Cirrhotics often demonstrate worse outcomes than their non-cirrhotic counterparts following orthopedic surgery; however, there are limited arthroplasty-focused data on this occurrence. Additionally, variances in postoperative outcomes among the different etiologies of cirrhosis have not been well described. The aim of this study is to evaluate the effect compensated cirrhosis had on postoperative outcomes following elective total knee arthroplasty (TKA).
In total, 1,734,568 patients who underwent primary TKA from 2006 to 2013 were identified using the Medicare Claims Database. Patients were divided into those with a history of compensated cirrhosis and those with no history of liver disease. Subgroup analysis was performed based on the etiology of cirrhosis. Multivariate logistic regression was used to evaluate postsurgical outcomes of interest.
Cirrhotic patients had higher risk of developing disseminated intravascular coagulation (odds ratio [OR] 2.76, P= .003), encephalopathy (OR 3.00, P < .001), han viral infections and alcoholism are associated with few adverse outcomes. Surgeons should be aware of these complications to properly optimize postoperative management.
Sarcopenia is associated with adverse outcomes in clinical situations such as elderly population, in-hospital setting and oncologic patients. However, no direct measurement of muscular mass is routinely available for clinicians. The aim of this study was to assess the correlation between thenar musculature of the nondominant hand evaluated by ultrasound and body fat-free mass.
In this one-center, cross-sectional, observational study, the width and depth of thenar muscles of both hands was assessed by ultrasonography. Nondominant hand musculature was taken as reference as a better estimator of total body muscular mass. These data were compared to body composition by bioimpedance analysis and dual-energy X-ray absorptiometry (DXA), hand grip strength, arm muscular area and physical activity (with International Physical Activity Questionnaire ). Statistical correlation was determined for each parameter.

Simulation training has been strongly encouraged to enhance radiology trainees' procedural competency. We aimed to assess whether a cadaveric simulation training session was effective in improving radiology residents' subjective technical ability, knowledge and confidence in performing fluoroscopic-guided joint injections.
As part of the residency program's procedural training curriculum, first year radiology resident participated in a cadaveric, musculoskeletal injection training session including a didactic lecture followed by a practical hands-on component. Trainees performed fluoroscopic guided hip and shoulder injections on fresh cadavers, supervised by two fellowship-trained musculoskeletal radiologists. Trainees' knowledge on indications, contraindications, preprocedural care, complications, and technical ability in performing the procedures, as well as their rating of overall session experience were evaluated with pre- and post-session questionnaires (5 point Likert-scale). The mean residents' scoraining significantly improved trainees' subjective knowledge, confidence, and technical ability in performing joint injections.

The aim of this study was to explore the effectiveness of an oral hygiene program combined with home-phone health promoting counseling for rural adults with metabolic syndrome.
Metabolic syndrome is an important risk factor for cardiometabolic diseases. Evidence indicates that the poor oral hygiene and unhealthy lifestyles are closely related to metabolic syndrome.
A prospective quasi-experimental design involved a total of 136 community adults, with metabolic syndrome living in two rural townships, were clustered and conveniently assigned to intervention and control group between June 2017 and December 2018. The primary outcome was the change in cardiometabolic biomarkers. The secondary outcome comprised changes in the health-promoting behaviors. The intervention effect was assessed using a generalized estimating equation model.
There were 80 and 56 participants in the intervention and control group, respectively. The mean age was 56 years and ranged from 33 to 70 years. The intervention group exhibiing model worth to replicate and evaluate. Further study should consider innovative strategies to improve the awareness and adoption of a healthy lifestyle in residents with cardiometabolic risks.

To characterize reasons for hormonal contraceptive (HC) use in 1,455 African-American women, aged 23-35 years.
The community-based cohort members were recruited from the Detroit, Michigan area in 2010-2012. HC history was collected via telephone interview.
Seven percent reported HC use exclusively for non-contraceptive purposes, and 49% reported non-contraceptive reasons in addition to contraception. Non-contraceptive reasons were reported for all HC types, but were most common for combined oral contraceptives. Primary reasons were for irregular cycles, heavy bleeding, and pain.
In this large cohort of Black women, HC use to treat menstrual problems was common.

