Nine criteria, including preoperative nasal congestion, snoring, nasal deformity (visuality), surgical treatment, postoperative nasal congestion, bruising\swelling, pain, patient satisfaction, and medical treatment, were examined. Results The number of likes for the videos with preoperative nasal deformity data (visuality) was found to be significantly higher than that for the videos without preoperative nasal deformity (visuality) data. Methods We conducted a systematic search in PubMed, EMBASE and Ovid Medline using appropriate keywords/MESH terms search strategy. All patients with thrombosis involving the deep venous sinuses were included if treatment records were available. Outcome measures included recanalization of the affected sinus, good functional outcome assessed by a modified Rankin scale (mRS) of 0-2 or reported independent functional outcomes, permanent neurological deficits, further hemorrhage and mortality. Results A total of 69 studies comprising 120 patients were included in the analysis. Anticoagulation was the most common treatment (85.8%), whilst local intrasinus thrombolysis was performed in 40.0% of the patients and mechanical endovascular modalities were employed in 20.0% of the patients. Recanalization of the occluded sinus was seen in 83.5% of the patients while 62.6% patients achieved good functional outcome. There was considerable morbidity with 60.7% having a permanent neurological deficit, 23.3% having further hemorrhage after admission and 18.6% mortality. In the cohort receiving anticoagulation, 65.3% achieved good outcome but intracranial hemorrhage at presentation was associated with poorer outcome, permanent deficits, further bleeding and mortality. Methods We performed a single center retrospective analysis of all patients treated by mechanical thrombectomy due to LVO using the Tigertriever. The angiographic and clinical success was evaluated by the modified thrombolysis in cerebral infarction score (mTICI) and the modified Rankin score (mRS). Results A total of 68 acute intracranial arterial occlusions in 61 patients (42 female, median age 77 years, range 43-92 years) were treated by mechanical thrombectomy using the Tigertriever. The overall successful reperfusion rate (mTICI 2b-3) was 85.3% (58/68 occlusions) with a first pass effect (mTICI 3) of 23.5% (16/68 occlusions). In 57 of the 68 occlusions the Tigertriever was used on an intention to treat approach with a success rate of 86.0% and in the 11 remaining occlusions where the Tigertriever was used as a bail-out device the success rate was 81.9%. In seven patients a mild subarachnoid hemorrhage occurred (11.5%) and one symptomatic intracerebral hemorrhage was observed (1.6%). At discharge 39.3% of the patients (24/61) had a favorable outcome (mRS 0-2). Conclusion The Tigertriever offers a safe and effective treatment option in ischemic stroke due to LVOs with reperfusion rates and a safety profile similar to alternative devices. The Tigertriever is a promising bail-out tool in complex cases. Its role as a first line device has to be evaluated in further prospective studies.

  • The developed method was successfully applied for screening adulterated dietary supplement samples.Chemerin is a recently discovered adipokine that participates in the regulation of many physiological and disorder-related processes in mammals, including metabolism, inflammatory reactions, obesity, and reproduction. We investigated how chemerin affects the transcriptome profile of porcine luteal cells. The luteal cells were acquired from mature gilts. After the in vitro culturing with and without chemerin, the total RNAs were isolated and high-throughput sequencing was performed. Obtained datasets were processed using bioinformatic tools. The study revealed 509 differentially expressed genes under the chemerin influence. Their products take part in many processes, important for the functions of the corpus luteum, such as steroids and prostaglandins synthesis, NF-κB and JAK/STAT signal transducing pathways, and apoptosis. The expression of the CASP3, HSD3B7, IL1B, and PTGS2 genes, due to their important role in the physiology of the corpus luteum, was validated using the quantitative real-time polymerase chain reaction (qPCR) method. The qPCR confirmed the changes of gene expression. Chemerin in physiological concentrations significantly affects the expression of many genes in luteal cells of pigs, which is likely to result in modification of physiological processes related to reproduction.Gases produced inside harvested fruit sensitively influence the continuing quality of the stored fruit and its maximum time of storability. In this work, the evolution of gaseous volatiles inside "Golden Delicious" apples were studied using CO2 laser photoacoustic spectroscopy with the aim of developing new methods for in-storage fruit quality monitoring. Methods A population cohort for analysis was obtained as all hospital admissions for colorectal cancer during the period 2008-2014, according to the Spanish Minimum Basic Data Set. The main measure was actual and expected mortality after the application of the considered mathematical model. A logistic regression model and a mortality score were created, and internal validation was performed. Results 115,841 hospitalization episodes were studied. Of these, 80% were included in the training set. The variables associated with in-hospital mortality were age (OR 1.06, 95%CI 1.05-1.06), urgent admission (OR 4.68, 95% CI 4.36-5.02), pulmonary disease (OR 1.43, 95%CI 1.28-1.60), stroke (OR 1.87, 95%CI 1.53-2.29) and renal