The subjects were randomly divided into Groups A (13 patients with 16 lesions, treated with 3% polidocanol foam sclerosing agent) and B (14 patients with 19 lesions, treated with pingyangmycin + dexamethasone), respectively. The clinical efficacies and adverse reactions were analyzed and compared between these two groups. The average number of treatment times for Group A was 2.45±0.6, with an efficacy rate of 87.50%, while the average number of treatment times for Group B was 2.07±0.4, with an efficacy rate of 84.21%. No significant difference was found in the average treatment times or efficacy rates between Groups A and B. In addition, the adverse reaction incidence for Groups A and B were 38.46 and 14.29%, respectively, with statistically significant differences between these two groups. In the present study, the role of miR-25 in PDAC cell proliferation was investigated. Upregulated expression of miR-25 was found in PDAC tissues and cell lines by reverse transcription-quantitative PCR. Cell proliferation was significantly enhanced by overexpression of miR-25 as shown by CCK-8 assay results. Meanwhile, overexpression of miR-25 also promoted G1-to-S phase transition of the cell cycle in Aspc-1 cells via flow cytometry analysis. However downregulation of miR-25 inhibited the tumor cell proliferation and cell cycle transition. Online software was used to predict the target gene for miR-25 and luciferase reporter assay confirmed that Abl interactor 2 (ABI2) was a target of miR-25 via direct binding of its 3' untranslated region with miR-25. This study analyzes the impact of extended coverage on adverse outcomes and aortic remodeling in patients with complicated acute type B aortic dissection (aTBAD). METHODS From January 2012 to October 2018, 91 patients underwent TEVAR for aTBAD. Median follow-up was 3.1 (interquartile range, 1.2-4.9) years and was complete in 94% of patients. The extent of aortic endograft coverage was categorized as standard (n = 39) or extended (n = 52). Contrast-enhanced imaging scans were analyzed to determine length of coverage, maximum aortic diameters, and false lumen (FL) status. RESULTS The mean age was 52.6 ± 13.9 years, and 66% were men. The most common indications for intervention were malperfusion (42%) and refractory pain (34%). Thirteen (14%) patients required a lumbar drain (preoperative n = 3; postoperative n = 10). Mean duration between scans was 2.0 ± 1.9 years. Length of aortic coverage was significantly longer in the extended group (241.7 ± 29.2 mm vs 180.8 ± 22.3 mm in the standard group; P less then .001). In-hospital and overall mortality were 6% and 11%, respectively. There were no cases of paraplegia, and the incidence of spinal cord ischemia was 3%. After TEVAR, there was a higher incidence of FL obliteration or thrombosis at the distal descending thoracic aorta in the extended group (53% vs 16% in the standard group; P = .004). CONCLUSIONS Extended TEVAR carries a low risk of spinal cord ischemia and improves FL remodeling of the descending thoracic aorta in patients with aTBAD. This case report describes the use of existing tools and prosthesis to create a novel miniature Y stent for a stenotic lobar bronchus that rapidly bifurcates, not allowing the insertion of a standard bronchial stent. BACKGROUND Bicuspid aortic valve-associated ascending thoracic aortic aneurysms (BAV-aTAAs) carry a risk of acute type A dissection. Biomechanically, dissection may occur when wall stress exceeds wall strength. Our aim was to develop patient-specific computational models of BAV-aTAAs to determine magnitudes of wall stress by anatomic regions. METHODS Patients with BAV-aTAA diameter greater than 4.5 cm (n = 41) underwent electrocardiogram-gated computed tomography angiography. Three-dimensional aneurysm geometries were reconstructed after accounting for prestress and loaded to systemic pressure. Finite element analyses were performed with fiber-embedded hyperelastic material model using LS-DYNA software (LSTC Inc, Livermore, CA) to obtain wall stress distributions. The 99th percentile longitudinal and circumferential stresses were determined at systole. RESULTS The 99th percentile longitudinal wall stresses for BAV-aTAAs at sinuses of Valsalva, sinotubular junction (STJ), and ascending aorta were 361 ± 59.8 kPa, 295 ± 67.2 kPa, and 224 ± 37.6 kPa, respectively, with significant differences in ascending aorta vs sinuses (P less then 1 × 10-13) and STJ (P less then 1 × 10-6). The 99th percentile circumferential wall stresses were 474 ± 88.2 kPa, 634 ± 181.9 kPa, and 381 ± 54.0 kPa for sinuses, the STJ, and the ascending aorta, respectively, with significant differences in the ascending aorta vs sinuses (P = .002) and STJ (P less then 1 × 10-13). CONCLUSIONS Wall stresses, both circumferential and longitudinal, were greater