As the scope of the 2019-nCoV infection further expands, it may produce different adaptive evolutions due to different environments. Finally, evolutionary genetic analysis can be a useful resource for studying the spread and virulence of 2019-nCoV, which are essential aspects of preventive and precise medicine.Worldwide, there have been few reports of β-lactamases causing penicillin resistance in Neisseria meningitidis. Computational approaches have become well-engrained in the processes of tailoring proteins for various biotechnological applications. Many tools and methods are developed and upgraded each year to satisfy the increasing demands and challenges of protein engineering. To help protein engineers and bioinformaticians navigate this emerging wave of dedicated software, we have critically evaluated recent additions to the toolbox regarding their application for semi-rational and rational protein engineering. These newly developed tools identify and prioritize hotspots and analyze the effects of mutations for a variety of properties, comprising ligand binding, protein-protein and protein-nucleic acid interactions, and electrostatic potential. Sci., 2019, 7, 4491-4502, DOI .In this paper, perfluorinated compounds (PFCs), such as perfluorobutyric acid (PFBA), perfluorooctanoic acid (PFOA) and perfluorododecanoic acid (PFDoA), were selected as typical representatives of perfluorinated carboxylic acids (PFCAs) to study the effects of PFCAs on the G protein-coupled estrogen receptor (GPER). The interaction mechanism of the three types of PFCAs with the GPER was investigated using steady-state fluorescence spectroscopy, ultraviolet-visible spectroscopy, three-dimensional fluorescence spectroscopy, and Fourier transform infrared spectroscopy combined with molecular docking and molecular dynamics simulations. Among these techniques, steady-state fluorescence and ultraviolet-visible spectroscopic analyses showed that PFBA, PFOA and PFDoA quenched the endogenous GPER fluorescence by combined dynamic and static quenching and non-radiative energy transfer. The binding constants (Ka) of PFCAs on the GPER were all larger than 105 L mol-1, indicating that their affinity for the GPER was strong. Fourier transform infrared spectroscopy and three-dimensional fluorescence showed that the secondary structure of the GPER changed after binding to PFCAs. Thermodynamic analysis showed ΔG 0, indicating that the interaction was mainly driven by hydrophobic forces; for the binding of PFDoA to the GPER, ΔH less then 0 and ΔS less then 0, suggesting that van der Waals force and hydrogen bonding were the main interaction forces. Molecular dynamics simulations suggested that the stability of the GPER-PFCA complexes was higher than that of the free GPER, and also that the structure and hydrophobicity of the GPER changed after binding to PFCAs. Molecular docking analysis showed that all three PFCAs could form hydrogen bonds with the GPER, which improved the stability of the complex.Stimuli-responsive polymers exhibit properties that make them ideal candidates for biosensing and molecular diagnostics. In this review, we highlight recent state-of-the-art examples of stimuli-responsive polymer-based systems for biosensing and bioimaging.Data-driven approaches have brought about a revolution in manufacturing; however, challenges persist in their applications to synthetic strategies. Their application to the deterministic navigation of reaction trajectories to stabilize crystalline solids with precise composition, atomic connectivity, microstructural dimensionality, and surface structure remains a frontier in inorganic materials research. The design of synthetic methodologies for the preparation of inorganic materials is often inefficient in terms of exploration of potentially vast design spaces spanning multiple process variables, reaction sequences, as well as structural parameters and reactivities of precursors and structure-directing agents. Reported synthetic methods are further limited in terms of the insight they provide into underlying chemical and physical principles. The recent surge in interest in accelerating the discovery of new materials can be considered as an opportunity to re-evaluate our approach to materials synthesis, and for considering new frameworks for exploration that are systematic and strategic in approach. Herein, we outline with the help of several illustrative examples, the challenges, opportunities, and limitations of data-driven synthesis design. The account collates discussion of design-of-experiments sampling methods, machine learning modeling, and active learning to develop experimental workflows that accelerate the experimental navigation of synthetic landscapes.Transforming flat two-dimensional (2D) sheets into three-dimensional (3D) structures by combining carefully made cuts with applied edge-loads has emerged as an exciting manufacturing paradigm in a range of applications from mechanical metamaterials to flexible electronics. In Kirigami, patterns of cuts are introduced that allow solid faces to rotate about each other, deforming in three dimensions whilst remaining planar. In other scenarios, however, the solid elements bend in one direction. In this paper, we model such bending deformations using the formulation of an elastic strip whose thickness and width are tapered (the 'tapered elastica'). We show how this framework can be exploited to design the tapering patterns required to create planar sheets that morph into desired axisymmetric 3D shapes under a combination of horizontal and vertical edge-loads. We exhibit this technique by recreating miniature structures with positive, negative, and variable apparent Gaussian curvatures. With sheets of constant thickness, the resulting morphed shapes may leave gaps between the deformed elements.

