Classical machine learning algorithms and a variety of feature engineering methods have become a mainstay in seizure prediction, yet performance has been variable. In this work, we first propose an efficient data pre-processing method that maps the time-series EEG signals into an image-like format (a "scalogram") using continuous wavelet transform. We then develop a novel convolution module named "semi-dilated convolution" that better exploits the geometry of wavelet scalograms and nonsquare-shape images. Finally, we propose a neural network architecture named "semi-dilated convolutional network (SDCN)" that uses semi-dilated convolutions to solely expand the receptive field along the long dimension (image width) while maintaining high resolution along the short dimension (image height). Results demonstrate that the proposed SDCN architecture outperforms previous seizure prediction methods, achieving an average seizure prediction sensitivity of 98.90% for scalp EEG and 88.45-89.52% for invasive EEG.Attention-based convolutional neural network (CNN) models are increasingly being adopted for speaker and language recognition (SR/LR) tasks. These include time, frequency, spatial and channel attention, which can focus on useful time frames, frequency bands, regions or channels while extracting features. However, these traditional attention methods lack the exploration of complex information and multi-scale long-range speech feature interactions, which can benefit SR/LR tasks. To address these issues, this paper firstly proposes mixed-order attention (MOA) for low frame-level speech features to gain the finest grain multi-order information at higher resolution. We then combine that with a non-local attention (NLA) mechanism and a dilated residual structure to balance fine grained local detail with convolution from multi-scale long-range time/frequency regions in feature space. The proposed dilated mixed-order non-local attention network (D-MONA) exploits the detail available from the first and the second-order feature attention analysis, but achieves this over a **** wider context than purely local attention.

    We aimed to clarify differences in femoral morphology associated with post-hip fracture knee pain (PHFKP).

    Medical records and radiographic examinations were retrospectively reviewed to determine the relationship between PHFKP development and femoral morphology. Leg length discrepancy (LLD) and neck-shaft angle discrepancy (NSAD) were measured from hip radiographs.

    202 were enrolled, of whom 64 (31.7%) developed PHFKP. The PHFKP group showed more varus NSA. Intertrochanteric femoral fractures (γ-nail or CHS) displayed a more varus NSA.

    The purpose of this study was to determine whether alcohol use disorder (AUD) patients undergoing reverse shoulder arthroplasty (RSA) have increased 1) lengths of stay (LOS); 2) complications; and 3) costs.