in the aortic root, sinuses, and STJ than in the ascending aorta on BAV-aTAAs. These results fill a fundamental knowledge gap regarding biomechanical stress distribution in BAV-aTAA patients, which when related to wall strength may provide prognostication of aTAA dissection risk by patient-specific modeling. Between 1999-2004, 3242 adults aged 20 to 49 years (weighted N=59 888 450; mean age, 33.8 ± 0.2 years) underwent submaximal treadmill exercise test in the National Health and Nutrition Examination Survey study. Weighted Cox proportional hazards regression were used to evaluate the association of cardiorespiratory fitness with premature death at 65 years or younger RESULTS During a mean follow-up of 13.8 years, 104 deaths (weighted deaths N =1 326 808) occurred. Low cardiorespiratory fitness was associated with an increased risk of premature death due to all-cause (Hazard ratio [HR], low vs. high 2.26; 95% CI, 1.10 to 4.64, p for trend=.036) and cancer mortality (HR low vs. moderate/high 6.53; 95% CI, 2.38 to 17.9). Further, this association was stronger in adults aged 35 to 49 years at baseline (HR, 4.17 [95% CI, 1.19 to 9.11]). CONCLUSION We observed an inverse association between cardiorespiratory fitness during middle adulthood and premature death, which was not detected in proceeding generations. These findings suggested that low cardiorespiratory fitness might be emerging to a new risk factor for early death among US Baby Boomers and Generation Xers.

OBJECTIVES The objectives were to verify the effect of additional rods and determine the optimal type of additional rod for preventing rod fracture (RF). RF is a frequent complication following 3-CO surgery in patients with adult spinal deformity. Two types of additional rod constructs were utilized to prevent RF. METHODS We evaluated data retrieved from a prospectively collected single-center database. Patients were classified into two groups depending on rod constructs conventional 2-rod and 2-rod constructs with additional rods. The additional rod constructs were also stratified into two groups according to the location that they covered only osteotomy site (short additional rod) or osteotomy site and lumbosacral junction (long additional rod). RESULTS A total of 48 patients (average age 67.6 years; follow-up rate 90.6%; 2-rod 25 cases and additional rod 23 cases) were included. No significant difference was observed in age, fusion level, operation time, blood loss, and rod composition. The incidence of RF in the additional rods (36%) was significantly lower than that in the 2-rod constructs (68%). Comparing the additional rod constructs, RF occurred in 7 cases (64%) in short additional rod and 2 cases (17%) in long additional rod. RF occurred at the level below the additional rod construct in 6 of 7 RF cases with short additional rod. CONCLUSIONS Additional rods covering the osteotomy site and lumbosacral junction reduced the incidence of RF following 3-CO surgery with pelvic fixation in short term. Children with EOS have diminished respiratory reserve which is associated with abnormal breathing and sleep quality in children. Currently, there are no criteria for referral of these children to evaluate breathing during sleep or data on the use of sleep treatments as part of their supportive care. A review of the 159 patients with EOS who were followed at a single institution from 2003 to 2016 identified 68 who underwent overnight polysomnograms (PSGs). Sixty-five of 68 (96%) had elevated apnea-hypopnea index (AHI) and a majority (56%) were prescribed nighttime respiratory support. A majority of young children ( less then 5 years) with PSG were referred for a history of snoring, apnea, or restless sleep; all 30 had abnormal PSGs. Twenty-seven (90%) had nighttime hypoxemia (nadir oxygen saturation values less then 92%). Eighteen (60%) were referred to otolaryngology, of whom 11 (37%) subsequently underwent tonsil and/or adenoid removal. In older children (≥ 5 years), those referred for PSGs had more severe restrictive chest wall disease [lower forced vital capacity (FVC) values] than those who were not sent for PSG. Correlation between FVC and apnea-hypopnea index, however, was not significant. Pre-operative coronal curve magnitude did not strongly correlate with nadir SaO2 or AHI in either age group. These results suggest that sleep studies are underutilized in the management of children with EOS. Inadequate and poor-quality sleep adversely affects growth, behavior, and cognitive function in children. Within each part, remarkable examples from the past 10 years cited in Retraction Watch are indicated. At last, possible solution on combating misconduct are suggested. RESULTS The number of publications are on the dramatic rise fostering a competition under which scholars are pushed to publish more.

