The structural characterizations of the isolated compounds were determined using physical data, comprehensive 1D and 2D NMR spectral analysis, and HRESIMS. All isolated saponins are hederagenin or pomolic acid glycosides conjugated with differentiable sugar units bound to C-3 and/or C-28 of the aglycone through ether and/or ester glycosidic linkages, respectively. Structural diversity of these isolated secondary metabolites would have a great impact on the future chemosystematic studies of this plant. Four saponins, obtained in good yield were evaluated for their anti-inflammatory activities in a rat model using the carrageenan-induced paw edema protocol. Two of these exhibited significant anti-inflammatory activities demonstrated through inhibition of the paw edema by 64 and 60%. Nevertheless, it remains obscure how the airborne bacteria especially the pathogenic bacteria vary on the small time scale of day and night. To investigate the nycterohemeral discrepancy of airborne total bacteria and pathogenic bacteria, PM2.5 samples were collected in Hangzhou between day and night. Microbial taxonomic information was obtained through 16S rRNA gene sequencing and "human pathogens database" screening. Further analyses based on Multiple Regression Matrices (MRM) approach and Concentration Weighted Trajectory (CWT) model were conducted to elucidate the effect of local environmental factors and long-range transport. The community composition of total bacteria tended to be similar in the daytime while pathogenic bacteria turned out to be homogeneous in the nighttime. To be vigilant, the diversity of airborne pathogenic bacteria echoed the frequency of anthropogenic activities with the pathogen inhalation rate roughly at 428 copies/h and 235 copies/h respectively in daytime and nighttime. The nycterohemeral discrepancy of total bacteria was principally driven by the filtering of environmental factors, i.e., CO and NO2, indicating that anthropogenic activities brought about the homogeneity. Airborne pathogenic bacteria coupled with the strong resistances of environmental filtering stood out from their non-pathogenic counterpart, which enabled the long-range transport. Indeed, the nycterohemeral discrepancy of pathogenic bacteria was shaped by the transport of air masses. This research filled the gaps in temporal variance of airborne microorganisms on the small time scale of day and night, providing crucial foundation for precisely predicting ecological and health effects of bioaerosols. In a lake basin, there is a mismatch between river and lake water quality targets and a method for setting specific water quality targets for these rivers is urgently needed. Using Dianchi Lake as an example, we proposed a lake basin water quality management system based on the river-lake water quality response relationship, coupled with a Soil and Water Assessment Tool (SWAT) basin hydrological model and Environmental Fluid Dynamics Code (EFDC) lake water quality hydrodynamic model. River water quality control requirements based on the river-lake water quality response were proposed, under the premise that the Dianchi Lake water quality reaches the required standard. Then, water quality control targets for rivers were determined, and corrected for influencing factors, such as current river water quality and composition of flow. Our systematic approach efficiently identified key lake basin pollution sources, and accurately located key points for water quality improvement and pollution control. Fatigue may persist for many months and interferes with participation in everyday activities and has a negative impact on social and family relationships, return to work, and quality of life. We previously developed and tested in a small uncontrolled pilot study a manualised, clinical psychologist-delivered, face-to-face intervention, informed by cognitive behavioural therapy (CBT). We then adapted it for delivery by trained therapists via telephone. We now aim to test the feasibility of this approach in a parallel group, randomised controlled feasibility trial (Post Stroke Intervention Trial In Fatigue, POSITIF). A feasibility trial is required in order to assess whether a full randomised controlled trial is indicated in order to evaluate this approach. Nutritional status among people living with HIV was assessed through anthropometry, body mass index; Underweight (body mass index 23 kg/m2). HIV related clinical factors such CD4 count, WHO stage, opportunistic infection, antiretroviral therapy regimen etc. were collected from the medical records. Socio-demographic data were collected using pretested structured questionnaire through interview technique. Multiple linear regression method was employed to determine the association between different independent