In a large-scale medical examination, the UK Biobank study has successfully imaged more than 32,000 volunteer participants with magnetic resonance imaging (MRI). Each scan is linked to extensive metadata, providing a comprehensive medical survey of imaged anatomy and related health states. Despite its potential for research, this vast amount of data presents a challenge to established methods of evaluation, which often rely on manual input. To date, the range of reference values for cardiovascular and metabolic risk factors is therefore incomplete. In this work, neural networks were trained for image-based regression to infer various biological metrics from the neck-to-knee body MRI automatically. The approach requires no manual intervention or direct access to reference segmentations for training. The examined fields span 64 variables derived from anthropometric measurements, dual-energy X-ray absorptiometry (DXA), atlas-based segmentations, and dedicated liver scans. With the ResNet50, the standardized framework achieves a close fit to the target values (median R[Formula see text]) in cross-validation. Interpretation of aggregated saliency maps suggests that the network correctly targets specific body regions and limbs, and learned to emulate different modalities. On several body composition metrics, the quality of the predictions is within the range of variability observed between established gold standard techniques.The implementation of agronomic activities, based on the use of biostimulants, is an important element of agroecological practices. Therefore, comprehensive research was carried on the use of biostimulants. A field experiment was performed in 2016-2018 with common bean of Mexican Black cultivar. In particular growing seasons, bean plants were treated with Kelpak SL (seaweed extracts) and Terra Sorb Complex (free amino acids) in the form of single and double spraying with two solutions concentrations. According to the obtained data, application of biostimulants increased the yield of bean. Better results were observed after the use of Kelpak SL. The application of preparations influenced nutritional and nutraceutical quality of bean seeds. Terra Sorb Complex caused the highest increase in proteins level. In the light of achieved data, biostimulants in similar level decreased the starch accumulation. The most promising results, in the context of nutraceutical value of bean, were obtained in the case of increasing level of fiber. A positive impact of biostimulants on the seeds antioxidant potential was noted, expressed by the increased synthesis of phenolics, flavonoid, anthocyanins and antioxidant activities. Results of this study, directly indicate economic benefits from the use of biostimulants, which are extremely important to the farmers.Bone marrow mesenchymal stromal cells (BM-****) have a critical role in tissue regeneration and in the hematopoietic niche due to their differentiation and self-renewal capacities. We found that there is a bias to the mutations occurring in SARS-CoV-2 variants, with disproportionate mutation to uracil (U). These point mutations to U are mainly derived from cytosine (C), which is consistent with the substrate specificity of host RNA editing enzymes, APOBECs. We also found the point mutations which are consistent with other RNA editing enzymes, ADARs. For the C-to-U mutations, the context of the upstream uracil and downstream guanine from mutated position was found to be most prevalent. Further, the degree of increase of U in SARS-CoV-2 variants correlates with enhanced production of cytokines, such as TNF-α and IL-6, in cell lines when compared with stimulation by the ssRNA sequence of the isolated virus in Wuhan. Humans have been suffered from viral infections over the centuries, such as influenza, HSV, and HIV, which have killed millions of people worldwide. However, the availability of effective treatments for infectious diseases remains limited until now, as most of the viral pathogens resisted to many medical treatments. Marine microbes are currently one of the most copious sources of pharmacologically active natural products, which have constantly provided promising antivirus agents. To date, a large number of marine microbial secondary metabolites with antiviral activities have been widely reported. In this review, we have summarized the potential antivirus compounds from marine microorganisms over the last decade. In addition, the structures, bioactivities, and origins of these compounds were discussed as well.Neuroblastoma (NB) is one of the most common pediatric malignancies. Easy metastasis, poor prognosis, and a high degree of heterogeneity of NB hinder its successful treatment. Several different therapeutic strategies have been developed to overcome these problems, including differentiation and targeted therapy. In this review, we summarize the recent development of differentiation modulators and targeted agents for treating NB. Several promising targets of NB were also discussed.
Saroglitazar is commonly used in India for managing hypertriglyceridemia in diabetes. This meta-analysis evaluated the efficacy and safety of saroglitazar in hypertriglyceridemia.
