The current graduate medical education curriculum allows trainees to graduate without getting focused training to develop empathy as a core competency domain. The tools to measure empathy inherently lack validity. The accurate measure of the provider's empathy comes from the patient's perspectives of their experience and their feedback, which is rarely reaching the trainee. The hidden curriculum in residency programs gives mixed messages to trainees due to inadequate role modeling by attending physicians. This narrative style manuscript portrays a teachable moment at the bedside vividly. The teaching team together reflected upon the lack of empathy, took steps to resolve the issue. The attending demonstrated role modeling as an authentic and impactful technique to teach empathy. The conclusion includes a proposal to include the patient's real-time feedback to trainees as an essential domain under Graduate Medical Education core competencies of professionalism and patient care.The Covid-19 pandemic made it necessary to adopt and establish complete or partial online delivery of our clinical teaching and learning. We developed an alternative approach with a combination of Problem based Learning asynchronous fora and Teacher-facilitated synchronous online discussions. Our aim is to share our educational practice and highlight the requirements and constraints, advantages and challenges of such an approach. It allowed a more student-centred experience, but clinical simulation and face-to-face patient care remain necessary. The Covid-19 pandemic has changed the landscape of dental education for the foreseeable future, with a reduced number of patients in dental clinics. Further study is therefore necessary to understand the lived experience of students and teachers to the adopted online teaching and learning approach.The complete chloroplast genome of Crotalaria pallida was obtained using the high-throughput sequencing technology in this article. A huge intraspecies variation was found with 248 SNPs and 97 INDELs among three assemblies of M. pteleifolia. Phylogenetic tree indicated that three assemblies of M. pteleifolia form a clade, sister to the genus Phellodendron and Casimiroa.

  • The aim of this study is to obtain the translocation factor by application of landfill leachate (LL) diluted in public irrigation water (IW). Pennisetum purpureum Schum (elephant grass) was cultivated for 83 days in an experimental water reuse unit. The present work was developed at the Experimental Water Reuse Unit (UERA), on the UFERSA campus in Mossoró, RN, Brazil. Plot irrigation was based on water balance and crop evapotranspiration (ETc). The concentration in the plant tissue (root and leaf) of the following heavy metals was measured to determine the respective translocation factors manganese (Mn), zinc (Zn), copper (Cu), nickel (Ni), cadmium (Cd), and lead (Pb). The experiment was set up in a randomized block design with five treatments (T1, plots irrigated only with IW; T2, 50% of LL dose plus IW; T3, 100% of LL dose plus IW; T4, 150% of LL dose plus IW; and T5, 200% of LL dose plus IW) and five replications. All treatments received LL plus IW depth of 491.02 mm for 83 days of P. purpureum cultivation. The data obtained were submitted to multivariate analysis plus the nonparametric Kruskal-Wallis test to compare the means. Pennisetum purpureum showed a potential to accumulate metals in its tissues, mainly Mn, Zn, and Cu. The treatments that most favored the extraction of these metals were T2 and T5; in this sense, P. purpureum was not efficient in translocating heavy metals, since the translocation factor observed in all treatments was below 1.0, indicating that the species used extract heavy metals from soil solution and keeps in yours roots. https://www.selleckchem.com/products/ag-120-Ivosidenib.html This suggests planting P. purpureum may not be a viable option to remediate environments highly contaminated with heavy metals.This study investigated the occurrence, distribution of several additive brominated flame retardants (BFRs) such as polybrominated diphenyl ethers (PBDEs) and some novel brominated flame retardants (NBFRs) in urban indoor dust collected from ten inner districts of Hanoi, Vietnam to assess the contamination status, emission sources, as well as their associated human exposure through indoor dust ingestion and health risks. Total concentrations of PBDEs and NBFRs in indoor dust samples ranged from 43 to 480 ng g-1 (median 170 ng g-1) and from 56 to 2200 ng g-1 (median 180 ng g-1), respectively. The most abundant PBDE congener in these dust samples was BDE-209 with concentrations ranging from 29 to 360 ng g-1, accounting for 62.6-86.5% of total PBDE levels. Among the NBFRs analyzed, decabromodiphenyl ethane (DBDPE) was the predominant compound with a mean contribution of 98.6% total NBFR amounts. Significant concentrations of DBDPE were detected in all dust samples (median 180 ng g-1, range 54-2200 ng g-1), due to DBDPE as a substitute for deca-BDE. Other NBFRs such as 1,2-bis(2,4,6-tribromophenoxy) ethane (BTBPE), pentabromoethylbenzene (PBEB) and 2,2',4,4',5,5'-hexabromobiphenyl (BB-153) were found at very low levels. Based on the measured BFR concentrations, daily intake doses (IDs) of PBDEs and NBFRs via dust ingestion at exposure scenarios using the median and 95th percentile levels for both adults and children were calculated for risk assessment. The results showed that the daily exposure doses via dust ingestion of all compounds, even in the high-exposure scenarios were also lower than their reference dose (RfD) values. Also, a careful selection of variable which includes economic growth (GDP per capita-Y), institutional quality, foreign direct investment (FDI), fossil fuels, and renewable energy consumption was undertaken in this study. The focus was on the interaction of institutional quality and FDI towards ascertainment of environment performance. Chile's quarterly data of 1996Q1 to 2018Q4 was utilized, and the following findings were made positive and negative shocks to the economic growth, institutional quality, and renewable energy impacted favorably and negatively on Chile's environment through reduction and promotion of emission, respectively. In contrast, positive and negative shocks to FDI and fossil fuels impact both negatively on the Chile's environment through increase in carbon emission. So institutional quality is vital in controlling the negative impact from FDI and fossil fuels.The novel SARS-CoV-2 outbreak was declared as pandemic by the World Health Organization (WHO) on March 11, 2020. Understanding the airborne route of SARS-CoV-2 transmission is essential for infection prevention and control. In this study, a total of 107 indoor air samples (45 SARS-CoV-2, 62 bacteria, and fungi) were collected from different wards of the Hajar Hospital in Shahrekord, Iran. Simultaneously, bacterial and fungal samples were also collected from the ambient air of hospital yard. Overall, 6 positive air samples were detected in the infectious 1 and infectious 2 wards, intensive care unit (ICU), computed tomography (CT) scan, respiratory patients' clinic, and personal protective equipment (PPE) room. Also, airborne bacteria and fungi were simultaneously detected in the various wards of the hospital with concentrations ranging from 14 to 106 CFU m-3 and 18 to 141 CFU m-3, respectively. The highest mean concentrations of bacteria and fungi were observed in respiratory patients' clinics and ICU wards, respectively.