factors and body mass index score. Results The prevalence of underweight was found to be 18.3% (95% CI 14.3-22.6). Most of the study participants were overweight/obese (39.1%). After subjection to multiple linear regression analysis, it was found that age, being male, being married, being in business occupation, smoking, hemoglobin level and antiretroviral therapy duration were significantly associated with body mass index score. Majority of the participants in our study lacked diversified food (62.3%). Conclusion Overweight/obesity is an emerging problem among people living with HIV. This group of participants should be screened for the presence of non-communicable disease. This study also highlights the importance of nutritional program being an integral part of HIV/AIDS continuum of care. However, the PD group showed lower body fat mass, whole-body fat percentage, and greater resting metabolic rate compared to controls (p less then 0.05), with no significant differences in musculoskeletal mass. Parkinson's disease postural instability/gait difficulty (PD-PIGD) subtype showed lower body fat parameters, increased fat-free mass, and higher resting metabolic rates. Conclusions These results suggest that PD patients present an increased resting metabolic rate associated with the postural instability/gait difficulty PD subtype, allowing a selective decrease of body fat mass and not musculoskeletal mass. Of note, several disease-related factors may contribute to this weight loss in PD patients, being a complex and multifactorial consequence. Our findings could likely be one of the many contributing factors. However, present findings may further add to our understanding of the phenomenon of weight loss in patients with PD. PD-related pharyngeal dysphagia (PDrPD) may significantly improve in up to half of patients following acute oral levodopa challenge. Objective The aim of this study was to investigate the effects of levodopa-carbidopa intestinal gel (LCIG) on PDrPD. Methods Forty-five PD patients under LCIG treatment were available for retrospective analysis. In all patients with PDrPD who underwent flexible endoscopic evaluation of swallowing (FEES) in the clinical "on-state" both before and after implementation of LCIG treatment, FEES videos were systematically reassessed. PDrPD was characterized using a PD-specific FEES score evaluating premature bolus spillage, penetration/aspiration, and pharyngeal residue. However, few studies have evaluated the clinical effectiveness of these programs. We compared the patient populations of those referred for osteoporosis management by FLS to those referred by primary care physicians (PCP), within the Canadian healthcare system in the province of Ontario. Specifically, we investigated if a referral from FLS is similarly effective as PCP at identifying patients at risk for future osteoporotic fractures and if osteoporosis therapies have been previously initiated. However, the PD group showed lower body fat mass, whole-body fat percentage, and greater resting metabolic rate compared to controls (p less then 0.05), with no significant differences in musculoskeletal mass. Parkinson's disease postural instability/gait difficulty (PD-PIGD) subtype showed lower body fat parameters, increased fat-free mass, and higher resting metabolic rates. Conclusions These results suggest that PD patients present an increased resting metabolic rate associated with the postural instability/gait difficulty PD subtype, allowing a selective decrease of body fat mass and not musculoskeletal mass. Of note, several disease-related factors may contribute to this weight loss in PD patients, being a complex and multifactorial consequence. Our findings could likely be one of the many contributing factors. However, present findings may further add to our understanding of the phenomenon of weight loss in patients with PD. PD-related pharyngeal dysphagia (PDrPD) may significantly improve in up to half of patients following acute oral levodopa challenge. Objective The aim of this study was to investigate the effects of levodopa-carbidopa intestinal gel (LCIG) on PDrPD. Methods Forty-five PD patients under LCIG treatment were available for retrospective analysis. In all patients with PDrPD who underwent flexible endoscopic evaluation of swallowing (FEES) in the clinical "on-state" both before and after implementation of LCIG treatment, FEES videos were systematically reassessed. PDrPD was characterized using a PD-specific FEES score evaluating premature bolus spillage, penetration/aspiration, and pharyngeal residue. Our cumulative results demonstrate that roxadustat administration can attenuate experimental pulmonary fibrosis via the inhibition of TGF-β1/Smad activation.Purpose (1) To compare the acetabular coverage