insufficiency (OR 7.26, 95%CI 6.65-7.94). The level of discrimination (area under the curve) was 0.83. Conclusions This mortality model is the first to be based on administrative clinical databases and hospitalization episodes. In many of these applications, one of the most common traffic pattern types is a periodic traffic patterns, however, the majority of existing analytical models target either saturated or unsaturated network traffic patterns. Furthermore, few of them can be directly extended to the periodic traffic scenario, since periodic traffic brings unstable load status to sensor nodes. To better characterize the WSNs with periodic traffic, we propose an accurate and scalable analytical framework for the IEEE 802.15.4 ****protocol. By formulating the relationship between clear channel assessment (CCA) and its successful probability from the perspective of channel state and node state, single node's behavior and whole network's performance under different network scales and traffic loads can be derived. Extensive simulations are conducted to validate the proposed framework in terms of both local statistics and overall statistics, and the results show that the model can represent the actual behavior and the real performance of both single node and whole network. Besides, as the simplified version of double CCAs mode (DS mode), single CCA mode (SS mode), is also analyzed with simple modifications on the proposed analytical framework. Combining the analytical framework with simulation results, the applicable network scenarios of two modes are also demonstrated respectively. Finally, an approximate distribution of one data packet's backoff duration is proposed. With this approximate distribution, a conservative estimation of data packet's average transmission latency in networks with given configurations can be easily carried out.The extracellular matrix (ECM) plays a key role during cell migration, proliferation, and differentiation by providing adhesion sites and serving as a physical scaffold. Elucidating the interaction between the cell and ECM can reveal the underlying mechanisms of cellular behavior that are currently unclear. Analysis of the deformation of the ECM due to cell-matrix interactions requires microscopic, three-dimensional (3-D) imaging methods, such as confocal microscopy and second-harmonic generation microscopy, which are currently limited by phototoxicity and bleaching as a result of the point-scanning approach. In this study, we suggest the use of optical coherence microscopy (OCM) as a live-cell, volumetric, fast imaging tool for analyzing the deformation of fibrous ECM. We optimized such OCM parameters as the sampling rate to obtain images of the best quality that meet the requirements for robust digital volume correlation (DVC) analysis. Visualization and analysis of the mechanical interaction between collagen ECM and human umbilical vein endothelial cells (HUVECs) show that cellular adhesion during protrusion can be analyzed and quantified.

  • A similar proportion of patients with and without rheumatic disease were hospitalised (23 (44%) vs 42 (40%)), p=0.50) but those with rheumatic disease required intensive care admission and mechanical ventilation more often (11 (48%) vs 7 (18%), multivariable OR 3.11 (95% CI 1.07 to 9.05)). Mortality was similar between the two groups (3 (6%) vs 4 (4%), p=0.69). Conclusions Patients with rheumatic disease and COVID-19 infection were more likely to require mechanical ventilation but had similar clinical features and hospitalisation rates as those without rheumatic disease. Methods All data pertaining to the epidemic status of malaria and malaria case were captured from Henan Province in 2018, and the epidemic status of malaria and the diagnosis and treatment of malaria cases were analyzed using a descriptive epidemiological method. Results A total of 174 malaria cases were reported in Henan Province in 2018, with a male-to-female ratio of 33.8∶1. The cases were predominantly found at 30- and 40- years, and farmer was the highest-risk population. All cases were imported for overseas countries and 96.55% (168/174) were from Africa. The cases were reported across 17 cities of the province, and 63.22% (110 cases) were detected in 4 cities of Zhengzhou, Luoyang, Xuchang and Puyang. Both the median duration from onset to initial admission and the median time from the initial admission to definitive diagnosis were 1 d. There were 6 cases (3.45%) with more than 7 days to visit a doctor, and there were 13 cases (7.47%) with definitive diagnosis of over 7 days (delay in definitive diagnosis). All malaria cases were reported within 24 h, and 98.28% (171/174) completed case investigations within 3 d. All 18 cities had achieved malaria elimination by December 2018. Conclusions All malaria cases reported in Henan Province in 2018 are imported from overseas countries. Intensifying malaria surveillance among returnees from Africa and Southeast Asia and timely identification and treatment of imported malaria cases are required to prevent the development of secondary malaria cases, so as to ensure the achievement of malaria elimination in Henan Province on schedule.Objective To evaluate the indoor effect of six human metabolic compounds for trapping adult Culex pipiens quinquefasciatus. In addition, Anisakis larvae infections were detected in fresh and live marine fish samples collected from local markets, andfish detected, the prevalence and intensity of Anisakis larvae infection was 64.0% and 8.1 larvae per fish, with high prevalence seen in Trichiurus haumela and