In contrast to one-stage four-electron oxidation of BQDs, stepwise two-electron reduction of tetracationic pentacenes occurs to give dicationic anthracenes and then the original BQDs, step-by-step. Since both tetracations and dications exhibit near-infrared (NIR) absorptions (-1400 nm) based on an intramolecular charge-transfer interaction, changes in not only their structures but also their UV-vis-NIR spectra can be controlled by redox stimuli. In this Communication, we present an unprecedented one-step π-extension to pentacene from non-fused benzene rings by oxidation, and subsequent two-stage deannulation to benzene rings via anthracene upon reduction. All structures were determined by single-crystal X-ray analyses, and their properties were characterized by spectroscopic and theoretical studies.The response of the spin state to in situ variation of the coordination number (CISSS) is a promising and viable approach to smart sensor materials, yet it suffers to date from insensitive detection. Herein, we present the synthetic access to a family of planar nickel(II) complexes, whose CISSS is sensitively followed by means of fluorescence detection. For this purpose, nickel(II) complexes with four phenazine-based Schiff base-like ligands were synthesized and characterized through solution-phase spectroscopy (NMR and UV-vis), solid-state structure analysis (single-crystal XRD), and extended theoretical modeling. All of them reveal CISSS in solution through axial ligating a range of N- and O-donors. CISSS correlates nicely with the basicity of the axial ligand and the substitution-dependent acidity of the nickel(II) coordination site. Remarkably, three out of the four nickel(II) complexes are fluorescent in noncoordinating solvents but are fluorescence-silent in the presence of axial ligands such as pyridine. As these complexes are rare examples of fluorescent nickel(II) complexes, the photophysical properties with a coordination number of 4 were studied in detail, including temperature-dependent lifetime and quantum yield determinations. Most importantly, fluorescence quenching upon adding axial ligands allows a "black or white", i.e. digital, sensoring of spin state alternation. Our studies of fluorescence-detected CISSS (FD-CISSS) revealed that absorption-based CISSS and FD-CISSS are super proportional with respect to the pyridine concentration FD-CISSS features a higher sensitivity. Chain entropy overcomes the enthalpically favored OH···O- contacts to favor random-coil conformations required for seamless, high-fidelity threading of dihexanol and didecanol phosphates inside cyanostars. These studies highlight how chain length and functional groups on phosphate's substituents can be powerful design tools to regulate binding and control assembly formation during phosphate recognition.

