    The emergence and increased spread of microbial resistance is a major challenge to all health care systems worldwide. In primary care, acute respiratory tract infection (ARTI) is the health condition most strongly related to antibiotic overuse.

    The RESIST program aims at optimizing antibiotic prescribing for ARTI in German primary care. By completing a problem-orientated online training course, physicians are motivated and empowered to utilize patient-centered doctor-patient communication strategies, including shared decision making, in the treatment of patients with ARTI.

    RESIST will be evaluated in the form of a nonrandomized controlled trial. Approximately 3000 physicians of 8 (out of 16) German federal states can participate in the program. Patient and physician data are retrieved from routine health care data. Physicians not participating in the program serve as controls, either among the 8 participating regional Associations of Statutory Health Insurance Physicians (control group 1) or among the rcare. Long-term follow up of up to 21 months will allow for investigating the sustainability of the intervention.

    Telehealth is an effective means to assist existing health care systems, particularly for the current aging society. However, most extant telehealth systems employ individual data sources by offline data processing, which may not recognize health deterioration in a timely way.

    Our study objective was two-fold to design and implement an integrated, personalized telehealth system on a community-based level; and to evaluate the system from the perspective of user acceptance.

    The system was designed to capture and record older adults' health-related information (eg, daily activities, continuous vital signs, and gait behaviors) through multiple measuring tools. State-of-the-art data mining techniques can be integrated to detect statistically significant changes in daily records, based on which a decision support system could emit warnings to older adults, their family members, and their caregivers for appropriate interventions to prevent further health deterioration. A total of 45 older adults recruited from of our personalized telehealth system in their future health management (ie, behavioral intention mean 4.45, SD 1.78).

    The proposed health monitoring system provides an example design for monitoring older adults' health status based on multiple data sources, which can help develop reliable and accurate predictive analytics.   • Thyroid dysfunction, both hypo- and hyperthyroidism, has been associated with cardiovascular disease. The aim of this study was to evaluate the association between thyroid dysfunction and atherosclerosis measured mostly by carotid intima-media thickness, as well as discuss whether L-T4 replacement is able to reverse or slow down the progression of atherosclerosis.

    The review was conducted according the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. We performed on PubMed a literature search from May 2004 to January 2020, using the search terms 'subclinical hypothyroidism' or 'thyroid disorders' and 'carotid artery', 'carotid intima-media thickness (IMT)', 'levothyroxine', and 'atherosclerosis'.

    Twenty-six studies were eligible and included in the analysis. Overall, the studies encompassed a total of 36.434 patients included in this review. Most studies indicated a proportional correlation between IMT and thyroid dysfunction. Levothyroxine (L-T4) replacement led to significant decrease of IMT after 1 year in most studies.

    Most studies have concluded that thyroid dysfunction is associated with arterial wall remodeling and, thus, with increased cardiovascular risk.     Thyroid dysfunction, both hypo- and hyperthyroidism, has been associated with cardiovascular disease. The aim of this study was to evaluate the association between thyroid dysfunction and atherosclerosis measured mostly by carotid intima-media thickness, as well as discuss whether L-T4 replacement is able to reverse or slow down the progression of atherosclerosis. The review was conducted according the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. We performed on PubMed a literature search from May 2004 to January 2020, using the search terms 'subclinical hypothyroidism' or 'thyroid disorders' and 'carotid artery', 'carotid intima-media thickness (IMT)', 'levothyroxine', and 'atherosclerosis'. Twenty-six studies were eligible and included in the analysis. Overall, the studies encompassed a total of 36.434 patients included in this review.     Ventricular function evaluation in coarctation of the aorta (CoA) has become more sophisticated and precise with speckle tracking, revealing subclinical changes. However, CoA stenting treatment effects in on myocardial strain are still controversial. This study aimed to estimate the extent to which changes in left ventricular global longitudinal strain (LV GLS) occur in patients with CoA who undergo stenting.

    The study included 21 patients with CoA (median age 15 years [8-39]) and 21 healthy individuals matched by age and gender. Clinical and echocardiographic evaluations were performed 1 day before, 6 months, and 1 year after stenting. Correlations between LV GLS and arm-leg gradient, isthmus gradient on echocardiogram, age at intervention, left ventricular mass, and ejection fraction were tested.

