The probe L and each metal ion bind through a 11 complex stoichiometry, and the plausible sensing mechanism is proposed based on the inhibition of the photoinduced electron transfer process (PET). The reversible chemosensor L showed high sensitivity toward Al3+ and Fe2+ ions, which was analyzed by fluorescence and UV-vis spectroscopy techniques up to nanomolar detection limits, 38.26 × 10-9 and 17.54 × 10-9 M, respectively. Compared with previous approaches, there is no additional heater used in this work, as the heat is directly generated by laser irradiation on the alkoxyamine/monomer system. The polymerization can be initiated for benchmark methacrylate monomers with spatial controllability, that is, only in the laser-irradiated area, opening the way for laser write or three-dimensional printing applications in the presence of fillers. Zn(II)-Rutin complexes have been described in the literature and distinct coordination modes proposed based on 1H NMR/MS and IR/UV-VIS experimental spectroscopic data 11/12 (Zn(II) binding to A-C rings) and 21 (Zn(II) binding to A-C-B rings) stoichiometry. Aiming to clarify these experimental findings and provide some physical insights into the process of complex formation in solution, we carried out density functional theory calculations of NMR and UV-VIS spectra for 25 plausible Zn(II)-Rutin molecular structures including solvent effect using the polarizable continuum model approach. The studied complexes in this work have 11, 12, 21, and 31 metal-ligand stoichiometry for all relevant Zn(II)-Rutin configurations. The least deviation between theoretical and experimental spectroscopic data was used as an initial criterion to select the probable candidate structures. Our theoretical spectroscopic results strongly indicate that the experimentally suggested modes of coordination (12 and 21) are likely to exist in solution, supporting the two distinct experimental findings in DMSO and methanol solution, which may be seen as an interesting result. Our predicted 12 and 21 metal complexes are in agreement with the experimental stoichiometry; however, they differ from the proposed structure. Besides the prediction of the coordination site and molecular structure in solution, an important contribution of this work is the determination of the OH-C5 deprotonation state of rutin due to metal complexation at the experimental conditions (pH = 6.7 and 7.20). We found that, in the two independent synthesis of metal complexes, distinct forms of rutin (OH-C5 and O(-)-C5) are present, which are rather difficult to be assessed experimentally. This study was conducted to compile the results of three independent previous studies on the genetic diversity of L. monocytogenes in a poultry production company in Spain and to determine the potential virulence and sanitizer resistance of the strains by using both genotype and phenotype analyses. L. monocytogenes was detected at three production stages a broiler abattoir, a processing plant, and retail stores marketing fresh poultry products from the same company. These three stages spanned three locations in three provinces of Spain. A set of 347 L. monocytogenes isolates representing 39 subtypes was obtained using pulsed-field gel electrophoresis (PFGE). A total of 28 subtypes (68%) had a full-length internalin A gene, and two subtypes had a phenotype with low potential for virulence because of a mutation in the prfA gene. A total of 32 subtypes (82%) were classified as benzalkonium chloride resistant (****R) and contained the resistance determinant bcrABC (21 subtypes, 54%) or the resistance gene qacH (11 subtypes, 28%). A total of 13 persistent ****R subtypes (minimum of 3 months between the first and last sample from with the isolate was recovered) were identified at the abattoir and processing plant. The three production stages shared a unique subtype (PFGE type 1), which had the mutation in the prfA gene and the bcrABC resistance determinant. Whole genome sequencing revealed this subtype to be sequence type 31. Limited genetic diversity was noted in the isolates studied, including some subtypes that were persistent in the environment of the investigated facilities. Given the high prevalence of ****R subtypes, these results support the association between resistance to biocides and persistence of L. monocytogenes. The objective of this article is to evaluate the influence of surface topographical features on bacterial attachment at various processing temperatures to determine the extent of bacterial colonization. Type 304 stainless steel surfaces and pathogenic Listeria monocytogenes Scott A were used for a detailed investigation. Two commonly used