Pneumatophorus japonicas. Conclusions The awareness of anisakiasis prevention and control knowledge is low among the high-risk populations living along the coastal areas of Jiangsu Province, and there are high-risk behaviors, such as eating raw or half-cooked food, using the same chopping board for raw and cooked food. In addition, the prevalence of Anisakis infections is high in the marine fish in these areas. The patient was taken for intraoperative blue light cystoscopy, which revealed multiple lesions that were then resected. This case highlights an important role for blue light cystoscopy in diagnosing patients with high suspicion for malignancy but negative white light cystoscopy. Intrauterine devices(IUD) are used by women worldwide as the most conventional method of reversible contraception. Migration of an IUD to pelvic or abdominal organs is considered rare. We report a case of a 67 year old female who had neglected to remove her IUD for 20 years. She visited our clinic because of recurrent UTI's the previous year. After assessment and clinical investigation it was found that the coil had a late onset migration to the urinary bladder, with calculus formation, causing the patients' symptoms. In our case we cystoscopically removed the encrusted coil after performing endoscopic cystolitholapaxy. The patient had an uneventful recovery and her UTI's subsided. The objective of this article is to report the first case of intravesical IUD migration at our clinic and raise awareness of forgotten contraceptive devices and their potential complications. Anterior urethral valves are a rare cause of obstructed voiding in adolescent children and are often unheard by adult urologists. In this case report, we discuss the management of two adolescent patients who were referred to us with obstructive voiding symptoms with a diagnosis of neurogenic bladder and posterior urethral valves respectively but on evaluation were found to have anterior urethral valves. This article highlights the need for considering anterior urethral valve as an important differential diagnosis in children and adolescents presenting with obstructive voiding symptoms so as to avoid delay in diagnosis and management in young boys. Adenocarcinomas represent 2% of bladder cancers and originate from the bladder urothelium or the urachus.1 Urachal adenocarcinoma constitutes less than 0.5% of all bladder cancers.2 We report a urachal adenocarcinoma case managed surgically with two incisions and an emphasis on cosmesis while maintaining oncologic principles. BACKGROUND/OBJECTIVE Ureteral triplication is a rare congenital malformation with a wide spectrum of presentation that requires a high index of suspicion in order to diagnose. To date, only about 100 cases have been published in medical literature. Smith et al.1 proposed a classification for ureteral triplication that describes it as three separate ureters and three separate ureteral orifices with no interconnection between ureters. In this video, we present a surgical approach for laparoscopic partial nephrectomy of both upper moieties in the context of ureteral triplication. MATERIALS AND METHODS The procedure is initiated with cystoscopy and retrograde pyelogram to better determine the pertinent anatomy. A ureteral catheter is inserted into the healthy ureter to facilitate its identification and reduce the risk of inadvertent injury. The patient is placed in the modified flank position. A 5 mm camera port is inserted using the Hasson technique and two additional ports are inserted under vision. The colon is deflected medially, and the kidney, ureters and blood supply are identified. Both upper pole ureters are transected. Variance of the blood supply can be encountered; therefore, highly selective dissection of the vessels is performed, and care is taken to preserve the main renal vessels. Upper pole partial nephrectomy is carried out using a harmonic scalpel at the demarcation line. RESULTS Operative time was 182 minutes. Discharge was on post-operative day 2 without intra or post-operative complications. Follow-up ultrasound 3 months post-operation demonstrated a normal lower pole moiety without hydronephrosis or hydroureter. CONCLUSIONS Laparoscopic partial nephrectomy with highly selective dissection of vessels in the context of a ureteral triplication anomaly is feasible and safe. Abnormal blood supply should be considered and addressed accordingly. The neurobiology of sex differences during language processing has been widely investigated in the past three decades. The present systematic review of the literature and meta-analysis aimed to determine the degree of agreement among studies reporting sex differences in cortical activity during language processing. Irrespective of the modality and the specificity of the language task, sex differences in the BOLD signal or cerebral blood flow was highly inconsistent across fMRI and PET studies. On the temporal side, earlier latency of auditory evoked responses for female compared to male participants were consistently observed in EEG studies during both listening and speaking. This case highlights an important role for blue light cystoscopy in diagnosing patients with high suspicion for malignancy but negative white light cystoscopy. Intrauterine devices(IUD) are used by women worldwide as the most conventional method of reversible contraception. Migration of an IUD to pelvic or abdominal organs is considered rare. We report a case of a 67 year old female who had neglected to remove her IUD for 20 years. She visited our clinic because of recurrent UTI's the