Geographical access to surgical facilities is 1 of the 6 indicators of the Lancet Commission on Global Surgery and has been measured extensively worldwide. Our objective is to determine the population with 2-h geographical access to facilities capable of performing the Bellwether procedures (laparotomy, cesarean delivery, and open fracture repair). This is the first study in Mexico to assess access to surgical facilities, including both the fragmented public sector and the private sector.

    In this cross-sectional study, conducted from June 2019 to January 2020, Bellwether capable surgical facilities from all health systems in Chiapas were geocoded and assessed through on-site data collection, Ministry of Health databases, and verified via telephone. Geospatial analyses were performed on Redivis.

    We identified 59 Bellwether capable hospitals, with 17.5% (n = 954,460) of the state residing more than 2h from surgical care in public and private health systems. Of those, 22 facilities had confirmed 24/7 Bellwether capability, and 23% (n = 1,178,383) of the affiliated population resided more than 2h from these hospitals.

    Our study shows that the Ministry of Health and employment-based health coverage could provide timely access to essential surgical care for the majority of the population. However, the fragmentation of the healthcare system leaves a gap that contributes to delays in care and unmet emergency surgical needs.

    To analyze the efficiency and cost-utility profile of ureteroscopy versus shock wave lithotripsy for treatment of reno-ureteral stones smaller than 2cm.

    Patients treated for urinary stones smaller than 2cm were included in this study (n = 750) and divided into two groups based on technique of treatment. To assess the cost-utility profile a sample of 48 patients (50% of each group) was evaluated.Quality of life survey (Euroqol 5QD-3L) before-after treatment was applied, Markov model was designed to calculate quality of life in each status of the patients (stone or stone-free with and without double-J stent) and to estimate the incremental cost-utility. Monte carlo simulation was conducted for a probabilistic sensitivity analysis. Chi-square was used for comparing qualitative variables and T student's for continuous variables.

    Shock wave lithotripsy group had 408 (54.4%) and ureteroscopy group had 342 (45.6%) patients. Of them, 56.3% were treated for renal stones and 43.7% for ureteral stones. Ureteroscopy produced slightly higher overall quality of patients' life, but produced a significant higher overall cost per quality-adjusted life year (QALY) than shock wave lithotripsy, exceeding the cost-utility threshold (20,000€/QALY). Sensitivity analysis confirmed results in 93.65% of cases. Difference was maintained in subgroup analysis (ureteral vs renal stones).

    Renal cell carcinoma (RCC) incidence has considerably increased during the last decades without any real impact on age-standardized mortality. It questions the relevance of aggressive treatments carrying potential side effects. Conservative management should be considered for frail patients. Comorbidity and frailty assessment in RCC patients is paramount before engaging a treatment.

At the metastatic stage, drug combinations based on immunotherapies and targeted therapies improved cancer outcomes at the price of significant toxicities. Frail patients are not always suitable for such strategies. Commonly used scores like the International Metastatic RCC Database Consortium or Memorial Sloan Kettering Cancer Center integrate features to define patients' risk groups, more specifically the Karnofsky Performance Score is an easy way to document the frailty.

As a consequence, PBK promotes the enrichment of MSL complex on CD276 promoter, leading to the increased histone H4 K16 acetylation and the activation of CD276 transcription. In addition, we show that CD276 is highly upregulated and associated with immune infiltrating levels in NPC. The role of HDAC2 on ULK1-NLRP3-pyroptosis pathway during ALF was detected in clinical samples. The mechanism was investigated in transfected cells or in ALF mouse model. The RNA-sequencing results revealed that ULK1 was a negative target regulatory molecule by HDAC2. During the process of pyroptosis, the HDAC2 exerted the antagonistic effect with ULK1 by the K68 acetylation site in L02 cells. Then the role of HDAC2 on ULK1-NLRP3-pyroptosis pathway in ALF mouse model was also detected. These models outperformed humans on average, including predictions by experienced psychiatrists.     Neurovascular injury is a critical complication in total hip arthroplasty (THA). However, neurovascular geographic variations around the hip joint in different body positions have not been examined. This study investigated the differences in hip neurovascular geography in the supine and lateral positions using magnetic resonance imaging (MRI).