Because they induce alteration in carcass decomposition and succession of necrophagous Diptera, they are important investigative tool in forensic entomology for determining the postmortem interval (PMI) and cause of death. Thus, this work aimed to evaluate the influence of Terbufos, the most commonly used organophosphate for suicides in Rio Grande do Norte, Brazil, in the decomposition process and necrophagous fly assemblies in intoxicated rat carcasses. For this, 150g female Wistar rats received, by gavage, 200µL of Terbufos (5mg/kg or 10mg/kg) or distilled water (control). Approximately 30 minutes after administration, animals were euthanized and distributed in suspended traps to decompose under environmental conditions. Decomposition was monitored daily, with photographic record and collection of the visiting dipterofauna until the dry phase. Data show that the higher dose of Terbufos i) accelerates carcass decomposition in 24h; ii) decreases the species richness and abundance of scavengers flies when compared to the control; iii) changes the succession pattern, delaying the arrival of important species for the PMI estimate and iv) causes 8% mortality of the visiting dipterofauna. This work provides relevant information about dipteran assemblages and changes in the cadaver decomposition process associated with Terbufos intoxication, which may assist in future investigative processes with suspected organophosphate poisoning.Fibrosis is the final common pathological feature of a wide variety of chronic kidney disease (CKD). As the current focus of molecular research, noncoding RNAs (ncRNAs), mainly microRNAs (miRNAs), long noncoding RNAs (lncRNAs) and circular noncoding RNAs (circRNAs), have powerful and abundant biological functions, which essentially makes them mediators of the physiological and pathological processes of various system diseases. The role of ncRNAs in renal fibrosis has also received great attention in recent years, but most research has mainly focused on miRNAs. In fact, although a large number of studies of lncRNAs have emerged recently, the role these molecules play in renal fibrosis haven't been fully understood till now. Thus, this review discusses the discovery of lncRNAs and their biological functions in different types of renal fibrosis, as well as the imminent applications of these findings in clinical use. Fifty-five and 53 markers for Th. bessarabicum and S. anatolicum chromatin, respectively, were assigned to chromosomal location(s), covering all seven chromosomes. We used poetic inquiry to explore the lived realities of transwomen in Namibia, a socially marginalised group globally. We conducted two focus group discussions with 15 transwomen and developed found poems. The poems captured issues of identity and belonging and illustrated the transwomen's journeys of self-acceptance, experiences of violence, rejection, agency and relationships. In order to enhance participant voice and enrich the poems, they were shared with some transwomen in a feedback and reflection session. This article unpacks this collaborative poetics approach. We share selected poems, explore their impact on participants, and discuss how this engagement with the poetry allowed for deeper discussion of participants' experiences. The collaboration around the poems allowed researchers and participants to find new ways to explore and address issues of discrimination and marginalization, and to actively engage larger and more diverse audiences.Borrelia miyamotoi, a hard tick-borne relapsing fever agent, was sampled in Ixodes ricinus and Ixodes persulcatus ticks from the Tver province in Russia (a sympatric region of both tick species) and examined by TaqMan-PCR targeting the 16S rRNA gene. The Limpet II uses a hybrid electromagnetic module as its core module to allow adhesion and locomotion capabilities. The adhesion capability is based on negative pressure adhesion utilizing suction cups. The locomotion capability is based on slip-stick locomotion. The Limpet II also has a sensor payload with nine different sensing modalities, which can be used to inspect and monitor offshore structures and the conditions surrounding them. We used poetic inquiry to explore the lived realities of transwomen in Namibia, a socially marginalised group globally. We conducted two focus group discussions with 15 transwomen and developed found poems. The poems captured issues of identity and belonging and illustrated the transwomen's journeys of self-acceptance, experiences of violence, rejection, agency and relationships. In order to enhance participant voice and enrich the poems, they were shared with some transwomen in a feedback and reflection session. This article unpacks this collaborative poetics approach. We share selected poems, explore their impact on participants, and discuss how this engagement with the poetry allowed for deeper discussion of participants' experiences. The Limpet II uses a hybrid electromagnetic module as its core module to allow adhesion and locomotion capabilities. The adhesion capability is based on negative pressure adhesion utilizing suction cups. The locomotion capability is based on slip-stick locomotion. The Limpet II also has a sensor payload with nine different sensing modalities, which can be used to inspect and monitor offshore structures and the conditions surrounding them. Japanese encephalitis virus (JEV) is the leading cause of viral encephalitis in Asia. JEV infection of **** and humans can lead to an uncontrolled inflammatory response in the central nervous system (CNS), resulting in a detrimental outcome. Pigs act as important amplification and reservoir hosts, and JEV infection of pigs is mostly subclinical. Information on virus spread in the CNS and immune responses controlling JEV infection in the CNS of pigs, however remains scarce.
Nine-week-old pigs were inoculated intranasal or intradermal with a relevant dose of 10