surface types, extruded and ground, were evaluated to determine differences in bacterial attachment on the same type of material. Fifteen surface topography parameters at three processing temperatures were studied to evaluate possible correlations with microbial attachment on these surfaces. Scanning electron microscopy, energy-dispersive X-ray spectroscopy, and confocal microscopy were used for both qualitative and quantitative analyses of surfaces. An analysis of variance and multivariate regression analysis were used to predict the attachment behavior of L. monocytogenes Scott A on stainless steel surfaces. Surface isotropy, average surface roughness, surface spacing, and processing temperatures were strongly correlated with bacterial attachment on 304 stainless steel material. Cross-sectional resting state functional connectivity (rsFC) studies in humans identified a circuit between the dorsal anterior cingulate cortex and the ventral striatum that negatively correlated with increased nicotine dependence severity but was unaffected by acute nicotine administration, suggesting a trait marker of addiction. However, whether this trait circuit dysregulation is predispositional to or resultant from nicotine dependence is unclear. Using a rat model of nicotine dependence with longitudinal fMRI measurements, we assessed the relationship between ACC-striatal rsFC and nicotine dependence severity. Data-driven modularity-based parcellation of the rat medial prefrontal cortex (mPFC) combined with seed-based connectivity analysis with the striatum recapitulated the cingulate-striatum relationship observed in humans. Furthermore, the relationship between cingulate-striatal brain circuits and nicotine dependence severity as indexed by the intensity of precipitated withdrawal, was fully statistically moderated by a predispositional insular-frontal cortical functional circuit. These data suggest that the identified trans-species ACC-striatal circuit relationship with nicotine dependence severity is dysregulated following chronic nicotine administration-induced dependence and may be biased by individual differences in predispositional insula-based striatal-frontal circuits, highlighting the circuit's potential as a biomarker of dependence severity.Repetitive transcranial magnetic stimulation (rTMS) is a commonly- used treatment for major depressive disorder (MDD). Here, we performed a randomized, sham-controlled clinical trial and measured electrophysiological, neuroimaging, and clinical changes before and after rTMS. Polycystic ovary syndrome (PCOS) is the most common cause of female infertility. Growth differentiation factor-8 (GDF-8) is expressed in the ovary and can be detected in human follicular fluid which provides an important microenvironment for maintaining physiological functions of the ovarian follicle. To date, the relationship between GDF-8 levels in follicular fluid and the risk of PCOS is completely unknown. In the present study, we show that during the process of the controlled ovarian hyperstimulation (COH), serum GDF-8 levels are higher on the day of gonadotropin administration and 14 days after embryo transfer in in vitro fertilization (IVF) patients with PCOS than they are in IVF patients without PCOS. Importantly, GDF-8 levels in follicular fluid at oocyte retrieval are also higher in PCOS patients than in non-PCOS patients. Treatment of primary human granulosa-lutein (hGL) cells with GDF-8 downregulates steroidogenic acute regulatory protein (StAR) expression and the inhibition is more pronounced in hGL cells from PCOS patients than it is in cells from non-PCOS patients. Importantly, high GDF-8 levels and low progesterone (P4) levels were associated with poor pregnancy outcomes in PCOS patients. The sample was predominately Caucasian (n = 14, 58%). Exercise planning (ρ=0.54), exercise outcome expectations (ρ=0.46), self-efficacy for overcoming barriers (ρ = 0.52), and exercise social support (ρ=0.58) were associated with GLTEQ scores. CONCLUSIONS Researchers might develop behavioral interventions that target exercise outcome expectations, exercise planning, self-efficacy for overcoming barriers, and exercise social support for changing physical activity in pediatric onset MS. The modeling framework developed here is broadly applicable and can be used to explain the biodiversity of many consumer-resource ecosystems and hence deepens our understanding of biodiversity in nature.