between dysplasia, borderline dysplasia, and control acetabulum in a quantitative three-dimensional manner, and (2) to evaluate correlations between the radiologic parameters and the three-dimensional zonal-acetabular coverage. Methods We reviewed contralateral hips CT images of sixteen to sixty years old patients who underwent one of three types of surgeries eccentric rotational acetabular osteotomy, curved intertrochanteric varus osteotomy, and total hip replacement with minimum 1-year follow-up from January 2013 to April 2018. A point-cloud model of the acetabulum created from CT was divided into 6 zones. Three-dimensional acetabular coverage was measured radially at intervals of 1 degree. Mean radial acetabular coverage for each zone was named ZAC (Zonal Acetabular Coverage) and was compared among the three sub-groups (control 25°≤lateral center-edge angle [LCEA] less then 40°; borderline 20°≤LCEA less then 25°; and dysplasia LCEA≤20°) statistically. Further,tic acetabulum is more similar to the dysplastic acetabulum than to the normal acetabulum.Objective To examine the association between bacterial-viral co-occurrence in the nasopharynx and the risk of community acquired pneumonia (CAP) in young children living in resource-limited settings. RSV and Influenza A/B were detected with a standardized polymerase chain reaction (PCR) method, and a simplified real-time quantitative PCR method, without sample pre-processing, was developed to detect bacterial pathogens in nasopharyngeal samples. Results A total of 109 CAP patients and 324 healthy controls were enrolled. Co-detection of H. influenzae and S. pneumoniae in nasopharyngeal swabs was linked with higher odds of CAP (aOR=3.2, 95% CI=1.1-9.5). Oral impacts on the quality of life were measured using a shortened form of the Oral Health Impact Profile (OHIP-26). Regression analysis were performed, adjusted for age, to estimate the relationship between the variables. Data were expressed as Confidence Intervals (95% CI), p-values (values less then 0.05 were considered statistically significant) and adjusted R2 values. Results Overall, the sample had a mean age of 42.6 ± 17.1 (range, 18 to 93 years), a mean cumulative BEWE score of 6.7 ± 4.4 and a mean total OHIP-26 score of 1.84 ± 0.59. For the cumulative sextant BEWE scores, 68.0% of the participants scored ≤ 8, 24.5% between 9 and 13 and 7.5 %, ≥ 14. A significant association was found between increasing BEWE score and the overall OHIP-26 total score (effect = 0.028; p = 0.002), implying a higher BEWE to be associated with a larger impact of oral conditions on daily life. Conclusion Higher levels of tooth wear were significantly associated with a deteriorating oral-health related quality of life amongst the participants. VR can be combined with neuroimaging to offer invaluable insight into how the targeted brain areas respond to stimulation during neurorehabilitation training. That, in turn, could eventually serve as a predictive marker for therapeutic success. Functional magnetic resonance imaging (fMRI) identified neuronal activity related to blood flow to reveal with a high spatial resolution how activation patterns change, and restructuring occurs after VR training. Portable and quiet, electroencephalography (EEG) conveniently allows the clinician to track spontaneous electrical brain activity in high temporal resolution. Then, functional near-infrared spectroscopy (fNIRS) combines the spatial precision level of fMRIs with the portability and high temporal resolution of EEG to constitute an ideal measuring tool in virtual environments (VEs). This narrative review explores the role of VR and concurrent neuroimaging in cognitive rehabilitation.Negative symptoms of schizophrenia comprise a group of severe symptoms contributing to high disease burden and poor long-term prognosis. Conceptual work has shown that these symptoms can be mapped onto, at least, two distinct dimensions apathy including the domains avolition, asociality and anhedonia, and diminished expression including the domains blunted affect and alogia. Growing evidence suggest that these dimensions have partly distinct behavioral, cognitive and neural correlates. Nonetheless, modulation and treatment of specific processes and brain correlates related to these negative symptom dimensions through behavioral, pharmacological, and brain stimulation interventions remains poorly understood. Here, we address this question by employing an integrative approach and comprehensively synthesizing the current literature on neuroimaging, behavior and clinical studies of both negative symptom dimensions. While considerable progress has been made, it remains an open challenge to develop integrative mechanistic pathophysiological models for apathy and diminished expression.