Twenty-five adults with PPCS were assigned to the experimental group (n = 14) or the control group (n = 11). Both groups rode a recumbent elliptical machine for 21 min at moderate intensity (65% predicted maximum heart rate) twice a week for 6 weeks, but only the experimental group received BFR and ** while riding. The variances of overall PPCS scale scores and their sub-domain scores for individuals during the 6-week intervention and 6-week follow-up period were calculated. During the intervention, the fluctuation of overall symptom severity, severity in the cognitive domain and severity in the mood domain were significantly less in the experimental group (p = 0.03; p = 0.02; p = 0.02). During the follow-up period, the number of symptoms remained more stable in the experimental group (p = 0.02), and a trend toward less fluctuation of symptom severity (p = 0.05) was also observed. The reduced number of symptoms in the cognitive and sleep domains remained more stable in the experimental group following the intervention (p = 0.007; p = 0.02). The severity of mood and sleep symptoms also remained more stable during the follow-up period in the experimental group (p = 0.04). In each case, processing followed by storage induced phenolic acid accumulation in the carrots. It is important to consider that the gas volume recorded is in part directly produced from microbial fermentation of substrates, but also indirectly from the buffering capacity of the medium. Thus, the contribution of each (direct and indirect) gas source to the overall production should be estimated. Another major factor affecting fermentation is the rate of passage, but closed batch systems cannot be adapted to its consideration.