    Before treatment, patients with CoA had lower LV GLS than the control group (-18.4% ± 1.96 vs -21.5% ± 1.37; P < .01), showing significant increase to -19.4% ± 2.1 at 6 months and -20.7% ± 2.19 at 1 year, P < .001. Only 28.5% (6 patients) had preserved GLS before treatment, improving to 80.9% (17 patients) in 1 year. The only variable correlated with low LV GLS values before treatment was age at intervention (Spearman's index = -0.571; P = .007).

The extracted wavelet coefficients are then modeled by a hidden Markov model (HMM) with Gaussian emission distributions. The association of a signal with a representative HMM is performed utilizing the expectation-maximization algorithm. Eventually, the signal is characterized by the set of parameters that describe the HMM. The Kullback-Leibler divergence is employed as the similarity measure of two signals, comparing their corresponding HMMs. To validate the performance of the proposed characterization scheme, which is denoted as the probabilistic signal characterization scheme (PSCS), a simulated and a real experiment have been considered. The measured signal is characterized by the proposed PSCS method, and the model parameters of the seabed are estimated by means of an inversion procedure employing a genetic algorithm. Motivated by the modelling of BBSAN using the wavepacket framework, a traversable microphone ring is used to decompose the acoustic pressure into azimuthal Fourier modes. Unlike noise radiated downstream, BBSAN is dominated by azimuthal modes 1-3, which are approximately 3-4 dB/St stronger than the axisymmetric component. Crucially, the relative contribution of successive modes to BBSAN is sensitive to the observer angle and jet operating condition. Four azimuthal modes are necessary to reconstruct the total BBSAN signal to within 1 dB/St accuracy for the conditions presented here. The analysis suggests, however, that the number of modes required to maintain this accuracy increases as the peak frequency shifts upward. The results demonstrate the need to carefully consider the azimuthal content of BBSAN when comparing acoustic measurements to predictions made by jet noise models built on instability theory.

Previously, airborne sound insulation models have been used to predict the associated measurement results with limited success. In this paper, the importance of accounting for the structure borne sound transmission between two glazing elements via the window frame on the prediction results is demonstrated. The decreased stiffness of the wall cavity as the depth is increased is the reason why sound transmission via the window frame needs to be considered. The reciprocity argument provided by Davy for the prediction of the directivity of sound radiating into a room is validated and it is shown that once the structure borne transmission is considered, an additional weighting term is not needed to compensate for the extra wall collisions which the sound experiences when radiated at grazing incidence.A probabilistic characterization scheme for acoustic signals with applications in acoustical oceanography is presented. This scheme aims at the definition of a set of stochastic observables that could characterize the signal. To this end, the signal is decomposed into several levels using the stationary wavelet packet transform. The extracted wavelet coefficients are then modeled by a hidden Markov model (HMM) with Gaussian emission distributions. The association of a signal with a representative HMM is performed utilizing the expectation-maximization algorithm. Eventually, the signal is characterized by the set of parameters that describe the HMM. The Kullback-Leibler divergence is employed as the similarity measure of two signals, comparing their corresponding HMMs. To validate the performance of the proposed characterization scheme, which is denoted as the probabilistic signal characterization scheme (PSCS), a simulated and a real experiment have been considered. The measured signal is characterized by the proposed PSCS method, and the model parameters of the seabed are estimated by means of an inversion procedure employing a genetic algorithm. Of the three components of supersonic jet noise, the focus is on the broadband shock-associated noise (BBSAN) component. Motivated by the modelling of BBSAN using the wavepacket framework, a traversable microphone ring is used to decompose the acoustic pressure into azimuthal Fourier modes. Unlike noise radiated downstream, BBSAN is dominated by azimuthal modes 1-3, which are approximately 3-4 dB/St stronger than the axisymmetric component. Crucially, the relative contribution of successive modes to BBSAN is sensitive to the observer angle and jet operating condition. Four azimuthal modes are necessary to reconstruct the total BBSAN signal to within 1 dB/St accuracy for the conditions presented here. The analysis suggests, however, that the number of modes required to maintain this accuracy increases as the peak frequency shifts upward. The results demonstrate the need to carefully consider the azimuthal content of BBSAN when comparing acoustic measurements to predictions made by jet noise models built on instability theory.