previous year. After assessment and clinical investigation it was found that the coil had a late onset migration to the urinary bladder, with calculus formation, causing the patients' symptoms. In our case we cystoscopically removed the encrusted coil after performing endoscopic cystolitholapaxy. The patient had an uneventful recovery and her UTI's subsided. The objective of this article is to report the first case of intravesical IUD migration at our clinic and raise awareness of forgotten contraceptive devices and their potential complications. Anterior urethral valves are a rare cause of obstructed voiding in adolescent children and are often unheard by adult urologists. In this case report, we discuss the management of two adolescent patients who were referred to us with obstructive voiding symptoms with a diagnosis of neurogenic bladder and posterior urethral valves respectively but on evaluation were found to have anterior urethral valves. This article highlights the need for considering anterior urethral valve as an important differential diagnosis in children and adolescents presenting with obstructive voiding symptoms so as to avoid delay in diagnosis and management in young boys. Adenocarcinomas represent 2% of bladder cancers and originate from the bladder urothelium or the urachus.1 Urachal adenocarcinoma constitutes less than 0.5% of all bladder cancers.2 We report a urachal adenocarcinoma case managed surgically with two incisions and an emphasis on cosmesis while maintaining oncologic principles. BACKGROUND/OBJECTIVE Ureteral triplication is a rare congenital malformation with a wide spectrum of presentation that requires a high index of suspicion in order to diagnose. To date, only about 100 cases have been published in medical literature. Smith et al.1 proposed a classification for ureteral triplication that describes it as three separate ureters and three separate ureteral orifices with no interconnection between ureters. In this video, we present a surgical approach for laparoscopic partial nephrectomy of both upper moieties in the context of ureteral triplication. MATERIALS AND METHODS The procedure is initiated with cystoscopy and retrograde pyelogram to better determine the pertinent anatomy. A ureteral catheter is inserted into the healthy ureter to facilitate its identification and reduce the risk of inadvertent injury. The patient is placed in the modified flank position. A 5 mm camera port is inserted using the Hasson technique and two additional ports are inserted under vision. The colon is deflected medially, and the kidney, ureters and blood supply are identified. Both upper pole ureters are transected. Variance of the blood supply can be encountered; therefore, highly selective dissection of the vessels is performed, and care is taken to preserve the main renal vessels. Upper pole partial nephrectomy is carried out using a harmonic scalpel at the demarcation line. RESULTS Operative time was 182 minutes. Discharge was on post-operative day 2 without intra or post-operative complications. Follow-up ultrasound 3 months post-operation demonstrated a normal lower pole moiety without hydronephrosis or hydroureter. CONCLUSIONS Laparoscopic partial nephrectomy with highly selective dissection of vessels in the context of a ureteral triplication anomaly is feasible and safe. Abnormal blood supply should be considered and addressed accordingly. The neurobiology of sex differences during language processing has been widely investigated in the past three decades. While substantial sex differences have been reported, empirical findings however appear largely equivocal. The present systematic review of the literature and meta-analysis aimed to determine the degree of agreement among studies reporting sex differences in cortical activity during language processing. Irrespective of the modality and the specificity of the language task, sex differences in the BOLD signal or cerebral blood flow was highly inconsistent across fMRI and PET studies. On the temporal side, earlier latency of auditory evoked responses for female compared to male participants were consistently observed in EEG studies during both listening and speaking. Overall, the present review and meta-analysis support the theoretical assumption that there are much more similarities than differences between men and women in the human brain during language processing. Subtle but consistent temporal differences are however observed in the auditory processing of phonetic cues during speech perception and production. To integrate auditory and visual signals into a unified percept, the paired stimuli must co-occur within a limited time window known as the Temporal Binding Window (TBW). The width of the TBW, a proxy of audiovisual temporal integration ability, has been found to be correlated with higher-order cognitive and social functions. A comprehensive review of studies investigating audiovisual TBW reveals several findings (1) a wide range of top-down processes and bottom-up features can modulate the width of the TBW, facilitating adaptation to the changing and multisensory external environment; (2) a large-scale brain network works in coordination to ensure successful detection of audiovisual (a)synchrony; (3) developmentally, audiovisual TBW follows a U-shaped pattern across the lifespan, with a protracted developmental course into late adolescence and rebounding in size again in late life; (4) an enlarged TBW is characteristic of a number of neurodevelopmental disorders; and (5) the TBW is highly flexible via perceptual and musical training.