Although CARD3 (caspase activation and recruitment domain 3) protein is an important factor involved in regulating immunity, inflammation, lipid metabolism, and apoptosis, its role in cerebral stroke is currently unknown. Methods and Results Using a mouse model of ischemia-reperfusion (I-R) injury based on transient blockage of the middle cerebral artery, we have found that CARD3 expression is upregulated in a time-dependent manner during I-R injury. Further animal study revealed that, relative to control ****, CARD3-knockout **** exhibited decreased inflammatory response and neuronal apoptosis, with reduced infarct volume and lower neuropathological scores. In contrast, neuron-specific CARD3-overexpressing transgenic (CARD3-TG) **** exhibited increased I-R induced injury compared with controls. Mechanistically, we also found that the activation of TAK1 (transforming growth factor-β-activated kinase 1) was enhanced in CARD3-TG ****. Furthermore, the increased inflammation and apoptosis seen in injured CARD3-TG brains were reversed by intravenous administration of the TAK1 inhibitor 5Z-7-oxozeaenol. Conclusions These results indicate that CARD3 promotes I-R injury via activation of TAK1, which not only reveals a novel regulatory axis of I-R induced brain injury but also provides a new potential therapeutic approach for I-R injury.Tularemia is a widely spread zoonotic disease in the northern hemisphere, caused by the bacterium Francisella tularensis. In humans, tularemia is an acute febrile illness with incidence peaks in late summer to early autumn of outbreak years, but there is no early warning system in place that can reduce the impact of disease by providing timely risk information. In this study, we revisit previously unpublished data on F. tularensis in water, sediment, soil, and small mammals from 1984 in northern Sweden. In addition, we used human case data from the national surveillance system for tularemia in the same year. In the environmental and small mammal material, bank vole (Myodes glareolus) samples from urine and bladder were the only samples that tested positive for F. tularensis. The prevalence of F. tularensis among trapped bank voles was 13.5%, although all six bank voles that were retrieved from owl nest boxes in early May tested positive. Forty-two human tularemia cases were reported from August to December in 1984. The relationship between smoking, blood pressure (BP) control and volume overload is insufficiently explored in patients on maintenance dialysis.Methods This is a retrospective cross-sectional cohort study, utilizing existing patients' data generated during routine ESRD care, including bimonthly protocol bioimpedance fluid assessment of the volume status.Results We analyzed the data of 63 prevalent patients receiving thrice weekly maintenance hemodiafiltration treatments at one rural dialysis unit in Hungary. The cohort's mean ± SD age was 61.5 ± 15.3 years; 65% male, 38% diabetic, with a mean arterial blood pressure (MAP) 99.5 ± 16.8 mmHg and Charlson score 3.79 ± 2.04. Of these, 38 patients were nonsmokers and 25 smokers. The nonsmokers' MAP was 94.3 ± 14.0 versus smokers' 105.9 ± 18.9 mmHg (p .002); nonsmokers took an average 0.73 ± 0.92 antihypertensive medications vs. 1.73 ± 1.21 for smokers (p .0001). Methods and Results Using a mouse model of ischemia-reperfusion (I-R) injury based on transient blockage of the middle cerebral artery, we have found that CARD3 expression is upregulated in a time-dependent manner during I-R injury. Further animal study revealed that, relative to control mice, CARD3-knockout mice exhibited decreased inflammatory response and neuronal apoptosis, with reduced infarct volume and lower neuropathological scores. In contrast, neuron-specific CARD3-overexpressing transgenic (CARD3-TG) mice exhibited increased I-R induced injury compared with controls. Mechanistically, we also found that the activation of TAK1 (transforming growth factor-β-activated kinase 1) was enhanced in CARD3-TG mice. Furthermore, the increased inflammation and apoptosis seen in injured CARD3-TG brains were reversed by intravenous administration of the TAK1 inhibitor 5Z-7-oxozeaenol. In humans, tularemia is an acute febrile illness with incidence peaks in late summer to early autumn of outbreak years, but there is no early warning system in place that can reduce the impact of disease by providing timely risk information. In this study, we revisit previously unpublished data on F. tularensis in water, sediment, soil, and small mammals from 1984 in northern Sweden. In addition, we used human case data from the national surveillance system for tularemia in the same year. In the environmental and small mammal material, bank vole (Myodes glareolus) samples from urine and bladder were the only samples that tested positive for F. tularensis. The prevalence of F. tularensis among trapped bank voles was 13.5%, although all six bank voles that were retrieved from owl nest boxes in early May tested positive. Forty-two human tularemia cases were reported from August to December in 1984. The cohort's mean ± SD age was 61.5 ± 15.3 years; 65% male, 38% diabetic, with a mean arterial blood pressure (MAP) 99.5 ± 16.8 mmHg and Charlson score 3.79 ± 2.04. Of these, 38 patients were nonsmokers and 25 smokers. The nonsmokers' MAP was 94.3 ± 14.0 versus smokers' 105.9 ± 18.9 mmHg (p .002); nonsmokers took an average 0.73 ± 0.92 