Robotic surgical technology has grown in popularity and applicability, since its conception with emerging uses in general surgery. The robot's contribution of increased stability and dexterity may be beneficial in technically challenging surgeries, namely, inguinal hernia repair. The aim of this project is to contribute to the growing body of literature on robotic technology for inguinal hernia repair (RIHR) by sharing our experience with RIHR at a large, academic institution. We performed a retrospective chart review spanning from March 2015 to April 2018 on all patients who had undergone RIHR at our university hospital. Extracted data include preoperative demographics, operative features, and postoperative outcomes. Data were analyzed with particular focus on complications, including hernia recurrence. A total of 43 patients were included, 40 of which were male. Mean patient age was 56 (range 18-85 years) and mean patient BMI was 26.4 (range 17.5-42.3). Bilateral hernias were diagnosed in 13 patients. All o discovered during a separate case and was repaired with temporary mesh. The use of the robot is safe and effective and should be considered an acceptable approach to inguinal hernia repair. Future prospective studies will help define which patients will benefit most from this technology.In the USA, the regulatory standard for demonstration of efficacy of a drug is evidence of clinical benefit from adequate and well-controlled clinical trials. Understanding the natural history of disease and how treatment is expected to alter its course, and gathering input from relevant stakeholders, such as patients, caregivers, and clinicians, is essential to understand the best way to measure clinical benefit in a clinical trial. Though pain intensity has been the primary outcome measure in clinical trials for pain, an array of measures assessing clinical outcomes from multiple perspectives can allow for more comprehensive interpretation of how a treatment affects patients' lives. Careful consideration should be given to how pain affects the feeling and functioning of each distinct patient population and which outcome assessment, or combination of outcome assessments, may be necessary to provide a more comprehensive view of the patient experience. The early stages of medical product development are an important opportunity to engage with regulatory agencies to discuss potential approaches to clinical trial design and outcome measurement strategies.
Lumbar drainage (LD) is one of the common treatment techniques in neurosurgery. There is a risk of secondary meningitis when using this modality. We aim to predict the probability of the complication by designing a nomogram.
A retrospective study was conducted in a teaching hospital. Data were collected and LD-related meningitis (LDRM) was identified, mainly based on clinical manifestations and cerebrospinal fluid analysis. Univariate analysis was used to screen the risk factors, and binary logistic analysis was performed to build the prediction model, which was furtherly transferred into a nomogram. The prediction performance was evaluated by receiver operating characteristic (ROC) curve, Hosmer-Lemeshow test, and nomogram calibration plot. Internal validation was processed by using ordinary bootstrapping.
A total of 273 patients who match the research criteria were enrolled, in which 37 cases (13.6%) were confirmed to have LDRM. Univariate analysis showed the risk factors included diabetes (p = 0.003), admission on surgical intensive care unit (p = 0.012), duration time (p < 0.001), site leakage (p < 0.001), and craniotomy (p < 0.001). In multivariate analysis, four of the variables were identified as independent risk factors to establish a prediction model, and a graphical nomogram was designed. The area under the ROC curve was 0.837, and the p value in the Hosmer-Lemeshow test was 0.610, with a mean absolute error in the calibration plot calculated as 0.022. The indices in the testing set were in good accordance with the original set when internal validation was performed.

While the expression of aquaporin-2 might be a key to response to tolvaptan, detailed mechanism of refractoriness to tolvaptan remains unknown. We experienced two patients with congestive heart failure and diabetic nephropathy, in whom the responses to tolvaptan were uniquely opposite. In one case, immunohistochemical staining showed expression of aquaporin-2 in the collecting duct despite severely reduced renal function, followed by the good response to tolvaptan with increased urine output. The subcutaneous injection of therapeutic monoclonal antibodies is increasingly used in the treatment of several diseases because of its convenience. Thus, a simple and accurate method of predicting the pharmacokinetics of monoclonal antibodies after a subcutaneous injection in humans would be a valuable tool for preclinical/clinical development. In this study, we investigated whether the pharmacokinetics of monoclonal antibodies after a subcutaneous injection in humans can be predicted using only pharmacokinetic data after a subcutaneous injection in cynomolgus monkeys.
First, we compared the accuracy of three approaches to predict the apparent clearance (CL/F) and apparent volume of distribution (V
/F) for 15 monoclonal antibodies in humans (1) allometric scaling from cynomolgus monkeys; (2) geometric mean of reported values in humans; (3) estimation from a regression line based on CL/F in humans [only V

We have previously demonstrated that ascorbic acid (AsA) deficiency causes inflammatory changes in the liver and intestine in Osteogenic Disorder Shionogi (ODS) rats, which are unable to synthesize AsA. We have suggested that AsA deficiency increased intestinal interleukine (IL)-6 production, stimulating hepatic acute phase proteins (APPs) expression via the portal vein. In this study, we determined whether these hepatic and intestinal inflammatory changes by AsA deficiency are induced in germ-free (GF) ODS rats. For 18 days, male specific pathogen-free (SPF) ODS rats were fed the basal diet containing 600 mg AsA/kg (control group) or the AsA-free diet (AsA-deficient group) in SPF conditions, while male GF ODS rats were fed the basal diet (control group) or the AsA-free diet (AsA-deficient group) in GF conditions. Firstly, AsA deficiency significantly elevated the hepatic expression of APPs in both SPF and GF rats. In hepatic mRNA levels of some APPs, significant interaction between GF and AsA-deficiency effects was observed. Secondly, AsA deficiency elevated intestinal IL-6 and IL-1β mRNA levels in both SPF and GF rats, and significant interaction between GF and AsA-deficiency effects was observed in these mRNA levels of jejunum and cecum. In SPF and GF rats, AsA deficiency elevated portal IL-6 concentration. These results show that AsA deficiency caused hepatic and intestinal inflammatory changes in both the GF and SPF ODS rats and indicate that AsA deficiency could directly induce intestinal inflammatory changes without the involvement of gut microbiota.We report the optimisation of a series of novel amide-piperidine (piperazine) derivatives using the multiple ligand approach with dopamine and serotonin receptors. Of the derivatives, compound 11 exhibited high affinity for the D2, 5-HT1A, and 5-HT2A receptors, but low affinity for the 5-HT2C and histamine H1 receptors and human ether-a-go-go-related gene (hERG) channels. In vivo, compound 11 reduced apomorphine-induced climbing, MK-801-induced hyperactivity and DOI-induced head twitching without observable catalepsy, even at the highest dose tested. In addition, it exhibited suppression in a CAR test. Furthermore, in a novel object recognition task, it displayed procognition properties. Therefore, compound 11 is a promising candidate multi-target antipsychotic.
