The new definition for metabolic associated fatty liver disease (MAFLD), formerly named non-alcoholic fatty liver disease (NAFLD), would undoubtedly have significant influence on diagnosis, epidemiology, and new drug research. We investigated the prevalence and risk factors of MAFLD among people living with HIV (PLWH).
In this cross-sectional study, transient elastography was performed in PLWH without significant alcohol intake and hepatitis B virus and hepatitis C virus infection. NAFLD was diagnosed as controlled attenuation parameter (CAP)≥248dB/m by transient elastography, and MAFLD was defined according to the 2020 international consensus. Advanced fibrosis was defined as liver stiffness measurement (LSM)≥10kPa.
Among the 361 PLWH enrolled, the prevalence of NAFLD and MAFLD were 37.67% and 34.90%, respectively. Compared with the non-MAFLD group, the prevalence of elevated alanine aminotransferase (ALT) level (44.44% vs 16.17%, P<0.001) and advanced fibrosis (19.05% vs 2.55%, P<0.001) were significantly higher in the MAFLD group. A positive correlation between LSM and CAP values was found in the MAFLD group (r
=0.350, P<0.001) but not in the non-MAFLD group. In multivariate analysis, independent risk predictors for MAFLD were higher ALT level (odds ratio [OR] 1.015, 95% confidence interval [CI] 1.003-1.028, P=0.018), higher uric acid (OR 1.005, 95% CI 1.002-1.009, P=0.003), higher total cholesterol (OR 1.406, 95% CI 1.029-1.921, P=0.032), and greater waist-height ratio (OR 1.291, 95% CI 1.196-1.393, P<0.001).
A third of PLWH had MAFLD, which was highly accordant with the prevalence of NAFLD. Routine screening for MAFLD is necessary in PLWH.

Here, we pursue the idea that different individual factors contribute differently to the amount of explicit and implicit intermanual transfer. In particular, we tested a group of judo athletes who show enhanced right-hemispheric involvement in motor control and a group of equally trained athletes. After adaptation to a 60° visual rotation, we estimated awareness of the perturbation and transfer to the untrained, non-dominant left hand in two experiments. We measured the total amount of intermanual transfer (explicit plus implicit) by telling the participants to repeat what was learned during adaptation, and the amount of implicit transfer by instructing the participants to refrain from using what was learned and to perform movements as during baseline instead. We found no difference between the total intermanual transfer of judokas and running experts, with mean absolute transfer values of 42.4° and 47.0°. Implicit intermanual transfer was very limited, but larger in judokas than in general sports athletes, with mean values of 5.2° and 1.6°. A multiple linear regression analysis further revealed that total intermanual transfer, which mainly represents the explicit transfer, is related to awareness of the perturbation, while implicit intermanual transfer can be predicted by judo training, amount of total training, speed of adaptation, and handedness scores. The findings suggest that neuronal mechanisms such as hemispheric interactions and functional specialization underlying intermanual transfer of motor learning may be applied according to individual predisposition.

In this review, we will elucidate the development of machine learning methods, especially the visual representation mechanism in deep learning on recognition tasks. Various AI-image analysis applications in endoscopy, radiology, and pathology are covered in gastroenterology and hepatology and reveal the enormous potentials for AI in assisting diagnosis, prognosis, and treatment. We also discuss the promises as well as pitfalls for AI in medical image analysis and pointing out future research directions.Kallmann syndrome (KS) is a rare human genetic disorder characterized by hypogonadotropic hypogonadism with the reduction or absence of olfactory sense. Mutations in multiple genes, including chemokine prokineticin-2 (PROK2), are considered to contribute to the abnormal migration of gonadotropin-releasing hormone neurons in the embryonic stage. However, the mechanisms of the different inheritance modes of KS have not been comprehensively determined. In this article, we present the case of one KS patient with the same mutation in PROK2 (c.223-4C>A) as his mother. RNA sequencing analysis of his leukocytes showed a new transcript of PROK2, which contained a partial intron (192 bp) compared to those of his parents. Furthermore, we observed that hsa-miR-3195 was expressed at low levels in his and his father's sera compared to his mother's. Unexpectedly, hsa-miR-3195 was also identified to specifically target the 192 bp intron of the aberrant PROK2 transcript of this patient. We determined that high expression of hsa-miR-3195 could efficiently target aberrant PROK2 and stabilize the normal function of PROK2 in vitro, which provided a probable explanation for the different phenotypes of the patient and his mother with the same genotype.