The amendment in this study likely operates through a combination of soil chemical changes and by influencing the soil-microbe-plant interaction.Highly persistent, toxic and bioaccumulative per - and polyfluoroalkyl substances (PFAS) represents a serious problem for the environment and their concentrations and fate remain largely unknown. The present study consists of a PFAS screening in sludges originating from 43 wastewater treatment plants (WWTPs) in the Czech Republic. To analyze an extended group of PFAS consisting of 32 PFAS, including GenX and other new replacements of older and restricted PFAS in sludge, a new method was optimized and validated using pressurized solvent extraction, followed by the SPE clean-up step to eliminate the observed matrix effects and LC-MS/MS. The results revealed high PFAS contamination of sewage sludge, reaching values from 5.6 to 963.2 ng g-1. The results showed that in the majority of the samples (about 60%), PFOS was the most abundant among the targeted PFAS, reaching 932.9 ng g-1. Approximately 20% of the analyzed samples contained more short-chain PFAS, suggesting the replacement of long-chain PFAS (especially restricted PFOA and PFOS). GenX was detected in 9 samples, confirming the trend in the use of new PFAS. The results revealed that significantly higher contamination was detected in the samples from large WWTPs (population equivalent > 50,000; p-value less then 0.05). Performance of the Fe3O4/La(OH)3 nanocomposite was evaluated in terms of sorption kinetics, sorption isotherms, different solution pH values, competing ions, and regenerative ability. The Fe3O4/La(OH)3 nanocomposite showed a nanosphere-like morphology with 97% magnetic separation efficiency, excellent phosphate removal capacity of 253.83 mg/g, 99% phosphate selectivity in the presence of chloride, nitrate, sulfate, fluoride, and calcium as competing ions and excellent reusability in ten cycles. Based on these findings, the Fe3O4/La(OH)3 nanocomposite was used to remove phosphate from river water. It was found that, in 60 min, a 0.1 g/L dosage of the nanocomposite was able to reduce the phosphate in the water from 0.087 mg/L to 0.002 mg/L. Microbial community analysis using 16S rRNA indicated that while phylum Proteobacteria dominated the bacterial composition throughout the 60 days, Actinobacteria increased with time. Many genera from Proteobacteria and Actinobacteria can synthesize a wide array of enzymes, indicating that these phyla are likely to play an important role in 62 FTS biotransformation. Higher biotransformation of 62 FTS was observed in leachate-added microcosms (∼21%), compared to DI water microcosm (∼14%), likely reflecting the substrate dependency of 62 FTS biotransformation. Substrate limiting conditions in DI water microcosm resulted in slightly greater formation of ∑(C4 - C6) PFCAs (∼14 mol%), compared with leachate added microcosms (10-13 mol%). The findings suggest that dilution of landfill leachate, (e.g., during wet seasons), likely results in reduced 62 FTS biotransformation and increased PFCAs formation compared to dry conditions.   • Obesity is known to affect thyroid functions. Recently, waist-height ratio (WHtR) has been considered as a useful marker of subclinical hypothyroidism in obese cases, but its relation with thyroid autoimmunity still remains unclear. We evaluated the effect of body fat mass, WHtR, and metabolic parameters on thyroid autoantibody levels in children with obesity.

    This is a cross-sectional study carried out with an obese (n=56, M/F29/26) and a healthy group (n=38, M/F19/19). All subjects underwent anthropometric measurements, laboratory investigations for thyroid function tests, thyroid peroxidase (TPO-ab) and thyroglobulin antibodies (Tg-ab), transaminase, blood glucose, insulin levels, and lipids after overnight fasting. Fat mass was estimated by multiple frequency bioimpedance analysis in the obese group, which was further divided into two subgroups according to the median of WHtR. All parameters were compared between the groups/subgroups.

    Prospective pilot study of paediatric patients treated with growth hormone, prescribed by the Paediatric Endocrinology Department, between February 2013 and September 2017. In all these patients, a fundus examination was performed before starting treatment and 3-4 months later.

    289 patients were included 244 had a growth hormone deficiency, 36 had short stature associated with small for gestational age, six had a mutation in the SHOX gene and three had Prader-Willi Syndrome. Five patients developed papilledema, all were asymptomatic and had a growth hormone deficiency one had a craniopharyngioma, two a polymalformative syndrome associated with hypothalamic-pituitary axis anomalies, one patient a non-specified genetic disease with hippocampal inversion and one patient with normal magnetic resonance imaging who had developed a primary pseudotumor cerebri syndrome years before.

In our series, at risk patients had growth hormone deficiency and hypothalamic-pituitary anatomic anomalies or genetic or chromosome diseases.     Active commuting to work is one way people can be physically active and can be influenced by state-level initiatives. The American Community Survey (ACS) is a potential data source to evaluate changes in active commuting at the state level, but state-level changes have not been well documented. We examined state-level changes in estimates of walking, biking, and taking transit to work (combined and separately) among employed persons between 2006 and 2017.