To evaluate and refine a newly proposed factor structure for the Adherence Barriers to Continuous Positive Airway Pressure Questionnaire (ABCQ) and to present psychometric data from a large, geographically diverse sample of children and young adults with sleep disordered breathing (SDB) treated with positive airway pressure (PAP).
A sample of 181 patients prescribed PAP for treatment of SDB, ages 8-21 years, and caregivers (n=234) of patients ages 2-21 years, completed the ABCQ during routine sleep medicine clinic visits. Adherence data from participants' PAP machines were obtained via electronic download, providing objective data on PAP adherence immediately preceding the clinic visit during which the ABCQ was completed.
A three-factor structure (1. Behavior, Beliefs, Environment, 2. Emotional Barriers, & 3. Physical Barriers) exhibited good model fit in confirmatory factor analysis. Results indicate that the ABCQ has strong psychometric properties, including good internal consistency among subscales and strong convergent validity with objectively measured PAP adherence. Analysis of the Receiver Operator Characteristic Curve (ROC) yielded an ABCQ total cut-off score of 46.5 for patient report and 53.5 for caregiver report. Scores above the cutpoint predicted nonadherence to PAP, defined as failure to use PAP for ≥4h on 70% of nights.

This retrospective observational study was conducted from 11 March to 17 April 2020 at the only hospital authorized to manage patients with COVID-19 in Reunion Island.
Over the study period, 164 out of 398 patients (41.2%) infected with COVID-19 were admitted to Félix Guyon University Hospital. Of these, 36 (22%) developed hypoxemic pneumonia. The results exhibited well-synthesized GG-F and the manufactured GG-F scaffolds showed similar property of GG scaffolds which confirms that the chemical modification does not affect the property of GG scaffolds. The in vitro and in vivo study exhibited biocompatibility of the GG-F material. Overall, the properly blended GG-F in GG did not influence the characteristics of the pristine GG except for the chemical property.
We have previously demonstrated that ascorbic acid (AsA) deficiency causes inflammatory changes in the liver and intestine in Osteogenic Disorder Shionogi (ODS) rats, which are unable to synthesize AsA. We have suggested that AsA deficiency increased intestinal interleukine (IL)-6 production, stimulating hepatic acute phase proteins (APPs) expression via the portal vein. In this study, we determined whether these hepatic and intestinal inflammatory changes by AsA deficiency are induced in germ-free (GF) ODS rats. For 18 days, male specific pathogen-free (SPF) ODS rats were fed the basal diet containing 600 mg AsA/kg (control group) or the AsA-free diet (AsA-deficient group) in SPF conditions, while male GF ODS rats were fed the basal diet (control group) or the AsA-free diet (AsA-deficient group) in GF conditions. Firstly, AsA deficiency significantly elevated the hepatic expression of APPs in both SPF and GF rats. In hepatic mRNA levels of some APPs, significant interaction between GF and AsA-deficiency effects was observed. Secondly, AsA deficiency elevated intestinal IL-6 and IL-1β mRNA levels in both SPF and GF rats, and significant interaction between GF and AsA-deficiency effects was observed in these mRNA levels of jejunum and cecum. In SPF and GF rats, AsA deficiency elevated portal IL-6 concentration. These results show that AsA deficiency caused hepatic and intestinal inflammatory changes in both the GF and SPF ODS rats and indicate that AsA deficiency could directly induce intestinal inflammatory changes without the involvement of gut microbiota.We report the optimisation of a series of novel amide-piperidine (piperazine) derivatives using the multiple ligand approach with dopamine and serotonin receptors. Of the derivatives, compound 11 exhibited high affinity for the D2, 5-HT1A, and 5-HT2A receptors, but low affinity for the 5-HT2C and histamine H1 receptors and human ether-a-go-go-related gene (hERG) channels. In vivo, compound 11 reduced apomorphine-induced climbing, MK-801-induced hyperactivity and DOI-induced head twitching without observable catalepsy, even at the highest dose tested. The adaptive role of amphibian oocyte melanic pigmentation and its molecular control are still elusive. Here we present evidence of a polymorphism in egg pigmentation in the emerald glass frog Espadarana prosoblepon. In Ecuadorian natural populations of this species, females can lay dark brown or pale eggs that develop into normal pigmented tadpoles and adults. This trait is a sex-limited phenotype that is inherited like a recessive allele that we called pale eggs like (pel). The pel phenotype is exclusive of oocyte cortical melanic pigmentation, which is reduced in comparison to wild type (wt) dark pigmented oocytes. Consequently, pel early embryos are paler in appearance, with reduced melanic pigmentation distributed to early blastomeres and embryonic ectoderm. However, these embryos form normal melanocyte derived pigmentation. Finally, we discuss the origin of this polymorphism and propose the use of E. prosoblepon as a model to study the adaptive role of egg pigmentation.Fish present remarkable malleability regarding gonadal sex fate. This phenotypic plasticity enables an organism to adapt changes in the environment by responding with different phenotypes. The gonad and the brain present this extraordinary plasticity. In this review, we provide a broad and comparative view of tremendous radiation of sex determination mechanisms to direct gonadal fate. New results have established that the driving mechanism involves early response to environmental stressors by the brain plus high plasticity of gonadal differentiation and androgens as by-products of stress inactivation. In addition to the stress axis, another two major axes - the hypothalamic-pituitary-gonadal axis and the hypothalamic-pituitary-thyroid axis, which are well known to participate in the regulation of reproduction - have been proposed as reinforcing the brain-gonadal interrelationships in the fate of the gonad.Jorge E. He began a longstanding tradition by organizing advanced training courses for young scientists from the region who would not have otherwise had the opportunity to experience the latest methods and concepts in biological research, courses that had world leading researchers as instructors. A constant focus of his efforts consisted in promoting the establishment of postgraduate programs in biology throughout the continent, coordinating international funding programs aimed at scientific development in the third world and, more recently, advocating for science education among children and school teachers as the only way to achieve scientific literacy in our societies. In this interview, we explore how these issues were addressed by him and his counterparts in other Latin American countries, at