Biological material of the patients is stored in a biobank, but only a limited number of samples can be analyzed with respect to the marker. The question arises as to which patients to sample, if the number of events preclude standard sampling designs.
Modifications of the nested case-control and the case-cohort design for the proportional hazards model are applied, that allow efficient sampling in situations where standard nested case-control and case-cohort are not feasible. These sampling designs are compared to simple random sampling and extreme group sampling, the latter including only patients with extreme outcomes, ie either with an event early in time or without an event until at least a point later in time.
The modified nested case-control design and the modified case-cohort design provide powerful methods for sampling in a clinical cohort with many events. The simple random sampling usually is less efficient. If focus is on precise estimation of a potential effect in terms of a hazard ratio, extreme group sampling is not competitive. However, limited CO2 toxicity data could make it challenging for resource managers to characterize the potential risk to nontarget species during CO2 applications. The present study quantified the toxicity of CO2 to 2 native riverine fishes, bluegill (Lepomis macrochirus) and fathead minnow (Pimephales promelas), using 12-h continuous flow-through CO2 exposure at 5, 15, and 25 °C water temperatures. Resulting survival indicated that bluegill (median lethal concentration [LC50] range 91-140 mg/L CO2 ) were more sensitive to CO2 than fathead minnow (LC50 range 235-306 mg/L CO2 ) across all water temperatures. Bluegill were also more sensitive to CO2 at 5 °C (LC50 91 mg/L CO2 , 95% CI 85-96 mg/L CO2 ) than at 25 °C (LC50 140 mg/L CO2 , 95% CI 135-146 mg/L CO2 ). Fathead minnow showed an opposite response and were less sensitive at 5 °C (LC50 306 mg/L CO2 , 95% CI 286-327 mg/L CO2 ) relative to 25 °C (LC50 235 mg/L CO2 , 95% CI 224-246 mg/L CO2 ). A control method for the RBP should be validated in wider range of conditions to clarify its benefits and pitfalls prior to clinical application. In this study, we evaluated pulsatility and cardiac load reduction obtained with the CBS control on valvular failure conditions with a mathematical model. Diastolic assist could reduce cardiac load on the left ventricle by decreasing external work of the ventricle even in MR cases while it was not so effective in AR cases. In the subject-dependent baseline, the tracking errors were found to be generally consistent across all phonemes and subjects, with less than 25% of the errors reported to be greater than 5.8 mm (median 3.9 mm). However, the inter-subject variability showed that current limitations of our system resulted in appreciable errors (median 55 mm, Q3 65 mm).
The tracking errors reported in the subject-dependent case demonstrated the potential of our system to generate a baseline of phoneme landmarks. We have identified areas of improvement that will reduce the gap between the subject-dependent and universal baseline, while lowering tracking errors to be comparable to the gold standard.

Thirty adult, male Wistar rats weighing 250-300 g were selected as subjects. Rats were randomized into 3 groups group 1, sham; group 2, renal I/R injury; and group 3, renal I/R+Tramadol. In order to obtain ischemia in groups 2 and 3, renal artery was clamped for 1 h. Total oxidant status (TOS) and total antioxidant capacity (TAC) were analyzed using biochemical assays in the serum samples.
TOS values were measured as 1.68±0.4 in group 1, 3.35±1.0 in group 2, and 3.49±0.9 in group 3. When group 1 was compared with group 2 and group 3, the TOS values of group 1 were significantly lower (p<0.05), whereas there was no difference between group 2 and group 3 (p>0.05). TAC values were measured as 1.65±1.4 in group 1, 1.85±0.1 in group 2, and 2.79±0.6 in group 3. The antioxidant status of group 1 was not significantly different from that of group 2 (p>0.05), whereas there was a significant difference between group 1 and group 3 (p>0.05).

A database search of PubMed, Embase, and Scopus (up to January 2019) and the World Health Organization trial registry was performed to identify all studies assessing infertile men with EDO undergoing TURED. The primary outcome measures included semen parameters and natural pregnancies. The secondary outcomes included complications, symptomatic improvement, and a change from in vitro fertilization to intrauterine insemination.
Of 3,277 articles screened, 29 studies with 634 patients were included in the study. Although outcomes varied considerably among studies, a general increase in all semen parameters postoperatively was observed. Semen volume (n=23 studies) improved in a median of 83.0% of patients (interquartile range [IQR] 37.5). Sperm motility and concentration (n=10 and n=21 studies) improved in a median of 63.0% (IQR 15.0) and 62.5% (IQR 16.5) of patients, respectively. The natural pregnancy rate across the studies was a median of 25.0% (IQR 15.7). Improvements in both the outcomes were greater in patients with congenital etiologies and partial EDO. Differences in surgical technique did not appear to affect outcomes.