Electronic databases were searched for RCTs involving diabetes patients receiving saroglitazar in intervention arm, and placebo/lipid/diabetes medication in the control arm. Primary outcome was to evaluate change in serum triglyceride and HbA1c. Secondary outcomes were to evaluate changes in other lipid parameters, glycaemia and adverse effects. To explore the relationship between symptomless multi-Variable apnea prediction (sMVAP) index and adverse outcomes of patients with Corona Virus Disease 2019 (COVID-19).

The clinical electronic medical records, nursing records, laboratory findings, and radiological examinations for all patients with laboratory confirmed Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection were reviewed. Cox proportional hazard ratio (HR) models were used to determine the risk factors associated with in hospital death.
A total of 97 patients were included in this study. The "Quartile 4" group 's ICU transfer rate was significantly higher than the "Quartile 1" group. Coronary heart disease, high d-dimer and sMVAP at admission were associated with increased odds of death.

In response to the HIV-AIDS pandemic, great strides have been made in developing molecular methods that accurately quantify nucleic acid products of HIV-1 at different stages of viral replication and to assess HIV-1 sequence diversity and its effect on susceptibility to small molecule inhibitors and neutralizing antibodies. Here, we review how knowledge gained from these approaches, including viral RNA quantification and sequence analyses, have been rapidly applied to study SARS-CoV-2 and the COVID-19 pandemic.
Recent studies have shown detection of SARS-CoV-2 RNA in blood of infected individuals by reverse transcriptase PCR (RT-PCR); and, as in HIV-1 infection, there is growing evidence that the level of viral RNA in plasma may be related to COVID disease severity. Unlike HIV-1, SARS-CoV-2 sequences are highly conserved limiting SARS-CoV-2 sequencing applications to investigating interpatient genetic diversity for phylogenetic analysis. Sensitive sequencing technologies, originally developed for HIV-1, will be needed to investigate intrapatient SARS-CoV-2 genetic variation in response to antiviral therapeutics and vaccines.

Despite ground-breaking innovations for most autoimmune diseases, the treatment of lupus nephritis has remained largely the same for decades because none of the tested drugs demonstrated superiority over standard-of-care in randomized controlled clinical trials.
Recently, the Belimumab in Subjects with Systemic Lupus Erythematosus - Lupus Nephritis trial tested belimumab, an inhibitor of B-cell activating factor, as an add-on therapy to steroids and either mycophenolate mofetil (MMF) or cyclophosphamide when given IV monthly over a period of 104 weeks at an effect size of 11% for a Primary Efficacy Renal Response. The NOBILITY trial reported positive results for the B-cell-depleting agent obinutuzumab as an add-on therapy to steroids and MMF when given IV every 6 months over a period of 76 weeks at an effect size of 22% for a complete renal response (CRR). The AURORA trial reported positive results for the calcineurin inhibitor voclosporin as an oral add-on therapy to low dose steroids and MMF when given twice daily over a period of 52 weeks at an effect size of 18.5% for a CRR.

Cardiovascular disease (CVD) is the leading cause of death in patients with chronic kidney disease (CKD). However, traditional CVD risk prediction equations do not work well in patients with CKD, and inclusion of kidney disease metrics such as albuminuria and estimated glomerular filtration rate have a modest to no benefit in improving prediction.
As CKD progresses, the strength of traditional CVD risk factors in predicting clinical outcomes weakens. However, traditional CVD risk prediction equations do not work well in patients with CKD, and inclusion of kidney disease metrics such as albuminuria and estimated glomerular filtration rate have a modest to no benefit in improving prediction. As CKD progresses, the strength of traditional CVD risk factors in predicting clinical outcomes weakens. Also, older adults are often omitted from public health research. Literatures are scarce in Ethiopia; there is limited information in the current study area among older adults using Min Nutritional Assessment (MNA). Therefore, this paper investigated the prevalence of malnutrition and its predictors among older adults people aged 65 years or above in Addis Ababa, Ethiopia.