a time when they had to start, essentially, from scratch.Alejandro Sánchez Alvarado represents a younger generation of Latin- American scientists that have achieved international scientific recognition (Fig. 1) .His work, together with those from other labs, has positioned the planaria Schmidtea mediterranea as a dynamic model system where the cellular and molecular basis of regeneration in metazoans can be probed. The most derived condition is direct development where the tadpole has been eliminated, and the most investigated direct developing frog is Eleutherodactylus coqui. To provide a different point-of-view, an imaginary interview with a coqui is conducted. He moved to the United Kingdom at age 18. This interview briefly explores his trajectory from Uruguay, through universities in the UK (Sussex, UCL, Cambridge and Oxford) and USA (Columbia) and how he was influenced by various mentors and experiences.Roberto Mayor is a prominent Chilean developmental biologist working in the UK and an advocate of the developmental biology discipline in Latin America. Roberto started as a preimplantation mouse developmental biologist during his undergraduate and graduate studies in Chile. Yet, he now uses Xenopus and zebrafish to elucidate the mechanisms that drive the directed collective locomotion of neural crest cells. What life events moulded the research career of Roberto across the years? This article addresses this question and provides a personal perspective on his scientific achievements. The story of Roberto is a mix of turns and cycles that ultimately guided him to the migrating neural crest. Turns that made him shift between model organisms and scientific topics.Cycles that drove him **** and forth between Chile and the UK and which have connected his early studies as an undergraduate student with the most recent work of his lab. For the first time, to the best of our knowledge, we propose a photonic fractional Fourier transformer (PFrFTer), which is used in chirp radar for detecting multiple non-cooperative targets. Based on photonic rotation of the time-frequency plane, the optimal fractional Fourier domain is formed, and the received broadband chirp signals are projected on it, where they behave as impulses. Moreover, through manipulating the fractional Fourier transform spectrum, the PFrFTer contributes to the cancellation of two ghost target sources, so that the ghost targets in multiple-target circumstances are removed. The simulation and experimental results show that the proposed PFrFTer can adapt to multiple non-cooperative targets environments and is immune to ghost targets at optimal working conditions, which agrees well with the theoretical analysis.We experimentally demonstrate a real-time quantum random number generator by using a room-temperature single-photon emitter from the defect in a commercial gallium nitride wafer. Due to the brightness of our single-photon emitter, the raw bit generation rate is about 1.8 MHz, and the unbiased bit generation rate is about 420 kHz after the von Neumann's randomness extraction procedure. Our results show that the commercial gallium nitride wafer has great potential for the development of integrated high-speed quantum random number generator devices.In this Letter, a deep learning solution (Y4-Net, four output channels network) to one-shot dual-wavelength digital holography is proposed to simultaneously reconstruct the complex amplitude information of both wavelengths from a single digital hologram with high efficiency. In the meantime, by using single-wavelength results as network ground truth to train the Y4-Net, the challenging spectral overlapping problem in common-path situations is solved with high accuracy.We propose a novel, to the best of our knowledge, cascade recurrent neural network (RNN)-based nonlinear equalizer for a pulse amplitude modulation (PAM)4 short-reach direct detection system. A 100 Gb/s PAM4 link is experimentally demonstrated over 15 km standard single-mode fiber (SSMF), using a 16 GHz directly modulated laser (DML) in C-band. The link suffers from strong nonlinear impairments which is mainly induced by the mixture of linear channel effects with square-law detection, the DML frequency chirp, and the device nonlinearity. Experimental results show that the proposed cascade RNN-based equalizer outperforms other feedforward or non-cascade neural network (NN)-based equalizers owing to both its cascade and recurrent structure, showing the great potential to effectively tackle the nonlinear signal distortion. With the aid of a cascade RNN-based equalizer, a bit-error rate (BER) lower than the 7% hard-decision forward error correction (FEC) threshold can be achieved when the receiver power is larger than 5 dBm. Compared with traditional non-cascade NN-based equalizers, the training time could also be reduced by half with the help of the cascade structure.A new, to the best of our knowledge, free-space resonant Sagnac interferometer scheme is proposed. Here, different methods to achieve this modulation are presented and compared.We propose and demonstrate a new, to the best of our knowledge, optical encoder, which can measure in-plane and out-of-plane displacements simultaneously and independently. The symmetrical structure of the optical path can eliminate the impact from out-of-plane displacement on the measurement of in-plane displacement. The innovative new geometry also facilitates the multi-reflected diffracted beam to interfere with the same-order diffracted beam, so as to eliminate the impact from in-plane displacement on the measurement of out-of-plane displacement. An experimental setup is established to verify the two-dimensional independent measurement. The experiment result coincides with the one measured by two independent interferometers. The output of spectrum analysis shows that the two-dimensional independent encoder can be used for nanometric measurement.We present a simple yet powerful technique to measure and stabilize the relative frequency noise between two lasers emitting at vastly different wavelengths. The noise of each laser is extracted simultaneously by a frequency discriminator built around an unstabilized ****-Zehnder fiber interferometer. Our protocol ensures that the instability of the interferometer is canceled and yields a direct measure of the relative noise between the lasers. As a demonstration, we measure the noise of a 895 nm diode laser against a reference laser located hundreds of nm away at 1561 nm. We also demonstrate the ability to stabilize the two lasers with a control bandwidth of 100 kHz using a Red Pitaya and reach a sensitivity of 1Hz2/Hz limited by detector noise. We independently verify the performance using a commercial frequency comb. This approach stands as a simple and cheap alternative to frequency combs to transport frequency stability across large spectral intervals or to characterize the noise of arbitrary color sources.