antihypertensive medications vs. 1.73 ± 1.21 for smokers (p .0001). Objectives To determine the awareness about autism among Medical and Allied-Medical Practitioners. Methods This cross-sectional survey recruited a sample of n=300 participants including n=150 Medical and n=150 Allied-Medical practitioners using convenience sampling from Benazir Bhutto Hospital and Autism Resource Centre, Rawalpindi, Pakistan between 20th May 2018 to 20th October 2018. Sample included both genders, aged 21 to 50 years. Basic demographic sheet and Knowledge about Childhood Autism among Health Workers (KCAHW) Questionnaire was used to collect data. Statistical analysis was done using SPSS 21. Results Study revealed the total mean KCAHW score of 15.20 ± 5.17 and 8.84 ± 6.31 for Allied-Medical and Medical practitioners respectively. While the Domain-I mean KCAHW scores was 6.28±2.10 and 3.68±2.41; Domain-II score 0.86 ± 0.35 and 0.45 ± 0.50; Domain-III score 3.28 ± 1.00 and 1.91 ± 1.65; and Domain-IV score of 4.83 ±1.72 and 2.80 ± 1.75 for Allied-Medical and Medical practitioners respectively. Results Out of 82 patients enrolled in this study, the mean age was 49.13 ± 14.8 years and 87.8% were male. The mean of EF was 31.16% and LVT was found in 6.1%. The spontaneous contrast was seen in 25.6% of patients indicating strong relationship with low EF (p less then 0.001). Among patients with low EF, ischemic heart disease (IHD) was identified in 50%, hypertension in 30.5%, diabetes mellitus (DM) type 2 in 23.2%, and hyperlipidemia 12.2%. Exactly 80% of LVT were detected in IHD patients with dilated cardiomyopathy (DCMP) and 80% of detected LVT were apical in site. Conclusion Cardiac patients with low ejection fraction developed left ventricular thrombosis, and most of the affected patients were ischemic heart disease with dilated cardiomyopathy. Interestingly, spontaneous contrast was found high significantly in these patients, which may reflect the continuous process of thrombus formation.Objective The third international consensus definition for sepsis and septic shock (sepsis 3) task force recently introduced qSOFA (quick sequential organ failure assessment) as a score for detection of patients at risk of sepsis outside of intensive care units. We performed this study to evaluate the validity of qSOFA for early detection and risk stratification of septic patients in emergency department. Methods We conducted this study in an emergency department of the largest university affiliated hospital in northwest of Iran from Sept 2015 to Sept 2016. One hundred and forty patients who were SIRS positive with a suspected infection without alternative diagnosis and a microbiological proven infection were enrolled in this study. qSOFA was calculated for each patient and correlated with sepsis grades and mortality. Results From 140 patients 84 (60%) had positive qSOFA score and 56 (40%) patients had negative qSOFA score. Our results showed that near half of patients with positive qSOFA expired during their stay in hospital while this was about 5% for patients with negative qSOFA. ROC curve of study regarding prediction of outcome with qSOFA showed an area under curve of 0.59. (P value 0.04). Time spent to sepsis detection was 16 minutes shorter with qSOFA score compared to SIRS criteria in this study. Conclusion In patients with suspected sepsis, qSOFA has acceptable value for risk stratification of severity, multi organ failure and mortality. It seems that education of medical staff and frequent screening of patients for warning signs can help to increase the value of qSOFA in prediction of mortality in critically ill septic patients.Objective To evaluate the outcomes of modified interlaminar decompression in patients with degenerative lumbar spinal stenosis (LSS). All patients with degenerative LSS who underwent modified interlaminar decompression during the study period were included in the study. The patients were followed up to one year after surgery. The data was entered into a structured questionnaire designed according to the study which was then analyzed using SPSS version 21. Results A total of 182 LSS cases were included in the study and 236 levels were operated during the study period. According to the records increased prevalence of LSS was found among males i.e. 58.8%. The common level with degenerative stenosis involved was L4-5. Good to excellent outcomes were observed in 93.9% patients in the 1st follow-up visit. The most common complication of surgery was dural tear followed by wound infection. We speculate that pyoverdine functions as a virulence determinant by capturing environmentally available iron for host bacterial cells, by limiting its availability for C. elegans worms, and by regulating and/or activating other intracellular virulence factors that ultimately kills C. elegans worms.Non-alcoholic fatty liver disease (NAFLD) is the most common form of chronic liver disease in adults and children. It is characterized by excessive accumulation of lipids in the hepatocytes of patients without any excess alcohol intake. With a global presence of 24% and limited therapeutic options, the disease burden of NAFLD is increasing. Thus, it becomes imperative to attempt to understand the dynamics of disease progression at a systems-level. Here, we decoded the emergent dynamics of underlying gene regulatory networks that were identified to drive the initiation and the progression