This distribution is explained by dissection along the fascial planes in retroperitoneum due to the digestive enzymes within the PFC. In genitourinary track, PFCs have been described to involve the kidneys and the ureters. We report a case of severe acute necrotizing pancreatitis in a 28-year-old male, chronic alcoholic, who on readmission developed features of cystitis. The urine was turbid but did not show significant bacteriuria. Close location of the PFC near the urinary bladder (UB) prompted evaluation of urinary lipase and amylase. Elevated urinary enzyme levels suggested a Pancreatico-vesical fistula, conclusive demonstration of which was established by CT cystography. Percutaneous drainage of the necrosum and stenting of pancreatic duct led to spontaneous healing of the pancreatico-vesical fistula. Our case reiterates the remarkable property of pancreatic enzymes to dissect the fascial planes which is demonstrated by decompression of PFC via UB causing spontaneous Pancreatico-vesical fistula. Further, presence of main pancreatic duct fistulization should prompt endoscopic-guided stenting to obliterate the communication with the fistula and accelerate healing.Parathyroid cyst is a rare clinical entity and often presents as a diagnostic challenge, especially when presents in conjunction with a coexistent parathyroid adenoma. Hence, use of adjunct anatomical imaging like ultrasound, CT scan or MRI is worthwhile to reduce chances of missing hyperfunctional parathyroid tissue, which can eventually lead to revision surgery for persistent primary hyperparathyroidism.Intracranial haemorrhage after supra-tentorial craniotomies can occur in a typical pattern and location which may suggest the diagnosis of remote cerebellar haemorrhage (RCH) which is quite a rare occurrence. The 'Zebra Sign' refers to a pattern of hyperdensity indicative of blood and hypodensity indicative of normal cerebellar parenchyma in a curvilinear, stripe-like fashion along the cerebellar folia and is a characteristic imaging finding in RCH.   • The solid-liquid distribution coefficient (Kd) is a key input parameter in radioecological models. However, its large variability hampers its usefulness in modelling transport processes as well as its accuracy in representing soil-radionuclide interactions. For the specific case of radiocaesium, the analyses of a Cs Kd soil dataset (769 entries) showed that values varied over a five order of magnitude range, and the resulting Cs Kd best estimate (calculated as a geometric mean = 2.5 × 103 L kg-1) lacked reliability and representativity. Grouping data and creation of partial datasets based on the experimental approach (short-term ( ~1 yr)) and soil factors affecting Cs interaction (i.e., the ratio of the radiocaesium interception potential (RIP) to the potassium content in soil solution (Kss); organic matter content (OM) and soil texture) succeeded in reducing variability a few orders of magnitude, with Cs Kd best estimates also differing by one-two orders of magnitude depending on the type of soil and experimental approach. The statistical comparison of the Cs Kd best estimates and related cumulative distribution functions of the partial datasets revealed a relevant effect of the sorption dynamics on Cs Kd values (with long-term values systematically higher than short-term ones), and that the RIP/Kss ratio was an excellent predictor of Cs Kd for short-term scenarios, whereas the RIP parameter could be predicted on the basis of texture information. The OM threshold to distinguish between OM threshold to distinguish between Mineral and Organic soils subclasses, regarding Cs interaction was determined to be 50% and 90% OM for short- and long-term scenarios, respectively. A rapid description based on the NIR technology is implemented to assess black tea tenderness and rankings. First, 700 standard samples from dianhong black tea of seven quality classes are scanned using a NIR system. The raw spectra acquired are preprocessed by Savitzky-Golay (SG) filtering coupled with standard normal variate transformation (SNV). Then, the multi-variable selection algorithm (IGA-PSO) is applied to compare with the single method (the IGA and PSO) and search the optimal characteristic wavelengths. Finally, the identification models are developed using a decision tree (DT), partial least-squares discriminant analysis (PLS-DA), and support vector machine (SVM) based on different kernel functions combined with the effective features from the above variables screening paths for the discrimination of black tea quality. The results show that the IGA-PSO-SVM model with a radial basis function achieves the best predictive results with the correct discriminant rate (CDR) of 95.28% based on selected four characteristic variables in the prediction process. The overall results demonstrate that NIR combined with a multi-variable selection method can constitute a potential tool to understand the most important features involved in the evaluation of dianhong black tea quality helping the instrument manufacturers to achieve the development of low-cost and handheld NIR sensors.
    The solid-liquid distribution coefficient (Kd) is a key input parameter in radioecological models. However, its large variability hampers its usefulness in modelling transport processes as well as its accuracy in representing soil-radionuclide interactions. For the specific case of radiocaesium, the analyses of a Cs Kd soil dataset (769 entries) showed that values varied over a five order of magnitude range, and the resulting Cs Kd best estimate (calculated as a geometric mean = 2.5 × 103 L kg-1) lacked reliability and representativity. Grouping data and creation of partial datasets based on the experimental approach (short-term ( ~1 yr)) and soil factors affecting Cs interaction (i.e., the ratio of the radiocaesium interception potential (RIP) to the potassium content in soil solution (Kss); organic matter content (OM) and soil texture) succeeded in reducing variability a few orders of magnitude, with Cs Kd best estimates also differing by one-two