Methods A cross-sectional study on pediatric emergency department consultations before and during the current COVID-19 pandemic (March-May 2019 vs. March-May 2020) was performed in two hospitals in the Campania region (Southern Italy) [i.e., Salerno University Hospital (Salerno) and Pediatric Regional Referral Emergency Hub "AORN Santobono-Pausillipon" (Naples)]. Results 29,368 consecutive ED pediatric patients (13,430 females; mean age ± SD = 5.4 ± 4.7 years) were seen in March-May 2019 and 9,133 (4,494 females; mean age ± SD = 5.9 ± 4.2 years) in March-May 2020. Resuscitation/emergency and urgent care pediatric ED consultations were 1,388 (4.7%, 95% CI 4.5-4.9) in the 2019 trimester, while they were 648 (7.1%, 95% CI 6.6-7.6) in the 2020 trimester (p less then 0.01). Mean pediatric ED daily consultations were 326.3 (95% CI 299.9-352.7) in the considered period of 2019 and 101.4 (95%CI 77.9-124.9) in the same period of 2020 (p less then 0.001). COVID-19 nasal swabs were performed for 385 children; of those, six resulted positive and four of them were hospitalized. Conclusions This work provides a unique snapshot of the pediatric EDs demands in the era of COVID-19. We witnessed a significant reduction of non-urgent health care demands during the pandemic but an increase of more severe urgent cases. The COVID-19 pandemic and the following lockdown unveiled the inappropriateness of the majority of pediatric ED consultations. Nevertheless, the current scenario highlighted the need for appropriate and timely clinical evaluations in the pediatric primary care to tackle late and more severe diagnoses in EDs.

At autopsy, hemorrhagic gastritis and cherry-red discoloration of the chest muscles were observed. Forensic toxicology showed cyanide in the blood and tissues. Cyanide poisoning could appear in different forms, and like our case, the symptoms can last for several days with nonspecific symptoms.

The present study investigated the erosive potential of children's mouthrinses on glass ionomer cement (GIC) samples after simulated toothbrushing.
Forty round-shaped samples of GIC were divided into 3 groups G1- cetylpyridinium chloride, G2- xylitol and triclosan and G3-Malva sylvestris and xylitol and G4-distilled water as a control group. Prior to the main tests, the samples were submitted to the surface roughness measurement (Ra) and weight analysis (W). Afterward, they were brushed twice day (2× / day) for 15 days and immersed in mouthrinses after the last daily brushing. The final surface roughness (R2) and weight (W2) were determined after completing the tooth brushing-mouth rinsing cycles and the real increase in roughness (ΔRa) and real weight loss (ΔW) were calculated. In addition, stereoscopic images taken at 30× magnification. The data was analyzed by one-way ANOVA and Tukey-test post hoc tests for intergroup comparison and the T-test for dependent samples (α = 0.05).

The current investigation evaluated parameters leading to the utilization of pulpectomy versus extraction for treatment of nonvital primary second molars.
This retrospective chart review identified patients up to 8-years of age with primary second molars treated by pulpectomy or extraction. Patients in the extraction group were age and gender-matched to the pulpectomy group. Demographic, clinical, radiographic and behavioral data were extracted for comparison. Chi-square, Fisher and T-test were performed for statistical analysis.
There were 23 patients in each group, with a mean age of 5 years (ranging 3-8 years, ±1.5 for pulpectomy and ±1.3 for extraction). Significantly more pulpectomies were performed in the mandible (p=0.002), specifically on the left side (p=0.0035). Internal and external root resorption were significantly higher in the extraction group (p=0.033 and p=0.007 respectively). Restorability was significantly lower in the extraction group (p<0.0001). Pre-procedural pain was reported by 76.5 percent of all patients, but pharmacologically treated in 15.2 percent. Nitrous oxide was administered to 73.9 percent of patients for behavior guidance.
Pathologic root resorption and non-restorability were significantly higher in the extraction group. Behavior and pathologic bone resorption did not influence treatment choice. A higher proportion of children reported pre-treatment pain and needed adjunctive behavior guidance than children who did not have pre-treatment pain or did not need adjunctive behavior guidance.