Data from 662 older adults were collected through face to face interviews using a structured questionnaire. This study was conducted to compare QTD in patients with ST-elevation myocardial infarction (STEMI) before and after treatment by streptokinase (SK) versus primary percutaneous coronary intervention (PCI).

Clinical documents follow certain architecture to specify their structure and semantics for exchange (e.g., HL7 C-CDA). Accordingly, the structural elements of clinical documents may influence document helpfulness for the online community. Member engagement is one of the indicators of community success. We collected 6514 clinical documents from a real-world online medical community, and normalized them with the structural elements of HL7 C-CDe quality of medical decisions.
The findings provide guide on how to improve the effectiveness of sharing clinical experience online. The new and in-depth insights may contribute to the success of online medical communities and the quality of medical decisions.
Hospital admission is a frequent occurrence among patients with cancer, and a significant proportion of patients admitted to medical units have cancer. Their hospital stay has features that may be different compared with patients without cancer. We performed a retrospective analysis of the characteristics of patients with cancer admitted for medical conditions.
We studied the administrative data of patients with solid cancer admitted to the medical department of a large referral hospital over a 12-month period and compared them with those of patients without cancer.
Seven thousand eight hundred two consecutive admissions were analysed, of which 1099 (14.1%) had a principal or associated diagnosis of cancer. Admissions were distributed across 12 units, with 44% concentrated in the medical oncology unit and 56% in other units. Baseline tumour burden was calculated as ∑i Response Evaluation Criteria in Solid Tumours 1.1 baseline target lesions (baseline tumour size[BTS]) or as sum of all measurable baseline lesions (total tumour burden [TTB]); the impact of both parameters on treatment outcomes was investigated.
One hundred fifty patients were included in the analysis. Median BTS and TTB were 79mm and 212mm, respectively. Objective response rate was found significantly associated with BTS (p<0.001) and TTB quartiles (p=0.006), with response rates progressively increasing with decreasing tumour burden quartiles. Both progression-free survival (PFS) (p=0.001) and overall survival (OS) (p<0.001) were significantly associated with BTS quartiles, with 26% of the patients progression-free and 56% alive at 12 months in the lower BTS quartile, compared with 3% and 24%, respectively, in the upper quartile. TTB was also significantly associated with OS (P=0.01) and borderline-significant for PFS (p=0.07). We performed a pooled analysis of phase III TRIBE and TRIBE2 studies comparing FOLFOXIRI/bevacizumab (bev) to doublets (FOLFOX or FOLFIRI)/bev. Patients were grouped in OMD versus non-OMD based on the European Society for Medical Oncology definition. Among patients with OMD, those with OMD/low TB were compared with all the others.
Of 1187 patients enrolled, 1096 were classified as OMD (N=312 [28%]) or non-OMD (N=784 [72%]). Among patients with OMD, 126 (40%) were OMD/low TB. OMD was associated with longer progression-free survival (14.0 versus 10.1 months; p<0.01) and overall survival (38.2 versus 22.0 months; p<0.01). These results were confirmed in multivariable models. The benefit provided by FOLFOXIRI/bev compared with doublets/bev did not differ in accordance with OMD and TB (p for interaction >0.05). Patients with OMD underwent LRTs more frequently (p<0.01) and those with OMD/low TB had higher chance to undergo LRTs after the first progression (p<0.01).

Of 1187 patients enrolled, 1096 were classified as OMD (N=312 [28%]) or non-OMD (N=784 [72%]). Among patients with OMD, 126 (40%) were OMD/low TB. OMD was associated with longer progression-free survival (14.0 versus 10.1 months; p<0.01) and overall survival (38.2 versus 22.0 months; p<0.01). These results were confirmed in multivariable models. The benefit provided by FOLFOXIRI/bev compared with doublets/bev did not differ in accordance with OMD and TB (p for interaction >0.05). Patients with OMD underwent LRTs more frequently (p<0.01) and those with OMD/low TB had higher chance to undergo LRTs after the first progression (p<0.01). OMD is a positive prognostic factor in mCRC. The benefit from the upfront treatment intensification is independent of the metastatic spread extent and TB. We previously reported a novel method of mesenteric excision for esophageal cancer surgery. The esophagus, trachea, recurrent laryngeal nerves (RLNs), and surrounding lymph nodes (LNs) are contained in a common mesenterium, which we termed the "mesotracheoesophagus". In addition, near-infrared (NIR) image-guided lymphatic mapping has recently been used. The purpose of this study was to confirm the feasibility of NIR image-guided lymphatic mapping for upper mediastinal LN dissection, and to confirm the oncological feasibility of our surgical approach.