orders of magnitude depending on the type of soil and experimental approach. The statistical comparison of the Cs Kd best estimates and related cumulative distribution functions of the partial datasets revealed a relevant effect of the sorption dynamics on Cs Kd values (with long-term values systematically higher than short-term ones), and that the RIP/Kss ratio was an excellent predictor of Cs Kd for short-term scenarios, whereas the RIP parameter could be predicted on the basis of texture information. The OM threshold to distinguish between OM threshold to distinguish between Mineral and Organic soils subclasses, regarding Cs interaction was determined to be 50% and 90% OM for short- and long-term scenarios, respectively. To be launched onto the market, the selected production method has to be scaled-up, and quality assurance implemented for the product to reach clinical trials, during which in vivo performance is evaluated. Lambs were randomly allocated to 1 of 4 treatment groups (1) Conscious EEG and surgical castration with no anaesthetic intervention (CON; n = 4); (2) Conscious EEG and surgical castration with pre-operative applied intra-testicular lignocaine injection (CON + LIG; n = 4); (3) surgical castration under minimal anaesthesia (MAM; n = 4); (4) and surgical castration with pre-operative lignocaine injection (2 mL lignocaine hydrochloride 20 mg/mL, under minimal anaesthesia (MAM + LIG; n = 4). Distinct differences in the EEG parameters Ptot, F50 and F95 between pre-and post-castration in conscious lambs were demonstrated in this study (p less then 0.01). In two-dimensional affective space, emotional arousal increases as hedonic valence ratings become increasingly pleasant or unpleasant. Cupping therapy images were more unpleasant and more arousing than the control images. Cluster analysis showed that the response to cupping therapy images had emotional characteristics similar to those for fear images. Individuals with a greater fear of pain rated cupping therapy images as more unpleasant and more arousing. Psychophysical analysis showed that individuals experienced unpleasant and aroused emotional states in response to the cupping therapy images. Our findings suggest that cupping therapy might be associated with unpleasant-defensive motivation and motivational activation.     The prevalence of erosive tooth wear in enamel and dentin was relatively high in Lithuania; the erosive tooth wear in enamel and dentin combined was 52% among 35- to 44-year-olds, 68% among 45- to 54-year-olds, 67% among 55- to 64-year-olds, and 63% among 65- to 74-year-olds. Lower fluoride level in drinking water was associated with erosive tooth wear in enamel. Male gender, residency in periurban/rural areas, older age, and presence of acid reflux were associated with higher odds, while higher D3MFS scores were associated with lower odds for erosive tooth wear in dentin. These results can be used to plan dental public health prevention.We evaluated the effects of a simulated workday of prolonged sitting on blood pressure (BP) and pulse wave velocity (PWV) and examined whether posture (seated vs. supine) affected responses. Participants (n = 25) were adults, with overweight/obesity and elevated BP, and performed seated desk work for 7.5 h. BP and PWV were measured in seated and supine postures at baseline (715 a.m.), midday (1205 p.m.), and afternoon (445 p.m.). Generalized linear mixed models evaluated the effects of prolonged sitting on BP and PWV within each posture and interactions by posture and sex. In the recommended postures, seated BP and supine carotid-femoral pulse wave velocity (cfPWV) and carotid-ankle pulse wave velocity (caPWV), but not carotid-radial pulse wave velocity (crPWV), significantly increased over the simulated seated workday (all p 0.05). A simulated workday of prolonged sitting increased seated BP and supine cfPWV and caPWV, and posture minimally influenced these responses. These results add to the evidence suggesting a deleterious effect of prolonged sitting on cardiovascular health.While several intoxications can be successfully treated with specific antidotes, intoxications with the steroid glycoside digitoxin still represent a major challenge. Besides conventional approaches, CytoSorb® hemoadsorption might be another treatment option. We report on an 81-year-old female patient treated in our intensive care unit (ICU) with severe digitoxin intoxication, acute renal failure, and urinary tract infection (UTI). As physiological digitoxin elimination kinetics are known to appear slow, and also in regard to the renal failure, the decision was made to initiate continuous renal replacement therapy combined with CytoSorb hemoadsorption. The patient was hemodynamically stabilized within the first 4 h of treatment and initially required catecholamines to be stopped within 24 h of treatment. Pre- and post-adsorber drug level measurements showed a rapid elimination of digitoxin. Antibiotic treatment with piperacillin/tazobactam was initiated, and despite CytoSorb hemoadsorption therapy and its known potential to reduce plasma concentrations of several drugs, the UTI was successfully treated. After 3 days of CytoSorb treatment, digitoxin plasma levels were stable and almost normalized, and no clinical signs of intoxication were present. Five days after presentation, the patient was transferred from the ICU in a stable condition. CytoSorb hemoadsorption may be an easily available, efficient, and less cost-intensive therapy option than treatment with the Fab fragment, which is the currently recommended therapy for digitalis intoxications. Therefore, the use of CytoSorb might represent an alternative treatment for life-threatening complications of digitoxin intoxications.
    Autism spectrum disorder (ASD) entails varied developmental pathways along the entire lifespan, demanding early and ongoing diverse and responsive interventions to children's needs. This study examined in situ education and development attained by children and youth with ASD in a school with a therapeutic and educational curriculum.