Whitening toothpastes are widely used. Hence, it is important to understand their effect on the surface properties of restorative materials. To evaluate the effect of three over-the-counter whitening toothpastes and toothbrushing simulation on microhardness of three restorative materials.
Forty cylindrical (10×2mm) specimens were prepared from each restorative material and randomly assigned into four groups/10 each according to the whitening toothpastes used and distilled water (control). All specimens were measured for microhardness (Baseline-T1). The specimens were brushed with a soft brush using an in vitro tooth-brushing simulator with the assigned whitening toothpaste using the same setting for brushing cycles/load for all groups. Specimens were then measured for microhardness (T2) similar to baseline.
The highest (mean±SD) microhardness after application of the whitening toothpastes and brushing was recorded for Intense White and Filtek Z250 XT (127.6±1.8), followed by Optic White and Fuji ll LC (toothpastes and brushing was lowest compared to other toothpastes and control.The aim of this study was to evaluate synergy and inhibitory effects of xylitol and erythritol on Streptococcus mutans and Streptococcus sobrinus growth and biomass production on a polystyrene plastic surface. Study design; S. mutans and sobrinus strains (American Type Culture Collection reference strains 31341, 35668, 25175, sobrinus 33478) were cultivated in media (Todd Hewitt Broth with 1% sucrose or heart-brain infusion broth with 1% sucrose) at differing concentrations of xylitol or erythritol in microtiter assay plates incubated for 48 hours. Bacterial growth was quantified and measured by optical density using a microplate reader. Experiments assessing synergy and biofilm growth were carried out also using microdilution assays. All four strains were inhibited by 30% (w/v) xylitol, and 15% erythritol at 150mg/ml erythritol, 2/4 strains had reduced growth; at 270mg/ml, 4/4 strains were inhibited. Bactericidal effects were not observed at any polyol concentration. Combinations of both polyols in a checker board array were used to determine if there were any benefits of polyol combinations. Results The combination studies yielded mixed outcomes with indifference in growth for strains 68 and 78, potential additive effect for strain 75 and possible antagonism for strain 41. Assessment of biomass formation and polyol interference were also performed post ****assessment. Physical activity (PA) behaviors during preschool settings can influence the health and development of children with developmental disabilities (DD). There is a need for a direct observation system that simultaneously assesses PA and preschool environmental contexts.
The purpose of this study was to develop an observation instrument for measuring PA and related contextual factors of preschoolers with DD, and to establish content validity and reliability.
Content validity was established through consultation with experts, informal observations in inclusive and special education preschools, and literature reviews. Categories and codes were identified and modified from existing observational systems for young children. Reliability was assessed in a convenience sample of preschool children with DD using a cross-sectional design. Data were collected using a momentary time sampling system (5-sec observe, 25-sec record) following a focal child. Inter-rater reliability was assessed during 20% of the observation simultaneous measurement of specific types and contexts of PA behaviors of preschoolers with DD and will be useful for describing PA and informing future interventions.
Transitions of care (TOC) are critical times for patients, and if not conducted effectively, can lead to adverse events, preventable treatment, and costly readmissions. TOC programs are implemented to improve patient care, prevent hospital readmissions, and lower financial penalties associated with readmissions. Medicare's hospital readmission reduction program (HRRP), a value-based care model, targets specific conditions and procedures. Although many TOC programs focus on HRRP conditions, there are few studies looking at prevalence of disease states seen in TOC programs embedded within an employer-based insurance program in which financial ramifications exist for all hospitalizations. There is a need to better understand the potential missed opportunities for employer-based programs solely addressing HRRP conditions.
To quantify admission diagnoses in a nontargeted employer-based TOC program.
We conducted a descriptive study of patients enrolled in a TOC program conducted by a self-insured grocery stor to be confirmed in future studies.
Most of the patients seen by an employer-based TOC program were for conditions not targeted by the HRRP. By including patients beyond those covered by the HRRP, such TOC programs may realize greater improvements in financial and clinical outcomes. These benefits, however, need to be confirmed in future studies.
Considering the amount of national and international scientific evidence regarding the use of anticoagulants in the periprocedural prevention of thromboembolism, the decision-making process when prescribing anticoagulants is complex. Due to frequent prescriptions for inpatients, the topic is of high significance. This decision-making process in our hospital is the main focus of the study.
