD), pentoxyresorufin O-deethylase (PROD), catalase (CAT), glutathione peroxidase (GPx), glutathione reductase (GR), glutathione S-transferase (GST), uridine diphospho-glucuronosyltransferase (UDPGT) and lipid peroxidase (LPO) were measured using standard methods. This outcome was observed when using the anodic oxidation with boron doped diamond (BDD), the extra production of persulfate was achieved after the SDS-sulfate released in solution and it reacts with hydroxyl radicals electrogenerated at BDD surface. However, this effect was not already tested by using active anodes. For this reason, the effect of sodium dodecyl sulfate (SDS) during the electrochemical treatment of caffeine was investigated by comparing non-active and active anodes performances. A significant decrease on the oxidation efficiency of caffeine was observed by using Ti/IrO2-Ta2O5 anode at high current density when SDS was added to the solution. Conversely, at BDD anode, the presence of SDS enhanced the degradation efficiency, depending on the applied current density. This behavior is mainly due to the degradation of SDS molecules, which allows to increase the amount of sulfate in solution, promoting the production of persulfate via the mechanism involving hydroxyl radicals when BDD is used. Meanwhile, no oxidation improvements were observed when Ti/IrO2-Ta2O5 anode was employed, limiting the caffeine oxidation. Results clearly showed that the surfactant concentration had little influence on the degradation efficiency, but this result is satisfactory for the BDD system, since it demonstrates that effluents with complex matrices containing surfactants could be effectively degraded using the electrooxidation technique. Degradation mechanisms were explained by electrochemical measurements (polarization curves) as well as the kinetic analysis. Costs and energy consumption were also evaluated. This pioneering study aimed to determine the activity concentrations of 210Po, 210Pb and uranium (234U, 235U, 238U) radionuclides in fruit bodies of wild bolete Boletus bainiugan Dentinger and to estimate its edible safety, which may give scientific evidence for the consumption of this species. The analyses were performed using alpha spectrometer after digestion, exchange resins separation and deposition. Measurement data were analysed and interpolation maps reflecting 210Po, 210Pb and uranium (234U, 235U, 238U) geographical distribution in Yunnan province (China) were presented. In addition, from the perspective of food safety, the possible related effective radiation dose to mushrooms consumers were estimated. The results indicated that 210Po, 210Pb and uranium (234U, 235U, 238U) radionuclides contents in B. bainiugan were significantly different with respect to geographical distribution, and their possible intake in a part of the region was considerably higher. A very interesting observation was done according to the values of 235U/238U activity ratio indicating the occurrence of uranium faction from the global fallout of nuclear weapon tests. Our results showed that the impacted communities consisted mostly of Fe(II) oxidizers and Fe(III) reducers, aromatic compound degraders and predator bacteria.
    The findings of the present study call for further research using other sensitive biomarkers of effect and different exposure scenarios. In this study, we have investigated biotransformation and oxidative stress responses in relation to tissue contaminant burden in the African sharptooth catfish (Clarias gariepinus) exposed to simulated leachate from a solid waste dumpsite in Calabar, Nigeria. Fish were exposed to simulated leachate, diluted to 00 (negative control), 110, 150, 1100 and phenanthrene (a PAH 50 μg/L used as a positive control) for 3, 7 and 14 days. Hepatic transcripts for cat, sod1, gpx1, gr, gst, cyp1a, cyp2d3, and cyp27 were analyzed by real-time PCR, while enzymatic assays for ethoxyresorufin O-deethylase (EROD), buthoxyresorufin O-deethylase (BROD), methoxyresorufin O-deethylase (MROD), pentoxyresorufin O-deethylase (PROD), catalase (CAT), glutathione peroxidase (GPx), glutathione reductase (GR), glutathione S-transferase (GST), uridine diphospho-glucuronosyltransferase (UDPGT) and lipid peroxidase (LPO) were measured using standard methods. Rats with dorsal column SCI crush injury, implanted with physical chitosan-collagen hydrogels (CCH) had severe granulomatous infiltration at the site of the dorsal column injury, which accumulated excess edema at 28 days post-surgery. More pronounced neuroprotective changes were observed with high dose (100 µg/50 µL) Serp-1 CCH implanted rats, but not with low dose (10 µg/50 µL) Serp-1 CCH. Rats treated with Serp-1 CCH implants also had improved motor function up to 20 days with recovery of neurological deficits attributed to inhibition of inflammation-associated tissue damage. In contrast, prolonged low dose Serp-1 infusion with chitosan did not improve recovery. These signaling cascades are involved in melanoma cell response and resistance to the therapeutics used in the clinic, including inhibitors of BRAFmut and MEK1/2, and immunotherapy. In addition, the relationships between sensitivity to non-apoptotic cell death routes and specific cell phenotypes have been demonstrated, suggesting that plasticity of melanoma cells can be exploited to modulate response of these cells to different cell death stimuli. In this review, the current knowledge on the non-apoptotic cell death signaling pathways in melanoma cell biology and response to anti-cancer drugs has been discussed.For most kinds of active polymer optical fibers, a homogeneous distribution of dye molecules over the entire fiber length and cross section is required. In this study, chemical bonding of dyes to poly(methyl methacrylate) (PMMA) by copolymerization is achieved within the polymerization process instead of dissolving the dyes in the monomers. In combination with an improved fabrication mechanism, this leads to homogeneous dye distribution within the preforms. A method for proving the integration of the dyes into the polymer chains has been developed using high-performance liquid chromatography (HPLC) and size exclusion chromatography (SEC). Prestructured core-cladding preforms with dye-doped poly(cylohexyl methacrylate-co-methyl methacrylate)-core have been prepared with the Teflon string technique and were heat-drawn to few mode fibers.This paper deals with the torque ripple minimization method based on the modulation of the phase currents of the permanent-magnet synchronous motor (PMSM) drive. The shape of the supply current waveforms reducing the torque ripple of the machine considered was determined on the basis of finite element analysis (FEA). In the proposed approach, the machine is supplied by a six-leg inverter in order to allow for the injection of zero sequence current harmonics. Two test PMSMs with fractional-slot concentrated windings (FSCW) and surface-mounted permanent magnets (SPMs) have been examined as a case study problem. Wide-range fractional analyses were performed using developed numerical models of the electromagnetic field distribution in the considered machines. The results obtained show that the level of torque ripple in FSCW PMSMs can be effectively reduced by the modulation of the phase currents under the six-leg inverter supply.

  • Aim To validate the Acuros®XB (AXB) dose calculation algorithm for a 6 MV beam from the Varian TrueBeam treatment units. Background Currently Anisotropic Analytic Algorithm (AAA) is clinically used on authors' department but AXB could replace it for VMAT treatments in regions where inhomogeneities and free air are present. https://www.selleckchem.com/products/lonafarnib-sch66336.html Materials and methods Two steps are followed in the validation process of a new dose calculation algorithm. The first is to check the accuracy of algorithm for a homogenous phantom and regular fields. Multiple fields of increasing complexity have been acquired using a Mapcheck diode