For the first time, to the best of our knowledge, we propose a photonic fractional Fourier transformer (PFrFTer), which is used in chirp radar for detecting multiple non-cooperative targets. Based on photonic rotation of the time-frequency plane, the optimal fractional Fourier domain is formed, and the received broadband chirp signals are projected on it, where they behave as impulses. Moreover, through manipulating the fractional Fourier transform spectrum, the PFrFTer contributes to the cancellation of two ghost target sources, so that the ghost targets in multiple-target circumstances are removed. The simulation and experimental results show that the proposed PFrFTer can adapt to multiple non-cooperative targets environments and is immune to ghost targets at optimal working conditions, which agrees well with the theoretical analysis.We experimentally demonstrate a real-time quantum random number generator by using a room-temperature single-photon emitter from the defect in a commercial gallium nitride wafer. Due to the brightness of our single-photon emitter, the raw bit generation rate is about 1.8 MHz, and the unbiased bit generation rate is about 420 kHz after the von Neumann's randomness extraction procedure. A 100 Gb/s PAM4 link is experimentally demonstrated over 15 km standard single-mode fiber (SSMF), using a 16 GHz directly modulated laser (DML) in C-band. The link suffers from strong nonlinear impairments which is mainly induced by the mixture of linear channel effects with square-law detection, the DML frequency chirp, and the device nonlinearity. Experimental results show that the proposed cascade RNN-based equalizer outperforms other feedforward or non-cascade neural network (NN)-based equalizers owing to both its cascade and recurrent structure, showing the great potential to effectively tackle the nonlinear signal distortion. With the aid of a cascade RNN-based equalizer, a bit-error rate (BER) lower than the 7% hard-decision forward error correction (FEC) threshold can be achieved when the receiver power is larger than 5 dBm. Compared with traditional non-cascade NN-based equalizers, the training time could also be reduced by half with the help of the cascade structure.A new, to the best of our knowledge, free-space resonant Sagnac interferometer scheme is proposed. This work shows that in common with woody biomass residues, carbonated agricultural biomass ash-based monoliths have potential as novel low-carbon construction products.We conducted a two-sample Mendelian randomization study to determine the association of smoking initiation with seven psychiatric disorders. We used 353 independent single-nucleotide polymorphisms associated with cigarette smoking initiation as instrumental variables at genome-wide significance threshold (p less then 5 × 10-8) from a recent genome-wide association study in 1,232,091 European-origin participants. Summary-level data for seven psychiatric disorders, including anxiety, bipolar disorder, insomnia, major depressive disorder, posttraumatic stress disorder, suicide attempts, and schizophrenia, was obtained from large genetic consortia and genome-wide association studies. The odds ratios of genetically predicted smoking initiation were 1.96 for suicide attempts (95% CI 1.70, 2.27; p = 4.5 × 10-20), 1.69 for post-traumatic stress disorder (95% CI 1.32, 2.16; p = 2.5 × 10-5), 1.54 for schizophrenia (95% CI 1.35, 1.75; p = 1.6 × 10-10), 1.41 for bipolar disorder (95% CI 1.25, 1.59; p = 1.8 × 10-8), 1.38 for major depressive disorder (95% CI 1.31, 1.45; p = 2.3 × 10-38), 1.20 for insomnia (95% CI 1.14, 1.25; p = 6.0 × 10-14) and 1.17 for anxiety (95% CI 0.98, 1.40; p = 0.086). Results of sensitivity analyses were consistent and no horizontal pleiotropy was detected in MR-Egger analysis. When analysed in a multivariate model with the other factors that independently predicted mortality in the sample (age, gender, self-rated health, smoking and plasma levels of glucose and adiponectin), increases in cf-DNA levels were associated with increased risk of mortality (hazard ratio [HR] for 0.1 µg increase in cf-DNA 1.017, 95% confidence interval [CI] 1.008-1.026, p = 0.0003). Inclusion of cf-DNA in the model improved the model fit and discrimination. We describe the pathological findings in autopsy and biopsy tissues submitted to the US Centers for Disease Control and Prevention (CDC) for the evaluation of suspected EVALI.
Between Aug 1, 2019, and Nov 30, 2019, we examined lung biopsy (n=10 individuals) and autopsy (n=13 individuals) tissue samples received by the CDC, submitted by 16 US states, from individuals with a history of e-cigarette, or vaping, product use; respiratory, gastrointestinal, or constitutional symptoms; and either pulmonary infiltrates or opacities on chest imaging, or sudden death from an undetermined cause. We also reviewed medical records, evaluated histopathology, and performed infectious dis individuals with suspected EVALI. Acute to subacute lung injury can have numerous causes; however, if it is identified in an individual with a history of e-cigarette, or vaping, product use, and no alternative cause is apparent, a diagnosis of EVALI should be strongly considered. A review of autopsy tissue pathology in suspected EVALI deaths can also identify alternative diagnoses, which can enhance the specificity of public health surveillance efforts.