Students' performance was examined with an array of instruments and tasks, including person and clock drawings, false belief, and bouba-kiki metaphor screens. Qualitative and quantitative analyses were performed to uncover associations, weaknesses, and strengths.

We report on an 81-year-old female patient treated in our intensive care unit (ICU) with severe digitoxin intoxication, acute renal failure, and urinary tract infection (UTI). As physiological digitoxin elimination kinetics are known to appear slow, and also in regard to the renal failure, the decision was made to initiate continuous renal replacement therapy combined with CytoSorb hemoadsorption. The patient was hemodynamically stabilized within the first 4 h of treatment and initially required catecholamines to be stopped within 24 h of treatment. Pre- and post-adsorber drug level measurements showed a rapid elimination of digitoxin. Antibiotic treatment with piperacillin/tazobactam was initiated, and despite CytoSorb hemoadsorption therapy and its known potential to reduce plasma concentrations of several drugs, the UTI was successfully treated. After 3 days of CytoSorb treatment, digitoxin plasma levels were stable and almost normalized, and no clinical signs of intoxication were present. Five days after presentation, the patient was transferred from the ICU in a stable condition. Current research and development approaches based on try-and-error methodologies are unable to easily fine-tune nanoparticle populations in order to overcome the numerous constraints of ocular administration routes, which is believed to hamper easy approval from regulatory agencies for these systems. The predictable quality and specifications of the product can be achieved through quality-by-design (QbD) implementation in both research and industrial environments, in contrast to the current quality-by-testing (QbT) framework. Mathematical modelling of the expected final nanoparticle characteristics by variation of operator-controllable variables of the process can be achieved through adequate statistical design-of-experiments (DoE) application. This multivariate approach allows for optimisation of drug delivery platforms, reducing research costs and time, while maximising the understanding of the production process. These relationships were not found to be significant for children with inadequately controlled asthma. Differences in parental attitudes toward feeding and exercise, and child eating and exercise behaviors, between families may help to explain the increased obesity risk for children with asthma.
    Growth arrest-specific factor 6 (GAS6) and the Tyro3, AXL, and MERTK (TAM) receptors counterbalance pro-inflammatory responses. AXL is a candidate receptor for SARS-CoV-2, particularly in the respiratory system, and the GAS6/AXL axis is targeted in current clinical trials against COVID-19. However, GAS6 and TAMs have not been evaluated in COVID-19 patients at emergency admission.

    GAS6 levels were higher in the SARS-CoV-2-positive patients, increasing progressively with the severity of the disease. Patients with initial GAS6 at the highest quartile had the worst outcome, with a 3-month survival of 65%, compared to a 90% survival for the rest. Soluble AXL exhibited higher plasma concentration in deceased patients, without significant differences in MERTK among SARS-CoV-2-positive groups. GAS6 mRNA was mainly expressed in alveolar cells and AXL in airway macrophages. Remarkably, THP-1 human macrophage differentiation neatly induces AXL, and its inhibition (bemcentinib) reduced cytokine production in human macrophages after LPS challenge.

Compared to WT ****, peritoneal macrophages in TNF-deficient **** inoculated with L. monocytogenes respond with M2 polarization by upregulating Arg1. Consistently, TNF blockade in vitro resulted in M2 polarization in peritoneal macrophages during L. monocytogenes infection. Additionally, TNF promotes the transition from M2 to M1 polarization in peritoneal macrophages. Further investigation of peritoneal macrophage polarization suggested the NF-κB pathway is involved in the TNF-dependent M2 to M1 shift. Visual acuity was counting fingers, and bilateral retinitis and panuveitis were revealed in ocular examination. The result of the vitreous sample using RT-PCR was positive for SARS-CoV-2 and negative for Herpesviridae viruses and mycobacterium tuberculosis. The patient was successfully treated with corticosteroid.Conclusion We report a case of bilateral neuroretinitis and panuveitisin a recovered COVID-19 patient and positive RT-PCR of the vitreous sample. It is suggested to apply intraocular sampling and evaluation for COVID-19 in patients with the new-onset of uveitis and/or retinitis during the pandemic.
    Medial elbow injury is common in baseball pitchers, with evidence of elbow valgus instability after only 60 consecutive pitches. However, the tissue-specific effects of repetitive pitching on medial elbow stabilizers are largely unknown.