AIMS Biliary intraductal tubular neoplasms that are non-mucinous and negative for MUC5AC are called intraductal tubulopapillary neoplasm (ITPNs). Intraductal tubular neoplasms with mucinous cytoplasm and MUC5AC positivity also occur, and their nature remains unclear although some pathologists may classify these as "IPNBs of gastric type". This study aimed to elucidate genetic features of biliary intraductal tubular neoplasms. METHODS Six resected cases of biliary intraductal neoplasm with >70% tubular configuration were characterized by the clinicopathological examination and whole exome sequencing, and obtained findings were compared between MUC5AC-positive (n=2) and -negative cases (n=4). RESULTS The intraductal tumours consisted of the pancreatobiliary-type epithelium with high-grade dysplasia arranged in ****-to-**** tubules. Both two MUC5AC-negative cases were non-invasive neoplasms developed in the liver, whereas all MUC5AC-positive cases had invasive carcinoma and were present in the intrahepatic (n=2), perihilar (n=1), and distal bile ducts (n=1). In an exome-sequencing study, MUC5AC-negative cases harboured mutations in CTNNB1, SF3B1, BAP1, and BRCA1 (one case each). KRAS mutations were observed in 3/4 MUC5AC-positive cases (75%) but none of MUC5AC-negative neoplasms. Compared to published data, known driver genes of other intraductal neoplasms of the pancreatobiliary systems (e.g., APC, CTNNB1, STK11, GNAS, and PIK3CA) were wild-type in all but one MUC5AC-negative case with CTNNB1 mutation. Chromatin modifiers (ARID1A, BAP1, and KMT2C) were also altered in MUC5AC-positive cases, similar to usual cholangiocarcinomas. CONCLUSIONS This exome-sequencing study suggested that MUC5AC-negative biliary ITPNs are genetically distinct from pancreatic ITPNs and IPNBs. All rights reserved.The present study investigates mean-level differences in eight comic styles (fun, benevolent humor, nonsense, wit, irony, satire, sarcasm, and cynicism) depending on the sociodemographic variables gender, age, education, political spectrum, and religious affiliation. A large and varied Chilean adult sample (N = 1,272, 60.1% women; age M = 39.94, SD = 17.33) was recruited in face-to-face interviews and online testing. They completed self-reports of the comic styles (the Comic Style Markers) and sociodemographic variables. Overall, small, but meaningful, differences in comic styles were found for the different sociodemographic groups. Men scored higher than women in all comic styles except for benevolent humor, and six styles decreased with age. Having lower education was associated with more cynicism, while the reverse pattern was found for satire and wit. Religious people showed lower scores in four styles than non-religious people, and cynicism was higher in those identifying with left wing rather than right-wing attitudes. AIMS Biliary intraductal tubular neoplasms that are non-mucinous and negative for MUC5AC are called intraductal tubulopapillary neoplasm (ITPNs). Intraductal tubular neoplasms with mucinous cytoplasm and MUC5AC positivity also occur, and their nature remains unclear although some pathologists may classify these as "IPNBs of gastric type". This study aimed to elucidate genetic features of biliary intraductal tubular neoplasms. METHODS Six resected cases of biliary intraductal neoplasm with >70% tubular configuration were characterized by the clinicopathological examination and whole exome sequencing, and obtained findings were compared between MUC5AC-positive (n=2) and -negative cases (n=4). RESULTS The intraductal tumours consisted of the pancreatobiliary-type epithelium with high-grade dysplasia arranged in back-to-back tubules. Both two MUC5AC-negative cases were non-invasive neoplasms developed in the liver, whereas all MUC5AC-positive cases had invasive carcinoma and were present in the intrahepatic (n=2), perihilar (n=1), and distal bile ducts (n=1). In an exome-sequencing study, MUC5AC-negative cases harboured mutations in CTNNB1, SF3B1, BAP1, and BRCA1 (one case each). KRAS mutations were observed in 3/4 MUC5AC-positive cases (75%) but none of MUC5AC-negative neoplasms. Compared to published data, known driver genes of other intraductal neoplasms of the pancreatobiliary systems (e.g., APC, CTNNB1, STK11, GNAS, and PIK3CA) were wild-type in all but one MUC5AC-negative case with CTNNB1 mutation. Chromatin modifiers (ARID1A, BAP1, and KMT2C) were also altered in MUC5AC-positive cases, similar to usual