In a standardised survey, a target group of physicians was asked about their decision-making process with regard to the prescription of anticoagulants in the periprocedural period. For this study a questionnaire was developed consisting of 12 survey questions. Data from 130 participants was collected and analysed.
Generally, there was a high level of agreement on the importance of venous thromboembolism prophylaxis. Significant differences exist between medical specialities. Physical activity (PA) behaviors during preschool settings can influence the health and development of children with developmental disabilities (DD). There is a need for a direct observation system that simultaneously assesses PA and preschool environmental contexts. The purpose of this study was to develop an observation instrument for measuring PA and related contextual factors of preschoolers with DD, and to establish content validity and reliability. Content validity was established through consultation with experts, informal observations in inclusive and special education preschools, and literature reviews. Categories and codes were identified and modified from existing observational systems for young children. Reliability was assessed in a convenience sample of preschool children with DD using a cross-sectional design. Data were collected using a momentary time sampling system (5-sec observe, 25-sec record) following a focal child. A total of 173 AF patients with an LA-LGE area on CMR imaging were enrolled. The clinical parameters, including invasively measured LA pressure, were compared between the patients with extensive LA-LGE (E-LGE, LGE extent ≥ 20%, n = 78) and those with small LA-LGE (S-LGE, LGE extent less then 20%, n = 95). The E-LGE group had higher peak LA pressures than the S-LGE group (23 versus 19 mmHg, p less then 0.001). The E-LGE group had more patients with non-paroxysmal AF (non-PAF) (51% vs. 34%), heart failure (9% vs. 0%), and higher NT pro-B-type natriuretic peptide (472 vs. 265 pg/ml) (all p less then 0.05). LA pressure ≥ 21 mmHg was an independent predictor of E-LGE (OR = 2.218; p = 0.019). In the paroxysmal AF (PAF) subgroup, freedom from atrial arrhythmia after catheter ablation was not different (81% vs 86%, log-rank p = 0.529). However, in the non-PAF subgroup, it was significantly higher in the S-LGE group than in the E-LGE group (81% vs 55%, log-rank p = 0.014). Increased LA pressure was related to the LA-LGE extent. Statistical analysis of bacterial load and quorum-sensing molecules showed a strong correlation between these biomarkers in infected lungs. This was accompanied by noticeable changes in the consistency of lung homogenates with increasing infection severity. Furthermore, viscoelastic properties of the lung homogenates strongly correlated with bacterial load and quorum sensing molecules. Heat production (HP) represents a major energy cost in lactating dairy cows. Better understanding of factors that affect HP will improve our understanding of energy metabolism. Our objective was to derive models to explain variation in HP of lactating Jersey cows. Individual animal-period data from 9 studies (n = 293) were used. The data set included cows with a wide range (min to max) in days in milk (44-410) and milk yield (7.8-43.0 kg/d). Diets included corn silage as the predominate forage source, but diets varied (min to max on DM basis) in crude protein (CP; 15.2-19.5%), neutral detergent fiber (NDF; 35.5-43.0%), starch (16.2-31.1%), and crude fat (2.2 to 6.4%) contents. Average HP was (mean ± standard deviation) 22.1 ± 2.86 Mcal/d, or 28.1 ± 3.70% of gross energy intake. The objective of this paper was to review literature in 5 areas of human resource management of a dairy farm (1) professional accreditation and professional development, (2) extension activities, (3) the role of the advisor, (4) standard operating procedures, and (5) employee training. Although there has been an increase in research in this area in recent years, this review identified numerous areas for future research, including the relationships between farmers and their advisors and employees, and the role of standard operating procedures on dairy farms. In addition, we suggest that future studies could benefit from increased use of participatory research methods.One aspect of a successful dairy business is attracting and retaining skilled staff. The ability to do this is influenced by workplace attractiveness, one aspect of which is worker comfort and safety. Working with heavy machinery and animals makes dairy farming a dangerous occupation relative to many other sectors. Milking is the most time-consuming task on pasture-based dairy farms, and the parlor is the logical site of a number of injuries, although little is known about their causes or their relationship to parlor design and management. To investigate worker safety further, we conducted an on-farm survey on New Zealand dairy farms between December 2018 and March 2019. The survey had 3 