Non-invasive tests that can identify patients with non-alcoholic steatohepatitis (NASH) at higher risk of disease progression are lacking. We report the development and validation of a blood-based diagnostic test to non-invasively rule in and rule out at-risk NASH (defined as non-alcoholic fatty liver disease [NAFLD] activity score [NAS] ≥4 and fibrosis stage ≥2).
In this prospective derivation and global validation study, blood samples, clinical data, and liver biopsy results from three independent cohorts with suspected NAFLD were used to develop and validate a non-invasive blood-based diagnostic test, called NIS4. Derivation was done in the discovery cohort, which comprised 239 prospectively recruited patients with biopsy-confirmed NASH (NAFLD NAS ≥3; fibrosis stage 0-3) from the international GOLDEN-505 phase 2b clinical trial. A complete matrix based on 23 variables selected for univariate association with the presence of at-risk NASH and avoiding high multi-collinearity was used to derive the model ostic that provides an effective way to non-invasively rule in or rule out at-risk NASH in patients with metabolic risk factors and suspected disease. Use of NIS4 in clinical trials or in the clinic has the potential to greatly reduce unnecessary liver biopsies in patients with lower risk of disease progression.

Staphylococcus aureus persistent bacteraemia is only vaguely defined and the effect of different durations of bacteraemia on mortality is not well established. Our primary aim was to analyse mortality according to duration of bacteraemia and to derive a clinically relevant definition for persistent bacteraemia.
We did a secondary analysis of a prospective observational cohort study at 17 European centres (nine in the UK, six in Spain, and two in Germany), with recruitment between Jan 1, 2013, and April 30, 2015. Adult patients who were consecutively hospitalised with monomicrobial S aureus bacteraemia were included. Patients were excluded if no follow-up blood culture was taken, if the first follow-up blood-culture was after 7 days, or if active antibiotic therapy was started more than 3 days after first blood culture. The primary outcome was 90-day mortality. Univariable and time-dependent multivariable Cox regression analysis were used to assess predictors of mortality. Duration of bacteraemia was define cutoff duration for persistent bacteraemia as 2 days or more despite active antibiotic therapy. Our results favour follow-up blood cultures after 24 h for early identification of all patients with increased risk of death and metastatic infection.

None.Anticipating and addressing the social implications of scientific work is a fundamental responsibility of all scientists. However, expectations for ethically sound practices can evolve over time as the implications of science come to be better understood. Contemporary researchers who work with ancient human remains, including those who conduct ancient DNA research, face precisely this challenge as it becomes clear that practices such as community engagement are needed to address the important social implications of this work. To foster and promote ethical engagement between researchers and communities, we offer five practical recommendations for ancient DNA researchers (1) formally consult with communities; (2) address cultural and ethical considerations; (3) engage communities and support capacity building; (4) develop plans to report results and manage data; and (5) develop plans for long-term responsibility and stewardship. Ultimately, every member of a research team has an important role in fostering ethical research on ancient DNA.Expanded carrier screening (ECS) for recessive monogenic diseases requires prior knowledge of genomic variation, including DNA variants that cause disease. The composition of pathogenic variants differs greatly among human populations, but historically, research about monogenic diseases has focused mainly on people with European ancestry. By comparison, less is known about pathogenic DNA variants in people from other parts of the world. Consequently, inclusion of currently underrepresented Indigenous and other minority population groups in genomic research is essential to enable equitable outcomes in ECS and other areas of genomic medicine. Here, we discuss this issue in relation to the implementation of ECS in Australia, which is currently being evaluated as part of the national Government's Genomics Health Futures Mission. We argue that significant effort is required to build an evidence base and genomic reference data so that ECS can bring significant clinical benefit for many Aboriginal and/or Torres Strait Islander Australians.