cholangiocarcinomas. CONCLUSIONS This exome-sequencing study suggested that MUC5AC-negative biliary ITPNs are genetically distinct from pancreatic ITPNs and IPNBs. All rights reserved.The present study investigates mean-level differences in eight comic styles (fun, benevolent humor, nonsense, wit, irony, satire, sarcasm, and cynicism) depending on the sociodemographic variables gender, age, education, political spectrum, and religious affiliation. A large and varied Chilean adult sample (N = 1,272, 60.1% women; age M = 39.94, SD = 17.33) was recruited in face-to-face interviews and online testing. They completed self-reports of the comic styles (the Comic Style Markers) and sociodemographic variables. Overall, small, but meaningful, differences in comic styles were found for the different sociodemographic groups. Men scored higher than women in all comic styles except for benevolent humor, and six styles decreased with age. Having lower education was associated with more cynicism, while the reverse pattern was found for satire and wit. Religious people showed lower scores in four styles than non-religious people, and cynicism was higher in those identifying with left wing rather than right-wing attitudes. To address both senescence and resistance to ferroptosis, we synthesized a modified, catalytic, and rapidly internalized carbon nanomaterial, poly(ethylene glycol)-conjugated hydrophilic carbon clusters (PEG-HCC) by covalently bonding the iron chelator, deferoxamine (DEF). This multifunctional nanoparticle, DEF-HCC-PEG, protected cells from both senescence and ferroptosis and restored nuclear and mitochondrial genome integrity in vitro and in vivo. We thus describe a potential molecular mechanism of hemin/iron-induced toxicity in ICH that involves a rapid induction of DSBs, senescence, and the consequent resistance to ferroptosis and provide a mechanistic-based combinatorial therapeutic strategy.The critical bottleneck of electrocatalytic CO2 reduction reaction (CO2RR) lies in its low efficiency at high overpotential caused by competitive hydrogen evolution. It is challenging to develop an efficient catalyst achieving both high current density and high Faradaic efficiency (FE) for CO2RR. Herein, we synthesized fluorine-doped cagelike porous carbon (F-CPC) by purposely tailoring its structural properties. The optimized F-CPC possesses large surface area with moderate mesopore and abundant micropores as well as high electrical conductivity. When used as catalyst for CO2RR, F-CPC exhibits FE of 88.3% for CO at -1.0 V vs RHE with a current density of 37.5 mA·cm -2. Experimental results and finite element simulations demonstrate that the excellent CO2RR performance of F-CPC at high overpotential should be attributed to its structure-enhanced electrocatalytic process stemming from its cagelike morphology.Successful translation of laboratory-based surface-enhanced Raman scattering (SERS) platforms to clinical applications requires multiplex and ultratrace detection of small biomarker molecules from a complex biofluid. Herein, we demonstrate a "confine-and-capture" approach for multiplex detection of two families of urine metabolites correlated with miscarriage risks, 5β-pregnane-3α,20α-diol-3α-glucuronide and tetrahydrocortisone. To enhance SERS signals by 1012-fold, we use specific nanoscale surface chemistry for targeted metabolite capture from a complex urine matrix prior to confining them on a superhydrophobic SERS platform. We then apply chemometrics, including principal component analysis and partial least-squares regression, to convert molecular fingerprint information into quantifiable readouts. The whole screening procedure requires only 30 min, including urine pretreatment, sample drying on the SERS platform, SERS measurements, and chemometric analyses. These readouts correlate well with the pregnancy outcomes in a case-control study of 40 patients presenting threatened miscarriage symptoms.Nanopores bear great potential as single-molecule tools for bioanalytical sensing and sequencing, due to their exceptional sensing capabilities, high-throughput, and low cost. We find that two sequential steps in the crystallization process-nucleation and layer growth-can occur on similar time scales and can be controlled independently using different growth parameters. Importantly, the layer growth process contributes significantly to the growth time for all conditions and will play a major role in determining material properties such