    This was a single-center prospective study of 15 healthy volunteers enrolled between January 2018 and March 2019. Each subject's bilateral hips were scanned with a 3-T MRI scanner in both the supine and lateral positions. In T1-weighted axial images at the level of the hip center, the anterior and posterior acetabular edges were defined as reference points at which retractors are commonly placed during surgery. We measured the distance between the anterior acetabular edge and the femoral nerve (dFN), femoral artery (dFA), and femoral vein (dFV), as well as that between the posterior acetabular edge and the sciatic nerve (dSN). The primary outcome measures were the distances in both the supine and lateral positions.

    dFN, dFA, and dFV in the supine and lateral positions (mm, mean±standard deviation) were 25.8±5.6 and 32.4±6.4 (p<0.0001), 25.7±4.5 and 32.2±5.0 (p<0.0001), and 26.5±4.8 and 32.3±5.1 (p<0.0001), respectively. Most of these elements moved anteromedially in the lateral position compared to the supine position. There was no significant difference in dSN between the supine and lateral positions (23.7±4.9 and 24.5±6.5 (p=0.46).

Typical symptoms of endometriosis were experienced by 76.8% general practitioners, but only 36% "often" or "always" referred to endometriosis when faced with these symptoms.     Medical malpractice is a very common occurrence that many medical providers will have to face; approximately 17,000 medical malpractice cases are filed in the United States each year, and more than 99% of all surgeons are faced with at least 1 instance of malpractice litigation throughout their careers. Malpractice litigation also carries a major economic weight, with medical malpractice spending resulting in an aggregate expenditure of nearly $60 billion annually in the United States. Orthopedic surgery is one of the most common subspecialties involved in malpractice claims. Currently, there are no comprehensive studies examining malpractice lawsuits within shoulder and elbow surgery. Therefore, the purpose of this work is to examine trends in malpractice claims in shoulder and elbow surgery.

    Acute renal rejection usually fails to be diagnosed before the increase in the serum creatinine levels, and the resultant damage to the renal tissues occur in varying degrees. We hypothesized that the combined detection of human leucocyte antigen-G (HLA-G) 14-bp insertion/deletion genotypes and kidney injury molecule-1 (KIM-1) and osteopontin (OPN) levels in serum might facilitate the prediction of acute renal allograft rejections in kidney transplant recipients.

    HLA-G 14-bp insertion/deletion genotypes and the serum KIM-1 and OPN levels of 77 kidney transplant recipients were determined and compared before operation and on days 1, 4, and 7 after the operation (32 in acute rejection [AR] group and 45 in stable allograft function [STA] group). The main aim of the present study is to highlight the importance of integrating different characterization techniques to describe the potential release of airborne graphene flakes in a graphene processing and production research laboratory. Specifically, the production and processing (i.e., drying) of few-layer graphene (FLG) through liquid-phase exfoliation of graphite are analysed by integrated characterization techniques. For this purpose, the exposure measurement strategy was based on the multi-metric tiered approach proposed by the Organization for Economic Cooperation and Development (OECD) via integrating high-frequency real-time measurements and personal sampling. Particle number concentration, average diameter and lung deposition surface area time series acquired in the worker's personal breathing zone (PBZ) were compared simultaneously to background measurements, showing the potential release of FLG. However, wetlands in the tropics and subtropics have been invaded by smooth cordgrass (Spartina alterniflora) affecting storage of blue C. To understand how S. alterniflora affects soil organic carbon (SOC) stocks, sources, stability, and their spatial distribution, we sampled soils along a 2500 km coastal transect encompassing tropical to subtropical climate zones. This included 216 samplings within three coastal wetland types a marsh (Phragmites australis) and two mangroves (Kandelia candel and Avicennia marina). Using δ13 C, Cnitrogen (N) ratios, and lignin biomarker composition, we traced changes in the sources, stability, and storage of SOC in response to S. alterniflora invasion. The contribution of S. alterniflora-derived C up to 40 cm accounts for 5.6%, 23%, and 12% in the P. australis, K. candel, and A. marina communities, respectively, with a corresponding change in SOC storage of +3.5, -14, and -3.9 t C ha-1 . SOC storage did not follow the trend in aboveground biomass from the native to invasive species, or with vegetation types and invasion duration (7-15 years). SOC storage decreased with increasing mean annual precipitation (1000-1900 mm) and temperature (15.3-23.4℃). Edaphic variables in P. australis marshes remained stable after S. alterniflora invasion, and hence, their effects on SOC content were absent. In mangrove wetlands, however, electrical conductivity, total N and phosphorus, pH, and active silicon were the main factors controlling SOC stocks. Mangrove wetlands were most strongly impacted by S. alterniflora invasion and efforts are needed to focus on restoring native vegetation. By understanding the mechanisms and consequences of invasion by S. alterniflora, changes in blue C sequestration can be predicted to optimize storage can be developed.
    To explore the perspectives of people with dementia on being cared for by others, on the future and on the end of life, and to evaluate the capability and willingness of participants to have these conversations.