Fifteen patients with resectable esophageal cancer underwent submucosal injection of indocyanine green (ICG), and underwent robot-assisted esophagectomy. The frequency of ICG positivity in the LN basins along the RLNs, and metastatic frequency were assessed. Regarding the oncological feasibility of our thoracoscopic esophagectomy, the recurrence patterns and survival of 72 consecutive patients who underwent curative resection from 2011 to 2016 were analyzed.
ICG-positive LN basins along the right and left RLNs were found in 12 (80% of 15) patients (3 patients positive for metastatic LNs) and 11 (73% of 15) patients (2 positive for metastatic LNs and 1 false-negative), respectively. All ICG-positive LN basins were found within the mesotracheoesophagus. The sensitivity was 5/6 (83%), and the negative predictive value was 6/7 (86%). Among the 72 patients, with a median follow-up period of 1644days, only 3 (4.2%) patients developed locoregional recurrence.

The predatory mite is the biocontrol agent employed in order to achieve the biological control. The proposed model successfully reproduces this density mediated indirect interactions between pests when their carrying capacities are increased. Moreover, the pests' final population levels may depend on their initial densities and those of their predator. With these results, the proposed model may have the potential to assess whether these indirect pest interactions disrupt or enhance biological control. Additionally, it can also be used as an ancillary tool to theoretically assess the effects of pest biocontrol strategies in the referred experimental setup.

Our study supports the importance of patient stratification based on EGFR copy number variation in clinical trials for GBM. The superiority of GC1118 over other EGFR mAbs in GBM tumors should be assessed in future studies.The mechanical properties and microstructure evolution caused by dynamic loads of 316L stainless steel, fabricated using the Laser Engineered Net Shaping (LENS) technique and hot forging method were studied. Full-density samples, without cracks made of 316L stainless steel alloy powder by using the LENS technique, are characterized by an untypical bi-modal microstructure consisting of macro-grains, which form sub-grains with a similar crystallographic orientation. Wrought stainless steel 316L has an initial equiaxed and one-phase structure, which is formed by austenite grains. The electron backscattered diffraction (EBSD) technique was used to illustrate changes in the microstructure of SS316L after it was subjected to dynamic loads, and it was revealed that for both samples, the grain refinement increases as the deformation rate increases. However, in the case of SS316L samples made by LENS, the share of low-angle boundaries (sub-grains) decreases, and the share of high-angle boundaries (grains of austenite) increases. Dynamically deformed wrought SS316L is characterized by the reverse trend a decrease in the share of high-angle boundaries and an increase in the share of low-angle boundaries. Moreover, additively manufactured SS316L is characterized by lower plastic flow stresses compared with hot-forged steel, which is caused by the finer microstructure of wrought samples relative to that of additive samples. In the case of additively manufactured 316L steel samples subjected to a dynamic load, plastic deformation occurs predominantly through dislocation slip, in contrast to the wrought samples, in which the dominant mechanism of deformation is twinning, which is favored by a high deformation speed and low stacking fault energy (SFE) for austenite.Arabinogalactan (AG) has been studied as a potential prebiotic in view of stimulating bifidobacteria presence in the gut microbiota. However, bifidobacteria prefer fermentation of oligosaccharides to that of polysaccharides. The contribution of other gut bacteria may allow better growth of bifidobacteria on AG. β-galactanases and β-galactosidases are the main enzymes for the degradation of AG. Additional enzymes such as α-L-arabinofuranosidase and β-L-arabinopyranosidase are required to remove the arabinose side chains. All of these predicted functions are encoded by the genomes of both Bifidobacterium longum subsp. longum NCC 2705 and Bacteroides caccae ATCC 43185. However, neither strain was able to grow significantly on AG, with 25% (B. longum subsp. longum NCC 2705) and 39% (Bac. caccae ATCC 43185) of AG degraded after 48-h fermentation, respectively. In this study, the β-galactanase, β-galactosidase, α-L-arabinofuranosidase, and β-L-arabinopyranosidase from both strains were investigated. The extracellulthermore, compared