of NAFLD. We developed a mathematical model to elucidate the dynamics of the HNF4α-PPARγ gene regulatory network. Our simulations reveal that this network can enable multiple co-existing phenotypes under certain biological conditions an adipocyte, a hepatocyte, and a "hybrid" adipocyte-like state of the hepatocyte. These phenotypes may also switch among each other, thus enabling phenotypic plasticity and consequently leading to simultaneous deregulation of the levels of molecules that maintain a hepatic identity and/or facilitate a partial or complete acquisition of adipocytic traits. These predicted trends are supported by the analysis of clinical data, further substantiating the putative role of phenotypic plasticity in driving NAFLD. Our results unravel how the emergent dynamics of underlying regulatory networks can promote phenotypic plasticity, thereby propelling the clinically observed changes in gene expression often associated with NAFLD.Forecasting vessel flows is important to the development of intelligent transportation systems in the maritime field, as real-time and accurate traffic information has favorable potential in helping a maritime authority to alleviate congestion, mitigate emission of GHG (greenhouse gases) and enhance public safety, as well as assisting individual vessel users to plan better routes and reduce additional costs due to delays. In this paper, we propose three deep learning-based solutions to forecast the inflow and outflow of vessels within a given region, including a convolutional neural network (CNN), a long short-term memory (LSTM) network, and the integration of a bidirectional LSTM network with a CNN (BDLSTM-CNN). To apply those solutions, we first divide the given maritime region into M × N grids, then we forecast the inflow and outflow for all the grids. Experimental results based on the real AIS (Automatic Identification System) data of marine vessels in Singapore demonstrate that the three deep learning-based solutions significantly outperform the conventional method in terms of mean absolute error and root mean square error, with the performance of the BDLSTM-CNN-based hybrid solution being the best.Vitamin D is associated with cardiovascular health through activating the vitamin D receptor that targets genes related to cardiovascular disease (CVD). GNSS sensors are successfully used to determine positions and velocities over a specified time period, with unprecedented accuracy, promoting research in many safety-critical areas, such as geophysics and geo-tectonics, tackling problems that torment traditional equipment and providing deformation products with absolute accuracy. Cyprus, being located at the Mediterranean fault, exhibits a very interesting geodynamic regime, which has yet to be investigated thoroughly. Accordingly, this research revolves around the estimation of crustal deformation in Cyprus using GNSS receivers. CYPOS (CYprus POsitioning System), a network of seven permanent GNSS stations has been operating since 2008, under the responsibility of the Department of Lands and Surveys. The continuous flow of positioning data collected over this network, offers the required information to investigate the behavior of the crustal deformation field of Cyprus using GNSS sensors for the first time. This paper presents the results of a multi-year analysis (11/2011-01/2017) of daily GNSS data and provides inferences of linear and nonlinear deforming signals into the position time series of the network stations. Specifically, 3D station velocities and seasonal periodic displacements are jointly estimated and presented via a data stacking approach with respect to the IGb08 reference frame.

Our results unravel how the emergent dynamics of underlying regulatory networks can promote phenotypic plasticity, thereby propelling the clinically observed changes in gene expression often associated with NAFLD.Forecasting vessel flows is important to the development of intelligent transportation systems in the maritime field, as real-time and accurate traffic information has favorable potential in helping a maritime authority to alleviate congestion, mitigate emission of GHG (greenhouse gases) and enhance public safety, as well as assisting individual vessel users to plan better routes and reduce additional costs due to delays. In this paper, we propose three deep learning-based solutions to forecast the inflow and outflow of vessels within a given region, including a convolutional neural network (CNN), a long short-term memory (LSTM) network, and the integration of a bidirectional LSTM network with a CNN (BDLSTM-CNN). To apply those solutions, we first divide the given maritime region into M × N grids, then we forecast the inflow and outflow for all the grids. Experimental results based on the real AIS (Automatic Identification System) data of marine vessels in Singapore demonstrate that the three deep learning-based solutions significantly outperform the conventional method in terms of mean absolute error and root mean square error, with the performance of the BDLSTM-CNN-based hybrid solution being the best.Vitamin D is associated with cardiovascular health through activating the vitamin D receptor that targets genes related to cardiovascular disease (CVD). GNSS sensors are successfully used to determine positions and velocities over a specified time period, with unprecedented accuracy, promoting research