    Data were from the ACS, a nationally representative annual household survey from the US Census Bureau. We estimated state-level prevalence of walking, bicycling, or taking transit to work (separately and in combination) in 2006 and 2017 and tested differences by year.

    The prevalence of active commuting to work varied widely among states (2017 range 1.7% in Alabama and Mississippi to 35.0% in New York). Changes from 2006 to 2017 also varied, with 8 states exhibiting a significant increase (Massachusetts [2.7 percentage points], New York [2.2], Hawaii [1.6], Illinois [1.3], Washington [1.3], New Jersey [1.2], Virginia [0.9], and Michigan [0.4]), and 12 exhibiting a significant decrease (South Dakota [-1.9], Idaho [-1.3], New Hampshire [-1.3], Wisconsin [-1.1], Maryland [-1.0], Nevada [-0.9], Ohio [-0.8], Mississippi [-0.6], Texas [-0.6], Florida [-0.5], Georgia [-0.4], and Indiana [-0.4]). The contributions of walking, bicycling, and taking transit also varied by state.

    Our aim was to evaluate the implementation of a widely available, before-school, physical activity program in a low-resource, racially/ethnically and socioeconomically diverse, urban school setting to identify adaptations needed for successful implementation.

    We used a collaborative effort with stakeholders to implement the Build Our Kids' Success (BOKS) program in 3 schools in Revere, Massachusetts. Program structure followed a preexisting curriculum, including 60-minute sessions, 3 mornings per week, over 2 sessions (spring and fall 2018). Programs had a capacity of 40 students per school per session and the ability to adapt as needed.

    We used a mixed-methods approach, guided by the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework. RE-AIM domains were assessed by use of baseline and follow-up student measures, parent interviews, and program administrative records.

    From a district of 11 schools, 3 schools (2 elementary, 1 middle) implemented the BOKS program. Program enrollment reached 82% capacity (188 of 230 potential participants).

Growth hormone treatment is a cause of pseudotumor cerebri syndrome. In our series, at risk patients had growth hormone deficiency and hypothalamic-pituitary anatomic anomalies or genetic or chromosome diseases. Fundus examination should be systematically screened in all patients in this at-risk group irrespective of the presence or not of symptoms. Active commuting to work is one way people can be physically active and can be influenced by state-level initiatives. The American Community Survey (ACS) is a potential data source to evaluate changes in active commuting at the state level, but state-level changes have not been well documented. We examined state-level changes in estimates of walking, biking, and taking transit to work (combined and separately) among employed persons between 2006 and 2017. Data were from the ACS, a nationally representative annual household survey from the US Census Bureau. We estimated state-level prevalence of walking, bicycling, or taking transit to work (separately and in combination) in 2006 and 2017 and tested differences by year. The prevalence of active commuting to work varied widely among states (2017 range 1.7% in Alabama and Mississippi to 35.0% in New York). Changes from 2006 to 2017 also varied, with 8 states exhibiting a significant increase (Massachusetts [2.7 percentage points], New York [2.2], Hawaii [1.6], Illinois [1.3], Washington [1.3], New Jersey [1.2], Virginia [0.9], and Michigan [0.4]), and 12 exhibiting a significant decrease (South Dakota [-1.9], Idaho [-1.3], New Hampshire [-1.3], Wisconsin [-1.1], Maryland [-1.0], Nevada [-0.9], Ohio [-0.8], Mississippi [-0.6], Texas [-0.6], Florida [-0.5], Georgia [-0.4], and Indiana [-0.4]). Our aim was to evaluate the implementation of a widely available, before-school, physical activity program in a low-resource, racially/ethnically and socioeconomically diverse, urban school setting to identify adaptations needed for successful implementation. We used a collaborative effort with stakeholders to implement the Build Our Kids' Success (BOKS) program in 3 schools in Revere, Massachusetts. Program structure followed a preexisting curriculum, including 60-minute sessions, 3 mornings per week, over 2 sessions (spring and fall 2018). Programs had a capacity of 40 students per school per session and the ability to adapt as needed. We used a mixed-methods approach, guided by the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework. RE-AIM domains were assessed by use of baseline and follow-up student measures, parent interviews, and program administrative records. From a district of 11 schools, 3 schools (2 elementary, 1 middle) implemented the BOKS program. Program enrollment reached 82% capacity (188 of 230 potential participants).