array. The accuracy of the algorithm was evaluated using the gamma analysis method. The second is to validate the algorithm in the presence of heterogeneous media. Planar absolute dose was measured with GafChromic®EBT2 film and was compared with the dose calculated by AXB. Gamma analysis was performed between Mapcheck measurements and AXB dose calculations, at a range of clinical source-surface distance. Results For SSDs ranging from 80 to 100 cm, the results show a minimum pass rate of 95% between AXB and Mapcheck acquisition. For open 6 MV photon beam interacting with a phantom with an air gap, the agreement after the air gap between AXB and GafChromic®EBT2 is less than 1% in the 3 × 3cm2 field and less than 2% in the 10 × 10 cm2 field. Conclusions AXB has advanced modelling of lateral electron transport that enables a more accurate dose calculation in heterogeneous regions and, compared with AAA, improves accuracy between different density interfaces. Background High Z materials absorb X-ray remarkably. Among Nano-science, Zinc Oxide nanoparticles are interesting semiconductors, producing reactive oxygen species when irradiated by photons. Therefore, it seems that dose enhancement originating by incorporating ZnO NPs in irradiated volume would increase the therapeutic ratio. Materials and methods Initially, the PRESAGE dosimeter was fabricated and calibrated. Then Zinc Oxide nanoparticles with an average particle size of about 40 nm were synthesized. At next step, various concentrations of the nanoparticles were incorporated into the PRESAGE composition and irradiated in radiation fields. Then, the mentioned processes were simulated. Results Practical measurements revealed that by incorporating 500, 1000 and 3000 μg ml-1 ZnO NPs into PRESAGE the dose enhancement factor of 1.36, 1.39, 1.44 for 1 × 1 cm 2 field size, 1.39, 1.41, 1.46 for 2 × 2 cm 2 and 1.40, 1.45 and 1.50 for 3 × 3 cm 2 could be found, respectively. Simulation results showed that in the mentioned condition, the dose enhancement factor of 1.05, 1.08, 1.10 for 1 × 1 cm 2 field size, 1.06, 1.09, 1.10 for 2 × 2 cm 2 and 1.08, 1.11 and 1.13 for 3 × 3 cm 2 could be derived, respectively. Conclusion The results of this study showed that dose enhancement increases by increasing concentration of Zinc Oxide nanoparticles. Many reasons such as photoelectric, pair production effects and even Compton scattering can cause dose enhancement for megavoltage beams.Aim To investigate the impact of Acuros XB (AXB) algorithm in the deep-inspiration breath-hold (DIBH) technique used for treatment of left sided breast cancer. Background AXB may estimate better lung toxicities and treatment outcome in DIBH. Materials and methods Treatment plans were computed using the field-in-field technique for a 6 MV beam in two respiratory phases - free breathing (FB) and DIBH. For a similar target coverage (p > 0.05) in the DIBH respiratory phase between the AXB and AAA algorithm, there was a slight increase in organ at risk (OAR) dose for AXB in comparison to AAA, except for mean dose to the ipsilateral lung. AAA predicts higher mean dose to the ipsilateral lung and lesser V20Gy for the ipsilateral and common lung in comparison to AXB. The differences in mean dose to the ipsilateral lung were 0.87 ± 2.66 % (p > 0.05) in FB, and 1.01 ± 1.07% (p less then 0.05) in DIBH, in V20Gy the differences were 1.76 ± 0.83% and 1.71 ± 0.82% in FB (p less then 0.05), 3.34 ± 1.15 % and 3.24 ± 1.17 % in DIBH (p less then 0.05), for the ipsilateral and common lung, respectively. Conclusion For a similar target volume coverage, there were important differences between the AXB and AAA algorithm for low-density inhomogeneity medium present in the DIBH respiratory phase for left sided breast cancer patients. Local control for T0-1 was 100% and for T2 67% at five years (p = 0.02). Regional-free recurrence survival was 92% at 5 years. Metastasis-free survival was 84% at 5 years. All cases of metastasis had histopathology of undifferentiated.