    This study aimed to investigate changes in the ulnar collateral ligament (UCL) and forearm flexor-pronator muscles (FPMs) during repetitive pitching and factors that relate to identified change. We hypothesized that repetitive pitching would increase elasticity of the medial elbow stabilizers and therefore induce laxity.

    Descriptive laboratory study.

    A total of 30 high school baseball pitchers participated (mean ± SD age, 16.6 ± 0.5 years). Each participant pitched 100 times (5 blocks of 20 pitches). The strain ratio, indicating elasticity in the UCL and FPMs, was measured using ultrasound before pitching and after every 20-pitch block. Data for each pitch block were compared using analysis of variance. Multiple regression analysis was used to investigate factors relamaintaining a large muscle volume to avoid excessive elbow flexion range of motion may prevent UCL laxity that develops during repetitive pitching.

Fixed effects (within-individual) regression models were used to estimate concurrent associations between exposure to gun violence and three outcomes depressive symptoms, anxiety symptoms, and aggression (both overall and separately for proactive and reactive aggression). The reverse direction (anxiety, depressive symptoms, and aggression predicting gun violence exposure) was also modeled.Results After controlling for covariates, exposure to gun violence was significantly associated with increases in reactive aggression and, to a lesser extent, increases in proactive aggression. Consistently, TNF blockade in vitro resulted in M2 polarization in peritoneal macrophages during L. monocytogenes infection. Additionally, TNF promotes the transition from M2 to M1 polarization in peritoneal macrophages. Further investigation of peritoneal macrophage polarization suggested the NF-κB pathway is involved in the TNF-dependent M2 to M1 shift. Conversely, treatment of peritoneal macrophage with a PPARγ agonist blunted the expression of M1 genes induced by TNF and reduced NF-κB signalling pathway activation. The result of the vitreous sample using RT-PCR was positive for SARS-CoV-2 and negative for Herpesviridae viruses and mycobacterium tuberculosis. The patient was successfully treated with corticosteroid.Conclusion We report a case of bilateral neuroretinitis and panuveitisin a recovered COVID-19 patient and positive RT-PCR of the vitreous sample. It is suggested to apply intraocular sampling and evaluation for COVID-19 in patients with the new-onset of uveitis and/or retinitis during the pandemic. Medial elbow injury is common in baseball pitchers, with evidence of elbow valgus instability after only 60 consecutive pitches. However, the tissue-specific effects of repetitive pitching on medial elbow stabilizers are largely unknown. This study aimed to investigate changes in the ulnar collateral ligament (UCL) and forearm flexor-pronator muscles (FPMs) during repetitive pitching and factors that relate to identified change. We hypothesized that repetitive pitching would increase elasticity of the medial elbow stabilizers and therefore induce laxity. Descriptive laboratory study. A total of 30 high school baseball pitchers participated (mean ± SD age, 16.6 ± 0.5 years). Each participant pitched 100 times (5 blocks of 20 pitches). The strain ratio, indicating elasticity in the UCL and FPMs, was measured using ultrasound before pitching and after every 20-pitch block. Data for each pitch block were compared using analysis of variance. They were interviewed 9 times over 5 years. Fixed effects (within-individual) regression models were used to estimate concurrent associations between exposure to gun violence and three outcomes depressive symptoms, anxiety symptoms, and aggression (both overall and separately for proactive and reactive aggression). The reverse direction (anxiety, depressive symptoms, and aggression predicting gun violence exposure) was also modeled.Results After controlling for covariates, exposure to gun violence was significantly associated with increases in reactive aggression and, to a lesser extent, increases in proactive aggression. In addition, gun violence exposure was associated with increased anxiety but not depressive symptoms.