objectives to identify common causes of injuries relating to milking; to describe variations in milking parlor design and management practices on dairy farms; and to identify management and parlor factors that contribute to milking injuries. Analysis of accident compensation data from the state-run accident insurer for a 5-yrduce fatigue in spring may be beneficial; however, the number of hours worked or number of consecutive days worked were not associated with the number of milking injuries. Those aged 20-29 yr had more injuries relative to their proportion of the workforce, indicating that more training may be beneficial (e.g., setting ergonomic milking routines). Female workers were at greater risk of injury than males; this finding may have been related to the height of the animal relative to the height of the milker.This study aimed to investigate the heat stability of dispersions from reconstituted reduced-calcium milk protein concentrate (RCMPC) with 80% protein or more. The tested RCMPC powders were produced from skim milk subjected to CO2 treatment before and during the process of ultrafiltration. The CO2 injection was controlled to obtain 0 (control, no CO2 injection), 20, 30, and 40% reduction in calcium levels in the RCMPC powders. The RCMPC powders were reconstituted to 10% (wt/wt) protein in deionized water. These dispersions were tested for heat stability in a rocking oil bath at 140°C at unadjusted, 6.5, 6.7, 6.9, and 7.1 pH. Calcium ion activity (CIA) and ionic strength measurements were carried out using a Ca ion-selective electrode and conductivity meter. Unadjusted pH of the dispersions varied from 6.8 in control to 5.96 in 40% RCMPC dispersions. The CIA of unadjusted dispersions ranged from 1.31 mM in control to 2.83 mM in 40% RCMPC. Heat stability, expressed as heat coagulation time (HCT) of unadjusted dispersions decreased as the level of Ca removal in powders increased (from 13.81 min in control to 0.46 min in 40% RCMPC) and was negatively correlated with the CIA of the dispersions. For control RCMPC dispersions, the minimum and maximum heat stability were observed at dispersion pH of 6.5 and 6.9, respectively, followed by a decrease at pH 7.1 (CIA was the lowest). Dispersions from 40% RCMPC and pH 7.1 had the maximum HCT of 30.94 min among all RCMPC dispersions at all pH values. Heat production (HP) represents a major energy cost in lactating dairy cows. Better understanding of factors that affect HP will improve our understanding of energy metabolism. Our objective was to derive models to explain variation in HP of lactating Jersey cows. Individual animal-period data from 9 studies (n = 293) were used. The data set included cows with a wide range (min to max) in days in milk (44-410) and milk yield (7.8-43.0 kg/d). Diets included corn silage as the predominate forage source, but diets varied (min to max on DM basis) in crude protein (CP; 15.2-19.5%), neutral detergent fiber (NDF; 35.5-43.0%), starch (16.2-31.1%), and crude fat (2.2 to 6.4%) contents. Average HP was (mean ± standard deviation) 22.1 ± 2.86 Mcal/d, or 28.1 ± 3.70% of gross energy intake. Eight models were fit to explain variation in HP (1) dry matter intake (DMI; INT); (2) milk fat, protein, and lactose yield (MILKCOMP); (3) INT and milk yield (INT+MY); (4) INT and MILKCOMP/DMI (INT+MILKCOMP); (5) mass of digested NDF,lk protein and protein digestion suggest that digestion and metabolism of protein and synthesis of milk protein contribute substantially to HP of a dairy cow.Dairy farms are increasing in size and moving from family to external labor. As such, dairy farmers now have the responsibilities of a human resource manager in addition to caring for their animals. The objective of this paper was to review literature in 5 areas of human resource management of a dairy farm (1) professional accreditation and professional development, (2) extension activities, (3) the role of the advisor, (4) standard operating procedures, and (5) employee training. Although there has been an increase in research in this area in recent years, this review identified numerous areas for future research, including the relationships between farmers and their advisors and employees, and the role of standard operating procedures on dairy farms. In addition, we suggest that future studies could benefit from increased use of participatory research methods.One aspect of a successful dairy business is attracting and retaining skilled staff. The ability to do this is influenced by workplace attractiveness, one aspect of which is worker comfort and safety. Working with heavy machinery and animals makes dairy farming a dangerous occupation relative to many other sectors. Milking is the most time-consuming task on pasture-based dairy farms, and the parlor is the logical site of a number of injuries, although little is known about their causes or their relationship to parlor design and management. To investigate worker safety further, we conducted an on-farm survey on