Microscope-assisted vasovasostomy (MAVV) is a standard procedure used to reverse vasectomies. Robotic surgery has been established primarily for technically demanding urological procedures and has also been recently implemented in male reproductive surgery. We aimed to review the current evidence of robot-assisted vasovasostomy (RAVV) and robot-assisted vasoepididymostomy (RAVE).
We performed a systematic literature review using PubMed to identify relevant original articles. We identified 2017 records through database search, and after removing duplicates, 782 records remained for further analysis.
In total, 12 human and three animal studies were selected. Reported vasal patency rate ranges were 88%-100% for RAVVs and 55%-61% for RAVEs. The sperm count and postoperative pregnancy rates of RAVV ranged between 8.4 × 10

ZnO quantum dots and CuFe2O4 nanoparticles were synthesized by chemical precipitation. The ZCF composite was created by the solvothermal method. We found that Fn-Rev-Ang rapidly crossed the BBB in **** and had drug-loading properties. Moreover, the brain MRI signal intensity ratio associated with Fn-Rev-Ang-GdDTPA was higher than that associated with Fn-GdDTPA alone. The explanations are based essentially on the experimental investigations of premixed biomass-derived gas explosions carried out in a rectangular half-open combustion chamber. The initial temperature T0 and pressure P0 were 300 K and 1.0 atm, respectively. The key parameters of explosive characteristics, such as flame propagation images and explosive overpressure, were analyzed by changing the porosity, the pore density of porous metal foams, and the gas components. The results show that the use of porous metal foam has a significant inhibitory effect on the gas explosion. Although the combustion structure of the flames is similar, the action of the porous metal foam during the experiment also shows the characteristics consistent with the obstacles. When the porosity of the porous foam is 97%, the flame can be stimulated to produce turbulence, and then the shock-flame interaction generated by the reflection of the lead shock wave can enhance the explosion propagation and promote the explosion escalation. However, with the increase of hole density, the existence of the porous metal foam by momentum loss and heat loss to curb the spread of the explosion not only hindered the flow of not flammable but also extracted energy from the expansion of the combustion products at the same time. In the case of fresh nonglutinous rice, the amounts of introduction for 4-vinylphenol and indole to GC were ca. 70 and 20 pg, respectively. While both peak areas decreased with increases in the time needed to maintain warmth, the decreasing behaviors differed slightly with a noteworthy rapid decrease for indole. For nonglutinous rice, the peak areas for 4-vinylphenol were almost the same, whether it was fresh (measured within 1 month from harvest) or aged (measured 6-12 months after harvest), but those of indole significantly decreased following storage. BACKGROUND Increasing studies have shown the important clinical role of immune and stromal cells in gastric cancer microenvironment. Based on information of immune and stromal cells in The Cancer Genome Atlas, this study aimed to construct a prognostic risk assessment model for gastric cancer. MATERIAL AND METHODS Based on the immune/structural scores, differentially expressed genes (DEGs) were filtered and analyzed. The aims of this study were to determine and improve accuracy of the Broselow Tape (BT) in estimating children's weight by adding body habitus parameters.
This cross-sectional study was conducted in an urban hospital pediatric clinic. Children up to 8 years old coming in for well-child visit were included. Children with acute illness or presence of any chronic condition potentially resulting in growth disturbance and out of BT height range were excluded. The following body habitus parameters were measured using the Centers for Disease Control and Prevention guidelines actual weight, predicted weight using BT (BTW), mid-thigh circumference (MTC), body mass index, mid-arm circumference, and waist-to-hip ratio.
A total of 301 children were enrolled. Of these, 151 were male (50%). Hispanics constituted 160 (53.2%). There was a positive linear association between BTW and actual weight in the overall cohort (adjusted R = 0.9164, P < 0.001). However, there was a difference in this association among children younger than 2 years and children older than 2 years (adjusted R = 0.89 vs 0.4841). Incorporating ****and/or waist circumference along with BTW in the model increased the accuracy, providing a better estimate of actual weight (adjusted R = 0.94, P < 0.001).
We conclude that there might be inaccuracies in the weight predicted by BT in our patient population, especially those weighing more than 15 kg and older than 2 years. Our study also demonstrates that ****correlates closely with the actual weight and could be used in addition to BT for more accurate weight estimation.

A video measuring machine (VMM) was used to measure the marginal gap. Statistical analysis was carried out using one-way ANOVA followed by paired t test and Tukey's test (α = .05).
The gap size was significantly different between the groups both before (P = .004) and after (P = .008) cementation, and it was significantly smaller in group O than group OSG (P = .002 before and P = .004 after cementation). Marginal gap increased in all groups following cementation (P < .001).