  • Soft tissue keratocysts (SKC) are extremely rare and show similar microscopic morphology to keratocystic odontogenic tumor. The aim was to investigate immunohistochemical (IHC) features and origin of SKCs developing in buccal mucosa and lateral facial deep region.

Total eight (8) cases were diagnosed four cases of FDCS and four cases of HS involving nodal and extra-nodal sites. Clinical, histopathological, immunohistochemistry (IHC) and therapeutic data of the eight cases were retrieved and analyzed.

    Descriptive statistics.

    Among the four patients of FDCS, two had nodal and two had extra-nodal disease. Mean tumor size was 6 cm. Tumor cells expressed CD23, CD21, CD45, CD68 and S100. One patient received adjuvant chemotherapy (Gemcitabine and Docetaxel). Median survival was 36 months. None of them developed distant metastasis.   • The continuous demand for thermoplastic polymers in a great variety of applications, combined with an urgent need to minimize the quantity of waste for a balanced energy-from-waste strategy, has led to increasing scientific interest in developing new recycling processes for plastic products. Glycol-modified polyethylene terephthalate (PETG) is known to have some enhanced properties as compared to polyethylene terephthalate (PET) homopolymer; this has recently attracted the interest from the fused filament fabrication (FFF) three-dimensional (3D) printing community. PET has shown a reduced ability for repeated recycling through traditional processes. Herein, we demonstrate the potential for using recycled PETG in consecutive 3D printing manufacturing processes. Distributed recycling additive manufacturing (DRAM)-oriented equipment was chosen in order to test the mechanical and thermal response of PETG material in continuous recycling processes. Tensile, flexure, impact strength, and Vickers micro-hardness tests were carried out for six (6) cycles of recycling. Finally, Raman spectroscopy as well as thermal and morphological analyses via scanning electron microscopy (SEM) fractography were carried out. In general, the results revealed a minor knockdown effect on the mechanical properties as well as the thermal properties of PETG following the process proposed herein, even after six rounds of recycling.We present a compact optical head design for wide-range and low noise displacement sensing using deep frequency modulation interferometry (DFMI). The on-axis beam topology is realised in a quasi-monolithic component and relies on cube beamsplitters and beam transmission through perpendicular surfaces to keep angular alignment constant when operating in air or in a vacuum, which leads to the generation of ghost beams that can limit the phase readout linearity. We investigated the coupling of these beams into the non-linear phase readout scheme of DFMI and implemented adjustments of the phase estimation algorithm to reduce this effect. The macro-mechanical material model was developed treating the Fiber-Reinforced Plastic (FRP) as an Equivalent Homogeneous Material (EHM). To reduce computational time, a numerical analysis was performed to investigate the influence of mass scaling, bulk viscosity, friction, strain rate strengthening, and cohesive surface modelling. A consideration must be made to minimize the dynamic effects in the FE prediction. The experimental work was carried out to investigate the effect of drill tip geometry on drilling forces and hole quality and to validate the FE results. The geometry of the drills used were either double-point angle or a "candle-stick" profile. The 3D drilling model accurately predicts the thrust force and hole quality generated by the two different drills. The results highlight the improvement in predicted results with the inclusion of cohesive surface modelling. The force signature profiles between the simulated and experimental results were similar. Furthermore, the difference between the predicted thrust force and those measured were less than 9%. When drilling with a double-angle drill tip, the inter-ply damage was reduced. This trend was observed in FE prediction.Like other sources of pollution, noise is considered to be one of the main concerns of citizens, due to its invisibility and the potential harm it can cause. Noise pollution could be considered as one of the biggest quality-of-life concerns for urban residents in big cities, mainly due to the high levels of noise to which they may be exposed. Such levels have proven effects on health, such as sleep disruption, hypertension, heart disease, and hearing loss. In a scenario where the number of people concentrated in cities is increasing, tools are needed to quantify, monitor, characterize, and quantify noise levels. This paper presents the ZARATAMAP project, which combines machine learning techniques with a geo-sensing application so that the authorities can have as much information as possible, using a low-cost embedded and mobile node, that is easy to deploy, develop, and use.