Multivariate analysis was utilized to identify significant risk factors for EAD. A nomogram was constructed according to the contributions of the risk factors. The predictive values were determined by discrimination and calibration methods. A cohort of 30 patients was recruited to validate this predictive model. Four independent risk factors, including donor age, intraoperative blood loss, preoperative alanine aminotransferase (ALT), and reperfusion total bilirubin, were identified and used to build the nomogram. The c-statistics of the primary cohort and the validation group were 0.846 and 0.767, respectively. The calibration curves for the probability of EAD presented an acceptable agreement between the prediction by the nomogram and the actual incidence. In conclusion, the study developed a new nomogram for predicting the risk of EAD following living donor liver transplantation. Fifty-nine PRASEF and 15 Penicillin-Susceptible, Ampicillin-Susceptible E. faecalis (PSASEF) clinical nonrepetitive isolates were evaluated. The effectiveness of five beta-lactams (ampicillin, amoxicillin, imipenem, penicillin, and piperacillin) was tested. All antimicrobial susceptibility tests were performed and interpreted according to the Clinical and Laboratory Standards Institute guidelines. Interpretative discrepancies, such as essential agreement, categorical agreement, and errors, were assessed. The acceptability was ≥ 90% for both categorical agreement and essential agreement. Etest proved to be an accurate method for testing beta-lactam susceptibilities of the emerging PRASEF isolates, disk diffusion presented poor performance, particularly for imipenem and piperacillin.
    Closure of large skin-soft tissue defects following soft tissue sarcoma (STS) resection has been a great challenge. The objective of this study was to evaluate the effectiveness of a novel, simple, and cheap skin-stretching device (bidirectional regulation-hook skin closure system, BHS) for closing large skin-soft tissue defects resulting from the removal of STS and the complications associated with the use of the BHS.

    From January 2017 to September 2018, 25 patients with STS underwent BHS therapy after tumor resection. BHS was used for two main clinical applications securing wound closure after high-tension suture closure and delayed wound closure. We described a detailed reconstruction procedure regarding this therapy. Wound closure and complications associated with BHS therapy were recorded. We also analyzed tumor recurrence and metastases.

    All patients were observed for 16-36 months with an average follow-up of 25.6 months. During the follow-up period, no significant functional restriction was observed and the final scar was aesthetically acceptable. Superficial wound infection occurred in six patients, wound edge ischemia in two patients, and small skin tears in two patients. Two patients developed pulmonary metastasis, two patients had a local recurrence, and one patient died of pulmonary metastasis.

    BHS therapy can effectively close large skin-soft tissue defects following STS resection and obtain acceptable functional results, without severe complications. However, larger studies are required to further evaluate the effectiveness, indications, and complications of BHS therapy.

    The study was conducted at the Medical University of Vienna. Attitudes of 2nd year undergraduate medical students towards traditional assessment formats, and their relation to students' learning, and students' attitude towards objectivity, was inquired using a self-developed questionnaire.

    459 students participated in this study. MCQs examinations were the most preferred assessment format and were chosen as the most objective format. Most students agreed that oral examinations are more appropriate for achieving long-term knowledge. Female students showed higher preference for oral examinations than male students. Students would prefer an assessment mix of 41.8% MCQs, 24.0% oral examinations, and 9.5% SAQs, if they were free to choose the assessment tools.

    Previously, it has been shown that obesity is a risk factor for recovery, regeneration, and tissue repair after blunt trauma and can affect the rate of muscle recovery and collagen deposition after trauma. To date, lung tissue regeneration and extracellular matrix regulation in obese **** after injury has not been investigated in detail yet.

    This study uses an established blunt thorax trauma model to analyze morphological changes and alterations on gene and protein level in lean or obese (diet-induced obesity for 16 ± 1 week) male C57BL/6 J **** at various time-points after trauma induction (1 h, 6 h, 24 h, 72 h and 192 h).

    Morphological analysis after injury showed lung parenchyma damage at early time-points in both lean and obese ****. At later time-points a better regenerative capacity of lean **** was observed, since obese animals still exhibited alveoli collapse, wall thickness as well as remaining filled alveoli structures. Although lean **** showed significantly increased collagen and fibronectin gene levels, analysis of collagen deposition showed no difference based on colorimetric quantification of collagen and visual assessment of Sirius red staining.