    Awareness about perspectives of people with dementia should decrease stigmatisation and improve their quality of life. Applying palliative care principles from an early stage is important to address diverse needs and to anticipate the future. Few studies investigate perspectives of people with dementia regarding palliative care, including advance care planning.

    Qualitative descriptive design.

    We performed in-depth interviews with 18 community-dwelling persons with dementia in South-Limburg, the Netherlands. Transcripts were analysed using an inductive content analysis. Two authors coded the data and regularly compared coding. All authors discussed abstraction into categories and themes. We followed the COREQ reporting guidelines.

    Five overarching themes derived from the interviews were as folingness of people with dementia to discuss the future and end-of-life topics. Public and professional awareness may facilitate opportunities for informal end-of-life discussions. Healthcare professionals should promote belongingness of persons with dementia and strive to build equal, trustful care relationships with them and their families.

The α1,3-galactosyltransferase gene-knockout (GalT KO) mice are able to produce natural anti-αGal antibodies apparently without any specific immunization. GalT KO mice are commonly used as a model immunological system for studying anti-αGal responses to Gal-positive xenografts in human. In this study, we compared the specificity of mouse and human αGal antibodies to realize the adequacy of the murine model. Using hapten-specific affinity chromatography antibodies against Galα1-3Galβ1-4GlcNAcβ epitope were isolated from both human and GalT KO mice blood sera. Specificity of isolated antibodies was determined using a printed glycan array (PGA) containing 400 mammalian glycans and 200 bacterial polysaccharides. However, wetlands in the tropics and subtropics have been invaded by smooth cordgrass (Spartina alterniflora) affecting storage of blue C. To understand how S. alterniflora affects soil organic carbon (SOC) stocks, sources, stability, and their spatial distribution, we sampled soils along a 2500 km coastal transect encompassing tropical to subtropical climate zones. This included 216 samplings within three coastal wetland types a marsh (Phragmites australis) and two mangroves (Kandelia candel and Avicennia marina). Using δ13 C, Cnitrogen (N) ratios, and lignin biomarker composition, we traced changes in the sources, stability, and storage of SOC in response to S. alterniflora invasion. The contribution of S. alterniflora-derived C up to 40 cm accounts for 5.6%, 23%, and 12% in the P. australis, K. candel, and A. marina communities, respectively, with a corresponding change in SOC storage of +3.5, -14, and -3.9 t C ha-1 . SOC storage did not follow the trend in aboveground biomass from the native to invasive species, or with vegetation types and invasion duration (7-15 years). SOC storage decreased with increasing mean annual precipitation (1000-1900 mm) and temperature (15.3-23.4℃). Edaphic variables in P. australis marshes remained stable after S. alterniflora invasion, and hence, their effects on SOC content were absent. In mangrove wetlands, however, electrical conductivity, total N and phosphorus, pH, and active silicon were the main factors controlling SOC stocks. Mangrove wetlands were most strongly impacted by S. alterniflora invasion and efforts are needed to focus on restoring native vegetation. By understanding the mechanisms and consequences of invasion by S. alterniflora, changes in blue C sequestration can be predicted to optimize storage can be developed. Few studies investigate perspectives of people with dementia regarding palliative care, including advance care planning. Qualitative descriptive design. We performed in-depth interviews with 18 community-dwelling persons with dementia in South-Limburg, the Netherlands. Transcripts were analysed using an inductive content analysis. Two authors coded the data and regularly compared coding. All authors discussed abstraction into categories and themes. We followed the COREQ reporting guidelines. Five overarching themes derived from the interviews were as folingness of people with dementia to discuss the future and end-of-life topics. Public and professional awareness may facilitate opportunities for informal end-of-life discussions. Healthcare professionals should promote belongingness of persons with dementia and strive to build equal, trustful care relationships with them and their families. Administrative health databases (AHD) are critical to guide health service management and can inform the whole spectrum of substance-related disorders (SRD). This study estimates prevalence and mortality rates of SRD in administrative health databases. The Quebec Integrated Chronic Disease Surveillance System