Polycystic ovary syndrome (PCOS) is the most common cause of female infertility. Growth differentiation factor-8 (GDF-8) is expressed in the ovary and can be detected in human follicular fluid which provides an important microenvironment for maintaining physiological functions of the ovarian follicle. To date, the relationship between GDF-8 levels in follicular fluid and the risk of PCOS is completely unknown. In the present study, we show that during the process of the controlled ovarian hyperstimulation (COH), serum GDF-8 levels are higher on the day of gonadotropin administration and 14 days after embryo transfer in in vitro fertilization (IVF) patients with PCOS than they are in IVF patients without PCOS. Importantly, GDF-8 levels in follicular fluid at oocyte retrieval are also higher in PCOS patients than in non-PCOS patients. Treatment of primary human granulosa-lutein (hGL) cells with GDF-8 downregulates steroidogenic acute regulatory protein (StAR) expression and the inhibition is more pronounced in hGL cells from PCOS patients than it is in cells from non-PCOS patients. Importantly, high GDF-8 levels and low progesterone (P4) levels were associated with poor pregnancy outcomes in PCOS patients. Our results provide the first evidence that aberrant expression of GDF-8 in the follicular fluid of PCOS patients results in abnormal P4 expression, which leads to poor pregnancy outcomes.BACKGROUND Papillary thyroid cancer (PTC) patients with clinical negative central lymph nodes (cN0), the use of prophylactic central lymph node dissection remains controversial. METHODS Messenger RNA was isolated from serum sample from 124 osteosarcoma patients, 60 periostitis patients and 60 healthy volunteers. The serum miR-194 level was then examined by quantitative real-time polymerase chain reaction (qRT-PCR). The bioinformatic analysis of the downstream targets of miR-194 was also performed. RESULTS The results showed serum miR-194 levels were significantly decreased in osteosarcoma patients compared to those in periostitis patients or healthy controls. Receiver-operating characteristic (ROC) analysis demonstrated that serum miR-194 had a good diagnostic value for identifying osteosarcoma subjects from periostitis patients and normal controls. In addition, serum miR-194 levels were dramatically increased following surgery in osteosarcoma cases. Moreover, low serum miR-194 expression was strongly correlated with positive metastasis and advanced clinical stage, as well as worse survival. Furthermore, serum miR-194 was confirmed to be an independent prognostic biomarker for osteosarcoma. Bioinformatic analysis showed that the downstream targeted genes of miR-194 were closely associated with cancer initiation and development. BACKGROUND, AIMS AND OBJECTIVES Lymphomatoid papulosis (LyP) is a CD30+ lymphoproliferative disorder that is rare and not well described within the pediatric subpopulation. We sought to review the literature and characterize clinical and pathologic features among pediatric and adolescent patients diagnosed with LyP at a tertiary care center. MATERIALS AND METHODS A retrospective cohort of 27 pediatric and adolescent patients (defined as less then 20 years old) diagnosed with LyP at the Weill Cornell Medicine Dermatopathology division from 2006 to 2016 was identified. Subsequently, we reviewed the histopathologic characteristics and collected clinical follow-up data from patients and their providers. The parameters assessed included the pathological LyP subtype including the immunohistochemical staining pattern, the development of secondary lymphoma, disease duration and rate of remission. RESULTS While type A was the most prevalent subtype, B and C subtypes were also frequently observed. CD8 predominance was a common finding, especially among type B LyP patients and those with eccrinotropic granulomatous features. None of the patients with clinical follow-up have developed secondary lymphoma, and some patients experienced remission of their disease. CONCLUSION While type A appears to be the dominant variant described in children, types B, C, and even the newly described variants E and F may occur more often than previously reported. Pediatric LyP may be more indolent than the adult variant, but close clinical follow-up is still warranted. We identified all US pathology residency programs accredited by the Accreditation Council for Graduate Medical Education (ACGME) and searched their websites for information regarding PGR, extracting data on their existence, frequency and timing. For a representative subgroup of institutions from all US regions and program sizes, we tabulated the 2017-2018 PGR titles and presenters (gender, degree(s), resident/fellow, faculty academic rank). We found that 71 of 142 (50%) ACGME-accredited programs had PGR, more often in programs with >12 residents (53/88, 60%). PGR were scheduled most commonly