The total CD3-positive T-cell percentages correlated positively between FC and MIF (r = 0.53, p = 0.05), but were significantly lower by MIF (1.62% vs. 3.4%, p = 0.009). The percentage of mutant NPM1-positive (NPM1c+) cells ranged from 9.7 to 90.8% (median 45.4%) and did not correlate with the NPM1 mutant allele fraction by NGS (p > 0.05). The percentage of CD34+/NPM1c+ cells ranged from 0 to 1.8% (median 0.07%). Patients ranged in age from 19 to 92 years (mean 55 years), with a slight female predominance (57%). Mean tumor size was 5.4 cm, with the largest tumor measuring 29 cm. Disease progression following surgery, defined as local recurrence, metastasis, or disease-related death, occurred in 56 patients (14%). Colorectal tumors were most likely to progress, followed by small bowel and gastric tumors. None of the esophageal tumors in this series progressed. The MEK2P128L mutation, which has been identified previously in solid tumor and histiocytosis patients, caused hyperactivated MAP kinase signaling, conferred IL-3 hypersensitivity and sensitized the cells to various MEK inhibitors. The RUNX1G60C mutation abolished the transcriptional activity of wild-type RUNX1 and functioned as a dominant negative form of RUNX1, resulting in enhanced self-renewal activity in hematopoietic stem/progenitor cells. Interestingly, ERK was heavily activated in both patients, highlighting a potential role for activation of MAPK signaling in iMCD-TAFRO pathogenesis and a rationale for exploring inhibition of the MAPK pathway as a therapy for iMCD-TAFRO. Activation of the EGFR-ZNF263 signaling axis in phenotypically normal astrocytes or glioblastoma cells triggers or enhances tumorigenic activities, while elevated expression of the EGFR-ZNF263 signaling components in glioblastoma tissues is associated with poor prognosis of the patients.