    Aim To validate the Acuros®XB (AXB) dose calculation algorithm for a 6 MV beam from the Varian TrueBeam treatment units. The first is to check the accuracy of algorithm for a homogenous phantom and regular fields. Multiple fields of increasing complexity have been acquired using a Mapcheck diode array. The accuracy of the algorithm was evaluated using the gamma analysis method. The second is to validate the algorithm in the presence of heterogeneous media. Planar absolute dose was measured with GafChromic®EBT2 film and was compared with the dose calculated by AXB. Gamma analysis was performed between Mapcheck measurements and AXB dose calculations, at a range of clinical source-surface distance. Results For SSDs ranging from 80 to 100 cm, the results show a minimum pass rate of 95% between AXB and Mapcheck acquisition. For open 6 MV photon beam interacting with a phantom with an air gap, the agreement after the air gap between AXB and GafChromic®EBT2 is less than 1% in the 3 × 3cm2 field and less than 2% in the 10 × 10 cm2 field. Conclusions AXB has advanced modelling of lateral electron transport that enables a more accurate dose calculation in heterogeneous regions and, compared with AAA, improves accuracy between different density interfaces. Therefore, it seems that dose enhancement originating by incorporating ZnO NPs in irradiated volume would increase the therapeutic ratio. Materials and methods Initially, the PRESAGE dosimeter was fabricated and calibrated. Then Zinc Oxide nanoparticles with an average particle size of about 40 nm were synthesized. At next step, various concentrations of the nanoparticles were incorporated into the PRESAGE composition and irradiated in radiation fields. Then, the mentioned processes were simulated. Results Practical measurements revealed that by incorporating 500, 1000 and 3000 μg ml-1 ZnO NPs into PRESAGE the dose enhancement factor of 1.36, 1.39, 1.44 for 1 × 1 cm 2 field size, 1.39, 1.41, 1.46 for 2 × 2 cm 2 and 1.40, 1.45 and 1.50 for 3 × 3 cm 2 could be found, respectively. Simulation results showed that in the mentioned condition, the dose enhancement factor of 1.05, 1.08, 1.10 for 1 × 1 cm 2 field size, 1.06, 1.09, 1.10 for 2 × 2 cm 2 and 1.08, 1.11 and 1.13 for 3 × 3 cm 2 could be derived, respectively. This study aimed to analyze the changing patterns and the tempo-geographic features of the COVID-19 epidemic, to provide further evidence for real-time responses. METHODS Daily data on COVID-2019 cases between 31st Dec. 2019 and 26th Feb. 2020 were collected and analyzed for Hubei and non-Hubei regions. Observed trends for new and cumulative cases were analyzed through joint-point regressions. Spatial analysis was applied to show the geographic distribution and changing pattern of the epidemic. RESULTS By 26th Feb. 2020, 78,630 confirmed COVID-19 cases had been reported in China. In Hubei, an increasing trend (slope=221) was observed for new cases between 24th Jan. and February 7th Feb. 2020, after which a decline commenced (slope=-868). However, as the diagnosis criteria changed, a sudden increase (slope=5530) was observed on 12th Feb., which sharply decreased afterward (slope=-4898). In non-Hubei regions, the number of new cases increased from 20th Jan. to 3rd Feb. and started to decline afterward (slope=-53). The spatial analysis identified Chongqing, Guangzhou, Shenzhen, Changsha, Nanchang, Wenzhou, Shanghai, Xinyang, Jining, and Beijing as the hotspots outside of Hubei province in China. CONCLUSION AND RELEVANCE The joint-point regression analysis indicated that the epidemic might have been under control in China, especially for regions outside of Hubei province. METHODS In this retrospective three-centers study, 372 non-severe COVID-19 patients during hospitalization were followed for more than 15 days after admission. Patients who deteriorated to severe or critical COVID-19 and patients who kept non-severe state were assigned to the severe and non-severe group, respectively. Based on baseline data of the two groups, we constructed a risk prediction nomogram for severe COVID-19 and evaluated its performance. RESULTS The training cohort consisted of 189 patients, while the two independent validation cohorts consisted of 165 and 18 patients. Among all cases, 72 (19.35%) patients developed severe COVID-19. We found that old age, and higher serum lactate dehydrogenase, C-reactive protein, the coefficient of variation of red blood cell distribution width, blood urea nitrogen, direct bilirubin, lower albumin, are associated with severe COVID-19. We generated the nomogram for early identifying severe COVID-19 in the training cohort (AUC 0.912 [95% CI 0.846-0.978], sensitivity 85.71%, specificity 87.58%); in validation cohort (0.853 [0.790-0.916], 77.5%, 78.4%). The calibration curve for probability of severe COVID-19 showed optimal agreement between prediction by nomogram and actual observation. Decision curve and clinical impact curve analysis