to monoculture of Bac. caccae ATCC 43185, the concentration of succinate significantly increased from 0.01 ± 0.01 to 4.41 ± 0.61 mM, whereas propionate significantly decreased from 13.07 ± 0.37 to 9.75 ± 2.01 mM in the coculture (p less then 0.05). All the samples were incubated for 240 h in the Ankom DaisyII incubator and then analysed for their uNDF contents. Dry matter, organic matter, and neutral detergent fibre digestibilities were estimated using the uNDF contents, and the results were compared with those of the former study. The digestibility values obtained using the uNDF method with pre-treatment were not statistically different from those determined with the total faecal collection. On the contrary, the uNDF method without the pre-treatment could not satisfactorily predict the digestibilities of pig diets.We utilized electrophoresis to control the fluidity of sample biomolecules in sample aqueous solutions inside the nanochannel for single-molecule detection by using a nanochannel-integrated nanogap electrode, which is composed of a nano-gap sensing electrode, nanochannel, and tapered focusing channel. Increasing coastal populations and urban development have led to the loss of estuarine habitats for fish and wildlife. Specifically, a decline in complexity and heterogeneity of tidal marshes and creeks is thought to negatively impact fish communities by altering the function of nursery grounds, including predator refuge and prey resources. To offset these impacts, numerous agencies are restoring degraded habitats while also creating new ones where habitat has been lost. To improve understanding of what contributes to a successful restoration, six quarterly sampling events using two gear types to collect small- and large-bodied fishes were conducted to compare the fish community structure and habitat characteristics at three natural, three restored, and three impacted (i.e. ditched) areas along the coast of Tampa Bay, Florida. Overall, impacted sites had significantly lower small-bodied and juvenile fish diversity than natural and restored areas, while restored sites harbored a greater number of fish species than impacted sites for both large- and small-bodied fish. Habitat features such as shoreline slope differentiated impacted and restored from natural areas. Although we did not find a direct correlation, habitat heterogeneity likely played a role in structuring fish communities. These findings provide guidance for future coastal restoration or modification of existing projects. Specifically, the habitat mosaic approach of creating a geographically compact network of heterogenous habitat characteristics is likely to support fish diversity, while decreasing shoreline slope in a greater amount of area within coastal wetland restorations would more closely mimic natural areas.Color-based image segmentation classifies pixels of digital images in numerous groups for further analysis in computer vision, pattern recognition, image understanding, and image processing applications. Various algorithms have been developed for image segmentation, but clustering algorithms play an important role in the segmentation of digital images. This paper presents a novel and adaptive initialization approach to determine the number of clusters and find the initial central points of clusters for the standard K-means algorithm to solve the segmentation problem of color images. The presented scheme uses a scanning procedure of the paired Red, Green, and Blue (RGB) color-channel histograms for determining the most salient modes in every histogram. Next, the histogram thresholding is applied and a search in every histogram mode is performed to accomplish RGB pairs. These RGB pairs are used as the initial cluster centers and cluster numbers that clustered each pixel into the appropriate region for generating the homogeneous regions. The proposed technique determines the best initialization parameters for the conventional K-means clustering technique. In this paper, the proposed approach was compared with various unsupervised image segmentation techniques on various image segmentation benchmarks. Furthermore, we made use of a ranking approach inspired by the Evaluation Based on Distance from Average Solution (EDAS) method to account for segmentation integrity. Increasing coastal populations and urban development have led to the loss of estuarine habitats for fish and wildlife. Specifically, a decline in complexity and heterogeneity of tidal marshes and creeks is thought to negatively impact fish communities by altering the function of nursery grounds, including predator refuge and