consists of linked AHD. Analyses were performed on data of all Quebec residents aged 12 and over and eligible for health-care coverage using the International Classification of Diseases (ninth or tenth revision) for case identification. Mortality rate ratios stratified by causes of death were obtained to calculate an excess of mortality. Since 2001-2002, the annual age-adjusted prevalence rate of diagnosed overall SRD remained stable (8.6 per 1000 in 2017-2018). This study detected land surface deformations in a form of an extensive subsidence bowl (with 28.5 km in diameter) with a maximum subsidence rate of 40 mm/year and a standard deviation within the bowl of less than 2 mm/year. The detected subsidence was associated with a 12 m drop in the water table level within the study area. The Persistent Scatterers with the highest deformations rate were spatially correlated with the depression cone of the groundwater level. These findings provide useful insights in understanding the groundwater regime of the area and have an important role in assessing regional hazards and driving mitigation measures towards managing uncontrolled groundwater overexploitation for sustainable management of groundwater resources.Poor air quality disproportionally impacts cities in low- and middle-income countries. The analysis finds that resuspended dust from unpaved roads is the largest local source of PM2.5 and can contribute over 30% of seasonally-averaged concentration across the city. Vehicles, industrial activity, and unpaved road dust combined are responsible for over 60% of PM2.5 pollution in Bogotá. A scenario analysis shows that paving roads can lead to PM2.5 decreases of nearly 10 μg/m3 by 2030 in some areas of the city, but air quality will deteriorate significantly over others in the absence of additional emissions control measures. Mitigation strategies designed to target the sectors with the largest contributions to PM2.5, including road cleaning systems, controls for industrial point sources, cleaner transportation fuels, and updated vehicle fleets, can largely avert projected increases in concentrations, although the impacts of different approaches vary throughout the city. This study is the first to use a comprehensive model to determine sector contributions to air pollution and inform potential emissions control policies in Bogotá, demonstrating an approach to guide pollution management in developing cities facing comparable challenges.Grazing exclosures have been promoted as an effective and low-cost land management strategy to recover vegetation and associated functions in degraded landscapes in the tropics. To address this gap, we assessed the effect of diversity on aboveground carbon (AGC) and the relative importance of the driving biotic (functional diversity, functional composition and structural diversity) and abiotic (climate, topography and soil) mechanisms. We used a dataset from 133 inventory plots across three altitudinal zones, i.e., highland, midland and lowland, in northern Ethiopia, which allowed local- (within altitudinal zone) and broad- (across altitudinal zones) environmental scale analysis of diversity-AGC relationships. We found that species richness-AGC relationship shifted from neutral in highlands to positive in mid- and lowlands as well as across the altitudinal zones. Structural diversity was consistently the strongest mediator of the positive effects of species richness on AGC within and across altitudinal zones, whereas functional composition linked species richness to AGC at the broad environmental scale only. Abiotic factors had direct and indirect effects via biotic factors on AGC, but their relative importance varied with altitudinal zones. Our results indicate that the effect of species diversity on AGC was altitude-dependent and operated more strongly through structural diversity (representing niche complementarity effect) than functional composition (representing selection effect). Our study suggests that maintaining high structural diversity and managing functionally important species while promoting favourable climatic and soil conditions can enhance carbon storage in grazing exclosures.

    To check the prices, availability, and affordability of the World Health Organization (WHO) key access antibiotics in private sector pharmacies of Lahore, Pakistan.

    A survey of WHO key access antibiotics from WHO essential medicine list 2017 was conducted in private sector pharmacies of 4 different regions of Lahore employing adapted WHO/HAI methodology. https://www.selleckchem.com/products/Celastrol.html The comparison of prices and availability between originator brands (OB) and lowest price generics (LPG) were conducted followed by the effect of medicine price differences on patient's affordability. The data were analyzed using a preprogrammed WHO Microsoft excel workbook.