indicated that nomogram conferred high clinical net benefit. CONCLUSION Our nomogram could help clinicians to early identify patients who will exacerbate to severe COVID-19, which will enable better centralized management and early treatment of severe patients. © The Author(s) 2020. We examined the association between patient-reported outcome measures and self-reported physical health at 1-year follow-up. DESIGN A retrospective cohort study. SETTING Data were collected as part of the Israeli Psychiatric Rehabilitation Patient-Reported Outcome Measurement program in Israel. PARTICIPANTS A total of 2581 psychiatric rehabilitation service users assessed between April 2013 and January 2016. MAIN OUTCOME MEASURES Self-reports on two consecutive years of physical health dichotomized as poor versus good. RESULTS More than one-third of participants reported having poor physical health. Multivariate regression analysis showed that quality of life (odds ratio [OR] = 0.71; 95% confidence interval [CI] 0.60-0.84) and lack of effect of symptoms on functioning (OR = 0.81; 95%CI 0.74-0.89) predict subsequent physical health, controlling for all other factors. Compared to a multivariate model with personal characteristics and self-reports on physical health at baseline (Model A), the model which also included patient-reported outcome measures (Model B) showed slightly better discrimination (c-statistic 0.74 vs. 0.76, respectively). CONCLUSIONS These results suggest that patient-reported outcome measures contribute to the prediction of poor physical health and thus can be useful as an early screening tool for people with serious mental illnesses living in the community, who are at risk of physical health problems. © The Author(s) 2020.   • Among humans, simply looking at infants can activate affiliative and nurturant behaviors. However, it remains unknown whether mere exposure to infants also activates other aspects of the caregiving motivational system, such as generalized defensiveness in the absence of immediate threats. Here, we demonstrate that simply viewing faces of infants (especially from the ingroup) may heighten vigilance against social threats and support for institutions that purportedly maintain security. Across two studies, participants viewed and rated one among several image types (between-subjects design) Infants, adult males, adult females, and puppies in Study 1, and infants of varying racial/ethnic groups (including one's ingroup) and puppies in Study 2. Following exposure to one of these image types, participants completed measures of intergroup bias from a range of outgroups that differed in perceived threat, belief in a dangerous world, right-wing authoritarianism and social-political conservatism (relative to liberalism). In Study 1 (United States), stronger affiliative reactions to images of infants (but not adults or puppies) predicted stronger perceptions of a dangerous world, endorsement of right-wing authoritarianism, and support for social-political conservatism (relative to liberalism). Study 2 (Italy) revealed that exposure to images of ingroup infants (compared to outgroup infants) increased intergroup bias against outgroups that are characterized as threatening (immigrants and Arabs) and increased conservatism. These findings suggest a predisposed preparedness for social vigilance in the mere suggested presence of infants (e.g., viewing images) even in the absence of salient external threats.Precision medicine, which includes comprehensive genome sequencing, is a potential therapeutic option for treating high-grade serous carcinoma (HGSC). However, HGSC is a heterogeneous tumor at the architectural, cellular, and molecular levels. Intratumoral molecular heterogeneity currently limits the precision of medical strategies based on the gene mutation status. This study was carried out to analyze the presence of 160 cancer-related genetic alterations in three tissue regions with different pathological features in a patient with HGSC. The patient exhibited histological heterogeneous features with different degrees of large atypical cells and desmoplastic reactions. TP53 mutation, ERBB2 and KRAS amplification, and WT1, CDH1, and KDM6A loss were detected as actionable gene alterations. Interestingly, the ERBB2 and KRAS amplification status gradually changed according to the region examined. The difference was consistent with the differences in pathological features. Electron beam lithography followed by low-temperature Au metal assisted chemical etching (MacEtch) is used to pattern high resolution, high aspect ratio, and vertical silicon nanostructures, used as a template. The core idea is to detect presence of person along its activities of daily living without deploying a device on person's body. The Wi-Fi signals received using non-wearable devices are converted into time-frequency scalograms. The occupancy is detected by classifying the scalogram images using an auto-encoder neural network.
