  • A thymic epithelial tumour is the most common primary tumour in the anterior mediastinum of adults. A few retrospective studies compared the short-term outcomes between robotic-assisted thymectomy (RAT) and video-assisted thymectomy (VAT). So, it is necessary to conduct a meta-analysis to further compare these 2 surgical techniques.

    EMBASE, Medline and Web of Science were used. Thesaurus terms and medical subject headings were used in Medline and EMBASE, respectively. The Newcastle-Ottawa scale was used for grading because the included studies were all case-control studies.

    Nine studies were included in the meta-analysis with a total of 723 patients, including 315 patients in the RAT group and 408 patients in the VAT group. The meta-analysis [odds ratio (OR) 0.24, 95% confidence interval (CI) 0.06-0.94; P = 0.041], indicating that RAT yielded a significantly lower rate of conversion compared with VAT. Duration of drainage with RAT was significantly less than that with VAT (weighted mean difference = -1.10; 95% CI -1.98 to -0.22; P = 0.014). The pooled analysis (weighted mean difference = -103.6; 95% CI -199.21 to -7.98; P = 0.034) suggested that patients in the RAT group had less drainage than those in the VAT group. The recurrence rates in both groups were comparable (OR 0.19, 95% CI 0.03-1.20; P = 0.078).

    Upper respiratory tract infections (URTIs) are common, mostly self-limiting, but result in inappropriate antibiotic prescriptions. Poor sleep is cited as a factor predisposing to URTIs, but the evidence is unclear.

    To systematically review whether sleep duration and quality influence the frequency and duration of URTIs.

    Three databases and bibliographies of included papers were searched for studies assessing associations between sleep duration or quality and URTIs. In this prospective cohort study, we examined whether receiving unemployment benefits is associated with lower mortality among the long-term unemployed. Census records from the 2006 Canadian Census Health and Environment Cohort (n=2,105,595) were linked to mortality data from 2006 to 2016. Flexible parametric survival analysis and propensity score matching were used to model time-varying relationships between long-term unemployment (20 weeks or more), unemployment benefit recipiency, and all-cause mortality. Mortality was consistently lower among unemployed individuals who reported receiving unemployment benefits relative to matched nonrecipients. For example, mortality at 2 years of follow-up was 18% lower (95% confidence interval (CI) 9%, 26%) among men receiving benefits and 30% lower (95% CI 18%, 40%) among women receiving benefits. After 10 years of follow-up, unemployment benefit recipiency was associated with 890 (95% CI 560, 1,230) fewer deaths per 100,000 men and 1,070 (95% CI 810, 1,320) fewer deaths per 100,000 women. Our findings indicate that receiving unemployment benefits is associated with lower mortality among the long-term unemployed. The thermodynamic properties of the two crystals were investigated through thermogravimetric analysis. The structural dynamics, particularly the role of the [NH3(CH2)3NH3] cation, were probed through 1H and 13C magic-angle spinning nuclear magnetic resonance spectroscopy as a function of temperature. The 1H and 13C NMR chemical shifts did not show any changes near TC. In addition, the 1H spin-lattice relaxation time (T1ρ) varied with temperature, whereas the 13C T1ρ values remained nearly constant at different temperatures. The T1ρ values of the atoms in [NH3(CH2)3NH3]ZnCl4 were higher than those in [NH3(CH2)3NH3]ZnBr4. The observed differences in the structural dynamics obtained from the chemical shifts and T1ρ values of the two compounds can be attributed to the differences in the bond lengths and halogen atoms. A sample of 21 participants were trained in a test-retest HRVB protocol. The results indicated that RF changed between Test and Retest sessions in 66.7% of participants. This instability could be related to the average of interbeat interval (IBI). HRV time domain parameters (SDNN and RMSSD) were significantly higher for RF than for other breathing rates, including 6 breath/min and free slow breathing. Free slow breathing showed a lower heart rate averages than RF and other slow breathing rates. Overall, our study suggests the relevance of assessing RF individually and before each HRVB session, because the maximum cardiovascular benefits in terms of increasing HRV were found only at RF. Statistical analysis was conducted to determine whether the combination of prophylactic G-CSF and antibiotics was more effective in preventing FN, compared with prophylactic G-CSF alone. Prophylactic PEG-G-CSF or G-CSF was administered in all 443 cycles. Only fluoroquinolones were used as prophylactic antibiotics and were administered in 328 cycles (74.0%). FN occurred in 5 cycles (1.1%). Prophylactic antibiotics were administered in 327 cycles (74.6%) in the FN-negative group and in only 1 cycle (20.0%) in the FN-positive group. Chi-square test indicated the incidence of FN was significantly lower in the group that received prophylactic antibiotics compared with the group that did not receive prophylactic antibiotics (P = 0.017). This case report documents aportomesentericvenous thrombosis in a postmenopausal woman with testosterone implant for the treatment of hypoactive sexual desire and discusses the importance of identifying hypercoagulable risk factors before initiating hormone replacement therapy. We want to improve the awareness of an unusual medical complication associated with hormone replacement therapy and shed light on how testosterone implants could facilitate a thrombotic event related to other risk factors such as obesity and chronic hypoxic states, as well as the importance of differential diagnosis in the evaluation of postmenopausal women on testosterone replacement therapy presenting with acute abdominal pain.