Summary statistics from genome-wide association studies for educational attainment, math ability, highest math class taken, cognitive performance, intelligence, and ALS were used. A two-sample Mendelian randomization (MR) design was applied to explore the potential causal associations between them.
Genetically predisposition to longer educational attainment and harder math class taken were associated with significantly lower statistical odds ratio of ALS. For per year increase in education completed there was a 21% lower (95% confidence interval [CI]=27-14%) in risk for ALS. For per 1-SD increase in the highest math class taken obtained there was a 19% lower (95% CI=9-28%) in risk for ALS. Genetically predisposition to math ability, cognitive performance, and intelligence did not decrease the risk of ALS.

Summary statistics from genome-wide association studies for educational attainment, math ability, highest math class taken, cognitive performance, intelligence, and ALS were used. A two-sample Mendelian randomization (MR) design was applied to explore the potential causal associations between them.
Genetically predisposition to longer educational attainment and harder math class taken were associated with significantly lower statistical odds ratio of ALS. For per year increase in education completed there was a 21% lower (95% confidence interval [CI]=27-14%) in risk for ALS. For per 1-SD increase in the highest math class taken obtained there was a 19% lower (95% CI=9-28%) in risk for ALS. Genetically predisposition to math ability, cognitive performance, and intelligence did not decrease the risk of ALS.
This study provides genetic support for a cauase, multiple sclerosis, or ALS.

Oncology faces an unprecedented challenge as patients require multidisciplinary care and are more likely to be immunosuppressed. Services in oncology have been transformed using minimal resources over a short period of time. This transformation continues and telemedicine is playing a key role.
We explore how services in oncology have transformed to deliver services including consultations, systemic anticancer therapy, and surgery for patients, while shielding them from contracting COVID-19. We assess the risks and benefits of the service transformation in the immediate, interim, and long term, and how telemedicine supports the process.

Four consultant radiologists, four consultant urologists, four urology trainees, and four nonclinician segmentation scientists were asked to segment a single slice of a lateral T3 prostate tumor on MRI ("Prostate 1"), an anterior zone prostate tumor MRI ("Prostate 2"), and a kidney tumor computed tomography (CT) scan ("Kidney"). Time taken and self-rated subjective accuracy out of a maximum score of 10 were recorded. Root mean square error, Dice coefficient, Matthews correlation coefficient, Jaccard index, specificity, and sensitivity were calculated using the radiologists as the ground truth.

Principal component analysis based on Jaccard distance of abundance showed 3 distinct outcomes relative to the source Gpx1/2-DKO microbiome. Association analyses of pathology and abundance of OTUs served to rule out 7-11 of 24 OTUs for involvement in the ileitis. Collections of OTUs were identified that appeared to be linked to ileitis in this animal model and would be classified as commensals. In Gpx1/2-DKO mice, host oxidant generation from NOX1 and DUOX2 in response to commensals may compromise the ileum epithelial barrier, a role generally ascribed to oxidants generated from mitochondria, NOX2 and endoplasmic reticulum stress in response to presumptive pathogens in IBD. Forty sound human maxillary first molars were collected and divided into four groups (n = 10) according to their preparation design for the fabrication of ZLS overlays (O) anatomical occlusal reduction, (OS) anatomical occlusal reduction with rounded shoulder, (OG) anatomical occlusal reduction with a central groove, and (OSG) anatomical occlusal reduction with rounded shoulder and central groove. A video measuring machine (VMM) was used to measure the marginal gap. Statistical analysis was carried out using one-way ANOVA followed by paired t test and Tukey's test (α = .05). The gap size was significantly different between the groups both before (P = .004) and after (P = .008) cementation, and it was significantly smaller in group O than group OSG (P = .002 before and P = .004 after cementation). Marginal gap increased in all groups following cementation (P < .001).