weekly, on Thursdays, and at noon. We analyzed 1019 PGR presentations from 41 institutions located in 26 US states. The most interesting findings were the significant variation in the histologic features including the variation in the amount and the distribution of primordial follicles, follicular cysts, and endosalpingiosis, within the ovary and between both ovaries in the same patient, the frequent presence of primordial follicles in the medulla, specifically in cases of multiple follicular cysts, and the frequent presence of endosalpingiosis. Mental health research evidence translation into policymaking is a 'wicked problem', often failing despite a robust evidence base. In a recent systematic review, we identified a gap in frameworks on agenda setting and actionability, and pragmatic, effective tools to guide action to link research and policy are needed. Responding to this gap, we developed the new EVITA 1.1 (EVIdence To Agenda setting) conceptual framework for mental health research-policy interrelationships in low- and middle-income countries (LMICs). We (1) drafted a provisional framework (EVITA 1.0); (2) validated it for specific applicability to mental health; (3) conducted expert in-depth interviews to (a) validate components and mechanisms and (b) assess intelligibility, functionality, relevance, applicability and effectiveness. To guide interview validation, we developed a simple evaluation framework. (4) Using deductive framewh researchers, policy agencies/planners, think tanks, NGOs and others within the mental health research-policy interface. BHV-0223 is a novel 40 mg rapidly sublingually disintegrating (Zydis®) formulation of riluzole, bioequivalent to conventional riluzole 50 mg oral tablets, that averts the need for swallowing tablets and mitigates first-pass hepatic metabolism, thereby potentially reducing risk of liver toxicity. DILIsym is a validated multi-scale computational model that supports evaluation of liver toxicity risks. DILIsym was used to compare the hepatotoxicity potential of oral riluzole tablets (50 mg BID) versus BHV-0223 (40 mg BID) by integrating clinical data and in vitro toxicity data. In a simulated population (SimPops), ALT levels >3x ULN were predicted in 3.9% (11/285) versus 1.4% (4/285) of individuals with oral riluzole tablets and sublingual BHV-0223, respectively. This represents a relative risk reduction of 64% associated with BHV-0223 vs. conventional riluzole tablets. Mechanistic investigations revealed that oxidative stress was responsible for the predicted ALT elevations. The validity of the DILIsym representation of riluzole and assumptions is supported by its ability to predict rates of ALT elevations for riluzole oral tablets comparable to that observed in clinical data. Combining a mechanistic, quantitative representation of hepatotoxicity with inter-individual variability in both susceptibility and liver exposure suggests that sublingual BHV-0223 confers diminished rates of liver toxicity compared to oral tablets of riluzole, consistent with having a lower overall dose of riluzole and bypassing first-pass liver metabolism. © The Author(s) 2020. Cross-cohort designs can aid causal inference through comparison of associations across populations with different confounding structures. We compared associations between maternal smoking in pregnancy and child conduct and hyperactivity problems, cognition and LBW across two cohorts born four decades apart. METHODS Two national UK cohorts born in 1958 (n = 12 415) and 2000/01 (n = 11 800) were compared. Maternal smoking in pregnancy and child birth weight was assessed at or shortly after birth. Parents rated children's conduct problems and hyperactivity, and children completed standardized tests of reading and mathematics. RESULTS Maternal smoking in pregnancy was less common and more strongly associated with social disadvantage in 2000/01 compared with 1958 (interactions P less then 0.001). Maternal smoking in pregnancy was robustly and equivalently associated with infant LBW in both cohorts [interactions boys odds ratio (OR) = 1.01 (0.89, 1.16), P = 0.838; girls OR = 1.01 (0.91, 1.17), P = 0.633]. Mental health research evidence translation into policymaking is a 'wicked problem', often failing despite a robust evidence base. In a recent systematic review, we identified a gap in frameworks on agenda setting and actionability, and pragmatic, effective tools to guide action to link research and policy are needed. Responding to this gap, we developed the new EVITA 1.1 (EVIdence To Agenda setting) conceptual framework for mental health research-policy interrelationships in low- and middle-income countries (LMICs). We (1) drafted a provisional framework (EVITA 1.0); (2) validated it for specific applicability to mental health; (3) conducted expert in-depth interviews to (a) validate components and mechanisms and (b) assess intelligibility, functionality, relevance, applicability and effectiveness. To guide interview validation, we developed a simple evaluation framework. (4) Using deductive framewh researchers, policy agencies/planners, think tanks, NGOs and others within the mental health research-policy interface. BHV-0223 is a novel 40 mg rapidly sublingually disintegrating (Zydis®) formulation of riluzole, bioequivalent to conventional riluzole 50 mg oral tablets, that averts the need for swallowing tablets and mitigates first-pass hepatic metabolism, thereby potentially reducing risk of liver toxicity. DILIsym is a validated multi-scale computational model that supports evaluation of liver toxicity risks. DILIsym was used to compare the hepatotoxicity potential of oral riluzole tablets (50 mg BID) versus BHV-0223 (40 mg BID) by integrating clinical data and in vitro toxicity data. In a simulated population (SimPops), ALT levels >3x ULN were predicted in 3.9% (11/285) versus 1.4% (4/285) of individuals with oral riluzole tablets and sublingual BHV-0223, respectively. This represents a relative risk reduction of 64% associated with BHV-0223 vs. conventional riluzole tablets. Mechanistic investigations revealed that oxidative stress was responsible for the predicted ALT elevations. The validity of the DILIsym representation of riluzole and assumptions is supported by its ability to predict rates of ALT elevations for riluzole oral tablets comparable to that observed in clinical data. Combining a mechanistic, quantitative representation of hepatotoxicity with inter-individual variability in both susceptibility and liver exposure suggests that sublingual BHV-0223 confers diminished rates of liver toxicity compared to oral tablets of riluzole, consistent with having a lower overall dose of riluzole and bypassing first-pass liver metabolism. © The Author(s) 2020. Background Cervical total disc replacement (CTDR) is complicated by adjacent segment degeneration (ASD). Since non-physiological spine kinematics after CTDR was postulated to cause ASD, M6-C prosthesis has been developed to better replicate the natural kinematics of the intervertebral disc. This retrospective cohort study aims to compare the short-term outcomes between patients receiving either the M6-C or Mobi-C prostheses. Methods Patients who had refractory radiculopathy and/or myelopathy secondary to cervical degenerative disc disease and underwent CTDR between March 2004 and April 2017 were included. All CTDRs were performed by a single surgeon at a single institution. Self-reported clinical outcomes and radiological parameters were evaluated at baseline and final follow-up between March 2004 and April 2018. Results Sixty-two patients with greater than 1-year follow-up or who developed HO within 12 months of surgery, were included in the study. The mean radiological follow-up was 29.0 months (3-84 months), which includes 7 patients with less than 12 months follow-up who also developed HO. The changes in clinical and radiological measures were comparable between M6-C and Mobi-C prostheses. Thirty-seven out of 52 spinal segments (71.2%) and 10 out of 16 spinal segments (62.5%) developed HO in M6-C and Mobi-C group respectively. There was no significant difference in the rate of HO between the two groups. Conclusions No short-term differences were found in clinical or radiological outcomes between patients who received either the M6-C or Mobi-C prosthesis. Further randomized trials with a long-term follow-up period are warranted to determine the safety and efficacy of M6-C prosthesis. 2019 Journal of Spine Surgery. This article investigates a series of actions pertaining to LTA by bringing in reflexive ethnography (Davies, C.A. [2008]. Reflexive Ethnography A Guide to Researching Selves and Others. Routledge London.). The aims are to add a humanistic element to the discipline in general and to feed another piece of applicability information to ethnographers in particular. Attempting at introducing such an approach largely qualitative as ethnography to the field of LTA can be useful because it may open up possibilities for balanced views in assessing language competence in future studies, in which social factors can also be taken explicitly into account for a decision making in LTA. For ethnographers, applicability of reflexive ethnography in LTA could mean the methodology achieves greater generalizability through a larger sample in the universe of