    A 55-year-old obese postmenotosterone implants as a treatment of hypoactive sexual desire in postmenopausal women can contribute to thrombotic events in the presence of additional prothrombotic risk factors.     Cystic echinococcosis (CE) is a parasitic zoonosis caused by infection with the larval stage of Echinococcus granulosus (s.l.). This study investigated the prevalence and potential risk factors associated with human CE in the towns and rural areas of Ñorquinco and Ramos Mexia, Rio Negro province, Argentina.

    To detect abdominal CE cysts, we screened 892 volunteers by ultrasound and investigated potential risk factors for CE using a standardized questionnaire. Prevalence ratio (PR) with 95% confidence intervals (CI) was used to measure the association between CE and the factors investigated, applying bivariate and multivariate analyses.

In Romania, the incidence of TC is rising, one of the reason may be the effect of Chernobyl nuclear accident in the past and the iodine deficiency. The purpose of this study was to describe the characteristics and outcome of children diagnosed with ****in Romania diagnosed from 1 January 2000 to 31 December 2018. During the study period we identified 77 cases of differentiated TC (papillary and follicular) and of these 20 cases (19.4%) were mTC. The aim of this study was to synthesize AgNPs using C. orbiculata and evaluate their antimicrobial and immunomodulatory properties. AgNPs were synthesized and characterized using Ultraviolet-Visible Spectroscopy (UV-Vis), Dynamic Light Scattering (DLS) and High-Resolution Transmission Electron Microscopy (HR-TEM). The antimicrobial activities of the nanoparticles against skin pathogens (Staphylococcus aureus, Staphylococcus epidermidis, Methicillin Resistance Staphylococcus aureus, Pseudomonas aeruginosa and Candida albicans) as well as their effects on cytokine production in macrophages (differentiated from THP-1 cells) were evaluated. The AgNPs from C. orbiculata exhibited antimicrobial activity, with the highest activity observed against P. aeruginosa (5 µg/mL). The AgNPs also showed anti-inflammatory activity by inhibiting the secretion of pro-inflammatory cytokines (TNF-alpha, IL-6 and IL-1 beta) in lipopolysaccharide-treated macrophages.   • Several studies of food literacy emphasise the acquisition of critical knowledge over context. This evaluation looks at how COVID-19 impacted food literacy in a country affected by the global pandemic. To our knowledge, there has been no systematic research that would allow a better understanding of the impact of uncertainty or enhanced perceived risks generated by a global crisis on the prevalence of household food literacy. This study looks at food literacy from a perceptive of how an event that has domesticated many of them can alter knowledge and the relationship people have with food. A cross-national survey including 10,004 Canadians was conducted ten months after the start of the pandemic. Results show that Canadians have learned new recipes. Canadians have also taken up gardening and have relied on several sources to gather information. This study provides some evidence that Canadians have become more food literate because of the COVID-19 pandemic, but less significantly than anticipated. To explore the role of Zfp217 in high-fat diet (HFD)-induced obese ****, global Zfp217 heterozygous knockout (Zfp217+/-) **** were constructed. It also discusses epileptiform detection as one of the steps towards advanced diagnoses of disorders of consciousness (DOCs) and their understanding. A performance comparison was carried out on the different algorithms investigated, and their advantages and disadvantages were explored.     Several studies of food literacy emphasise the acquisition of critical knowledge over context. This evaluation looks at how COVID-19 impacted food literacy in a country affected by the global pandemic. To our knowledge, there has been no systematic research that would allow a better understanding of the impact of uncertainty or enhanced perceived risks generated by a global crisis on the prevalence of household food literacy. This study looks at food literacy from a perceptive of how an event that has domesticated many of them can alter knowledge and the relationship people have with food. A cross-national survey including 10,004 Canadians was conducted ten months after the start of the pandemic. Results show that Canadians have learned new recipes. Canadians have also taken up gardening and have relied on several sources to gather information. This study provides some evidence that Canadians have become more food literate because of the COVID-19 pandemic, but less significantly than anticipated. The success of this intervention can be used to further develop and implement workshops and other training sessions to enhance the skills and knowledge of Primary Care Physicians in similar settings that face a shortage of well-equipped physicians.
    COVID-19 has ensured that countries enforced complete lock down to prevent the spread of infection. Despite millions of people working from home during this pandemic situation, police personnel 'the front line workers' who deal with the general population to ensure their safety and wellbeing are at risk of stress and other mental health problems in addition to the risk of infection itself.