  • To date, no data of water poisoning episodes recorded were associated with MCs presence in the water. However, this might be underestimated due to a lack of monitoring facilities and/or a lack of public health staff trained for recognizing symptoms of MCs intoxication since the presence of high MCs concentration was reported in Maputo and Gaza provinces.Potato tocosh is a naturally processed potato for nutritional and curative purposes from traditional Peruvian medicine. The aim of this study was to investigate the acute and sub-acute toxicity of tocosh flour (TF). For sub-acute toxicity, TF was administered orally to rats daily once a day for 28 days at doses of 1000 mg/kg body weight (BW). Animals were observed for general behaviors, mortality, body weight variations, and histological analysis. At the end of treatment, relative organ weights, histopathology, hematological and biochemical parameters were analyzed. For acute toxicity, TF was administered orally to **** at doses of 2000 and 5000 mg/kg BW at a single dose in both sexes. Early identification of nutrition risk is important to prevent or treat undernutrition. This study describes the prevalence rates of nutrition risk in community-dwelling older adults (aged ≥ 65) using the same validated tool across different countries and aims to identify differences in nutritional risk factors. Cross-sectional data was obtained from three datasets including participants from the Netherlands (NL), Canada (CA) and New Zealand (NZ). Seniors in the Community Risk Evaluation for Eating and Nutrition II (SCREEN II) was used to assess nutritional risk factors and prevalence of risk. Differences between countries were tested with logistic and linear regression. Sensitivity analyses were conducted to test the influence of sampling strategy. A total of 13,340 participants were included, and 66.3% were found to be at high nutrition risk. After stratifying the data for method of data sampling, prevalence rates showed some differences across countries (NL 61.5%, NZ 68.2%, CA 70.1%). Risk factor items that contributed to nutrition risk also differed among countries NZ and CA participants scored higher for weight change, skipping meals, problems with meal preparation, use of meal replacements, problems with biting and chewing, low fluid intake and problems with doing groceries, as compared to participants in NL. Low intake of fruits and vegetables and meat were more prevalent in NL. In this study, we determined the effect of intratympanic administration of rodent bone marrow MSCs (BM-MSCs) on the inner ear in an immunocompetent rat model. The administration of MSCs did not lead to the generation of any oxidative stress in the rat inner ear. There was no significant production of proinflammatory cytokines, tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-6 and IL-12, due to BM-MSCs administration into the rat cochlea. BM-MSCs do not activate caspase 3 pathway, which plays a central role in sensory cell damage. Additionally, transferase dUTP nick end labeling (TUNEL) staining determined that there was no significant cell death associated with the administration of BM-MSCs. This review assesses the correlation between social and dietary changes over the course of human evolution and the adaptation of the human microbiota to those changes. In addition, it discusses the main dietary strategies for modulating the microbiota and the difficulties of putting them properly into practice.

  • ETHNOPHARMACOLOGICAL RELEVANCE Type 2 diabetes mellitus (T2DM) is currently one of the most prominent and global chronic conditions. Huanglian Decoction (HLD) is a traditional Chinese medicine (TCM) preparation that has been used to treat T2DM for thousands of years in China. However, its mechanism of action at the metabolic level is still unclear. The purpose of this work is to study the mechanism of HLD in treating T2DM based on metabolomics and network pharmacology. MATERIALS AND METHODS In this study, metabolomics combined with network pharmacology was used to elucidate the therapeutic mechanism of HLD in T2DM. Serum samples were collected from rats with T2DM, induced by a high-sugar and high-fat diet combined with streptozotocin (STZ), to measure the levels of biochemical markers. Urinary metabolomics-based analysis using high-performance liquid chromatography coupled with mass spectrometry (HPLC-MS) was conducted to evaluate the differential metabolites from multiple metabolic pathways. The potential site of compression was identified on MRI where the DP artery ran deep to the EHB tendon near the first and second tarsometatarsal joints. On diagnostic arteriogram, there was notching of the dorsalis pedis over the talus bone. The dorsalis pedis Doppler signal was obliterated upon plantar flexion. A longitudinal incision was made over the artery in the area of compression. The flexor retinaculum was incised. Abnormal fibrous