Between Aug 1, 2019, and Nov 30, 2019, we examined lung biopsy (n=10 individuals) and autopsy (n=13 individuals) tissue samples received by the CDC, submitted by 16 US states, from individuals with a history of e-cigarette, or vaping, product use; respiratory, gastrointestinal, or constitutional symptoms; and either pulmonary infiltrates or opacities on chest imaging, or sudden death from an undetermined cause. We also reviewed medical records, evaluated histopathology, and performed infectious dis individuals with suspected EVALI. Acute to subacute lung injury can have numerous causes; however, if it is identified in an individual with a history of e-cigarette, or vaping, product use, and no alternative cause is apparent, a diagnosis of EVALI should be strongly considered. A review of autopsy tissue pathology in suspected EVALI deaths can also identify alternative diagnoses, which can enhance the specificity of public health surveillance efforts. We report the development and validation of a blood-based diagnostic test to non-invasively rule in and rule out at-risk NASH (defined as non-alcoholic fatty liver disease [NAFLD] activity score [NAS] ≥4 and fibrosis stage ≥2). In this prospective derivation and global validation study, blood samples, clinical data, and liver biopsy results from three independent cohorts with suspected NAFLD were used to develop and validate a non-invasive blood-based diagnostic test, called NIS4. Derivation was done in the discovery cohort, which comprised 239 prospectively recruited patients with biopsy-confirmed NASH (NAFLD NAS ≥3; fibrosis stage 0-3) from the international GOLDEN-505 phase 2b clinical trial. Our primary aim was to analyse mortality according to duration of bacteraemia and to derive a clinically relevant definition for persistent bacteraemia. We did a secondary analysis of a prospective observational cohort study at 17 European centres (nine in the UK, six in Spain, and two in Germany), with recruitment between Jan 1, 2013, and April 30, 2015. Adult patients who were consecutively hospitalised with monomicrobial S aureus bacteraemia were included. Patients were excluded if no follow-up blood culture was taken, if the first follow-up blood-culture was after 7 days, or if active antibiotic therapy was started more than 3 days after first blood culture. The primary outcome was 90-day mortality. Univariable and time-dependent multivariable Cox regression analysis were used to assess predictors of mortality. Contemporary researchers who work with ancient human remains, including those who conduct ancient DNA research, face precisely this challenge as it becomes clear that practices such as community engagement are needed to address the important social implications of this work. To foster and promote ethical engagement between researchers and communities, we offer five practical recommendations for ancient DNA researchers (1) formally consult with communities; (2) address cultural and ethical considerations; (3) engage communities and support capacity building; (4) develop plans to report results and manage data; and (5) develop plans for long-term responsibility and stewardship. Ultimately, every member of a research team has an important role in fostering ethical research on ancient DNA.Expanded carrier screening (ECS) for recessive monogenic diseases requires prior knowledge of genomic variation, including DNA variants that cause disease. The composition of pathogenic variants differs greatly among human populations, but historically, research about monogenic diseases has focused mainly on people with European ancestry. By comparison, less is known about pathogenic DNA variants in people from other parts of the world. Consequently, inclusion of currently underrepresented Indigenous and other minority population groups in genomic research is essential to enable equitable outcomes in ECS and other areas of genomic medicine. Here, we discuss this issue in relation to the implementation of ECS in Australia, which is currently being evaluated as part of the national Government's Genomics Health Futures Mission. Chorea is defined by the presence of abnormal, involuntary, continuous, random movements that results from a number of autoimmune, hereditary, vascular, metabolic, drug-induced and functional (psychogenic) causes. Chorea may present at all stages of life, from newborns to elderly individuals. While Huntington disease is the main suspicion in adults presenting with chorea, once a drug-induced or parkinsonian dyskinesia have been ruled out; Huntington disease exceptionally presents with chorea in children. Sydenham chorea is considered the most common cause of acute childhood-onset chorea, but its prevalence has decreased in Western countries. However, in younger children other etiologies such as dyskinetic cerebral palsy, anti-NMDAR receptor encephalitis, other autoimmune conditions, or mutations in NKX2-1, ADCY-5, FOXG1, GNAO1, GPR88, SLC2A1, SQSTM1, ATP8A2, or SYT-1 should be considered. In this manuscript, we review the main causes, diagnosis and management of chorea in children.
Hypertensive urgency is associated with a high risk for cardiovascular events and mortality in the United States and Europe, but data from low-income countries and interventions to improve outcomes are lacking.
We conducted a 1-year prospective study of the prevalence and outcomes of hypertensive urgency (blood pressure (BP) ≥180 mm Hg/120 mm Hg without end-organ damage) in a busy outpatient clinic in Tanzania.
Of 7,600 consecutive adult outpatients screened with 3 unattended automated BP measurements according to standard protocol, the prevalence of hypertensive crisis was 199/7,600 (2.6%) (BP ≥180 mm Hg/120 mm Hg) and the prevalence of hypertensive urgency was 164/7,600 (2.2%). Among 150 enrolled patients with hypertensive urgency, median age was 62 years (54-68), 101 (67.3%) were women, and 53 (35%) were either hospitalized or died within 1 year. In a multivariate model, the strongest predictor of hospitalization/death was self-reported medication adherence on a 3 question scale (hazard ratio 0.06, P < 0.001); 90% of participants with poor adherence were hospitalized or died within 1 year.

Evidence on the association between living conditions and health outcomes among these populations are limited. This study investigated the risk factors associated with illnesses among camp-dwelling internally displaced persons (IDPs) in northern Nigeria. Methods A cross-sectional study was conducted in nine camps in 2016. Self-reported data on socio-demography, resource utilization and disease outcomes were collected. Association between health conditions and various factors, including sanitation and healthcare access, was investigated. Results Data from 2253 IDPs showed 81.1% (CI = 79.5-82.7) experienced one or more health conditions; however, over 20% did not access healthcare services. Most common diseases were malaria, fever, typhoid and diarrhoea. Multivariable logistic regression presented as adjusted odds ratios(aOR) and 95% confidence intervals(CIs) showed factors significantly associated with increased likelihood of illnesses included being female (aOR = 1.53;CI = 1.19-1.96), overcrowding (aOR = 1.07;CI = 1.00-1.36), long-term conditions (aOR = 2.72;CI = 1.88-3.94), outdoor defecation (aOR = 2.37;CI = 1.14-4.94) and presence of disease-causing vectors (aOR = 3.71;CI = 1.60-8.60). Conclusion Most diseases in the camps were communicable. Modifiable risk factors such as overcrowding and poor toilet facilities were associated with increased poor health outcomes. Non-leaflet resection techniques including loop chordal replacement are being used with increasing frequency, but the long-term results of these techniques are still unknown. The aim of this study was to compare the long-term results of loop neochord replacement with leaflet resection techniques in patients undergoing minimally invasive mitral valve (MV) repair for MV prolapse.
Between 1999 and 2014, 2134 consecutive MV prolapse patients underwent minimally invasive MV repair with isolated loop (n = 1751; 82.1%) or resection techniques (n = 383, 17.9%) at our institution. Follow-up data were available for 86% of patients with a mean follow-up time of 6.1 ± 4.3 years.