    The mean availability of OB products was 45.20% and the availability of LPGs was 40.40%. The OBs of co-amoxiclav, clarithromycin and metronidazole and LPGs of azithromycin and ciprofl purchasing efficiency, excluding taxes and regulating mark-ups. This could increase the affordability of patients to complete their antibiotic therapy with subsequent reduction in antimicrobial resistance.
    There is dire need to properly implement price control policies to better regulate fragile antibiotic supply system so that the availability of both OB and LPG of key access antibiotics should be increased. The prices could be reduced by improving purchasing efficiency, excluding taxes and regulating mark-ups. This could increase the affordability of patients to complete their antibiotic therapy with subsequent reduction in antimicrobial resistance.
    To investigate osteointegration at the graft-bone interface and the prevention of osteoarthritis after anterior cruciate ligament (ACL) reconstruction using a silk-collagen scaffold with both ends modified by hydroxyapatite (HA) in a rabbit model.

    The HA/silk-collagen scaffold was fabricated using a degummed, knitted silk scaffold, collagen I matrix, and simulated body fluid (SBF). The HA/silk-collagen scaffold was rolled up to make a graft for replacing the native ACL in the experimental group (HA group), and the silk-collagen scaffold was used in the control (S group). All specimens were harvested at 16 weeks postoperatively to evaluate graft-bone healing and osteoarthritis prevention.

    Histological staining revealed the massive formation of more mature bone at the tendon-bone interface, and immunohistochemistry staining revealed more collagen I and osteocalcin deposition in the HA group than in the S group. Higher signals indicating more bone mineral formation were detected in the HA group than in the S group, which was consistent with the results of biomechanical testing. Better osteoarthritis prevention was also observed in the HA group, indicating a more stable knee joint in the HA group than in the S group.

    To describe the key qualities and unique roles of peer support workers in the care of people who inject drugs during and after hospitalization.

    We conducted a qualitative study. Key stakeholders were recruited including people who use drugs who had been hospitalized, healthcare team members, peer support workers, and employers of peer support workers. Data were collected from 2019 to 2020 using semi-structured interviews that were audio-recorded, transcribed, and analyzed thematically.

    Fourteen participants were interviewed 6 people who use drugs who had been hospitalized, 5 healthcare team members, 2 peer support workers, and 1 employer of peer support workers. At the core of the data was the notion of peer workers acting as a bridge. We found four themes that related to functions of this bridge overcoming system barriers, advocacy, navigating transitions within the healthcare system, and restoring trust between HCPs and PWUD. We found two themes for building a strong bridge and making the role of a peer support worker function effectively (training and mentorship, and establishing boundaries). We found three themes involving characteristics of an effective peer worker (intrinsic qualities, contributions of shared experiences, and personal stability).

This study aimed to use anonymised veterinary clinical data from the VetCompass™ Programme to report the frequency of common disorders of dogs under primary veterinary care in the UK during 2016 and to explore effects associated with age, sex and neuter status.

    From an available population of 905,543 dogs under veterinary care at 886 veterinary clinics during 2016, the current study included a random sample of 22,333 (2.47 %) dogs from 784 clinics. Prevalence for each disorder was calculated at the most refined level of diagnostic certainty (precise-level precision) and after grouping to a more general level of diagnostic precision (grouped-level precision). The most prevalent precise-level precision disorders recorded were periodontal disease (prevalence 12.52 %, 95 % CI 12.09-12.97), otitis externa (7.30 %, 95 % CI 6.97-7.65) and obesity (7.07 %, 95 % CI 6.

This study aimed to use anonymised veterinary clinical data from the VetCompass™ Programme to report the frequency of common disorders of dogs under primary veterinary care in the UK during 2016 and to explore effects associated with age, sex and neuter status. From an available population of 905,543 dogs under veterinary care at 886 veterinary clinics during 2016, the current study included a random sample of 22,333 (2.47 %) dogs from 784 clinics. Prevalence for each disorder was calculated at the most refined level of diagnostic certainty (precise-level precision) and after grouping to a more general level of diagnostic precision (grouped-level precision). The most prevalent precise-level precision disorders recorded were periodontal disease (prevalence 12.52 %, 95 % CI 12.09-12.97), otitis externa (7.30 %, 95 % CI 6.97-7.65) and obesity (7.07 %, 95 % CI 6.