    Among humans, simply looking at infants can activate affiliative and nurturant behaviors. However, it remains unknown whether mere exposure to infants also activates other aspects of the caregiving motivational system, such as generalized defensiveness in the absence of immediate threats. Here, we demonstrate that simply viewing faces of infants (especially from the ingroup) may heighten vigilance against social threats and support for institutions that purportedly maintain security. Across two studies, participants viewed and rated one among several image types (between-subjects design) Infants, adult males, adult females, and puppies in Study 1, and infants of varying racial/ethnic groups (including one's ingroup) and puppies in Study 2. Following exposure to one of these image types, participants completed measures of intergroup bias from a range of outgroups that differed in perceived threat, belief in a dangerous world, right-wing authoritarianism and social-political conservatism (relative to liberalism). In Study 1 (United States), stronger affiliative reactions to images of infants (but not adults or puppies) predicted stronger perceptions of a dangerous world, endorsement of right-wing authoritarianism, and support for social-political conservatism (relative to liberalism). Study 2 (Italy) revealed that exposure to images of ingroup infants (compared to outgroup infants) increased intergroup bias against outgroups that are characterized as threatening (immigrants and Arabs) and increased conservatism. These findings suggest a predisposed preparedness for social vigilance in the mere suggested presence of infants (e.g., viewing images) even in the absence of salient external threats.Precision medicine, which includes comprehensive genome sequencing, is a potential therapeutic option for treating high-grade serous carcinoma (HGSC). However, HGSC is a heterogeneous tumor at the architectural, cellular, and molecular levels. Intratumoral molecular heterogeneity currently limits the precision of medical strategies based on the gene mutation status. This study was carried out to analyze the presence of 160 cancer-related genetic alterations in three tissue regions with different pathological features in a patient with HGSC. The patient exhibited histological heterogeneous features with different degrees of large atypical cells and desmoplastic reactions. TP53 mutation, ERBB2 and KRAS amplification, and WT1, CDH1, and KDM6A loss were detected as actionable gene alterations. Interestingly, the ERBB2 and KRAS amplification status gradually changed according to the region examined. The difference was consistent with the differences in pathological features.   • Recently, we have reported that dentate mossy cells (MCs) control memory precision via directly and functionally innervating local somatostatin (SST) inhibitory interneurons. Here, we report a discovery that dysfunction of synaptic transmission between MCs and SST cells causes memory imprecision in a mouse model of early Alzheimer's disease (AD). Single-cell RNA sequencing reveals that miR-128 that binds to a 3'UTR of STIM2 and inhibits STIM2 translation is increasingly expressed in MCs from AD ****. Silencing miR-128 or disrupting miR-128 binding to STIM2 evokes STIM2 expression, restores synaptic function, and rescues memory imprecision in AD ****. The total reported with cancer was divided by total number of relatives to derive a percentage of cancer reporting for each pedigree. The percentages of cancer reporting were analyzed using column proportions, nonparametric tests, and a Poisson regression. Cancer reporting in first- and second-degree relatives was highest among non-Hispanic Whites and Ashkenazi Jewish individuals, with median percentages of 22% and 27%, respectively. However, the significance of high-density lipoprotein cholesterol (HDL-C) level, low-density lipoprotein cholesterol (LDL-C) level and the HDL-C/LDL-C ratio (H/L ratio) as markers for disease progression remain unknown. Aim The aim of this study was to evaluate the role of HDL-C levels, LDL-C levels and the H/L ratio as markers of disease progression in patients admitted to the intensive cate unit with acute pancreatitis. Methods This retrospective study was conducted at a tertiary critical care center in China. Plasma HDL-C and LDL-C levels were measured in 166 patients with acute pancreatitis. The associations between HDL-C, LDL-C, H/L