admissible observations. A primary research question is whether a change in using CEFR-oriented placement test scores can be justified using empirical reasoning. Participatory observations, situational weigh-in, and statistical analyses are found capable of synergizing using a reflexive-ethnographic perspective for a justifiable decision-making process. © 2020 Published by Elsevier Ltd.The present work investigated the ability of algal biomass Chlorella vulgaris to remove mercury from aqueous solutions. The mercury biosorption process was studied through batch experiments 35 °C temperature with regard to the influence of contact time, initial mercury concentration, pH and desorption. The maximum adsorption capacity was registered at pH 6. The adsorption conduct of Hg(II) was defined by pseudo second order well rather pseudo first order as the experimental data (qe) come to an agreement with the calculated value. The kinetics of adsorption was fast and a high capacity of adsorption occurred within only 90 min. The adsorption data were signified by many models but Langmuir (qmax = 42. mg g-1) & Freundlich fitted well having regression coefficients near to unity. The thermodynamic parameters were also suited well as negative value of free energy cope up to spontaneity, positive value of the randomness described by ΔS attributed to affinity of Hg+2 towards algal bioadsorbant and high positive value of heat of enthalpy designates that the adsorption process is expected due to robust interactions between the Hg(II) ions and various functional groups on surface of algal bioadsorbant. Field emission scanning electron microscopy integrated with energy dispersive X-ray spectroscopy analysis before and after adsorption of Hg(II) reveals the adsorption of metallic ions over the surface. FTIR study supported the existence of various functional groups (carboxylix, amines, hydroxyls, amides etc.) helped in adsorption. Scrub typhus, a disease caused by Orientia tsutsugamushi, affects more than one billion people globally with an average fatality rate of 6%. Humans are accidentally infected through the bite of trombiculid mite larvae (chiggers). Chiggers feed on hosts' extracellular fluid for survival and development. O. tsutsugamushi is maintained throughout the chigger's lifespan and over several generations. Although disease-related knowledge is essential in designing effective control strategies, many personnel in related sectors are unfamiliar with this disease and its vector. To tackle this issue, we developed a distance learning tool using educational videos on scrub typhus- and vector-related topics. The learning method is facilitated online, and students and tutors are not required to be physically present at the same place and time, thus allowing flexibility and accessibility. Knowledge improvement of 34 participants from related sectors was evaluated by pre- and post-test questionnaires. Although 54% of participants had prior knowledge of scrub typhus, 76.5% still lack basic knowledge of vector identification. After the distance learning, the average score increased significantly from the baseline (p less then 0.05). Most participants showed interest in the topic and learning method. These results suggest that the distance learning method was promising in distributing health-related information and might be applied to other diseases and communities.BACKGROUND Inflammation is often considered relating to pediatric obstructive sleep apnea (OSA). We conducted a study investigating cytokines, including Il-17 and Il-23, in children with OSA before and after adenotonsillectomy (T&A), compared with controls. METHODS Children with OSA between age 4 and 12 receiving T&A were prospectively followed. Evaluation before and reevaluation six months after the treatment were done, including polysomnography (PSG), blood tests, and questionnaires. Blood samples were obtained to determine the values of high-sensitivity-C-reactive-protein (HS-CRP); tumor-necrosis-factor-alpha (TNF-α); and interleukin (IL)-1, 6, 10, 12, 17, and 23. We compared the results with an age-matched control group. RESULTS We included 55 OSA children and 32 controls. Children with OSA presented significant improvement after T&A in complaints, signs, apnea hypopnea index (AHI) (p less then 0.001), mean oxygen desaturation index (p less then 0.001), and mean oxygen saturation (p = 0.010). Upon entering this study, children with OSA had significantly higher cytokine levels than the controls and significant changes in HS-CRP (p = 0.013), TNF-α (p = 0.057), IL-1β (p = 0.022), IL-10 (p = 0.035), and IL-17 (p = 0.010) after T&A. Children with improved but persistently