    The aim of this study is to explore the perception of police personnel towards the disease, factors influencing stress and coping abilities of them amid COVID-19.

    A cross-sectional qualitative study was conducted using in-depth interviews among randomly selected police personnel working in COVID-19 situation across the district of Karaikal, Puducherry.

    Of the 32 police personnel, majority were of age group 36-45 (43.75%), followed by 25-35 (37.5%). More than half of them were married (78.12%). Around 62.5% had been in the present occupation for more than 5 years. The in depth interviews were group under 5 themes namely perception, practice they follow amid COVID-19, stress they go through, challenges faced, coping strategies used by them.

  • The use of aptamers in bioanalytical and biomedical applications exploits their ability to recognize cell surface protein receptors. Targeted therapeutics and theranostics come to mind in this regard. However, protein receptors occur on both cancer and normal cells; as such, aptamers are now taxed with identifying high vs. low levels of protein expression. Inspired by the flexible template mechanism and elegant control of natural nucleic acid-based structures, we report an allosteric regulation strategy for constructing a structure-switching aptamer for enhanced target cell recognition by engineering aptamers with DNA intercalated motifs (i-motifs) responsive to the microenvironment, such as pH. Structure-switching sensitivity can be readily tuned by manipulating i-motif sequences. However, structure-switching sensitivity is difficult to estimate, making it equally difficult to effectively screen modified aptamers with the desired sensitivity. To address this problem, we selected a fluorescent probe capable of detecting G-quadruplex in complicated biological media.Coronavirus disease 2019 (COVID-19) has spread worldwide and become an emerging life-threatening pandemic disease since 2019. This study aimed to evaluate the basic knowledge, awareness, personal hygiene and healthy practices regarding COVID-19 during its outbreak among the Palestinian population in the Gaza Strip. It is a cross-sectional survey carried out between July and August 2020 and included 458 participants, aged 18 years or older, belonging to the five Gaza Strip governorates, Palestine, with a variety of socio-economic status. Sociodemographic data and data on COVID-19 knowledge, awareness, personal hygiene and healthy practices were collected via email and social media applications (WhatsApp and Facebook) from the study participants. Statistical analyses were performed using Vr 22 of the SPSS software. Overall, the mean ± SD age of the participants was 26.5 ± 4.2 years. Our findings showed that most of the Gazan adults had very good knowledge regarding COVID-19 in terms of host sources, causative agents, incubation period, symptoms, transmission, treatment and prevention. Additionally, most of the participants had a high level of personal hygiene and healthy practices during the COVID-19 outbreak, where personal hygiene and healthy practices against COVID-19 was 77.4%. Furthermore, a higher educational level did not affect the quality of personal hygiene and healthy practice behaviours during the COVID-19 outbreak. Although most of the Gazans had good knowledge regarding the healthy practices, a significant percentage of the population did not practice the hygiene protocol. In the present study, the spike (S) protein of severe acute respiratory coronavirus 2 (SARS-CoV-2) circulating in Bangladesh was genetically investigated to elucidate the diversity of mutations and their prevalence. The nucleotide sequence of the S protein gene was determined for 15 SARS-CoV-2 samples collected from eight divisions in Bangladesh, and analysed for mutations compared with the reference strain (hCoV-19/Wuhan/WIV04/2019). All the SARS-CoV-2 S genes were assigned to B.1 lineage in G clade, and individual S proteins had 1-25 mutations causing amino acid substitution/deletion. A total of 133 mutations were detected in 15 samples, with D614G being present in all the samples; 53 were novel mutations as of January 2021. In the present study, the spike (S) protein of severe acute respiratory coronavirus 2 (SARS-CoV-2) circulating in Bangladesh