in many safety-critical areas, such as geophysics and geo-tectonics, tackling problems that torment traditional equipment and providing deformation products with absolute accuracy. Cyprus, being located at the Mediterranean fault, exhibits a very interesting geodynamic regime, which has yet to be investigated thoroughly. Accordingly, this research revolves around the estimation of crustal deformation in Cyprus using GNSS receivers. CYPOS (CYprus POsitioning System), a network of seven permanent GNSS stations has been operating since 2008, under the responsibility of the Department of Lands and Surveys. The continuous flow of positioning data collected over this network, offers the required information to investigate the behavior of the crustal deformation field of Cyprus using GNSS sensors for the first time. This paper presents the results of a multi-year analysis (11/2011-01/2017) of daily GNSS data and provides inferences of linear and nonlinear deforming signals into the position time series of the network stations. RESULTS The inclusion criteria for the review were fulfilled in 8 studies. The overall quality of the individual studies was judged to be moderate at best. The data were methodologically homogenous enough to be pooled for the meta-analysis in only 2 studies. The results of the meta-analysis revealed that tonsil surgery has a positive effect on the growth direction of the mandible (p less then 0.001). CONCLUSIONS There is modest evidence that suggests that tonsil surgery has a positive effect on the dentofacial development in children with hypertrophied tonsils. We investigated the plasma cytokine, chemokine and growth factor signatures and their clinical significance in pediatric ITP patients during acute, chronic and follow-up stages as well as the effects of intravenous immunoglobulin (IVIg) treatment, by using the Multiplex technology. In acute ITP before and/or after IVIg treatment we found significantly increased plasma levels of the pro- (tumour necrosis factor-α (TNF-α), interleukin IL-15) and anti- (IL-1 receptor antagonist (Ra), IL-10 and the growth factor interferon γ-induced protein (IP-10)) inflammatory cytokines, compared to healthy controls. Except for IL1-Ra, these cytokines decreased to normal levels in chronic patients. We firstly assessed the interactions among PM components in generating the OP based on DTT consumption rate and AA depletion. We started with the standard solution sequence (from 0.005 to 10 μM), including quinones (PQ, 1,2-NQ, and 1,4-NQ) and metals (Cu, Mn, and Fe). The interactions between metals were antagonistic interactions in DTT consumption. Cu showed antagonistic interaction with PQ, but additive with 1,2-NQ and 1,4-NQ. Mn interacted synergistically with 1,4-NQ in DTT consumption but antagonistically with PQ (where CPQ less then 2.5 μM) and 1,4-NQ (where CPQ less then 2 μM). Fe showed synergistic with quinones in investigated concentration range (from 0.01 to 5 μM). Plastic litter pollution is increasing in the seas and oceans worldwide, raising concern on the potential effects of plasticizer additives on marine fauna. In this study, muscle samples of 30 bogues (Boops boops; Linneaus, 1758) from the North Western Mediterranean Sea were analysed to assess the concentrations of 19 organophosphate flame retardant (OPFR) compounds and to inspect any relationship with microplastic ingestion and relative levels of anthropization. Out of the 19 OPFRs analysed, 6 compounds were detected, being tri-n-butyl phosphate (TNBP), 2-ethylhexyldiphenyl phosphate (EHDPP) and triphenylphosphine oxide (TPPO) the most abundant. As expected, OPFR concentrations were higher in samples collected off the most anthropized area of the city of Barcelona than in those from the Cap de Creus Marine Protected Area, while no significant correlation was detected between OPFR concentrations and microplastic ingestion. The results of this manuscript provide a first evidence of OPFR presence in the muscle of the bogue and identify the coastal area off Barcelona as a possible concentration area for contaminants, further supporting the use of the bogue as an indicator species of plastic pollution in the Mediterranean Sea. The aim of this study was to investigate the bioaccessibility of pesticide residues in blueberries (commercial and sample from controlled field trial) from Serbia, involving the presence of a complex food matrix and to assess the potential risk to human health. The presence of nine active substances (azoxystrobin, boscalid, fludioxonil, cyprodinil, pyrimethanil, pyridaben, pyriproxyfen, acetamiprid and thiametoxam) in initial blueberry samples was determined in concentration range from 5.15 μg/kg for thiametoxam to 187 μg/kg for azoxystrobin. Clothianidin, metabolite of thiametoxam, was not detected in any blueberry sample. However, after in vitro digestion, the content of initially detected pesticides residues was significantly decreased or it was below limit of quantification resulting in the total bioaccessibility of about 15%. Azoxystrobin, pyrimethanil and fludioxonil was quantified in digestive juice at concentrations which were about 81%, 37% and 10% less than the inital concentration, respectively. The presence of food matrix during digestion of blueberries even more severely reduced concentration of pesticide residues (total bioaccessibility was about 7%) compared to digestion without the food matrix.