ratio, as well as other inflammatory index and mortality, were analyzed. Multivariate cox analysis based on two models was used to determine the independent prognostic factor. Predictive ability of in-hospital mortality for variables was determined using the receiver operating characteristics curves. Results Significantly higher H/L ratios at admission were observed in patients with acute pancreatitis who died compared with survivors (0.93 vs. 0.64, p less then 0.001). The area under the ROC curve for H/L ratio-based prediction of mortality was 0.658. When clinical confounders were included in multivariable cox regression analysis, the association was preserved (Model A HR = 1.587, p = 0.011; Model B HR = 1.332, p = 0.032). The mortality risk in different groups defined by an H/L ratio cutoff value was significantly different, based on survival curve analysis. Conclusion The H/L ratio at the time of admission to the ICU appears to be a biomarker of disease progression in patients with acute pancreatitis.Background Lactococcus members belonging to lactic acid bacteria are widely used as starter bacteria in the production of fermented dairy products. From kimchi, a Korean food made of fermented vegetables, Lactococcus raffinolactis WiKim0068 was isolated and its genome was analyzed. Results The complete genome of the strain WiKim0068 consists of one chromosome and two plasmids that comprises 2,292,235 bp, with a G + C content of 39.7 mol%. Analysis of orthoANI values among Lactococcus genome sequences showed that the strain WiKim0068 has > 67% sequence similarity to other species and subspecies. In addition, it displayed no antibiotic resistance and can metabolize nicotinate and nicotinamide (vitamin B3). Conventional text mining pipelines provide tools to automatically extract entities and relationships from the scientific literature, and partially replace expert curation, but such machine learning frameworks often require a large amount of labeled training data and thus lack scalability for both larger document corpora and new relationship types. Results We developed an application of Snorkel, a weakly supervised learning framework, for extracting chemical reaction relationships from biomedical literature abstracts. For this work, we defined a chemical reaction relationship as the transformation of chemical A to chemical B. We built and evaluated our system on small annotated sets of chemical reaction relationships from two corpora curated bacteria-related abstracts f on MetaCyc documents enriched with chemical reactions to a general set of articles related to bacteria.Objective Coronary slow flow (CSF) is characterized by delayed opacification of distal epicardial coronary arteries without significant coronary stenosis. In addition, The changes of lipoprotein-associated phospholipase A2 (Lp-PLA2) as a significant predictive factor for CSF remain controversial. The study aims to investigate the association between plasma Lp-PLA2 and CSF. Methods In this retrospective study, 170 consecutive patients who underwent coronary angiography were enrolled in Beijing Anzhen Hospital from January 2017 to September 2019, and were divided into CSF group and normal control groups. According to coronary blood flow rate measured by the thrombolysis in myocardial infarction frame count (TFC) method, CSF was defined as TFC > 27. Serum Lp-PLA2 levels were measured in an enzyme-linked immunosorbent assay. Results Lp-PLA2 levels were higher in the CSF group than in the control group (288.6 ± 50.3 versus 141.9 ± 49.7, P less then 0.001) and were significantly correlated with the mean coronary artery thrombolysis in myocardial infarction (TIMI) frame count (r = 0.790, P less then 0.001). Logistic regression analysis showed that high Lp-PLA2 was independently associated with CSF after adjustment for conventional risk factors (OR = 1.040, CI = 1.022-1.059, P less then 0.001). Male sex (OR = 2.192, CI = 1.161-4.140, P = 0.016) and hypertension (OR = 1.965, CI = 1.034-3.736, P = 0.039) were also CSF risk factors. Receiver-operating characteristic curve (ROC) analysis showed that Lp-PLA2 levels can predict CSF severity; the predictive power was higher than the other risk factors. Conclusion Our study demonstrated that patients with CSF had higher circulating levels of Lp-PLA2 than normal controls. After adjustment for potential confounders, increased Lp-PLA2 was independently associated with presence of CSF.