The 30-day mortality was 0.8% for all patients (loop 0.7%, resection 1.6%; P = 0.09). Leaflet resection was associated with more moderate or more mitral regurgitation on predischarge echocardiography (P = 0.003). The 1-, 5- and 10-year survival rates were 98 ± 1%, 95 ± 1% and 86 ± 2% for the loop technique versus 97 ± 1%, 92 ± 1% and 81 ± 2% for resection patients, respectively (P = 0.

To determine the role of magnetic resonance imaging (MRI) MOCART (Magnetic Resonance Observation of Cartilage Repair Tissue) 1 and 2.0 scores in the assessment of postoperative outcome after autologous matrix-induced chondrogenesis (AMIC) for the treatment of osteochondral lesions of the talus (OLTs). It was hypothesized that preoperative patient factors or OLT morphology are associated with postoperative MOCART scores; yet postoperative clinical outcome is not.

This exploratory qualitative study seeks to understand providers' perspectives on international adoptee and family preparation and adjustment to life in the U.S. Eleven psychosocial and five medical care providers participated in hour-long, semi-structured, recorded interviews focused on their experiences caring for internationally adopted children with HIV (IACH) and their adoptive parents. Transcribed interviews were analyzed to identify emergent themes. Providers described considerable variation among families who adopted children. Some had grown biological children and several adopted multiple children with special needs. Most were connected to communities of faith which served as an inspiration to adopt and offered support. Serious medical and HIV-related issues were minimal. Psychosocial concerns were more common and included attachment, adjustment, and behavioral issues. Participants noted that adoptive parents were well informed about HIV, but less prepared for cognitive delays and emotional challenges. Some providers experienced or expected to have challenges offering sexual and reproductive health education to adolescents due to their adoptive parents' religious beliefs on sexuality. Additional support managing behavioral and emotional challenges, as well as sexual education, may be needed especially as IACH transition into adolescence and young adulthood.Metal complexes have extensive applications in catalysis, however, the efficient evaluation of Lewis acidity of metal complexes is still a challenge. Herein, we report a method by using electrospray ionization mass spectrometry (ESI-MS) to evaluate the Lewis acidity of metal complexes in the presence of a reference Lewis base, in which the value of the Lewis acidity can be quantized by the bond dissociation energy (BDE) of the resultant Lewis acid-base pairs. Using this method, the Lewis acidity of tetradentate Schiff-base metal complexes (designated as salenMX), a class of common metal complexes in the homogeneous catalysis, was studied in detail. For the salenM(III)X complexes (M = Al, Cr, Fe, Co), the Lewis acidity tendency is Al > Cr > Fe > Co due to a strong affinity between the Al complex and the reference Lewis base while a weak affinity concerning on the Co complex. Additionally, the effect of ligand steric and electronic nature on the Lewis acidity was studied by using Co complex. Furthermore, density functional theory (DFT) was employed to calculate the BDE, which consists with the results obtained from ESI-MS. This is the first study to examine the impact of posttraumatic stress disorder (PTSD), major depressive disorder (MDD), insomnia, and substance use disorders on suicidal ideation/behavior among PLWH using electronic medical record (EMR) data. We also evaluated the mutual influence of interactions between PTSD and substance use disorders on suicide risk, consistent with the substance abuse, violence exposure, and HIV/AIDS "SAVA" syndemic model. Participants (n = 2336) were HIV+ individuals recruited through the Center for AIDS Research (CFAR). Participants provided informed consent for extraction of their EMR. Some providers experienced or expected to have challenges offering sexual and reproductive health education to adolescents due to their adoptive parents' religious beliefs on sexuality. Additional support managing behavioral and emotional challenges, as well as sexual education, may be needed especially as IACH transition into adolescence and young adulthood.Metal complexes have extensive applications in catalysis, however, the efficient evaluation of Lewis acidity of metal complexes is still a challenge. Herein, we report a method by using electrospray ionization mass spectrometry (ESI-MS) to evaluate the Lewis acidity of metal complexes in the presence of a reference Lewis base, in which the value of the Lewis acidity can be quantized by the bond dissociation energy (BDE) of the resultant Lewis acid-base pairs. Using this method, the Lewis acidity of tetradentate Schiff-base metal complexes (designated as salenMX), a class of common metal complexes in the homogeneous catalysis, was studied in detail. For the salenM(III)X complexes (M = Al, Cr, Fe, Co), the Lewis acidity tendency is Al > Cr > Fe > Co due to a strong affinity between the Al complex and the reference Lewis base while a weak affinity concerning on the Co complex. Additionally, the effect of ligand steric and electronic nature on the Lewis acidity was studied by using Co complex. Furthermore, density functional theory (DFT) was employed to calculate the BDE, which consists with the results obtained from ESI-MS. The ESI-MS method provides a convenient and efficient method for evaluating the Lewis acidity of metal complexes.Persons living with HIV (PLWH) are significantly more likely to die by suicide compared to the general population. This is the first study to examine the impact of posttraumatic stress disorder (PTSD), major depressive disorder (MDD), insomnia, and substance use disorders on suicidal ideation/behavior among PLWH using electronic medical record (EMR) data. We also evaluated the mutual influence of interactions between PTSD and substance use disorders on suicide risk, consistent with the substance abuse, violence exposure, and HIV/AIDS "SAVA" syndemic model. Participants (n = 2336) were HIV+ individuals recruited through the Center for AIDS Research (CFAR). Participants provided informed consent for extraction of their EMR.
