The shortage of available donor lungs is a continuing clinical problem. Lobar lung transplant and the extension of donor lung criteria may expand the pool of donor lungs. We here report a case of lobar lung transplant from a marginal deceased donor with bronchiectasis and fibrosis at the left upper lobe and a history of treated pulmonary tuberculosis. Our experience with this case suggests that a lobar lung transplant can be considered not only for size mismatches but also for use of locally damaged lungs. In addition, lungs from a donor who received treatment for pulmonary tuberculosis may be feasible for transplant with chemoprophylaxis for latent tuberculosis infection. Our current case indicates one possible option to overcome the persistent shortage of available donor lungs.
The steatosis of graft liver is an important factor in liver transplant that determines the graft function in the recipient and the recovery of the remnant liver in the living donor. We analyzed the data of living donors from our center to evaluate whether magnetic resonance imaging and magnetic resonance spectroscopy can replace liver biopsy.

From May 2010 to May 2019, data from a total of 239 living donors was collected. There were 84 patients who had no magnetic resonance imaging or magnetic resonance spectroscopy data, and they were excluded. The result of preoperative liver biopsy was compared with preoperative magnetic resonance imaging and magnetic resonance spectroscopy data. The steatosis was defined by the degree of macrosteatosis.

The magnetic resonance imaging of the fat fraction was a good parameter to predict fatty changes between normal and fatty liver groups (3.09 ± 3.38% for normal 7.48 ± 4.07% for fatty liver; P < .001). The magnetic resonance spectroscopy was also a good parameter sive procedure. Moreover, preoperative magnetic resonance spectroscopy can determine an anatomic variation of the bile duct, which helps improve the safety of the living donor. However, more clinical data and further studies are needed to ensure that preoperative magnetic resonance spectroscopy is essential.
Kidney dysfunction is common in liver transplant candidates and is a well-established predictor of increased mortality after liver transplant. However, the best method for determination of the glomerular filtration rate before liver transplant remains unclear.

We analyzed the performance of the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation and the Modification of Diet in Renal Disease (MDRD) Study equation, before liver transplant, compared with radionuclide glomerular filtration rate and examined the association of the 2 equations with a composite outcome of stage 4 chronic kidney disease, initiation of chronic dialysis, or patient death.

We studied 426 consecutive adult liver transplant recipients from 1990 to 2014. The correlation coefficient of the radionuclide glomerular filtration rate with the Chronic Kidney Disease Epidemiology Collaboration equation was 0.61 and with the Modification of Diet in Renal Disease Study equation was 0.58. The Modification of Diet in Renal Disease Study equation showed a bias of -4.7 mL/min and precision of 32.9 mL/min, whereas the Chronic Kidney Disease Epidemiology Collaboration equation showed a bias of -11.1 mL/min but was more precise (28.1 mL/min). Only the Chronic Kidney Disease Epidemiology Collaboration equation remained significantly associated with the composite outcome in the multivariable analysis.

And then, we confirmed the change of intracellular ROS level and apoptosis with respective methods. The mitochondrial dynamics was assessed with confocal microscope imaging and western blotting.Results We found that the level of mitochondrial H2O2 greatly increased compared to cytosolic H2O2 and it affected cytosolic H2O2 generation after glutamate treatment. In addition, we confirmed that mitochondrial Prx5 provides more effective neuroprotection than cytosolic Prx5.Discussion Overall, our study reveals the mechanisms of cytosolic and mitochondrial ROS in glutamate toxicity. Our findings suggest that mitochondrial ROS and Prx5 are attractive therapeutic targets and that controlling these factors be useful for the prevention of neurodegenerative diseases.In patriarchal societies like Kenya, understanding men's perceptions and attitudes on family planning is critical given their decision-making roles that affect uptake of contraception. Yet, most programmes mainly target women as primary users of contraceptive methods since they bear the burden of pregnancy. However, women-focused approaches tend to overlook gender power dynamics within relationships, with men wielding excessive power that determines contraception use or non-use. A qualitative study involving focus group discussions and in-depth interviews was conducted in the two predominantly Muslim communities of Lamu and Wajir counties, Kenya. Open-ended questions explored perspectives, attitudes and men's understanding of contraception, family size, decision making on family planning and general views on contraceptive use. Thematic content analysis was used. Findings show that men in Wajir and Lamu held similar viewpoints of family planning as a foreign or western idea and associated family planning with ill health and promiscuity.