was genetically investigated to elucidate the diversity of mutations and their prevalence. The nucleotide sequence of the S protein gene was determined for 15 SARS-CoV-2 samples collected from eight divisions in Bangladesh, and analysed for mutations compared with the reference strain (hCoV-19/Wuhan/WIV04/2019). All the SARS-CoV-2 S genes were assigned to B.1 lineage in G clade, and individual S proteins had 1-25 mutations causing amino acid substitution/deletion. A total of 133 mutations were detected in 15 samples, with D614G being present in all the samples; 53 were novel mutations as of January 2021. On the receptor-binding domain, 21 substitutions including ten novel mutations were identified. Other novel mutations were located on the N-terminal domain (S1 subunit) and dispersed sites in the S2 subunit, including two substitutions that remove potential N-glycosylation sites.   • Gemcitabine-docetaxel (G-D) combination is an effective chemotherapy for patients with advanced soft tissue and bone sarcoma, first developed with G administered on days 1 and 8, and D on day 8 every 21 days and later modified to be administered every 14 days in 2012. The 14-day regimen has become increasingly adopted. However, its efficacy and toxicities have not been compared. We identified 161 patients with metastatic or locally advanced soft tissue and bone sarcoma treated with either a 14-day or 21-day regimen within Northern California Kaiser Permanente from 1 January 2017 to 30 July 2020 and compared the outcomes and toxicity profiles of patients treated with the either regimen. Seventy-nine (49%) and 82 (51%) patients received the 14-day and the 21-day regimen, respectively, with similar response rate (22.8% and 15.8%, p = 0.26), median progression-free survival (PFS, 4.0 and 3.2 months, p = 0.15), and median overall survival (OS, 12.6 and 14.7 months, p = 0.55). Subset analysis of the untreated patients (approximately 60% of the entire cohort) as well as the patients with leiomyosarcoma only (approximately 50% of the entire cohort) showed that OS was not significantly different between the two regimens. Febrile neutropenia requiring hospitalization occurred in 10 and one patients (p = 0.006) and intolerance leading to discontinuation of chemotherapy occurred in 12 and two patients (p = 0.006) treated with the 21-day and the 14-day regimens, respectively. CDKN2A deletion/mutation or CDK4 amplification was associated with worse median OS (p = 0.06), while a RB1 deletion/mutation was associated with better median PFS (p = 0.05), and these two genomic alterations were mutually exclusive. Our data demonstrate that, compared to the traditional 21-day G-D regimen, the 14-day G-D regimen is equally effective but safer. All components of suicidality were more likely in those with major depressive disorder, bipolar disorder, generalized anxiety disorder, alcohol use disorder, and chronic pain. Age above 65, being male, and a monthly household income of ≥ SGD 10,000 were associated with a lower likelihood of suicidal ideation.   • The cosmetic industry needs new preservatives that are effective, natural, safe, cost effective, sustainable and compliant with regulatory standards. This necessity has posed challenges requiring obligations, bioinformatics and bioinspiration as driving forces.

    Twenty natural extracts were selected from the Greenpharma Database with parameter filters corresponding to development constraints and antimicrobial properties. We confirmed using minimum inhibition concentration (MIC) assays that eight of the extracts have good bactericidal properties and that one has a high antifungal activity. The latter was purified hinokitiol, a bioproduct from Aomori Hiba wood. This substance provides high resistance against putrefaction; for instance, old Japanese temples were made of Aomori Hiba wood. The combination of hinokitiol with levulinic acid, another bioproduct, demonstrated complementary antimicrobial activities and synergistic effects in ****studies and measurements according to Kull synergy index. Further, the mixture Hinoline® was tested at 2% in challenge tests and fulfilled criteria A of different standards. It also exerted complementary preservative effects with potassium sorbate and beneficial effects in unbalanced skin microbiota.