New Zealand dairy farms between December 2018 and March 2019. The survey had 3 objectives to identify common causes of injuries relating to milking; to describe variations in milking parlor design and management practices on dairy farms; and to identify management and parlor factors that contribute to milking injuries. Analysis of accident compensation data from the state-run accident insurer for a 5-yrduce fatigue in spring may be beneficial; however, the number of hours worked or number of consecutive days worked were not associated with the number of milking injuries. Those aged 20-29 yr had more injuries relative to their proportion of the workforce, indicating that more training may be beneficial (e.g., setting ergonomic milking routines). Female workers were at greater risk of injury than males; this finding may have been related to the height of the animal relative to the height of the milker.This study aimed to investigate the heat stability of dispersions from reconstituted reduced-calcium milk protein concentrate (RCMPC) with 80% protein or more. The tested RCMPC powders were produced from skim milk subjected to CO2 treatment before and during the process of ultrafiltration. The CO2 injection was controlled to obtain 0 (control, no CO2 injection), 20, 30, and 40% reduction in calcium levels in the RCMPC powders. The RCMPC powders were reconstituted to 10% (wt/wt) protein in deionized water. These dispersions were tested for heat stability in a rocking oil bath at 140°C at unadjusted, 6.5, 6.7, 6.9, and 7.1 pH. Calcium ion activity (CIA) and ionic strength measurements were carried out using a Ca ion-selective electrode and conductivity meter. Unadjusted pH of the dispersions varied from 6.8 in control to 5.96 in 40% RCMPC dispersions. The CIA of unadjusted dispersions ranged from 1.31 mM in control to 2.83 mM in 40% RCMPC. Heat stability, expressed as heat coagulation time (HCT) of unadjusted dispersions decreased as the level of Ca removal in powders increased (from 13.81 min in control to 0.46 min in 40% RCMPC) and was negatively correlated with the CIA of the dispersions. For control RCMPC dispersions, the minimum and maximum heat stability were observed at dispersion pH of 6.5 and 6.9, respectively, followed by a decrease at pH 7.1 (CIA was the lowest). Dispersions from 40% RCMPC and pH 7.1 had the maximum HCT of 30.94 min among all RCMPC dispersions at all pH values. We evaluate our approach via simulation and apply it to QTL from two MPPs the Collaborative Cross (CC) and the Drosophila Synthetic Population Resource (DSPR). We find that, although posterior inference of the exact allelic series is often uncertain, we are able to distinguish biallelic QTL from more complex multiallelic cases. Additionally, our allele-based approach improves haplotype effect estimation when the true number of functional alleles is small. Our method, Tree-Based Inference of Multiallelism via Bayesian Regression (TIMBR), provides new insight into the genetic architecture of QTL in MPPs.Mangrove ecosystems provide important ecological benefits and ecosystem services, including carbon storage and coastline stabilization, but they also suffer great anthropogenic pressures. Microorganisms associated with mangrove sediments and the rhizosphere play key roles in this ecosystem and make essential contributions to its productivity and carbon budget. Understanding this nexus and moving from descriptive studies of microbial taxonomy to hypothesis-driven field and lab studies will facilitate a mechanistic understanding of mangrove ecosystem interaction webs and open opportunities for microorganism-mediated approaches to mangrove protection and rehabilitation. Such an effort calls for a multidisciplinary and collaborative approach, involving chemists, ecologists, evolutionary biologists, microbiologists, oceanographers, plant scientists, conservation biologists, and stakeholders, and it requires standardized methods to support reproducible experiments. We further identified that the β-diversity of the microbial community varied with Ta and showed diverse responses during the 3 ccates that both gut microbiota and their host were more adaptive after repeated acclimations. The method makes Tn-seq more user-friendly and accessible to laboratories with more-limited bioinformatic resources. Finally, we created a saturated transposon-mutant library in Mycobacterium abscessus and demonstrated the usefulness of the method in analysis of genes involved in colony morphology, as well as in analysis of the whole Tn-mutant library, with identification of over 8,000 unique mutants.IMPORTANCE Transposon insertion sequencing is a powerful tool, but many researchers are discouraged by the apparent technical complexity of preparing the genomic library for deep sequencing and by the complicated computational analysis needed for insertion site identification. Our proposed method makes the preparation of the library easy and straightforward, relying on well-known