as compositional uniformity, dopant density, and impurity incorporation. The results are understood through theoretical simulations correlating the growth dynamics, liquid droplet, and experimental parameters. The key insights discussed here are not restricted to Au-catalyzed GaAs nanowire growth but can be extended to most compound nanowire growths in which the different growth species has very different solubility in the catalyst particle.The cancer cell membrane contains an arsenal of highly specific homotypic moieties that can be used to recognize its own kind. These cell membranes are often used to coat spherical nanoparticles to enhance nanomedicines' targeting specificities and uptakes. A sphere, however, has only a point contact with a surface at any given time. It is shown here that, by retaining a flatter morphology of the cracked cell membrane through stiffening with in situ synthesized gold nanomaterials, an increased area of interaction could be maintained and hence improve upon the in vitro and in vivo homotypic targeting capabilities between cancer cell types. This enhancement is especially important in vivo as any nanomedicine with targeting moieties probably has a single pass at interacting with the target cell before subsequent system clearance. Possible future clinical applications may involve the usage of a patient's autologous tumor biopsy tissues, which are very limited in supply, and therefore ensuring that we capitalize on the entire collective surface area of the cancer cell membrane available becomes an important consideration in the design and delivery our cell membrane-derived nanomedicines.

AIM Pylorus-preserving gastrectomy (PPG) is widely used for early gastric cancer located in the middle third of the stomach. The minimization of the extent of gastrectomy may increase the risk of metachronous multiple gastric cancer (MGC). We report the findings of a study that was conducted to evaluate the prevalence of ****after PPG. METHODS The clinical data of 533 patients who underwent PPG for gastric cancer between 1993 and 2018 were reviewed. The clinicopathological characteristics at the time of the primary treatment that were predictive of the development of ****were explored. The median (range) observation period was 112.4 (8.1-290.7) months. RESULTS Metachronous ****was diagnosed in 33 of the 533 patients. The cumulative 5-year and 10-year event rates were 3.3% and 6.2%, respectively. The patient gender, presence/absence of synchronous ****and the macroscopic type of the primary gastric cancer were significantly associated with the risk of development of metachronous MGC. Multivariate analysis identified the presence of synchronous MGC (hazard ratio [HR] 4.828, 95% confidence interval [CI]; 1.611-12.30, p = 0.004) and Type 0-IIa primary gastric cancer (HR 2.810, 95% CI; 1.113-7.090, p = 0.029) as independent factors associated with the risk of development of MGC. All the patients could be treated by surgical or endoscopic resection for the metachronous MGC. Recurrence was observed in one patient. CONCLUSIONS There was quite a few incidence of development of metachronous ****after PPG. Nevertheless, PPG remains reasonable treatment option, if adequate postoperative surveillance can be ensured.BACKGROUND Percutaneous cholecystostomy (PC) is often performed for patients with acute cholecystitis who are too high risk for cholecystectomy. The purpose of this retrospective study was to evaluate the outcomes of this cohort of patients over a 5-year period. METHODS A retrospective analysis of all patients treated with PC for acute cholecystitis in a tertiary centre teaching hospital was conducted. The study period ranged from January 2010 to December 2015. Clinical data were extracted from the hospitals' electronic database system, as well as reviewing clinical notes and imaging reports. The aims of this study were to detect the reason PC was undertaken as opposed to surgery, the subsequent definitive management of patients initially treated with PC, the incidence of common bile duct stones (CBDS), the complications from PC, and the 30-day mortality. RESULTS A total of 96 patients were identified. The total number of patients with CBDS was 27 (28.1%). Fourteen (14.6%) patients were shown to have CBDS on a check cholangiogram) is warranted to detect missed CBDS. Immunohistochemistry was used to detect VISTA, PD-1, PD-L1, and CD8 in tissue microarrays from 919 patients with IDC (N = 341 in the exploratory