    The role of urodynamic study in the diagnosis and prognostication of interstitial cystitis/bladder pain syndrome (IC/BPS) is still controversial. This study evaluated the correlation of the baseline voiding dysfunctions and long-term treatment outcome in patients with non-Hunner's ulcer IC (NHIC).

    A total of 211 NHIC patients were enrolled. All patients underwent videourodynamic (VUDS) examination at baseline to identify their voiding conditions and received subsequent treatments. The duration, co-morbidities, treatments, changes in OSS and VAS, maximum bladder capacity (MBC), glomerulations, GRA and flare-up rate showed no significant difference among different subgroups. When we divided patients by their storage and voiding conditions, patients with non-hypersensitivity bladder (HSB) (n=32) had significantly greater MBC (P=.002) whereas those with HSB with (n=76) or without (n=103) voiding dysfunction had higher glomerulation (P=.021). When we analysed voiding dysfunction subgroups by GRA, patients with a GRA of ≥2 had a significantly shorter duration of disease (13.9±8.6years, P=.021). There were also significant associations between GRA and the changes of OSS (P<.001) and VAS (P<.001).

The CAR group showed an increased abundance of Christensenellaceae R-7 group, Marvinbryantia, Ruminococcaceae NK4A214, Lachnospiraceae UCG-002 and Desulfuromonas in the rumen compared with ALF, and CAR group mainly enhanced starch and sucrose metabolism, fructose and mannose metabolism, photosynthesis and d-alanine metabolism in the rumen compared with ALF.

    CAR diet positively changed the fatty acid profile of longissimus dorsi muscle and significantly altered the composition and function of the microbiota in the rumen, ileum and cecum.

The current study revealed a significant difference in the relative abundance of microbiota, where DM is associated with an enormous increase of Bacteroides spp., Clostridium spp., E. coli, and Fusobacterium spp., and a significant decrease in Bifidobacteria spp., and Lactobacillus spp., in contrast with normal control. Adults with TRD were randomly assigned to receive a single intravenous infusion, with a duration of 40 min, of either esketamine 0.25 mg/kg or ketamine 0.5 mg/kg. We assessed trait dissociation with the Dissociative Experience Scale (DES) and, to evaluate induced dissociation, the Clinician-Administered Dissociative States Scale (CADSS) was used. Thirty-two subjects received esketamine and 29 received ketamine. The groups had similar median DES scores (p = 0.26). More than 30% of the patients in both groups had DES scores ≥30 points. The median CADSS score in the esketamine group was equivalent to that in the ketamine group (p = 0.40). Every 5 points increment in the DES was associated with a 10.9% (95% CI 4.5-17.8%) increase in the CADSS, in an exponential fashion when the two groups were pooled together. Subjects with high trait dissociation had a higher risk of induced dissociation state (relative risk [RR] 1.41, 95% CI 1.11-1.78) and very high induced dissociation (RR 3.05, 95% CI 1.14-8.15). Induced dissociation was not a serious adverse effect. The findings suggest that trait dissociation is a predictor of induced dissociation by Ketamine or Esketamine in TRD subjects. The results showed that ADHD patients had significant changes in occurrence, transitions times and dFC strength of the cingulo-opercular network (CON) and sensorimotor network (SMN) in the cool state. The severity of ADHD symptoms showed significant correlations with the regional amplitude of dFC fluctuations in the ventral medial prefrontal cortex (vmPFC), anterior medial prefrontal cortex (amPFC) and precuneus. These findings could provide insights on the state-dependent dynamic changes in large-scale brain connectivity and network configurations in ADHD.Callous unemotional (CU) traits differentiate subtypes of conduct disorder (CD). Brain functional networks were constructed and network/nodal properties, including small-world properties and network/nodal efficiency, were calculated. Topological analysis revealed that, compared with HCs, CD-HCU group were characterized by decreased small-worldness (σ), decreased global efficiency, and increased path length (λ). These variables were similar between the CD-LCU and HC groups. Self-reported CU traits in CD patients correlated negatively with global efficiency and positively with λ. Regional analysis revealed diminished nodal efficiency in the right amygdala in the CD-HCU group compared with HCs. The content of O2- and H2O in the silkworm gonads increased significantly at 24 h, 72 h and 120 h after NaF stress. The activity of mitochondrial complexes I, III, IV and V in the silkworm testis was significantly greater than that in the control group. RT-PCR analysis suggested that the mRNA transcription levels of NADH-CoQ1, Cyt c reductase, Cyt c oxidase and ATP synthase genes were up-regulated significantly. Histopathological investigation showed that the damage to the silkworm testis was more severe with increasing NaF exposure times. These results indicated that NaF stress affects the NADH respiratory chain of the mitochondrial electron transport chain and increases the activity of related enzyme complexes, thus destroying the balance of the electron transport chain. Subsequently, the content of ROS in cells significantly increases, thus resulting in oxidative stress reactions in cells. These results enable better understanding of the testis-damaging molecular toxicological mechanism of NaF.