prey resources. To offset these impacts, numerous agencies are restoring degraded habitats while also creating new ones where habitat has been lost. To improve understanding of what contributes to a successful restoration, six quarterly sampling events using two gear types to collect small- and large-bodied fishes were conducted to compare the fish community structure and habitat characteristics at three natural, three restored, and three impacted (i.e. ditched) areas along the coast of Tampa Bay, Florida. Overall, impacted sites had significantly lower small-bodied and juvenile fish diversity than natural and restored areas, while restored sites harbored a greater number of fish species than impacted sites for both large- and small-bodied fish. Habitat features such as shoreline slope differentiated impacted and restored from natural areas. Although we did not find a direct correlation, habitat heterogeneity likely played a role in structuring fish communities. These findings provide guidance for future coastal restoration or modification of existing projects. Specifically, the habitat mosaic approach of creating a geographically compact network of heterogenous habitat characteristics is likely to support fish diversity, while decreasing shoreline slope in a greater amount of area within coastal wetland restorations would more closely mimic natural areas.Color-based image segmentation classifies pixels of digital images in numerous groups for further analysis in computer vision, pattern recognition, image understanding, and image processing applications. Various algorithms have been developed for image segmentation, but clustering algorithms play an important role in the segmentation of digital images. This paper presents a novel and adaptive initialization approach to determine the number of clusters and find the initial central points of clusters for the standard K-means algorithm to solve the segmentation problem of color images. The presented scheme uses a scanning procedure of the paired Red, Green, and Blue (RGB) color-channel histograms for determining the most salient modes in every histogram. Next, the histogram thresholding is applied and a search in every histogram mode is performed to accomplish RGB pairs. These RGB pairs are used as the initial cluster centers and cluster numbers that clustered each pixel into the appropriate region for generating the homogeneous regions. The proposed technique determines the best initialization parameters for the conventional K-means clustering technique. In this paper, the proposed approach was compared with various unsupervised image segmentation techniques on various image segmentation benchmarks. Furthermore, we made use of a ranking approach inspired by the Evaluation Based on Distance from Average Solution (EDAS) method to account for segmentation integrity. After constructing the brain network for each subject, we extracted three types of link measures, including fiber anisotropy, fiber length and density. We then performed a targeted genetic association analysis of brain-wide connectivity measures using general linear regression models. Age at scan and gender were included in the regression model as covariates. For fair comparison of the genetic effect on different measures, fiber anisotropy, fiber length and density were all normalized with mean as 0 and standard deviation as one.We aim to discover the abnormal brain-wide network alterations under the control of 34 AD risk SNPs identified in previous large-scale genome-wide association studies. Results After enforcing the stringent Bonferroni correction, rs10498633 in SLC24A4 were found to significantly associated with anisotropy, total number and length of fibers, including some connecting brain hemispheres. Results (1) The rate of karyotypic abnormalities were 25, 6.3 and 2.9% in primary myelofibrosis (PMF), polycythemia vera (PV) and essential thrombocythemia (ET) patients, respectively. (2) The mutation rate of JAK2-V617F was 62.5%, and that of the CALR, MPL and EZH2 genes was 4.2% in PMF. The mutation rates of JAK2-V617F and JAK2-12exon were 91.3 and 1.3% in PV, respectively. The mutation rates of JAK2-V617F and CALR were 69.1 and 11.8% in ET, respectively. (3) Patients with JAK2-V617F mutation than with the wild-type gene were more often female in PMF (P = 0.027); had higher peripheral blood white blood cell (WBC) counts (P = 0.006), platelet (PLT) count (P = 0.001) and splenomegaly (P less then 0.05) in PV; and had higher WBC (P = 0.001), hemoglobin concentrations (P = 0.001), lower PLT (P = 0.037), splenomegaly and endogenous coagulopathy (P less then 0.05) in ET.