Successful integration of health care in rural and underserved communities requires attention to power structures, trust, and disciplinary boundaries that inhibit team-based integration of behavioral and primary health care. This paper reports on perceived successes and ongoing challenges of integrating primary and behavioral health care from the perspectives of providers, community leaders, and community members. Data collection consisted of semi-structured qualitative interviews and focus groups conducted as part of a regional health equity assessment in northern Arizona. The authors explore barriers and successes in integrating health care in rural clinics using the perspective of a social ecological framework and the mediating role of culture. Differing expectations, differing professional areas, and interpersonal interactions were primary factors challenging movement toward integrated health care. Results suggest that providers and policymakers working toward health care integration should consider culture and interpersonal interaction as dynamic mediators, particularly in underserved and rural health care contexts.BACKGROUND U.S. medical schools have been unsuccessful in creating a diverse physician workforce. Implicit bias is pervasive in medicine, including potentially in medical school admissions. METHODS We invited all 2018-2019 interviewees at one U.S. medical school to complete the eight-item Everyday Discrimination Scale (EDS) asking about experiences of bias during interview experiences to date. RESULTS Three hundred forty-seven (30%) of 1,175 interviewees completed the survey, with participant demographic characteristics matching those of the broader interviewee pool. Seventy-two (21%) responded affirmatively to one or more EDS items. Gender, age, race, religion, and sexual orientation were all sources of discrimination. Those reporting bias had completed more interviews (5.2 vs. 3.9, P less then .05) and were more likely to be Latinx (30.6% vs. 16.4%, P less then .05) than their counterparts. Only three (4%) reported the incident to the institution where it occurred. CONCLUSION Further work exploring experiences of bias during medical school admissions and how to decrease their frequency is warranted.Compared with the general population, justice-involved youth have substantially higher rates of several health conditions. Less is known about their use of health services to address these conditions. Using data from a statewide survey of 217 youth in juvenile correctional facilities and 164,832 youth in public schools, we examined selfreported health (health overall, weight status, disability, asthma, allergy, mental health) and receipt of care. Justice-involved youth reported a high number of physical health concerns; however, physical health conditions were not related to receipt of care. Youth who reported experiencing depressive symptoms with or without suicidal ideation, and those who had attempted suicide, were more likely than their peers without these mental health issues to have received mental health treatment in the past year. Urban residency [a danger sign associated with pregnancy (AOR=4.01, 95% CI=1.85-8.69), childbirth (AOR=2.47, 95% CI=1.12-5.43) and the postpartum period (AOR=3.39, 95% CI=1.60-7.18)]; having antenatal care follow-up in their last pregnancy [a danger sign associated with pregnancy (AOR=3.47, 95% CI=1.95-6.18), childbirth (AOR=2.36, 95%CI=1.41-3.96) and the postpartum period (AOR=2.48, 95% CI=1.49-4.13)]; and giving birth at a health institution (AOR=1.43, 95% CI=1.01-2.02) were significant factors. Increasing antenatal-care coverage, educating women and increasing institutional delivery would increase mothers' knowledge about obstetric danger signs.BACKGROUND AND AIMS Hepatitis C virus (HCV) infection is highly prevalent among homeless individuals, but the scope of HCV-associated liver fibrosis in this population is poorly understood. METHODS Using the FIB-4 Index, we describe the prevalence and correlates of advanced fibrosis among a retrospective cohort of all homeless-experienced adults with HCV seen at Boston Health Care for the Homeless Program (BHCHP) over a one-year period. RESULTS Of 832 BHCHP patients with HCV, 15.8% had advanced fibrosis. In multivariable regression analysis, alcohol use disorder (adjusted odds ratio [aOR] 2.50, 95% confidence interval [CI] 1.65-3.81) and having unknown or poorly characterized housing circumstances (aOR 2.88, 95% CI 1.02-8.14, relative to housed patients) were independently associated with advanced fibrosis. CONCLUSIONS The prevalence of advanced fibrosis in this cohort of homeless adults with HCV appears similar to national estimates among housed individuals, but their psychosocial complexity is greater, especially among those who are older.

In multivariable regression analysis, alcohol use disorder (adjusted odds ratio [aOR] 2.50, 95% confidence interval [CI] 1.65-3.81) and having unknown or poorly characterized housing circumstances (aOR 2.88, 95% CI 1.02-8.14, relative to housed patients) were independently associated with advanced fibrosis.