As these complexes are rare examples of fluorescent nickel(II) complexes, the photophysical properties with a coordination number of 4 were studied in detail, including temperature-dependent lifetime and quantum yield determinations. Most importantly, fluorescence quenching upon adding axial ligands allows a "black or white", i.e. digital, sensoring of spin state alternation.   • We present results showing the capability of concrete-based information processing substrate in the signal classification task in accordance with in materio computing paradigm. As the Reservoir Computing is a suitable model for describing embedded in materio computation, we propose that this type of presented basic construction unit can be used as a source for "reservoir of states" necessary for simple tuning of the readout layer. We present an electrical characterization of the set of samples with different additive concentrations followed by a dynamical analysis of selected specimens showing fingerprints of memfractive properties. As part of dynamic analysis, several fractal dimensions and entropy parameters for the output signal were analyzed to explore the richness of the reservoir configuration space. In addition, to investigate the chaotic nature and self-affinity of the signal, Lyapunov exponents and Detrended Fluctuation Analysis exponents were calculated. Moreover, on the basis of obtained parameters, classification of the signal waveform shapes can be performed in scenarios explicitly tuned for a given device terminal.The hydrophobic tails of aliphatic primary alcohols do insert into the hydrophobic core of a lipid bilayer. Using provincial panel data of China from 2005 to 2015, the present study intends to shed light on the associations between government intervention and food safety performance under two scenarios of local government-competition and noncompetition. This will be accomplished through an exploratory spatial data analysis and a spatial econometric model. The results reveal negative associations between food safety performance and government intervention without considering local government competition. As was also observed, government intervention not only inhibits the improvement of food safety in the region, but also has a negative spatial spillover effect on food safety in neighboring provinces. This is the result after considering government competition, thus, showing the competitive strategic interaction of the "race to the bottom". Further analysis reveals that, if geographically similar regions are selected as reference objects, the food safety performance of each province will have a stronger tendency to compete for the better. If regions with similar economic development levels are selected as reference objects, food safety performance will have a stronger tendency to compete for the worse. Squat jump (SJ), countermovement jump (CMJ), 5-metre sprint (5M), 20-metre shuttle run (20MSR), taekwondo specific agility test (TSAT), multiple frequency speed of kick test (FSKTMULT), total kicks, and kick decrement index (KDI), as well as body composition were evaluated. Results indicate that there are no significant differences (p > 0.05) in the factors group and time factor and group by time interaction (p > 0.05). Although percentage and effect size increases were documented for post-intervention fitness components in TSAT, total kicks, KDI, and 20MSR, responders and non-responders were also documented.   • To investigate the relationship between Leucine-rich repeat G-protein coupled receptor 5 (Lgr5) expression and clinicopathological features of colorectal cancer and its diagnostic and prognostic values.

    The cancer and adjacent tissues of 98 patients with colorectal cancer undergoing resection in Jiangsu Cancer Hospital from 2011 to 2013 were enrolled. The intestinal mucosal tissue of 50 healthy subjects was enrolled as a control group. Western blot was used to detect the expression level of Lgr5 in colorectal cancer, Kaplan-Meier method to analyze the correlation of Lgr5 expression with the 5-year survival rate, Cox regression to analyze prognostic risk factors for the patients, and receiver operating characteristics (ROC) curve to analyze the diagnostic value of Lgr5 of the disease.

    The expression level of Lgr5 in the intestinal mucosal and adjacent tissues was significantly lower than that in the cancer tissue (p<0.05). The 5-year survival in the Lgr5 low expression group was higher than that in the Lgr5 high expression group (p=0.002). Lgr5 was an independent risk factor for the prognosis of colorectal cancer. The sensitivity, specificity and the area under the curve (AUC) of Lgr5 were 90.00%, 79.59% and 0.880, respectively (p<0.001).

    Small cell lung cancer (SCLC) patients unresponsive or relapsing within 90 days following frontline chemotherapy have poor prognosis and they should be treated with different chemotherapy regimens other than those used in the first-line regimen. Currently there is no globally accepted standard chemotherapeutic regimen for the treatment of these patients. This retrospective study was designed to compare CAV (Cyclophosphamide, Doxorubicin, Vincristine), weekly topotecan and weekly irinotecan regimens and to evaluate the efficacy of the three regimens in patients with chemotherapy resistant/refractory (CRR) SCLC.