cohort and = 578 in the validation cohort). VISTA was expressed on the immune cells of 29.1% (267/919) of the samples and on the tumor cells of 8.2% (75/919). VISTA was more frequently expressed in samples that were estrogen receptor-negative, progesterone receptor-negative, human epidermal growth factor receptor 2-positive, poorly differentiated, human epidermal growth factor receptor 2-enriched, and consisting of basal-like tumors. VISTA on immune cells correlated with PD-1, PD-L1, stromal CD8, and tumor-infiltrating lymphocyte expression and was an independent prognostic factor for improved relapse-free and disease-specific survival in patients with estrogen receptor-negative, progesterone receptor-negative, and basal-like IDC. The interactions between different I-waves are reflected in the facilitatory peaks elicited with a short-intracortical facilitation (SICF) protocol when two pulses are sent to M1 at precise intervals (i.e., 1.3, 2.5 or 4.1 ms). The minimization of the extent of gastrectomy may increase the risk of metachronous multiple gastric cancer (MGC). We report the findings of a study that was conducted to evaluate the prevalence of MGC after PPG. METHODS The clinical data of 533 patients who underwent PPG for gastric cancer between 1993 and 2018 were reviewed. The clinicopathological characteristics at the time of the primary treatment that were predictive of the development of MGC were explored. The median (range) observation period was 112.4 (8.1-290.7) months. RESULTS Metachronous MGC was diagnosed in 33 of the 533 patients. The cumulative 5-year and 10-year event rates were 3.3% and 6.2%, respectively. The patient gender, presence/absence of synchronous MGC and the macroscopic type of the primary gastric cancer were significantly associated with the risk of development of metachronous MGC. Multivariate analysis identified the presence of synchronous MGC (hazard ratio [HR] 4.828, 95% confidence interval [CI]; 1.611-12.30, p = 0.004) and Type 0-IIa primary gastric cancer (HR 2.810, 95% CI; 1.113-7.090, p = 0.029) as independent factors associated with the risk of development of MGC. All the patients could be treated by surgical or endoscopic resection for the metachronous MGC. Recurrence was observed in one patient. CONCLUSIONS There was quite a few incidence of development of metachronous MGC after PPG. Nevertheless, PPG remains reasonable treatment option, if adequate postoperative surveillance can be ensured.BACKGROUND Percutaneous cholecystostomy (PC) is often performed for patients with acute cholecystitis who are too high risk for cholecystectomy. The purpose of this retrospective study was to evaluate the outcomes of this cohort of patients over a 5-year period. METHODS A retrospective analysis of all patients treated with PC for acute cholecystitis in a tertiary centre teaching hospital was conducted. The study period ranged from January 2010 to December 2015. Clinical data were extracted from the hospitals' electronic database system, as well as reviewing clinical notes and imaging reports. The aims of this study were to detect the reason PC was undertaken as opposed to surgery, the subsequent definitive management of patients initially treated with PC, the incidence of common bile duct stones (CBDS), the complications from PC, and the 30-day mortality. This study aimed to investigate the expression of VISTA and its association with clinicopathologic parameters as weNll as with the key immune markers including programmed cell death-1 (PD-1) and PD-1 ligand-1 (PD-L1) in invasive ductal carcinoma (IDC) of the breast. Immunohistochemistry was used to detect VISTA, PD-1, PD-L1, and CD8 in tissue microarrays from 919 patients with IDC (N = 341 in the exploratory cohort and = 578 in the validation cohort). VISTA was expressed on the immune cells of 29.1% (267/919) of the samples and on the tumor cells of 8.2% (75/919). VISTA was more frequently expressed in samples that were estrogen receptor-negative, progesterone receptor-negative, human epidermal growth factor receptor 2-positive, poorly differentiated, human epidermal growth factor receptor 2-enriched, and consisting of basal-like tumors. VISTA on immune cells correlated with PD-1, PD-L1, stromal CD8, and tumor-infiltrating lymphocyte expression and was an independent prognostic factor for improved relapse-free and disease-specific survival in patients with estrogen receptor-negative, progesterone receptor-negative, and basal-like IDC.