In addition, a retrospective patient record review was conducted.Results One hundred and twelve manual physiotherapists returned completed surveys. Although respondents indicated psychological factors were assessed, they rarely used psychological questionnaires. Significant but negligible associations were found for age and working hours and the use of specific questionnaires. From 95 patient records reviewed, seven were identified that mentioned one psychological factor each during history taking.Conclusions Dutch MPTs, regardless of their age and work characteristics, rarely use psychological questionnaires in patients with LBP, although they report addressing these constructs implicitly during history taking. Educational and implementation strategies are needed to warrant the use of psychological constructs and validated psychological questionnaires at all phases of the clinical reasoning process.
    Bridging stent stability is crucial for efficacy and safety of branched aortic endovascular repair (bEVAR) of thoracoabdominal aortic aneurysms (TAAAs). In this study, we assess the performance of the new Viabahn Balloon-Expandable endoprosthesis (VBX) in bEVAR. Based on our learning curve we give recommendations for a safe and effective use of the device.

    We prospectively collected the data of patients with TAAAs undergoing bEVAR between December 2017 and December 2019. All patients with implantation of at least 1 VBX stent-graft as bridging stent were included in our single-center analysis. Demographic, comorbidity, and computed tomography angiography (CTA) data of 112 patients were retrospectively evaluated. Primary endpoint was a composite of branch-related technical success and freedom from target vessel instability. Secondary endpoints were clinical and ongoing clinical success.

    Primary endpoint technical success was achieved in all patients (100%) with a freedom from target vessel instability of e should be considered to avoid type Ic endoleak and edge stenosis. Based on this experience longer landing zones and 2-step deployment of VBX are useful for successful bridging also of challenging target vessels.The performance and energy consumption of a gas-liquid-solid inverse fluidised bed bioreactor (GLS-IFBBR) using polyethylene (PE) particles with different surface coatings (zeolite, lava rock, activated carbon and multi-plastic) as media for synthetic wastewater treatment were investigated at loading rates of 1.64-3.38 kg COD/(m3·d) and 0.17-0.34 kg N/(m3·d) to determine the optimum carrier media. The results showed that PE coated with other inorganic materials could increase the nutrient removal efficiency at the same influent conditions. Compared with other media, PE coated with zeolite (PEZ) was the optimal carrier particles in this study as reflected by the highest COD and nitrogen removal, stable effluent, low biomass yield at different hydraulic retention times (HRT). In addition, the energy consumption of lavarock-coated PE (PEL) with a highest density was the lowest.

This study aims to determine whether blood levels of the extracellular sulfatases (SULF1 and SULF2) and their bio-activity can serve as novel biomarkers for NSCLC early detection.

    Using human plasma specimens from NSCLC patients, nonmalignant COPD patients, and healthy individuals, we determined the association between plasma SULF levels and the presence of NSCLC. We assessed the plasma SULF levels as a function of sex and age. We also evaluated the plasma levels of heparin-binding factors potentially mobilized by the SULFs. To increase test specificity of blood SULF2 as a biomarker for the early diagnosis of NSCLC, we investigated the presence of a tumor-specific SULF2 isoform released in the blood, which could be used as a biomarker alone or in multiplex assays.