bands were identified that were lysed anterior to the artery. The EHB tendon was released and transferred distally to the extensor hallucis longus tendon. A completion angiogram showed a persistently patent dorsalis pedis artery with plantar flexion. She was discharged one day post-operatively without issues. On follow-up, the patient was ambulatory with complete resolution of her pain. Arterial duplex demonstrated normal velocities through the dorsalis pedis in all positions. CONCLUSION Symptomatic PAD is a rare presentation in young adults and is usually due to nonatherosclerotic pathophysiology. We present a rare case of dorsalis pedis artery entrapment syndrome. Given the mechanical nature of obstruction, surgical correction was an effective treatment. PURPOSE Visual review of individual spectra in magnetic resonance spectroscopic imaging (MRSI) data benefits from the application of spectral smoothing; however, if this processing step is applied prior to spectral analysis this can impact the accuracy of the quantitation. This study aims to analyze the effect of spectral denoising and apodization smoothing on the quantitation of whole-brain MRSI data obtained at short TE. METHODS Short-TE MRSI data obtained at 3 T were analyzed with no spectral smoothing, following (i) Gaussian apodization with values of 1, 2, 4, 6, and 8 Hz, and (ii) denoising using principal component analysis (dnPCA) with 3 different values for the number of retained principal components. The mean lobar white matter estimates for four metabolites, signal-to-noise ratio (SNR), spectral linewidth, and confidence intervals were compared to data reconstructed using no smoothing. Additionally, a voxel-wise comparison for N-acetylaspartate quantitation with different smoothing schemes was performed. RESULTS Significant pairwise differences were seen for all Gaussian smoothing methods as compared to no smoothing (p less then 0.001) in linewidth and metabolite estimates, whereas dnPCA methods showing no statistically significant differences in these measures. Confidence intervals decreased, and SNR increased with increasing levels of apodization smoothing or dnPCA denoising. CONCLUSION Mild Gaussian apodization (≤2 Hz at 3 T) can be applied with minimal (1%) errors in quantitation; however, smoothing values greater than that can significantly affect metabolite quantification. In contrast, mild to moderate dnPCA based denoising provides quantitative results that are consistent with the analysis of unsmoothed data and this method is recommended for spectral denoising. This research seeks to explore the drinking patterns, perceptions, and stigma of drink driving behavior of injury patients at Kilimanjaro Christian Medical Center in Moshi, Tanzania. METHODS This mixed methods study incorporated the Perceived Alcohol Stigma (PAS), an additive Likert scale, and the Alcohol Use Disorders Identification Test (AUDIT). Results are reported as medians with IQRs. Additionally, focus groups with injury patients, their families, and community members (n = 104) were conducted and analyzed in pairs using an inductive thematic content analysis approach. RESULTS Those who self-reported driving after ingesting 3 or more alcoholic drinks had a median AUDIT score (median=11.0) significantly higher than those who denied drink driving (median=5.5, p less then 0.01). The PAS showed a high overall stigma against those who use alcohol but differed for drink drivers, drinkers, and abstainers (median= 20.8, 23.9, 34.9, p less then 0.01). Thematic content analysis highlighted a 'disapproving of drink drivers,' that 'problematic drinking is a drinking behavior which negatively affects others,' and a 'passiveness toward drinking and drink driving.' CONCLUSIONS Stigma against those who use alcohol is present in Tanzania. We conducted a retrospective cohort study in a large tertiary care center and obtained data through review of electronic health records. Among patients with NVAF and VTE on apixaban, there were no differences in rates of major bleeding (MB) and clinically relevant nonmajor bleeding (CRNMB) in the overweight and obese populations when compared to normal weight and underweight individuals. The multivariate adjusted analysis for rivaroxaban found that the odds of CRNMB for patients with BMI less then 25 was 5.37 (95% CI 1.50-19.32) times higher than that of BMI ≥25. Moreover, patients on medications that had known interactions with DOACs had 6.40 times higher odds of CRNMB than patients without such interactions (95% CI 1.49-27.57), which was not accounted for by the effects of aspirin and plavix alone. Exosomes were isolated for RNA extraction. The exosomal RNA samples from pigs on day 9 of the estrus cycle and pregnancy were used for small-RNA sequencing. A total 321 miRNAs were identified in all samples. Twenty eight differentially abundant miRNAs were identified between the pregnant and control groups.