Methods This retrospective study was conducted at a tertiary critical care center in China. Plasma HDL-C and LDL-C levels were measured in 166 patients with acute pancreatitis. The associations between HDL-C, LDL-C, H/L ratio, as well as other inflammatory index and mortality, were analyzed. Multivariate cox analysis based on two models was used to determine the independent prognostic factor. Predictive ability of in-hospital mortality for variables was determined using the receiver operating characteristics curves. Results Significantly higher H/L ratios at admission were observed in patients with acute pancreatitis who died compared with survivors (0.93 vs. 0.64, p less then 0.001). The area under the ROC curve for H/L ratio-based prediction of mortality was 0.658. When clinical confounders were included in multivariable cox regression analysis, the association was preserved (Model A HR = 1.587, p = 0.011; Model B HR = 1.332, p = 0.032). The mortality risk in different groups defined by an H/L ratio cutoff value was significantly different, based on survival curve analysis. Conclusion The H/L ratio at the time of admission to the ICU appears to be a biomarker of disease progression in patients with acute pancreatitis.Background Lactococcus members belonging to lactic acid bacteria are widely used as starter bacteria in the production of fermented dairy products. From kimchi, a Korean food made of fermented vegetables, Lactococcus raffinolactis WiKim0068 was isolated and its genome was analyzed. Results The complete genome of the strain WiKim0068 consists of one chromosome and two plasmids that comprises 2,292,235 bp, with a G + C content of 39.7 mol%. Analysis of orthoANI values among Lactococcus genome sequences showed that the strain WiKim0068 has > 67% sequence similarity to other species and subspecies. In addition, it displayed no antibiotic resistance and can metabolize nicotinate and nicotinamide (vitamin B3). Conclusion These results augments our understanding of the genus Lactococcus and suggest that this new strain has potential industrial applications.Background Enzymatic and chemical reactions are key for understanding biological processes in cells. Curated databases of chemical reactions exist but these databases struggle to keep up with the exponential growth of the biomedical literature. Conventional text mining pipelines provide tools to automatically extract entities and relationships from the scientific literature, and partially replace expert curation, but such machine learning frameworks often require a large amount of labeled training data and thus lack scalability for both larger document corpora and new relationship types. Results We developed an application of Snorkel, a weakly supervised learning framework, for extracting chemical reaction relationships from biomedical literature abstracts. For this work, we defined a chemical reaction relationship as the transformation of chemical A to chemical B. We built and evaluated our system on small annotated sets of chemical reaction relationships from two corpora curated bacteria-related abstracts f on MetaCyc documents enriched with chemical reactions to a general set of articles related to bacteria.Objective Coronary slow flow (CSF) is characterized by delayed opacification of distal epicardial coronary arteries without significant coronary stenosis. In addition, The changes of lipoprotein-associated phospholipase A2 (Lp-PLA2) as a significant predictive factor for CSF remain controversial. The study aims to investigate the association between plasma Lp-PLA2 and CSF. Methods In this retrospective study, 170 consecutive patients who underwent coronary angiography were enrolled in Beijing Anzhen Hospital from January 2017 to September 2019, and were divided into CSF group and normal control groups. According to coronary blood flow rate measured by the thrombolysis in myocardial infarction frame count (TFC) method, CSF was defined as TFC > 27. Serum Lp-PLA2 levels were measured in an enzyme-linked immunosorbent assay. Results Lp-PLA2 levels were higher in the CSF group than in the control group (288.6 ± 50.3 versus 141.9 ± 49.7, P less then 0.001) and were significantly correlated with the mean coronary artery thrombolysis in myocardial infarction (TIMI) frame count (r = 0.790, P less then 0.001). Logistic regression analysis showed that high Lp-PLA2 was independently associated with CSF after adjustment for conventional risk factors (OR = 1.040, CI = 1.022-1.059, P less then 0.001). Male sex (OR = 2.192, CI = 1.161-4.140, P = 0.016) and hypertension (OR = 1.965, CI = 1.034-3.736, P = 0.039) were also CSF risk factors. Receiver-operating characteristic curve (ROC) analysis showed that Lp-PLA2 levels can predict CSF severity; the predictive power was higher than the other risk factors. Conclusion Our study demonstrated that patients with CSF had higher circulating levels of Lp-PLA2 than normal controls. After adjustment for potential confounders, increased Lp-PLA2 was independently associated with presence of CSF.