molecular biology techniques. In addition, the results obtained from the deep sequencing are easily analyzed in terms of transposon insertion site identification, placing library preparation and analysis within the reach of more researchers in the microbiology community, including those with less computational and bioinformatic resources and experience. The aim of this study was to assess the variability and medium-term AMR trends within the mostly healthy human population of a single city. We monitored over 36 months (November 2015 to November 2018) the AMR level in the city of Copenhagen, Denmark, by taking bi-weekly sewage samples from the inlets of the three main water treatment plants, extracting the DNA, performing metagenomic sequencing, and read-mapping against a database of known AMR genes. We found that the AMR level was surprisingly stable with no periodic variability and no signs of drift over the measured period. We found, however, that the seemingly random variations at each site correlate in time with each other, suggesting that the variations we see are due to real environmental changes in the occurrence of AMR.IMPORTANCE The Copenhagen sewage resistome is surprisingly stable in time. The implication is that, at least for cities that are comparable to Copenhagen in terms of sewer infrastructure, few or even single samples provide a robust picture of the resistome within a city.Pretargeted radioimmunotherapy (PRIT) has been investigated as a multi-step approach to decrease relapse and toxicity for high-risk acute myeloid leukemia (AML). To overcome these limitations we designed anti-murine and anti-human CD45 bispecific antibody constructs using 30F11 and BC8 antibodies, respectively, combined with an anti-yttrium (Y)-DOTA single-chain variable fragment (C825) to capture a radiolabeled ligand. The bispecific construct targeting human CD45 (BC8-Fc-C825) had high uptake in leukemia HEL xenografts [7.8 ± 0.02% percent injected dose/gram of tissue (% ID/g)]. Therapy studies showed that 70% of **** with HEL human xenografts treated with BC8-Fc-C825 followed by 44.4 MBq (1,200 μCi) of 90Y-DOTA-biotin survived at least 170 days after therapy, while all nontreated controls required euthanasia because of tumor progression by day 32.

Mycobacterium (M.) bovis can infect cats and is a demonstrated zoonosis. We describe an outbreak of M. bovis in pet cats across England and Scotland associated with feeding a commercial raw food diet. Forty-seven cats presented with (pyo)granulomatous lesions, lymphadenopathy, pulmonary and/or alimentary disease over a one-year period where M. bovis infection was suspected or definitively diagnosed, and the cats all consumed the same specific brand of commercial raw venison pet food. Infection with M. bovis genotype 10a was confirmed by culture and DNA typing of isolates in a small number of cases (n = 5); PCR was used in combination with or as an alternative to culture (n = 12) and/or infection with a Mycobacterium tuberculosis complex group organism was strongly suggested by positive responses to an interferon-gamma release assay (IGRA; n = 34). Asymptomatic at-risk cats were screened by IGRA, identifying a further 83 infected cats. The five culture-positive cases were distributed across areas of England and Scotland at low risk of endemic bovine tuberculosis. Investigations revealed affected cats were mainly indoor-only, and had been fed the same commercial raw food as at least part of their diet. This diet was recalled by the manufacturer due to failure of statutory meat inspection of the component venison. As far as possible, other sources of infection were explored and excluded, including wildlife contact, access to raw milk and living with people with active M. bovis infection. Four owners and one veterinary surgeon were found to have high likelihood of latent tuberculosis infection. One owner required treatment. Although it was not possible to conclusively demonstrate a zoonotic origin for these infections, neither was it possible to eliminate the possibility. A native renal biopsy diagnosis of IgAN is a predictor of graft loss, with a relative risk of 47% but it is difficult to predict graft survival and progressive allograft dysfunction in these patients. Deletion of complement factor H-related genes 1 and 3 (delCFHR3-1) has been associated with a decreased risk of developing IgAN on native kidneys, but the impact on the graft in IgAN-transplanted patients is unknown. We hypothesized that delCFHR3-1 is also associated with the processes that influence graft survival in transplant recipients with IgAN and tested whether cellular senescence is involved in mediating graft damage. We found that patients carrying two copies of CFHR1-3 had a worse outcome (P = .000321) and presented increased FHR1 deposits at glomerular and tubulointerstitial level associated with higher expression of the senescence marker p16INK4a (P = .001) and tubulointerstitial fibrosis (P = .005). Interestingly, FHR1 