abnormal AHI did not have all cytokine levels normalized, particularly IL-23 and HS-CRP. CONCLUSION Sleep-disordered breathing can persist after T&A and can continue to have a negative inflammatory effect. HS-CRP and IL-23 may serve as blood markers for the persistence of sleep-disordered breathing after T&A.Caffeine is the most consumed psychostimulant worldwide. The aim of this study was to evaluate the ability to cross the BBB of triterpenoid saponins occurring in Astragalus mongholicus roots. The research was carried out using in silico methods combined with postmortem studies on the brain tissues of **** treated with isolated astragaloside IV (AIV). Firstly, to estimate the ability to cross the BBB by the tested saponins, new quantitative structure-activity relationship (QSAR) models were established. The reliability and predictability of the model based on the values of the blood-brain barrier penetration descriptor (logBB), the difference between the n-octanol/water and cyclohexane/water logP (ΔlogP), the logarithm of n-octanol/water partition coefficient (logPow), and the excess molar refraction (E) were both confirmed using the applicability domain (AD). The critical leverage value h* was found to be 0.128. The relationships between the standardized residuals and the leverages were investigated here. The application of an in vitro acetylcholinesterase-inhibition test showed that AIV can be recognized as the strongest inhibitor among the tested compounds. Therefore, it was isolated for the postmortem studies on brain tissues and blood using semi-preparative HPLC with the mobile phase composed of water, methanol, and ethyl acetate (1.72.116.2 v/v/v). The results of the postmortem studies on the brain tissues show a regular dependence of the final concentration of AIV in the analyzed brain samples of animals treated with 12.5 and 25 mg/kg b.w. of AIV (0.00012299 and 0.0002306 mg, respectively, per one brain). Blood samples were obtained to determine the values of high-sensitivity-C-reactive-protein (HS-CRP); tumor-necrosis-factor-alpha (TNF-α); and interleukin (IL)-1, 6, 10, 12, 17, and 23. We compared the results with an age-matched control group. RESULTS We included 55 OSA children and 32 controls. Children with OSA presented significant improvement after T&A in complaints, signs, apnea hypopnea index (AHI) (p less then 0.001), mean oxygen desaturation index (p less then 0.001), and mean oxygen saturation (p = 0.010). Upon entering this study, children with OSA had significantly higher cytokine levels than the controls and significant changes in HS-CRP (p = 0.013), TNF-α (p = 0.057), IL-1β (p = 0.022), IL-10 (p = 0.035), and IL-17 (p = 0.010) after T&A. Children with improved but persistently abnormal AHI did not have all cytokine levels normalized, particularly IL-23 and HS-CRP. CONCLUSION Sleep-disordered breathing can persist after T&A and can continue to have a negative inflammatory effect. HS-CRP and IL-23 may serve as blood markers for the persistence of sleep-disordered breathing after T&A.Caffeine is the most consumed psychostimulant worldwide. The aim of this study was to evaluate the ability to cross the BBB of triterpenoid saponins occurring in Astragalus mongholicus roots. The research was carried out using in silico methods combined with postmortem studies on the brain tissues of mice treated with isolated astragaloside IV (AIV). Firstly, to estimate the ability to cross the BBB by the tested saponins, new quantitative structure-activity relationship (QSAR) models were established. The reliability and predictability of the model based on the values of the blood-brain barrier penetration descriptor (logBB), the difference between the n-octanol/water and cyclohexane/water logP (ΔlogP), the logarithm of n-octanol/water partition coefficient (logPow), and the excess molar refraction (E) were both confirmed using the applicability domain (AD). The critical leverage value h* was found to be 0.128. The relationships between the standardized residuals and the leverages were investigated here. The application of an in vitro acetylcholinesterase-inhibition test showed that AIV can be recognized as the strongest inhibitor among the tested compounds. Therefore, it was isolated for the postmortem studies on brain tissues and blood using semi-preparative HPLC with the mobile phase composed of water, methanol, and ethyl acetate (1.72.116.2 v/v/v). The results of the postmortem studies on the brain tissues show a regular dependence of the final concentration of AIV in the analyzed brain samples of animals treated with 12.5 and 25 mg/kg b.w. of AIV (0.00012299 and 0.0002306 mg, respectively, per one brain).