  • The aim of this study was to investigate the demographic and clinical characteristics of patients with multiple sclerosis (MS) diagnosed between 1986 and 2015. 333 patients with definite MS were divided into four subgroups according to the following diagnostic criteria Group A) Poser (n = 145), Group B) McDonald 2000 (n = 66), Group C) McDonald 2005 (n = 62), and Group D) McDonald 2010 (n = 60). We investigated 1) patient sex and age at diagnosis, 2) symptoms and number of relapses that prompted MS diagnosis, 3) time between first symptoms suggestive of MS and confirmed diagnosis, and 5) Expanded Disability Status Scale (EDSS) score at disease onset. The overall female-to-male ratio was 2.31, but in the subgroups it differed significantly (A - 1.9; B - 1.6; C - 4.7; D - 3.6). The mean age at diagnosis (in years) decreased from 39.6 ± 13.3 in Group A to 29.9 ± 9.3 in Group D, p less then 0.001. Pyramidal signs remained the most common manifestation regardless of the diagnostic criteria, although an increased trend of visual dysfunction was observed (A - 16%, B - 14%, C - 19%, D - 23,3%; A vs D, p less then 0.001). The number of relapses before diagnosis decreased from median 4.0 to 2.5 in Group A and Group D, p less then 0.001. Time from the first symptom to diagnosis shortened from 88.9 ± 80.2 months (Group A) to 33.6 ± 68.2 months (Group D), p less then 0.0001. Mean EDSS score at diagnosis also decreased A - 4.4 ± 2.3; B - 3.1 ± 1.7; C - 2.7 ± 1.3; D - 2.8 ± 1.4, p less then 0.001. Our study indicates significant differences in demographic and clinical characteristics of MS diagnosed according to the changing criteria.INTRODUCTION Marfan syndrome (MFS) and related connective tissue disorders (CTDs) are increasingly recognised. In this study we describe a multicentre cohort of adults with MFS and related CTDs, with a particular focus on results from genetic testing. METHODS All patients with MFS and related CTDs were identified from the databases of five centres in the canton of Zurich. Echocardiographic and clinical findings including systemic Marfan score, use of medication and genetic results were retrospectively analysed. MFS was diagnosed using the revised Ghent criteria (including FBN1 genetic testing if available); other CTDs (Loeys-Dietz syndrome) were diagnosed by genetic testing only. RESULTS A cohort of 103 patients were identified (62 index patients, 41 relatives of family members) 96 patients with MFS and 7 patients with other CTD, 54 males (52%), median age 23 years (range 1–75). The median systemic Marfan score was 5 fulfilling the revised Ghent criteria for MFS underwent genetic testing, often leading to or confirming the diagnosis of MFS. Other CTDs could be discriminated best by genetic testing. With respect to the diagnosis of MFS and related CTDs, the usefulness of the systemic score is limited, showing the importance of genetic testing, which enabled definitive diagnosis in 95% of tested patients. Patient education on medical treatment still has to be improved. Little evidence is available on their epidemiology, diagnosis and therapy. We aimed to evaluate current practices and provide recommendations. METHOD We systematically reviewed articles from PubMed, Embase, OVID-Medline and the Cochrane Library. RESULTS Forty-four publications reported pneumothoraces after ERCP in 49 patients (74% females). Twenty-one patients (43%) had atypical gastrointestinal anatomy, including peri-ampullary diverticula (n = 8), surgical or endoscopic alterations (n = 7), local tumours (n = 3), ulcerations (n = 2) and ectopic papilla (n = 1). Precut (14%) or standard (39%) sphincterotomies were performed. Cannulation was unsuccessful in six ERCPs. Pneumothoraces occurred bilaterally (45%), on the right (37%) or left side (6%), or no side was stated (12%). Nineteen tension pneumothoraces occurred. Perforations were mainly caused by the sphincterotome, peri-ampullary (41%), or the endoscope, distant from the papilla of Vater (23%). Pneumothoraces were sometimes diagnosed only after hospital discharge (14%). Eighteen patients underwent surgical therapy with drainage, repair and/or bypass. Conservative treatment included antibiotics and bowel rest. Most pneumothoraces were drained; 14% resolved without pleural drainage. CONCLUSIONS A post-ERCP pneumothorax should be considered when subcutaneous emphysema, cardiovascular instability or respiratory distress occurs. However, there is no evidence of the clinical superiority of prosthesis compared with other treatments. Disc prosthesis is associated with considerable risks of late severe complications, especially in revision surgery. The method has been suggested as a treatment for young active patients, which is highly questionable considering  the risk of late severe adverse events and complex revision surgery. Globally the method is declining and in many health care financing models it is not an approved treatment. In Sweden, the use of disc prosthesis is increasing.Importance Atrial fibrillation (AF) affects more than 6 million people in the United States; however, **** AF remains undiagnosed. Given that more than 265 million people in the United States own smartphones (>80% of the population), smartphone applications have been proposed for detecting AF, but the accuracy of these applications remains unclear. Objective To determine the accuracy of smartphone camera applications that diagnose AF. Data Sources and Study Selection MEDLINE and Embase were searched until January 2019 for studies that assessed the accuracy of any smartphone applications that use the smartphone's camera to measure the amplitude and frequency of the user's fingertip pulse to diagnose AF. Data Extraction and Synthesis Bivariate random-effects meta-analyses were constructed to synthesize data. The study followed the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) of Diagnostic Test Accuracy Studies reporting guideline. Main Outcomes and Measures Sensitivity and specificity were measured with bivariate random-effects meta-analysis.