deposits were associated with increased complement activation as demonstrated by C5b-9 deposits. These data support both the role of FHR1 in mediating complement activation and tubular senescence, and suggest the possibility of genotyping delCFHR3-1 to predict graft survival in IgAN-transplanted patients.The combination of drought and heat affects forest ecosystems by deteriorating the health of trees, which can lead to large-scale die-offs with consequences on biodiversity, the carbon cycle, and wood production. It is thus crucial to understand how drought events affect tree health and which factors determine forest susceptibility and resilience. We analyze the response of Central European forests to the 2018 summer drought with 10 × 10 m satellite observations. By associating time-series statistics of the Normalized Difference Vegetation Index (NDVI) with visually classified observations of early wilting, we show that the drought led to early leaf-shedding across 21,500 ± 2,800 km2 , in particular in central and eastern Germany and in the Czech Republic. High temperatures and low precipitation, especially in August, mostly explained these large-scale patterns, with small- to medium-sized trees, steep slopes, and shallow soils being important regional risk factors. Early wilting revealed a lasting impact on forest productivity, with affected trees showing reduced greenness in the following spring. Our approach reliably detects early wilting at the resolution of large individual crowns and links it to key environmental drivers. PD-L1 immunohistochemistry is currently performed in patients with advanced non-small cell lung carcinoma (NSCLC) to identify responders to immune checkpoint inhibitors. Cell blocks from fine needle aspiration of NSCLC are frequently used for diagnostic purposes. The aims of the study are to analyze (a) the distribution of PD-L1 in cell blocks, in comparison to biopsies and surgical specimens; (b) the concordance of PD-L1 in specimens of the same patients.
PD-L1 analyses conducted in NSCLCs were retrieved. Cell blocks were prepared with the self-clotting method. PD-L1 was performed with Dako 22C3 on the Ventana BenchMark ULTRA platform. Results were divided by tumor proportion score (TPS) in 3 categories <1%; 1% to 49%; ≥50%.
A total of 483 samples from 456 patients was collected 120 cell blocks (24.8%), 307 endoscopic or transthoracic biopsies (63.6%), 56 surgical specimens (11.6%). TPS was <1% in 230 samples (47.8%), 1% to -49% in 136 (28.3%) and ≥ 50% in 115 (23.9%); in two samples material was insufficient.

Four owners and one veterinary surgeon were found to have high likelihood of latent tuberculosis infection. One owner required treatment. Deletion of complement factor H-related genes 1 and 3 (delCFHR3-1) has been associated with a decreased risk of developing IgAN on native kidneys, but the impact on the graft in IgAN-transplanted patients is unknown. We hypothesized that delCFHR3-1 is also associated with the processes that influence graft survival in transplant recipients with IgAN and tested whether cellular senescence is involved in mediating graft damage. We found that patients carrying two copies of CFHR1-3 had a worse outcome (P = .000321) and presented increased FHR1 deposits at glomerular and tubulointerstitial level associated with higher expression of the senescence marker p16INK4a (P = .001) and tubulointerstitial fibrosis (P = .005). Interestingly, FHR1 deposits were associated with increased complement activation as demonstrated by C5b-9 deposits. These data support both the role of FHR1 in mediating complement activation and tubular senescence, and suggest the possibility of genotyping delCFHR3-1 to predict graft survival in IgAN-transplanted patients.The combination of drought and heat affects forest ecosystems by deteriorating the health of trees, which can lead to large-scale die-offs with consequences on biodiversity, the carbon cycle, and wood production. It is thus crucial to understand how drought events affect tree health and which factors determine forest susceptibility and resilience. We analyze the response of Central European forests to the 2018 summer drought with 10 × 10 m satellite observations. By associating time-series statistics of the Normalized Difference Vegetation Index (NDVI) with visually classified observations of early wilting, we show that the drought led to early leaf-shedding across 21,500 ± 2,800 km2 , in particular in central and eastern Germany and in the Czech Republic. High temperatures and low precipitation, especially in August, mostly explained these large-scale patterns, with small- to medium-sized trees, steep slopes, and shallow soils being important regional risk factors. Early wilting revealed a lasting impact on forest productivity, with affected trees showing reduced greenness in the following spring. Our approach reliably detects early wilting at the resolution of large individual crowns and links it to key environmental drivers.