The 5-year survival in the Lgr5 low expression group was higher than that in the Lgr5 high expression group (p=0.002). Lgr5 was an independent risk factor for the prognosis of colorectal cancer.   • The findings are important for investigating the effects of fungal elicitors on the biosynthesis of vinblastine and vincristine, as well as other terpenoid indole alkaloids (TIAs), in C. roseus QN cell suspension cultures.Knee osteoarthritis is a chronic joint disease which damages articular cartilage. In its severe stages, it results in impairments in balance and muscle strength loss, which affect daily life activities such as walking or climbing stairs. This study sought to investigate associated factors with stair-climbing ability in this population, with special interest in measuring the relevance of postural balance for this task. Forty-four patients scheduled to undergo unilateral total knee arthroplasty were assessed. Timed up and go test, stair ascent-descent test, three different isometric strength tests (knee flexion, knee extension and hip abduction), active knee extension and flexion range of movement and static postural balance assessment were evaluated. Spearman's correlation coefficients and multiple linear regression analysis determined the strength of association between the different variables and stair-climbing time. No significant association between the stair-climbing time and static balance was found. Significant associations were found between stair-climbing time and timed up and go (r = 0.71; p less then 0.0001) and maximal knee extensor strength (r = -0.52; p = 0.0003). One-year increase in age was associated with 0.15 s (95% CI 0.00 to 0.30) slower stair-climbing time. The major barrier in treating MM is the occurrence of primary and acquired therapy resistance to anticancer drugs. Novel covalent kinase inhibitors, such as the clinically approved BTK inhibitor ibrutinib (IBR) and the preclinical phytochemical withaferin A (WA), have, therefore, gained pharmaceutical interest. Remarkably, WA is more effective than IBR in killing BTK-overexpressing glucocorticoid (GC)-resistant MM1R cells. To further characterize the kinase inhibitor profiles of WA and IBR in GC-resistant MM cells, we applied phosphopeptidome- and transcriptome-specific tyrosine kinome profiling. In contrast to IBR, WA was found to reverse BTK overexpression in GC-resistant MM1R cells. Furthermore, WA-induced cell death involves covalent cysteine targeting of Hinge-6 domain type tyrosine kinases of the kinase cysteinome classification, including inhibition of the hyperactivated BTK. Covalent interaction between WA and BTK could further be confirmed by biotin-based affinity purification and confocal microscopy. In contrast to IBR, WA was found to reverse BTK overexpression in GC-resistant MM1R cells. Furthermore, WA-induced cell death involves covalent cysteine targeting of Hinge-6 domain type tyrosine kinases of the kinase cysteinome classification, including inhibition of the hyperactivated BTK. Covalent interaction between WA and BTK could further be confirmed by biotin-based affinity purification and confocal microscopy. Furthermore, the American Diabetes Association guidelines increasingly highlight the importance of multifactorial management and optimizing medication regimens that include cardiovascular, renal, and/or weight benefits in patients with type 2 diabetes mellitus. Glucagon-like peptide-1 receptor agonists belong to a novel class of type 2 diabetes mellitus agents that are becoming increasingly prevalent owing to their ability to improve glycemic status without the risk of hypoglycemia. Currently, there are three US Food and Drug Administration-approved glucagon-like peptide-1 receptor agonists, subcutaneous semaglutide, dulaglutide, and liraglutide, that also have an indication for reducing major adverse cardiovascular events in patients with type 2 diabetes mellitus and established cardiovascular disease. However, these agents are not often the first options because of their subcutaneous administration. Nevertheless, co-formulation of oral semaglutide with an absorption enhancer has shown to increase its bioavailability and has made its oral absorption possible. Participants were shown two faces sequentially and were asked to judge whether the faces' top halves were identical or different. The first face was a static face or a dynamic face rotated in depth at 30°, 60°, and 90°. The second face was either a different front-view static face (Experiment 1a, study-test inconsistent) or identical to the first face (Experiment 1b, study-test consistent). In Experiment 2, study-test consistency was manipulated within subjects, and inverted faces were included. Our results show that study-test consistency significantly enhanced the holistic processing of upright and inverted faces; this study-test consistency effect and holistic processing were not modulated by motion and viewpoint changes via depth rotation. Interestingly, we found holistic processing for moving study-test consistent inverted faces, but not for static inverted faces. What these results tell us about the nature of holistic face representation is discussed in depth with respect to earlier and current theories on face processing.A widely held though debatable claim is that the picture of an object like a frying pan automatically elicits features of a left/right-handed grasp action even in perceptual tasks that make no demands on the observer to consider the graspable properties of the depicted object. Here, we sought to further elucidate this claim by relying on a methodology that allowed us to distinguish between the influence of motor versus spatial codes on the selection of a left/right-handed response while electroencephalographic data were recorded. In our experiment, participants classified images of frying pans as upright or inverted using a left/right key press or by making a left/right-handed reach-and-grasp action towards a centrally located response element while we recorded electroencephalographic (EEG) data. In line with previous evidence (Bub, Masson, & van Noordenne, Journal of Experiment Psychology Human Perception and Performance, 47(1), 53-80, 2021), these two modes of responding generated distinct correspondence effects on performance induced by the same set of images. In all experiments, we found that mean size estimations were more erroneous for higher than smaller variance displays. More critically, there was a tendency to overestimate the mean, driven by variance in both task-relevant and task-irrelevant features. We discuss these findings in relation to limitations in concurrent summarization ability and outlier discounting in ensemble perception.
